Abstract:Pancreatic pseudocyst is the most common pancreatic cystic disease in clinical practice.In the surgical treatment of pancreatic pseudocysts,most surgeons preferred laparoscopic surgery in recent years.The options and the timing of laparoscopic surgery for pancreatic pseudocysts in different situations are explored in the paper.Pancreatic pseudocysts during the observation period, the imaging examination to detect whether the cyst has disappeared or increased, such as cysts found to be enlarged or still can not dissipate after 6 months,the largest diameter greater than 6 cm,and clinical symptoms,surgical drainage should be considered treatment.Surgery based on the location of the cyst and surgical experience of surgical options.Pancreatic cyst often choose laparoscopic cyst-gastric anastomosis,far from the stomach cyst should choose laparoscopic cyst-jejunal anastomosis.Laparoscopic surgery for the treatment of pancreatic pseudocyst has a unique advantage, short operation time, less bleeding, less trauma, less postoperative complications,rapid recovery,is a safe and effective treatment options.

Abstract:Objective To evaluate retrospectively on the results of endoscopic pancreatic pseudo- cysts drainage and aimed to assess its safety and efficacy.Methods All consecutive patients who underwent endoscopic pancreatic pseudocyst drainage in three hospitals from January 2001 to June 2006 were included in this study.The patients' records were reviewed,and some long-term follow-up data were obtained by tele- phone at the end of the follow-up period in July 2006.Results A total of 24 patients were included(16 men and 8 women;median age 49.3 years).The technical success rate of the drainage procedure was 100% and the mortality rate was 0.Complications occurred in 8 patients(33.3%),3(12.5%)of which were ma- jor:hemorrhage in one case and secondary infection in another both of them need surgical intervention,stent shift in one case,additional endoscope treatment was required.During a median follow-up period of 21.2 months,two cases lost follow up.Among the rest 22 cases,their pseudocysts disappeared in 17 patients;two were found with much smaller persistent cyst without clinical symptoms and no additional treatment was taken in more than two years later.Recurrent cyst was never found.Overall success endoscopic drainage occurred in 19 patients(86.4%)Conclusion Endoscopic ultrasound guided transgastric drainage is an effective and safe treatment for pancreatic pseudocysts with appropriated choice of puncture site,correct installation of drainage and conjugated with antibiotics.Bleeding and secondary infection were the major complications.
Clinical Diagnosis and Surgical Treatment of Pancreatic Pseudocyst (With Clinical Analysis of 88 Cases)

Abstract:Objective:To evaluate the result of diagnosis and surgical treatment of pancreatic pseudocyst.Methods:The clinical data of 88cases diagnosed as the pancreatic pseudocyst and treated by surgical method from January 2002 to Apirl 2006 were retrospectively analyzed.Results:Seventy-four cases having B-US,there were 73 positive diagnosis(98.6%);CT performed in 71 cases there were 70 positive diagnosis(98.6%);MRI performed in 11 cased the diagnosis were all positive.The operative methods performed in 88 cases included Roux-en-Y cystojejunostomy(39 cases),cystogastrostomy(34 cases),pancreatoduodenectomy(2 cases),externa1 drainage(9 cases),excision of body and tail of pancrea+splenectomy(3 cases),cystogastrostomy+Roux-en-Y cystojejunostomy(1 case).The postoperative complications of Roux-en-Y cystojejunostomy and cystogastrostomy were lower than externa1 drainage's(χ2=12.065,P<0.0125;χ2=8.246,P<0.0125),and no significant difference between Roux-en-Y cystojejunostomy's and cystogastrostomy's(χ2=0.347,P>0.0125).The recurrence rates of cystogastrostomy and externa1 drainage were greater than Roux-en-Y cystojejunostomy's(χ2=7.39,P<0.0125;χ2=9.06,P<0.0125),and no significant difference between externa1 drainage and cystogastrostomy(χ2=0.328,P>0.0125).Conclusions:Pancreatic pseudocyst is diagnosed correctly by imaging examinations such as B-US、CT、MRI etc.Pseudocyst's predilection site was situated mainly in pancreatic body and tail.Externa1 drainage was performed exclusively on the patient whose pancreatic pseudocyst was ruptured or infected because of more postoperative coplications and high reccurence rate.Compare Roux-en-Y cystjoejunostomy to cystogastrostomy,the recurrence rate of Roux-en-Y cystojejunostomy was lower than cystogastrostomy's,but no significant difference in the postoperative complications,so Roux-en-Y cystjoejunostomy was the first choice for the treatment of pancreatic pseudocyst.Other operative methods could be developed according to patient and hospital's conditions.
Endoscopic Retrograde Cholangiopancreatography in Diagnosis and Treatment of Pancreatic Pseudocysts Communicating Main Pancreatic Duct

Abstract:Objective:To explore imaging characteristics and the terapeutic effectiveness of endoscopic retrograde cholan-giopancreatography (ERCP) in diagnosing and treating pancreatic pseudocysts communicated with the pancreatic ductal system. Methods;Data of 42 patients diagnosed and treated with ERCP were analyzed retrospectively. The etiologies of the pancreatic pseudocysts included chronic pancreatitis in 38 patients, acute pancreatitis in 3 patients, and trauma in 1 patient. Results:A total of 44 pancreatic pseudocysts were found by ERCP. There were 20 cases in the head of the pancreas, 8 cases in the body and 16 in the tail mean sizes being 4. 1 cm×3. 0 cm,3. 7 cm×2. 7 cm, and 3. 9 cm×2. 4 cm respectively. Of the 13 patients undergoing ERCP pancreatic stent insertion, 9(69. 9%) achieved complete resolution of the symptoms, 2 had incomplete resolution, and 2 had no improvement and were submitted to surgery. Eleven patients received surgical treatment and 18 received internal medical treatment. Conclusion:ERCP management is an effective alternative to conventional surgical treatment of pancreatic pseudocysts communicated with the pancreatic ductal system.

Abstract:Objective To assess the safety and the efficacy of endoscopic ultrasound (EUS)-guided transmural drainage of pancreatic pseudocysts (PPC).Methods A total of 17 patients with PPC who underwent EUS to detect the optimal site and depth of puncture.The needle was punctured into the PPC cavity through endoscopic biopsy hole,cyst fluid was drained with a syringe.The guide wire was inserted along the pinhole under X-ray,and then the needle-knife was sent along the guide wire to cut the gastric wall and pseudocysts wall,followed by balloon dilation.The way of drainage was selected according to the cyst fluid properties.The technical success rate,treatment success rate,complication occurring rate and the skills were evaluated.Results Four patients were with nasalcystic catheter drainage,9 patients with double pigtail stents internal drainage,and 4 patients with nasal-cystic catheter and double pigtail stents combination drainage.The treatment success rates were 3/4,7/9,and 4/4 respectively.Only 1 patient subsequently developed bleeding from puncture site after stent successively placed,and was turned to surgery because of ineffective endoscopic treatment.Infection occurred in 4 patients during drainage,two of those were switched to surgical resection due to poor medical treatment response,and the other 2 were cured with intravenous infusion of antibiotics sensitive to cyst fluid bacteria and metronidazole rinse PPC.The median follow-up duration was 28.5months,and there was none of recurrence.Conclusions EUS-guided transmural drainage of PPC is safe.Stent placement and nasal-cystic catheter play an important role in PPC treatment.

Abstract:Pancreatic duct stone is thought not only to be the cause of abdominal pain but also to be a risk factor for pancreatic cancer. Several treatment options have been implemented in the treatment of pancreatic duct stones. Stone location is a critical factor in selecting treatment. We present the results of 27 endoscopic treatments and 35 surgical treatments performed in three hospitals at a single university between January 2000 and January 2005. The results were compared retrospectively in terms of success rate of stone removal, length of hospital stay, complications, pain relief, and changes of pancreatic functions. In our study, endoscopy resulted in a similar success rate of stone removal and short-term pain relief rate as the surgical approach and with a shorter length of hospital stay. However, the surgical group had a more favorable long-term clinical result, as well as a lower number of hospital readmissions at the 5-year follow-up point. Based on long-term results, surgical treatment is more effective than endoscopy.

Abstract:Advances in radiological techniques have led to an increase in the number of diagnoses of pancreatic pseudocyst, which is the most common pancreatic cyst lesion disease, accounting for two-thirds of all pancreatic cyst lesions. Historically, the management of pancreatic pseudocyst has been achieved through the use of conservative treatments and surgery; however, due to the complications and recurrence rate associated with these techniques, the management of pancreatic pseudocyst is challenging. Surgeons and gastroenterologists have attempted to determine the optimal management technique to treat pancreatic pseudocyst to reduce complications and the recurrence rate. From these investigations, percutaneous catheter, surgical and endoscopic drainage with ultrasonography guidance have become promising management techniques. The present review aimed to summarize the diagnostic and therapeutic methods used for the management of pancreatic pseudocyst and to compare percutaneous catheter, surgical and endoscopic drainage.

Abstract:Pancreatic pseudocysts (PPs) are collections of pancreatic secretions that are lined by fibrous tissues and may contain necrotic debris or blood. The interventions including percutaneous, endoscopic or surgical approaches are based on the size, location, symptoms and complications of a pseudocyst. With the availability of advanced imaging systems and cameras, better hemostatic equipments and excellent laparoscopic techniques, most pseudocysts can be found and managed by laparoscopy. We describe a case of a 30-year-old male patient with a pancreatic pseudocyst amenable to laparoscopic cystogastrostomy. An incision was made through the anterior gastric wall to expose the posterior gastric wall in close contact with the pseudocyst using an ultrasonically activated scalpel. Then, another incision was made for cystogastrostomy to obtain complete and unobstructed drainage. The patient recovered well after operation and was symptom-free during a 6-mo follow-up, suggesting that laparoscopic cystogastrostomy is a safe and effective alternative to open cystogastrostomy for minimally invasive management of PPs.
Treatment of Pancreatic Pseudocyst: a Report 32 Cases

Abstract:[Objective] To analyze the diagnosis and treatment of pancreatic pseudocyst. [Method] This study included 32 pancreatic pseudocyst cases, conservative management, percustaneous tube drainage and operation therapies were choicend according to the time of pseudocyst formation, the thickness of pseudocyst wall and the location of the pseudocyst. Eight patients were cured conservatively; 6 patients underwent perentaneous drainage. External drainage was performed in patients, internal drainage in 9 cases, and cyst excision in 2 cases. [Results] There was no operative mortality. All patients were followed up for 3 months to 6 years. The in cadence of complications in this patients was 16.7%(4/24), with recurrence rate of 6.3%(2/32). [Conclusion] Pancreatic pseudocyst should be managed individually according the course and patients′ clinical condition.
Treating of Pancreatic Pseudocyst Through Endoscopic Transpapillary Cyst Drainage (ETCD)

Abstract:Objective To determine the effectiveness of ETCD ( endoscopic transpapillary cyst drainage) in treatmen! of pancreatic pseudocysts. Methods Twenty-six patients with pancreatic pesudocysts were selected. They are presented some indications for treatment and the pesudocyst were ranged in size from 1.5 to 15 cm. Of 23 cases 38 sessions of ETCD were performed. In two of thirty-eight cases the cyst bulges were invisible in the stomach or duodenum due to its compression. In one case ETCD was employed after percutaneous drainage of the pseudocyst, the other one was treated after combination therapy of plastic stent ing because of jaundice. Results Improvement of abdominal pain, changes in appetite, body weight and fat in stool were recorded in follow-ups of 60 ~ 850 days. There was only one asymptomatic recurrence. One case was complicated with pancreatic abscess. There was no mortality related to the procedure. Surgical intervention became necessary in three cases because of inadequate drainage or complication. Conclusion ETCD treatment of symptomatic pancreatic pseudocysts with ductal communication, the transpapillary pancreatic duct stenting is a safe, effective modality and should be considered as the first line therapy; on the other hand for those pancreatic pseudocyst without communication to the main pancreatic duct it is not effective.

Abstract:Background and aimPancreatic pseudocysts (PPC) are a complication that occurs in acute and chronic pancreatitis. They comprise 75 % of cystic lesions of the pancreas. There are scarce data about surgical versus endoscopic treatment on PPC. The aim of this study was to compare both treatment modalities regarding clinical success, complication rate, recurrence, hospital stay and cost.MethodsRetrospectively, data obtained prospectively from 2000 to 2012 were analyzed. A PPC was defined as a fluid collection in the pancreatic or peripancreatic area that had a well-defined wall and contained no solid debris or recognizable parenchymal necrosis. Clinical success was defined as complete resolution or a decrease in size of the PPC to 2 cm or smaller.ResultsOverall, 64 procedures in 61 patients were included: 21 (33 %) cases were drained endoscopically guided by EUS and 43 (67 %) cases were drained surgically. The clinical success of the endoscopic group was 90.5 versus 90.7 % for the surgical group (P = 0.7), with a complication rate of 23.8 and 25.6 %, respectively (P = 0.8), and a mortality rate of 0 and 2.3 % for each group, respectively (P = 0.4). The hospital stay was lower for the endoscopic group: 0 (0–10) days compared with 7 (2–42) days in the surgical group (P < 0.0001). Likewise, the cost was lower in the endoscopic group (P < 0.001). The recurrence rate was similar in both groups: 9.5 and 4.5 % respectively (P = 0.59). The two recurrences found in the endoscopic group were associated with stent migration, and the recurrence in the surgical group was due to the type of surgery performed (open drainage).ConclusionEndoscopic treatment of PPC offers the same clinical success, recurrence, complication and mortality rate as surgical treatment but with a shorter hospital stay and lower costs.

Abstract:Background: Pancreatic pseudocyst is a complication of acute and chronic pancreatitis. Although surgery considered the gold standard, there is a rapid shift towards endoscopic treatment owing to its therapeutic outcome and minimal invasive involvement. This study aims to present the clinical profile and outcome analysis of the endoscopic drainage of pancreatic pseudocysts in Dhulikhel Hospital. Methods: This is a retrospective review of all patients who were diagnosed with pancreatic pseudocyst between January 2015 and December 2018 in Dhulikhel Hospital. The retrospective data were on patient characteristics, etiology, location of the cyst, other clinical characteristics. Results: The study included 51 patients and the average age of the patients in this study was 39 years and among them 62.7% were female. The mean size of pseudocyst was 7.89 cm, and the average days of hospital stay was 13.64. The most common etiology was idiopathic and more than half of the patient's cyst was in head and/or body, 15 and 36 underwent conservative and therapeutic management respectively. The technical success rate was at 94% and reported increased pancreatic pseudocyst in Dhulikhel Hospital from 2015-18. Conclusions: The study findings highlight the increased trend of pancreatitis pseudocyst as a complication of acute or chronic pancreatitis. Endoscopic drainage of pseudocyst with plastic stent is an established method of managing it. However, pancreatic pseudocyst even larger than 6 cm can undergo spontaneous resolution.Hence, conservative management should be considered first.

Abstract:This article aims to elucidate the classification of and optimal treatment for pancreatic pseudocysts. Various approaches, including endoscopic drainage, percutaneous drainage, and open surgery, have been employed for the management of pancreatic pseudocysts. However, no scientific classification of pancreatic pseudocysts has been devised, which could assist in the selection of optimal therapy. We evaluated the treatment modalities used in 893 patients diagnosed with pancreatic pseudocysts according to the revision of the Atlanta classification in our department between 2001 and 2010. All the pancreatic pseudocysts have course of disease >4 weeks and have mature cysts wall detected by computed tomography or transabdominal ultrasonography. Endoscopic drainage, percutaneous drainage, or open surgery was selected on the basis of the pseudocyst characteristics. Clinical data and patient outcomes were reviewed. Among the 893 patients, 13 (1.5%) had percutaneous drainage. Eighty-three (9%) had type I pancreatic pseudocysts and were treated with observation. Ten patients (1%) had type II pseudocysts and underwent the Whipple procedure or resection of the pancreatic body and tail. Forty-six patients (5.2%) had type III pseudocysts: 44 (4.9%) underwent surgical internal drainage and 2 (0.2%) underwent endoscopic drainage. Five hundred six patients (56.7%) had type IV pseudocysts: 297 (33.3%) underwent surgical internal drainage and 209 (23.4%) underwent endoscopic drainage. Finally, 235 patients (26.3%) had type V pseudocysts: 36 (4%) underwent distal pancreatectomy or splenectomy and 199 (22.3%) underwent endoscopic drainage. A new classification system was devised, based on the size, anatomical location, and clinical manifestations of the pancreatic pseudocyst along with the relationship between the pseudocyst and the pancreatic duct. Different therapeutic strategies could be considered based on this classification. When clinically feasible, endoscopic drainage should be considered the optimal management strategy for pancreatic pseudocysts.
Endoscopic Combined Drainage of One Giant and Multiple Small Pancreatic Pseudocysts: A Case Report

Abstract:A pancreatic pseudocyst (PPC) is a frequent complication of pancreatitis, often stemming from alcohol, gallstones, or hyperlipidemia. Endoscopic treatment of PPC has become the mainstream treatment. A case of one giant and multiple small PPCs was observed, manifesting as repeated abdominal bloating, abdominal pain, nausea, and vomiting after meals. Initial computed tomography scans revealed the presence of multiple PPCs. Despite ineffective medical treatment, the pseudocysts progressively increased. In response, we conducted a combined endoscopic intervention, involving Hot AXIOS (Boston Scientific, Marlborough, MA) stenting through endoscopic ultrasound-guided transmural drainage (EUS-TMD) and the placement of the endoscopic nasopancreatic drainage (ENPD) mimic stent through endoscopic retrograde pancreatography (ERP). Remarkably, after nine months of postoperative follow-up, the patient had no discomfort symptoms and the cyst disappeared. We conducted a literature review on endoscopic combined drainage for PPCs, which is still controversial. Our presented case serves as a demonstration that endoscopic combined drainage can effectively and successfully manage giant and multiple PPCs.

Abstract:Pancreatic pseudocyst (PPC) and walled-off necrosis (WON) develop as late complications of acute pancreatitis that have been historically managed surgically. With the advancement in endoscopic equipment and the evolution of endoscopic surgery, the management of PPC has evolved considerably in recent years from surgical drainage to transmural endoscopic drainage. Till the end of the 20th century, a limited number of surgeons performed laparoscopic drainage of PPCs. Due to the steep learning curve needed for performing advanced laparoscopic suturing, a majority of studies conducted during this period have compared open surgical drainage with endoscopy. The efficacy of these modalities has largely been evaluated using retrospective studies and a few meta-analyses particularly due to the low-volume caseload of individual centres. Also, these studies include PPC and WON together in data analysis despite WON being a distinct entity. There are limited prospective well-designed clinical trials comparing endoscopic and laparoscopic management of pure PPCs. There is also a lack of specific recommendations for the management of PPCs. Considerable overlap of indications between these two modalities exists. The efficacy of endoscopic transmural drainage as an index intervention when compared to laparoscopy has not been proven in the research literature. Previous studies have not considered multiple endoscopic interventions within a four-week period of index intervention as a failure. We reviewed the literature using appropriate MeSH terms on the PubMed search engine for articles comparing laparoscopic and endoscopic transmural management of PPCs according to our inclusion and exclusion criteria. Seven articles were identified for inclusion in the qualitative synthesis. This scoping review was conducted to answer some pertinent unanswered questions, identify gaps in knowledge regarding the laparoscopic vs endoscopic management of PPCs, and guide further research.

Abstract:AIM:To compare short- and long-term outcomes of laparoscopic vs open distal pancreatectomy for solid pseudopapillary tumor (SPT) of the pancreas.METHODS:This retrospective study included 28 patients who underwent distal pancreatectomy for SPT of the pancreas between 1998 and 2012. The patients were divided into two groups based on the surgical approach: the laparoscopic surgery group and the open surgery group. The patients' demographic data, operative results, pathological reports, hospital courses, morbidity and mortality, and follow-up data were compared between the two groups.RESULTS:Fifteen patients with SPT of the pancreas underwent laparoscopic distal pancreatectomy (LDP), and 13 underwent open distal pancreatectomy (ODP). Baseline characteristics were similar between the two groups except for a female predominance in the LDP group (100.0% vs 69.2%, P = 0.035). Mortality, morbidity (33.3% vs 38.5%, P = 1.000), pancreatic fistula rates (26.7% vs 30.8%, P = 0.728), and reoperation rates (0.0% vs 7.7%, P = 0.464) were similar in the two groups. There were no significant differences in the operating time (171 min vs 178 min, P = 0.755) between the two groups. The intraoperative blood loss (149 mL vs 580 mL, P = 0.002), transfusion requirement (6.7% vs 46.2%, P = 0.029), first flatus time (1.9 d vs 3.5 d, P = 0.000), diet start time (2.3 d vs 4.9 d, P = 0.000), and postoperative hospital stay (8.1 d vs 12.8 d, P = 0.029) were significantly less in the LDP group than in the ODP group. All patients had negative surgical margins at final pathology. There were no significant differences in number of lymph nodes harvested (4.6 vs 6.4, P = 0.549) between the two groups. The median follow-up was 33 (3-100) mo for the LDP group and 45 (17-127) mo for the ODP group. All patients were alive with one recurrence.CONCLUSION:LDP for SPT has short-term benefits compared with ODP. Long-term outcomes of LDP are similar to those of ODP.

Abstract:Objective To evaluate the value of laparoscopic extracorporeal drainage of pancreatic pseudocyst in the treatment of pancreatic pseudocyst . Methods Clinical data of 25 patients with pancreatic pseudocysts from July 2012 to July 2015 were retrospectively analyzed .For 21 patients with pancreatic pseudocyst not connecting with the main pancreatic duct , laparoscopic pancreatic pseudocyst external drainage was performed .For 4 patients with cysts connecting with the main pancreatic duct for more than 5 cm, cyst gastric drainage or cyst jejunum Roux-Y anastomosis was performed . Results In 21 patients receiving laparoscopic pancreatic pseudocyst external drainage , the average operative time was 65.8 min (range, 55-88 min), the average bleeding was 50 ml (range, 20-78 ml), and the average hospitalization time was 8.5 days (7 -13 days).No conversion to open surgery was required.In 2 cases receiving cyst gastric drainage , the operation time was 72 and 75 min, the intraoperative blood loss was 70 and 82 ml, and the hospitalization time was 7 and 7.5 d.In 2 cases receiving cyst jejunum Roux-Y anastomosis, the operation time was 112 and 122 min, intraoperative blood loss was 120 and 105 ml, and hospital stay was 9 and 9.5 d.The 23 patients were followed up for 6-24 months ( mean, 15 months ) , without cysts recurrence . Conclusion For patients with pancreatic pseudocysts not communicating with the main pancreatic duct , laparoscopic pancreatic pseudocyst external drainage has good results .

Abstract:Forty-three cases of pancreatic pseudocysts (PPC) are reviewed. Various emergency and elective surgical therapeutical options have been employed: while external drainage has confirmed to represent the best procedure in emergency occurrences, cystojejunostomy has proved to be the treatment that better provides both optimal functional results and minimal mortality/morbidity in elective situations. Our study also confirms that the thickness of the pseudocystic wall is the only feature apt to determine whether or not a PPC is susceptible of internal surgical derivation, and that abdominal ultrasound scanning is the most sensitive and reliable diagnostic method to ascertain this prerequisite, thus providing a well-timed strategy for elective derivative operations.
