Abstract:The importance of early initiation of palliative care for patients with cancer has been well established. The American Society of Clinical Oncology "recommends early specialist palliative care involvement, especially for patients with uncontrolled symptoms and/or QOL [quality-of-life] concerns." 1 Multiple studies have reported improved end-of-life (EOL) care with early palliative care involvement in a number of different cancer types. 1 -3 Despite this, many patients still receive aggressive care at the EOL. This raises questions regarding how to improve the implementation and acceptance of early palliative care among patients and health care practitioners. In this study, Mah et al 2 estimated the association of palliative care initiation timing with EOL quality indicators among 8297 patients with ovarian cancer who died between 2006 and 2018 in Ontario, Canada, using linked health care administrative data. Mah et al 2 selected 5 of 15 previously established recommended quality indicators for EOL cancer care. 4 These quality indicators represented aggressive and supportive care, including new hospitalization or intensive care unit (ICU) admission in the final month of life, emergency department (ED) visits, physician house calls, or late chemotherapy within the last 14 days of life. 2 ,4 Mah et al 2 found that palliative care initiation prior to the last 3 months of life was associated with decreased rates of aggressive EOL care (hazard ratio [HR], 0.47 [95% CI, 0.37-0.60]) including ICU admissions (HR, 0.46 [95% CI, 0.27-0.76]) and acute care hospital death (HR, 0.54 [95% CI, 0.45-0.65]). Strengths of the study by Mah et al 2 included the use of province-wide population-based data that are generalizable, the exclusion of patients who died within 3 months of diagnosis who did not have the option for early palliative care initiation, and the sensitivity analyses. A limitation of their dataset was missing disease stage in 52.3% of included patients. However, the outcomes of less aggressive EOL care and increased supportive care use persisted regardless of early- vs late-stage disease and even after removing patients with missing disease stage. Similar trends have been reported across other Canadian provinces and internationally for EOL cancer care. 1 ,3 ,5 Despite the finding by Mah et al 2 that early initiation of palliative care was associated with decreased aggressive EOL care, it is interesting to note that there remained a high proportion of patients with ED visits (>25%) in the last 2 weeks of life. 2 Furthermore, nearly one-half of patients were hospitalized in the last month of life, and more than one-third died in acute care hospitals. Unfortunately, the dataset did not capture hospice use to better understand the location of EOL. Nonetheless, these results by Mah et al 2 reiterate the high resource use associated with aggressive care in this patient population and highlights where improvements are necessary. A US study by Alishahi Tabriz et al 6 suggested that more than one-half (51.6%) of ED visits by individuals with cancer were potentially preventable, with the most common presenting concern being pain (36.9%). 6 This study by Mah et al 2 also reported a 74% increase in the number of potentially preventable ED visits, with almost 30% of visits resulting in unplanned hospitalizations. Some, possibly many, of these unplanned admissions might reflect inadequate community resources, a problem which must be addressed in addition to implementing changes to promote earlier, more proactive palliative care. Furthermore, although Mah et al 2 found that there was a lower risk of new ICU admissions with early initiation of palliative care, there was an approximately 2.5-fold increase in ICU admissions at the EOL for 2012 to 2018 compared with 2006. 2 US Surveillance, Epidemiology, and End Results Program and Medicare data from 2003 to 2017 also noted an increase in new ICU admissions among older patients with cancer at the EOL, from 23.7% to 28.7%. 7 While the proportional differences in new ICU admissions may reflect variances in care styles between countries, it does not explain the temporal increase in EOL ICU admission seen across both studies. Why does there seem to be a trend in increasing ICU admissions at the EOL despite clear advantages of early palliative care initiation, which should raise discussion regarding goals of care and subsequently decrease ICU admissions among patients with advanced malignant neoplasms? Whether this increase is secondary to advances in ICU care, changes in care styles, or patient preferences over time remains unknown but poses interesting questions for future studies.
