Abstract:Introduction: Short chain fatty acids (propionate, butyrate and acetate) are produced in the colon by the anaerobic fermentation of carbohydrates by the gut bacteria, and account for many physiological interactions between the gut microbiota and the host.Among these, it has very recently been shown that gut-derived propionate regulate blood pressure by inducing vasodilation.The liver is the main site of propionate clearance.We have recently shown that propionate levels are increased in patients with cirrhosis and correlates with the severity of portal hypertension, raising the possibility that propionate plays a pathogenic role in the hemodynamic abnormalities of cirrhosis, characterized by arterial hypotension, portal hypertension and arterial vasodilation.The aim of this study was to assess, in an animal model of cirrhosis, if propionate contributes to cirrhosis hemodynamic derangement.Methods: We studied a total of 42 rats with cirrhosis induced by common bile duct ligation and sham operated rats 28 days after the surgery.An additional group of CBDL rats was treated with metronidazole from day 24-28 after the surgery, since this is the most effective therapy to decrease gut propionate production.Main readouts were plasma and cecal propionate levels, and systemic and splanchnic hemodynamics (portal pressure (PP), mean arterial pressure (MAP) and superior mesenteric artery blood flow (SMBF).Results: As expected CBDL rats showed an increased portal pressure (PP), decreased mean arterial pressure (MAP) and increased superior mesenteric artery blood flow (SMABF).Further, these rats showed significantly increased plasma propionate levels as compared with sham rats (0.13 vs 0.7 mM respectively;p=0.047), together with a significant down-regulation in the liver expression of the enzymes responsible for propionate metabolism (PCC, MUT and MCEE).Cecal content did not show a significant increase in propionate in CBDL rats, altogether suggesting that increased propionate was the result of impaired liver metabolism.Metronidazole treatment induced a marked decrease in cecal propionate and decreased plasma propionate in CBDL rats.This was associated with a decrease in PP (from 14.7 mmHg in untreated rats to 12.9; p=0.038) and in SMABF (from 24.4 to 17.5 ml/min; p=0.024), and an increase in MAP (from 85 to 96 mmHg; p=0.001).In metronidazole treated rats, restoration of high plasma propionate levels with an i.v.infusion of propionate abrogated beneficial hemodynamic effects of metronidazole.Conclusion: Metronidazol treatment improves systemic and splanchnic hemodynamics in cirrhotic rats.This effect is, at least in part, mediated by a decrease in propionate, since it is reversed by restoring plasma propionate levels.Altogether our data suggest that decreasing circulating propionate might improve hemodynamics in cirrhosis.
