Changes of Thyroid Hormone Receptor Activity in Hypertensive Atrial Fibrillation

Abstract:Objective: To investigate the changes of thyroid hormone receptor (TR) activity in hypertensive atrial fibrillation (AF). Methods: Using a case–control study design, 103 patients with hypertensive AF (paroxysmal AF: 48, persistent AF: 55) were included into the study groups. A total of 50 age-matched patients with hypertension, but without AF were selected as the control group. Collect demographic data and laboratory parameters, and thyroid hormone receptor activity of peripheral blood lymphocyte and nucleus were measured by radioligand method. Results: The maximum binding capacity (MBC) was markedly decreased in the hypertensive AF group as compared with the hypertensive non-AF group, and it was significantly lower in persistent AF than paroxysmal AF (36.10±12.40 vs 65.22±30.90, P<0.001). The dissociation constant (Kd) of TR of peripheral blood lymphocyte was also lower in the hypertensive AF group as compared with the hypertensive non-AF group, and it was significantly lower in persistent AF than paroxysmal AF (0.77±0.43 vs 1.02±0.41, P<0.001). MBC and Kd of nucleus also had the same phenomenon. The thyroid hormone receptor activity of peripheral blood lymphocyte was in negative correlation with left atrial diameter (LAD), and after adjustment of some clinical parameters, the partial coefficient was −0.448 for MBC, −0.296 for Kd, respectively (P<0.01). MBC and Kd of nucleus were also in negative correlation with LAD. Conclusions: In hypertensive patients, decreased MBC of TR was associated with increased incidence of AF, and increased affinity of TR was associated with increased incidence of AF. MBC and Kd of TR were in negative correlation with LAD.
