Abstract:Objective. To determine the level of IFN-alpha in labial salivary glands, plasma, and peripheral blood cells from patients with primary Sjogren's syndrome (pSS).Methods. Labial salivary gland biopsy specimens, plasma, and peripheral blood cells from patients with pSS were investigated. The IFN-alpha-positive cells, measurable IFN-alpha level, and IFN-alpha gene mRNA level were determined by using immunohistochemistry, ELISA, and real-time PCR, respectively. Statistical analysis was performed by Student t test or Fisher's exact test.Results. About 60% of patients (22/37) with pSS had significantly higher scores of IFN-alpha-positive cells in labial gland biopsy and most IFN-alpha-positive cells were localized predominantly in the lymphocytes and ductal epithelial cells. But in 3 of the control samples (3/24), the IFN-alpha-positive cells existed only in the ductal epithelial cells with lower scores. Forty-three percents of the patients with pSS were found with detectable IFN-alpha concentration in plasma (>= 12.5 pg/ mL), and their concentration was higher than that of control group. Furthermore, the IFN-alpha mRNA levels in peripheral blood cells were up-regulated in the patients with pSS.Conclusion. No matter in labial salivary glands or hematoplasma, or peripheral blood cells, IFN-alpha expression levels are up-regulated in patients with primary Sjogren's syndrome. Abstract:Primary Sjögren’s syndrome (pSS) is a systemic autoimmune disease with exocrine gland dysfunction and multi-organ involvement. Recent progress in understanding the pathogenesis of pSS offers an opportunity to find new biomarkers for the diagnosis and assessment of disease activity. Screening noninvasive biomarkers from the saliva and tears has significant potential. The need for specific and sensitive biomarker candidates in pSS is significant. This review aims to summarize recent advances in the identification of biomarkers of Sjögren syndrome, trying to identify reliable, sensitive, and specific biomarkers that can be used to guide treatment decisions.

Abstract:Sj(o)gren's syndrome(SS)is a chronic autoimmune inflammatory disease characterized by predominant infiltration of lymphocytes in exocrine glands as well as presence of several autoantibodies in sera.Primary SS(pSS)is the condition in the absence of other definitively diagnosed autoimmune diseases.The pathogenesis of pSS remains unclear.Currently,pSS is considered to be induced by multiple environmental factors and endogenous antigens,which trigger a self-perpetuating inflammatory response in susceptible individuals,lead to abnormal cellular and humoral immunity followed by the impairment of exocrine glands,production of autoantibodies and damage to target organs.Although no ideal therapy is available,biological agents have offered new clues to the treatment of pSS.
[The Expression of Interferon-Alpha in Peripheral Blood of Patients with Primary Sjogren's Syndrome].

Abstract:PURPOSETo investigate the interferon-alpha level in peripheral blood of patients with primary Sjogren's syndrome (pSS), and explore preliminarily the correlation between pSS and interferon-alpha.METHODSELISA and real-time PCR were used to evaluate the interferon-alpha level in peripheral blood in 37 patients with pSS and 24 patients with non-pSS. Statistical analysis was performed by Student's t test or Fisher's exact test using SAS6.12 software package.RESULTSThe results demonstrated that 43.24% (16/37) patients with pSS had detectable IFN-alpha concentration in plasma (>or=12.5 pg/mL), the other 21 patients (56.76%) with pSS had undetectable IFN-alpha concentration(<12.5 pg/mL). All 24 controls had undetectable IFN-alpha expression.There was significant difference between the two groups(P=7.02 x 10(-5)). Furthermore, the mRNA expression levels of IFN-alpha gene in peripheral blood cells were over-expressed in patients with pSS(P=0.0124).CONCLUSIONSThe study reveals that interferon-alpha level in peripheral blood with pSS patients is overexpressed.

Abstract:Primary Sjögren’s syndrome (pSS) is a chronic autoimmune disease, characterized by lymphocytic infiltration into exocrine glands, which causes dry eyes, dry mouth, and systemic damage. Although the precise etiology of pSS is not clear yet, highly activated B cells, abundant anti-SSA/Ro, and anti-SSB/La autoantibodies are the hallmarks of this disease. Follicular helper T cells (Tfh), a subset of CD4+T cells, with cell surface receptors PD-1 and CXCR5, express ICOS, transcription factor Bcl-6, and a cytokine IL-21. These cells help in the differentiation of B cells into plasma cells and stimulate the formation of germinal center (GC). Previous studies have demonstrated abundant Tfh cells in the peripheral blood and salivary glands (SGs) of the patients with pSS, correlated with extensive lymphocytic infiltration of the SGs and high disease activity scores. Patients with pSS who are treated with abatacept (CTLA-4 Ig) show fewer circulating Tfh cells, reduced expression of ICOS, and lower disease activity scores. Recently identified follicular regulatory T (Tfr) cells, a subset of regulatory T cells, control the function of Tfh cells and the GC reactions. Here, we summarize the observed alterations in Tfh and Tfr cell numbers, activation state, and circulating subset distribution in pSS. Our goal is to improve the understanding of the roles of Tfh and Tfr cells (surface marker expression, cytokine production, and transcription factors) in the pathogenesis of pSS, thus contributing to the identification of candidate therapeutic agents for this disease.

Association of Antibodies to Interferon-Inducible Protein-16 With Markers of More Severe Disease in Primary Sjögren's Syndrome

Abstract:Objective: Interferon-inducible protein-16 (IFI16) is an intracellular DNA receptor involved in innate immunity. We evaluated the frequency, phenotypic characteristics, and clinical associations of anti-IFI16 antibodies in patients with primary Sjögren's syndrome (SS), and quantitated expression levels of IFI16 in SS and control salivary gland lysates. Methods: Anti-IFI16 antibodies were assayed by enzyme-linked immunosorbent assay using sera from patients with primary SS (n = 133) and from healthy controls (n = 47). Sera from systemic lupus erythematosus (SLE) patients (n = 132) were included as disease controls. Immunoprecipitation of in vitro transcription-translated IFI16 was used to determine which portion of IFI16 the antibodies recognized. Expression of IFI16 in salivary gland lysates was quantitated by immunoblotting. Results: Anti-IFI16 antibodies were present in the sera of 38 of 133 SS patients (29%) compared to 1 of 47 healthy controls (2.1%) (SS versus controls; P < 0.0002) and in 31 of 132 SLE controls (24%). In SS, anti-IFI16 antibodies were associated with an abnormal Schirmer's test (P = 0.003), hyperglobulinemia (P = 0.02), antinuclear antibody ≥1:320 (P = 0.01), germinal center-like structures in labial salivary gland lymphoid infiltrates (P = 0.01), and higher focus scores (3.4 versus 2.4; P = 0.005). High-titer IFI16 antibodies were directed against an epitope outside the N-terminus in 9 of 13 SS patients (69%). IFI16 was expressed in 4 of 5 (80%) of SS and 1 of 6 (17%) of control labial salivary glands. Conclusion: Anti-IFI16 antibodies are a prominent specificity in primary SS and are associated with markers of severe disease. IFI16 is expressed at higher levels in SS salivary glands compared to controls. These high levels in disease target tissue may contribute to the ongoing anti-IFI16 immune response.
Interleukin 17E Associates with Haematologic Involvement and Autoantibody Production in Primary Sjögren's Syndrome.

Abstract:OBJECTIVES:Primary Sjögren's syndrome (pSS) is one of the most prevalent systemic autoimmune diseases characterised by inflammation and tissue damage of exocrine glands, especially salivary or lacrimal gland. IL-17 related immune response is pathogenic with proinflammatory feature in pSS. However, whether IL-17E, an IL-17 family member, is involved in pSS pathogenesis or not, has not been determined.METHODS:Serum levels of IL-17E and IL-17A as comparison in 107 patients with pSS and 42 healthy controls were determined with multiplex cytokine assays. EULAR Sjögren's syndrome disease activity index (ESSDAI) score was calculated. Laboratory parameters were measured by standard laboratory techniques. The inflammatory infiltration of minor labial gland biopsies was graded based on numbers of lymphocyte and quantified by Focus Score (FS). Expression of IL-17E and IL-17A in the biopsy was evaluated with immunohistochemistry.RESULTS:Significantly elevated IL-17E in pSS patients associated with ESSDAI, haematologic disorders and autoantibody production, including anti-nuclear antibodies (ANA), rheumatoid factor (RF) and anti-SSA antibodies were found. Histopathological features showed that expression of IL-17E was found in labial salivary gland and correlated with lymphocytic infiltration.CONCLUSIONS:IL-17E expression in pSS patients was increased and associated with haematologic disorders, autoantibody production and lymphocytic infiltration in salivary gland. This finding indicated that IL-17E is involved in pSS pathogenesis.

Abstract:Objective To investigate the expression of interleukin-17 in the salivary gland and peripheral blood of patients with primary Sjogren's syndrome (pSS).Methods Biopsy specimens of labial glands were collected from 30 patients and 5 controls.Immunohistochemistry examination was conducted to detect IL-17 expression cells (Th-17),while CD45RO and CD20 were tested by using monoclonal antibodies.IL-17 levels were assessed by ELISA in 30 pSS patients and 20 healthy controls.Results Th-17 cell numbers in the salivary glands of pSS patients with few infiltrating lymphocytes were lower than that with massive infiltrating lymphocytes (15±5 vs 21±8,P＜0.05).However,the percentage of Th-17 positive cells over the total infiltrating lymphocytes in the salivary glands of pSS patients with few infiltrating lymph-ocytes was higher than that with massive infiltrating lymphocytes (P＜0.05).There was no expression of IL-17 in the controls.The distribution pattern of Th-17 was shown that Th-17 cells were distributed evenly in the salivary glands of pSS patients with few infiltrating lymphocytes,but only appeared in the periphery zone of massive or focal infiltration in the glands of pSS patients with massive infiltrating lymphocytes.The amount of Th-17 positive cells were associated with the disease activity which was evaluated by levels of erythrocyte sedimentation rate (r=0.557,P＜0.05).There was no significant differ-ence in IL-17 levels of peripheral blood between patients with pSS and normal controls.Conclusion The results of this study have shown that Th-17 is expressed in the salivary glands of patients with pSS,which indicates that IL-17 may play an important role in the pathogenesis of salivary gland destruction.
Expression of interferon regulatory factor 1, 3, and 7 in primary Sjögren syndrome.

Abstract:The aim was to investigate the level of interferon regulatory factor (IRF) 1, 3, and 7 in peripheral blood cells from patients with primary Sjogren syndrome (pSS) and to determine whether and where IRF1 exists in the parotid glands of pSS.Peripheral blood cells and parotid gland biopsy specimens from patients with pSS were studied. The IRF1, IRF3, and IRF7 gene mRNA levels in peripheral blood cells were calculated by using real-time PCR. The IRF1-positive cells in the parotid glands with pSS were observed by using immunohistochemistry and immunofluorescence. Statistical analysis was performed by Student t test.Compared with 24 control samples, the IRF1 mRNA levels in peripheral blood cells of 37 cases with pSS were up-regulated (P < .05), but the IRF3 and IRF7 mRNA levels of pSS were not up-regulated (P > .05). Relative quantitative levels of IRF1 mRNA were 2.17-fold higher in pSS patients than control subjects. The IRF1-positive cells of the pSS group were localized in the epithelial islands, lymphocytes, and ductal epithelial cells of the parotid glands. In all control subjects, the IRF1-positive cells were localized only to the ductal epithelial cells of parotid glands as determined by immunohistochemical staining or immunofluorescence. The scores of IRF1-positive cells of pSS were significantly higher than that of control samples (P < .05).These findings indicate that IRF1 mRNA levels are up-regulated in the peripheral blood cells of pSS patients. Also, IRF1-positive cells exist in the epithelial islands, lymphocytes, and ductal epithelial cells of the parotid glands of individuals affected by pSS, but are limited to the ductal epithelial cells of healthy control subjects.
Autoantibodies against interferon-γ reduce the frequency of pulmonary fibrosis and concentration of C-reactive protein in patients with primary Sjögren's syndrome.

Abstract:Primary Sjögren's syndrome (pSS) is a prototypic autoimmune disease characterized by immune injury to the salivary and lacrimal glands. In addition, some patients with pSS develop several systemic complications, such as renal, pulmonary or neurological manifestations. The establishment of type I interferons (IFN) pathway designates pSS an autoimmune disease characterized by an “IFN-signature” [1], but type II IFN, IFN-γ, also participates in the pathogenesis of pSS. Expression of high levels of IFN-γ mRNA and serum IFN-γ is found in conjunctiva and labial salivary glands in patients with pSS and correlates with degree of T-cell infiltration [2–4]. In animal models, high levels of IFN-γ can be detected in NOD/ShiLtJ and NOD-derived congenic C57BL/6.NOD-Aec1Aec2 mice newborned [5,6]. High production of IFN-γ was also found in muscarinic type 3 receptor−/− (M3R−/−) mice treated with free-form extracellular peptides of M3R [7]. IFN-γ-producing M3R-reactive T cells play a crucial role in the generation of SS-like sialadenitis via the induction of apoptosis [8]. However, few studies have been performed on the anti-IFN-γ autoantibodies (aAbs) in patients with pSS.
Association between IL-7 and primary Sjögren's syndrome: A single-center study and a systematic scoping review

Abstract:BackgroundThere is a lack of single marker reflecting systemic activity of primary Sjögren's syndrome (pSS). Our aim is to determine the association between interleukin(IL)-7 and pSS by combining a single-center study and a systematic scoping review.MethodsThere were 58 patients with pSS and 30 healthy controls (HCs) included in the single-center study. The multiplex immunoassay was used for detecting concentrations of IL-7, IL-4, IL-9, IL-10, IL-17, interferon(IFN) -γ, INF-α and INF-β in sera. pSS patients were evaluated for systemic activity in accordance with the European League against Rheumatism SS Disease Activity Index (ESSDAI). In the systematic scoping review, all studies regarding association of IL-7 with pSS were included.ResultspSS patients showed higher serum IL-7 levels (P = 0.028 vs HCs) which were associated with neutrophil, neutrophil-to-lymphocyte ratio(NLR), platelet-to-lymphacyte ratio(PLR), red blood cell distribution width(RDW), erythrocyte sedimentation rate(ESR), immunoglobulin(Ig)G, C-reactive protein (CRP), fever, lymphadenopathy and ESSDAI. Serum IL-7 correlated with Th cell related cytokines. Findings of the systematic scoping review on 15 articles were as follows. Firstly, IL-7 expression in salivary glands (SGs), serum and saliva increased in pSS, while IL-7 receptor(IL-7R) expression increased in SGs but decreased in peripheral blood. Secondly, increased IL-7 was mainly from SGs of pSS patients, but whether IL-7 was mainly produced by SG epithelial cells remained to be validated. Thirdly, IL-7 could exert a key role in pSS immunopathology. Although IL-7 had no direct effect on SGECs, it could activate B and T cells and stimulate secretion of IL-17, IL-4 and IFNs.ConclusionNot only are IL-7 and soluble IL-7R potential biomarkers for monitoring pSS activity, but also targeting IL-7/IL-7R pathway may be a promising therapeutic strategy.

[Serum IL-21 Level in Patients with Primary Sjogren's Syndrome and Clinical Significance of IL-21].

Abstract:AIM:To investigate the relation between the change of serum IL-21 level and other laboratory indexes by detecting serum IL-21 level from patients with primary Sjogren's syndrome (pSS) and explore the role of IL-21 in pSS.METHODS:The level of serum IL-21 from 40 pSS patients diagnosed according to 2002 revised international classification criteria for pSS and 30 healthy persons were detected by enzyme-linked immunosorbent assay(ELISA) respectively. Meanwhile, FT3, FT4, TSH, TgAb, TPOAb were detected by Immunochemiluminescent technique. Double diffusion method, Westergren and automatic gel method detected ant-SSA and ant-SSB antibody, ESR and serum protein electrophoresis respectively. The relationship between serum IL-21 level and other clinical symptom of the patients was analyzed.RESULTS:The level of serum IL-21 in patients with pSS (1051+/-335) ng/L was obviously higher than that in healthy control (466+/-90 ng/L), (P<0.05). The levels of serum IL-21 in the self-antibody positive group, parotid gland swelling pain group, rash group and incorporating hypothyroidism group were higher than those in their negative control groups, and there were significant differences(P<0.05). In addition, The level of serum IL-21 level in pSS patients showed positive correlation with gamma-globulin (r=0.719, P<0.05) and ESR level(r=0.745, P<0.05).CONCLUSION:The evident increase of serum IL-21 level in pSS patients has positive correlation with the levels of gamma-globulin and ESR, which suggests that IL-21 may play a critical role in the pathogenesis of pSS.
Characteristics of Primary Sjögren’s Syndrome Patients with IgG4 Positive Plasma Cells Infiltration in the Labial Salivary Glands

Abstract:The purpose of this study was to investigate the characteristics of primary Sjögren’s syndrome (pSS) patients with IgG4 positive (IgG4+) plasma cell infiltration in labial salivary glands (LSGs). Paraffin sections of LSGs from 336 pSS patients were stained with IgG4 and IgG monoclonal antibodies. According to the infiltration of IgG4+ plasma cells, patients were divided and clinical and serological characteristics were analyzed and compared. Based on the infiltration of IgG4+ plasma cells in the LSGs, patients were divided into three subgroups, low IgG4, moderate IgG4, and high IgG4 groups. A negative association between the number of infiltrated IgG4+ plasma cells and the disease characteristics was observed. We found that the higher the IgG4+ expression in plasma cells, the lower the positive rates of serum anti-SSA antibodies, anti-SSB antibodies, antinuclear antibodies (ANA), and rheumatoid factor (RF). Besides, patients from the high IgG4 group had the highest frequency of interstitial lung disease (ILD, 30.6%) and tubulointerstitial nephritis (TIN, 13.9%), but the lowest frequency of leucopenia (13.9%), thrombocytopenia (11.1%), and abnormal thyroidal function (0%). PSS patients with different IgG4+ plasma cells infiltration in the LSGs had distinctive clinical and laboratory characteristics. It may help us to further understand the role of IgG4+ plasma cells in pSS.
Biomarkers of Interstitial Lung Disease Associated with Primary Sjögren's Syndrome.

Abstract:Abstract Objectives The aim of this study was to investigate serum biomarkers linked to primary Sjögren's syndrome (pSS)-associated interstitial lung disease (ILD). Methods 69 pSS patients were consecutively enrolled and evaluated via quantitative ILD scoring based on high-resolution computed tomography (HRCT). Biomarkers of interest were assessed by multiplex enzyme-linked immunosorbent assays (ELISAs). Results Among consecutively enrolled patients with pSS, the presence of pSS–ILD was 50% based on the presence of radiographically defined interstitial lung abnormalities (ILA) meeting specified criteria for mild/moderate (ILA 2) or severe (ILA 3) disease. Age, immunoglobulin M (IgM), C-reactive protein (CRP), and serum levels of eotaxin/CCL11, Krebs von den Lungen-6 (KL-6), TNFα, and TGFα were significantly higher in the combined pSS–ILD group (ILA 2 + ILA 3) than in the pSS–no-ILD and pSS–indeterminate ILD groups (ILA 0 and ILA 1, respectively) in unadjusted analyses (p < 0.05 for all variables). A binary logistic regression model revealed that disease duration and KL-6 levels were associated with the presence of pSS–ILD (p < 0.05). Complementary least absolute shrinkage and selection operator (LASSO) modeling showed that age, KL-6, and TNF-α effectively differentiated pSS–ILD (ILA 2 + ILA3) from pSS without ILD (ILA 0 + ILA 1), with an area under the curve (AUC) of 0.883 (p value < 0.0001). Conclusions Patient age, disease duration, and serum levels of both KL-6 and TNFα were the most discriminating factors associated with the presence of ILD in our pSS patients. Higher levels of CRP, IgM, eotaxin, TGFα, and TNFα should also prompt the search for occult as well as clinically evident lung involvement based on statistically significant univariate associations with pSS–ILD. Abstract:Sjögren's syndrome is a chronic autoimmune disease characterized by lymphocytic infiltration of the salivary and lachrymal glands resulting in dry eyes and mouth. Genetic predisposition, pathogenic infections and hormones have been implicated in the pathogenesis of the disease. Studies in the last several years have revealed marked over-expression of the type I interferon (IFN)-inducible genes in the peripheral blood and salivary glands of patients with Sjögren's syndrome. The expression of the type I IFN-inducible genes in Sjögren's syndrome also positively correlates to titers of anti-Ro and anti-La autoantibodies, which are typical for this disease. Plasmacytoid dendritic cells (pDC) are the major source of type I IFN production and activated pDC are detected in minor salivary gland biopsies from patients with primary Sjögren's syndrome. In addition, polymorphisms in genes important both for the production and response to type I IFN are associated to increased risk for Sjögren's syndrome. Because type I IFN bears a variety of biological functions, such as defense against viral infections and activation of the immune system, these results suggest that the type I IFN system has an important role in the pathogenesis of Sjögren's syndrome. A variety of mechanisms causing an activation of the type I IFN system are discussed in this review. Given the pivotal role of type I IFN in the disease process, therapeutic interventions targeting the type I IFN signaling pathway have the potential to benefit the patients with elevated type I IFN status and such hypothesis needs to be carefully evaluated in clinical development.

Abstract:Objective To investigate the relationship between anti-lymphocyte antibody (ALA) and clinical and immunological parameters in primary Sj(o)gren syndrome (pSS).The significance of ALA and lymphopenia in pSS was explored.Methods Serum samples were obtained from 143 patients with pSS.Clinical and laboratory data were collected.Sera from 45 healthy individuals were collected as the controls.Serum ALA IgG was detected by indirect immunofluorescence test (IIFT).x2 test and t test were used for statistical analysis.The reasons of lymphopenia in pSS patients were analyzed.The correlation between ALA,lymphopenia and clinical manifestations as well as laboratory parameters was analyzed.Results ① Of the 143 pSS patients,25.2％(36/143) were serological positive for ALA,which was significantly higher than the normal group (6.5％,5/77) (x2=27.496,P＜O.01).② The prevalence of leukopenia (55.6％,20/36),lymphopenia (77.8％,28/36),anti-nuclear antibody(80.5％,29/36) and C3 decrease(50.0％,18/36) was higher in pSS patients with ALA than those without (x2=8.296,25.348,3.929,3.866,P＜0.05).③ 46.2％(66/143) patients had lymphopenia.Comparing with patients with normal lymphocyte count,the incidence of ALA positive,leukopenia,PLT decrease and anti-SSB antibodies positivity was significantly higher in patients with lymphopenia (x2=27.496,51.344,5.320,8.950,7.782; P＜0.05).Conclusion The ALA is likely to be a meaningful sera marker for pSS patients with lymphopenia,and this suggests that it has an important role in the clinical progress of SS.
