Abstract:BACKGROUND:Screening for latent tuberculosis (TB) is mandatory in inflammatory bowel disease (IBD) before starting anti-tumor necrosis factor therapy. Data on the utility of screening tests in populations with moderate background risk of TB are limited. This study aims to evaluate the performance of interferon-gamma release assay (IGRA) with QuantiFERON-TB Gold in IBD patients in a TB endemic region.METHODS:Two hundred sixty-eight consecutive adult IBD patients and 234 healthy controls were prospectively recruited. Detailed clinical history, chest x-ray findings, and IGRA results were documented for all individuals. The IGRA positive rates between IBD patients, with or without immunosuppressant, and healthy controls were compared.RESULTS:The IGRA result was positive in 21.9% of IBD patients and 19.2% of healthy controls (P = 0.535). IBD patients on immunosuppressive therapy had a significantly lower IGRA positive rate (13.0% versus 29.6%; P = 0.002) compared with immunosuppressant-naive IBD patients. This difference seemed to be most prominent for patients taking azathioprine (11.8% versus 27.3%, P = 0.006).CONCLUSIONS:IGRA results are negatively impacted by immunosuppressive therapy. Current guidelines suggesting TB screening before anti-tumor necrosis factor therapy may be inadequate in patients already on immunosuppressive drugs. Latent TB testing seems best performed before the initiation of immunosuppressive therapies in IBD patients.
The Diagnostic Efficiency Evaluation of Combination Tuberculosis Interferon-Gamma Release Assay with Human Leukocyte Antigen-G for Tuberculosis

Abstract:Objective To study the expression of IFN-γby tuberculosis specific T cells ( TB-IGRA) and Human Leukocyte Antigen-G( HLA-G) in the peripheral blood of tuberculosis patients and its diagnostic value in tuberculosis.Methods This was a retrospective cohort study . Study cases were all from Resparation Department of Shaoxing Municipal Hospital from January 2015 to December 2015. These patients were devided into tuberculosis group (TB) (34 cases), lung cancer group (LCa) (25cases) and non tuberculous pneumonia group (Non-TB pneumonia) (25cases) according to diagnosis, meanwhile, health group (35 cases) was set.IFN-γand sHLA-G were detected by ELISA , and mHLA-G was detected by Flowcytometre.The receiver operating characteristic curve ( ROC) was used to evaluate the diagnostic efficacy of TB-IGRA, sHLA-G and mHLA-G.Results The expression of TB-IGRA, sHLA-G and mHLA-G were higher in the TB group ( with the median of 182.5 ng/L,99.44 U/ml and 13.63%) than in the LCa group (117.0 ng/L,82.3 U/ml and 9.12%), Non-TB pneumonia group (80.84 ng/L,34.2 U/ml and 10.92%) and health group ( 30.5 ng/L, 31.28 U/ml and 8.18%) ( P<0.05 ) .The AUC-ROC of TB-IGRA, sHLA-G and mHLA-G were 0.842(95%CI, 0.761-0.922), 0.746(95% CI, 0.652-0.840) and 0.706(95%CI,0.605-0.806).The sensitivity for TB-IGRA, sHLA-G and mHLA-G was 88.24%, 70.59% and 67.65%, and the specificity was 60%, 72.94%and 70.59%at the cutoff value of 35 ng/L, 60 U/ml and 10% respectively.The sensitivity and specificity of combination detection for TB-IGRA and HLA-G were 88.24%and 81.18%.Conclusions TB-IGRA, sHLA-G and mHLA-G were all increased in TB, while, the quantitative detection of TB-IGRA is more helpful for diagnosis .The combination detection of TB-IGRA, sHLA-G and mHLA-G can improve the sensitivity in the diagnosis of tuberculosis .

Abstract:Objective To investigate the status quo of tuberculosis (TB) screening among patients with inflammatory bowel diseases (IBD) before receiving infliximab (IFX) therapy in China.Methods IBD patients receiving IFX treatment during January 2009 to December 2016 from 22 hospitals nationwide were enrolled retrospectively.Clinical data of TB screening before IFX therapy,including previous TB history,tuberculin test (TST test),interferon-γrelease assay (IGRA) and chest image results (chest X-ray and chest CT) were all recorded.TB screening result whether was latent tuberculosis infection (LTBI)judged by the doctor and clinical data related to whether receiving prophylactic treatment were collected.Results A total of 1711 IBD patients were enrolled,including 1606 (93.9％) cases receiving TB screening with TST or IGRA combining chest X-ray or CT scan before IFX treatment,663 (38.7％) cases with IGRA combining chest CT scan,and only 504 (29.5％) cases with complete TST plus IGRA combining chest CT scan,while 77(4.5％) cases had no chest imaging screening and 66(3.9％) cases did not receive any screening for TST or IGRA.TB screening results before IFX treatment showed 7.1％(121/1711) of IBD patients were LTBI,in whom 27 (27/121,22.3％) IBD patients with LTBI were neglected by clinicians,presenting as omission of judgment for LTBI or failure to take prophylactic anti-TB treatment during IFX treatment.Conclusions The TB screening before IFX treatment in China is not sufficient at present.The LTBI prevalence of IBD patients in China may be on the low side and clinicians have no enough knowledge of LTBI.It is recommended that IBD patients should undergo a standard TB screening before IFX therapy.

Abstract:Background: An increasing number of patients with inflammatory bowel diseases (IBD) are being treated with infliximab in China and are at increased risk of activating tuberculosis infection. Real-world epidemiological data on tuberculosis infection in patients with IBD receiving infliximab are scarce in China, where tuberculosis is endemic. The aim of our study is to investigate the risk of tuberculosis in a large cohort of IBD patients treated with infliximab in China. Methods: An observational study on all tuberculosis cases identified in IBD patients receiving infliximab was performed in 23 tertiary referral hospitals between 2009 and 2016 in China. Results of tuberculosis screening tests, which included interferon-gamma releasing assay (IGRA), tuberculin skin test (TST), chest X-ray, and computed tomography of the chest, prophylaxis against tuberculosis prior to infliximab therapy, and events of active tuberculosis infection after infliximab therapy were recorded and analyzed. Results: 1233 IBD patients receiving infliximab were recruited with a median follow up of 13 months (interquartile range [IQR] 6–24). 98% of the patients underwent screening tests prior to infliximab therapy. Screening results suggested the proportion of Chinese IBD patients with latent tuberculosis infection (LTBI) prior to infliximab therapy is 7.22% (89/1233). Twelve cases of active tuberculosis infection occurred after infliximab therapy. The incidence of active tuberculosis infection in Chinese IBD patients receiving infliximab therapy is 746 per 100,000 person-years, which is 9 times higher than that of the general Chinese population (75 per 100,000 person-years). Among IBD patients with LTBI, the incidence of subsequent active tuberculosis after infliximab therapy is 8 times higher than patients without LTBI (5.62% [5/89] versus 0.61% [7/1144], p<0.01). 97% of 89 LTBI patients and 30% of 1144 patients without LTBI received prophylaxis against tuberculosis. After prophylaxis, the incidence of tuberculosis infection didn't decrease in IBD patients without LTBI (0.57% [2/348] versus 0.62% [5/797], p=0.638). Conclusions: Infliximab therapy increased the incidence of tuberculosis in Chinese IBD patients. Routine prophylaxis may not reduce the risk of active tuberculosis infection in Chinese IBD patients without LTBI. Tuberculosis screening should be strongly recommended in Chinese IBD patients before initiating infliximab therapy.

Abstract:As Crohn’s disease (CD) has become more and more common in developing countries, tuberculosis still remains relatively common, thus differentiating CD from intestinal tuberculosis poses a great challenge to clinicians’ worldwide. There have been no good diagnostic methods until now. Recently, more cases using the TSPOT.TB test, an interferon-gamma (IFN-c) release assay of whole blood, could be highly specific for Mycobacterium tuberculosis. A 61-year-old male had abdominal distension and irregular stooling for 20 years. For the past 10 years his abdominal pain was more noticeable and was accompanied with green mucosy stools. His condition became more severe 2 years ago with right lower quadrant (RLQ) pain, nausea, and vomiting. B ultrasound showed a hypoechoic mass in the right lower quadrant diagnosed as ‘‘appendix abscess.’’ A second B ultrasound showed disappearance of the mass following antibiotic therapy. However, during the last year the RLQ pain returned and he underwent surgery to relieve intestinal obstruction caused by appendicitis and intestinal adhesions. One month later a partial colonoscopic examination was performed but inflammation and stricturing prevented a full examination. Double contrast barium enema showed stricturing and shortening of the ileocecum, ascending colon, and hepatic flexure. A second colonoscopy was successful and revealed ulceration of the cecum and ascending colon. Histology showed chronic inflammation. At the same time the patient also complained of severe pain in his joints. The medical team suspected CD or intestinal tuberculosis (TB). Differentiating CD from TB can be quite difficult. In this case our patient had a negative tuberculin test, but a strong positive T-SPOT result. Later, because the patient also had articule cavity dropsy, we obtained fluid from the cavity, which was found to contain tuberculosis antibodies, suggesting he had intestinal TB. We commenced antituberculosis treatment, which improved the patient’s clinical symptom and endoscopic presentation (Fig. 1). A second case was a 35-year-old female suffering from recurrent abdominal pain for 2 years which became more severe over 20 days. Colonoscopy showed multiple ulcers in the ascending colon and ileocecus and was diagnosed with CD. The T-SPOT assay was performed and was positive. FIGURE 1. Abstract:Background Anti-tumour necrosis factor (anti-TNF) therapy increases the risk of tuberculosis (TB). Given limitations of screening techniques, it remains uncertain if patients receiving anti-TNF in TB endemic regions should be screened for latent infection with chemoprophylaxis restricted to those with proven infection, or if all patients should receive chemoprophylaxis. Aims To compare the incidence of active TB with infliximab (IFX) following targeted and universal TB chemoprophylaxis, and to determine the rates of adverse events (AE) related to TB chemoprophylaxis Methods A multi-centre retrospective cohort study was performed at 18 hospitals in China of 1968 adult patients with IBD receiving IFX from 2009 to 2017. TB screening prior to IFX was performed with chest X-ray and/or computed tomography [CT] and immune reactivity testing (interferon-gamma release assay and/or tuberculin skin test). Patients were followed-up for a minimum of 3 months after IFX discontinuation, or until last hospital visit if IFX therapy was ongoing. Targeted strategy was defined as TB chemoprophylaxis only for patients with a positive latent TB screen, with universal strategy defined as TB chemoprophylaxis for all patients. Results Mean follow-up was 1.07 +/- 0.87 years with a total follow-up of 2102 patient-years. There were 1433 patients in the targeted and 483 patients in the universal TB chemoprophylaxis groups, with no significant difference in the incidence rates of active TB between groups (673.3 per 100 000 population per year vs 891.5 per 100 000 population per year, P = 0.60). In the targeted group, 55/1433 patients received TB chemoprophylaxis compared with 483/483 in the universal group, with significantly fewer AEs related to TB chemoprophylaxis in the targeted compared to the universal group (0.35% (5/1433) vs 6.8% (33/483), P < 0.05). Conclusions In this study of patients receiving IFX in a TB endemic area, universal chemoprophylaxis was not associated with a reduced risk of active TB when compared to a targeted chemoprophylaxis strategy, and AEs were more common. This supports the use of targeted TB chemoprophylaxis when anti-TNF therapy is initiated in TB endemic regions.

Abstract:Abstract Background Anti-tumor necrosis factor (TNF) therapy increased the risk of tuberculosis(TB), and the characteristic clinical feature of western patients with inflammatory bowel disease (IBD) receiving anti-TNF therapy were predominantly extrapulmonary TB. We aimed to analyze the clinical features of patients with IBD who in TB-endemic area (China) that developed active tuberculosis after receiving anti-TNF therapy. Methods Patients from 9 hospital in 7 provinces developed active TB after the initial treatment with infliximab. Demographics, interval between the time at the first dose of anti-TNF therapy and active TB development, tests for latent TB infection (LTBI), concomitant medications, and the details of diagnosis were analyzed in our study. The diagnosis of active TB and LTBI were made based on comprehensive evaluation including TB skin test (TST), interferon gamma releasing assay (IGRA), X-ray test and computed tomography (CT). Results Totally 43 patients developed active TB during infliximab therapy. Among 43 patients, 6 patients had a past TB history and 11 patients were confirmed as LTBI before infliximab treatment. Six patients with LTBI received anti-TB prophylaxis as the same time as anti-TNF treatment. The mean time between initiation of infliximab therapy and active tuberculosis diagnosis in patients with LTBI was significantly shorter than patients without LTBI (216.9±83.2 days versus 410.9±220.3 days, p=0.042). Among 43 active TB patients, 29 patients (29/43, 67.4%) had isolated pulmonary tuberculosis and 11 patients (11/43, 25.6%) had pulmonary TB concomitantly with other organ involvement. Conclusion IBD patients with LTBI develop TB earlier than those without LTBI during anti-TNF treatment, and history of treatment and anti-TB prophylaxis didn’t fully prevent active TB. The most common disease site is lung in China, most of cases were diagnosed by imaging. Chinese physicians should be aware of the potential for TB development during anti-TNF therapy, especially pulmonary tuberculosis.

Abstract:Background Biological therapies are increasingly used to treat inflammatory bowel disease (IBD) and other immune-mediated diseases. However, real-world epidemiological data on the risk of tuberculosis (TB) in these patients are scarce. We investigated the incidence of TB in patients with immune-mediated diseases in a population-based setting and stratified the risk of TB among different biologics. Methods Data on patient demographics, disease diagnosis, types of biologics and TB infections were collected from a territory-wide computerised database of patient records managed by the Hong Kong Hospital Authority. We calculated the incidence rates (IRs) of TB infections in subjects treated with different biologics between 2006 and 2015, and reported standardised incidence ratio (SIR) by comparing with the general population. Subgroup analyses were performed based on the types of immune-mediated diseases and biologic drugs. Results A total of 2485 patients with immune-mediated diseases were identified (10.6% IBD, 77.7% rheumatology, and 11.8% dermatology). 54 subjects developed active TB during 6921 person-years of follow-up. The mean age was 43 years, and the median follow up duration was 748 days. The overall SIR for TB in patients with included immune-mediated disease was 10.91. Patients with IBD had an over 17-fold increased risk of TB compared to the general population (SIR, 17.53), and patients on infliximab had a nearly 26-fold increased risk of TB compared to the general population (SIR, 25.95). Risk of TB was highest in the age group 50–59 years (SIR, 15.72). The risk of TB with infliximab was higher than etanercept (hazard ratio [HR], 4.10) and adalimumab (HR 2.08). No significant difference in the risk of TB was observed among the subgroups of IBD, rheumatology and dermatology. Conclusions In the population-based study, biological therapy is associated with higher risk of TB in patients with immune-mediated diseases compared with the general population, and infliximab is associated with the highest risk of TB amongst all biologics. This study suggests patient and biologic selection may be important in balancing the benefit and risk of TB when treating patients with immune-mediated diseases.

Abstract:Background and Aims: Distinguishing Crohn's disease (CD) from intestinal tuberculosis (ITB) is a clinical challenge. This meta-analysis assessed the clinical usefulness of Interferon-gamma releasing assay (IGRA) and anti-Saccharomyces cerevisiae antibody (ASCA) in the diagnosis of ITB and CD, respectively. Methods: Systematic search without language restriction was conducted in AMED, EBM, MEDLINE, EMBASE, and Google Scholar until September 2013. Studies that have evaluated performance of IGRA (QuantiFERON-TB Gold or T-SPOT. TB) or ASCA in distinguishing ITB from CD were eligible. Main outcome measures included sensitivity and specificity. Random-effects models were used to combine estimates from studies with significant heterogeneity. Area under the curve (AUC) was used to measure accuracy of the tests. Results: Eleven studies (five IGRA, three ASCA, three IGRA and ASCA) involving 1081 subjects were included. The pooled sensitivity, specificity, positive likelihood ratio, and negative likelihood ratio of IGRA for the diagnosis of ITB was 81% (95% CI, 75-86%), 85% (95% CI, 81-89%), 6.02 (95% CI: 4.62-7.83), and 0.19 (95% CI: 0.10-0.36), respectively. The AUC was 0.92. The pooled sensitivity and specificity of ASCA for the diagnosis of CD was 33% (95% confidence interval [CI], 27%-38%) and 83% (95% CI, 77-88%), respectively with an AUC of 0.58. T-SPOT. TB showed a higher sensitivity than QuantiFERON-TB Gold for the diagnosis of ITB. Conclusions: IGRA and ASCA have a high specificity for the diagnosis of ITB. Both IGRA and ASCA may have a supplementary role in the differential diagnosis between ITB and CD when conventional workup is not diagnostic.
P482 Selective Prophylactic Anti-Tuberculosis Strategy is Superior for Chinese Patients with Inflammatory Bowel Disease Receiving Infliximab Treatment: a Multi-Centre Retrospective Study

Abstract:During anti-TNF therapy, inflammatory bowel disease (IBD) patients either with or without latent tuberculosis infection (LTBI) active TB can develop. It remains unclear whether IBD patients without LTBI receiving prophylactic anti-TB has any clinical value or which prophylactic anti-TB strategy (for both LTBI and non-LTBI or only for LTBI) is superior. Furthermore, the optimum treatment regimen for IBD patients with LTBI receiving infliximab (IFX) in China are both unclear. This study was to investigate the optimal prophylactic anti-TB strategies and treatment regimens in Chinese IBD subjects receiving IFX treatment. IBD patients receiving IFX at 18 academic hospitals in China were enrolled. Incidence and risk factors for active TB during IFX treatment were studied. The incidence of active TB in IBD patients with IFX therapy receiving selective (strategy I: only anti-TB prophylaxis for LTBI) and conventional (strategy II: anti-TB prophylaxis for both LTBI and non-LTBI) prophylactic anti-TB strategies was compared. Further subgroup analysis investigated the incidence rate of active TB in LTBI and non-LTBI patients and examined effective prophylactic treatment regimens for LTBI. A total of 1968 IBD patients receiving IFX treatment were enrolled. The incidence rate of TB was 999.07 per 100000 population per year. At baseline, 166 cases (8.43%) tested positive for LTBI and 1802 cases negative for LTBI prior to IFX treatment. Of 1433 cases receiving strategy I and 483 cases receiving strategy II, 10 and 5 cases developed active TB, respectively. Figure 1. The incidence of active TB in IBD patients with IFX therapy receiving different strategy. And the incidence of active TB was not significantly reduced in IBD patients receiving strategy I compared with those receiving strategy II (0.07% vs. 1.04%, p = .67). The incidence of active TB (2.63% vs. 11.54%, p = .048) was significantly reduced but not eradicated in LTBI patients receiving prophylactic anti-TB therapy during IFX treatment, but these findings were not seen in non-LTBI patients. Furthermore, INH treatment for 6 months significantly decreased the incidence rate of active TB (0% vs. 11.54%, p = 0.045) in LTBI patients. Selective prophylactic anti-TB strategy may be superior for Chinese patients with IBD receiving IFX treatment and INH treatment for 6 months could be an effective treatment regimen for LTBI.
P040 Latent Tuberculosis Infection In A Patient With A Negative Interferon Gamma Release Assay Prior to Biologic Initiation for Crohn's Disease

Abstract:Biologic therapy has revolutionized the treatment of inflammatory bowel disease (IBD). Nevertheless, it is well known that biologics are associated with an increased risk of opportunistic infections such as tuberculosis (TB). Screening tests indicated before biologic initiation, namely interferon gamma release assay (IGRA) or tuberculin skin tests (TST), may be falsely negative. We present the case of a male with uncontrolled Crohn's disease (CD) with a negative screen for TB and how a high index of suspicion led to the discovery of latent tuberculosis infection (LTBI) and the appropriate deferment of biologic therapy. A 63-year-old South Asian male with coronary artery disease status post bypass graft, prostate cancer, diabetes and iron deficiency anemia presented with fatigue and dizziness for one week. He reported dark stools since immigration from Bangladesh two years ago. Vital signs and examination were unremarkable. Laboratory studies revealed a gradual decline in hemoglobin from 9 g/dL to 7 g/dL over several months. Esophagogastroduodenoscopy was unremarkable. Colonoscopy revealed friability, inflammation and erythema of the ileocecal valve and terminal ileum (TI) with minimal strictures. Biopsy was consistent with CD. He was started on azathioprine. Subsequent computed tomography (CT) enterography revealed thickened TI extending over a length of 9-10 centimeters. Due to disease severity and continued symptoms, he was referred to our tertiary care center where it was determined that he may benefit from biologic therapy. Patient underwent IGRA testing which was negative. Given his immigration from an endemic area and use of immunosuppressants, a CT of the chest was obtained which was significant for tree-in-bud opacities within the right upper lobe and multiple nodules. Transbronchial biopsy of the largest nodule showed necrotizing granulomatous inflammation. Bronchoalveolar lavage culture was positive for Mycobacterium tuberculosis. The patient was diagnosed with LTBI. Anti-tuberculin therapy was initiated, and biologic therapy was postponed. Biologic agents are increasingly utilized in the treatment of IBD. These agents confer an increased risk of infections i.e. TB and poor outcomes from severe disease. Patients with IBD are also susceptible to infections due to the immunosuppression imposed by the disease itself. The risk of TB varies with the biologic used with the highest relative risks associated with adalimumab and infliximab. Screening for LTBI is indicated in all patients for which biologics are being considered. The goal is to identify and treat individuals at high risk for TB reactivation. A thorough history and examination should be performed to assess pertinent signs and symptoms. While IGRA has demonstrated higher sensitivity and specificity compared to TST, immunosuppressed patients may have a false negative result as in our case. Combination testing may be the most sensitive approach though evidence is limited. In cases of high suspicion, imaging with plain radiograph or CT scan is a useful tool in supporting the diagnosis of LTBI and may show predominantly upper lobe cavitary disease. Biologic therapy for IBD should be deferred until at least one month of treatment for LTBI has been completed.
Screening of Latent Tuberculosis Infection by Interferon-Γ Release Assays in Rheumatic Patients: a Systemic Review and Meta-Analysis

Abstract:The aim of this study is to assess the diagnostic value of interferon-γ release assays (IGRAs) for latent tuberculosis infection (LTBI) in patients with rheumatic disease before receiving biologic agents. MEDLINE and EMBASE databases were used for searching studies concerning the evaluation on the performance of IGRAs [QuantiFERON-TB Gold (QFT-G), QuantiFERON-TB Gold In-Tube (QFT-GIT) and T-SPOT.TB] in rheumatic patients before biological therapy. After assessing the quality of all studies included in the review, we summarized the results in subgroups using forest plots and calculated pooled estimates if applicable. The search identified 11 studies with a total sample size of 1940 individuals. Compared with the tuberculin skin test (TST), the pooled agreements in QFT-G/GIT and T-SPOT.TB were 72 % (95 % confidence interval (CI) 65, 78 %) and 75 % (95 % CI 67, 83 %), respectively. BCG vaccination was positively correlated with positive rates of TST (pooled odds ratio (OR) 1.64, 95 % CI 1.06, 2.53). Compared with TST, IGRAs were better associated with the presentence of one or more tuberculosis (TB) risk factors. Neither steroid nor disease-modifying anti-rheumatic drugs (DMARDs) significantly affect positive IGRA results. In contrast, TST positivity was significantly impacted by the use of steroid (pooled OR 0.45, 95 % CI 0.30, 0.69), but less significantly by the use of DMARDs (pooled OR 0.78, 95 % CI 0.50, 1.21). In conclusion, in rheumatic patients with previous BCG vaccination or currently on steroid therapy, IGRAs would be the better choice to identify LTBI by decreasing the false-positivity and false-negativity rate compared with conventional TST.

Abstract:Background: Tuberculosis (TB) is a leading cause of fever of unknown origin (FUO). In recent years, interferon-gamma release assays (IGRAs) have been widely utilized and the cut-off values given by the manufacturers are set in countries where rates of TB are not as high. Methods: A prospective cohort study was conducted in a Chinese general hospital to evaluate the diagnostic performance of T-SPOT.TB (T-SPOT) and QuantiFERON-TB Gold (QFT) in detecting active TB (ATB) in a high TB endemic area. Test results were compared with the culture and clinically confirmed diagnosis. Further, we explored an alternative method of interpreting IGRAs by increasing the cut-off values. Results: The sensitivity and specificity of T-SPOT in detecting ATB were 85.3% (95% CI 81.6-94.0%) and 71.8% (95% CI 67.3-76.0%), respectively. The sensitivity and specificity of QFT were 72.3% (95% CI 62.8-80.1%) and 77.0% (95% CI 72.7-80.8%), respectively. Receiver operating characteristic analysis was used for evaluation of different cut-off values. When the cut-off values were adjusted as 125 spot-forming cells (SFCs)/2.5*10(5) cells for T-SPOT and 4.0 IU/ml for QFT, the specificity could be improved to > 90.0% (90.3% and 94.1%, respectively), and the sensitivity were 43.1% and 41.6%, respectively. The new adjusted cut-off values were validated in another independent validation cohort. Conclusion: The adjusted cut-off values of the two assays considerably improved the diagnostic value when applied to FUO patients in clinical settings.

Abstract:Background:Anti-tumor necrosis factor drugs are known to reactivate latent tuberculosis (TB). Current guidelines recommend screening for latent tuberculosis infection, with either tuberculin skin test (TST) or interferon gamma release assays such as QuantiFERON-TB Gold (QFT-G). Given the high rates of anergy to TST among immunosuppressed inflammatory bowel disease (IBD) patients, there is considerable interest in evaluating the superiority of interferon gamma release assays over TST in this patient population to diagnose latent tuberculosis infection. We compared the performance of TST and QFT-G for screening latent TB among immunosuppressed IBD patients based on prevalence, mortality risk from reactivation TB, and costs.Methods: A decision analytical model was constructed to compare 1-year outcomes and costs of using TST or interferon gamma release assay in an immunosuppressed IBD population.Results: Under the base case scenario, for every 1000 patients screened, the QFT-G strategy resulted in 0.53 deaths from reactivation TB compared with 1.92 deaths using TST. The QFT-G strategy results in 1.85 reactivation TB versus 6.7 reactivation TB using TST. The model was not sensitive to background prevalence of latent TB. The cost of QFT-G would have to be more than double for the TST strategy to become more cost effective. QFT-G also remains the cost-effective option unless the sensitivity of the TST improves by 400%.Conclusions: Under a broad range of parameter values, the QFT-G strategy dominates the TST strategy in cost-effectiveness. Consideration should be given to QFT-G as the preferred method of identifying latent TB in all immunosuppressed IBD patients.
The In-Vitro Interferon-Gamma Release Assay for the Diagnosis of Tuberculosis and Mycobacterium Tuberculosis Infections

Abstract:OBJECTIVE:To investigate the in-vitro interferon-gamma (IFN-gamma) release assay based on three different Mycobacterium tuberculosis antigens in the diagnosis of tuberculosis and Mycobacterium tuberculosis infections.METHODS:The peripheral blood mononuclear cells (PBMC) were collected from the patients with tuberculosis (tuberculosis group, n = 57), patients with lung cancer (lung cancer group, n = 29), and healthy controls (healthy control group 2, n = 27). The PBMCs were co-cultured for 5 days with different antigens: purified protein derivatives (PPD) of tuberculin, early secretary antigenic target 6,000 protein (ESAT6) and 38,000 antigen. The protein levels of IFN-gamma were detected by ELISA, and the results were compared to those with the tuberculin skin test (TST).RESULTS:(1) For healthy controls, the TST was positively related to the history of BCG vaccination and the closeness of contact with sputum-positive tuberculosis patients (P = 0.047, P = 0.041 respectively). The ESAT6 based IFN-gamma release assay was only significantly related to the closeness of contact with sputum-positive tuberculosis patients (P = 0.005), but not to the history of BCG vaccination. (2) There was no significant difference of the TST results among the three groups (P > 0.05). (3) The receiver operating characteristic (ROC) curve analysis indicated that the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of the IFN-gamma release assay based on 38,000 antigen were 64.9%, 89.3%, 77.0%, 86.0%, and 71.0% respectively for the diagnosis of tuberculosis.CONCLUSIONS:IFN-gamma release assay based on ESAT6 appears to be better than TST in the diagnosis of infection of Mycobacterium tuberculosis, while IFN-gamma release assay based on 38,000 may be helpful for the diagnosis of tuberculosis.

Abstract:BACKGROUND: Active tuberculosis infection represents a very common and significant threat to HIV-infected patients. But measures to accurately detect it are limited. OBJECTIVE: To compare and analyze the diagnostic efficacy of T-SPOT.TB alone and in combination with TST in HIV-infected patients in China. METHOD: TST (tuberculin skin test) and T-SPOT.TB were performed on 131 HIV-infected patients admitted in Beijing You'an Hospital and Beijing Ditan Hospital between Oct, 2010 and Jul, 2012, who were initially diagnosed as suspected ATB (active TB). The patients were further categorized into ATB and Not ATB based on clinical and cultural evidences. The performance of TST and T-SPOT.TB were analyzed and compared. RESULTS: The sensitivity and specificity of T-SPOT.TB were 41.3% and 94.6%, respectively, both higher than TST (12.9% and 91.8%). By combining T-SPOT.TB and TST, the sensitivity did not increase, but specificity was elevated to 100%. TST, T-SPOT.TB and their combinations all performed better in patients with extra-pulmonary diseases than with pulmonary disorders. False-positive T-SPOT.TB results were found to be associated with history of prior TB. In addition, concomitant bacterial infections and low CD4 counts were associated with increased ATB risk. CONCLUSIONS: T-SPOT.TB is superior in screening ATB in HIV-infected patients in China over traditional TST. Additional TST would help to confirm a positive T-SPOT.TB result. Both tests work better for patients with extra-pulmonary conditions.

Abstract:Objective Interferon. release assay (IGRA) is commonly used to diagnose latent TB infection (LTBI). Immunosuppressive therapy may affect its performance but data are conflicting. We aimed to determine the effect of immunosuppressive therapy on the performance of IGRA in patients with autoimmune diseases.Methods We searched PubMed, MEDLINE, EMBASE and the Cochrane Library up to December 2014. We included studies that reported the IGRA results in patients with autoimmune disease with or without immunosuppressive therapy. The pooled effect of immunosuppressive therapy on IGRA was estimated using a Peto fixed-effects model.Results We included 17 studies with 3197 participants in the meta-analysis. Among the subjects, 71.5% were taking immunosuppressive therapy and 56.7% had received Bacillus Calmette-Guerin vaccination. Compared with patients not on immunosuppressants, patients receiving immunosuppressive therapy were less likely to have a positive IGRA result (OR 0.66, 95% CI 0.53 to 0.83, I-2=23%), especially patients receiving anti-tumour necrosis factor (anti-TNF) treatment (OR 0.50, 95% CI 0.29 to 0.88). The use of immunosuppressive therapy was also associated with a lower rate of positive tuberculin skin test result (OR 0.51, 95% CI 0.42 to 0.61).Conclusions Our meta-analysis showed that IGRA results are negatively affected by immunosuppressive therapy. IGRA alone may not be sufficiently sensitive to diagnose LTBI in patients on immunosuppressive therapy. Patients should preferably be screened for LTBI before initiation of immunosuppressive therapy, especially before anti-TNF therapy.

Abstract:Background: Bronchoscopic examinations are vital to diagnose pulmonary diseases. However, as coughing is triggered during and after the procedure, it is imperative to take measures against nosocomial infections, especially for airborne infections like tuberculosis (TB). The interferon-γ release assay (IGRA) has recently been established as a method to evaluate the infection status of TB. We aimed to ascertain the efficacy of IGRA and clinical findings in estimating the prevalence of active TB before bronchoscopy. Methods: We obtained IGRA results from 136 inpatients using a QuantiFERON-TB Gold In-Tube test. Bronchoscopy samples were cultured in Mycobacteria Growth indicator tubes and 2% Ogawa solid medium. We evaluated the adjusted effects of multiple clinical variables on active TB status using a logistic regression model. In addition, multiple variables were converted into a decision tree to predict active TB. Results: Five (3.7%) patients were diagnosed with culture-positive TB, two of whom were simultaneously diagnosed with non-small-cell lung carcinoma or small-cell lung carcinoma. The multivariate analysis suggested the probability of predicting active TB using the IGRA [odds ratio (OR), 72.7; 95% confidence interval (CI), 3.169-1668; P=0.007] and decreased estimated glomerular filtration rate (eGFR) (OR, 0.937; 95% CI, 0.882-0.996; P=0.038) in patients undergoing bronchoscopy. A decision tree validated the use of these two variables to predict active TB. Conclusions: IGRA test results are useful for predicting active TB before bronchoscopy. This strategy could identify patients who require antibiotic therapy to prevent TB or who are in the active phase of TB.
Screening of tuberculosis infection in household contacts by interferon γ release assay and tuberculin skin test to predict active tuberculosis in China

Abstract:The purpose of the present study is to investigate the performance of interferon γ release assay (IGRA) and tuberculin skin test (TST) in household contacts of tuberculosis patients ,and to evaluate the predictive values of the two tests for disease progression from latent tuberculosis infection (LTBI) to active tuberculosis in China .Household contacts of active pulmonary tuberculosis patients were offered both IGRA (T-SPOT .TB) and TST ,and then followed up for 6 years .The final cohort comprised 121 household contacts .T-SPOT .TB was positive in 49 .6% of the contacts;the positive rate was associated with increasing age﹝ OR=1 .03 (95% CI 1 .00-1 .05) ,P=0 .032﹞ and contacting with smear-positive patients ﹝ OR=2 .13 (95% CI 1 .01-4 .51) ,P=0 .048﹞ .TST≥5 mm and TST≥10 mm accounted for 57 .5% and 37 .2% of the contacts , respectively . TST ≥ 10 mm was positively associated with contacting with smear-positive patients﹝ OR=2 .30 (95% CI 1 .05-5 .06) ,P=0 .038﹞ .Seven cases progressed to tuberculosis within 626 person-years of follow-up ,showing an overall incidence rate of 11 .0 per 1 000 person-years .The positive predictive value (PPV ) and negative predictive value (NPV ) was 8 .3% and 96 .7% for T-SPOT .TB .The PPVs for TST≥5 mm and TST≥10 mm were 7 .7% and 9 .5% respectively;NPVs were both 95 .8% .In conclusion , there is a high incidence rate of active tuberculosis among household contacts compared to general population in China .The predictive values of IGRA and TST for progression to active tuberculosis among household contacts are comparable .
