Abstract:PURPOSE:Complement depletion commonly occurred in septic patients, but this problem was often underestimated during the treatment process. This study was designed to determine the association between complement depletion and T-cell immunosuppression. METHODS:From November 2011 to March 2012, patients with severe abdominal sepsis were prospectively enrolled in a single center. The baseline levels of complement C3 were used to stratify subjects into 2 groups. Plasma levels of complement components, percentage of regulatory T cells (Tregs), and T-cell immunity indexes were monitored at times after admission. The relationship between C3 depletion and T-cell response was investigated, with clinical outcomes explored meanwhile. RESULTS:A total of 60 patients aged 43.9 ± 11.3 years were included within the period. C3 depletion, occurring in 65% of enrolled subjects, was strongly correlated with Treg expansion (P = .001) and decreased CD4(+)/CD8(+) ratio (P = .008). This depletion was also related to prolonged hospital stay (P = .001), delayed time to operation (P < .001), increased postoperative complications (P = .036), and hospital expenditure (P < .001). CONCLUSIONS:Complement C3 depletion was found to be linked to the expansion of Tregs during abdominal sepsis. Such depletion and associated immunosuppression should be paid close attention in the critical care.
A Metabolite from Commensal Candida Albicans Enhances the Bactericidal Activity of Macrophages and Protects Against Sepsis

Abstract:The gut microbiome is recognized as a key modulator of sepsis development. However, the contribution of the gut mycobiome to sepsis development is still not fully understood. Here, we demonstrated that the level of Candida albicans was markedly decreased in patients with bacterial sepsis, and the supernatant of Candida albicans culture significantly decreased the bacterial load and improved sepsis symptoms in both cecum ligation and puncture (CLP)-challenged mice and Escherichia coli-challenged pigs. Integrative metabolomics and the genetic engineering of fungi revealed that Candida albicans-derived phenylpyruvate (PPA) enhanced the bactericidal activity of macrophages and reduced organ damage during sepsis. Mechanistically, PPA directly binds to sirtuin 2 (SIRT2) and increases reactive oxygen species (ROS) production for eventual bacterial clearance. Importantly, PPA enhanced the bacterial clearance capacity of macrophages in sepsis patients and was inversely correlated with the severity of sepsis in patients. Our findings highlight the crucial contribution of commensal fungi to bacterial disease modulation and expand our understanding of the host-mycobiome interaction during sepsis development.
Effect of Interleukin-36β on Activating Autophagy of CD4+CD25+ Regulatory T Cells and Its Immune Regulation in Sepsis.

Abstract:Background. CD4(+)CD25(+) regulatory T cells (Tregs) play an essential role in sepsis-induced immunosuppression. How, the effects of interleukin 36 (IL-36) cytokines on CD4(+)CD25(+) Tregs and their underlying mechanism(s) in sepsis remain unknown. Methods. Our study was designed to investigate the impacts of IL-36 cytokines on murine CD4(+)CD25(+) Tregs in presence of lipopolysaccharide (LPS) and in a mouse model of sepsis induced by cecal ligation and puncture (CLP). IL-36-activated autophagy was evaluated by autophagy markers, autophagosome formation, and autophagic flux. Results. IL-36 alpha, IL-36 beta, and IL-36 gamma were expressed in murine CD4(+)CD25(+) Tregs. Stimulation of CD4(+)CD25(+) Tregs with LPS markedly up-regulated the expression of these cytokines, particularly IL-36 beta. IL-36 beta strongly suppressed CD4(+)CD25(+) Tregs under LPS stimulation and in septic mice challenged with CLP, resulting in the amplification of T-helper 1 response and the proliferation of effector T cells. Mechanistic studies revealed that IL-36 beta triggered autophagy of CD4(+)CD25(+) Tregs. These effects were significantly attenuated in the presence of the autophagy inhibitor 3-methyladenine or Beclinl knockdown. In addition, early IL-36 beta administration reduced the mortality rate in mice subjected to CLP. Depletion of CD4(+)CD25(+) Tregs before the onset of sepsis obviously abrogated IL-36 beta-mediated protection against sepsis. Conclusions. These findings suggest that IL-36 beta diminishes the immunosuppressive activity of CD4(+)CD25(+ )Tregs by activating the autophagic process, thereby contributing to improvement of the host immune response and prognosis in sepsis.
Complement Depletion Deteriorates Clinical Outcomes of Severe Abdominal Sepsis: A Conspirator of Infection and Coagulopathy in Crime?

Abstract:Background: The complement depletion commonly occurred during sepsis, but it was often underestimated compared with severe infection or coagulation dysfunction.Objective: This study was designed to investigate the alteration of complement system in patients with severe abdominal sepsis and evaluate the role of complement depletion in prognosis of such patients. The relationship between complement depletion and infection or coagulopathy was also explored.Methods: Forty-five patients with severe abdominal sepsis were prospectively conducted among individuals referral to SICU. Currently recommended treatments, such as early goal-directed resuscitation, source control and antibiotics therapy, were performed. Acute physiology and chronic health evaluation II (APACHE II) and sepsis related organ failure assessment (SOFA) scores were employed to evaluate severity. Plasma levels of C3, C4, CRP, PCT, D-dimer and other parameters were detected within eight times of observation. The 28-day mortality, length of stay, and postoperative complications were compared between complement depletion and non-complement depletion groups.Results: Within the study period, eight (17.8%) patients died, five of them suffering from complement depletion. The overall incidence of complement depletion was 64.4%. At admission, mean complement C3 and C4 levels were 0.70 and 0.13 mg/mL, respectively. Using ROC analysis for mortality prediction, the area under the curve of C3 was 0.926 (95% CI, 0.845-0.998, P<0.001), with optimal cutpoint value of 0.578 mg/mL. Complement C3 depletion was shown to be no correlation to severity scores, however, strongly correlated with elevated D-dimer, PCT concentrations and increased postoperative complications.Conclusions: Complement C3 depletion was found to be connected to poor prognosis in severe abdominal sepsis. This depletion seems to be associated with coagulopathy and aggravated infection during sepsis, which should be paid close attention in critical care.
Exogenous C3 Postpones Complement Exhaustion and Confers Organ Protection in Murine Sepsis

Abstract:Background. Sepsis in human being is a challenging and life-threatening problem. Complement activation is an essential event in sepsis. The present study observed the dynamic levels of complement components in sepsis and evaluated the role of exogenous complement protein in outcomes. The relationship between complement and inflammatory cytokines was also investigated.Materials and Methods. Colon ascendens stent peritonitis (CASP) surgery was performed in wild-type C57BL/6 mice to induce sepsis. After 6 h of CASP, a single intraperitoneal injection of human purified C3 (HuC3, 1 mg) was carried out, with 200 uL phosphate-buffered saline injection for control purpose. Plasma levels of C3, complement factor H (CFH), and inflammatory cytokines at different time points were detected. Bacterial burden and organ damage were evaluated after 24 h of surgical procedure.Results. The plasma C3 levels began to fall at 6 h post CASP, followed by an irreversible process of consumption. A single injection of HuC3 stabilized C3 levels for about 6 h, decreasing the 24 h mortality from 60% to 20%. Administration of exogenous C3 reduced bacterial burden and attenuated organ injury in sepsis. Plasma levels of CFH and TNF-alpha were correlated with the depletion of C3.Conclusion. We demonstrated a consumptive depletion of complement components toward septic peritonitis. Exogenous C3 supplementation in early stage of sepsis is helpful to sustain C3 levels, with enhanced bacterial clearance and improved outcomes. (C) 2011 Elsevier Inc. All rights reserved.
Exogenous C3 Protein Enhances the Adaptive Immune Response to Polymicrobial Sepsis Through Down-Regulation of Regulatory T Cells.

Abstract:Background: The role of complement system in bridging innate and adaptive immunity has been confirmed in various invasive pathogens. It is still obscure how complement proteins promote T cell-mediated immune response during sepsis. The aim of this study is to investigate the role of exogenous C3 protein in the T-cell responses to sepsis.Methods: Sepsis was induced by colon ascendens stent peritonitis (CASP) in wild-type C57BL/6 mice, shamoperated mice for control. Human purified C3 protein (HuC3, 1 mg) was intraperitoneally injected at 6 h post-surgery, with 200 mu l phosphate-buffered saline as control. The levels of C3 and cytokines, the expression of FOXP3 and NF-kappa B, and the percentages of CD4(+) T-cell subsets were compared among the groups at given time points.Results: The polymicrobial sepsis produced considerable release of TNF-alpha and IL-10, and caused complement C3 exhaustion. Exogenous C3 administration markedly improved the 48 h survival rate, as compared with nontreatment (40% vs. 5%, P<0.01). The expression of FOXP3 protein was increased during sepsis, but can be suppressed by HuC3 administration. A single injection of HuC3 postponed the decline of differentiated ml cells, and depressed the activation of Th2/Th17 cells. Besides, the Th1-Th2 shift in late stage of sepsis can be controlled under C3 supplementation. The suppression of NF-kappa B pathway might be related to the appearance of immunocompromise.Conclusions: The study confirmed the important role of exogenous C3 in up-regulation of adaptive immune response to sepsis. Macrophage S1PR2 Drives Sepsis-induced Immunosuppression by Exacerbating Mitochondrial Fragmentation

Abstract:Macrophage mitochondrial dysfunction is associated with immunosuppression and poor prognosis of septic patients. Mitochondrial fragmentation drives mitochondrial dysfunction. Our previous study has found that S1PR2 regulates macrophage phagocytosis during sepsis, while the role of S1PR2 in immunosuppression and the mechanisms remain further studied. This study aimed to unveil the relationship between macrophage mitochondrial fragmentation and sepsis-induced immunosuppression, as well as the S1PR2-related mechanisms thereof. Peripheral blood monocytes were collected from healthy controls (n = 12), nonseptic critical controls (n = 13) and septic patients (n = 19). Peritoneal macrophages were harvested from wildtype and S1pr2-/- mice (MMRRC strain iD, 12830) after cecal ligation and puncture (CLP). Mitochondrial ultrastructure was evaluated using transmission electron microscopy. The impact of mitochondrial ultrastructure alteration on immunosuppression of monocytes-macrophages was evaluated. Compared with nonseptic and healthy controls, peripheral blood monocytes from septic patients exhibited increased S1PR2 expression, mitochondrial fragmentation, and mitochondrial dysfunction. Mitochondrial fragmentation was negatively associated with HLA-DR expression. S1PR2 expression was positively correlated with mitochondrial fragmentation and negatively correlated with HLA-DR expression. In mice subjected to CLP, S1PR2 depletion ameliorated macrophage mitochondrial fragmentation and dysfunction, boosted immunity, and improved survival. Mechanistically, in response to sepsis, S1PR2 activates ROCK I to induce Drp1 phosphorylation, resulting in Drp1-dependent mitochondrial fragmentation of macrophages. Drp1 inhibition by Mdivi-1 mitigated S1PR2-induced macrophage immunosuppression and improved the prognosis of mice following CLP. In conclusion, S1PR2-induced mitochondrial fragmentation is a crucial factor mediating septic immunosuppression, highlighting its potential as a promising therapeutic target in sepsis.
Increased Susceptibility of S1PR3-Deficient Mice to Escherichia Coli Sepsis is Associated with an Impaired Bactericidal Activity in Macrophage

Abstract:Abstract Background Sepsis, a leading cause of death in critically ill patients worldwide, is characterized with imbalanced proinflammatory responses and compromised bacterial clearance in host immunity. Monocytes/macrophages play a crucial role in elimination of bacteria through innate immunity receptors. Methods We explored the effect of Sphingosine-1-phosphate receptor 3 (S1PR3) gene deficiency on mortality, organ injury, and bacterial clearance in cecal ligation and puncture (CLP) or Escherichia coli (E. coli)-induced sepsis. Signaling molecules responsible for S1PR3-mediated bactericidal function were dissected in peritoneal macrophages. Furthermore, S1PR3 specific agonist (GPS-725.017) was evaluated for treatment of sepsis. Results After sepsis onset, S1pr3−/− mice showed increased mortality and organ injury, which attributed to markedly decreased E. coli killing. Mechanically, S1PR3 regulated activity of the NADPH oxidase NOX2 complex and phagolysosomal maturation through recruiting the intracellular class III phosphatidylinositol kinase Vps34 to the phagosome. S1PR3 specific agonist (GPS-725.017) enhanced NOX2 activity in E. coli-containing phagosomes and bactericidal function in wild-type macrophages. Additionally, the survival of mice treated with GPS-725.017 was improved when compared with those treated with vehicle. Conclusion S1PR3 is an important receptor to connect bacterial phagosome to ubiquitous cellular machinery responsible for control of bacterial killing in macrophage. Interventions targeting S1PR3 signaling may serve as promising therapeutic approaches for sepsis.
Changes in Early T-Cell Subsets and Their Impact on Prognosis in Patients with Sepsis: A Single-Center Retrospective Study

Abstract:Objective: To analyze the early changes in CD3+, CD4+, and CD8+T-cell subset counts in patients with sepsis and their correlation with prognosis to provide a feasible basis for clinical immunomodulation in sepsis. Methods: This is a single-center retrospective study. The study enrolled sepsis patients (meeting SEPSIS 3.0 definition) who were admitted to the Department of Intensive Care Unit at the First Hospital of Jilin University from July 5th, 2018 to December 5th, 2019 and were aged 18 years or above. In addition, these patients underwent cellular immune testing (CD3+, CD4+, CD8+ T lymphocyte counts, and CD4+/CD8+ ratio) within 24 hours of ICU admission. Patient's clinical data including age, gender, infection site, APACHE II score, SOFA score, length of ICU stay, mechanical ventilation time, ICU mortality, 28-day mortality, and 3-year survival status were collected. The prognostic indicators and survival of the decreased and nondecreased groups of different subsets of T lymphocyte counts and CD4+/CD8+ ratio were compared. Results: A total of 206 patients were enrolled, with 76.7% having a decrease in CD3+ T lymphocyte count, 76.7% having a decrease in CD4+ T lymphocyte count, and 63.6% having a decrease in CD8+ T lymphocyte count. Furthermore, 21.8% had a lower CD4+/CD8+ ratio. Analysis showed that the CD3+ T lymphocyte count decreased group had a longer length of ICU stay [11 d (4, 21) vs. 7 d (4, 17), P=0.03], increased percentage of mechanical ventilation (67.5% vs. 51.0%, P=0.04), and extended mechanical ventilation time [144 h (48, 360) vs. 96 h (48, 144), P=0.04] compared to the nondecreased group. The 28-day mortality was higher in the decreased group of CD4+/CD8+ ratio compared to the nondecreased group (33.3% vs. 25.5%, P=0.29); however, the difference did not reach statistical significance. Logistic regression analysis revealed no significant correlation between the decrease in CD4+/CD8+ ratio and 28-day mortality (P=0.11). The 3-year follow-up revealed that the CD4+/CD8+ decreased group had a lower survival rate than the nondecreased group (33.3% vs. 53.4%, P=0.01). Conclusions: In the early stage of sepsis, most patients showed a decrease in CD3+, CD4+, and CD8+T-cell subsets, as well as in the CD4+/CD8+ ratio. The decrease in CD3+ and CD4+/CD8+ was related to some poor prognosis.
The effect of human complement C3 protein applied at different times in treatment of polymicrobial sepsis

Abstract:Objective This study was designed to investigate the effect of exogenous complement C3 administration on outcomes of sepsis and identify an optimal time for this therapy. Materials and methods Colon ascendens stent peritonitis (CASP) was performed to induce sepsis in C57BL/6 mice, with sham-operated mice as control. Human purified C3 (HuC3, 1 mg) was administered via intraperitoneal injection, with 200 µl phosphate-buffered saline as control. Mice were categorized by the initiation time of HuC3 treatment. Survival, bacterial burden, vital organ damages, histology changes, and expression of C3 were compared between the groups. Results CASP-induced sepsis caused rapid complement C3 depletion and severe organ damage. Vital organs suffered from substantial bacterial loads. Exogenous C3 applied in the early stage of sepsis was associated with attenuated organ injuries, enhanced bacterial clearance, and improved survival. Exogenous C3 application promoted the synthesis of C3 in the early stage of sepsis. It appears that 6 h post-CASP surgery is the optimal time for HuC3 therapy. Conclusion The study confirmed the positive effect of exogenous C3 on treatment of polymicrobial sepsis. C3 supplementation prior to the appearance of complement depletion could protect vital organs and its administration in the early stage of sepsis should be encouraged .

Abstract:The development of sepsis invovles the dysfunction of immunity due to an imbalance between the hyperimmune response and the immunoparalysis. Immune cells in both the innate and acquired immune system, including neutrophils, macrophages, dendritic cells, T cells and NK cells, are actively involved in the process. The interaction between immune cells, proinflammatory and anti-inflammatory cytokines contribute to the immunoparalysis in sepsis. Abnormal CD4(+) and CD8(+) T cell responses are major components of the deregulated acquired immune response in sepsis. Immune dysfunction of regulatory T cells (Tregs) contributes to the pathogensis of sepsis. Furthermore, IL-7 is essential for the replenishment and survival of T cells, which represents a promising target for immunotherapy of sepsis. In this review, we discusse the the immunoparalysis in the sepsis, with a focus on the deregulation of T cell response.
Septic Macrophages Induce T Cells Immunosuppression in a Cell-Cell Contact Manner with the Involvement of CR3

Abstract:Background: In addition to excessive inflammation, immunosuppression has been recognized as a contributing factor to poor prognosis of sepsis. Although it has been reported that T cells can become functionally impaired during sepsis, the underlying mechanisms responsible for this phenomenon remain unclear. This study aims to elucidate the mechanisms by which macrophages induce immunosuppression in T cells. Methods: In an in vivo setting, C57BL-6J mice were subjected to cecal ligation and puncture (CLP) with or without depletion of macrophages, and the functions of T cells were assessed. In vitro experiments involved direct co-culture or separate culture of T cells and septic macrophages using a transwell system, followed by analysis of T cell immunity. Additionally, a siRNA targeting CD18 on macrophages was utilized to investigate the role of complement receptor 3 (CR3). Results: Both macrophages and T cells exhibited immunosuppression during sepsis. In the in vivo experiments, the absence of macrophages partially alleviated T cell immunosuppression, as evidenced by restored vitality, increased production of TNF-α and IFN-γ, elevated CD8+ T cell levels, and decreased CD25+ T cell levels. In the in vitro experiments, direct co-culture of T cells with septic macrophages resulted in diminished T cell immunity, which was improved when T cells and macrophages were separated by a chamber wall. The expression of CR3 (CD11b/CD18) was upregulated on septic macrophages, and silencing of CD18 led to decreased TNF-α production by T cells, reduced CD4+ T cell numbers, and increased CD25+ T cell numbers. Conclusion: In sepsis, macrophages induce immunosuppression in T cells through direct cell-cell contact, with the involvement of CR3.
Complement C5a Regulates IL‐17 by Affecting the Crosstalk Between DC and Γδ T Cells in CLP‐induced Sepsis

Abstract:Complement 5a (C5a) and Interleukin-17 (IL-17) are two important inflammatory mediators in sepsis. Here we studied the mechanisms underlying regulation of IL-17 by anaphylatoxin C5a. We found that C5a blockade increased the survival rate of mice following cecal ligation and puncture (CLP)-induced sepsis and decreased IL-17 expression in vivo. IL-17 was secreted mainly by gamma delta T cells in this model. Importantly, our data suggest that C5a participates in the regulation of IL-17 secretion by gamma delta T cells. Dendritic cells (DC) were found to act as a "bridge" between C5a and gamma delta T cells in a mechanism involving IL-6 and transforming growth factor beta (TGF-beta). These results imply that C5a affects the cross-talk between DC and gamma delta T cells during sepsis development, and this may result in a large production of inflammatory mediators such as IL-17.

Abstract:Background: Human neutrophil peptides 1-3 are endogenous cationic antimicrobial peptides implicated in host defense against microbes. The genes encoding human neutrophil peptides 1-3 (DEFA1/DEFA3) exhibit copy number variations. This study was designed to determine whether DEFA1/DEFA3 copy number variations conferred susceptibility to infection-induced complications such as severe sepsis. Methods: This case-control study was performed in 179 patients with severe sepsis and 233 healthy blood donors and was replicated in an independent cohort of 112 cases and 118 controls. Plasma levels of human neutrophil peptides 1-3, tumor necrosis factor-alpha, interleukin-6, and interleukin-10 were detected. Results: The genotype of DEFA1/DEFA3 with more than eight copies was more frequent in patients with severe sepsis than in controls (55.9% vs. 31.3%; P = 1.13 x 10(-6), odds ratio 2.77, 95% confidence interval 1.85-4.16). After adjustment for age and gender, logistic regression analysis confirmed the association of the genotype of more than eight copies with an increased risk of severe sepsis (P = 2.25 x 10(-5), odds ratio 2.66, 95% confidence interval 1.69-4.19). This established association was replicated in a second age- and gender-matched case-control cohort (P = 0.02, odds ratio 1.90, 95% confidence interval 1.11-3.27). Furthermore, compared with those with fewer copies, the patients carrying more than eight copies of DEFA1/DEFA3 presented significantly lower plasma levels of human neutrophil peptides 1-3, tumor necrosis factor-alpha, interleukin-6, and interleukin-10 (P = 0.039, 0.017, 0.030, and 0.029, respectively). Conclusions: DEFA1/DEFA3 is an important genetic component participating in host immune response to severe sepsis. A higher copy number of DEFA1/DEFA3 (>8 copies) is significantly associated with the risk of severe sepsis.

Abstract:Sepsis engenders distinct host immunologic changes that include the expansion of myeloid-derived suppressor cells (MDSCs). These cells play a physiologic role in tempering acute inflammatory responses but can persist in patients who develop chronic critical illness. The origins and lineage of these MDSC subpopulations were previously assumed to be discrete and unidirectional; however, these cells exhibit a dynamic phenotype with considerable plasticity. Using Cellular Indexing of Transcriptomes and Epitopes by Sequencing followed by transcriptomic analysis, we identify a unique lineage and differentiation pathway for MDSCs after sepsis and describe a novel MDSC subpopulation. Additionally, we report that the heterogeneous response of the myeloid compartment of blood to sepsis is dependent on clinical outcome.

Abstract:Background Immune suppression contributes to nosocomial infections (NIs) and poor prognosis in sepsis. Recent studies revealed that CD71 + erythroid cells had unappreciated immunosuppressive functions. This study aimed to investigate the values of CD71 + erythroid cells (CECs) in predicting NIs and prognosis among adult septic patients. The potential factors associated with the expansion of CECs were also explored. Methods In total, 112 septic patients and 32 critically ill controls were enrolled. The frequencies of CD71 + cells, CD71 + CD235a + cells, and CD45 + CECs were measured by flow cytometry. The associations between CECs and NIs and 30-day mortality were assessed by ROC curve analysis and Cox and competing-risk regression models. Factors associated with the frequency of CECs were identified by linear regression analysis. Results The percentage of CD71 + cells, CECs, and CD45 + CECs were higher in septic patients than critically ill controls. In septic patients, the percentages of CD71 + cells, CECs, and CD45 + CECs were associated with NI development, while CD71 + cells and CECs were independently associated with 30-day mortality. Linear regression analysis showed that the levels of interleukin (IL)-6 and interferon (IFN)-γ were positively associated with the frequencies of CD71 + cells, CECs, and CD45 + CECs, while IL-10 was negatively associated with them. Additionally, the levels of red blood cells (RBCs) were negatively associated with the percentage of CD45 + CECs. Conclusions CECs were expanded in sepsis and can serve as independent predictors of the development of NI and 30-day mortality. Low levels of RBCs and high levels of IL-6 and IFN-γ may contribute to the expansion of CECs in sepsis. Trial Registration ChiCTR, ChiCTR1900024887. Abstract:Background: Antigen presentation on monocyte surface to T-cells by Major Histocompatibility Complex, Class II (MHC-II) molecules is fundamental for pathogen recognition and efficient host response. Accordingly, loss of Major Histocompatibility Complex, Class II, DR (HLA-DR) surface expression indicates impaired monocyte functionality in patients suffering from sepsis-induced immunosuppression. Besides the impact of Class II Major Histocompatibility Complex Transactivator (CIITA) on MHC-II gene expression, X box-like (XL) sequences have been proposed as further regulatory elements. These elements are bound by the DNA-binding protein CCCTC-Binding Factor (CTCF), a superordinate modulator of gene transcription. Here, we hypothesized a differential interaction of CTCF with the MHC-II locus contributing to an altered monocyte response in immunocompromised septic patients. Methods: We collected blood from six patients diagnosed with sepsis and six healthy controls. Flow cytometric analysis was used to identify sepsis-induced immune suppression, while inflammatory cytokine levels in blood were determined via ELISA. Isolation of CD14++ CD16-monocytes was followed by (i) RNA extraction for gene expression analysis and (ii) chromatin immunoprecipitation to assess the distribution of CTCF and chromatin modifications in selected MHC-II regions. Results: Compared to healthy controls, CD14++ CD16-monocytes from septic patients with immune suppression displayed an increased binding of CTCF within the MHC-II locus combined with decreased transcription of CIITA gene. In detail, enhanced CTCF enrichment was detected on the intergenic sequence XL9 separating two subregions coding for MHC-II genes. Depending on the relative localisation to XL9, gene expression of both regions was differentially affected in patients with sepsis. Conclusion: Our experiments demonstrate for the first time that differential CTCF binding at XL9 is accompanied by uncoupled MHC-II expression as well as transcriptional and epigenetic alterations of the MHC-II regulator CIITA in septic patients. Overall, our findings indicate a sepsis-induced enhancer blockade mediated by variation of CTCF at the intergenic sequence XL9 in altered monocytes during immunosuppression.

Abstract:<b>Rationale:</b> Evident immunosuppression has been commonly seen among septic patients, and it is demonstrated to be a major driver of morbidity. Nevertheless, a comprehensive view of the host immune response to sepsis is lacking as the majority of studies on immunosuppression have focused on a specific type of immune cells. <b>Methods:</b> We applied multi-compartment, single-cell RNA sequencing (scRNA-seq) to dissect heterogeneity within immune cell subsets during sepsis progression on cecal ligation and puncture (CLP) mouse model. Flow cytometry and multiplex immunofluorescence tissue staining were adopted to identify the presence of 'mature DCs enriched in immunoregulatory molecules' (mregDC) upon septic challenge. To explore the function of mregDC, sorted mregDC were co-cultured with naïve CD4<sup>+</sup> T cells. Intracellular signaling pathways that drove mregDC program were determined by integrating scRNA-seq and bulk-seq data, combined with inhibitory experiments. <b>Results:</b> ScRNA-seq analysis revealed that sepsis induction was associated with substantial alterations and heterogeneity of canonical immune cell types, including T, B, natural killer (NK), and myeloid cells, across three immune-relevant tissue sites. We found a unique subcluster of conventional dendritic cells (cDCs) that was characterized by specific expression of maturation- and migration-related genes, along with upregulation of immunoregulatory molecules, corresponding to the previously described 'mregDCs' in cancer. Flow cytometry and <i>in stiu</i> immunofluorescence staining confirmed the presence of sepsis-induced mregDC at protein level. Functional experiments showed that sepsis-induced mregDCs potently activated naive CD4<sup>+</sup> T cells, while promoted CD4<sup>+</sup> T cell conversion to regulatory T cells. Further observations indicated that the mregDC program was initiated via TNFRSF-NF-κB- and IFNGR2-JAK-STAT3-dependent pathways within 24 h of septic challenge. Additionally, we confirmed the detection of mregDC in human sepsis using publicly available data from a recently published single-cell study of COVID-19 patients. <b>Conclusions:</b> Our study generates a comprehensive single-cell immune landscape for polymicrobial sepsis, in which we identify the significant alterations and heterogeneity in immune cell subsets that take place during sepsis. Moreover, we find a conserved and potentially targetable immunoregulatory program within DCs that associates with hyperinflammation and organ dysfunction early following sepsis induction.

Abstract:Purpose: The pathogenesis of T cell subsets in sepsis during the body's resistance to infection is currently unknown. We aimed to investigate the dynamics and molecular mechanisms of T cells during the development of sepsis. Patients and methods: Perform single-cell data analysis on peripheral blood mononuclear cells (PBMCs) specimen samples from seven healthy controls, five early-stage sepsis patients, and four late sepsis patients, and the atlas were mapped and analyzed using reference mapping to identify the T cell subpopulations specific to early sepsis. Expression network upstream to investigate the changes of regulatory transcription factors and pathways by pySCENIC. Results: Twenty-two CD4+ T-cell subpopulations and 10 CD8+ T-cell subpopulations were identified by mapping analysis. At the early stage of sepsis, we observed altered ratios of multiple immune cells in PBMCs. Three cell types CD4 Tn cells, CD8 (GZMK+ early Tem), and CD8 (ZNF683+CXCR6- Tm) showed an upward trend (p < 0.05) in the early stages of sepsis compared to normal and returned to normal levels after two weeks. In addition, we found the presence of four sepsis-specific transcription factors (MXI1, CHD1, ARID5A, KLF9) in these three types of cells, which were validated in two external datasets. The differentially expressed genes in CD4 Tn cells, CD8 (GZMK+ early Tem), and CD8 (ZNF683+CXCR6- Tm) cells between the healthy group and the early-stage sepsis group are commonly enriched in the allograft rejection pathway. In addition, it was found that CD8 cells exhibit a trend towards differentiation into CD8 Temra cells in sepsis. Conclusion: We successfully depicted the dynamic changes of T cell subsets during sepsis onset and progression, which provides important clues for an in-depth understanding of T cells' function and regulatory mechanisms during sepsis pathogenesis.
Decreased complement C3 levels are associated with poor prognosis in patients with COVID-19: A retrospective cohort study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>To describe the humoral immune feature of patients with coronavirus disease 2019 (COVID-19).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The levels of total immunoglobulins (IgG, IgM, IgA, and IgE), complement (C3, C4) results were retrospectively analyzed in COVID-19 patients. Univariable and multivariable logistic regression were performed to explore the risk factors associated with the in-hospital death.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Result</h3><p>A total of 236 patients were enrolled in this study, of which 169 were transferred to another institution or discharged (survival group) and 67 died in hospital (non-survival group). Compared with survivors, the levels of IgA and IgE in non-survivors increased significantly, and level of complement C3 decreased. Non-survivors also showed higher incidence of chest tightness, breath shortness and dyspnoea; higher levels of inflammatory indicators, leukocytes and neutrophils; and low levels of lymphocyte subsets. Multivariable regression showed increasing odds of in-hospital death associated with older age (HR: 1.099; 95%CI: 1.057–1.143; <em>p</em> < 0.0001), d-dimer greater (HR: 1.294; 95%CI: 1.138–1.473; <em>p</em> < 0.0001) and decreased complement C3 level (HR: 0.073; 95%CI: 0.007–0.722; <em>p</em> = 0.025) on admission. Finally, in survival COVID-19 patients whose humoral immunity was re-examined, C3 levels tended to increase, while in non-survivors it decreased.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Low level of complement C3 may be an alert to the admitted COVID-19 patients with additional management. Inhibition of the complement pathway might be an effective therapeutic to COVID-19 patients.</p>
