Abstract:Low left ventricular ejection fraction (LVEF) and the presence of restrictive LV filling pattern are poor prognosticators in heart failure patients with reduced EF (HFREF). We sought to investigate whether acoustic cardiography can identify these high-risk HFREF subgroups.A total of 127 HFREF patients (EF<50%) were enrolled into our study. All patients underwent acoustic cardiographic and echocardiographic examinations. Acoustic cardiographic parameters included S3 score (probability that the third heart sound exists), electromechanical activation time (EMAT, interval from Q wave to the first heart sound; %EMAT is the proportion of cardiac cycle that EMAT occupies), and systolic dysfunction index (SDI, a derived variable from the combination of %EMAT, S3 score, QRS duration and QR interval). Receiver operating characteristic curves were used to determine diagnostic utility of acoustic cardiography.SDI discriminated (area under curve [AUC], 0.79; 95% confidence interval [CI], 0.71-0.87) patients with severely impaired EF (EF ≤ 35%) from those with moderately impaired EF (35%<EF<50%) with an SDI > 5 that yielded 87% sensitivity and 60% specificity. An S3 score>4 identified patients with restrictive LV filling pattern with 0.76 AUC (95% CI, 0.67-0.84), 81% sensitivity and 55% specificity.SDI and S3 score obtained by acoustic cardiography identified HFREF patients with severely impaired systolic and diastolic function, respectively. This simple, bedside technology may be used as a screening tool to identify the sickest HFREF patients for more intensive therapy.
Validation of the Use of Foreign Gas Rebreathing Method for Non-Invasive Determination of Cardiac Output in Heart Disease Patients

Abstract:Objective: To compare a new device (Innocor) for non-invasive measurement of cardiac output (CO) by foreign gas rebreathing method with conventional techniques used in the measurements of cardiac function. Methods: Cardiac outputs measured by Innocor (CO RB ) were compared with CO obtained by echocardiography (CO EC ), Swan-Ganz thermodilution (CO TD ), and left ventricle radiography (CO LVR ) in 34 patients subjected to cardiac catheterization. Values obtained from the four methods were analyzed by linear regression and paired values were compared by the method of Bland and Altman in SPSS. Results: There was strong positive correlation ( r =0.94) between Innocor cardiac output values and the corresponding values obtained by thermodilution and between CO EC and CO LVR values. Thermodilution appears to overestimate cardiac output when compared to the values obtained with Innocor by (0.66±0.22) L/min ( P <0.0001). There was no correlation between data obtained by Innocor and the corresponding CO EC and CO LVR values. Conclusion: Innocor CO RB is an easy, safe and well established method for non-invasive measurement of cardiac output with good prospects for clinical application in heart disease patients.

Abstract:INTRODUCTION:Chronic obstructive lung disease is not only a major cause of morbidity and mortality, but is also the major reason for intensive care unit (ICU) admission. Cardiac function is often impaired in this disease, but its association with clinical outcome has not been fully established.METHODS AND ANALYSIS:This is a prospective observational study conducted in a 47-bed mixed ICU of a tertiary academic teaching hospital. The study will be performed from January 2014 to December 2015. All patients meeting the diagnostic criteria of acute exacerbation of chronic obstructive pulmonary disease and admitted to the ICU are potentially eligible for the present study. The relevant demographics and laboratory measurements have been obtained. Transthoracic echocardiography was performed immediately after ICU admission by experienced intensivists. The Cox proportional hazard regression model has been fitted by using a stepwise forward selection and backward elimination technique. If linear assumption is not satisfied, the linear spline function will be used.ETHICS AND DISSEMINATION:The study protocol was approved by the ethics committee of Jinhua municipal central hospital. The results will be published in a peer-reviewed journal and shared with the worldwide medical community.TRIAL REGISTRATION NUMBER:The study protocol is registered at ClinicalTrials.gov (NCT02099279).
Correlation and Influencing Factors Between End-Tidal Carbon Dioxide and Arterial Carbon Dioxide Tension in Children with Congenital Heart Disease

Abstract:The pediatric congenital heart diseases are usually divided into cyanotic and non-cyanotic heart diseases. Due to the hemodynamic change caused by either left-right or right-left shunt, the value of arterial-to-end-tidal carbon dioxiede tension dif-ference (P(a- et)CO2) increases, which makes it difficult using pressure of end-tidal carbon dioxide (PETCO2) to predict pressure of arterial carbon dioxide (PaCO2). Therefore, in order to avoid excessive artery blood gas analysis, it is important to estimate and pre-dict PaCO2 exactly, especially in pediatric patients. The present review gathers various factors that probably influence P(a-et)CO2 and try to make a simple conclusion, based on published data recently.
Indirect cardiac output assessment in a swine pediatric acute respiratory distress syndrome model

Abstract:Purpose: To investigate the correlation between volume of carbon dioxide elimination (V̇CO2) and end-tidal carbon dioxide (PETCO2) with cardiac output (CO) in a swine pediatric acute respiratory distress syndrome (ARDS) model. Methods: Respiratory and hemodynamic variables were collected from twenty-six mechanically ventilated juvenile pigs under general anesthesia before and after inducing ARDS, using oleic acid infusion. Results: Prior to ARDS induction, mean (SD) CO, V̇CO2, PETCO2, and dead space to tidal volume ratio (Vd/Vt) were 4.16 (1.10) L/min, 103.69 (18.06) ml/min, 40.72 (3.88) mmHg and 0.25 (0.09) respectively. Partial correlation coefficients between average CO, V̇CO2, and PETCO2 were 0.44 (95% confidence interval: 0.18-0.69) and 0.50 (0.18-0.74), respectively. After ARDS induction, mean CO, V̇CO2, PETCO2, and Vd/Vt were 3.33 (0.97) L/min, 113.71 (22.97) ml/min, 50.17 (9.73) mmHg and 0.40 (0.08). Partial correlations between CO and V̇CO2 was 0.01 (-0.31 to 0.37) and between CO and PETCO2 was 0.35 (-0.002 to 0.65). Conclusion: ARDS may limit the utility of volumetric capnography to monitor CO.
Noninvasive Evaluation of Pulmonary Vascular Development in Congenital Heart Disease with Decreased Pulmonary Blood Flow

Abstract:Objective To seek a new method for noninvasive estimation of pulmonary artery. Methods Pulmonary artery diameters were measured at proximal, middle, distal portions on DSA and tapes of echocardiography in 112 patients with congenital heart disease with decreased pulmonary blood flow. The correlation was analyzed between the results of echocardiography and angiocardiography. The correlative analysis was done between echocardiography pulmonary artery index (EPAIs) and postoperative outcome. Results Excellent correlations between echocardiography and angiocardiography were obtained (the main pulmonary artery: r=0.79, proximal portion of the left pulmonary artery: r=0.73, proximal portion of the right pulmonary artery: r=0.77, distal portion of the left pulmonary artery: r=0.69, distal portion of the right pulmonary artery: r=0.89). The length of day in ICU, mechanical ventilation time, dose of inotropic drugs correlated well with EPAI, when EPAI≤115 mm2/m2, postoperative hemodynamics was unstable, the frequency of low cardiac output syndrome and mortality significantly increased. Conclusion EPAI is an excellent morphological indicator of pulmonary blood flow and development status of pulmonary vessels. It may improve the decision-making strategy.
Non-invasive Cardiac Output Measurement in Children Using Bioreactance:A Comparison with Echocar-Diography

Abstract:Objective To investigate the value of measurement of cardiac output in children by bio-reactance versus echocardiography.Methods Pediatric patients admitted in pediatric department of Peking University First Hospital from September to December 2012 who needed hemodynamic monitoring were enrolled prospectively.Cardiac index(CI)and stroke volume(SV)were measured by echocardiography and non-invasive cardiac output measurement(NICOM)and compared by Spearman correlation and Bland-Alt-man analysis.Results Thirty patients were included.The median age was 7.25 years.CI[M(P5 ,P95 )] measured by NICOM and echocardiography were correlated significantly[3.42(2.28,4.92)L /(min?m2 ) vs.3.51 (2.94,4.85 )L/(min?m2 ),R =0.385,P =0.035 ].Bland-Altman analysis revealed a bias of-0.22 L/(min?m2 )(P =0.051 ),limits of agreement of -1.40 to 0.95 L/(min?m2 ).SV[M(P5 ,P95 )] measured by NICOM and echocardiography were correlated more significantly [36.3 (12.6,87.8 )ml vs.39.4(14.7,86.9)ml,R =0.768,P ﹤0.001 ].Bland-Altman analysis revealed a bias of -3.1 ml(P =0.176),limits of agreement of -27.4 to 21.2 ml.Conclusion There is no significant difference between NICOM and echocardiography for the measurement of CI and SV in pediatric patients.Further validation studies need to be conducted before routine clinical use.
Clinical significance of pulse indicator continuous cardiac output in pediatric patients undergoing open-heart surgery

Abstract:Objective To evaluate the clinical significance of PiCCO in pediatric patients with three different types of congenital heart disease[tetralogy of Fallot(TOF),ventricular septal defect(VSD),VSD+pulmonary hypertension(PH)]after open-heart surgery.Methods From August 2010 to June 2011,30 patients of 0-3 years old who underwent open-heart surgery were prospectively included in this study.There were 10 cases with tetralogy of Fallot,10 cases with VSD and pulmonary hypertension,and 10 cases with VSD without pulmonary hypertension.PiCCO analyzer continuously measured hemodynamic data on 0 h,4 h,8 h,12 h,24 h,48 h,72 h after operation,including cardiac index(CI),global end-diastolic volume index(GEDVI),extra vascular lung water index(EVLWI)etc;LAP,CVP,PAP,PVRI,inotropic score,plasma lactate levels and PaO2/FiO2 were also measured and recorded;Plasma CK-MB activity,CK-MB mass,troponin were tested before surgery,and on 4 h,24 h,48 h,72 h after surgery.Results(1)There were 10 patients each in VSD,VSD+PH and TOF group.(2)At the time of 4 h after surgery,the CK-MB activity,CK-MB mass,troponin levels get the highest value,and than gradually reduced.(3)The cardiac index in all patients decreased significantly to the lowest nadir on 8-12 h after surgery,but not significant compare to 0 h postoperative.CI in VSD+PH and TOF group was significantly improved on 48 h after surgery compare to 0 h postoperative.The CI of TOF group was significantly lower than the VSD and the VSD+PH group at all the time postoperative.(4)VSD+PH group of patients had significantly higher mPAP and PVRI than that of VSD and TOF group,while the mPAP and PVRI of TOF group showed no significant difference with the VSD group.(5)GEDVI was significantly lower in TOF and VSD+PH group than VSD group during the early postoperative period.(6)The EVLWI was significantly higher in VSD+PH group than TOF group at 48 h after surgery,whereas it was significantly higher in TOF group than VSD group.Conclusions The use of PiCCO in pediatric patients after open-heart surgery in this study reveal a continuous detailed data such as CI,GEDVI,EVLW,SVRI and PVRI,which were not clear in 3 different types of congenital hearty diseases to guide accurate diagnose and treatment.
Analysis of P(v-a)CO2/C(a-v)O2 Ratio and Other Perfusion Markers in a Population of 98 Pediatric Patients Undergoing Cardiac Surgery

Abstract:Background: The so-called Low Cardiac Output Syndrome (LCOS) is one of the most common complications in pediatric patients with congenital heart disease undergoing corrective surgery. LCOS requires high concentrations of inotropes to support cardiac contractility and improve cardiac output, allowing for better systemic perfusion. To date, serum lactate concentrations and central venous oxygen saturation (ScVO2) are the most commonly used perfusion markers, but they are not completely reliable in identifying a state of global tissue hypoxia. The study aims to evaluate whether the venoarterial carbon dioxide difference/arterial-venous oxygen difference ratio [P(v-a)CO2/C(a-v)O2] can be a good index to predict the development of LCOS in the aforementioned patients, so as to treat it promptly. Methods: This study followed a population of 98 children undergoing corrective cardiac surgery from June 2018 to October 2020 at the Department of Cardiac Surgery of University Hospital Integrated Trust and their subsequent admission at the Postoperative Cardiothoracic Surgery Intensive Care Unit. During the study, central arterial and venous blood gas analyses were carried out before and after cardiopulmonary bypass (CPB) (pre-CPB and post-CPB), at admission to the intensive care unit, before and after extubation, and at any time of instability or modification of the patient's clinical and therapeutic conditions. Results: The data analysis shows that 46.9% of the children developed LCOS (in line with the current literature) but that there is no statistically significant association between the P(v-a)CO2/C(a-v)O2 ratio and LCOS onset. Despite the limits of statistical significance, however, a 31% increase in the ratio emerged from the pre-CPB phase to the post-CPB phase when LCOS is present. Conclusions: This study confirms a statistically significant association between the most used markers in adult patients (serum lactate concentration, ScVO2, and oxygen extraction ratio-ERO2) measured in the pre-CPB phase and the incidence of LCOS onset, especially in patients with hemodynamic instability before surgery.
Development the Low Cardiac Output Syndrome Risk Score for Children after Congenital Heart Surgery

Abstract:Background Low cardiac output syndrome(LCOS) is a common serious complication after pediatric congenital heart disease. This research aims to develop the risk score using clinical and laboratory indicators to predict LCOS. Methods 118 children with congenital heart disease who underwent cardiopulmonary bypass surgery with pulse index continuous cardiac output(PICCO) monitoring after surgery in Shanghai Children's Medical Center from January 2023 to December 2023, and cytokines were collected before and after surgery. We divided the patients into development and validation datasets by calendar date. According to the cardiac index (CI) monitored by PICCO, the patients were divided into LCOS group (CI < 2L/min/m2) and non-LCOS group (CI ≥ 2L/min/m2). R software was used for data collation and analysis. Nomogram model were evaluated using ROC analysis. Results Patients in LCOS group have higher IL-5(p = 0.045), higher IL-8(p = 0.002), higher IL-10(p = 0.020), higher Sa-vO2(p = 0.004) and higher cTnI(p = 0.025) compared with those in non-LCOS group. IL-10(OR = 2.04,P = 0.012) and Sa-vO2(OR = 2.08,P = 0.001) were associated with the occurrence of LCOS, which were independent risk factors contained in the nomagram. Good agreement by nomogram could be detected in the calibration curve for the risk of LCOS. The optimal cutoff values for IL-10 for predicting LCOS was 8.7 (sensitivity: 53.1%, specificity: 69.8%) and Sa-vO2 was 34% (sensitivity: 65.6%, specificity: 73.3%), respectively. Combined IL-10 and Sa-vO2 to predict LCOS was 42.3 with the sensitivity 71.9% and specificity 64%. Conclusions Combined IL-10 and Sa-vO2 which reflect the state of oxygen supply and immune response could be a new index to predict LCOS after congenital heart surgery which were easily to be tested than other invasive tools.
Carotid Doppler Ultrasonography for Hemodynamic Assessment in Critically Ill Children

Abstract:An accurate assessment of cardiovascular performance is essential to predict and evaluate hemodynamic response to interventions. The objective of this prospective study was to assess whether point-of-care ultrasonography of the common carotid artery (CCA) can estimate the stroke volume (SV) and cardiac index (Ci) of critically ill children. Participants underwent Doppler ultrasonography of the left CCA and transthoracic echocardiography (TTE). Variables measured by TTE were SV and Ci. Carotid blood flow (CBF) was calculated based on both systolic velocity-time integral (CBF(s)) and total velocity-time integral (CBF(t)). Carotid corrected flow time(CFT)was also determined. A total of 50 children were enrolled. The median age and weight of participants were 36.0 months and 14.2 kg, respectively. Both CBF(s) and CBF(t) correlated very strongly with SV (ρ = 0.98 and 0.97, respectively) and Ci (ρ = 0.96 and 0.92, respectively). Agreement analysis showed low biases and clinically acceptable percentage errors between variables measured by TTE (SV and Ci) and those estimated by Doppler ultrasonography. Linear regression analysis revealed that the Ci of mechanically ventilated children can be estimated by the following equation: [Formula: see text]. CFT did not significantly correlate with SV or Ci (ρ = 0.27 and 0.05, respectively). Doppler ultrasonography of the left CCA is able to estimate the SV and Ci of critically ill children. Therefore, the CDU may be considered as an alternative for estimating Ci in critically ill children when TTE is not feasible or available.

Abstract:Background: Optimization of cardiac output (CO) has been evidenced to reduce postoperative complications and to expedite the recovery. Likewise, CO and other dynamic cardiac parameters can describe the systemic blood flow and tissue oxygenation state and can be useful in different clinical fields. This study aimed to validate the qCO monitor (Quantium Medical, Barcelona, Spain), a new device to estimate CO and other related parameters in a continuous, fully non-invasive way using advanced digital signal processing of impedance cardiography. Methods: The LiDCOrapidv2 (LiDCO Ltd, London, UK) was used to compare the performance of the qCO in 15 patients during major surgery under general anesthesia. Full surgeries were recorded and cardiac output obtained by both devices was compared by using correlation and Bland-Altman analysis. Results: The Bland-Altman analysis showed sufficient agreement with a mean bias of -0.03 ± 0.71 L/min. Conclusions: The findings showed that both systems offered comparable values and thus the non-invasive measurement of CO with qCO is a promising, feasible method. Further investigation will be required to validate this new device against calibrated devices and outcome studies would also be highly recommended.

Abstract:Arterial pressure wave form analysis for cardiac output (CO) and stroke volume variation(SVV) measurement is a new, semi-invasive technique. It does not require thermodilution or dye dilution, but rather bases its calculations on arterial wave-form characteristics in conjunction with patient demographic data (age, gender, height, weight and so on). This method shows quite accuracy and convenience though some defect. In some patients, such as aortic reflux and mitral reflux, it can not measure CO ac-curately.

Abstract:<h3 class="article-section__sub-title section1"> Objective</h3><p>Bronchial asthma is a common chronic inflammatory airway disease, which may be associated with pulmonary hypertension and cardiac dysfunction. The aim of this study was to evaluate the ability of 2D‐speckle tracking echocardiography (2D‐STE) and tissue doppler imaging (TDI) to detect subtle cardiac dysfunction in pediatric patients with mild to moderate bronchial asthma.</p><h3 class="article-section__sub-title section1"> Methodology</h3><p>The study included 30 children with mild to moderate bronchial asthma and 27 age‐matched healthy controls. Both groups underwent pulmonary function tests, TDI and 2D‐STE. Myocardial performance index (MPI), S′, E′, A′ velocities, global strain of left ventricle (LV), right ventricle (RV), and right atrium (RA) were measured.</p><h3 class="article-section__sub-title section1"> Results</h3><p>RV diastolic function was impaired in the patient group, as the tricuspid E′ velocity was significantly lower in the patients when compared with the controls (16 [14‐17] vs 16 [17‐19] cm/s, <i>P</i> = .044), while the RV‐MPI was significantly higher in patients when compared to controls (0.30 [0.27‐0.36] vs 0.30 [0.30‐0.30], <i>P</i> = .001). The global RV longitudinal strain, RA strain, and LV strain did not show significant differences between the test and the control groups. RV systolic parameters and LV systolic and diastolic parameters did not differ significantly between the two groups. </p><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Pediatric patients with mild to moderate bronchial asthma may have early RV diastolic dysfunction with preserved other cardiac functions.</p>

Abstract:Background and Objective: Estimating cardiac output (CO) and other hemodynamic parameters from blood pressure (BP) measurements could aid in selecting appropriate antihypertensive treatments in clinical settings. Our research group has developed a non-invasive method to estimate CO based on total arterial compliance (Ct). However, its validity in populations with various cardio-metabolic chronic diseases remains uncertain. The objective of this study is to assess the validity of our CO estimation method across different patient with varied clinical conditions. Methods: A cross-sectional study was conducted involving 217 adult patients who underwent ambulatory transthoracic Doppler echocardiograms, irrespective of valvular or cardiac pathology, or medication use, provided they had normal aortic valvular anatomy. Stroke volume was calculated as the product of the cross-sectional area of the aortic annulus and the average velocity of the left ventricular outflow tract. Simultaneously, oscillometric BP measurements were obtained. Subsequently, the CO values obtained via Doppler echocardiography and those estimated based on Ct were compared to determine validity. Ct was estimated as: Ct/BSA= 38/PPth + 4/5 × Td/T - 3/7, where BSA denotes body surface area, Td signifies diastolic time, T represents the cardiac period, and PPth refers to the theoretical pulse pressure. Results: Of the participants, 49.7% were men, with an average age of 54.0±14.0 years. Among them, three had a history of acute myocardial infarction, one had a pacemaker, nineteen were diabetic, and one hundred thirty-six were hypertensive. CO by echocardiography averaged 4.9±1.0 L/min, ranging from 2.0 to 8.5 L/min. The agreement of CO between the new method and Doppler echocardiography was good. Bland-Altman plots showed that the differences between the methods were randomly distributed along the mean difference line (Figure). Conclusions: The novel method for noninvasive estimation of CO based on Ct with brachial oscillometric BP measurements in patients with various clinical conditions is deemed valid. Its implementation in clinical practice has the potential to transform patient assessment.

Correlation between cerebral oxygen saturation and cardiac function

Abstract:Objective To prospectively evaluate the correlation between cerebral oxygen saturation (ScO2) and cardiac function and its clinical values.Methods Eighty-four consecutive American Society of Anesthesiologists (ASA) grade Ⅰ-Ⅳ patients undergoing off-pump coronary artery bypass grafting (OPCABG) receiving general anesthesia were included in this study.ScO2,hemodynamic parameters including heart rate (HR),mean arterial pressure (MAP),central venous pressure (CVP),mean pulmonary arterial pressure (MPAP) and pulmonary artery wedge pressure (PAWP),continuous invasive cardiac output monitoring parameters including cardiac output (CO),cardiac index (CI),stroke volume (SV),stroke index (SI),systemic vascular resistance index (SVRI),pulmonary vascular resistance index (PVRI),left ventricular stroke work index (LVSWI),right ventricular stroke work index (RVSWI) and venous oxygen saturation (SvO2) and left ventricular ejection fraction (LVEF) were collected.The patients were divided into normal cardiac function group (n =42,CI＞2) and cardiac dysfunction group (n =42,C3I＜2) according to the median CI.The above-mentioned parameters were compared between the groups.Receiver operating characteristic (ROC) curve analysis was used to identify the clinical value of ScO2in the monitoring of cardiac function.Results ScO2 was positively correlated with LVEF (r =0.215,P =0.049),MAP (r=0.248,P=0.023),differential pressure between MAP and CVP (MAP-CVP,r=0.220,P=0.045),CO (r=0.347,P=0.001),CI (r=0.276,P=0.011),SV (r=0.320,P=0.003),SI (r=0.249,P=0.022) and SvO2 (r =0.225,P =0.040).ScO2 was negatively correlated with MPAP (r =-0.478,P＜0.001),SVRI (r=-0.418,P＜0.001) and PVRI (r=-0.531,P＜0.001).But ScO2was not correlated with HR,CVP,PAWP,LVSWI or RVSWI (all P＞0.05).Compared with those in the cardiac dysfunction group,ScO2 was significantly increased (P＜0.01) but CVP,MPAP and PAWP were significantly decreased (P＜0.05 or P＜0.01) in normal cardiac function group.ScO2 (are aunder concentration-time curve [AUC] =0.648,95％ CI:0.531-0.765,P =0.020) had larger area under ROC curve than LVEF (AUC =0.492,95 ％ CI:0.365-0.620,P=0.904),HR (AUC=0.526,95％CI:0.399-0.652,P=0.687),MAP (AUC=0.446,95％CI:0.322-0.571,P=0.398),CVP (AUC=0.281,95％CI:0.173-0.388,P=0.001),MPAP (AUC=0.268,95％CI:0.157--0.379,P＜0.001),MAP-CVP (AUC=0.538,95％CI:0.413-0.664,P=0.546) and PAWP (AUC=0.362,95％CI:0.243-0.482,P=0.030).ScO2(AUC=0.648,95％CI:0.531-0.765,P=0.020) had smaller area under ROC curve than CO (AUC=0.972,95％CI:0.939-1.005,P＜0.001),SVRI (AUC=0.041,95％CI:0.005-0.087,P＜0.001),PVRI (AUC=0.163,95％CI:0.079-0.248,P＜0.001),SV (AUC=0.885,95％CI:0.809-0.961,P＜0.001),SI (AUC=0.944,95％CI:0.895-0.993,P＜0.001),LVSWI (AUC=0.860,95％CI:0.781--0.938,P＜0.001),RVSWI (AUC=0.824,95％CI:0.734-0.914,P＜0.001) and SvO2(AUC=0.707,95％CI:0.594-0.819,P=0.001).ROC curve analysis showed that the detection rate of CI＞2 by ScO2 was 66 ％,and the sensitivity and specificity were all 0.643.Conclusion ScO2 is significantly related to cardiac function.It has important clinical value in real-time monitoring of cardiac function.
Correlation and concordance of carotid Doppler ultrasound and echocardiography with invasive cardiac output measurement in critically ill patients

Abstract:Background: Critical care management heavily relies on accurate cardiac output (CO) measurement. Echocardiography has been a mainstay in non-invasive cardiac monitoring; however, its comparability to invasive methods warrants further exploration. Recent studies have suggested the potential of carotid Doppler measurements as a promising approach to estimate CO. Despite this potential, the literature presents mixed outcomes regarding its reliability and accuracy. This study aims to evaluate the correlation and concordance between carotid Doppler ultrasonography and invasive hemodynamic monitoring in estimating CO in critically ill patients. Furthermore, it assesses the concordance and correlation between echocardiography CO and the standard invasive CO measurements. Methods: This concordance study involved critically ill adults requiring invasive CO measurement. Patients with arrhythmias, severe valvulopathy, pregnancy, and poor acoustic window were excluded. Statistical analyses comprised univariate analysis, Wilcoxon signed-rank test, Spearman correlation, and intraclass correlation coefficient. Ethical approval was granted by the institution's ethics committee. Results: A total of 49 critically ill patients were included, predominantly male (63.27%), with a median age of 57 years. Diagnoses included subarachnoid hemorrhage (53.06%) and heart failure (8.16%). Mean cardiac index was 3.36 ± 0.81 L/min/m2 and mean cardiac output was 5.98 ± 1.47 L/min. Spearman correlation coefficient between echocardiography and invasive CO measurements was 0.58 (p-value = p < 0.001), with an ICC of 0.59 for CO and 0.52 for cardiac index. Carotid measurements displayed no significant correlation with invasive CO. Conclusion: There is a moderate correlation and concordance between echocardiography and invasive CO measurements. There is no significant correlation between carotid variables and invasive CO, underscoring the necessity for cautious interpretation and application, particularly in patients with distinctive cerebral blood flow dynamics.
Validation of an Ultrasound Dilution Technology for Cardiac Output Measurement and Shunt Detection in Infants and Children

Abstract:OBJECTIVE: To validate cardiac output measurements by ultrasound dilution technology (COstatus monitor) against those obtained by a transit-time ultrasound technology with a perivascular flow probe and to investigate ultrasound dilution ability to estimate pulmonary to systemic blood flow ratio in children.DESIGN: Prospective observational clinical trial.SETTING: Pediatric cardiac operating theater in a university hospital.MATERIAL AND METHODS: In 21 children (6.1 ± 2.6 kg, mean ± SD) undergoing heart surgery, cardiac output was simultaneously recorded by ultrasound dilution (extracorporeal arteriovenous loop connected to existing arterial and central venous catheters) and a transit-time ultrasound probe applied to the ascending aorta, and when possible, the main pulmonary artery. The pulmonary to systemic blood flow ratio estimated from ultrasound dilution curve analysis was compared with that estimated from transit-time ultrasound technology.RESULTS: Bland-Altman analysis of the whole cohort (90 pairs, before and after surgery) showed a bias between transit-time ultrasound (1.01 ± 0.47 L/min) and ultrasound dilution technology (1.03 ± 0.51 L/min) of -0.02 L/min, limits of agreement -0.3 to 0.3 L/min, and percentage error of 31%. In children with no residual shunts, the bias was -0.04 L/min, limits of agreement -0.28 to 0.2 L/min, and percentage error 19%. The pooled co efficient of variation was for the whole cohort 3.5% (transit-time ultrasound) and 6.3% (ultrasound dilution), and in children without shunt, it was 2.9% (transit-time ultrasound) and 4% (ultrasound dilution), respectively. Ultrasound dilution identified the presence of shunts (pulmonary to systemic blood flow ≠ 1) with a sensitivity of 100% and a specificity of 92%. Mean pulmonary to systemic blood flow ratio by transit-time ultrasound was 2.6 ± 1.0 and by ultrasound dilution 2.2 ± 0.7 (not significant).CONCLUSION: The COstatus monitor is a reliable technique to measure cardiac output in children with high sensitivity and specificity for detecting the presence of shunts.

Abstract:STUDY OBJECTIVE: To determine if an initial (before treatment) prehospital end-tidal carbon dioxide (EtCO2) measurement in adult, non-chronic obstructive pulmonary disease (COPD), asthmatic patients predicts patient outcomes.METHODS: This is a retrospective chart review of EtCO2 assessment data in a convenience sample of adult, asthmatic patients transported via advanced life support (ALS) units to a large, urban, academic hospital. Initial EtCO2 measurements were obtained routinely on all respiratory distress patients in the field, and emergency department physicians were unaware of the results. Data were analyzed using descriptive statistics, including percentages, means, and 95% confidence intervals (CI).RESULTS: We reviewed data for prehospital initial EtCO2 measurements on 299 unique asthma patients (repeat visits by same patient were not included). Mean (SD) age was 43.1 years (12.5) and 142 (47.5%) were male. The mean EtCO2 measurement was 38.8 mmHg (SD ± 9.5; CI: 37.7-39.9; range: 14-82). Examination of initial EtCO2 measurements by deciles revealed that extreme values, in the lowest (14-28 mmHg) and highest (50-82 mmHg) deciles, experienced more markers of poor outcome than less extreme measurements. Patients were thus dichotomized by extreme (n = 59) or nonextreme (n = 240) EtCO2 measurements. More extreme patients were ultimately intubated (30.5 vs. 5.8%; p < 0.001; positive predictive value (ppv) = 30.5% ), and/or admitted to the intensive care unit (ICU) (28.8 vs. 6.7%; p <0.001; ppv = 28.8%), and/or died (5.1 vs. 0%; p = 0.007 [Fisher's exact test]; ppv = 5.1%), than nonextreme patients, respectively.CONCLUSION: Extreme (both low and high) prehospital initial EtCO2 measurements may be associated with markers of poor patient outcomes. Future work will prospectively determine whether the addition of this information improves early recognition of severe asthma episodes beyond clinical assessment.

Abstract:Objectives: For patients with obstructive sleep apnea hypopnea syndrome (OSAHS), chronic inflammation and hemodynamic oscillations caused by respiratory events contribute to cardiovascular disease (CVD). In this study, a physiological marker named oscillatory coupling factor (OCF) exacted from cardiac output (CO) was introduced. This study aimed to evaluate the clinical value of OCF and tentatively explore its predictive value of cardiovascular prognosis in OSAHS patients.Methods: An impedance cardiogram (ICG) was used to continuously obtain the participants' CO with simultaneous polysomnography. Participants were divided into three groups: an OSAHS-CVD-group (n = 19); an OSAHS + CVD- group (n = 34); and an OSAHS + CVD + group (n = 36). The OCF was exacted from the CO by using empirical mode decompensation-based detrended fluctuation analysis (EMD-DFA).Results: The OCF values were: OSAHS + CVD + group [1.20 (0.98-1.78)] > OSAHS + CVD-group [1.14 (1.02-1.94)] > OSAHS-CVD-group [0.95 (0.56-1.16)], (p = 0.001). A Spearman test showed that OCF was positively correlated with age, apnea/hypopnea index (AHI), microarousal index (MAI), oxygen desaturation index (ODI), and negatively correlated with the lowest SpO(2). Ten participants were treated by one-night continuous positive airway pressure (CPAP): their AHI decreased from 44.9 (18.0-72.9)/hour to 1.25 (0.07.5)/hour, and their OCF fell from 1.17 (1.10-1.69) to 1.08 (0.96-1.23) (p = 0.038). Seventy-seven participants were effectively followed up. Seven participants developed CVD events or newly diagnosed CVD; their OCFs were distributed on a relatively high level [1.18 (1.01-1.56)].Conclusion: The OSAHS participants had higher OCFs than those without OSAHS, while CVD made the OCFs even higher; CPAP could rectify this change. Oscillatory coupling factor may be a physiological marker of cardiopulmonary coupling and have potential cardiovascular prognostic value for people with OSAHS. (C) 2015 Elsevier B.V.
