Rapid Determination of Pneumococcal Serotypes 6A, 6B, 6C, and 6D by Combination of PCRs to Capsular Gene Loci of Streptococcus Pneumoniae

Abstract:Background: The introduction of protein conjugate pneumococcal vaccine in many countries worldwide has led to capsular replacement in S. pneumoniae causing disease and nasopharyngeal carriage among children. Serotypes, such as serotypes 6C and 6D, were newly described in 2007 and 2009 respectively. Active surveillance on pneumococcal disease and carriage requires prompt detection of changes in serotype distribution and identification of new serotypes. We describe the use of multiple PCRs based on the capsular loci to detect serotypes of 6A, B, C and D from pneumococci belonging to serogroup 6. Methods: Serotypes of S. pneumoniae were identified using multiple multiplex PCRs for determination of 40 serotypes according to the CDC protocol. This protocol only typed serogroup 6 strains into 6C and non-6C serotypes. Strains belonging to serogroup 6 were further typed into 6A, 6B, 6C and 6D serotypes based on a combination of three PCRs, using specific primers as described [1,2]. Primers that differentiate serotypes 6A and 6B were based on single-nucleotide polymorphism in wciP of the capsular gene loci between 6A and 6B (nucleotide 584a [195N] in 6B and 584g [195S] in 6A) that encodes rhamnosyl transferase. An additional homologous recombination of wciNb resulting in the substitution of wciN of 6A (wciN6A) is present in serotype 6C whilst if wciNb was introduced into the serotype 6B operon it will be designated 6D [2]. Results: 242 isolates of S. pneumoniae belonging to serogroup 6 in a nasopharyngeal carriage study were further typed in 6A, B, C, and D serotypes using the above PCRs and comprised 58, 114, 60, and 6 isolates respectively. All strains, except four isolates (1.7%), were untypable and were discrepant to the results as used by the CDC PCR methods to determine 6C and non-6C serotypes. Conclusion: The specific PCRs enable prompt identification of serotypes 6A, B, C, and D, and allow rapid detection of new serotypes belonging to serogroup 6 in pneumococcal surveillance studies.
