Abstract:AIM: To assess neovascularization within human carotid atherosclerotic soft plaques in patients with ischemic stroke.METHODS: Eighty-one patients with ischemic stroke and 95 patients without stroke who had soft atherosclerotic plaques in the internal carotid artery were studied. The thickest soft plaque in each patient was examined using contrast-enhanced ultrasound. Time-intensity curves were collected from 5 s to 3 min after contrast injection. The neovascularization within the plaques in the internal carotid artery was evaluated using the ACQ software built into the scanner by 2 of the experienced investigators who were blinded to the clinical history of the patients.RESULTS: Ischemic stroke was present in 7 of 33 patients (21%) with grade. plaque, in 14 of 51 patients (28%) with grade. plaque, in 26 of 43 patients (61%) with grade. plaque, and in 34 of 49 patients (69%) with grade. plaque (P < 0.001 comparing grade. plaque with grade I plaque and with grade. plaque and P = 0.001 comparing grade. plaque with grade. plaque and with grade. plaque). Analysis of the time intensity curves revealed that patients with ischemic stroke had a significantly higher intensity of enhancement (IE) than those without ischemic stroke (P < 0.01). The wash-in time (WT) of plaque was significantly shorter in stroke patients (P < 0.05). The sensitivity and specificity for IE in the plaque were 82% and 80%, respectively, and for WT were 68% and 74%, respectively. There was no significant difference in the peak intensity or time to peak between the 2 groups.CONCLUSION: This study shows that the higher the grade of plaque enhancement, the higher the risk of ischemic stroke. The data suggest that the presence of neovascularization is a marker for unstable plaque. (C) 2010 Baishideng. All rights reserved.
Neovascularization in the Carotid Atherosclerotic Plaque is Not Associated with Inflammatory Cell Aggregation

Abstract:Background Monocytes and macrophages in atherosclerotic plaque lead to plaque instability. The aim of the study was to determine if plaque neovascularization led to inflammation. Methods Patients were consecutively enrolled if their carotid intimal media thickness was >2 mm, as revealed by duplex ultrasound. The patients then underwent dynamic contrast enhanced magnetic resonance imaging (DCE MRI) and fluorine-18 fluorodeoxyglucose (18F-FDG) positron emission tomography combined with computed tomography (PET CT). A target to background ratio (TBR) of ≥1.25 or <1.25 served as the cutoff point for the presence and absence of inflammation, respectively. Results Twenty-six patients underwent bilateral carotid DCE MRI and 24 patients also underwent PET CT. One hundred and fifty-five plaques were evaluated by both DCE MRI and PET CT. There was no significant difference in plaque morphology between the TBR ≥1.25 (n=61) and TBR <1.25 (n=94) groups. No significant differences were found in plasma volume and transfer constant between the TBR ≥1.25 and TBR <1.25 groups. Conclusion Our study did not find a significant correlation between plaque neovascularization and the aggregation of inflammatory cells.
Contrast-Enhanced Sonographic Characteristics of Neovascularization in Carotid Atherosclerotic Plaques

Abstract:Purpose. To evaluate neovascularization within carotid atherosclerotic plaques with contrast-enhanced sonography.Methods. We used contrast-enhanced sonography to examine 63 patients with carotid atherosclerotic plaques. The features of neovascularization within the plaques were analyzed and correlated with plaque size and echogenicity.Results. There were 81 atherosclerotic plaques, 62 of which (43 soft and 19 mixed) enhanced after injection of a contrast agent. The enhancement occurred from the carotid wall to the center of the plaque with a short-line pattern in 36 plaques, whereas 26 plaques enhanced from both the carotid wall and the carotid lumen, with a sparse spot pattern. The arrival time of contrast was shorter (p < 0.001) and time to peak was longer (p < 0.001) in the plaques than in the carotid lumen. Time to peak was shorter, whereas enhanced intensity was greater in soft plaques than in mixed plaques (p < 0.01 and p < 0.05, respectively). Among the 19 unenhanced plaques, 6 were hard, 3 were calcified, 3 were soft, and 7 were mixed. The thickness of the unenhanced plaques was <2.4 mm.Conclusion. Contrast-enhanced sonography allows the noninvasive, dynamic evaluation of neovascularization within carotid plaques, and the presence of neovascularization may correlate with plaque morphology. Abstract:To assess the neovascularization within human carotid atherosclerotic soft plaques in patients with ischemic stroke using real-time contrast-enhanced ultrasonography. Eighty-six patients with ischemic stroke confirmed by computed tomography or magnetic resonance imaging who had soft atherosclerotic plaques in the carotid artery were enrolled. Ninety-seven controls with soft carotid atherosclerotic plaques but no clinical stroke were recruited at the same hospital and matched for age and gender. The thickest soft plaque in each patient (selecting the side relevant to the stroke) and control was examined with real-time ultrasonography following an i.v. injection of 2.4 mL of the microbubble contrast agent SonoVue using CPS at 7MHz on a Sequoia system at an MI of 0.35. Time-intensity curves were collected from 5 seconds to 3 minutes after the injection. An experienced off-site radiologist who was blinded to the clinical history of the patients and controls analyzed the neovascularization within the plaques quantitatively using the ACQ software built into the scanner. Analysis of the time intensity curves revealed that patients who had suffered ischemic stroke had a significantly higher intensity of enhancement than those without ischemic stroke (P<0.01). The wash-in time of plaque was significantly shorter in the stroke patients (P<0.05). This study confirms that contrast-enhanced ultrasonography can assess neovascularization within carotid atherosclerotic plaques. Patients with ischemic stroke had significantly greater signal intensity and shorter wash-in time than controls without ischemic stroke, suggesting that the presence of neovascularization is a marker for unstable plaque.
Observations on the morphology and inflammatory reaction of carotid atherosclerotic plaques and its clinical manifestations

Abstract:Objective: To evaluate the relationship between the morphology, the inflammatory reaction and clinical symptoms of carotid atherosclerotic plaques. Methods: Nine patients with symptomatic carotid stenosis were selected. Four of them had embolus detachment from the plaques, and 5 had not. All patients underwent carotid endarterectomy. Some of the atherosclerotic plaques specimens were obtained for routine hematoxylin and eosin (HE) staining and the morphological changes were observed; the other specimens were obtained for immunohistochemical staining, and the expressions of CD3, CD45RO, CD20, alpha-smooth muscle actin (α-SMA) and proliferating cell nuclear antigen (PCNA) were observed under light microscope, and the relationship between the degree of inflammatory cell infiltration and the embolus detachment was studied. Results: 1Light microscopic examination showed some newly formed blood vessels, inflammatory cell infiltration and reduction of smooth muscle cells at the plaque shoulders and fibrous caps. The inflammatory cells mainly distributed around the blood vessels. The more obvious the vascular proliferation, the more infiltrated inflammatory cells and the less smooth muscle cells were. 2The results of immunohistochemistry showed that the infiltrated inflammatory cells were chiefly CD3- and CD45RO-positive T-lymphocytes, The expression of α-SMA in the fibrous cap of the plaque showed heterogeneously, and there was no expression in some part of the plaques. The staining of PCNA was positive, and it meant that the cellular proliferation was in the active phase. The carotid atherosclerotic plaques in patients with embolus detachment had more obvious inflammatory cellular infiltration. Conclusion: The T-lymphocytes dominated inflammatory reaction plays an important role for the instability of the carotid atherosclerotic plaques.
The effect of neovascularization in carotid plaque on clinical manifestations

Abstract:Objective To explore the effect of angiogenesis in carotid atherosclerotic plaque.Methods From Jan 2016 to Aug 2016,Carotid artery plaque was abtained in 52 cases after carotid endarterectomy at the Department of Cardiovascular Surgery of China-Japan Friendship Hospital.Patients were divided into symptomatic group and asymptomatic group.Specimens were stained with HE and Movat,and the density,size,distribution and morphology of neovascularization were counted.Results The density of neovascularization in the symptomatic group and the asymptomatic group were 5.27 ± 0.46 and 2.30 ±0.29,respectively (P ＜ 0.001),the average cross-sectional area of neovascularization in the symptomatic group was (2.26±0.21) mm2 and (1.00 ±0.48) mm2 in the asymptomatic group (P=0.02).In the distribution,the symptomatic group and the asymptomatic group were 3.37 ± 0.46/ mm2,1.32 ±0.16/mm2 in basal part,3.71 ±0.42/mm2,1.56 ±0.20/mm2 in the shoulder part,3.48 ±0.44/mm2,1.55 ± 0.21/ mm2 in the fibrous cap,respectively (P ＜ 0.001).Conclusion The density and cross-sectional area of neovascularization in the symptomatic group were larger than those in the asymptomatic group,irregular branching vessels were dominant.
Neutrophil extracellular traps mediate the crosstalk between plaque microenvironment and unstable carotid plaque formation

Abstract:The development of unstable carotid atherosclerotic plaques is associated with the induction of neutrophil extracellular traps (NETs) via the activation of diverse inflammatory mediators in the circulating bloodstream. However, the underlying mechanisms through which NETs influence the microenvironment of atherosclerotic plaques and contribute to the development of unstable carotid plaques remain largely elusive. The objective of this study was to elucidate the role of myeloid differentiation protein 1 (MD-1, LY86)-induced NETs underlying the crosstalk between unstable plaque formation and the plaque microenvironment. We employed bioinformatics analysis to identify key genes associated with carotid-unstable plaque, followed by comprehensive validation using various experimental approaches on tissue specimens and plasma samples classified based on pathological characteristics. Patients with carotid-unstable plaques exhibited elevated plasma concentrations of MD-1 (LY86), while patients with stable plaques demonstrated comparatively lower levels. Furthermore, soluble MD-1 was found to induce the formation of NETs through activation of Toll-like receptor signaling pathway. The proliferative and immature vascularization effects of NETs on endothelial cells, as well as their inhibitory impact on cell migration, are directly correlated with the concentration of NETs. Additionally, NETs were found to activate the NF-κB signaling pathway, thereby upregulating ICAM1, VCAM1, MMP14, VEGFA, and IL6 expression in both Human umbilical vein endothelial cells (HUVECs) and HAECs. Subsequently, a significant increase in intraplaque neovascularization by NETs results in poor carotid plaque stability, and NETs in turn stimulate macrophages to produce more MD-1, generating a harmful positive feedback loop. Our findings suggest that soluble MD-1 in the bloodstream triggers the production of NETs through activation of the Toll-like receptor signaling pathway and further indicate NETs mediate a crosstalk between the microenvironment of the carotid plaque and the neovascularization of the intraplaque region. Inhibiting NETs formation or MD-1 secretion may represent a promising strategy to effectively suppress the development of unstable carotid plaques.
Expression of NF-κB, CD68 and CD105 in Carotid Atherosclerotic Plaque.

Abstract:Atherosclerotic plaque vulnerability is associated with cerebrovascular events in patients with carotid atherosclerosis. The aim of this study was to investigate the expression of inflammatory factors in carotid artherosclerotic plaques in order to explore its clinical significance in patients with carotid stenosis. Forty three patients with carotid stenosis were divided into symptomatic group (n=24) and asymptomatic group (n=19) based on clinical manifestation. All patients were treated with selective standard carotid endarterectomy (CEA); the carotid atherosclerotic plaques were removed surgically and studied pathologically to investigate the expression of nuclear factor-kappa κ (NF-κB), CD68 and CD105. The plaques were grouped into stable and unstable plaques based on thickness of the fibrous cap and the area of lipid-rich core in the plaques. The proportion of unstable plaques were significantly higher in symptomatic group than in asymptomatic group (70.8% vs. 63.2%, P=0.026). Results of immunohistochemisty staining showed that the expression of NF-κB, CD68 and CD105 in unstable plaques was higher than stable plaques (P<0.001). The association of the higher expression of these factors with instability of carotid plaque needs to be clarified in future study.

Association of cardiovascular risk factors and intraplaque neovascularization in symptomatic carotid plaque

Abstract:Background and purpose: Cardiovascular risk factors are known to contribute to the formation of atherosclerotic plaques, which can result in carotid stenosis. However, the extent to which these factors are associated with intraplaque neovascularization, a key indicator of plaque vulnerability, remains unclear. To investigate this relationship, a study was conducted utilizing contrast-enhanced ultrasound (CEUS) to assess intraplaque neovascularization in symptomatic patients. Methods: A cohort of 157 symptomatic patients underwent evaluation using Contrast-Enhanced Ultrasound (CEUS) imaging to assess carotid intraplaque neovascularization, which was quantified based on the degree of plaque enhancement. The collected data encompassed baseline patient characteristics, results from biochemical examinations, cardiovascular risk factors, and medication usage history. Regression analyses were conducted to elucidate the relationship between carotid plaque neovascularization and various cardiovascular risk factors. Results: Patients with intraplaque neovascularization were more prone to have diabetes mellitus (OR 3.81, 95% CI 1.94-7.46, p < 0.001), dyslipidemia (OR 2.36, 95% CI 1.22-4.55, p = 0.011) and hypertension (OR 2.92, 95% CI 1.50-5.71, p = 0.002). Smoking increased the risk of having intraplaque neovascularization (OR 2.25, 95% CI 1.12-4.54, p = 0.023). Treatment with statins was significantly lower in patients with intraplaque neovascularization (OR 0.37, 95% CI 0.19-0.72, p = 0.003). In the multivariate analysis, diabetes mellitus (OR 3.27, 95% CI 1.10-9.78, p = 0.034) was independently related to the presence of intraplaque neovascularization. Meanwhile, compared to the patients in the first tertile of serum glucose (< 6.20 mmol/L), the patients in the third tertile (> 13.35 mmol/L) had the most significance of intraplaque neovascularization (OR 5.55, 95% CI 1.85-16.66, p = 0.002). Conclusion: The findings indicated that diabetes mellitus is a significant cardiovascular risk factor that is strongly associated with carotid intraplaque neovascularization.

Abstract:Atherosclerosis is the focal expression of a systemic disease affecting medium- and large-sized arteries, in which traditional cardiovascular risk factor and immune factors play a key role. It is well accepted that circulating biomarkers, including C-reactive protein and interleukin-6, reliably predict major cardiovascular events, including myocardial infarction or death. However, the relevance of biomarkers of systemic inflammation to atherosclerosis progression in the carotid artery is less established. The large majority of clinical studies focused on the association between biomarkers and subclinical atherosclerosis, that is, carotid intima-media thickening (cIMT), which represents an earlier stage of the disease. The aim of this work is to review inflammatory biomarkers that were associated with a higher atherosclerotic burden, a faster disease progression, and features of plaque instability, such as inflammation or neovascularization, in patients with carotid atherosclerotic plaque, which represents an advanced stage of disease compared with cIMT. The association of biomarkers with the occurrence of cerebrovascular events, secondary to carotid plaque rupture, will also be presented. Currently, the degree of carotid artery stenosis is used to predict the risk of future cerebrovascular events in patients affected by carotid atherosclerosis. However, this strategy appears suboptimal. The identification of suitable biomarkers could provide a useful adjunctive criterion to ensure better risk stratification and optimize management.

Risk Factor Differences in Calcified and Non-Calcified Cervical Carotid Plaque

Abstract:Background: Vascular calcification is associated with the inflammatory process. Here, we investigated the relationship of serum lipoprotein-associated phospholipase A2 (Lp-PLA2) and traditional risk factors with carotid plaque calcification in patients with ischemic stroke (IS) or transient ischemic attack (TIA). Methods: Patients with IS or TIA were divided into the calcification (Ca) and non-calcification (nCa) groups according to the results of the CTA of carotid artery with plaque. LP-PLA2 level, demographics, vascular risk factors, and other clinical data were compared to analyze the risk factors for carotid plaque calcification. Results: A total of 260 patients were enrolled, including 134 patients in Group Ca and 126 patients in Group nCa. Multiple logistic regression showed Lp-PLA2 (p = 0.016), age (p = 0.010), and diabetes (p = 0.010) were independent risk factors of carotid artery plaque calcification. The Spearman's rank correlation coefficients showed Lp-PLA2 (r = 0.372, p = 0.000), age (r = 0.325, p = 0.000), and diabetes (r = 0.421, p = 0.000) had a positive correlation with the calcification of the carotid artery. Conclusions: Serum Lp-PLA2, age, and diabetes were associated with carotid plaque calcification in patients with cerebral infarction or TIA. The inflammatory process mediated by Lp-PLA2 and diabetes may be involved in the pathogenesis of carotid calcification.
Study on expression of platelet membrane glycoproteins in cerebral infarction with carotid atherosclerosis

Abstract:Objective To investigate how activating platelet affects on the onset of atherosclerotic cerebral infarction. Methods Positive rates of PAC-1 and CD62P,which are both platelet membrane glycoproteins,were measured by flow cytometry in blood samples while carotid artery were evaluated by color Doppler ultrasound in 76 patients with atherosclerotic cerebral infarction and 53 healthy examination persons. Results The positive rates of PAC-1 and CD62P in the group of cerebral infarction with carotid atherosclerosis were the highest(PAC-1:15.1±5.9,CD62P:16.2±7.3). The positive rates in the cerebral infarction groups no matter with or without carotid atherosclerosis were significantly higher than those in the corresponding control groups(P<0.05),and the rates in the groups with carotid atherosclerosis were significantly higher than those in the groups without carotid atherosclerosis both in cerebral infarction groups and in control groups(P<0.05). In the group of cerebral infarction with carotid atherosclerosis,positive rates of PAC-1 and CD62P in the group with carotid atheromatus plaque were significantly higher than those in the group without atheromatus plaque(P<0.05). In the group of cerebral infarction with carotid atheromatus plaque,positive rates of PAC-1 and CD62P in the group with unstable carotid atheromatus plaque were significantly higher than those in the group with stable plaque(P<0.05). Conclusion The levels of platelet activation were higher in cerebral infarction patients with carotid atherosclerosis,and was much higher in those patients with carotid atheromatus plaque,especially with unstable plaque. The level of platelet activation is related with carotid atherosclerosis as well as cerebral infarction. We suspect activating platelet may play an important role in the onset and progress of cerebral infarction with carotid atherosclerosis.
Comparison between superb microvascular imaging and histopathology for the evaluation of carotid artery atherosclerotic plaque neovascularization

Abstract:Objective To investigate the significance of superb microvascular imaging (SMI) for the detection of neovascularization in carotid atherosclerotic plaques.Methods SMI detected 28 carotid atherosclerotic plaques in 28 patients who underwent carotid endarterectomy.The SMI grade of carotid plaque neovascularization was assigned based on the following grading system:Grade 0,no neovascularization;grade 1,some neovascularization;grade 2,neovascularization observed in the shoulder of the plaque;and grade 3,widely distributed new blood vessels.SMI was used to evaluate the degree of blood flow,maximum flow velocity,and resistance index of the new vessels in the plaque.Microvascular density was measured with CD31 staining.Results Clear correlations were observed between neovascularization density on histopathological staining of the plaque and the degree of blood flow detected with SMI (rs=0.788,P ＜ 0.001).The classification of neovascular blood flow in the plaque was positively correlated with plaque thickness (rs=0.686,P=0.002).Of the 28 cases,neovascularization was detected in 23;of these 23 cases,the direction of blood flow was from the adventitia to the intima in 17 (75％) cases,while 6 (25％) cases showed an irregular direction of blood flow.The mean neovascular flow velocity was (5.620±0.131) cm/s,and the mean neovascular resistance index was 0.660 ± 0.090.Conclusion SMI is useful for the evaluation of carotid atherosclerotic plaque neovascularization.
Analysis of MicroRNAs Associated With Carotid Atherosclerotic Plaque Rupture With Thrombosis

Abstract:Atherosclerosis is a progressive vascular wall inflammatory disease, and the rupture of atherosclerotic vulnerable plaques is the leading cause of morbidity and mortality worldwide. This study intended to explore the potential mechanisms behind plaque rupture and thrombosis in ApoE knockout mice. The spontaneous plaque rupture models were established, and left carotid artery tissues at different time points (1-, 2-, 4-, 6-, 8-, 12-, and 16-week post-surgery) were collected. By the extent of plaque rupture, plaque was defined as (1) control groups, (2) atherosclerotic plaque group, and (3) plaque rupture group. Macrophage (CD68), MMP-8, and MMP-13 activities were measured by immunofluorescence. Cytokines and inflammatory markers were measured by ELISA. The left carotid artery sample tissue was collected to evaluate the miRNAs expression level by miRNA-microarray. Bioinformatic analyses were conducted at three levels: (2) vs. (1), (3) vs. (2), and again in seven time series analysis. The plaque rupture with thrombus and intraplaque hemorrhage results peaked at 8 weeks and decreased thereafter. Similar trends were seen in the number of plaque macrophages and lipids, the expression of matrix metalloproteinase, and the atherosclerotic and plasma cytokine levels. MiRNA-microarray showed that miR-322-5p and miR-206-3p were specifically upregulated in the atherosclerotic plaque group compared with those in the control group. Meanwhile, miR-466h-5p was specifically upregulated in the plaque rupture group compared with the atherosclerotic plaque group. The highest incidence of plaque rupture and thrombosis occurred at 8 weeks post-surgery. miR-322-5p and miR-206-3p may be associated with the formation of atherosclerotic plaques. miR-466h-5p may promote atherosclerotic plaque rupture via apoptosis-related pathways.

Abstract:Objective: Inflammation is a central driver of atherogenesis and eventual plaque rupture. This study aimed to evaluate the association between residual inflammatory risk (RIR) and vulnerable plaques in the carotid artery in patients with ischemic stroke. Methods: Patients with acute ischemic stroke were enrolled from January 2021 to July 2022. They were divided into four groups: RIR only (LDL-C <2.6 mmol/L and hsCRP ≥2 mg/L), residual cholesterol risk (RCR) only (LDL-C ≥2.6 mmol/L and hsCRP <2 mg/L), both risk or residual cholesterol and inflammatory risk (RCIR) (LDL-C ≥2.6 mmol/L and hsCRP ≥2 mg/L), and neither risk (LDL-C <2.6 mmol/L and hsCRP <2 mg/L). Vulnerable plaques were determined if it had a low attenuated plaque CT value of 1.1, which indicated a positive remodeling. Results: Out of the 468 enrolled patients, 157 (33.5%) were detected to have vulnerable plaques. The proportion of patients with neither risk, RIR, RCR, and RCIR were 32.9%, 28.6%, 18.8%, and 19.7%, respectively. Patients with vulnerable plaques exhibited a higher prevalence of hyperlipidemia ( P = 0.026), higher proportion of RIR ( P = 0.015), a higher ratio of stroke subtypes of large artery atherosclerosis ( P = 0.012), and high leukocyte counts ( P < 0.001). The logistic regression analysis detected that RIR was associated with vulnerable plaques after adjusted for major confounding factors (OR 1.98, 95% CI 1.13–3.45, P = 0.016), especially in the large artery atherosclerosis subtype (OR 2.71, 95% CI 1.08–6.77, P = 0.034). Conclusions: In patients with ischemic stroke, RIR is associated with the vulnerability of carotid plaques, especially for those with the large artery atherosclerosis subtype. Therefore, further studies investigating the interventions to modulate inflammation in these patients may be warranted.

Abstract:Objective Our research use contrast-enhanced ultrasound (CEUS) to evaluate intraplaque neovascular-ization in carotid atherosclerotic plaque and its relationship with severity and prognosis of cerebral infarction patients. Methods Patients with acute cerebral infarction admitted to the department of neurology of Peking University people’s hospital were selected. All subjects were divided into non-plaque group and plaque group according to the presence or ab-sence of carotid plaques. CEUS was performed on the plaque group. The IPN status was graded as grade 0,grade 1,grade 2 and grade 3. Subjects with grade 0 and grade 1 carotid plaque were divided into stable-plaque group. Subjects with grade 2 and grade 3 carotid plaque were divided into vulnerable-plaque group. NIHSS score and 90-days mRS score were compared between the non-plaque group,the stable-plaque group and the vulnerable-plaque group. Results There was no significant difference in NIHSS score and 90-days mRS score between the non-plaque group and the stable-plaque group (P>0. 05). Patients in the vulnerable-plaque group had significantly higher NIHSS score and 90-days mRS score than those in the non-plaque group and stable-plaque group respectively (P<0. 01). Spearman correlation analysis suggested that the grading of IPN was positively correlated with NIHSS score and 90-days mRS score respectively ( P =0. 441,0. 404, P <0. 01). Conclusion The higher the plaque grading of IPN,the more severe clinical symptoms and worse outcome those cerebral in-farction patients have. CEUS can distinguish between stable plaques and vulnerable plaques according to the degree of IPN, which is helpful for the risk stratification of patients with acute cerebral infarction.
Association of angiogenesis-associated genes with atherosclerotic plaque progression, intraplaque hemorrhage, and immune infiltration

Abstract:Mounting evidence suggests that intraplaque angiogenesis is associated with the progression of atherosclerotic plaques and the development of intraplaque hemorrhage. The specificity of intraplaque immune cell infiltration may be associated with abnormalities in the structure and function of the nascent capillaries. Here, we analyzed expression levels of angiogenesis-associated genes in early and advanced carotid atheromatous plaque tissues as well as in stable and intraplaque hemorrhage plaques. Expression profiles of advanced arterial plaques based on angiogenesis-associated genes were classified into subtypes by performing a consensus clustering analysis. The correlation between the immune microenvironment of plaques and expression of angiogenesis-associated genes was also explored using the single sample gene set enrichment analysis method and the CIBERSORT algorithm. We identified hub angiogenesis-associated genes showing similar expression patterns throughout plaque adverse progression, and constructed a prediction model using the random forest algorithm. Receiver operating curves were constructed to evaluate efficacy in identification of intraplaque hemorrhage in a plaque. Our results suggest that heterogeneity of angiogenesis-related genes may promote the malignant development of plaques and cause plaque rupture. In conclusion, we propose a model based on expression of angiogenesis-related genes to predict the risk of plaque rupture.

Abstract:The objective of this study was to investigate the correlation between the amount of blood flow in the area of neovascularization within a carotid atherosclerotic plaque by superb microvascular imaging (SMI) and the microvessel density (MVD) determined by histopathological staining. Twenty-eight carotid atherosclerotic plaques were detected by SMI in 28 patients who underwent carotid endarterectomy. SMI was graded according to the visual methods as follows: grade I: no appearance of neovascularization within the plaque; grade II: punctate neovascularization; grade III: one or two linear neovascularizations within the plaque; and grade IV: multiple (> 2) linear neovascularizations throughout the plaque. The neovascularization density was determined by the CD31 complex staining method. There was a significant correlation between the density of neovascularization in histopathologic plaques and the blood flow grade found by SMI (r = 0.788, p < 0.001). A significant difference was observed in SMI blood flow grade between the symptomatic and asymptomatic groups (chi(2) = 2.634, p = 0.036). The MVD of plaques in the symptomatic group was significantly higher than that in the asymptomatic group (t = 2.530, p = 0.018). The SMI-based classification was positively correlated with plaque thickness. SMI, which is a new nonultrasound contrast-enhanced imaging method, can effectively evaluate neovascularization in carotid atherosclerotic plaques and can be used as a novel method for the clinical prediction of stroke risk.
