Abstract:Objective To evaluate the effect of therapeutic hypercapnia on cerebral oxygen metabo?lism in the patients undergoing thoracoscopic surgery in beach chair position ( BCP ) . Methods Sixty pa?tients of both sexes, aged 18-32 yr, with body mass index of 19-24 kg∕m2 , of American Society of Anes?thesiologists physical statusⅠorⅡ, scheduled for elective bilateral thoracic sympathectomy performed via a thoracoscope, were divided into control group ( group C ) and hypercapnia group ( group H ) , with 30 patients in each group using a random number table. After induction of anesthesia, all the patients in both groups were tracheally intubated and mechanically ventilated using the ventilation regimen low tidal vol?ume intermittent positive pressure ventilation combined with low level of positive end?expiratory pressure ( 5 cmH2 O) , maintaining arterial carbon dioxide partial pressure ( PaCO2 ) at 35-45 mmHg. PaCO2 was maintained at 45-55 mmHg by adjusting the respiratory rate after the patients were placed in BCP in group H. Anesthesia was maintained with target?controlled infusion of propofol and intermittent intravenous boluses of rocuronium and sufentanil. Bispectral index value was maintained at 45-55. Before anesthesia induction ( baseline) , at 5 min after intubation, and at 5, 10, 15 and 20 min after the patients were placed in BCP, blood samples were taken from the radial artery and jugular bulb for blood gas analysis, jugular ve?nous bulb oxygen saturation was measured, and arteriovenous blood O2 content difference, cerebral O2 ex?traction rate, and venous to arterial blood lactate concentration difference were calculated. Results Com?pared with group C, PaCO2 and jugular venous bulb oxygen saturation were significantly increased, and ar?teriovenous blood O2 content difference and cerebral O2 extraction rate were significantly decreased at at 5, 10, 15 and 20 min after the patients were placed in BCP in group H ( P<0?05) , and there was no signifi?cant change in venous to arterial blood lactate concentration difference at each time point between the two groups ( P>0?05) . Conclusion Therapeutic hypercapnia can improve the cerebral oxygen metabolism in the patients undergoing thoracoscopic surgery in BCP .

Abstract:We investigated sublingual tissue PCO2 during hemorrhagic and septic shock. Hemorrhagic shock was induced in 10 rats. Sublingual PCO2 increased from 45 to 125 mm Hg and arterial pressure declined from 138 to 49 mm Hg, end-tidal PCO2 decreased from 35 to 13 mm Hg, and cardiac index fell from 290 to 77 ml/min/kg. Arterial blood lactate increased from 0.9 to 15.8 mmol/L. Gastric PCO2 was measured in five animals and it increased from 46 to 87 mm Hg. No significant changes were observed in eight "sham" bled animals including the five animals in which gastric PCO2 was measured. Highly significant linear correlations (p < 0.001) between sublingual PCO2 and gastric PCO2 (r = 0.71), cardiac index (r = -0.74), and arterial lactate (r = 0.59) were documented. We subsequently investigated sublingual PCO2 in five animals in which sepsis was induced by intravenous infusion of live Staphylococcus aureus. Like hemorrhagic shock, highly significant linear correlations were observed between end-tidal PCO2 and cardiac index and between sublingual PCO2 and arterial blood lactate. Sublingual PCO2 promises to serve as a technically simple, noninvasive, and rapid response quantitator of severity of circulatory shock states.

Abstract:Background. Earlier studies had demonstrated that gastric partial pressure of carbon dioxide (PCO2) of the gastric wall is an early indication of perfusion failure. Because hypercarbia is a general phenomenon of perfusion failure, this study investigated whether sublingual and buccal mucosal tissue PCO2 also serve as a sensitive indicator of systemic blood flow during hemorrhagic shock.Methods and Results Hemorrhagic shock was induced in five male domestic pigs. Buccal and sublingual PCO2 increased from 60 to 129 mm Hg (p < 0.01) in parallel with average decreases in car- diac output to 44% (from 6.1 to 2.8 L/minute; p < 0.01), and mean arterial pressure to 47% (from 115 to 57 mm Hg; p < 0.01), over the 2-hour interval of shock. According to colored microspheres used for measurements, buccal mucosal flow decreased to 35 % (from 6.3 to 2.2 mL/minute/100 g; p < 0.01) in close parallel with sublingual blood flow, which decreased to 34 % (from 7.2 to 2.5 mL/ minute/100 g; p < 0.01). Liver flow decreased to 56% (from 152 to 85 mL/ minute/100 g; p < 0.01), and renal flow to 47 % (from 272 to 128 mL/minute/100 g; p < 0.01) of baseline values. The procedure yielded to overall comparable volume exchanges in different animals. After reinfusion of shed blood, buccal PCO2 like sublingual PCO2 was restored to approximately baseline values (respectively, from 56 to 71 mm Hg and from 60 to 71 mm Hg; p = not significant), together with arterial pressure, cardiac output, and endtidal CO2 (EtCO2) (respectively, from 115 to 115 mm Hg, from 6.1 to 6.3 L/minute, and from 35 to 39 mm Hg; p = not significant), but there was delayed reversal of lactic acidosis (from 0.7 to 2.5 mmol/L; p < 0.01). In five unbled control animals, no significant changes were observed over the same interval.Conclusion. The current study extends the rationale for noninvasive measurements to both buccal and sublingual mucosa for diagnosis and quantitation of hemorrhagic shock severity.
Mucosal and cutaneous capnometry for the assessment of tissue hypoperfusion

Abstract:In critically ill patients, tissue hypoperfusion is an important cause leading to multi-organ dysfunction and death, and it cannot always be detected by measuring standard global hemodynamic and oxygen-derived parameters. Gastric intramucosal partial pressure of carbon dioxide (PCO2) as measured by gastric tonometry has been recognized to be of clinical value as a prognostic factor, in assessing the effects of particular therapeutic interventions, and as an end-point of resuscitation. However, this technique has several limitations that have hampered its implementation in clinical practice. The sublingual tissue bed has been shown to be damaged in models of shock, and microcirculatory changes in this area may indicate imminent changes in other important organs. The measurement of sublingual mucosal PCO2 (PslCO2) by sublingual capnography is technically simple, noninvasive and gives near instantaneous results. Clinical studies have established that high PslCO2 values and, more especially, high PslCO2 gap (PslCO2 - arterial PCO2) values are correlated with impaired microcirculatory blood flow and a poor outcome in critically ill patients. Sublingual capnography seems to be the ideal noninvasive monitoring tool to evaluate the severity of shock states and the adequacy of tissue perfusion. However, clinical studies are needed to determine the clinical utility of PslCO2 gap monitoring as end-point target to guide resuscitation in critically ill patients.
Decreases in Mesenteric Blood Flow Associated with Increases in Sublingual Pco2 During Hemorrhagic Shock

Abstract:We investigated relationships between visceral blood flow, intestinal wall carbon dioxide tension (PCO2), and sublingual PCO2 during hemorrhagic shock. In five pigs, cardiac output declined 81% and superior mesenteric blood flow 77% during hemorrhage. Duodenal PCO2 increased from an average of 50 to 121 mmHg and sublingual PCO2 concurrently increased from an average of 46 to 101 mmHg. Within 60 min after reinfusion of shed blood, duodenal and sublingual PCO2 returned to baseline values. Decreases in mesenteric blood flow were correlated with increases in sublingual (r= 0.91; P < 0.001) and duodenal (r = 0.89; P < 0.001) tissue PCO2. In five randomized "sham hemorrhage" control animals, neither decreases in cardiac output or mesenteric blood flow nor increases in duodenal or sublingual PCO2 were observed. Decreases in mesenteric blood flow during hemorrhage were therefore associated with early and comparable increases in tissue PCO2 in both visceral and sublingual sites.

Abstract:STUDY OBJECTIVES:To compare sublingual tissue PCO(2), a disarmingly simple and noninvasive measurement of the severity of perfusion failure, with gastric tonometric PCO(2) during hemorrhagic shock in five male domestic pigs weighing between 35 and 40 kg.DESIGN:Prospective animal study.SETTING:Animal laboratory in a research institution.PARTICIPANTS:Domestic pigs.INTERVENTIONS:Hemorrhagic shock was induced by a modification of the Wigger's method. BP was maintained at 50 mm Hg for 120 min followed by reinfusion of shed blood at a rate of 100 mL/min with the aid of an infusion pump.MEASUREMENTS AND RESULTS:During bleeding, the mean arterial pressure decreased from an average of 127 to 42 mm Hg, and cardiac output decreased from 7.7 to 2.4 L/min. Arterial blood lactate concentration concurrently increased from 1.2 to 13.9 mmol/L. Sublingual PCO(2) (PslCO(2)) increased from 59 to 105 mm Hg, and gastric PCO(2) increased from 61 to 111 mm Hg. The correlation between time-coincident sublingual and gastric measurements of PCO(2) was r = 0.91 (p<0.0001). Bland-Altman analyses demonstrated a close correspondence between the two measurements. The reinfusion of shed blood promptly reversed the hemodynamic abnormalities and reestablished gastric and PslCO(2) to near baseline values. This contrasted with a delayed reversal of lactic acidosis.CONCLUSIONS:Under experimental conditions of hemorrhagic shock, sublingual capnometry yielded measurements that were interchangeable with those of gastric tonometry.
Effects of Hydroxyethyl Starch 130/0.4 on Pulmonary Capillary Leakage and Cytokines Production and NF-κB Activation in CLP-Induced Sepsis in Rats

Abstract:Sepsis and resulting multiple system organ failure are the leading causes of mortality in intensive care units. Hydroxyethyl starch (HES) 130/0.4 was a novel preparation, developed to improve the pharmacokinetics of current medium molecular weight HES solutions. This study was designed to explore the effects of HES 130/0.4 on pulmonary capillary permeability (PCP), production of cytokines, and activation of transcription factor in septic rats induced by cecal ligation and puncture (CLP).Adult male Sprague Dawley rats were randomly divided into six groups (six rats/group): saline controls (30 ml/kg); CLP plus saline (30 ml/kg); CLP plus HES (7.5, 15, or 30 ml/kg, respectively), and HES alone (30 ml/kg). Mean arterial blood pressure and heart rate were monitored during the experiment process. Myeloperoxidase (MPO) activity, wet/dry weight ratio, PCP, tumor necrosis factor alpha (TNF-alpha), interleukin (IL)-6, IL-10, and nuclear factor-kappa B (NF-kappaB) were investigated at 6 h.We demonstrated that CLP could provoke significant injury in lung, characterized by increase in PCP, wet/dry weight ratio, MPO activity, TNF-alpha, IL-6, and IL-10 level, and NF-kappaB activation. Without obvious influence on systemic macro-hemodynamics, HES 15 ml/kg and 30 ml/kg significantly reduced CLP-induced elevation of pulmonary capillary permeability, wet/dry weight ratio, and production of IL-6. Meanwhile, HES 15 ml/kg increased IL-10 level and HES 7.5, 15, and 30 ml/kg suppressed MPO activity, TNF-alpha level, and NF-kappaB activation.HES 130/0.4 can inhibit CLP-induced PCP by attenuating pulmonary inflammation and NF-kappaB activation in vivo.
Buccal Capnometry for Quantitating the Severity of Hemorrhagic Shock

Abstract:We have recently demonstrated that measurement of buccal mucosal Pco(2) (P(BU)co(2)) is a reliable alternative to sublingual mucosal Pco(2) for measuring the severity of hemorrhagic shock. We hypothesized that measurement of P(BU)co(2) would serve as a continuous and a more sensitive and specific measurement for predicting survival during hemorrhagic shock than conventional measurements and thereby better guide initial management. Four groups of five pentobarbital anesthetized Sprague-Dawley rats were randomly assigned to be bled over 30 min in amounts estimated to be 25%, 30%, 35%, or 40% of total blood volume. With an optical Pco(2) sensor applied noninvasively to the mucosa of the left inner cheek, P(BU)co(2) was continuously measured together with arterial pressure, end-tidal Pco(2), and intermittent measurement of cardiac output, arterial blood lactate, and base deficit. Surviving animals had free access to water and food but no other treatment during the 72-h interval after recovery from anesthesia. After an estimated 40% blood loss, all animals died within 1 h. In the remaining animals, arterial pressure, end-tidal carbon dioxide, cardiac index, blood lactate, and base deficit each failed to discriminate among animals with 35%, 30%, and 25% acute blood losses. This contrasted with P(BU)co(2), which discriminated between the magnitude of massive blood loss and untreated survival. Buccal mucosal Pco(2) was predictive of outcome after rapid bleeding when compared with arterial pressure, end-tidal carbon dioxide, cardiac index, arterial blood lactate, and base deficit. This measurement is therefore likely to serve as a useful guide for the immediate management of hemorrhagic shock.

Abstract:Arterial pressure is a widely used measurement for estimating the severity of hemorrhagic shock and to guide its management. However, this capability is reduced when very low arterial pressure values cannot be reliably measured by noninvasive methods. Moreover, hypoperfusion may be masked by compensatory hemodynamic changes, and therefore, in the presence of near normal blood pressure, tissue hypoperfusion may progress undetected. Accordingly, hypercarbia is a general phenomenon of perfusion failure, which occurs in coincidence of the onset of hypotension and is promptly reversed with restoration of normal blood flows. Increases in buccal mucosa PCO2 are highly correlated with increases in gastric wall and sublingual mucosa PCO2 and decreases in tissue blood flows during hemorrhagic shock. In both clinical and experimental settings, tissue PCO2 measured in the oral mucosa proved to be a practical and reliable measurement for the diagnosis of circulatory failure states and an indicator of its severity. In contrast to intraarterial pressure, buccal PCO2 discriminated between short- and long-term survival after large-volume blood loss. Buccal PCO2 measurement therefore emerges as a useful predictor for survival and outcome and a useful guide to manage fluid resuscitation during hemorrhagic shock.
Increases In Tissue Pco2 During Circulatory Shock Reflect Selective Decreases In Capillary Blood Flow

Abstract:Objectives: Tissue Pco(2) reflects metabolic alterations due to circulatory failure during circulatory shock. This study addresses simultaneous changes in gastric and buccal tissue PCO2 with changes in microcirculatory blood flow in a rat model of circulatory shock induced by cecal ligation and puncture.Design: Prospective controlled laboratory study.Setting: University-affiliated research laboratory.Subjects: Male breeder Sprague-Dawley rats.Interventions: Induction of polymicrobial, abdominal sepsis by cecal ligation and puncture.Measurements and Main Results: Tissue PCO2 was continuously measured with the aid of a miniature carbon dioxide electrode. Using orthogonal polarization spectral imaging, recordings of the microcirculation were taken at baseline and hourly intervals until death and compared with sham-operated animals. Gastric and buccal tissue PCO2 values progressively increased in animals after cecal ligation and puncture and terminated in death. Microcirculatory blood flow in vessels >20 mu m was well preserved during progression of shock, whereas there was an early and progressive decrease in microcirculatory blood flow in vessels <20 mu m, mostly representing capillaries. Tissue PCO2, the tissue PCO2-PaCO2 gradient, and blood flow in vessels <20 mu m were highly correlated. This contrasted with sham control animals in which no significant hemodynamic, blood gas, lactate, microcirculatory, and tissue PCO2 abnormalities were observed.Conclusions: These observations suggest that microcirculatory failure in capillaries appears as an early defect in close association with anaerobic metabolism during progression of circulatory shock in an animal model of septic peritonitis.

Abstract:Objective To investigate the feasibility and predictive value of sublingual PCO2 (PSL CO2) measurements as a noninvasive and early indicator of systemic perfusion failure. Design A prospective, criterion study. Setting Emergency department and medical and surgical intensive care units of an urban community medical center. Participants and Patients Five normal human volunteers and 46 patients with acutely life-threatening illness or injuries. Interventions Intra-arterial or automated cuff blood pressure and arterial blood lactate (LAC) were measured concurrently with PSL CO2. Results PSL CO2 in five healthy volunteers was 45.2 +/- 0.7 mm Hg (mean +/- SD). Twenty-six patients with physical signs of circulatory shock and LAC >2.5 mmol/L had a PSL CO2 of 81 +/- 24 mm Hg. This contrasted with patients admitted without clinical signs of shock and LAC of <2.5 mmol/L who had a PSL CO2 of 53 +/- 8 mm Hg (p < .001). The initial PSL CO2 of 12 patients who died before recovery from shock was 93 +/- 27 mm Hg, and this contrasted with 58 +/- 11 mm Hg (p < .001) in hospital survivors. Increases in PSL CO2 were correlated with increases in LAC (r2 = .84; p < .001). When PSL CO (2) exceeded a threshold of 70 mm Hg, its positive predictive value for the presence of physical signs of circulatory shock was 1.00. When it was <70 mm Hg, it predicted survival with a predictive value of 0.93. Conclusion PSL CO2 may serve as a technically simple and noninvasive clinical measurement for the diagnosis and estimation of the severity of circulatory shock states. Abstract:In both clinical and experimental settings, tissue P(CO2) measured in the oral mucosa is a practical and reliable measurement of the severity of hypoperfusion. We hypothesized that a threshold level of buccal tissue P(CO2) (P(CO2) BU)) would prognosticate the effects of volume repletion on survival. Twenty pentobarbital-anesthetized Sprague-Dawley male breeder rats, each weighing approximately 0.5 kg, were randomly assigned to one of four groups. Animals were bled over an interval of 30 min in amounts estimated to be 25, 30, 35, or 40% of total blood volume. One-half hour after the completion of bleeding, each animal received an infusion of Ringer lactate solution over the ensuing 30 min in amounts equivalent to two times the volume of blood loss. P(CO2) BU) was measured continuously with an optical P(CO2) sensor applied noninvasively to the mucosa of the left cheek. Arterial pressure and end-tidal CO2 were measured over the same interval. Neurological deficit and 72-h survival were recorded. Aortic pressures were restored to near baseline values for each of the four groups after fluid resuscitation. This contrasted with the improvement of P(CO2) BU), which differentiated between animals with short and long durations of postintervention survival. After electrolyte fluid resuscitation in rats subjected to rapid bleeding, noninvasive measurement of P(CO2) BU) was predictive of outcomes. Neither noninvasive end-tidal P(CO2) nor invasive aortic pressure measurements achieved such discrimination. Accordingly, P(CO2) BU) fulfills the criterion of a noninvasive and reliable measurement to guide fluid management of hemorrhagic shock.
Experimental Studies of Gastric Intramucosal Ph During Hemorrhagic Shock

Abstract:A simultaneous determination of arterial blood HCO 3 - concentration and of intraluminal PCO 2 makes it possible to calculate gastric intramucosal pH(pHi)by using Henderson-Hasselbalch equation. This study was designed to find out the relationship between pHi and intestinal tissue oxygenation. Results showed that there was a great decrease in pHi and portal lactate concentration(Lac p) during hemorrhagic shock. pHi reflected not only the intestinal tissue oxygenation but also the systemic oxygenation, and it correlated markedly with gastrointestinal mucosal damage. Based on these results, we conclude that pHi may be a good parameter to indicate the adequacy of tissue oxygenation.

Abstract:Our article, “Gastric Intramural Pco2 During Peritonitis and Shock” (Chest 1993; 104:1254-58), addressed increases in gastric intramural CO2 and H+ in experimentally induced peritonitis with circulatory shock. It concluded, “acid-base measurements on the stomach wall are indicative of the severity of visceral ischemia and therefore of perfusion failure. However, the prognostic value of intramural H+ was better than that of Pco2. The rate of change of these parameters differs contingent on the mechanism underlying the low visceral blood flow state. Predictability of intramural H+ and Pco2 measurements with respect to outcome was earlier with hemorrhagic than with septic shock. [The Effect of Two Methods Guided Fluid Therapy to the Patient Undergoing Open Gastrointestinal Surgery].

Abstract:OBJECTIVE:To investigate the effect of central venous blood oxygen saturation (ScvO2) and venous-arterial PCO2 (P(cv-a)CO2) guided fluid therapy on tissue perfusion, gastrointestinal function recovering and outcome of the patients who undergoing open gastrointestinal surgery. METHODS:Forty patients undergoing open gastrointestinal surgery were randomly divided into 2 groups (n = 20 each): ScvO2 guided fluid therapy (group S) and P(cv-a) CO2 guided fluid therapy (group P). All the patients were infused 10 ml/kg lactated Ringer's (LR) solution before anesthesia induction, they were all also given a continuous lactated Ringer's (LR) solution's infusion at the speed of 2 ml·kg(-1)·h(-1) during the operation. While, 6%HES 130/0.4 (free flex 6%HES 130/0.4, Fresenius Kabi) infusion was different between the 2 groups, when the patients of group S's central venous blood oxygen saturation < 75% or venous-arterial PCO2 in the patients of P group ≥6 mm Hg, then infused 6%HES 130/0.4. Arterial and central venous blood gas analyses were performed every 20 minutes after skin incision, measure the venous and arterial lactate value, and record the anal exhaust time after surgery, postoperative complications and mortality in 28 days. RESULTS:Compared with group S, the arterial lactate value in T4 (after operation began 80 min) were significantly decreased in group P (P = 0.013), and venous lactate value in T5 (after operation began 100 min) were also lower (P = 0.044), other lactate value were not different (P > 0.05) . The anal exhaust time was not different between the two groups (P = 0.673). All the patients were survival, and there were no obvious postoperative complications. CONCLUSION:Compared to group S, there was a transient improvement in tissue perfusion in group P, but there were no difference in complications and mortality.
