Abstract:To study the relevance between the proteins of circadian clock genes BMAL1 and Ki-67 in nasopharyngeal carcinoma (NPC) as well as their effects on clinical characteristics and prognosis of NPC patients. Real-time fluorescent quantitative PCR was used to detect the expression of BMAL1 mRNA in carcinoma tissues of 36 NPC cases and tissues of 20 cases of chronic nasopharyngitis. Imunohistochemistry was used to detect the expression of BMAL1 protein in carcinoma and para-carcinoma tissues and the expression of Ki-67 protein in carcinoma tissues of 90 NPC cases. The expression level of BMAL1 mRNA of NPC was significantly lower than that of chronic nasopharyngitis (4.670 + 0.279 and 6.641 + 0.511 P=0.002). The expression level of BMAL1 mRNA of NPC with distant metastasis was significantly lower than that of NPC without distant metastasis (3.377±0.662 and 5.040±0.272 P=0.011). The expression level of BMAL1 protein in NPC tissues was significantly lower than that in para-carcinoma tissues (2.792±0.469 and 29.223±1.244 P=0.000). There was no statistical significance in the difference between the expression of sex, age and tumor stage (P>0.05). The OS in the group with high expression of BMAL1 was significantly higher than that of the group with low expression, and the OSs were respectively 92.6%, 69.2%, 62.3% and 59.1%, 40.9%, 0% after 1, 3 and 5 years (P=0.000). The PFS, RFS and DMFS in the group with high expression after 1, 3 and 5 years were higher than these in the group with low expression, however, P＞0.05. The Cox multivariate analysis of BMAL1 protein expression was an independent prognostic factor affecting OS (P=0.032). The OS in the group with high Ki-67 expression was significantly lower than that in the low expression group. The OSs after 1, 3 and 5 years were respectively 88.6%, 61.7%, 61.7% and 100%, 100%, 100% (P=0.035). The Pearson correlation analysis found that the expression of BMAL1 protein and Ki-67 protein had a negative correlation (P=0.021). The expression of BMAL1 gene in NPC is lower than that in chronic nasopharyngitis and para-carcinoma tissues. High expression of BMAL1 protein and low expression of Ki-67 protein can significantly prolong the OS in NPC patients. The expression of BMAL1 protein is an independent prognostic factor affecting OS. The expression levels of BMAL1 and Ki-67 are negatively correlated, which is likely to become an indicator to evaluate the prognosis of NPC.
