Abstract:Background: Gastric Cancer is one of the most lethal malignancies worldwide. Gamma-glutamyl transpeptidase (GGT) is an enzyme mainly involved in cellular glutathione homeostasis. We aim to explore the clinical value of GGT in gastric cancer.Results: Among 322 patients enrolled, 65/82 patients were determined as GGT positive in serum/tumor, respectively. High tumor GGT expression is significantly associated with lymph node metastasis, histological subtype, and Her2 expression. Kaplan-Meier curve shows that high tumor GGT patients have shorter overall survival (P (log-rank) = 0.001) and progress-free survival (P (log-rank) = 0.001). Patients with both high tumor and serum GGT have the poorest prognosis. The multivariable Cox analysis shows that the hazard ratio of overall survival for high tumor GGT is 1.69 (95% CI 1.19-2.37). High serum GGT is a poor prognostic factor in adjuvant chemotherapy hazard ratio = 2.18, 95% CI (1.15-4.47). These findings were further validated in six online datasets. Gene Sets Enrichment Analysis showed that GGT promotes cancer progression through EMT, KRAS, SRC and PKCA pathways.Methods: Tumor GGT and serum GGT levels were evaluated with immunohistochemistry staining and enzymatic assay, respectively. Kaplan-Meier curve and Cox regression model were used to test the association between GGT and gastric cancer prognosis. Independent datasets from Gene Expression Omnibus and Gene Sets Enrichment Analysis were applied to validate the findings and explore the potential mechanisms.Conclusion: Both tumor GGT and serum GGT are poor prognostic factors in gastric cancer. Patients with high tumor and serum GGT levels require more intense treatment and follow-up.

Abstract:Objective Through the determination of serum levels of pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ) in early gastric carcinoma(EGC) patients,to investigate diagnostic value of the PGⅠ,PGⅡ,and the PGⅠ/PGⅡlevels in the early diagnosis of gastric carcinoma.Methods Test the serum PGⅠ,PGⅡlevels of the normal control group(CON group,n=30),early gastric carcinoma group(EGC group,n = 32),and gastric ulcer group(GU group,n = 38) by chemiluminescence microparticle immunoassay. Results The serum PGⅠ,PGⅠ/PGⅡlevels of EGC group were lower than CON group and GU group(P0.01).The PGⅡlevels of EGC group and the control group PGⅡlevels were not significantly different(P0.05).The serum PGⅡlevels in gastric ulcer patients were higher than those in early gastric carcinoma patients(P0.01).Conclusion Detected the serum levels of PGⅠ,PGⅡand PGⅠ/PGⅡratio,can be used as a noninvasive screening method of early gastric carcinoma.

Abstract:Using a 640‐protein microarray to screen novel serum biomarkers in gastric cancer, TGFβ RIII and carboxypeptidase A2 levels were first discovered to be elevated in gastric cancer. The combination of TGFβ RIII and carboxypeptidase A2 showed a high diagnostic value (AUC = 0.801). Gastric cancer (GC) has high rates of morbidity and mortality, and this phenomenon is particularly evident in coastal regions where local dietary habits favor the consumption of pickled foods such as salted fish and vegetables. In addition, the diagnosis rate of GC remains low due to the lack of diagnostic serum biomarkers. Therefore, in this study, we aimed to identify potential serum GC biomarkers for use in clinical practice. To identify candidate biomarkers of GC, 88 serum samples were first screened using a high‐throughput protein microarray to measure the levels of 640 proteins. Then, 333 samples were used to validate the potential biomarkers using a custom antibody chip. ELISA, western blot, and immunohistochemistry were then used to verify the expression of the target proteins. Finally, logistic regression was performed to select serum proteins for the diagnostic model. As a result, five specific differentially expressed proteins, TGFβ RIII, LAG‐3, carboxypeptidase A2, Decorin and ANGPTL3, were found to have the ability to distinguish GC. Logistic regression analysis showed that the combination of carboxypeptidase A2 and TGFβ RIII had superior potential for diagnosing GC (area under the ROC curve [AUC] = 0.801). The results suggested that these five proteins alone and the combination of carboxypeptidase A2 and TGFβ RIII may be used as serum markers for the diagnosis of GC.

Abstract:Objective:To study the relationship between CEA、CAl9-9 and CA125 and gastric cancer and its clinical value.Method:Serum CEA、CAl9-9 and CA125 were measured by immunofluorescence (IFT) respectively in 40 patients with gastric cancer,20 patients with atrophic gastritis with intenstinalmetaplasia and 40 normal individuals.Results:Taking each of the three tumor marker (CEA、CAl9-9 and CA125) as the diagnostic criteria of gastric cancer,the positiverate was 40%,52.5%,65% respectively.The diagnostic rate of combination assay of 3 markers was 92.5% in gastric cancer Conclusion:The measurement of the three tumor markers might be useful in diagnosis in patients with gastric cancer.but also be useful in monitoring response and in predicting the prognosis of patients with gastric cancer treated with operation.

Abstract:Objective To evaluate the clinical significance of combined serum tumor markers assay in patients with gastric or colorectal carcinoma. Methods Serum level of 12 common tumor markers, including CA19-9,NSE,CEA,CA242 Jerritin,Beta-HCG,AFP,free-PSA,PSA,CA125,HGH and CA153, was measured with multi-tumor markers protein biochip detective system in 179 cases of gastric and colorectal carcinoma, 82 patients with benign digestive disease and 160 healthy volunteers. Results Cancer patients had significantly higher positive rates than that of two other controls (P 0. 01) . The levels of 5 serum tumor markers (CA19-9,CEA,CA242,CA125 and CA153) were significantly higher in gastric cancer and colorectal carcinoma than that of two other controls ( P 0. 01). The simultaneously combined assay raised sensitivity (72.07%%) and negative predictive rate (79.25%); the serial combined assay raised specificity (92. 15%) and positive predictive rate (84.67%). The recurrence or metastasis rate was 68. 89% in patients whose serum tumor markers failed to decrease to normal after operation. The recurrence or metastasis rate was 77. 78% in patients whose serum tumor markers reelevated after operation. Conclusion Combined serum tumor markers assay is of value for the diagnosis and postoperative follow up of gastric and colorectal carcinoma.

Abstract:Department of Surgical Oncology, The First Affiliated Hospital, College of Medicine, Zhejiang University, Hangzhou, People’s Republic of China Purpose: To investigate the prognostic value of combined serum carcinoembryonic antigen (CEA) levels and fibrinogen/albumin ratio (FAR) in patients with resectable gastric cancer (GC). Introduction: This retrospective study evaluated the CEA, fibrinogen, and albumin levels and other clinicopathological features of GC patients. The prognostic significance of these factors for overall survival (OS) was assessed using Kaplan–Meier curves and univariate and multivariate Cox proportional models. Patients and Methods: A total of 267 patients were included. The optimal cutoff values of CEA and FAR were 3.2 ng/mL and 0.086, respectively. Patients were stratified into three groups based on this cutoff value: CEA-FAR=0 (CEA <3.2 ng/mL and FAR <0.086), CEA-FAR=1 (CEA ≥3.2 ng/mL or FAR ≥0.086), and CEA-FAR=2 (CEA ≥3.2 ng/mL and FAR ≥0.086). Results: Higher CEA-FARwas strongly associated with age, tumor size, tumor invasion, lymph node status, and TNM stage (all P<0.05). The OS rates differed significantly between these 3 groups (88.9% vs 65.0% vs 46.9%, P<0.001). Multivariate analysis showed that CEA-FAR was an independent prognostic factor for OS (P<0.001). The area under the curve was larger for CEA-FAR than for either CEA or FAR alone (0.683, 0.644, and 0.669, respectively). Conclusion: Preoperative CEA-FAR could be a potential blood marker for predicting tumor progression and the prognosis of GC patients. Patients with a higher CEA-FAR should undergo extensive follow-up.

Abstract:OBJECTIVE:To investigate the clinical value of tumor markers CEA, CA19-9, CA72-4 and CA242 in the diagnosis and prognosis of patients with gastric cancer.METHODS:One hundred and sixty gastric cancer patients who had received treatment from 2002 to 2007 at the Beijing Cancer Hospital were retrospectively analyzed. Blood samples were taken from patients upon admission to the hospital, and CEA, CA19-9, CA72-4, CA242 levels were detected. Statistical analysis was performed to identify the clinical value of these tumor markers in diagnosis and prognosis.RESULTS:On initial diagnosis, the positive rates of CEA, CA19-9, CA72-4 and CA242 were 37.7%, 26.7%, 37.6% and 21.3%, respectively, and the positive rate of combined detection was 62.9%. CEA was more frequently positive in patients with lymph node metastasis (P=0.029); CA72-4 was more frequently positive in patients with vascular involvement and advanced stage (P=0.039, P=0.011). Multivaraite analysis showed that CA72-4 was an independent prognostic factor (P=0.012). Patients with positive CA72-4 carried a 2.147-fold increased risk of death than those with negative CA72-4. Kaplan-Meier analysis showed that patients with positive CA19-9 or positive CA72-4 had worse survival than those with negative CA19-9 or CA72-4 (P=0.006, P=0.002).CONCLUSIONS:Tumor markers including CEA, CA19-9, CA72-4 and CA242 have clinical significance and prognostic value in patients with gastric cancer. Combined detection of four tumor markers can increase the positive rate. CA72-4 is an independent prognostic factor. CA19-9 and CA72-4 are associated with the prognosis of patients with gastric cancer.

Abstract:BACKGROUND/AIMS:To explore the diagnosis value and clinical significance of combined detection of serum pepsinogen I (PG I), pepsinogen II (PG II), PG I/II and CA242 in patients with stomach diseases.METHODOLOGY:Serum PG I, PG II and CA242 were detected with time-resolved fluoroimmunoassay (TRFIA) method. Serum levers of the four markers in gastric carcinoma were compared with that in chronic superficial gastritis, chronic atrophic gastritis, gastric ulcer and normal controls. The four indices were analyzed to judge their diagnosis value and the relationship with the biology behavior of gastric carcinoma.RESULTS:The serum concentration of PG I in gastric carcinoma and in chronic atrophic gastritis were remarkably lower than that in controls (P < 0.05). The serum concentration of CA242 in gastric carcinoma was significantly higher than that in controls (P < 0.05).CONCLUSIONS:To detect the levers of serum PG I, PG II, PG I/II would help to judge the metastasis and prognosis of gastric carcinoma. Combined detection of the four indices could increase the positive rate of diagnosis in gastric carcinoma.

Abstract:AIM: To explore the preliminary identification of serum protein pattern models that may be novel potential biomarkers in the detection of gastric cancer. METHODS: A total of 130 serum samples, including 70 from patients with gastric cancer and 60 from healthy adults, were detected by surface-enhanced laser desorption and ionization time-of-flight mass spectrometry (SELDI-TOF-MS). The data of spectra were analyzed by Biomarker Patterns Software (BPS). Thirty serum samples of gastric cancer patients and 30 serum samples of healthy adults were grouped into the training group to build models, and the other 70 samples were used to test and evaluate the models. The samples of the test group were judged only with their peaks' height and were separated into cancer group or healthy control group by BPS automatically and the judgments were checked with the histopathologic diagnosis of the samples. RESULTS: Sixteen mass peaks were found to be potential biomarkers with a significant level of P<0.01. Among them, nine mass peaks showed increased expression in patients with gastric cancer. Analyzed by BPS, two peaks were chosen to build the model for gastric cancer detection. The sensitivity, specificity, and accuracy of the model were 90%, 36/40, 86.7%, 26/30, and 88.6%, 62/70, respectively, which were greatly higher than those of clinically used serum biomarkers CEA (carcinoembryonic antigen), CA19-9 and CA72-4. Stage I/II gastric cancer samples of the test group were all judged correctly. CONCLUSION: The novel biomarkers in serum and the established model could be potentially used in the detection of gastric cancer. Abstract:e15053 Background: Gastric cancer is the fourth most common cancer worldwide. In China, most cases are diagnosed as metastatic or recurrent gastric cancer (MRGC) with the 5-year survival rate being less than 5 %. Abstract:Background: Gastric cancer is a kind of malignant tumor which is prevalent all over the world. Although some progress has been made in the treatment of gastric cancer, its prognosis is still not optimistic, so it is of great significance to find reliable prognostic indicators to guide the treatment and management of patients with gastric cancer. Aim: To explore the relationship between serum levels of five biomarkers [carcinoembryonic antigen (CEA), carbohydrate antigen (CA) 19-9, CA72-4, CA24-2, and ferritin] and prognosis in patients with gastric cancer. Methods: This study included 200 patients with gastric adenocarcinoma, and conducted an in-depth analysis of their baseline characteristics, relationship between tumor markers and staging, and prognosis. The study found that CA19-9 has a significant correlation with tumor stage, the average levels of CA24-2, CEA, CA72-4 and ferritin were slightly increased disregarding the stage of tumor. Survival analysis showed that increases in CEA, CA19-9, CA24-2, and ferritin were all associated with shortened overall survival of patients. Further multivariate analysis revealed that elevated serum CA72-4 levels were an independent adverse prognostic factor. Results: This study reveals that there is a significant correlation between the expression levels of serum tumor markers CEA, CA19-9, CA72-4, CA24-2 and ferritin in patients with gastric cancer and prognosis, and can be used as important indicators for prognostic evaluation of gastric cancer. In particular, markers that appear abnormally elevated initially may help identify gastric cancer patients with poor prognosis. Conclusion: Serum CEA and CA19-9 play an important role in the prognosis assessment of gastric cancer, and are effective tools to guide clinical practice and optimize individualized treatment strategies for gastric cancer patients.

Abstract:BACKGROUND Gastric cancer is a global health concern that poses a significant threat to human well-being. AIM To detecting serum changes in carcinoembryonic antigen (CEA), carbohydrate antigens (CA) 724, CA242, and CA19-9 expression among patients with gastric cancer. METHODS Eighty patients diagnosed with gastric cancer between January 2020 and January 2023 were included in the observation group, while 80 patients with benign gastric diseases were included in the control group. Both groups were tested for tumor markers (CA724, CEA, CA242, and CA19-9]. Tumor marker indicators (CA724, CEA, CA242, and CA19-9) were compared between the two groups, assessing positive rates of tumor markers across various stages in the observation group. Additionally, single and combined detection of various tumor markers were examined. RESULTS The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value observed for the combined detection of CA724, CEA, CA242, and CA19-9 were higher than those of CA724, CEA, CA242, and CA19-9 individually. Therefore, the combined detection of CA724, CEA, CA242, and CA19-9 has a high diagnostic accuracy and could reduce the occurrence of missed or misdiagnosed cases, facilitating the early diagnosis and treatment of patients. CONCLUSION CA724, CEA, CA242, and CA19-9 serum levels in gastric cancer patients significantly surpassed those in non-gastric cancer patients (P < 0.05). Their combined detection can improve the diagnostic accuracy for gastric cancer, warranting clinical promotion.

Abstract:from NC (AUC=0.79).High serum ANGPTL2 was significantly associated with advanced T stage (p<0.01),lymph node (p=0.0004) and liver metastasis (p = 0.02) in CRC.Furthermore, Kaplan-Meier curves revealed that high serum ANGPTL2 was significantly associated with poor disease free survival (p=0.01) and overall survival (p=0.03) in CRC.Similarly, serum ANGPTL2 levels in GC patients were also significantly higher compared to NC (p<0.0001), had a promising AUC (0.83) to distinguish GC from NC.In addition, serum ANGPTL2 levels also could discriminate early GC patients (stage I) from NC (AUC=0.80).High ANGPTL2 in serum was significantly associated tumor progression, consequently emerged as an independent predicting recurrence marker (HR=5.05,p=0.0004) and prognostic marker (HR=3.6,p=0.01) in GC.Conclusion: Our study first reports that serum ANGPTL2 was a novel biomarker for diagnosis in patients with GC and CRC.In addition, serum ANGPTL2 levels were significantly associated with tumor progression and prognostic outcomes in both GC and CRC.In particular, serum ANGPTL2 was an independent predictive biomarker of recurrence and prognosis in patients with GC.

Abstract:Objective To study the clinical significance of serum sCD44v6 in diagnosis and therapy of the patients with gastric carcinoma. Methods Levels of sCD44v6 in serum samples from 88 patients with gastric carcinoma,40 patients with gastric ulcer and 52 normal subjects were determined by ELISA.Results The mean level of serum sCD44v6 in the patients with gastric caricionoma was 301 4±109 5ng/ml,higher than that in the patients with gastric ulcer and in normal subjects. After radical surgery,the serum sCD44v6 levels were lower than those in the patients with gastric carcinoma before surgery. Our study also demonstrated that the serum sCD44v6 levels were significantly correlated with the phage of disease,the type of tumor,the lymph node metastasis and the depth of invasion.Conclusion The serum levels of sCD44v6 may be useful in diagnosis,assessment of surgery effectiveness and prognosis,especially as a metastasis indicator in the patients with gastric carcinoma.

Abstract:BACKGROUND:Gastric cancer (GC) is a highly aggressive cancer type associated with significant mortality owing to delayed diagnosis and non-specific symptoms observed in the early stages. Therefore, identification of novel specific GC serum biomarkers for screening purposes is an urgent clinical requirement.METHODS:This study recruited a total of 432 serum samples from 296 GC patients split into the mining and testing sets. We aimed to screen for reliable protein biomarkers from matched serum samples based on mass spectrometry, followed by comparison with three representative conventional markers using receiver operating characteristic and survival curve analyses to ascertain their potential values as diagnostic and prognostic biomarkers for GC.RESULTS:We identified an apoC-III fragment with confirmation in an independent test set from a second hospital. We found that the diagnostic ability of this fragment performed better than current standard GC diagnostic biomarkers both individually and in combination in distinguishing patients with GC from healthy individuals. Moreover, we found that this apoC-III protein fragment represents a more robust potential prognostic factor for GC than the three conventional markers.CONCLUSIONS:In view of these findings, we suggest that apoC-III protein fragment is a novel diagnostic and prognostic biomarker, a complement to conventional biomarkers in detecting GC.

Abstract:Purpose To study the expression of osteopontin(OPN) and OCT2 in gastric carcinoma and their correlation with clinicpathologic features and survivial time. Methods Tissue microarray (TMA) technique and immunohistochemistry (IHC) were used to detect the expression of OPN and OCT2 in 99 gastric carcinoma specimens. Results Immunohistochemistry results revealed that: from the 99 cases, the positive rates of OPN and OCT2 protein expression were 60.61 %, 54.55 %, respectively; there were significant relationships of OPN protein expression with tumor diameter, invasion depth, vessel invasion, lymph node and distant metastasis; there were significant relationships of OCT2 protein expression with tumor diameter, invasion depth, vessel invasion, lymph node and distant metastasis; There was a positive relationship of OPN protein with OCT2 protein expression; the mean survival time in cases with OPN and OCT2 protein positive expression was significantly shorter than those without OPN and OCT2 protein expression. Conclusion The expression of OPN and OCT2 protein can predict the invasion and metastasis of gastric carcinoma; they can be used as markers of prognosis of gastric carcinoma in clinical practice.
[Protein Profiles of Serum in Gastric Cancer at a High-Risk Area by SELDI-TOF Mass Spectrometry].

Abstract:OBJECTIVE:To explore the specific and sensitive biomarkers for gastric cancer detection, a surface-enhanced laser desorption and ionization protein chip mass spectrometry (SELDI-TOF-MS) was used to generate protein profiles of serum in gastric cancer at a high-risk area.METHODS:A total of 36 gastric cancer cases and 46 subjects with superficial gastritis were selected from Linqu county, Shandong province, a high-risk area of gastric cancer. Serum samples were collected and Q10 protein chips were used to detect the serum proteomic patterns, and the sensitivity and specificity were assessed.RESULTS:For the comparison of the gastric cancer group (26 out of 36 gastric cancer cases) versus superficial gastritis group (37 out of 46 subjects), the 6 most discriminating peaks (m/z 8587, 6945, 8243, 3899, 7035, and 9943) were identified by the ProteinChip Data Analysis System (ZUCIPDAS). The sensitivity and specificity of this pattern were 88.5% and 97.3%, respectively. A total of 19 subjects (10 gastric cancer cases and 9 superficial gastritis subjects) was selected to test the accuracy of this pattern by using blind method, and the sensitivity and specificity were 80.0% and 88.9% ,respectively.CONCLUSION:Our findings suggest that SELDI profiling of serum might be a potential for gastric cancer detection and screening in high-risk population.

Abstract:Objective: To establish the two-dimensional gel electrophoresis pH 4-7 maps of human gastric carcinoma and paired paracancerous mucosa,and to analyze differently expressed proteins of human gastric carcinoma and paired paracancerous mucosa.Methods: The total proteins of human gastric carcinoma and paired paracancerous mucosa were extracted,separated and analyzed by two-dimensional electrophoresis(2-DE).Results:By comparing and analyzing pH 4-7 protein expression maps of 5 cases of gastric carcinoma and paired paracancerous mucosa,128 differently expressed protein spots were detected,of which 56 expressed only in gastric carcinoma,27 in normal gastric mucosa tissues,32 more highly and 13 more lowly in gastric carcinoma than in paracancerous mucosa tissues. Conclusion: Proteins express differently in gastric carcinoma from paired paracancerous mucosa.Searching for differently expressed proteins between the two types of tissues may throw light on the molecular mechanism of gastric carcinoma.
Proteomic and serological markers for diagnosing cardia gastric cancer and precursor lesions in a Chinese population

Abstract:Cardia gastric cancer (CGC) is prevalent in East Asia, and noninvasive, cost-effective screening methods are needed. This study investigated the diagnostic value of serum pepsinogen (PG), gastrin-17 (G-17), Helicobacter pylori (H. pylori) antibodies, and proteomic profiling for CGC and precancerous lesions. We conducted a case-control study involving biopsy-confirmed patients with CGC (n = 60), low-grade intraepithelial neoplasia (CLGD, n = 60), high-grade intraepithelial neoplasia (CHGD, n = 64), and healthy controls (n = 120) matched for age and sex from high-incidence areas in China. Serological markers including PGI, PGII, G-17, and H. pylori were measured using ELISA and Western blot, while plasma protein markers were assessed using Olink® technology. The VSOLassoBag algorithm and nine machine learning (ML) algorithms were employed to identify crucial features and construct predictive models. Various evaluation metrics, including the area under the receiver-operating-characteristic curve (AUC), were utilized to compare predictive performance. Elevated PGII levels, decreased PGR, and H. pylori infection were significantly associated with an increased risk of CGC and precancerous lesions (P for trend < 0.05). The eXtreme Gradient Boosting (XGBoost) model performed best in discriminative ability among the 9 ML models. Following feature reduction based on predictive performance, a final explainable XGBoost model was developed, incorporating five protein biomarkers (CDHR2, ICAM4, PTPRM, CDC27, and FLT1). This model exhibited excellent performance in distinguishing individuals with CGC and precancerous lesions from healthy controls (AUC = 0.931 for CGC, 0.867 for CHGD, and 0.763 for CLGD), surpassing the traditional serological marker-based model. This study underscores the diagnostic potential of serological markers and proteomic profiling in the detection of CGC. Further validation and exploration of combined biomarker approaches are warranted to enhance early diagnosis and improve outcomes in high-risk populations.
