Co-Blocking Tlr and Cd154 Leads Profound Protection Against Renal Ischemia-Reperfusion Injury in Mice

Abstract:Background: TLR signaling pathway plays a role in renal ischemia reperfusion injury (IRI). Studies have demonstrated blocking TLR signaling can effectively reduce renal IRI. Meanwhile, studies found blocking co-stimulation signaling pathway such as CD154-CD40 can also attenuate renal IRI. This study try to explore if co-blocking both TLR and CD154-CD40 signaling pathways can provide stronger protection against renal IRI. Material and methods: Renal IRI was induced by clamping the left renal pedicle for 80min in Balb/c mice. STTJ(a MyD88 inhibitor) and MR1(antiCD154 monoclonal antibody) were given intraperitoneally 48h, 24h and 4h before renal ischemia. The therapeutic effect was evaluated in terms of survival curve, renal function, histological and immunopathologic examinations. Renal NF-κB activation and the downstream cytokines IL-1, IL-6, TNF-a, ICAM-1, IL-10 in 24h were assessed by ELISA and real-time PCR. Bcl-2, bcl-xl, caspase-3 protein expression were assessed by western blot. Results: The mortality of STTJ group declined from 87.5% (control group) to 62.5%, while the STTJ+MR1 group greatly decreased to 12.5% by day 7(as shown in Fig 1). Compared with saline-treated mice(167.4±19.81), STTJ, MR1 and STTJ+MR1 treated mice exhibited lower serum Cr levels (24.83±17.28, 154.8±8.044, 24.14±8.295; P=0.0005,P=0.3386,P=0.0001 versus control group). The activation of NF-κB was inhibited, so were the downstream cytokines IL-1, IL-6, TNF-a, ICAM-1. However, the IL-10 level was increased remarkably in STTJ+MR1 group. The histological and immunopathologic score of renal tissues show an alleviative injury in STTJ+MR1 group.[Figure 1]Conclusions: These data suggest that blocking either the TLR or CD154 shows gentle protection against renal IRI. While co-blocking both TLR and CD154 can attenuate renal IRI better. This result indicates the innate and acquired immune system involved in IRI simultaneously, and the co-blocking of both is required in treatments against IRI.
