Abstract:Diffuse optical tomography (DOT) is emerging as a noninvasive functional imaging method for breast cancer diagnosis and neoadjuvant chemotherapy monitoring. In particular, the multimodal approach of combining DOT with x-ray digital breast tomosynthesis (DBT) is especially synergistic as DBT prior information can be used to enhance the DOT reconstruction. DOT, in turn, provides a functional information overlay onto the mammographic images, increasing sensitivity and specificity to cancer pathology. We describe a dynamic DOT apparatus designed for tight integration with commercial DBT scanners and providing a fast (up to 1 Hz) image acquisition rate to enable tracking hemodynamic changes induced by the mammographic breast compression. The system integrates 96 continuous-wave and 24 frequency-domain source locations as well as 32 continuous wave and 20 frequency-domain detection locations into low-profile plastic plates that can easily mate to the DBT compression paddle and x-ray detector cover, respectively. We demonstrate system performance using static and dynamic tissue-like phantoms as well as in vivo images acquired from the pool of patients recalled for breast biopsies at the Massachusetts General Hospital Breast Imaging Division.

Abstract:We present a new method to simultaneously reconstruct the images of oxyhemoglobin, deoxyhemoglobin, and water concentrations, as well as the volume fraction images of the scattering particles using continuous wave multispectral diffuse optical tomography with the absorption and scattering spectral prior constraints. In this method, the nonlinear relationship between the reduced scattering coefficient and the volume fraction and the size of the particles is linearized, allowing direct reconstruction of the volume fraction of scattering particles in tissues. The method is validated by a series of numerical simulations, phantom experiments, and in vivo clinical experiments. The initial clinical results indicate that the volume fraction of scattering particles in a malignant tumor is higher than that in a benign tumor. (c) 2007 Optical Society of America.

Abstract:We present a two-step reconstruction method that can qualitatively and quantitatively improve the reconstruction of tissue refractive index (RI) distribution by phase-contrast diffuse optical tomography (PCDOT). In this two-step method, we first recover the distribution of tissue absorption and scattering coefficients by conventional diffuse optical tomography to obtain the geometrical information of lesions, allowing the incorporation of geometrical information as a priori in the PCDOT reconstruction using a locally refined mesh. The method is validated by a series of phantom experiments and evaluated using in vivo data from 42 human subjects. The results demonstrate clear contrast of RI between the lesion and the surroundings, making the image interpretation straightforward. The sensitivity and specificity from these 42 cases are both 81% when RI is used as an imaging parameter for distinguishing between malignant and benign lesions.

Abstract:Diffuse optical tomography (DOT) is an ongoing medical imaging modality in which tissue is illuminated by near-infrared light from an array of sources, the multiply-scattered light which emerges is observed with an array of detectors, and then a model of the propagation physics is used to infer the localized optical properties of the illuminated tissue. The three primary absorbers at these wavelengths, water and both oxygenated and deoxygenated hemoglobin, all have relatively weak absorption. This fortuitous fact provides a spectral window through which we can attempt to localize absorption (primarily by the two forms of hemoglobin) and scattering in the tissue. The most important current applications of DOT are detecting tumors in the breast and imaging the brain. We introduce the basic idea of DOT and review the history of optical methods in medicine as relevant to the development of DOT. We then detail the concept of DOT, including a review of the tissue's optical properties, modes of operation for DOT, and the challenges which the development of DOT must overcome. The basics of modelling the DOT forward problem and some critical issues among the numerous implementations that have been investigated for the DOT inverse problem, with an emphasis on signal processing. We summarize with some specific results as examples of the current state of DOT research.

Abstract:Diffuse optical breast imaging utilizes near-infrared (NIR) light propagation through tissues to assess the optical properties of tissues for the identification of abnormal tissue. This optical imaging approach is sensitive, cost-effective, and does not involve any ionizing radiation. However, the image reconstruction of diffuse optical tomography (DOT) is a nonlinear inverse problem and suffers from severe illposedness due to data noise, NIR light scattering, and measurement incompleteness. An image reconstruction method is proposed for the detection of breast cancer. This method splits the image reconstruction problem into the localization of abnormal tissues and quantification of absorption variations. The localization of abnormal tissues is performed based on a well-posed optimization model, which can be solved via a differential evolution optimization method to achieve a stable reconstruction. The quantification of abnormal absorption is then determined in localized regions of relatively small extents, in which a potential tumor might be. Consequently, the number of unknown absorption variables can be greatly reduced to overcome the underdetermined nature of DOT. Numerical simulation experiments are performed to verify merits of the proposed method, and the results show that the image reconstruction method is stable and accurate for the identification of abnormal tissues, and robust against the measurement noise of data.

Abstract:This article presents a pilot study of multispectral diffuse optical tomography for noninvasively imaging volume fraction and mean size of cellular scattering components in the breast. Cellular morphology images for a total of 14 cases (four malignant breast and ten benign lesions) were obtained. Analyzing the images based on the pathological findings of the cases studied, we found that light scattering in the breast was contributed from both the nucleus and organelles such as mitochondria and nucleolus. Based on the image analysis of these 14 cases, we found that the differences in the mean size and volume fraction between the malignant and benign lesions are significant. The contrast ratio of the average mean size and volume fraction between malignant and benign lesions were calculated to be 3.38 and 2.63. These initial results suggest that cellular mean size and volume fraction may be two new criteria that could be used to differentiate malignant from benign lesions.

Abstract:Diffuse optical imaging (DOI) is a noninvasive optical technique that employs near-infrared (NIR) light to quantitatively characterize the optical properties of thick tissues. Although NIR methods were first applied to breast transillumination (also called diaphanography) nearly 80 years ago, quantitative DOI methods employing time- or frequency-domain photon migration technologies have only recently been used for breast imaging (i.e., since the mid-1990s). In this review, the state of the art in DOI for breast cancer is outlined and a multi-institutional Network for Translational Research in Optical Imaging (NTROI) is described, which has been formed by the National Cancer Institute to advance diffuse optical spectroscopy and imaging (DOSI) for the purpose of improving breast cancer detection and clinical management. DOSI employs broadband technology both in near-infrared spectral and temporal signal domains in order to separate absorption from scattering and quantify uptake of multiple molecular probes based on absorption or fluorescence contrast. Additional dimensionality in the data is provided by integrating and co-registering the functional information of DOSI with x-ray mammography and magnetic resonance imaging (MRI), which provide structural information or vascular flow information, respectively. Factors affecting DOSI performance, such as intrinsic and extrinsic contrast mechanisms, quantitation of biochemical components, image formation/visualization, and multimodality co-registration are under investigation in the ongoing research NTROI sites. One of the goals is to develop standardized DOSI platforms that can be used as stand-alone devices or in conjunction with MRI, mammography, or ultrasound. This broad-based, multidisciplinary effort is expected to provide new insight regarding the origins of breast disease and practical approaches for addressing several key challenges in breast cancer, including: Detecting disease in mammographically dense tissue, distinguishing between malignant and benign lesions, and understanding the impact of neoadjuvant chemotherapies.

Abstract:Multispectral and phase-contrast diffuse optical tomography are used to track treatment progress in a patient with locally advanced invasive carcinoma of the breast cancer during neoadjuvant chemotherapy. Two types of chemotherapy treatment including four cycles of Adriamycin/Cytoxin (AC cycles) and twelve cycles of Taxol/Herceptin (TH cycles) were applied to patient. A total of eight optical exams were performed before and within the chemotherapy. Images of tissue refractive index, and absorption and scattering coefficients, as well as oxy-hemoglobin and deoxy-hemoglobin concentrations along with scattering particle volume fraction and mean diameter of cellular components were all obtained. The tumor was identified through absorption and scattering images. Tumor shrinkage was observed during the course of chemotherapy from all the optical images. Our results show that oxy-hemoglobin, deoxy-hemoglobin and total hemoglobin in tumor decreased after chemotherapy compared to that of before chemotherapy. Significant changes in tumor refractive index along with tumor cellular morphology during the entire chemotherapy are also observed.

Abstract:Rationale and Objectives. Near-infrared diffuse optical tomography (DOT) is an emerging imaging technology that has the potential to offer enhanced contrast resolution over the existing technologies for detection and diagnosis of breast cancer. Thus far, the clinical evaluation of DOT has been largely limited to solid tumors. A pilot clinical study focused on DOT imaging of breasts with cysts is presented.Materials and Methods. Six cases were studied using the recently developed compact, parallel-detection DOT system. Images characterizing the tissue absorption and scattering were obtained with a finite element-based reconstruction algorithm. The optical images were compared with the mammograms and sonograms. In one case, in vitro measurements of optical properties were conducted for the fluid obtained from needle aspiration.Results. Substantial contrast between cyst and adjacent parenchyma is observed. For the six cases evaluated, the locations and sizes of cysts imaged optically are accurate and consistent with the mammographic and sonographic findings. For the case that aspiration was performed, the absorption and scattering coefficients imaged in the cyst region are quantitatively accurate compared with that measured in vitro from the fluid aspirated.Conclusion. This pilot study shows that cysts ranging from 1-4 cm in diameter can be quantitatively imaged. They can be differentiated from solid breast tumors because cysts generally demonstrate lower absorption and scattering coefficients compared with the surrounding normal tissue, whereas solid tumors show concurrent higher absorption and scattering related to the normal tissue.

Abstract:Diffuse optical tomography (DOT) has been investigated as an alternative imaging modality for breast cancer detection thanks to its excellent contrast to hemoglobin oxidization level. However, due to the complicated non-linear photon scattering physics and ill-posedness, the conventional reconstruction algorithms are sensitive to imaging parameters such as boundary conditions. To address this, here we propose a novel deep learning approach that learns non-linear photon scattering physics and obtains an accurate three dimensional (3D) distribution of optical anomalies. In contrast to the traditional black-box deep learning approaches, our deep network is designed to invert the Lippman-Schwinger integral equation using the recent mathematical theory of deep convolutional framelets. As an example of clinical relevance, we applied the method to our prototype DOT system. We show that our deep neural network, trained with only simulation data, can accurately recover the location of anomalies within biomimetic phantoms and live animals without the use of an exogenous contrast agent.

Abstract:A multi-modal optical imaging system for quantitative 3D bioluminescence and functional diffuse imaging is presented, which has no moving parts and uses mirrors to provide multi-view tomographic data for image reconstruction. It is demonstrated that through the use of trans-illuminated spectral near infrared measurements and spectrally constrained tomographic reconstruction, recovered concentrations of absorbing agents can be used as prior knowledge for bioluminescence imaging within the visible spectrum. Additionally, the first use of a recently developed multi-view optical surface capture technique is shown and its application to model-based image reconstruction and free-space light modelling is demonstrated. The benefits of model-based tomographic image recovery as compared to 2D planar imaging are highlighted in a number of scenarios where the internal luminescence source is not visible or is confounding in 2D images. The results presented show that the luminescence tomographic imaging method produces 3D reconstructions of individual light sources within a mouse-sized solid phantom that are accurately localised to within 1.5mm for a range of target locations and depths indicating sensitivity and accurate imaging throughout the phantom volume. Additionally the total reconstructed luminescence source intensity is consistent to within 15% which is a dramatic improvement upon standard bioluminescence imaging. Finally, results from a heterogeneous phantom with an absorbing anomaly are presented demonstrating the use and benefits of a multi-view, spectrally constrained coupled imaging system that provides accurate 3D luminescence images.

Abstract:SignificanceFrequency-domain diffuse optical tomography (FD-DOT) could enhance clinical breast tumor characterization. However, conventional diffuse optical tomography (DOT) image reconstruction algorithms require case-by-case expert tuning and are too computationally intensive to provide feedback during a scan. Deep learning (DL) algorithms front-load computational and tuning costs, enabling high-speed, high-fidelity FD-DOT.AimWe aim to demonstrate a simultaneous reconstruction of three-dimensional absorption and reduced scattering coefficients using DL-FD-DOT, with a view toward real-time imaging with a handheld probe.ApproachA DL model was trained to solve the DOT inverse problem using a realistically simulated FD-DOT dataset emulating a handheld probe for human breast imaging and tested using both synthetic and experimental data.ResultsOver a test set of 300 simulated tissue phantoms for absorption and scattering reconstructions, the DL-DOT model reduced the root mean square error by 12%±40% and 23%±40%, increased the spatial similarity by 17%±17% and 9%±15%, increased the anomaly contrast accuracy by 9%±9% (μa), and reduced the crosstalk by 5%±18% and 7%±11%, respectively, compared with model-based tomography. The average reconstruction time was reduced from 3.8 min to 0.02 s for a single reconstruction. The model was successfully verified using two tumor-emulating optical phantoms.ConclusionsThere is clinical potential for real-time functional imaging of human breast tissue using DL and FD-DOT.

Abstract:Rationale and Objectives. We sought to investigate the utility of phase-contrast diffuse optical tomography (PCDOT) for differentiation of malignant and benign breast masses in humans and to compare PCDOT with conventional diffuse optical tomography (DOT) for analysis of breast masses in humans.Materials and Methods. Thirty-five breast masses were imaged in 33 patients (mean age, 51 years; range, 22-80) using PCDOT. Images characterizing the tissue refractive index, and absorption and scattering coefficients of breast masses were obtained with a finite element-based reconstruction algorithm. Theses images were then analyzed and compared with the biopsy/pathology results for all the cases examined.Results. Malignant lesions tended to have a decreased refractive index, allowing them to be discriminated from benign lesions in most cases, whereas absorption and scattering images were unable to accurately discriminate benign from malignant lesions. The sensitivity, specificity, false-positive value, and overall accuracy for refractive index imaging were 81.8%, 70.8%, 29.2%, and 74.3%, respectively. The accuracy of refractive index imaging increases with increasing patient age.Conclusion. Refractive index is a new parameter for optical imaging that may be helpful in differentiating between malignant and benign masses in the breast.
In Vivo Studies of 144 Breast Masses by Phase Contrast Diffuse Optical Tomography

Abstract:We report our continued study of phase-contrast diffuse optical tomography (PCCDOT) for evaluating its fidelity in distinguishing malignant breast lesions form benign ones. 144 breast masses were examined from 134 patients, aging from 22~82 with the mean age of 56. Tissue optical parameters including refractive index, and absorption and scattering coefficients, were obtained and compared with their corresponding biopsy/pathology reports. In consistent with our previous study, malignant masses tended to have a decreased refractive index relative to their surrounding normal tissue, which acts as the key character to differentiate them from benign masses. The results show that the specificity is improved significantly over the previous smaller scale study (85% vs. 70%) due to the addition of significantly more benign cases, while the sensitivity stays about the same (81% vs. 82%) due to the similar number of malignant cases used compared to the smaller scale study.

Abstract:Multimodal imaging platforms offer a vast array of tissue information in a single image acquisition by combining complementary imaging techniques. By merging different systems, better tissue characterization can be achieved than is possible by the constituent imaging modalities alone. The combination of optical coherence tomography (OCT) with non-linear optical imaging (NLOI) techniques such as two-photon excited fluorescence (TPEF), second harmonic generation (SHG) and coherent anti-Stokes Raman scattering (CARS) provides access to detailed information of tissue structure and molecular composition in a fast, label-free and non-invasive manner. We introduce a multimodal label-free approach for morpho-molecular imaging and spectroscopy and validate the system in mouse skin demonstrating the potential of the system for colocalized acquisition of OCT and NLOI signals.
