Double Action Alginate-Antacid Tablets Provide Effective and Safe Relief of Acid Reflux and Dyspepsia Symptoms in Chinese Patients With Gastro-Esophageal Reflux Disease (GERD)

Abstract:Background H2RAs decrease proton pump activation and theoretically, should interfere with PPI action when given concomitantly.On the other hand, PPIs are acid labile drugs and are generally administered in acid-resistant formulation.Therefore, simultaneous administration with H2RA may have some validity.However, expert opinion holds that this approach would decrease the effect of the PPIs.The aim of this study was to compare the pharmacodynamic effect of simultaneous administration of a PPI and H2RA.Hypothesis Simultaneous administration of PPI and H2RA will decrease the effectiveness of PPI to increase the intra-gastric pH.Methods This was a randomized crossover double-blinded prospective study performed on 21 healthy volunteers in a 3 arm design.Arm 1, subjects received H2RA (ranitidine 300 mg) once daily for one week.Arm 2, subjects received PPI (omeprazole 40 mg) once daily for one week.Arm 3, subjects received both H2RA and PPI at the same time for one week.All drugs were taken at 0800 and followed in 30 minutes by standard breakfast and 8 hour intragastric pH recording on day 7.The subjects were randomized so each received the 3 treatment modalities in different sequences.A 1-week washout period was given between each treatment modality.End point was length of time and percent time gastric pH remained < 4. Results The combination of ranitidine and omeprazole showed significantly shorter time of gastric pH < 4 (median 66.05 min) than either omeprazole or ranitidine alone (120.8 min; p < 0.02 and 338.9 min; p < 0.0001 respectively).Likewise, the percent time of gastric pH < 4 was significantly lower when omeprazole and ranitidine were given in combination (median: 13.95%) than either omeprazole or ranitidine alone (25.95%; p < 0.03 and 71.3%; p < 0.0001 respectively).Conclusion When PPI&H2RA are administered concomitantly 30 minutes before meals, H2RA did not decrease the effectiveness of PPI.Rather, combination of H2RA and PPI improved gastric acid control.Thus, the results failed to support the initial hypothesis of this study.Administration of H2RA and PPI together is reasonable.
