Abstract:To compare the effects of sulfonylureas and metformin versus metformin on lipid profiles, blood pressure, and adverse events. PubMed, EMbase, Chinese BioMedical Literature on disc, China National Knowledge Infrastructure, VIP database, and Wanfang database were searched for randomized controlled trials (RCTs), from inception to August 2012. Key outcomes were low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG), total cholesterol (TC), blood pressure (BP), hemoglobin A1c (HbA1c), fasting insulin, and adverse events. Twenty RCTs were included in the analysis. Compared to metformin, the combination therapy of sulfonylureas and metformin slightly reduced HDL-C [−0.03, 95 % CI (−0.06, −0.01)] and HbA1c (−0.79, 95 % CI −0.96 to −0.63). However, it showed little effects on LDL-C, TG, TC, and BP. Glipizide plus metformin significantly increased fasting insulin [2.33, 95 % CI (1.94, 2.73)]. Hypoglycemia and nervous system side events were more frequent among patients treated with sulfonylureas plus metformin than metformin alone (RR = 6.79, 95 % CI 3.79–12.17; RR = 1.27, 95 % CI 1.03–1.57; respectively), but less in digestive symptoms (RR = 0.75, 95 % CI 0.67–0.84). Combination therapy with sulfonylureas and metformin may be more effective than metformin alone in improving HbA1c and reducing gastrointestinal reactions. But it had disadvantage of decreasing HDL-C, increasing the risk of hypoglycemia and nervous system side events.

Abstract:Sulfonylureas have been used for type 2 diabetes treatment over 50 years. Since publication of the University Group Diabetes Program in 1970, the cardiovascular safety of sulfonylureas has been contentious. Although several hypotheses linking sulfonylurea to detrimental cardiovascular events exist, none provide conclusive evidence. Presently, several observational studies suggest that sulfonylureas are associated with increased adverse cardiovascular outcome; however, randomized controlled trials have demonstrated a neutral effect. Evaluating cardiovascular safety of sulfonylureas should be based on comprehensive consideration, such as differential drugs, doses, durations, age of patient, body mass index. To date, there is lack of head-to-head study, which is focused on cardiovascular outcome of sulfonylureas. Fortunately, the current ongoing large clinical trials may clarify the cardiovascular safety of sulfonylureas.
Sulphonylureas monotherapy and risk of hospitalization for heart failure in patients with type 2 diabetes mellitus: A population-based cohort study in China.

Abstract:Purpose The risk of heart failure associated with sulphonylureas is unclear. We evaluated the association between sulphonylureas and hospitalization of heart failure (HHF) in patients with type 2 diabetes mellitus (T2DM) in China. Methods A retrospective cohort study was implemented using the Yinzhou Regional Health Care Database (YRHCD). We identified 15 752 adult patients with T2DM who were newly exposed to sulphonylurea monotherapy (N = 12 487) or acarbose monotherapy (N = 3265) from January 2010 to September 2016. Cox proportional hazards models weighted by inverse probability of treatment weights were used to compare the risk of HHF between initiators of sulphonylurea and acarbose. Results During a median follow-up of 0.55 (0.49, 1.11) and 0.49 (0.35, 0.70) years for sulphonylureas and acarbose initiators separately, 320 patients developed HHF, with 279 events in sulphonylureas group, and 41 events in acarbose group. The incidence rates of HHF among sulphonylureas initiators and acarbose initiators were 22.2 (95% CI 19.6-24.9) and 18.3 (95% CI 13.2-24.9) per 1000 person-years, respectively. The adjusted hazard ratio (aHR) of HHF for sulphonylureas vs acarbose was 1.61 (95% CI 1.14-2.27). When stratified by history of heart failure, aHR was 1.55 (95% CI 0.79-3.06) in patients with a history of heart failure, and 1.64 (95% CI 1.10-2.45) in patients with no history of heart failure. Conclusions Our study suggested that use of sulphonylureas monotherapy compared with acarbose monotherapy for initial treatment of T2DM for approximately 0.5 years are significantly associated with a higher risk of HHF.
Short term effects of sodium-glucose cotransporter 2 inhibitors on lipid regulation in coronary artery disease patients undergoing Percutaneous Coronary Intervention: A Propensity-Matched Analysis

Abstract:Abstract Background Sodium-glucose cotransporter 2 (SGLT2) inhibitors are currently widely used hypoglycemic drugs. Due to its cardiorenal protective effect independent of hypoglycemic effect, it is turning out to be progressively significant in the cardiovascular field. However, the effect of SGLT2 inhibitors on lipids is still controversial. We aimed to investigate whether the use of SGLT2 inhibitors could further down-regulate LDL-cholesterol (LDL-C) level of patients undergoing percutaneous coronary intervention (PCI) with background hypolipidemic therapy. Methods The longitudinal data were collected from Hospital Information System (HIS) of the Sir Run Run Shaw Hospital. SGLT2 inhibitor users and nonusers undergoing PCI were selected. Lipid data at baseline and 1-month follow-up after PCI were recorded. Propensity score 1:1 matching (PSM), inverse probability of treatment weighting (IPTW), and overlap weighting (OW) were performed to adjust baseline demographic and clinical characteristics. Results We identified 2451 coronary artery disease patients undergoing percutaneous coronary intervention. On the one month follow up after PCI, the serum lipid profile of all patients showed that triglyceride (TG) (1.26 [0.94;1.75] vs 1.47 [1.06;2.06], P < 0.001), total cholesterol (TC) (3.32 [2.83;3.90] vs 4.20 [3.51;5.13], P < 0.001), LDL-cholesterol (LDL-C) (1.69 [1.27;2.13] vs 2.43 [1.81;3.12], P < 0.001), and VLDL-cholesterol (VLDL-C) (0.34 [0.14;0.57] vs 0.46 [0.22;0.74], P < 0.001) decreased significantly; Free fatty acids (FFA) (0.48 [0.33;0.61] vs 0.45 [0.28;0.56], P < 0.001) increased; HDL-cholesterol (HDL-C) and lipoprotein(a) (Lpa) have no significant difference (P > 0.05). There was no significant difference in the changes of seven indexes of serum lipid profile in PSM cohort, including 361 SGLT2 inhibitor users and 361 nonusers (P > 0.05). Similar trends were observed in IPTW and OW cohorts (P > 0.05). Conclusions In coronary artery disease patients undergoing percutaneous coronary intervention, SGLT2 inhibitors do not confer benefit or harm on lipid regulation in the context of the original hypolipidemic strategy.
Sulfonylurea and Cancer Risk among Patients with Type 2 Diabetes: A Population-Based Cohort Study.

Abstract:BackgroundCurrent evidence of the association between the use of sulfonylurea and cancer risk is highly conflicting and little evidence of this association is from the mainland Chinese population. This study aimed to evaluate the potential effects of sulfonylurea use on cancer risk among patients with type 2 diabetes mellitus (T2DM).MethodsA retrospective cohort study of T2DM patients who were new users of sulfonylurea or metformin was conducted using the Yinzhou Regional Health Care Database. A marginal structural Cox model was used to estimate the hazard ratio (HR) of cancer associated with the use of sulfonylurea compared with metformin, with time-varying confounders controlled by inverse probability weighting. Secondary analyses using different glucose-lowering drugs (GLDs) as comparator and sensitivity analyses for potential bias due to latency period, model misspecification, missing data, analyses strategy (intention-to-treat and per-protocol), and diagnosis validation were performed to examine the robustness of the results.ResultsAfter fully controlling for time-varying confounding, baseline confounding, and competing risk, the use of sulfonylurea was not associated with the risk of any cancer (HR 1.09; 95% CI, 0.93–1.27), compared with the use of metformin. In the secondary analyses, compared with α - glucosidase inhibitors, thiazolidinediones, glinides, other GLDs except sulfonylure and insulin, and T2DM patients not treated with sulfonylureas, the HRs of the association between sulfonylurea use and cancer risk were 0.92 (95% CI; 0.78–1.08), 0.89 (95% CI; 0.66–1.19), 0.85 (95% CI; 0.71–1.02), 1.04 (95% CI; 0.89–1.22), and 1.07 (95% CI; 0.99–1.16), respectively. The results of analyses for various subgroups, risk of site-specific cancers, cumulative duration, dose-response relationship, and sensitivity analyses of different latency periods and missing data were generally consistent with the findings of the primary analyses.ConclusionNo association between sulfonylurea use and cancer risk was found in this study after properly controlling biases due to time-varying confounders and other sources. Further studies on the association between sulfonylurea use and the risk of cancer by using data from a Chinese population with higher representativeness are needed.
Comparison Of Antidiabetic Drugs Added To Sulfonylurea Monotherapy In Patients With Type 2 Diabetes Mellitus: A Network Meta-Analysis

Abstract:AimsThis study aimed to investigate the efficacy and safety of dual therapy comprising sulfonylurea (SU) plus antidiabetic drugs for the treatment of type 2 diabetes mellitus (T2DM).MethodsWe searched the PubMed, Cochrane library, and Embase databases for randomized clinical trials (>= 24 weeks) published up to December 28, 2017. Subsequently, we conducted pair-wise and network meta-analyses to calculate the odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (Cls) of the outcomes.ResultsThe final analyses included 24 trials with a total of 10,032 patients. Compared with placebo, all treatment regimens were associated with a significantly higher risk of hypoglycemia, except the combinations of SU plus sodium-glucose co-transporter-2 inhibitor (SGLT-2i) [OR, 1.35 (95% CI: 0.81 to 2.25)] or alpha-glucosidase inhibitor (AGI) [OR, 1.16 (95% CI: 0.55 to 2.44)]. Notably, the combination of SU plus glucagon-like peptide-1 receptor agonist (GLP-1 RA) was associated with the most significant increase in the risk of hypoglycemia. Furthermore, all SU-based combination regimens reduced the glycated hemoglobin (HbA1c) and fasting plasma glucose levels (FPG). However, only combinations containing SGLT-2i [MD, -1.00 kg (95% CI: -1.73 to -0.27)] and GLP-1 RA [MD, -0.56 kg (95% CI: -1.10 to -0.02)] led to weight loss.ConclusionsOur findings highlight the importance of considering the risk of hypoglycemia when selecting antidiabetic drugs to be administered concomitantly with SU. Although all classes of antidiabetic drugs improved glucose control when administered in combination with SU, SGLT-2i might be the best option with respect to factors such as hypoglycemia and body weight.

Abstract:BackgroundIncreasing evidence suggests that oral hypoglycemic agents used in type 2 diabetes mellitus (T2DM) may affect cancer risk. Sulfonylureas (SUs) are the most frequently used antidiabetic medications for T2DM. Whether using SUs has any effect on cancer has received considerable attention. The aim of this study was to assess the effects of SUs on cancer risk in T2DM patients.MethodsPublished studies were identified in PubMed, EMBASE, and the Cochrane Register of Clinical Studies, and was searched for additional information to identify randomized controlled trials (RCTs), cohort studies, and case-control studies. The abstracts and full text were screened, data collected, and the risk of bias assessed for each individual study.ResultsSeventy-seven studies (33 RCTs, 27 cohort studies, and 17 case-control studies) were analyzed. The RCTs did not report a difference in the risk of malignant tumor between SU-treated T2DM patients and controls (odds ratio [OR] 0.96; 95% confidence interval [CI] 0.78-1.18); cohort studies showed that cancer risk was higher in patients using SUs than metformin (relative risk 1.60 [95%CI 1.37-1.87]; adjusted hazard ratio 1.13 [95%CI 1.06-1.19]), and case-control studies suggested a trend for increased cancer risk in those using SUs compared with non-SU users (adjusted OR 1.13; 95%CI 0.93-1.37).ConclusionsThe available evidence clearly shows that SUs can significantly increase the risk of cancer compared with metformin. Although the evidence suggests the possibility that SU users may have a higher risk of cancer than those using alternative medications in addition to metformin, it remains inadequate to enable definitive conclusions to be drawn.

Abstract:Aims: The utilization of sulfonylurea (SU) for the management of Type 2 Diabetes Mellitus (T2DM) has witnessed a decline, attributed to the rising popularity of alternative medications and uncertainties surrounding the cardiovascular risk profile of SUs. This study aimed to investigate the potential association between SU intake and the incidence of cardiovascular events in patients with T2DM. Methods: A retrospective cohort study, based on a general practice (GP) registry, was designed, encompassing patients diagnosed with T2DM between 2005 and 2014.Follow-up persisted until the occurrence of a cardiovascular event, loss to follow-up, or until December 31, 2022. Comparative analyses were conducted between patients, receiving SU treatment and those without RESULTS: Data from a cohort comprising 5589 patients revealed that 13 % and 13.1 % of individuals in the comparator group and the SU group, respectively, experienced a cardiovascular event. However, no statistically significant elevation in the risk of cardiovascular events was observed after SU usage. Furthermore, the glycated haemoglobin (HbA1c) levels were significantly higher in the SU group (7.0 % vs. 6.4 %, p < 0.001). Conclusions: The findings from this study indicate that the use of sulfonylureas SUs is not associated with a statistically significant increase in the risk of cardiovascular events among patients with type T2DM. These results contribute to the ongoing discourse on the safety and efficacy of SU therapy in diabetes management.

Abstract:BackgroundDipeptidyl peptidase-IV (DPP-4) inhibitors and sulfonylureas are two important second-line anti-diabetic agents. The objective of this research was to evaluate the efficacy and safety of DPP-4 inhibitors compared with sulfonylureas by meta-analytic approach of available randomized studies. MethodsWe searched MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials databases up to 30 June 2013, collecting all randomized clinical trials with a treatment duration of 18weeks. Data on glycated haemoglobin (HbA(1c)), body weight, hypoglycaemia, total adverse events, and cardiovascular events were retrieved and analysed. ResultsThe analysis included 12 randomized studies comprising 10982 patients with type 2 diabetes mellitus. On the basis of meta-analysis, sulfonylureas lowered HbA(1c) significantly more than DPP-4 inhibitors with weighted mean difference (WMD) of 0.105 [95% confidence interval (CI) 0.103 to 0.107]. The results were consistent in trials with longer (>32weeks) or shorter (32weeks) duration; however, DPP-4 inhibitors showed greater reduction in HbA(1c) compared with the second-generation sulfonylureas and in patients with baseline eGFR<50mL/min/1.73m(2). Patients treated with DPP-4 inhibitors are less likely to achieve HbA(1c)<7% compared with sulfonylureas [Mantel-Haenszel odds ratio (MH-OR) 0.91; 95% CI 0.84 to 0.99]. DPP-4 xinhibitors were associated with a reduction in body weight (WMD -1.652; 95% CI -1.658 to -1.646) and lower risk of hypoglycaemia (MH-OR, 0.13; 95% CI 0.11 to 0.16), total adverse events (MH-OR, 0.79; 95% CI 0.72 to 0.87), and cardiovascular events (MH-OR, 0.53; 95% CI 0.32 to 0.87) compared with sulfonylureas. ConclusionAlthough DPP-4 inhibitors are less efficacious compared with sulfonylureas, they demonstrate a beneficial effect on body weight, episodes of hypoglycaemia, and total adverse events. Abstract:BACKGROUND This study aimed to investigate the patterns of use of antidiabetic medication among patients with newly diagnosed type 2 diabetes mellitus (T2DM), focusing on the comparison in glycemic control between sulfonylureas and metformin. MATERIAL AND METHODS Data from patients newly diagnosed and treated for T2DM between 2011 and 2014, who were ³18 years of age were obtained from the Yinzhou Regional Health Care Database, and patterns of medication and glycemic control were analyzed. The Poisson probability distribution was used to determine the rate ratio (incidence density ratio) of uncontrolled hyperglycemia between sulfonylureas and metformin. Cox regression analysis was used to determine the association between initial treatment with sulfonylureas and metformin and the requirement for additional medications. RESULTS Of the 4,017 patients included in the study, 33.58% began treatment with sulfonylureas and 20.41% began treatment with metformin, and during follow-up, 21.13% and 22.68%, respectively were treated with a second drug. After adjustment for body mass index (BMI) and fasting blood glucose (FBG), the rate ratio of uncontrolled blood glucose for sulfonylurea monotherapy compared with metformin monotherapy was 1.30 (95% CI, 1.17-1.45). Patients who began treatment with sulfonylureas were 18% less likely to progress to dual medication compared with metformin (HR=0.82; 95% CI, 0.68-0.99), CONCLUSIONS Sulfonylurea monotherapy was the most common initial treatment for patients with newly diagnosed T2DM and was associated with an increased risk of uncontrolled hyperglycemia, but patients were less likely to receive additional drugs when compared with patients initially treated with metformin monotherapy.

Abstract:Aims To examine whether the cardiovascular effects of glucagon-like peptide-1 (GLP-1) receptor agonists are attenuated by concurrent sulfonylurea (SU) therapy in a post-hoc analysis of the Exenatide Study of Cardiovascular Event Lowering (EXSCEL). Methods We investigated whether SUs, as a class or by specific type, modulated the effects of once-weekly exenatide (EQW) on EXSCEL cardiovascular outcomes in intent-to-treat analyses of all trial participants, categorized as SU users or nonusers. Marginal structural models were used to evaluate whether there were differential EQW effects by SU category on major adverse cardiovascular events (MACE), depending on duration of SU use (6, 12, and 18 months). EQW-by-SU type interaction p-values and hazard ratios (95 % CIs) for EQW versus placebo for each baseline SU type (glibenclamide, gliclazide, glimepiride, other SUs) were calculated. Results Neither SU use nor baseline SU type modified the effect of EQW on time to MACE (p interaction = 0.88 and 0.78, respectively), nor did individual SU types, including glibenclamide (a systemically wide-acting SU). Conclusions SUs did not modulate the effect of EQW on cardiovascular outcomes, suggesting that SU treatment choices need not be altered to optimize the cardiovascular effects of GLP-1 receptor agonists in people with type 2 diabetes.

Predictive Factors of Primary and Secondary Sulfonylurea Failure in Type 2 Diabetic Subjects

Abstract:BackgroundThe aim of the present study was to investigate the value of clinical characteristics in predicting sulfonylurea (SU) treatment responses. MethodsAll 747 subjects in the present study were from the Xiaoke Pill Clinical Trial evaluating the safety and efficacy of SU. Subjects had the same initial dose of glibenclamide and drug doses were adjusted every 4weeks during the 48-week follow-up period. Primary SU failure was defined as a <10% reduction in fasting plasma glucose (FPG) at the end of Week 4 from randomization, whereas secondary SU failure was defined as a consecutive confirmed level of FPG >7mmol/L. ResultsKaplan-Meier survival analysis revealed that lower baseline basal disposition index (DI (b)) and higher FPG predict secondary SU failure. Inversely, higher baseline DI (b) and lower FPG predict primary SU failure. Primary SU failure can predict secondary SU failure. The combination of lower baseline DI (b) with primary SU failure is a better predictor of secondary SU failure than lower baseline DI (b) or primary SU failure alone. ConclusionsPatients with good glycemic control and higher -cell reserve at entry are more likely to experience primary SU failure but are less likely to experience secondary SU failure. By combining baseline DI (b) with initial response at the first month of treatment, we can get quick information about the long-term efficacy of SU treatment.

Abstract:AimsThe present study aims to determine the effects of sodium-glucose cotransporter 2 (SGLT-2) inhibitors on the serum uric acid (SUA) levels of patients with type 2 diabetes mellitus (T2DM) in Asia. MethodsPubMed, CENTRAL, Embase and Cochrane Library databases were searched for randomized controlled trials (RCTs) of SGLT-2 inhibitors in patients with T2DM up to July 15, 2021, without language or date restrictions. ResultsA total of 19 studies (4218 participants) with high quality were included in the present network meta-analysis. All of the included SGLT-2 inhibitors (empagliflozin, dapagliflozin, canagliflozin, ipragliflozin, luseogliflozin and tofogliflozin) significantly decreased SUA levels compared with those of the control (total standard mean difference [SMD] -0.965, 95% CI [−1.029, −0.901], P = 0.000, I2 = 98.7%) in patients with T2DM. Subgroup analysis and meta-regression showed that the combined analysis of different inhibitors may lead to heterogeneity of the results. Therefore, among the SGLT-2 inhibitors, the results of the subsequent network meta-analysis revealed that luseogliflozin and dapagliflozin ranked the highest in terms of lowering SUA level among the SGLT-2 inhibitors. Moreover, the network meta-analysis declared that luseogliflozin (1 mg and 10 mg) and dapagliflozin (5 mg) led to a superior reduction in SUA in patients with T2DM. ConclusionsSGLT-2 inhibitors could significantly reduce SUA levels in patients with T2DM, especially luseogliflozin (1 mg and 10 mg) and dapagliflozin (5 mg) possess the best effects. Therefore, SGLT-2 inhibitors look extremely promising as an anti-diabetes treatment option in patients with T2DM with high SUA.This article is protected by copyright. All rights reserved.

Abstract:Antidiabetic sulfonylureas is widely used in clinical therapy of type 2 diabetes recently.The effects are mainly mediated by closure of ATP-sensitive K+(KATP)channels after binding of sulfonylureas to their receptor,SUR1,which is a constitutive part of the KATP channel as well as KIR6.2.This results in membrane depolarization that culminates in Ca2+ entry through voltage-gated L-type Ca2+ channels and the onset of Ca2+-dependent insulin release.But at the same time,too much calcium coming into the cells will induce the overloaded calcium.High concentration of reactive oxygen species were produced during the activation of islet beta cells.The overloaded calcium and excessive reactive oxygen species will hurt and induce the apoptosis of islet cells.A number of reports declared that sulfonylureas have effects on apoptosis of islet cells,but they have different mechanisms.In fact,the mechanism of different kinds of sulfonylurea′s effect on islet cells apoptosis has not been elucidated.There still have many debates about it.Recent researches have showed that new formulation of sulfonylurea,such as glimepiride,may have different effect on islet cells apoptosis.
Pharmacologic Differences of Sulfonylureas and the Risk of Adverse Cardiovascular and Hypoglycemic Events

Abstract:OBJECTIVE Sulfonylureas have been associated with an increased risk of cardiovascular adverse events and hypoglycemia, but it is unclear if these risks vary with different agents. We assessed whether the risks of acute myocardial infarction, ischemic stroke, cardiovascular death, all-cause mortality, and severe hypoglycemia differ between sulfonylureas grouped according to pancreas specificity and duration of action. RESEARCH DESIGN AND METHODS Using the U.K. Clinical Practice Research Datalink, linked with the Hospital Episodes Statistics and the Office for National Statistics databases, we conducted a cohort study among patients with type 2 diabetes initiating monotherapy with sulfonylureas between 1998 and 2013. Adjusted hazard ratios (HRs) and 95% CIs were estimated using Cox proportional hazards models, comparing use of pancreas-nonspecific, long-acting sulfonylureas (glyburide/glimepiride) to pancreas-specific, short-acting sulfonylureas (gliclazide/glipizide/tolbutamide). RESULTS The cohort included 17,604 sulfonylurea initiators (mean [SD] follow-up 1.2 [1.5] years). Compared with specific, short-acting sulfonylureas (15,741 initiators), nonspecific, long-acting sulfonylureas (1,863 initiators) were not associated with an increased risk of acute myocardial infarction (HR 0.86; CI 0.55–1.34), ischemic stroke (HR 0.92; CI 0.59–1.45), cardiovascular death (HR 1.01; CI 0.72–1.40), or all-cause mortality (HR 0.81; CI 0.66–1.003), but with an increased risk of severe hypoglycemia (HR 2.83; CI 1.64–4.88). CONCLUSIONS The nonspecific, long-acting sulfonylureas glyburide and glimepiride do not have an increased risk of cardiovascular adverse events compared with the specific, short-acting sulfonylureas gliclazide, glipizide, and tolbutamide. However, nonspecific, long-acting sulfonylureas glyburide and glimepiride have an increased risk of severe hypoglycemia.

Abstract:There is a universal agreement that when lifestyle modifications are inadequate to control the hyperglycemia, metformin should be considered the first line of pharmacological therapy. However, when metformin monotherapy fails, there is no consensus as to which drug should be added. Many new classes of drugs that are currently available including DPP-4 inhibitors, GLP-1 agonists and SGLT-2 inhibitors have undergone cardiovascular outcome trials which had been made mandatory by the regulatory authority and thereby established their cardiovascular safety or at least neutrality. Though sulfonylureas are one of the widely prescribed drugs both in developed and developing countries and have proven their efficacy for glycemic control and prevention of microvascular complications, there is considerable uncertainty about its cardiovascular safety which has been going on for nearly five decades. In this review, we will critically analyze the efficacy and cardiovascular safety of sulphonylureas, based on the latest available literature to clarify their role in our day-to-day clinical practice.
Comparative Efficacy and Safety of Glucose-Lowering Drugs as Monotherapy for Type 2 Diabetes: A Systematic Review and Indirect Treatment Comparison

Abstract:Background: There are multiple alternative glucose-lowering drugs available as initial monotherapy for patients with type 2 diabetes (T2DM) with contraindications or intolerance of metformin. However, little systematic and comparative data is available for them as initial monotherapy. We compared the efficacy and safety of them as monotherapy for T2DM. Methods: In this systematic review and indirect treatment comparison, PubMed, Embase, Web of Science, CNKI, WanFang Data, and Chongqing VIP were searched for randomized controlled trials (1990-2020) comparing a glucose-lowering drug monotherapy with placebo (or lifestyle-intervention) in adult T2DM patients. The drug classes included metformin, SUs, TZDs, glinides, AGIs, DPP-4is, SGLT2is, GLP-1RAs, and insulins. The drugs were firstly compared with placebo/lifestyle-intervention using meta-analysis, then compared against each other using indirect treatment comparison. Weighted mean difference with 95% CIs were calculated for HbA1c, FPG, BMI, TC, HDL-C, and SBP. Relative risk with 95% CIs were calculated for hypoglycemia. This study is registered with PROSPERO (CRD42020170769). Findings: 185 studies assessing 23 drugs from eight drug classes were included (no eligible study was found for insulin). SUs were most efficacious in reducing HbA1c and FPG but increased hypoglycemic risks. Metformin was most efficacious in lowering BMI. GLP-1RAs were most efficacious in reducing TC. TZDs were most efficacious in increasing HDL-C. SGLT2is were most efficacious in lowering SBP. Overall, GLP-1RAs comprehensively controlled HbA1c, FPG, BMI, TC, and SBP, whilst metformin controlled HbA1c, FPG, BMI, TC, and HDL-C, and both of them did not increase hypoglycemic risks. Our observations also showed that although drug class is important, drugs within the same class may display important differences in treatment effects. Interpretation: Our results, along with individualized patient characteristics, preferences, and clinical needs, should be considered when selecting a drug as monotherapy for T2DM in clinical practice. Comparison of Glucose-Lowering Drugs As Second-Line Treatment for Type 2 Diabetes: A Systematic Review and Meta-Analysis

Abstract:Background: Multiple glucose-lowering drugs are available as add-ons to metformin for a second-line treatment for type 2 diabetes. However, no systematic and comparative data are available for them in China. We aimed to compare the effects of glucose-lowering drugs added to metformin in China. Methods: PubMed, Embase, Web of Science, CNKI, WanFang Data, and Chongqing VIP from 1 January 2000 until 31 December 2020 were systematically searched for randomized controlled trials comparing a glucose-lowering drug added to metformin with metformin in Chinese type 2 diabetes patients. Drug classes included sulfonylureas (SUs), glinides (NIDEs), thiazolidinediones (TZDs), α-glucosidase inhibitors (AGIs), dipeptidyl peptidase-4 inhibitors (DPP-4is), sodium-glucose cotransporter-2 inhibitors (SGLT2is), glucagon-like peptide-1 receptor agonists (GLP-1RAs), and insulins (INSs). Two reviewers independently screened studies, extracted data, and appraised the risk of bias. Results: 315 trials were included. In patients receiving metformin alone, the addition of NIDEs produced the greatest additional HbA1c reductions (1.29%; 95% CI 0.97, 1.60); while INSs yielded both the largest additional FPG reductions (1.58 mmol/L; 95% CI 1.22, 1.94) and 2 hPG reductions (2.52 mmol/L; 95% CI 1.83, 3.20). INS add-ons also conferred the largest additional HDL-C increases (0.40 mmol/L; 95% CI 0.16, 0.64), whereas AGI add-ons generated the greatest TC reductions (1.08 mmol/L; 95% CI 0.78, 1.37). The greatest incremental SBP reductions (6.65 mmHg; 95% CI 4.13, 9.18) were evident with SGLT2i add-ons. GLP-1RA add-ons had the greatest BMI reductions (1.96 kg/m2, 95% CI 1.57, 2.36), meanwhile with the lowest (0.54 time) hypoglycemia risk. Overall, only the GLP-1RA add-ons demonstrated a comprehensive beneficial effect on all outcomes. Furthermore, our results corroborated intraclass differences among therapies. Given the limited evidence, we could not reach a conclusion about the optimal therapies regarding mortality and vascular outcomes. Conclusion: The results suggested a potential treatment hierarchy for clinicians and patients, with the GLP-1RA add-ons being most preferred based on their favorable efficacy and safety profiles; and provided a unified hierarchy of evidence for conducting country-specific cost-effectiveness analyses.

Abstract:Purpose Sodium glucose cotransporter 2 (SGLT2) inhibitors are shown to cause small, but significant changes of lipid profiles, we aim to investigate whether such altered lipid profiles can be translated into clinically meaningful changes in dyslipidemia. Methods PubMed, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL) were searched for randomized controlled trials (RCTs) that compared SGLT2 inhibitors with placebo or other oral glucose-lowering drugs in patients with type 2 diabetes mellitus and reported the events of dyslipidemia. A random-effect meta-analysis was performed to calculate the pooled estimates with risk ratio (RR) for dyslipidemia risk and weighted mean difference for lipid profiles with their 95% confidential intervals (CIs). Results Of 2427 studies identified, 15 RCTs involving 7578 patients were included. This meta-analysis found no association between SGLT2 inhibitors and risk of dyslipidemia (RR: 1.13; 95% CI: 0.91-1.40). However, SGLT2 inhibitors were significantly associated with increases in total cholesterol by 0.15 mmol/L, low-density lipoprotein cholesterol by 0.12 mmol/L, and high-density lipoprotein cholesterol by 0.07 mmol/L while they can significantly decrease triglycerides by -0.12 mmol/L compared to controls. Conclusions SGLT2 inhibitors were not associated with increased risk of dyslipidemia. Further trials with longitudinal assessment are needed to assess the effect of SGLT2 inhibitors on trajectories of changes of lipid metabolism.
