Abstract: Objective To further determine the safety and therapeutic effects of autologous hematopoietic stem cell transplantation (HSCT) in newly diagnosed type 1 diabetes（T1DM）by enlarging sample size and following up. MethodsHematopoietic stem cells were mobilized with cyclophosphamide and granulocyte colonystimulating factor and then collected from peripheral blood by leukapheresis and cryopreserved.The cells were infused intravenously or by splenic arterial cannula after conditioning with cyclophosphamide and rabbit antithymocyte globulin. The daily exogenous insulin requirements， serum levels of hemoglobin A1c， islet β-cell function and anti-islet antibody titers were compared between pre-HSCT and different times following HSCT. The adverse effects during and after AHST were recorded. Results3 patients became insulin free for different periods. At 3， 6 and 12 months after AHST， mean levels of hemoglobin A1c were significantly lower（P<0.05）and mean levels of C-peptide were higher than before treatment. The above four islet antibodies became negative in all patients during different time after AHST. Hypothyroidism was diagnosed in one patient 6 months after AHST.There were no severe adverse effects such as myelosuppression and hemorrhagic cystitis. Conclusions After AHST， beta cell function is increased and blood glucose control is improved in all patients. But additional studies should be performed to confirm whether AHST can be routinely conducted in clinic.

Abstract:Objective To determine the safety and the therapeutic efficacy of autolagous nonmyeloablative hematopoietic stem cell transplantation (AHST) in newly-onset type 1 diabetes mellitus patients. Methods Fifteen patients with type 1 diabetes mellitus were enrolled. Hematopoietic stem cells were mobilized with cyclophosphamide and granulocyte colony-stimulating factor and then collected from peripheral blood by leukapheresis and cryopreserved. The cells were injected intravenously after conditioning with cyclophosphamide and rabbit antithymocyte globulin. Serum levels of HbA1c, C-peptide levels, and anti-glutamic acid decarboxylase antibody (GAD-Ab)titers were measured before and after AHST. Meanwhile, adverse event was recorded.Results The average age of 18 patients (6 males and 12 females)was ( 18.8±4.4 )years, the mean follow-up was ( 414± 150 ) days. 67 % ( 12/18 ) patients became insulin free, the earliest one happened at 2 weeks after AHST, and the latest one at 6 months. 4 cases resumed insulin use because of influenza and other reasons resulting in the rise of blood glucose level. Currently, 8 patients (44.4%) were completely free of insulin therapy, and the remaining cases reduced the insulin dosage by 67.3% ±22.4%. 18 cases had lowered GAD-Ab level, the negative rate was 33.3% (6/18 ). Fasting and postprandial 2 h C-peptide levels increased significantly after A HST. Area under the curve for C-peptide ( AUCC ) increased much more markedly, and it could be maintained for 1 year. Duringtransplantation,all patients had varying degrees of gastrointestinal reactions, hair loss, fever, bone marrow suppression, and other side effects. 5 patients received blood component transfusion. No damage or other severe adverse events of heart, liver, kidney, and other organs were observed. Most side effects gradually disappeared after 2-4 weeks. The recovery of neutropenia was the slowest. Conclusion Autologous hematopoietic stem cell transplantation for treatment of newly-onset type 1 diabetes with residual islet function showed a certain effect and high safety. The widened use of this new technique should be cautious until the therapeutic mechanism has been further studied.

Abstract:Objective To explore the efficacy and safety of autologous hematopoietic stem cell transplantation(AHST) in newly diagnosed type 1 diabetes mellitus.Methods Hematopoietic stem cells were mobilized with cyclophosphamide and granulocyte colonystimulating factor and then collected from peripheral blood by leukapheresis and cryopreserved.The cells were infused intravenously after conditioning with cyclophosphamide and rabbit antithymocyte globulin.To compare the daily dose of exogenous insulin requirements,serum levels of hemoglobin A1c,islet β-cell function during the mixed-meal tolerance test,and anti-islet antibody titers were measured before and at different times following AHST.The adverse effects during and after AHST were recorded.Results The patient has been free from insulin for more than 9 months since AHST.Serum levels of hemoglobin A1c were maintained at less than 7%,and islet β-cell function was greatly improved after AHST.There were no severe adverse effects such as myelosuppression and hemorrhagic cystitis.Conclusions Very encouraging results are obtained in the patient treated with AHST.There is definite therapeutic effects and safety in a short-term.But further follow-up is necessary to confirm the duration of insulin independence and the mechanisms of action of the procedure.

Abstract:Type 1 diabetes is pathologically characterized by autoimmune insulitis-related islet β-cell destruction.Although intensive insulin therapy for patients with type 1 diabetes can correct hyperglycemia,this therapy does not effectively prevent diabetes-related cardiovascular complications.Hence,preservation of natural β-cell function is critical for the prevention of diabetes-related complications.Receut studies have shown that autologous hematopoietic stem cell transplantation ( A HSCT) is a new promising approach for the treatment of type 1 diabetes by reeonstitution of immunotolerance and preservation of islet β-cell function.Here we introduce the key results of a clinical trial ( NCT 01341899,ClinicalTrials.gov) performed in Chinese type 1 diabetes patients ( most of them with diabetic ketoacidosis at onset ) and discuss the potential mechanisms underlying the action of HSCT and its prospects in the clinical management of type 1 diabetes.

Abstract:In this study, we investigate whether bone marrow mononuclear cells (BM-MNC) or peripheral blood mononuclear cells (PB-MNC) have therapeutic efficacy in type 2 diabetes (T2D).Search terms included stem cell, bone marrow cell, peripheral blood cell, umbilical cord blood and T2D in MEDLINE, the Cochrane Controlled Trials Register, EMBASE, the Wanfang Database, the China Science and Technology Periodical Database and China Journal Net.Fifteen trials met our inclusion criteria (n = 497). One group included 266 cases with BM-MNC therapy and the other group contained 231 cases with PB-MNC treatment. Glycosylated hemoglobin was decreased after BM-MNC or PB-MNC therapy compared with that before (12 months: P < 0.001; 6 months: P < 0.001; 3 months: P < 0.05). Fasting plasma glucose was reduced in BM-MNC therapy group compared with control after 12-month follow-up (P < 0.001) and after BM-MNC therapy compared with that before (9 months: P < 0.001) but was not obvious in other stages. Meanwhile, the analysis showed that C-peptide level increased after BM-MNC and PB-MNC therapy compared with the control therapy (12 months: P < 0.001) and with that before therapy (6 months: P < 0.05). Insulin requirement reduction was also observed in patients receiving BM-MNC therapy (3, 6, 9 and 12 months: P < 0.05).To a certain extent, BM-MNC or PB-MNC therapy for T2D demonstrated superiority of glycemic control, increased insulin biosynthesis and elevated insulin secretion from existing β-cells and might prevent islet cell loss.

Abstract:Context: Autologous hematopoietic stem cell transplantation (AHSCT) has the potential to induce clinical remission in patients with newly diagnosed type 1 diabetes. Objective: The objective of the study was to examine the impact of AHSCT on lymphocytes and pancreatic beta-cell function. Design: This was a nonrandomized, open-label prospective study. Patients and Interventions: Thirteen patients with new onset of type 1 diabetes, 10 of them with diabetic ketoacidosis, were subjected to AHSCT with cryopreserved CD34(+) progenitor cells and followed up for 31-54 months. Main Outcome Measures: The numbers of different subsets of lymphocytes and the levels of serum cytokines, islet antibodies, C-peptide, and plasma glycosylated hemoglobin were longitudinally measured. Results: The numbers of different subsets of lymphocytes, except for CD8(+) T cells, in the patients before AHSCT were significantly lower than those in controls. However, all lymphocytes gradually recovered after AHSCT, accompanied by decreased levels of serum autoantibodies, IL-1, IL-17, and TNF-alpha. After AHSCT, 11 of 13 patients required significantly reduced doses of insulin for adequate glycemic control, accompanied by reduced levels of glycosylated hemoglobin but increased C-peptide concentrations. Three patients achieved exogenous insulin independence for 7-54 months. The survival of remaining beta-cells was associated positively with the preexisting beta-cell function but negatively with preexisting autoantibodies (P < 0.05). The numbers of infused CD34(+) cells were positively correlated with the concentrations of serum IL-10, IL-4, TGF-beta, and fasting C-peptide but negatively correlated with the levels of serum TNF-beta and insulin doses after AHSCT (P < 0.05). Conclusion: AHSCT modulated lymphocytes and preserved beta-cell function in Chinese patients with new onset of type 1 diabetes and diabetic ketoacidosis. (J Clin Endocrinol Metab 97: 1729-1736, 2012)

Abstract:BackgroundThis study explored the details of the immune response after autologous hematopoietic stem cell transplantation (AHSCT) treatment in type 1 diabetes mellitus.MethodsPeripheral blood mononuclear cells (PBMCs) from 18 patients with type 1 diabetes mellitus were taken at baseline and 12 months after AHSCT or insulin-only therapy. The lymphocyte proliferation, mRNA expression and secretion of pro-inflammatory and anti-inflammatory cytokines belonging to T-helper type 1 (Th1), T-helper type 17 (Th17) and regulatory T (Treg) cells in PBMC culture supernatants were assessed.ResultsCompared with patients receiving insulin-only treatment, the patients receiving AHSCT treatment showed better residual C-peptide secretion, lower anti-GAD titers and less exogenous insulin dosages after 12 months of follow-up. AHSCT treatment was associated with significantly reduced Th1 and Th17 cell proportions as well as decreased IFN-γ, IL-2, IL-12p40 and IL-17A levels in the PBMC culture supernatants (all P < 0.05). Although there was no significant Treg cell expansion after AHSCT treatment, we observed increased IL-10, TGF-β and Foxp3 mRNA expression and increased TGF-β levels. However, we found no significant changes in the T-cell subpopulations after insulin treatment, except for higher IL-12p40 mRNA expression and a lower proportion of Treg cells.ConclusionsAHSCT treatment was associated with decreased expansion and function of Th1 and Th17 cells, which may explain the better therapeutic effect of AHSCT compared with the traditional intensive insulin therapy.Trial registrationClinicaltrials.gov NCT00807651. Abstract:Objective: Autologous nonmyeloablative hematopoietic stem cell transplantation (AHST) was the first therapeutic approach that can improve beta cell function in type 1 diabetic (T1D) patients. This study was designed to investigate the potential mechanisms involved.Design and methods: We applied AHST to nine T1D patients diagnosed within six months and analyzed the acute responses in peripheral blood for lymphocyte subpopulation as well as for genomic expression profiling at the six-month follow-up.Results: We found six patients obtained insulin free (IF group) and three remained insulin dependent (ID group); C-peptide production was significantly higher in IF group compared to ID group. The acute responses in lymphocytes at six-month follow-up include declined CD3(+)CD4(+), CD3(+)CD8(+) T cell population and recovered B cell, NK cell population in both groups but with no significant differences between the two groups; most immune-related genes and pathways were up-regulated in peripheral blood mononuclear cell (PBMC) of both groups while none of transcription factors for immune regulatory component were significantly changed; the IF group demonstrated more AHST-modified genetic events than the ID group and distinct pattern of top pathways, co-expression network as well as 'hub' genes (eg, TCF7 and GZMA) were associated with each group.Conclusions: AHST could improve the islet function in newly diagnosed T1D patients and elimination of the islet specific autoreactive T cells might be one of the mechanisms involved; T1D patients responded differently to AHST possibly due to the distinct transcriptional events occurring in PBMC.

Abstract:Type 1 diabetes (T1D) is characterized by severe damage to pancreas islet function through immunological attack; therefore, it is also called 'insulin-dependent diabetes'. The present study aimed to evaluate the safety and clinical efficacy of autologous haematopoietic stem cell transplantation (AHSCT) in adolescent patients with newly diagnosed T1D. A phase-II prospective, parallel-assignment, non-randomized trial was conducted from March 2008 to December 2011 with 40 T1D patients, of whom 20 received AHSCT therapy and 20 were treated only with insulin injections. Of these patients, 14 (70%) in the AHSCT group became insulin-independent for 1.5 to 48 months compared with only one patient in the Insulin group. Of these 14 AHSCT patients, 11 relapsed within a median time of 19.5 (range 5.5-1) months and resumed insulin use. By the end of the 4-year follow-up, the difference in daily insulin dosages between the AHSCT and Insulin groups had become smaller (0.49±0.32IU/kg/day vs. 0.79±0.18IU/kg/day, respectively; P<0.01). C-peptide levels increased significantly at 3 months in both groups and later decreased, with the insulin group showing more rapid deterioration. Most of the adverse events in the AHSCT group were transplantation complications. Our data suggest that AHSCT treatment was well tolerated and slowed deterioration of islet β-cell function while significantly decreasing daily insulin dosages. However, because of the high relapse rate, more information on longer-term outcomes is needed before AHSCT can be routinely considered for T1D patients.SIGNIFICANCE:although this was a non-randomized clinical study, this phase-II trial demonstrated the beneficial effects of AHSCT in patients with newly diagnosed T1D by increasing C-peptide levels and inducing insulin independence, while showing its safety and good tolerability compared with conventional intensive insulin therapy. Thus, these results are helpful for increasing our understanding of the use of haematopoietic stem cell therapy in the treatment of T1D and for evaluating whether it can become more widespread in future.

Abstract:The direetion of "radical" treatment ( complete remission ) for type 1A diabetes is to correct immune disorder and to full repair damaged pancreatic islet cells.It includes the application of immune modulators and islet β-cell replacement therapy.Autologous hematopoietic stem cell transplantation (AHSCT) is a new promising approach for the treatment of type 1A diabetes by reconstitution of immunotolerance and promoting β-cell regeneration.The candidates for AHSCT are limited to the people with early-onset diabetes and keeping obvious residual islet β-cell function.Because of the potential mechanisms underlying the action of AHSCT is still not very clear,careful balancing of the pros and coins of A HSCT is still needed by further trials and intensive studies.

Abstract:The efficacy of stem cell (SC) transplantation in patients with type 1 diabetes mellitus (T1DM) has remained to be fully elucidated. In the present study, a systematic review and meta-analysis was performed to determine the clinical outcomes. Electronic databases, including PubMed, MEDLINE, WanFang and the Cochrane Library were screened for relevant studies published until January 13, 2018. The references of retrieved papers, systematic reviews and trial registries were manually screened for additional papers. Two authors were involved in screening the titles in order to select eligible studies, extract data and assess the risk of bias. Studies were pooled using a random-effects model as well as the Begg's funnel plot and subgroup analysis was performed using Stata 14.0 software. A total of 47 studies were retrieved for detailed evaluation, of which 22 met the inclusion criteria. No substantial publication bias was identified. The meta-analysis revealed that SC therapy increased C-peptide levels when compared with placebo treatment in randomized-controlled trials [RCT; standardized mean difference (SMD), 0.93; 95% confidence interval (CI) 0.23-1.63] and self-controlled trials (SMD, 0.66; 95% CI, -0.22 to 1.54). An analysis demonstrated that SC therapy was more efficient at reducing the glycated hemoglobin level compared with the control group in RCTs (SMD, 0.56; 95% CI; 0.06-1.06; and SMD, 1.63; 95% CI, 0.92-2.34, respectively). The graphs demonstrated that SC transplantation resulted in a reduction of insulin requirement. Furthermore, subgroup analyses revealed that patient age, medical history and the SC injection dose may be sources of the heterogeneity observed. The greatest benefit of SC transplantation was seen in patients aged ≥18 years or a medical history of <3 months. In addition, the SC injection dose of ≥107 IU/kg/day was more effective than <107 IU/kg/day when the cellular composition included mesenchymal SCs and hematopoietic SCs. In conclusion, SC therapy represents an efficient option for patients with T1DM. This systematic review was registered at the International prospective register of systematic reviews (no. 42018093930).

Abstract:Objective:To explore the efficacy and safety of non-myeloablative autologous peripheral hematopoietic stem cell (HSC) and single allogeneic umbilical cord mesenchymal stem cell (MSC) transplantation in the treatment of patients with type 1 diabetes mellitus (T1DM).Methods:This was a retrospective case-control study. Nine participants receiving HSC transplantation and 8 participants receiving MSC transplantation during August 2006 and December 2010 in Department of Endocrinology, Nanjing Drum Tower Hospital Clinical College of Xuzhou Medical University were selected for analysis, meanwhile 10 matched participants who received insulin therapy alone during the same time period were enrolled as the control. Insulin injection dose, glycated hemoglobin A 1c (HbA 1c), fasting C-peptide (FCP) and 2h-postprandial C-peptide (2hCP) were collected before and 12 months after transplantation, and the change range of each index was calculated. Clinical remission was defined as a 10% increase from baseline in the levels of FCP and/or 2hCP. The adverse reactions of patients during the two kinds of stem cell transplantation and follow-up were observed to judge their safety. T-test, one-way analysis of variance, Mann-Whitney U test, Kruskal-Wallis H test, Fisher′s exact test were used to compare between the groups. Results:At the 12 th month of follow-up, the ratio of clinical remission in the HSC-treated, MSC-treated and control group were 7/9, 2/8, and 1/10 respectively, with HSC-treated group showing significantly higher ratio than that of control group ( P=0.005). By the end of follow-up, the HbA 1c of the three groups were (6.5±1.3)%, (7.7±1.5)% and (9.2±3.5)%, respectively. The decrease of insulin injection dose and increase of FCP in HSC transplantation group were significantly higher than those in control group, and the increase of 2hCP in HSC transplantation group was significantly higher than that in MSC transplantation group and control group ( P<0.05). Two recipients of HSC-treated group achieved insulin independence, but none in the MSC-treated group or control group. Compared to baseline in each group, HbA 1c ( t=-3.85, P=0.008) and insulin injection dose ( t=-2.47, P=0.039) were decreased and 2hCP ( Z=-2.07, P=0.039) were increased at the end of follow-up in HSC-treated group, while HbA 1c ( t=-3.11, P=0.017) and 2hCP ( Z=-2.38, P=0.016) were decreased in MSC-treated group. Considering the safety of stem cell transplantation, all recipients in the HSC-treated group experienced some adverse events, while none presented in the MSC-treated group. Conclusions:Both stem cell therapies improved glycemic control and preserved residue β-cell function in T1DM patients. HSC transplantation had a tendency to be superior to MSC transplantation in improving β-cell function, while the latter presented a higher safety profile.

Abstract:Autoimmune insulitis is the main pathological characteristic of type 1 diabetes. Immunotherapies can effectively prevent and/or extenuate autoimmune insulitis by modulating different effectors in the autoimmune response. Stem cells can be induced into insulin-secreting cells, also they can regulate immune function. Here reviews the recent studies on immunotherapies and stem cell transplantation for the treatment of type 1 diabetes.

Abstract:Previous studies have shown that several types of stem cells can differentiate into insulin-secreting islet beta-cells and that these cells can reduce blood glucose in some trials, but there has been no report of a long-term follow-up. We assessed the long-term effects of the use of autologous bone marrow mononuclear cells in the treatment of type 2 diabetes mellitus (T2DM). Based on the willingness to receive implantation of bone marrow mononuclear cells, One hundred and eighteen patients with T2DM were divided into two groups; the patients in group I were treated with autologous bone marrow mononuclear cells and patients in group II were treated with insulin intensification therapy. Mononuclear cells from bone marrow were injected back into the patient's pancreas via a catheter. Patients were followed-up after the operation at monthly intervals for the first 3 months and thereafter every 3 months for the next 33 months, the occurrence of any side effects and the results of laboratory examinations were evaluated. There were no reported acute or chronic side effects in group I and both the HbA1c and C-peptide in group I patients were significantly better than either pretherapy values or group II patients during the follow-up period. These data suggested that the implantation of autologous bone marrow mononuclear cells for the treatment of T2DM is safe and effective. This therapy can partially restore the function of islet beta-cells and maintain blood glucose homeostasis in a longer time.

Abstract:Islet transplantation has developed rapidly in the recent years and has obtained certain good clinical effect,this paper reviewed the effect of islet transplantation in the treatment of type 1 dia-betes,especially in blood glucose control and risk of the process,compared the merits and demerits of islet transplantation and pancreas transplantation,and summed up the currently deficiencies of the islet transplantation and the outlook of the future.

Abstract:Background: This meta-analysis was first conducted to evaluate the efficacy and safety of transplantation of mesenchymal stem cells in the treatment of type 1 and type 2 diabetes mellitus (T1DM and T2DM). Methods: We systematically searched PubMed, ScienceDirect, Google Scholar, CNKI, EMBASE, Web of Science, MEDLINE, and the Cochrane Library for studies published from the establishment of the databases to November 2020. Two researchers independently screened the identified studies, based on inclusion and exclusion criteria. The combined standard mean difference (SMD) and 95% confidence interval (CI) of data from the included studies were calculated using fixed- or random-effects models. Results: We included 10 studies in our meta-analysis (4 studies on T1DM and 6 on T2DM, with 239 participants) to examine the efficacy of mesenchymal stem cells (MSCs) therapy in the treatment of diabetes mellitus. According to the pooled estimates, the glycated hemoglobin (HbA1c) level of the MSC-treated group was significantly lower than it was at baseline (mean difference (MD) = -1.51, 95% CI -2.42 to -0.60, P = 0.001). The fasting C-peptide level of the MSC-treated group with T1DM was higher than that of the control group (SMD = 0.89, 95% CI 0.36 to 1.42, P = 0.001), and their insulin requirement was significantly lower than it was at baseline (SMD = -1.14, 95% CI -1.52 to -0.77, P < 0.00001). Conclusion: Transplantation of mesenchymal stem cells has beneficial effects on diabetes mellitus, especially T1DM, and no obvious adverse reactions.
