Value of TIMI Myocardial Perfusion Frame Count for Prediction of Microvascular Dysfunction in Reperfused STEMI Patients, View of Cardiac Magnetic Resonance Early after Reperfusion

Abstract:Reperfusion is the preferring therapy for ST-segment elevation myocardial infarction (STEMI) in the initial 12 hours after onset of symptom. Despite the restoration of epicardial coronary flow, myocardial microvascular perfusion has been proved to be a more important factor for patient’s prognosis. Several methods have been established to evaluate microvascular perfusion including Thrombolysis In Myocardial Infarction (TIMI) flow grade, TIMI myocardial perfusion grading (TMPG) and myocardial blush grading (MBG) . However, the value of these modalities were largely restricted due to their subjective inherence. Recently, we developed a new quantitative method named TIMI Myocardial Perfusion Frame Count (TMPFC) for more objective assessment of microvascular perfusion. Former studies suggested it a better way against aforementioned methods in predicting 30-day and 6-month major adverse cardiovascular events (MACE) in reperfused STEMI patients. Current data aimed to disclose the direct relationship between TMPFC and results of Cardiac magnetic resonance (CMR), which is the golden standard for heart function, necrotic area and microvascular dysfunction.
