Abstract:The clinical treatment of nasopharyngeal carcinoma (NPC) in China started in 1940s is based on our own clinical staging system. In 1965, we established the first TNM staging system for NPC. Advances in technology and therapy led to continuous updates in clinical staging. Currently, the Chinese staging system of 2008 is commonly used in China. Compared with the 7th edition of the American Joint Committee on Cancer (AJCC) staging system, which is widely used internationally, the N classification was based on magnetic resonance imaging (MRI) as well as the lymph node criteria of the Radiation Therapy Oncology Group (RTOG) consensus guidelines. However, several studies showed that these two staging systems were associated with limitations. Additional international studies from different cancer centers are needed to improve and evolve a universally accepted staging system for NPC.

Comparative study of nasopharyngeal carcinoma staging system between the chinese 2008 and'92 Fuzhou

Abstract:Objective To carry out a comparative study between the Chinese 2008 and'92 staging system of nasopharyngeal carcinoma (NPC). Methods A total of 777 patients presented with untreated nondisseminated NPC who had received MRI scan of nasopharynx and neck were studied retrospectively. The clinical materials and information of imaging were collected. All patients were restaged according to the Chinese 2008 and 92 staging system of nasopharyngeal carcinoma. Distribution of T, N stage, survival and prognostic value were compared. 513 patients of the 777 cases were treated with conventional radiotherapy,264 cases with intensity modulated radiation therapy. Results The 3-year follow-up rate was 97.6%. The consistency of T stages was 95.0%. T, N and clinical stage distributions in two systems were similar ( Kappa = 0. 93, P = 0. 000; Kappa = 0. 58, P = 0. 000; Kappa = 0. 74, P = 0. 000). Local failure-free survival and disease specific survival were also similar. There was no difference of distant metastasis between N0 and N1(x2 = 1.94,P=0. 164), and a marginal difference between N1 and N2(x2 =3.83,P=0.051) in the Chinese'92 staging system. However, although there was also no difference of distant metastasis-free survival between No and N1a(x2 =0. 07,P =0. 797), ) the difference of overall survival among N1b, N2, and N3 were significant ( x2 = 4. 95, P = 0. 026; x2 = 6. 74, P = 0. 009) in the Chinese 2008 staging system. Conventional radiotherapy or intensity modulated radiation therapy was not a prognostic factor for survival ( x2 = 3.60,P =0. 058). It is reasonable for the Chinese 2008 staging system integrated lymph node characteristics such as laterality, level and extranodal neoplastic spread into the N staging criteria ( x2 = 6. 59, P = 0. 010; x2 =4.78,P=0. 029;x2=9.32,P=0. 002). Conclusions For the Chinese 2008 staging system, it was reasonable to simplify the previous T stage. The N stage showed a better predictive role of distant failure.The role of retropharyngeal lymph node in stage system needs further investigation.
MRI Study on the Relationship Between the Invasion Patterns and Staging of Nasopharyngeal Carcinoma

Abstract:Objective To investigate the peri-nasopharyngeal invasion patterns of nasopharyngeal carcinoma (NPC) on MRI and its relationship with tumor staging. Methods One thousand five hundred and seventy-three patients with newly diagnosed NPC which were histo-pathologically proved were retrospectively studied. The MRI manifestations and invasion patterns of the NPCs were elevated according to the 2008 Tstaging system of NPC. Z test was used to analyze the rate of adjacent structures invasion in NPCs. Results The structures invaded by NPCs included pharyngobasilar fascia in 1299 cases (82. 58% ); parapharyngeal space, 1090 ( 69. 29% ); nasal cavities, 304 ( 19. 33% ); oropharynx, 49 ( 3. 12% ); carotid space,514(32. 68% ); medial pterygoid muscle, 661 (42. 02% ); lateral pterygoid muscle, 210( 13. 35% ); skull base bones, 943(59. 95% ); cranial nerves, 630(40. 05% ) and paranasal sinuses, 242 ( 15.38% ). The T-stage distribution was T1, 242 cases ( 15.38% ); T2, 288 ( 18. 31% ); T3, 410 (26. 06% ) and T4,633 (40. 24% ). Among the cases with nasal cavities invasion, 90. 46% (275/304)showed the involvement of the structures seen in T3 or T4 stage, which was found in all cases with oropharynx invasion. In addition,69. 14% (457/661) of cases with medial pterygoid muscle invasion and 92. 15% (223/242) of cases with paranasal sinuses invasion showed the involvement of structures seen in T4 stage. As for the invasion patterns of NPC, the lateral invasion of pharyngobasilar fascia was more frequent than upward invasion of skull base (Z = 14. 025, P ＜ 0. 01 ) and downward invasion of oropharynx ( Z = 45.032, P ＜ 0. 01 ), and the downward invasion of oropharynx was less frequent than upward invasion of skull base ( Z = 34. 301, P ＜ 0. 01 ) and forward invasion of nasal cavities ( Z = 14. 404, P ＜ 0. 01 ). Conclusion NPC has a predilection of lateral invasion rather than upward and downward invasion, and its upward and forward invasion are more common than downward invasion.
Comparison of the Chinese 2008 and the 7th Edition Ajcc Staging Systems for Nasopharyngeal Carcinoma

Abstract:Although nasopharyngeal carcinoma (NPC) is rare in other parts of world, it is endemic in certain regions, especially in Southern Chinese. The seventh edition of the American Joint Committee on Cancer (AJCC) staging manual defines new rules for classifying nasopharyngeal carcinoma (NPC). The Chinese 1992 staging system had also been revised one year ago. The new name was Chinese 2008 staging system, which was characteristic as its N staging criteria using lymph node variables basing on magnetic resonance imaging (MRI) instead of palpation examination. The current study conducted in Southern China, compares the two in predicting NPC prognosis. A total of 799 patients presented with untreated nondisseminated NPC who had received MRI scan of the nasopharynx and neck were studied retrospectively. The clinical materials and information of imaging were collected. All patients were restaged according to the AJCC staging system and Chinese 2008 staging system of nasopharyngeal carcinoma. Distribution of patients, survival and prognostic value were compared. The median follow-up for the whole group was 36 months (range 2-54). By two staging systems, patient distributions in T, N and clinical stage were similar. Only 71 (8.9%) patients had N0 disease, which caused the percentage of patients with stage I was merely 1.8%. Using AJCC system, 3-year disease specific survival rates for stage I, II, III and IV were 100%, 93.8%, 83.9% and 73.8%, respectively. Statistical significance was obtained among adjacent stages except for stage I and stage II. Corresponding survival curves were similar by the Chinese 2008 system. Free from local failure survival curves for stage T1, T2 and T3 were not statistically divided by the two staging systems. For the Chinese 2008 system, the distant failure-free survival curves for N0 and N1a were overlap, and survival curves showed better separation between N1b, N2, and N3. However, for AJCC staging system, no statistical significance was found for distant metastasis rates between N0 vs. N1, N2 vs. N3. Both systems have the similar prognostic value. However, it appeared that the Chinese 2008 system was slightly better in N classification, when considering its new reliable criteria. There were no statistical differences for local relapse rates between T1, T2 and T3. The percentage of patients with stage N0 and stage I were small, when retropharyngeal lymph node (RLN) was defined as N1 disease.
A Comparative Analysis of CT and MRI in Detecting Skull Base Invasion of Nasopharyngeal Carcinoma

Abstract:[Purpose] To investigate the detective ability of CT and MRI in skull base invasion of nasopharyngeal carcinoma (NPC). [Methods] One hundred and twenty eight newly diagnosed patients with pathologically confirmed NPC were examined by both CT and MRI. [Results] The overall positive rate of skull base invasion detected by CT and MRI was 19.53%(25/128) and 49.22%(63/128), with significant difference (P0.05). There were 6 cases(24.00%)with single site involved and 19 cases(76.00%) with multi-sites involved in CT scans. There were 18 cases (28.57%)with single site involved and 45 cases(71.43%) with multi-sites involved in MRI. [Conclusion] The detective ability of MRI for skull base invasion in NPC is superior to that of CT. MRI imaging is more sensitive than CT especially in detecting NPC marrow invasion. It will benefit the staging of NPC and the clinical treatment strategy.

Abstract:OBJECTIVES:The Chinese 1992 staging system for nasopharyngeal carcinoma (NPC) was revised in 2008 and was renamed as the Chinese 2008 staging system. The seventh edition of the American Joint Committee on Cancer (AJCC) staging manual also defined new rules for classifying NPC in 2010. The purpose of the current study is to compare the 2 in terms of patient distribution and efficacy in predicting prognosis.METHODS:A total of 816 patients with untreated nondisseminated NPC who underwent magnetic resonance imaging scan of the nasopharynx and neck were studied retrospectively. All magnetic resonance imaging scans were reevaluated independently by 2 radiologists specialized in head and neck cancers. All patients were restaged according to the Chinese 2008 staging system and the AJCC staging system of NPC.RESULTS:Using the 2 staging systems, the consistency for patient distributions in T, N, and overall stages was found to be moderate, with the κ value of 0.65, 0.54, and 0.46, respectively. According to the Chinese 2008 and the AJCC staging systems, the proportion of patients in stages I, II, III, and IV accounted for 2.3% versus 2.3%, 11.0% versus 23.7%, 39.4% versus 49.1%, and 47.3% versus 24.9%, respectively. The AJCC T classification was better in predicting the 5-year local relapse-free survival, whereas the Chinese 2008 N classification was superior in predicting the 5-year distant metastasis-free survival. However, survival curves for the 5-year overall survival were comparable in both systems.CONCLUSIONS:We revealed a slightly better patient distribution of overall stage with AJCC comparing with the Chinese 2008 staging system. The prognostic value of AJCC T classification was considered to be better, whereas that of Chinese 2008 N classification was superior.

Abstract:Abstract The tumor, node, metastases (TNM) staging system for nasopharyngeal carcinoma (NPC) has undergone important refinement throughout the years to match the evolving technology in imaging and the changing landscape of treatment. In the latest edition of the American Joint Committee for Cancer/Union for International Cancer Control TNM staging system, stage-defining criteria are more precise and more applicable when magnetic resonance imaging is commonly used. It has been demonstrated to have stronger prognostic power in contemporary clinical series with the use of modern treatment technique including intensity-modulated radiotherapy and combination chemotherapy. Nonetheless, there is room for further improvement in the prognostication of NPC as other factors including novel biomarkers, nomograms, radiotherapy dosimetry, and certain clinical parameters have been identified to influence the treatment outcome. This chapter will go through the progress in the staging system of NPC and future directions in achieving a more personalized method of prognostication.
[New Clinical Staging System for Carcinoma of Nasal Cavity].

Abstract:OBJECTIVE To establish a new staging system based on analysis of several presently used clinical staging systems for carcinoma of nasal cavity. METHODS The data of 122 patients treated from 1985 to 1997 in the cancer center of Sun Yat-sen University were analyzed, and a new clinical staging system was established using computer optimizing and screening combined with the clinical results. The survival analysis was performed by Kaplan-Meier estimates, and the multivariate analysis was achieved by Cox proportional hazard model. RESULTS The flaws in the presently used clinical staging systems proposed by Zhuang, Qiu, Department of Head and Neck of Cancer Center of Sun Yat-sen University and University of Florida and the AJCC'2002, were as follows: insufficient consideration of the modern tomography resulting in indefinite location of the tumor in clinical practice, the uneven distribution of patients in different stages, being unable to separate survival curves of different stages, and not containing of all necessary clinical staging information in some staging systems. However, based on our new staging system, the cases distributed in T1, T2, T3 and T4 was 16, 32, 42 and 32, and the 5-year survival rate was 78.8%, 64.6%, 49.9% and 30.0%, respectively. The cases distributed in stage I, II, III and IV was 16, 26, 45 and 35, and the 5-year survival rate was 78.8%, 68.4%, 51.3% and 29.0%, respectively. The overall 5-year survival rate was 61.6%. CONCLUSION Compared to the presently used clinical staging systems, the new staging system may have more advantages in various parameters for the clinical staging in the carcinoma of nasal cavity, and may be worth to be widely and clinical used.

Abstract:Objective To compare the agreement among Chinese 1992, 2008 and UICC 2010 staging systems of nasopharyngeal carcinoma (NPC) and evaluate their predictive value of radiotherapeutic prognosis.Methods 347 NPC patients without distant metastasis treated in our hospital from 2000 to 2005 were retrospectively analyzed.Every patient was categorized into T, N, and clinical stage by Chinese 1992, 2008 and UICC 2010 staging systems, respectively.Kappa value was used to evaluate the agreement among three systems.Kaplan-Meier method was used to analyze the 5-year overall survival (OS), local-free survival (LFS) and distant metastasis-free survival (DMFS), the difference between subgroup was tested by Logrank.Results The agreement of clinical stage, T and N stage between Chinese 2008 and UICC 2010 staging system was better than that of them compared to 1992 staging system, Kappa value were 0.700、0.881 and 0.722.The agreement of T stage was better than N and clinical stage among these three staging system.The difference of OS between stageⅢ and stage Ⅳ was significant in Chinese 2008 and UICC 2010 staging system (χ2=4.48,P=0.034;χ2=8.88,P=0.003), and with no different in 1992 staging system (χ2=0.40,P=0.526).There was no significant difference of LFS between T1 and T2,T2 and T3,T3 and T4 in all staging systems (χ2=1.85,0.53,0.50,P=0.174,0.467,0.479;χ2=1.25,2.10,1.99,P=0.264,0.148,0.159;χ2=0.77,0.60,0.87, P=0.381,0.441,0.350).There were no significant differencesin 1992 staging system, while there was significant differences of DMFS between N1 and N2, N2 and N3 in 2008 stage system, N1 and N2 in UICC 2010 stage system.Conclusions The predictive value of Chinese 2008 and UICC 2010 staging system for prognosis were similar, and were better than that of 1992 staging system in NPC.
Impact of magnetic resonance imaging versus CT on nasopharyngeal carcinoma: primary tumor target delineation for radiotherapy

Abstract:BACKGROUND: Our aim was to assess the capacity of CT versus MRI for delineating to the primary tumor extent of nasopharyngeal carcinoma (NPC) in treated patients.METHODS: From December 1997 to April 2000, 258 patients with NPC were enrolled. We focused on the primary tumor extension and the discrepancy between CT and MRI. The delineation of tumor invasion was crucial for determination of the gross tumor volume (GTV) before radiation therapy.RESULTS: A total of 104 patients (40.3%) had intracranial infiltration detected by MRI, whereas CT showed negative findings (p = 6.879 x 10(-11)). Once the pterygopalatine fossa was involved, the chance of intracranial invasion was increased (96.1%). The detectable percentage of pterygopalatine fossa involvement accompanying intracranial invasion was higher with MRI than with CT (96.1% vs 56.9%).CONCLUSIONS: More detailed information about T and N classification of NPC was provided by MRI than by CT, which led to better target delineation for radiotherapy.
Value of Cranial Nerve Involvement Found on MRI in Staging of Nasopharyngeal Carcinoma

Abstract:Objective To identify the value of cranial nerve involvement found on magnetic resonance imaging (MRI) in staging of nasopharyngeal carcinoma (NPC).Methods A retrospective analysis was performed on the clinical data of 816 patients with NPC initially diagnosed by MRI during 2005 -2007.The patients with stage T4 NPC were divided into cranial nerve subgroup,which was defined as patients who had cranial nerve involvement on MRI,but without cranial nerve palsy or other T4-related anatomical structure involvement,and other subgroup,which was defined as other patients with stage T4 NPC ;the patients with stage Ⅳ a NPC were divided into cranial nerve subgroup,which was defined as patients with T4N0-2 NPC who were classified into the cranial nerve subgroup of patients with stage T4 NPC,and other subgroup,which was defined as other patients with stage Ⅳa NPC.The survivals of the subgroups were compared.The Kaplan-Meier method was used for calculating survival rates,and the survival differences were analyzed by the log-rank test.Results The follow-up rate was 94.1％.Cranial nerve involvement was seen in 38.5％ of all patients.Local recurrence-free survival,distant metastasis-free survival,and disease-related survival curves of patients with stage T2 and T3 NPC and the cranial nerve subgroup of patients with stage T4 NPC coincided,but they were separated from the curves of the other subgroup of patients with stage T4 NPC.There was no significant difference in 5-year disease-related survival rate between the cranial nerve subgroup of patients with stage Ⅳa NPC and the patients with stage Ⅲ NPC (x2 =0.18,P =0.674),but significant differences were found when this subgroup was compared with patients with stage Ⅱ NPC (x2 =5.86,P =0.015) and the other subgroup of patients with stage Ⅳa NPC (x2 =7.23,P =0.007).Conclusions The patients with stage T4/Ⅳa NPC who have cranial nerve involvement on MRI,but without other T4-related anatomical structure involvement,have similar local recurrence-free survival rate and distant metastasis-free survival rate as those with stage T2 and T3 NPC and have a similar disease-related survival rate as those with stage Ⅲ NPC,and their prognosis is better than that of the other subgroup of patients with stage T4/Ⅳa NPC.It may lead to overtreatment for patients if they are classified as stage T4/Ⅳa and receive the treatment regimen for stage Ⅳ NPC.

Abstract:BACKGROUND AND PURPOSE:The purpose of this study was to evaluate the 7th edition UICC/AJCC staging system for nasopharyngeal carcinoma (NPC) patients who were treated with intensity modulated radiation therapy (IMRT).MATERIALS AND METHODS:The clinical data of 1241 NPC patients with initial magnetic resonance imaging (MRI) scans were studied retrospectively. All MRIs were independently reevaluated and restaged according to the 7th edition by two radiologists specializing in head and neck cancers. Analysis of prognostic factors in local relapse-free survival (LRFS), distant metastasis-free survival (DMFS), disease-specific survival (DSS), and overall survival (OS) were performed.RESULTS:The proportion of patients in Stage I, II, III, IVA and IVB were 4.8%, 26.2%, 45.4%, 18.4%, and 5.2%, respectively. The differences of LRFS between T1 and T2, and between T2 and T3 were not significant (P=0.055 and 0.605, respectively). Hazard ratios (HRs) for DSS and OS between T2 and T3 or between T3 and T4 differed significantly, but not between T1 and T2. The differences of DMFS between N0 and N1, between N1 and N2 were significant. However no significant difference was found in DMFS between N2 and N3a, or between N2 and N3b. For patients with T1-T3 disease, although skull base infiltration did not impact local failure, it was an independent prognostic factor for both distant failure and cancer death.CONCLUSION:When treated with IMRT, the difference in the LRFS, DSS, and OS between T1 and T2 patients diminished, indicating that it is rational to merge T2 into T1. The prognostic value of the N classification of the current staging system had not changed much compared to the 6th edition.
Evaluation of 7th Edition of Ajcc Staging System for Nasopharyngeal Carcinoma

Abstract:Purpose: To evaluate and improve the 7(th) edition International Union against Cancer/American Joint Committee on Cancer staging system for nasopharyngeal carcinoma.Methods: A retrospective review of the data from 905 patients with biopsy-proven non-disseminated nasopharyngeal carcinoma was performed. All the patients were examined by magnetic resonance imaging (MRI) and received radiotherapy.Results: Satisfied distributions among the stages were observed in the 7th edition staging systems. LRFS only differed in classifications betweenT1 and T3, T1 and T4 (P=0.022 and P=0.016, respectively). Significant differences were observed between patients without and with masticator space involvement for OS, DMFS and PFS (p<0.05). No statistically significant differences in LRFS were observed among different groups with anatomical masticator space involvement. The DMFS between N2 and N3b, N3a and N3b were lack of significance (P=0.060 and P=0.59). The T category and N category were independent prognostic factors for the major endpoints in the Cox multivariate regression analysis (P<0.01).Conclusion: This study confirmed the prognostic value of the 7(t)h edition UICC/AJCC staging system, the revisions of the 7(th) edition staging system are acceptable. However, our study also revealed limitations in the current staging system and suggested some potential modifications in future revision.
Staging of T2 and T3 nasopharyngeal carcinoma: Proposed modifications for improving the current AJCC staging system

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objectives</h3><p>We aimed to reconstitute T2 and T3 stage classification in nasopharyngeal carcinoma (NPC) cases and verify its utility in clinical settings.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Materials and Methods</h3><p>We enrolled 792 NPC patients. Cox proportional hazards model was used to compare the effect sizes (hazard ratio [HR]) of the cranial structure invasion on survival and select the structures for up‐staging or downstaging T2 and T3 NPC. The samples were reclassified and the survival curves for T2 and T3 stages were analyzed. The proposed new staging system was validated on an external sample (n = 433).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Thirteen cranial structures were examined. American Joint Committee on Cancer (AJCC) T3 stage patients with the invasion of the base of the sphenoid (HR = 2.58, 95% CI = 1.16‐5.77) or base of the pterygoid (HR = 2.00, 95% CI = 0.84‐4.77) had significantly lower hazard ratios than T2 stage patients with the invasion of soft tissues in the bilateral parapharyngeal space (HR = 5.26, 95% CI = 2.02‐13.68) and single/bilateral carotid sheath (HR = 7.78, 95% CI = 3.06‐19.76). T3 stage with the invasion of the above‐mentioned bones was reclassified as T2, and T2 stage with the invasion of the above‐mentioned soft‐tissue structures was reclassified as T3. Survival analysis showed a significant difference between the reclassified T2 and T3 stages (<i>P</i> < 0.001). The results were replicated in the validation samples. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The proposed staging system for defining T2 and T3 stage NPC appears to be superior to the AJCC 8th edition. It could improve prognosis and optimize the treatment selection.</p></section>

Does MRI-Detected Cranial Nerve Involvement Affect the Prognosis of Locally Advanced Nasopharyngeal Carcinoma Treated with Intensity Modulated Radiotherapy?

Abstract:Nasopharyngeal carcinoma (NPC) is one of the common cancers in South China. It can easily invade into cranial nerves, especially in patients with local advanced disease. Despite the fact that the magnetic resonance imaging (MRI) findings are not always consistent with the symptoms of CN palsy, MRI is recommended for the detection of CN involvement (CNI). However, the prognostic impact of MRI-detected CNI in NPC patients is still controversial. To investigate the prognostic value of MRI detected CNI, we performed a retrospective analysis on the clinical data of 375 patients with NPC who were initially diagnosed by MRI. All patients had T3-4 disease and received radical intensity modulated radiation therapy (IMRT) as their primary treatment. The incidence of MRI-detected CNI was 60.8%. A higher incidence of MRI-detected CNI was observed in T4 disease compared with T3 disease (96.8% vs. 42.8%, P<0.001), and a higher incidence was also found in patients with Stage IV disease compared with those with Stage III disease (91.5% vs. 42.3%; P<0.001). The local relapse-free survival (LRFS), distant metastasis-free survival (DMFS), and overall survival (OS) of patients with T3 disease, with or without MRI-detected CNI, was superior to that of patients with T4 disease (P<0.05). No significant differences in LRFS, DMFS or OS were observed between T3 patients with or without MRI-detected CNI. The survival of Stage III patients with or without MRI-detected CNI was significantly superior to that of Stage IV patients (P<0.01), but there was no significant difference between Stage III patients with or without MRI-detected CNI for all endpoints. Therefore, when treated with IMRT, MRI-detected CNI in patients with NPC does not appear to affect the prognosis. In patients with clinical T3 disease, the presence of MRI-detected CNI is not sufficient evidence for defining T4 disease.
Proposal for the 8th edition of AJCC/UICC staging system for nasopharyngeal cancer in the era of intensity-modulated radiotherapy

Abstract:Objective\r\nAn accurate staging system is crucial for cancer management. With the development of cancer staging systems and therapeutic methods, the applicability and improvement of staging systems should be evaluated constantly.\r\n\r\n\r\nMethods\r\nThe clinical data of 1609 nasopharyngeal carcinoma patients without metastasis at initial diagnosis, who were admitted to two tumor centers in Hong Kong and Mainland China and received intensity-modulated radiotherapy (IMRT), were analyzed retrospectively based on the 7th edition of the American Joint Committee on Cancer (AJCC) or International Union Against Cancer (UICC) staging system, and all the patients underwent magnetic resonance imaging (MRI) before treatment.\r\n\r\n\r\nResults\r\nAmong the T3/T4 patients without involvement of other anatomic structures, overall survival (OS) showed no significant differences between the patients with masticator space (medial pterygoid muscle and/or lateral pterygoid muscle) involvement, prevertebral muscle involvement, and parapharyngeal space involvement. The OS was similar between the patients with extensive soft tissue (soft tissues other than the structures mentioned above) involvement and those with intracranial involvement or cranial nerve involvement. Only 2% of the patients had lymph node metastasis\u003e6 cm above the supraclavicular fossa (SCF), with an OS similar to that of the patients with lower cervical lymph node metastasis. Replacing SCF with the lower neck (below the caudal border of the cricoid cartilage) did not affect the risk difference between different N stages. With the proposed T and N staging systems, the OS showed no significant differences between T4N0-2 and T1-4N3 patients.\r\n\r\n\r\nConclusions\r\nAfter a review by AJCC/UICC staging system preparatory committees, the changes recommended for the 8th edition include changing medial pterygoid muscle or lateral pterygoid muscle involvement from T4 to T2, adding prevertebral muscle involvement to T2 stage, replacing SCF with the lower neck and combining this with a maximum lymph node diameter of\u003e6 cm as N3 stage, and integrating T4 and N3 as stage ⅠVA. These changes result in a better risk difference between adjacent stages and achieve the optimal balance between clinical practicability and global applicability.\r\n\r\n\r\nKey words: \r\nNasopharyngeal neoplasms/radiotherapy; Prognosis; TNM staging system

Abstract:Background: Nasopharyngeal carcinoma (NPC) is a common tumor in China. Accurate stages of NPC are crucial for treatment. We therefore aim to develop radiomics models for discriminating early-stage (I–II) and advanced-stage (III–IVa) NPC based on MR images. Methods: 329 NPC patients were enrolled and randomly divided into a training cohort (n = 229) and a validation cohort (n = 100). Features were extracted based on axial contrast-enhanced T1-weighted images (CE-T1WI), T1WI, and T2-weighted images (T2WI). Least absolute shrinkage and selection operator (LASSO) was used to build radiomics signatures. Seven radiomics models were constructed with logistic regression. The AUC value was used to assess classification performance. The DeLong test was used to compare the AUCs of different radiomics models and visual assessment. Results: Models A, B, C, D, E, F, and G were constructed with 13, 9, 7, 9, 10, 7, and 6 features, respectively. All radiomics models showed better classification performance than that of visual assessment. Model A (CE-T1WI + T1WI + T2WI) showed the best classification performance (AUC: 0.847) in the training cohort. CE-T1WI showed the greatest significance for staging NPC. Conclusion: Radiomics models can effectively distinguish early-stage from advanced-stage NPC patients, and Model A (CE-T1WI + T1WI + T2WI) showed the best classification performance.
