Abstract:Objective To investigate the effect of different doses of rosuvastatin on serum high-sensitivity C-reactive protein( hs-CRP)and interleukin-6( IL-6) in patients with acute coronary syndrome(ACS).Methods 72 ACS patients were randomized into high dose group( n= 36,rosuvastatin was 20mg/d based routine therapy) and low dose group( n= 36,rosuvastatin was 10mg/d based routine therapy).Each group was given the same basic treatments,and taken the different doses of rosuvastatin for 4 weeks.Levels of blood lipids,serum hs-CRP and IL-6 were measured before and after the treatment; adverse reaction was monitored at the same time.Results Levels of total cholesterol( TC),low density lipoprotein C( LDL-C),serum h-s CRP and IL-6 in two groups were significantly decreased before and after the treatment( P< 0.01),while high density lipoprotein C( HDL-C) was significantly rose( P < 0.05).After the treatment,levels of serum h-s CRP and IL-6 decreased more significantly in the high dose group than those in the low dose group( P < 0.01),but no significant difference was found in the adverse reaction( P < 0.05).Conclusions Rosuvastatin,especially high dose,may reduce the levels of serum hs-CRP and IL-6 in ACS patients in the early time,without increasing adverse reaction.

Abstract:Although several studies have suggested that intensive statin pretreatment could reduce the incidence of procedure-related myocardial infarction in western population, the data on the effect in Asian patients have been still limited. The aim of the study was to investigate the efficacy and safety of intensive atorvastatin load in Chinese patients undergoing elective PCI. A total of 1202 patients with stable angina or non-ST-segment elevation acute coronary syndrome (NSTE-ACS) scheduled to undergo PCI received either intensive statin treatment (80 mg atorvastatin daily x 2 days before PCI and 40 mg daily x 30 days after PCI) or usual care. The primary endpoint was incidence of major adverse cardiac events (cardiac death, myocardial infarction, or unexpected target vessel revascularization) within 30 days after PCI. Safety endpoints include the incidence of contrast induced nephropathy (CIN), ALT/AST >3 upper limit of normal (ULN), CK >5 ULN. The incidence of 30-day MACE did not significantly differ between the intensive group and control group (19.4 vs 18.3%, P = 0.63). Multivariate analysis revealed age (OR = 1.024, 95% CI 1.003-1.045, P = 0.023) and total stent length as an independent predictor of 30-day MACE (OR = 1.012, 95% CI 1.007-1.018, P < 0.0001). The incidence of CIN was comparable between intensive group and control group (4.09 vs 4.39%, P = 0.795). No significant differences were observed in other safety profile at all follow-ups between treatment groups. The ISCAP trial demonstrated that serial intensive atorvastatin therapy did not improve the clinical outcome with similar safety profile comparing with usual care among Chinese patients undergoing elective PCI.

Abstract:East Asians are the most populous race in the world and their health status is an important global issue. Compared with Caucasian populations, East Asian patients have a different benefit/risk ratio when using antithrombotic treatment. Despite this observation, treatment strategies in East Asian patients are mostly based on the American and European guidelines. Despite a lower platelet inhibitory response to clopidogrel, East Asian patients show a similar or even a lower rate of ischemic event occurrence and higher bleeding risk compared with Caucasian patients. For potent P2Y12 inhibitors (ticagrelor and prasugrel), East Asian patients have shown less favorable net clinical benefits compared with Caucasian patients, which may be related to differences in pharmacokinetic/pharmacodynamic profiles and therapeutic zone of antiplatelet effect. This updated consensus mainly focuses on state-of-the-art and current controversies in the East Asian population. In addition, when East Asian patients are administered potent P2Y12 receptor inhibitors, the strategies and ongoing trials to overcome the related hurdles are discussed.
Efficacy and Safety of Short-Term Intensive Statin Therapy in Patients with Acute Coronary Syndrome

Abstract:Objective To evaluate the efficacy and safety of short- term high doses of atorvastatin intensive treatment for patients with acute coronary syndrome (ACS) during hospitalization. Methods A total of 355 ACS patients who were treated in our hospital were enrol ed into this study. Among them 180 cases received atorvastatin 80mg at admission and atorvastatin 80mg/night during the hospitalization (intensive group), and other 175 cases received atorvastatin 20mg/night) from admission (convention group). The serum levels ALT, AST, Cr, TG, TC, HDL- C, LDL- C, creatine kinase (CK) and creatine kinase isoenzyme (CK- MB) at admission and 1 week after admission were measured in both groups. Results The TC, TG and LDL- C levels were significantly lower and HDL- C levels were significantly higher at 1 week after treatment than those at baseline levels in both groups (al P<0.01);while The TG levels in intensive group were significantly higher than those in conventional group 1 week after treatment (P<0.05). The ALT, AST, Cr, CK and CK- MB levels were improved significantly in both groups 1 week after treatment (P<0.01);while compared with the conventional group, the ALT and AST levels in intensive group were markedly increased (P<0.01). Conclusion The short- term intensive statin therapy is as safe as the conventional treatment, but is more effective for pa-tients with acute coronary syndrome.
Clinical Analysis of 373 Patients with Non-ST Elevation Acute Coronary Syndrome in a Teaching Hospital

Abstract:Aim To analysis of patients with non-ST elevation acute coronary syndrome in 2008,and scale the gap between real world pratice and evidence-based guidelines.Methods Clinical data of the dianosis and management of 373 patients with non-ST elevation acute coronary syndrome from January to December in 2006,and scale the gap between real world pratice and evidence-based guidelines.Results Elevated cardiac markers,chest X-ray,echocardiography,dynamic electrocardiogram,exercise electrocardiograph (ECG),coronary angiography were used more extensively,but the usage of cardiac stress imaging and electron-beam computed tomography was low.Significant increases in the use of coronary angiography,aspirin,using 300 mg first dosage of aspirin,β-blocker,angiotensin converting enzyme inhibitor (ACEI),thienopyridine,statins and invasive strategies such as percutaneous coronary intervention (PCI) (51.1%),early invasive intervention occurred.The utilization rate of morphine sulfate,nitrates,unfractionated heparin were low.Conclusions There are great progress in the dianosis and management of patients with non-ST elevation acute coronary syndrome.Still,further improvements are needed for optimal implementation of these guidelines such as the use of β-blocker,adding dosage of β-blocker,platelet glycoprotein Ⅱb/Ⅲa inhibitor,low-molecular-weight heparin.Prompt and effective measures should be taken to enhance the education of evidence-based guidelines.

Abstract:It is well-known that elevated low-density lipoprotein cholesterol (LDL-C) is a causal risk factor for atherosclerotic cardiovascular disease (ASCVD), statins are cornerstone drugs for the cause-based treatment of ASCVD, which has created a new era for ASCVD therapy. However, statin intolerance is not clinically uncommon, which there are several issues with confusion and misunderstandings. Hence, a file named Chinese Expert Consensus on the Diagnosis and Management Strategy of Patients With Statin Intolerance, like a navigator, has recently been published written by a team of experts from the Cardiovascular Metabolic Medicine Professional Committee, Expert Committee of the National Center for Cardiovascular Diseases aiming to enhance the standardized clinical application of statins and improve the prevention and clinical outcome. In this article, author briefly summarized the key points of above consensus in order to helping to comprehending the content of the consensus suggestions.

Abstract:Statins,inhibitors of 3-hydroxy-3-methylglutaryl coenzyme reductase, can significantly reduce the incidence of athero-sclerotic cardiovascular disease(ASCVD)events.Recently,some guidelines and consensus have suggested that the patients with acute coronary syndromes(ACS)should start high-intensity statin therapy early after admission.But, Chinese patients tend to have higher incidence of side effects in statins treatment than Caucasians.Therefore,particular attention should be paid to the adverse reactions in Chinese population after high-intensity statin treatment.In this article, we reviewed the safety of high-intensity statin therapy in ACS patients in China,and provides a reference for clinical practice.

Abstract:Background: Myocardial necrosis is related to higher incidence of cardiovascular events and higher mortality rates during follow-up. Statins have been demonstrated to lower the risk of periprocedural myocardial infarction. The aim of this study is to evaluate the safety and efficacy of intensive statin therapy and the long-term outcome of patients in the Asian population. Study design: Approximately 1,100 patients with stable angina or non-ST elevation acute coronary syndrome undergoing selective percutaneous coronary intervention (PCI) are enrolled in this study. Patients are randomized either to the experimental group (80 mg atorvastatin 80 mg/day x 2 days before and 40 mg/day x 30 days after PCI) or to the control group (usual care). Creatine kinase-MB (CK-MB), troponin I (cTnI), and serum creatine are measured at 24 hours after the procedure. The total follow-up period is 6 months. The primary objective is to evaluate the efficacy of intensive atorvastatin treatment compared with usual care in reducing 30-day primary cardiovascular endpoints in patients undergoing selective PCI. Secondary endpoints are changes in myocardial biomarkers (cTnI, CK-MB) and hs-CRP, CIN morbidity, 6-month clinical outcomes, and safety. Conclusion: The result of the ISCAP study will provide important evidence on the efficacy and safety of periprocedural serial intensive statin treatment in Asian patients with coronary artery disease undergoing selective PCI. Abstract:Objective To evaluate the efficacy and safety of large doses intensive rosuvastatin treatment in short_term among the pa-tients with acute coronary syndrome (ACS ) during hospitalization.Methods A total of 131 ACS patients were enrolled into this study.They were randomly divided into the intensive group and conventional group.The patients in the intensive group were given 20mg ro-suvastatin as soon as admission ,20mg rosuvastatin every night during hospitalization.The patients in the general group were given 10mg every night during hospitalization.All the patients were given the biochemical indexes detection both on admission and after one week’s treatment.Results Age ,gender ,complications ,smoking ,overweight ,creatinine(Cr) ,creatine kinase(CK) ,and glomeru-larfiltrationrate(GFR)weresimilarbetweentwogroups (P>0.05).Levelsofbasicalanineaminotransferase (ALT)inconventional group were higher than that in intensive group.After one week’s treatment ,alanine aminotransferase (ALT) and aspartate amino-transferase (AST ) were higher in both groups ,which had a significantly difference than before ( P<0.05 ) ,but still in normal range.Compared with 10mg group ,there was no significant increase in the proportion of 3_times increased in ALT ,and AST in 20mg group.No differences were found in CK ,Cr and GFR after one week’s therapy in both groups.After one week’s therapy ,blood total choles-terol ,triglycerides and LDL cholesterol were lower than before in both groups ,but LDL_C of intensive treatment were significantly lower than conventional treatment.HDL_C had no significantly differences in conventional group but significantly higher in the inten-sive group after one week’s treatment.Conclusions The 20mg short_term intensive rosuvastatin treatment is as safe as the 10mg gen-eral treatment ,and effectively reduce the lipid and cholesterol levels ,and increase the high density lipoprotein.
Rationale and Design of China Intensive Lipid Lowering with Statins in Acute Coronary Syndrome: the CHILLAS Study.

Abstract:Background Current guidelines recommended intensive low-density lipoprotein cholesterol lowering with statins, aiming at the target of 70 mg/dL (1.81 mmol/L) of low-density lipoprotein cholesterol for those with very high risk of coronary artery events. However, there is no multicenter study assessing the effect of intensive lipid lowering therapy with statins on acute coronary syndrome (ACS) in Chinese population.Methods An open-label multicenter study is planned to evaluate whether intensive treatment with statins for 2 to 5 years results in more reduction of cardiovascular events in patients with ACS compared to the standard statin therapy. A total of 1,600 patients will be randomly assigned to receive intensive statin therapy (atorvastatin, 20 or 40 mg/d, or equivalent dose of other statins) or standard statin therapy (atorvastatin, 10 mg/d, or equivalent dose of other statins). Both groups will receive dietary counseling. Over the follow-up period, the primary outcome measure is the time to occurrence of cardiac death, nonfatal acute myocardial infarction, revascularization with either percutaneous coronary intervention or coronary-artery bypass grafting, documented unstable angina or severe heart failure requiring emergency hospitalization, and stroke. The planned duration for enrollment is between December 2006 and December 2009.Conclusions The CHILLAS study will be the first multicenter study in a Chinese population using a patient-level analysis to compare the effects and safety of intensive statin therapy with that of standard-dose statin therapy, which may provide new evidence and therapeutic standards for the treatment of ACS. Abstract:Dyslipidemia has been widely recognized as a significant risk factor for coronary atherosclerosis disease (CAD). In fact, lipid variability has emerged as a more reliable predictor of cardiovascular events. In this study, we aimed to examine the variability in plasma lipids under two different lipid-lowering regimens (intensive statin therapy versus the combination of conventional-dose statins with ezetimibe). In total, we have retrospectively examined 1275 patients with CAD from January 2009 to April 2019 and divided them into two groups: intensive statin group and conventional-dose statins combined with ezetimibe group. All patients were followed up for at least 1 year. Lipid variability was verified by standard deviation (SD), coefficient of variation (CV), and variability independent of mean (VIM) triple methods. Multiple linear regression and subgroup analyses were performed. In the overall participants, the mean age was 62.3 ± 10.4 years old, and 72.8% were male. Multivariate linear regression analysis indicated that the intensive statin group had lower variability in terms of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and non-high-density lipoprotein cholesterol (non-HDL-C) in all SD, CV, and VIM triple methods than statins combined with ezetimibe group (P for all <0.05). Similar results were established in the subgroup analyses based on atorvastatin or rosuvastatin, diabetes mellitus or not, and hypertension or not (P for all < 0.05). Thus, we can conclude that intensive statin therapy could contribute in lowering lipid variability than conventional-dose statins combined with ezetimibe therapy among patients with CAD.
Effect of Simvastatin on Acute Coronary Syndrome Complicated with Atrial Fibrillation

Abstract:Objective: To investigate the effect of simvastatin on acute coronary syndrome complicated with atrial fibrillation.Methods: A total of 112 cases with acute coronary syndrome complicated with atrial fibrillation were randomly divided into treatment group(60 cases) and control group(52 cases).Besides conventional treatment,simvastatin was used at 40 mg/d for 4 weeks,then 20 mg/d.Therapy of simvastatin was irregular in control group with total course less than 8 weeks,and the total course was 12 months.Hepal and renal function,high senstive C-reaction protein,blood lipid,reoccurrence of paroxysmal atrial fibrillation,incidence of sustained or permanent atrial fibrillation and status of cardiovascular diseases.Results: There was insignificant difference in blood lipid and C-reactive protein before and after treatment in control group(P0.05).After treatment,the levels of total cholesterol,low-density cholesterol and C-reactive protein were significantly decreased(P0.05),and high-density lipoprotein was significantly increased(P0.05) in treatment group.6 cases(10%) had reoccurrence of paroxysmal atri-al fibrillation or had paroxysmal transforming into sustained or permanent one in treatment group,which was significantly lower than control group(17/52,36.5%)(P0.05).And 8 cases had cardiovascular problem,significantly lower than control group(22/52,42.3%)(P0.05).Conclusion: Simvastatin can regulate blood lipid,anti-atrial fibrillation,and decrease the incidence of cardiovascular diseases.

Abstract:The clinical benefits of statins have well‐established and recognized worldwide. Although statins are well‐tolerated generally, however, the report of statin‐related adverse event and statin intolerance are common in China, which results in insufficient use of statins and poor adherence. The main reason may be attributed to confusions or misconceptions in the clinical diagnosis and management in China, including the lack of unified definitions and diagnostic standards, broad grasp of diagnosis, and unscientific management strategies. Based on that, this consensus carefully summarized the statin‐related gene polymorphism and statin usage issue among Chinese population, and comprehensively reviewed global research data on statin intolerance, referenced guidelines, and consensus literature on statin intolerance in foreign and different regions, proposes an appropriate and easy to implement statin intolerance definition as well as corresponding diagnostic criteria and management strategies for Chinese clinicians, in order to improve the clinical application of statin drugs and enhance the prevention and treatment level of atherosclerotic cardiovascular disease in China.

The Effects and Safety of Short-term Intensive Statin Therapy on Glomerular Filtration Rate in Patients with Acute Coro -nary Syndrome

Abstract:Objective To evaluate the clinical efficacy and kidney safety of short-term intensive statin therapy in patients with acute coronary syndrome (ACS) .Methods A total of 199 ACS patients admitted in Hangzhou First People’s Hospital were enrolled into this study .The patients were randomly assigned to receive atorvastatin 80mg/night during hospitalization ,and 40mg/night after discharge for one month (intensive group ,n = 99) ;or receive atorvastatin 20 mg/night during hospitalization and 20 mg/night after discharge for one month (control group ,n = 100) .The biochemical indexes were measured and compared on the admission and after one-month treatment .Results After one-month treatment ,the total cholesterol (TC) ,triglycerides (TG) and LDL cholesterol (LDL-C) of inten-sive group were significantly lower ,and the high density lipoprotein cholesterol (HDL-C) was significantly higher than baseline values (P< 0 .05) .The Glomerular filtration rates were significantly improved compared with baseline values (P < 0 .05) .Compared with the control group ,the liver enzymes and creatine kinase in intensive group were not significantly changed ,but the creatinine level was significantly decreased (P< 0 .05) .Meanwhile ,the blood homocysteine and ultra-sensitivity C-reactive protein levels were also sig-nificantly decreased compared with the baseline valies (P< 0 .05) .Conclusions In the circumstance of drug safety ,short-term inten-sive statin therapy can effectively reduce TC ,TG ,LDL-C and homocysteine levels in patients while improve the glomerular filtration rate and renal protective effect ,which resulted to higher HDL cholesterol levels .

Abstract:Introduction: The risk of recurrent stroke is high in patients with intracranial atherosclerotic stenosis (ICAS). Statin use has been demonstrated to decrease the incidence of stroke by reducing atherosclerotic plaque burden. However, its effect on the hemodynamic situation and cerebral perfusion status has not yet been validated. With the use of computed tomography perfusion (CTP), we aim to evaluate the impact of Rosuvastatin on cerebral hemodynamic changes, as well as the downstream perfusion.Method: Cerebral bood flow evaluation of intensive rosuvastatin therapy in patients with intracranial arterial atherosclerotic stenosis (CEIRIS) is a single-center, prospective, randomized, parallel-group, and open-label trial, and it will include 50 participants as estimated. Patients with moderate to severe (50%-99%) ICAS are randomized 1:1 to 10 mg/day or 20 mg/day Rosuvastatin and followed every 13 weeks until 52 weeks. The primary outcome for the trial is the change in the relative regional cerebral blood flow evaluated by CTP after 52 weeks of Rosuvastatin treatment. The secondary outcomes are cerebral blood volume, change in the degree of stenosis of the target vessel and lipid parameters.Conclusion: The CEIRIS trial about the effects of statin on the temporal hemodynamic progression of ICAS may extend our understanding of the basic pathophysiology and mechanisms of stroke in ICAS patients.

Effects of Ezetimibe-Statin Combination Therapy on Coronary Atherosclerosis in Acute Coronary Syndrome

Abstract:Background: The results of previous clinical trials on the effects of ezetimibe-statin combination therapy on atherosclerosis are inconsistent, and the anti-atherosclerotic effect of ezetimibe remains controversial.Methods and Results:We conducted a prospective, randomized open-label study at 10 centers. One hundred and twenty-eight statin-naïve patients with acute coronary syndrome (ACS) undergoing intravascular ultrasound (IVUS)-guided percutaneous coronary intervention were randomized to receive either 2 mg/day pitavastatin plus 10 mg/day ezetimibe, or 2 mg/day pitavastatin. One hundred and 3 patients had evaluable IVUS of non-culprit coronary lesions at baseline and at follow-up. The primary endpoint was the percentage change in non-culprit coronary plaque volume (PV) and lipid PV on integrated backscatter IVUS. Mean low-density lipoprotein cholesterol was reduced from 123 mg/dL to 64 mg/dL in the combination therapy group (n=50) and 126 mg/dL to 87 mg/dL in the statin alone group (n=53; between-group difference, 16.9%, P<0.0001). The percent change in PV was -5.1% in the combination therapy group and -6.2% in the statin alone group (P=0.66), although both groups had reduction of PV compared with baseline (both P<0.01). The percent change in lipid PV did not differ between the groups (4.3 vs. -3.0%, P=0.37). Conclusions: In statin-naïve patients with ACS, combined therapy with ezetimibe and statin did not result in a significant change in coronary plaque regression or tissue component compared with statin alone. Benefits of intensive lipid-lowering therapies in patients with acute coronary syndrome: a systematic review and meta-analysis

Abstract:Background: Previous meta-analyses have investigated the efficacy of lipid-lowering therapies for atherosclerotic cardiovascular disease; however, few have focused on patients with acute coronary syndrome (ACS). This meta-analysis aimed to compare the benefits of intensive lipid-lowering therapy with those of background statin therapy in patients with ACS. Methods: Searches were performed on PubMed, Embase, the Cochrane Central Register of Controlled Trials, and ClinicalTrials.gov databases for articles published until April 13, 2023. Randomized controlled trials that compared intensive lipid-lowering therapies and background statin therapies in patients with prior ACS and recorded the outcome of three-point major cardiovascular events (MACE) were included. The risk ratio (RR) with 95% confidence interval (CI) was used as a measure of primary and secondary outcomes. Results: Nine trials involving 38,640 patients with ACS were identified. Pooled results suggested that intensive lipid-lowering therapies are associated with a reduction in the risk of three-point MACE (RR, 0.88; 95% CI, 0.83-0.94; p < 0.001), recurrent ACS (RR, 0.82; 95% CI, 0.71-0.96; p = 0.013), nonfatal myocardial infarction (MI) (RR, 0.87; 95% CI, 0.81-0.93; p < 0.001), stroke (RR, 0.83; 95% CI, 0.73-0.94; p = 0.003), and unstable angina-related hospitalization (RR, 0.57; 95% CI, 0.33-0.99; p = 0.046), but not all-cause mortality (RR, 0.94; 95% CI, 0.82-1.07; p = 0.329), cardiovascular disease-related mortality (RR, 0.96; 95% CI, 0.88-1.06; p = 0.457) or coronary revascularization (RR, 0.89; 95% CI, 0.79-1.00; p = 0.057). Conclusions: Intensive lipid-lowering therapies may reduce the risk of three-point MACE, recurrent ACS, nonfatal MI, stroke, and hospitalization for unstable angina in patients with ACS undergoing background statin therapy. These results may assist in clinical decision-making for the secondary prevention of cardiovascular events to initiate intensive lipid-lowering therapies immediately after ACS.
