Efficacies of intratympanic methylprednisolone injection in three types of hearing loss

Abstract:Objective To report treatment outcomes of intratympanic injection of methylprednisolone in various types of hearing loss. Methods Data from 48 patients with acute low-frequency sensorineural hearing loss (ALHL, n=36), sudden sensorineural hearing loss with diabetes (SSHL-DM, n=7) or sudden sensorineu-ral hearing loss failing oral steroids hormonal treatment (SSHL-FOH, n=5) were collected from March 2012 to June 2014. Injections of 0.5-0.7 ml (20-28 mg) methylprednisolone were administered. For ALHL patients, hearing test was repeated 5 days after the first injection in the office and a second injection was given if no no-ticeable hearing improvement. SSHL-DM and SSHL-FOH patients were hospitalized and received intratym-panic methylprednisolone injections every two days for a total of 5 injections. Results The average thresholds at 0.25, 0.5 and 1 kHz in ALHL patients was 51.25 ± 6.84 dB HL before treatment and 19.36 ± 4.85 dB HL af-ter treatment. Hearing completely recovered in 32 cases and improved in 4 cases, yielding an effective rate of 100%. The average threshold for 0.5, 1, 2, 4 and 8 kHz in SSHL-DM patients was 78.31 ± 14.16 dB HL be-fore treatment and 49.36 ± 12.69 dB HL after treatment, with complete hearing recovery in 2 cases, signifi-cant improvement in 2 cases and some improvement in 1 case, yielding a total effective rate of 71.43%. Hear-ing improvement occurred mainly at 0.5, 1 and 2 kHz, with little improvement at 4 and 8 kHz. The average threshold for 0.5, 1, 2, 4 and 8 kHz in SSHL-FOH patients was 79.25 ± 16.28 dB HL before treatment and 69.36 ± 12.27 dB HL after treatment, showing significant improvement in 2 cases, some improvement in 2 cas-es and no change in 1 case, yielding a total effective rate of 60%. Hearing improvement also occurred mainly at 0.5, 1 and 2 kHz with no obvious change for 4 and 8 kHz in this group. Conclusion Intratympanic injection of methylprednisolone can effectively improve low-frequency hearing loss, while having limited efficacy for high-frequency loss. ALHL appears to respond best to intratympanic injection of methylprednisolone, although the treatment can also be used in SSHL-DM as the first choice treatment, which helps reduce potential systemic adverse effects of steroids. In SSHL-FOH, it may be used as a salvage therapy.
