Abstract:Objective To evaluate endoscopic ultrasonography (EUS) for diagnosis and treatment of ectopic pancreas in upper gastrointestinal tract.Methods Data of 52 patients with ectopic pancreas diagnosed by EUS and confirmed by pathological examination were reviewed.The feature of EUS image and diagnostic accuracy were retrospectively analyzed.Results Out of 52 patients, 41 (78.85% ) were pathologically diagnosed as having ectopic pancreas, 6 were stromal tumors, 4 were leiomyoma and 1 was nerve sheath tumor.All ectopic pancreases presented as protruding lesions, in which 37 were located at gastric antrum, 2 in duodenum, 1 in gastric body and 1 in gastric fundus.Under EUS, 1 case appeared as hyperechoic, 10 as isoechoic, 18 as hypoechoic and 12 as mixed echoic.The lesion involved mucosa in 2 cases, muscularis mu-cosae in 8, submucosa in 30 and muscularis propria in 1, in which 15 cases from submucosa underwent endoscopic resection.Bleeding occurred in 2 cases and no other complications occurred.There was one relapse during the 6 months of followed-up.Conclusion EUS is valuable and safe for the diagnosis of ectopic pancreas in upper gastrointestinal tract.Depth of lesions revealed by EUS can help further treatment.

Abstract:With the development of technology and accessories, the role of endoscopic ultrasound (EUS) has evolved from diagnostics to therapeutics. In order to characterise the therapeutic role of EUS, we searched Web of Knowledge database and reviewed articles associated with therapeutic EUS. There are two modalities for the therapeutic purpose: drainage and fine-needle injection. EUS-guided drainage is a promising procedure for the treatment of peripancreatic fluid collection and biliary obstruction; EUS-guided fine-needle injections such as celiac plexus neurolysis, for the purpose of pain relief for pancreatic cancer and chronic pancreatitis, has emerged as a promising procedure. The aim of the study was to perform a comprehensive and conscientious review on the techniques, complications and clinical outcomes of those EUS-based procedures.

Abstract:ObjectiveThe diagnoses of patients with duodenal protruding lesions are difficult when using conventional examinations such as computed tomography (CT) and conventional endoscope etc. Thus, we investigated the clinical value of endoscopic ultrasonography (EUS) with miniature ultrasonic probes on the diagnosis and treatment of duodenal protruding lesions.MethodsPatients with duodenal protruding lesions who were indicated for EUS were examined by EUS with 12∼15 MHz miniature ultrasonic probes and double-cavity electronic endoscope. According to diagnosis of EUS, those patients were indicated for biopsy and treatment received biopsy, endoscopic resection or surgical excision. The postoperative histological results were compared with the preoperative diagnosis of EUS. Those patients without endoscopic resection or surgical excision were periodically followed up with EUS.ResultsA total of 169 patients with duodenal protruding lesions were examined by EUS, of which 40 were diagnosed with cysts, 36 with inflammatory protruding or polyp, 25 with Brunner’s gland adenoma, 19 with ectopic pancreas, 17 with gastrointestinal stromal tumor, 12 with extrinsic compression, 12 with minor papilla, 6 with lipoma, 1 with adenocarcinoma and 1 with lymphoma. After EUS examinations, 75 patients received biopsy, endoscopic resection or surgical excision respectively. The postoperative histological results of 70 patients were completely consistent with the preoperative diagnosis of EUS, with 93.33% diagnostic accuracy. The results of follow-up with EUS indicated that duodenal cyst, Brunner’s gland adenoma, ectopic pancreas, gastrointestinal stromal tumor and lipoma remained unchanged within 1∼3 years. No related complications occurred among all patients that received EUS examinations.ConclusionEUS is an effective and reliable diagnostic method for duodenal protruding lesions.

Abstract:Objective To explore the value of endoscopic ultrasonography in the diagnosis of pancreatolithiasis and the treatment of pancreatolithiasis.Methods The clinical data of 4 patients with pancreatolithiasis treated in our hospital from January 2008 to December 2010 were retrospectively analyzed.Results Four cases of pancreatolithiasis were diagnosed by EUS,manifested as the imageological features of chronic pancreatitis,and showed dot-like or arc-shaped strong echo with acoustic shadows within the pancreas.The pancreatic duct wall was irregular,dilatant or saccular dilation.Two cases were complicated with pancreatic carcinoma.One case was complicated with ampullary carcinoma.One case had abdominal discomfort.They underwent pancreaticoduodenectomy,choledochojejunostomy,pancreatolithotomy plus side-to-side pancreaticojejunostomy,and ERCP+EST+Stent respectively.Conclusion Imaging is the primary means of diagnosis of pancreatolithiasis,and EUS is the best imaging method.There was high incidence of pancreatolithiasis complicated with pancreatic carcinoma,so we should attach importance to this.The different treatments should be given based on patients' situations.

Abstract:Endoscopic ultrasound (EUS) is one of the most important modalities for the diagnosis of digestive tract diseases. EUS has been evolving ever since it was introduced. New techniques such as elastography and contrast enhancement have emerged, increasing the accuracy, sensitivity and specificity of EUS for the diagnosis of digestive tract diseases including pancreatic masses and lymphadenopathy. EUS-elastography evaluates tissue elasticity and therefore, can be used to differentiate various lesions. Contrast-enhanced EUS can distinguish benign from malignant pancreatic lesions and lymphadenopathy using the intravenous injection of contrast agents. This review discusses the principles and types of these new techniques, as well as their clinical applications and limitations.
EUS for Choosing Best Endoscopic Treatment of Mesenchymal Tumors of Upper Gastrointestinal Tract

Abstract:AIM: To evaluate the value of endoscopic ultrasonography (EUS) in the choice of endoscopic therapy strategies for mesenchymal tumors of the upper gastrointestinal tract.METHODS: From July 2004 to September 2010, 1050 patients with upper gastrointestinal mesenchymal tumors (GIMTs) were diagnosed using EUS. Among them, 201 patients underwent different endoscopic therapies based on the deriving layers, growth patterns and lesion sizes.RESULTS: Using EUS, we found 543 leiomyomas and 507 stromal tumors. One hundred and thirty-three leiomyomas and 24 stromal tumors were treated by snare electrosection, 6 leiomyomas and 20 stromal tumors were treated by endoloop, 10 stromal tumors were treated by endoscopic mucosal resection and 8 stromal tumors were treated by endoscopic submucosal dissection. Complete resection of the lesion was achieved in all cases. Of the mesenchymal tumors, 90.38% diagnosed by EUS were also identified by pathohistology. All wounds were closed up nicely and no recurrence was found in the follow-up after 2 mo.CONCLUSION: EUS is an effective means of diagnosis for upper GIMTs and is an important tool in choosing the endoscopic therapy for GIMTs, by which the lesions can be treated safely and effectively. Abstract:Objective: To examine the values of endoscopic ultrasonography (EUS) on diagnosis and treatment of esophageal hamartoma. Methods: We compared and analyzed various kinds of imaging examinations such as barium esophagram, contrast-enhanced computed tomography (CT) and conventional gastroscopy in retrospectively reviewing the clinical data of an esophageal hamartoma patient seen in our clinic in the First Affiliated Hospital, School of Medicine, Zhejiang University, Hangzhou, China. Having received various imaging examinations, this patient was finally diagnosed with esophageal hamartoma and underwent gastroscopic resection of hamartoma with the diagnostic information obtained from EUS. The patient had been regularly followed up for 13 months after treatment. Results: Barium esophagram, CT and conventional gastroscopy detected the lesion, but were unable to distinguish it from common esophagopolypus and other submucosal lesions, and unable to determine etiopathogenisis. EUS detected the hamartoma and identified its internal structure, echo, exact size, depth of invasion, origin and the relationship between adjacent tissues and organs, differentiating the lesion from other submucosal tumors and clearly defining the diagnosis. EUS-guided fine needle aspiration (FNA) also helped to identify the etiological diagnosis. Conclusion: EUS was superior to other imaging means in diagnosis and treatment of hamartoma.
The Value of Endoscopic Ultrasonography for Management of Esophageal Leiomyoma

Abstract:Objective To investigate clinical value of endoscopic ultrasonography (EUS) on diagnosis and treatment of esophageal leiomyoma.Methods The clinical feature of patients diagnosed as esophageal leiomyoma by EUS was analyzed.All patients underwent conventional gastroscopy and EUS examination as well as blood test.The EUS findings were compared with histopatholoical diagnosis for endoscopically or surgically resected specimens.All patients were followed-up with EUS for 14.3 months (range 2-36 months) for leiomyoma recurrence.Results A total of 191 patients were diagnosed with esophageal leiomyomas by EUS.Clinical manifestation,serum examination,conventional gastroscopy and spiral CT were not helpful for diagnosing esophageal leiomyoma,One hundred and sixteen patients with esophageal leiomyomas were treated by either endoscopic resection (111 cases) or surgical excision (5 cases).of which,101 resected specimens were pathologically confirmed as leiomyomas.The diagnostic accuracy of EUS was 87%.The follow-up study showed that there was no change of leiomyomas in 75 untreated patients and no recurrence was found in 116 surgically treated patients.Conclusion Esophageal leiomyoma is a benign tumor,EUS plays a very important role in diagnosis and treatment of this disease.

Abstract:Pancreas is a retroperitoneally located organ, and thus general CT and MRI have a very limited specificity and sensitivity in diagnosing pancreatic diseases. However, endoscopic ultrasonography (EUS) with a better differential diagnostic capability of detecting pancreatic morphology and properties through gastric wall has been widely accepted by physicians and become one of the first-line methods compared with other techniques. Currently, common technologies used in the diagnosis and treatment of pancreatic diseases include EUS-guided fine needle aspiration (EUS-FNA), EUS-guided drainage, EUS-guided pancreatic cysts ablation, EUS-assisted radiotherapy, and EUS-guided celiac plexus neurolysis (EUS-CPN), etc. Recent innovative applications include contrast-enhanced EUS with Doppler mode, contrast-enhanced harmonic EUS, and EUS elastography. Value of Endoscopic Ultrasound in Distinguishing Ectopic Pancrease and Stromal Tumor in Stomach

Abstract:Objective To explore the value of endoscopic ultrasound (EUS) in differential diagnosis of ectopic pancreas and stromal tumor in stomach.Methods The clinical data and endoscopic ultrasound findings of 40 patients with ectopic pancrea and 141 patients with stromal tumor were retrospectively analyzed,including the location,size,growth pattern,layer of origin,internal echo pattern and so on.All patients were diagnosed pathologically.Results Ectopic pancreas were predominantly located in the antrum (92.5％) and stromal tumor were mainly located in the gastric body (45.4％),the antrum (23.4％) and the fundus (22.0％),there was a significant difference in lesion' s location between two diseases (P ＜0.001).There was no statistical difference in the ratio of longest/shortest diameter of the lesion size(P =0.057).But there was statistical difference in lesion size between ectopic pancrease and stromal tumor [(19.98 ± 12.80)mm vs (11.25 ± 3.61)mm].Mural growth pattern was most common in ectopic pancrease (82.5％) and mural growth pattern (37.6％),extraluminal growth pattern (24.8％) and intraluminal growth pattern (22.7％) in stromal tumor (P ＜0.001).The third layer (75.0％) was predominant in ectopic pancrease while the fourth (54.6％) and the second layers (34.0％) in stromal tumor (P ＜0.001).With regard to internal echo pattern,hypoechoic (32.5％),intermediate (27.5％) and heterogeneous (27.5％) was predominant in ectopic pancrease and homogeneous hypoechoic lesion (72.3％) in the stromal tumor (P ＜ 0.001).Conclusions There were statistical differences between ectopic pancrease and stromal tumor in the location,size,layer of origin,growth pattern and internal echo pattern under EUS.EUS plays an important role in identification of ectopic pancrease and stromal tumor.

Abstract:Objective To evaluate the diagnosis and management of ectopic pancreas.Methods The clinical data of 62 cases of ectopic pancreas,which were diagnosed by endoscopic uhrasonography (EUS)or pathologic findings between July 2006 and December 2007 were retrospectively analyzed.The cases were divided into 4 groups according to different ways of management.Group A included 37 patients,who were diagnosed as having ectopic pancreas(＜19mm)by EUS only and were followed up via phone call every 3 months.Eight patients in group B were diagnosed by EUS as having submucosal lesions suspected as ectopic pancreas,and underwent operation because of large size or difficulty in diagnosis.Eight patients in group C received operation for other diseases and the eetopic pancreases were found in operation.Group D included 9 patients who underwent surgery due to malignant tumors.Results Ectopic pancreas were most commonly found as a single lesion in gastric antrum(35/62,56.5%)with mean size of 9.2±5.4 mm.All patients in group A were asymptomatic,of which 10 received followed-up endoscopy and no changes in size of the lesion were found.All patients in group B,C and D were diagnosed as ectopic pancreas pathologically.Conclusion Ectopic pancrea is relatively common and asymptomatic,only part of them could be diagnosed clinically.Carcinoma arising from the ectopic pancreas is rare and lesion of small size diagnosed by EUS could be followed up endoseopieally.
Application Status and Prospect of Endoscopic Ultrasonography in Pancreatic Neoplasms

Abstract:The diagnosis of pancreatic neoplasms is always a big challenge for clinicians. Some examination methods, such as endoscopic ultrasonography(EUS),CT and MRI,can be used for the diagnosis of pancreatic neoplasms. Among them EUS is often taken as a second - line diagnostic method for such diseases. It is conducive to find small pancreatic lesions,and evaluate tumor body size and lymph node metastases. What's more,it is simple and easy to operate with high sensitivity,and this is other examinations do not have. Moreover,the clinical treatment can be enhanced with improved diagnosis accuracy by pathological results found by endoscopic ultrasonography guided fine - needle aspiration( EUS - FNA). As the incidence of pancreatic neoplasms showing an upward trend,EUS may have a higher application value afterwards. This article systematically and detailedly presented the merits and demerits of many common diagnostic methods for pancreatic neoplasms,focusing on EUS, in the hope of providing a reference for readers.
[The Clinical Value of Endoscopic Ultrasonography in Early Diagnosis of Pancreatic Tumor].

Abstract:To investigate the clinical value of endoscopic ultrasonography (EUS) and intraductal ultrasonography (IDUS) compared with the other imaging examination in the early diagnosis of pancreatic tumor.Retrospect studied of 188 cases of small pancreatic lesions (less than 3 cm in diameter) detected by EUS, IDUS and the other imaging examination at Changhai hospital from October 1992 to September 2006.(1) The accurate rate of small pancreatic cancer diagnosed by EUS was 95.6%, and it is better compared with US (58.6%), CT (77.4%), MRI (76.2%) and ERCP (85.3%). The most common endosonographic features of small pancreatic cancer were alike round, irregular edge, hypoechoic mass with uniformity internal echo. (2) IDUS was performed in 25 cases of small pancreatic lesions, the accurate rate of diagnosis was 100% (25/25), and it is better compared with US 32.0% (8/25), CT 52.9% (9/17) and MRI 57.9% (11/19). (3) EUS guided FNA (EUS-FNA) was performed in 18 cases of small pancreatic lesions, the accurate rate of diagnosis was 66.7% (12/18). (4) The accurate rate of pseudocyst diagnosis by EUS was 100.0% (27/27), it is better compared with US 52.0% (13/25), CT 66.7% (12/18), MRI 82.4% (14/17) and ERCP 78.9% (15/19); and the overall accurate rate of diagnosis of cystic pancreatic tumors by EUS was 57.7% (15/26), it is better compared with US 19.2% (5/26), CT 36.4% (8/22), MRI 37.5% (6/16) and ERCP 50.0% (7/14).EUS and IDUS were better than the other imaging examinations in the detection of small pancreatic tumors.

Abstract:Objective To analyze the sensitivity and accuracy of endoscopic ultrasonography in diagnosing pancreatic cancer and assessing tumor features. Methods 126 patients with pathologically confirmed pancreatic cancer ( PC ) who had received endoscopic ultrasonography (EUS), Ultrasonography (US) and spiral computed tomography (CT) 15 days before diagnosis were selected as the subjects of the present study. The results of three imaging modalities and the results of some categories of PC on EUS were retrospectively analyzed and treated with the t-test or u-test to assess the significance of the differences of these results. Results The diagnostic rate of EUS was higher than that of US,especially in diagnosing small PC; the difference was significant(P0. 05). There was no significant difference in sensitivity and accurate rate between EUS and CT. Combination of EUS with CT greatly increased the diagnostic sensitivity and accurate rate, and the difference was very significant between the combined modality and using US,EUS or CT alone(P0. 05). There was no significant difference between EUS and CT in delineating the indirect signs of the tumors. Conclusions EUS is a preferable technique in detecting PC and should be used routinely for detecting small PC in particular. Combined use of EUS and CT to detect PC is recommended.

Abstract:Pancreatic cystic lesions (PCLs) have been screened out more frequently due to the progress of imaging techniques. Of these PCLs, the most common lesions are pancreatic pseudocysts, serous cystadenomas, mucinous cystadenomas and intraductal papillary mucinous neoplasms. Once these lesions are detected, further differentiation between neoplastic and non-neoplastic cysts, benign and malignant cysts should be made. Endoscopic ultrasound (EUS) is an important tool in the diagnostic process of PCLs. Fundamental B-mode endoscopic ultrasonography can delineate the cyst morphology and inner structure well; contrast enhanced EUS can demonstrate images of microcirculation of the cystic wall, and discriminate mural nodules from mucous clots; EUS-fine needle aspiration and biopsy can help achieve cystic fluid analysis and yield a higher diagnostic rate in histological pathology examinations. The aim of this paper is to review the role of EUS in discriminating the differences between common PCLs.
The value of EUS in diagnosing chronic abdominal pain of suspected pancreatic origin

Abstract:Objective To evaluate the diagnostic value of EUS in patients with chronic abdominal pain of suspected pancreatic origin. Methods The EUS findings and related clinical data of 106 patients with chronic abdominal pain of suspected pancreatic origin （ excluding the patients with suspected pancreatic malignancies） from 1991 to 2004 in PUMCH were retrospectively analyzed. Results （ 1 ） The principal disease interpreting the chronic abdominal pain of suspected pancreatic origin （ excluding pancreatic malignancies） was chronic pancreatitis （CP）（57. 5% ）, the following contributions were other pancreatic diseases （18. 9% ） and unknown diseases （11.5% ）. （2） The sensitivity and specificity of EUS for diagnosing CP was 95. 1% and 64. 4% respectively, the positive predictive value （ PPV ） and negative predictive value （NPV） was 78.4% and 90. 6% respectively. （5） Abnormalities of pancreatic parenchyma structure based on EUS were the main findings （90. 2% ） in patients with CP and non-homogeneous echo pattern combined with hyper echoic dots or calcification was the predominant feature （52. 5% ）. The value of isolated inhomogeneity and focal enhanced echogenicity for diagnosing CP were limited （ P 〉 O. 05）. Abnormalities of pancreatic ductal system were presented in 65.9% of patients with CP and dilation of pancreatic duct was the major feature （54. 4% ）. CP with focal mass （inflammatory pseudotumor） was usually presented as hypo echoic mass in the pancreatic head based on EUS （90%）, which was similar to the EUS feature of pancreatic cancer. （4） The general accordant rate based on EUS with ERCP or BT-PABA were 77.8% and 70. 4% respectively, and the correct rate based on combine diagnosis were 100% and 95. 2%. Conclusion CP is the main source of chronic abdominal pain of suspected pancreatic origin （excluding pancreatic malignancies）. EUS has good sensitivity but inadequate specificity for diagnosing CP, while ERCP may be more sensitive than EUS for detecting pancreatic ductal lesions. Pancreatic parenchymal abnormalities contribute the major EUS features of CP but the value of isolated inhomogeneity and focal enhanced echogenicity for diagnosing CP are limited. Abstract:Endoscopic ultrasound (EUS) has become important in a variety of clinical settings. Echoendoscopes may be categorised into radial and linear configurations. Radial devices are used for diagnostic imaging, whereas linear echoendoscopes also facilitate image guided tissue sampling and intervention. EUS is an established primary diagnostic tool for a number of conditions including choledocholithiasis and biliary microlithiasis. It is therefore well suited to the investigation of the aetiology of pancreatitis where simpler measures fail to identify the aetiology. It can also be used to identify chronic non-calcific pancreatitis. EUS is important in the secondary evaluation of abnormalities detected by other imaging modalities-for example, cystic pancreatic lesions. The high resolution of EUS allows more detailed image based analysis than other imaging modalities. The ability to sample cyst fluid significantly increases the accuracy of lesion characterisation. Most importantly, EUS has become indispensable in the staging of a variety of upper gastrointestinal tract tumours. If resection is being considered, the high resolution images obtained via EUS are invaluable for local tumour staging. EUS guided tissue sampling permits accurate nodal staging without relying on lymph node size as proxy for malignant infiltration. In patients with contraindications to magnetic resonance imaging, EUS is an alternative for the staging of rectal carcinoma. It is used in the staging of lung cancer, often in combination with endobronchial ultrasound. Finally, EUS is used therapeutically in image guided drainage (such as gastrocystostomy in pancreatic pseudocyst) and coeliac plexus neurolysis in patients with abdominal pain caused by pancreatic cancer or pancreatitis.

Abstract:Endoscopic ultrasound (EUS) is an integrated diagnostic technique merging endoscope and ultrasound to examine the digestive system. EUS has emerged as a primary diagnostic method for pancreatic diseases due to its distinctive benefits. Over the past four decades, EUS has undergone a transformation, shifting its role from primarily diagnostic to increasingly therapeutic. Additionally, in recent years, EUS has emerged as an increasingly prominent adjunctive or alternative approach to conventional surgical interventions. This review provides a comprehensive analysis of current technological approaches in the treatment of pancreatic diseases. The dynamic interplay with diverse therapeutic approaches has reinvigorated EUS and shaped its trajectory in the management of pancreatic diseases.

Diagnostic value of endoscopic ultrasonography guided fine needle aspiration in patients with pancreatic cancer

Abstract:Objective To investigate the clinical characteristics of pancreatic cancer and the diagnostic value of endoscopic ultrasonography fine needle aspiration (EUS-FNA) in patients with pancreatic cancer.Methods EUS-FNA was performed in 35 patients with pancreatic lesions detected by CT scanning,ultrasonography or clinically suspected diagnosis.Results There were 23 males and 12 females patients with the age ranging from 46 to 80 years old.Twelve cases located in pancreatic head,5 cases in pancreatic neck,8 cases in pancreatic body,10 cases in pancreatic tail.The sensitivity,specificity and positive predictive value of EUS-FNA were 91％,100％ and 100％,respectively.The positive rate of histopathology and cytopathology was 88％ (31/35).EUS-FNA was conducted on all 35 patients without any serious complications,such as acute pancreatitis,pancreatic infection,intra-abdominal bleeding.Conclusion EUSFNA for the pancreas is a safe and effective modality for diagnosis and treatment of pancreatic cancer.
