Abstract:Introduction: We analyzed trends of major causes of death in Tianjin, China, from 1999 through 2004 to better inform disease prevention and control programs and policies. Methods: To report all-cause deaths among Tianjin residents from 1999 through 2004, we standardized mortality rates to the world population in 2000. We analyzed age, sex, and geographic distribution of deaths from different causes and the leading causes of death in Tianjin. Results: The 5 leading causes of death in Tianjin were cardiovascular disease, cerebrovascular disease, malignant neoplasm, chronic lower respiratory disease, and injuries and poisoning. Mortality in Tianjin declined from 0.60% in 1999 to 0.48% in 2004. Noncommunicable diseases accounted for more than 80% of all deaths. Infant and maternal mortality in Tianjin were low. Life expectancy of Tianjin residents increased every year but was consistently longer in women. When deaths from the main chronic diseases are not considered, life expectancy lengthens substantially. Conclusion: Chronic diseases are the leading cause of death in Tianjin, China. China should commit additional resources to supporting chronic disease prevention and control programs, including proven special health promotion projects.
Spatial and Temporal Trends in the Mortality Burden of Air Pollution in China: 2004-2012.

Abstract:While recent assessments have quantified the burden of air pollution at the national scale in China, air quality managers would benefit from assessments that disaggregate health impacts over regions and over time. We took advantage of a new 10 x 10 km satellite-based PM2.5 dataset to analyze spatial and temporal trends of air pollution health impacts in China, from 2004 to 2012. Results showed that national PM2.5 related deaths from stroke, ischemic heart disease and lung cancer increased from approximately 800,000 cases in 2004 to over 1.2 million cases in 2012. The health burden exhibited strong spatial variations, with high attributable deaths concentrated in regions including the Beijing-Tianjin Metropolitan Region, Yangtze River Delta, Pearl River Delta, Sichuan Basin, Shandong, Wuhan Metropolitan Region, Changsha-Zhuzhou-Xiangtan, Henan, and Anhui, which have heavy air pollution, high population density, or both. Increasing trends were found in most provinces, but with varied growth rates. While there was some evidence for improving air quality in recent years, this was offset somewhat by the countervailing influences of in-migration together with population growth. We recommend that priority areas for future national air pollution control policies be adjusted to better reflect the spatial hotspots of health burdens. Satellite-based exposure and health impact assessments can be a useful tool for tracking progress on both air quality and population health burden reductions. (C) 2016 Published by Elsevier Ltd.

Abstract:Abstract Objective Compare the urban-rural disparity in cancer mortality and changing trend during the past 18 years in Tianjin, China. Methods Cancer death data were obtained from Tianjin All Cause of Death Registration System (CDRS), which covers the whole population of Tianjin. We calculated and compared the constituent ratio of cancer deaths, age-standardized mortality rate(ASR)and changing trends between urban and rural areas. Results From 1999 to 2016, a total of 245,744 cancer deaths were reported, accounting 21.7% of all deaths in Tianjin. The ASR of total cancer mortality was higher in urban areas than in rural areas. A total of 33,739 persons were avoided dying of cancers in rural area compared to the urban death level from 1999 to 2016, which was 40.1% compare to the current level of rural areas. But the gap between urban and rural areas became narrowed gradually. The urban-rural ratios (urban/rural) of total cancer mortality changed from 1.76 (125.7/71.5)[95%CI,1.67,1.84] in 1999 to 1.11 (99.6/90.0)[95%CI,1.06,1.15] in 2016. The ASR of lung, liver and esophagus cancer became higher in rural areas than in urban areas in 2016. Conclusion Cancer transition was obviously occurred in Tianjin and showed different speeds and big gap between urban and rural areas. Much more attention was needed to pay in rural areas which still have increasing trends in most cancers mortality recently.

Abstract:Objective. To examine and describe coronary heart disease (CHD) mortality and its pattern of change (trend) by sex, age, and area of residence (urban versus rural) in Tianjin, China, within the context of epidemiological transition, and compare it with current trends in the Americas and Europe.Methods. A total of 104 393 cases of CHD death in Tianjin occurring between 1999 and 2008 were monitored. Death due to CHD was coded using International Classification of Diseases (ICD) standards (ninth and tenth revisions). Standardized CHD mortality rates and their trends were analyzed by age, sex, and urban versus rural residence.Results. During the 10-year study period, the proportion of total deaths due to CHD in Tianjin increased significantly (from 16% to 24%) and age-standardized CHD mortality increased slightly (with no statistical differences), in contrast to CHD mortality trends in various countries in the Americas and Europe, which are declining. No difference was found in Tianjin's CHD mortality trend by sex. Overall CHD mortality was consistently higher among older age groups, males, and residents of urban areas. The proportion of CHD deaths occurring outside hospitals was 55.81%, with a declining trend over the study period. Rural areas had a higher proportion of outside-hospital CHD mortality than urban areas, but no difference was found across age groups.Conclusions. From 1999 to 2008, CHD mortality in Tianjin varied by sex, age, and urban versus rural area of residence. Future research to identify CHD risk factors and the populations most vulnerable to the disease is recommended to help strengthen CHD prevention. Strategies for CHD control similar to those used in various developed countries in the Americas and Europe should be developed to reduce the CHD burden in China.
Analysis on cause of death in residents of Minhang District of Shanghai from 1996 to 2015

Abstract:Objective To analyze changes and trends of the mortality and causes of death in the residents of Minhang District of Shanghai from 1996 to 2015.Methods Crude death rates (CDR) and age-standardized death rates (ADR) were calculated,respectively.Joinpoint regression was used to analyze the trends in the leading causes of death.Permutation test was used to find whether the joinpoints were statistically significant based on P value＜0.05.Results The elderly population in Minhang District accounted for 18.07％ of the total population in 2015,which increased by 73.89％ than it in 1996.The CDRs of all causes for resent 20 years gradually increased with the annual percentage change (APC) of 0.62％ (P＜0.05),but decreased significantly after standardization (APC =-3.73％,P＜0.05).In 2015,the top five causes of death were circulatory disease;neoplasms;respiratory disease;endocrine,nutritional and metabolic diseases;injury and poisoning in the total population,males and females registered in Minhang District.ADRs of circulatory disease,neoplasms,respiratory disease and injury and poisoning decreased to-3.16％,-1.86％,-8.03％ and -4.96 ％,respectively (P＜0.05).ADRs of endocrine,nutritional and metabolic diseases significantly increased during 1996 to 2001 (APC=16.58％,P＜0.05) and thereafter remained stable.Conclusions The issue of population aging in Minhang District is getting worse,and chronic non-communicable diseases and injury and poisoning can be identified as major public health concerns at present.

Abstract:BACKGROUND: China has experienced a remarkable epidemiological and demographic transition during the past three decades. Far less is known about this transition at the subnational level. Timely and accurate assessment of the provincial burden of disease is needed for evidence-based priority setting at the local level in China.METHODS: Following the methods of the Global Burden of Disease Study 2013 (GBD 2013), we have systematically analysed all available demographic and epidemiological data sources for China at the provincial level. We developed methods to aggregate county-level surveillance data to inform provincial-level analysis, and we used local data to develop specific garbage code redistribution procedures for China. We assessed levels of and trends in all-cause mortality, causes of death, and years of life lost (YLL) in all 33 province-level administrative units in mainland China, all of which we refer to as provinces, for the years between 1990 and 2013.FINDINGS: All provinces in mainland China have made substantial strides to improve life expectancy at birth between 1990 and 2013. Increases ranged from 4.0 years in Hebei province to 14.2 years in Tibet. Improvements in female life expectancy exceeded those in male life expectancy in all provinces except Shanghai, Macao, and Hong Kong. We saw significant heterogeneity among provinces in life expectancy at birth and probability of death at ages 0-14, 15-49, and 50-74 years. Such heterogeneity is also present in cause of death structures between sexes and provinces. From 1990 to 2013, leading causes of YLLs changed substantially. In 1990, 16 of 33 provinces had lower respiratory infections or preterm birth complications as the leading causes of YLLs. 15 provinces had cerebrovascular disease and two (Hong Kong and Macao) had ischaemic heart disease. By 2013, 27 provinces had cerebrovascular disease as the leading cause, five had ischaemic heart disease, and one had lung cancer (Hong Kong). Road injuries have become a top ten cause of death in all provinces in mainland China. The most common non-communicable diseases, including ischaemic heart disease, stroke, chronic obstructive pulmonary disease, and cancers (liver, stomach, and lung), contributed much more to YLLs in 2013 compared with 1990.INTERPRETATION: Rapid transitions are occurring across China, but the leading health problems and the challenges imposed on the health system by epidemiological and demographic change differ between groups of Chinese provinces. Abstract:BackgroundIn the 12th Five-Year Plan, the Chinese government set the goal of increasing life expectancy by one year. The purpose of this study is to examine the impact of major causes of death on the life expectancy of the Chinese people between 1950 and 2010 and predict changing trends to identify major issues requiring future attention.MethodsA continuous database organised by population and death data on diseases by age group between 1950 and 2010 were created from A Province in Eastern China. The diseases were classified into four categories by the International Classification of Diseases-10 (ICD-10): infectious and parasitic diseases, chronic diseases, accidental injuries, and maternal diseases. Potential gains in life expectancy (PGLEs) were applied to reflect the impact on life expectancy caused by deaths from various diseases, by using the cause-eliminated life table.ResultsThe PGLEs of infectious and parasitic diseases decreased from 15.59 years in 1950, to 0.07 year in 2010, and have remained low since 2000. However, the PGLEs of chronic diseases increased from 8.70 years in 1950, to 13.36 years in 2010, and indicated an increasing future trend. The two opposite trends exhibited a ‘scissors-like difference’. The proportion of accidental injuries and maternal diseases in the death spectrum was low. The PGLEs of accidental injuries decreased from 2.95 years in 1950, to 0.86 year in 2010, maintaining a low level, while the PGLEs of maternal diseases dropped from 0.56 to 0.002 year during the same period, approaching zero.ConclusionsThe findings of this study provide useful information, which could contribute to a more effective allocation of public health programmes. In recent years, chronic diseases and accidental injuries have emerged as major factors influencing life expectancy. Primary and secondary prevention actions, such as public education, modification of behaviours, and introduction of safety measures should be emphasised in efforts to promote life expectancy. The morbidity and mortality rates of infectious, parasitic, and maternal diseases should be maintained at low levels.

Abstract:The aim of the study was to determine whether the area of Tanggu, Tianjin Binhai New Economic Developing Area, China, is subject to similar effects of ambient particulate matter less than 10 micrometres in aerodynamic diameter (PM10) similar to other areas of China. This study was designed to investigate cause-specific mortality risks associated with air pollution in this geographical region. The present study used a time-series analysis to explore the relationship between PM10 and the cause-specific mortalities for non-accidental, cardiovascular, and cardiopulmonary mortality from 1 January 2006 to 31 December 2010. A 10 μg/m3 increment of PM10 was associated with a 1.02% (95% confidence interval (CI): 0.48, 1.56) increase in cardiovascular mortality, and a 0.88% (95% CI: 0.36, 1.39) increase in cardiopulmonary mortality. In addition, the effects from PM10 appear to be consistent with multi-pollutant models. The results show that there are strong associations between daily cardiovascular and cardiopulmonary mortality and ambient PM10 exposure.
Trend analysis of lung cancer mortality and years of life lost (YLL) rate from 1999 to 2016 in Tianjin, China: Does the lung cancer burden in rural areas exceed that of urban areas?

Abstract:BackgroundThe aim of this study was to examine the trends in the mortality rate and years of life lost (YLL) rate of lung cancer in Tianjin, China, during the period from 1999 to 2016. MethodsLung cancer death data were obtained from Tianjin residents' all‐cause death monitoring system, which covers the whole population of Tianjin. Crude mortality rate, age‐standardized mortality rate, truncated rate (35–64 years), YLL and age‐standardized YLL rate data were calculated and trends examined. ResultsFrom 1999 to 2016, a total of 93 358 lung cancer deaths were reported in Tianjin, which accounted for 38.0% of all cancer deaths (93 358/245744). The crude mortality rate of lung cancer had increased 58.5% from 1999 (40.15/100000) to 2016 (63.64/100000), average annual percent change (AACP) = 2.9%, P < 0.01. However, the age‐standard YLL rate had decreased to 13.3% in 2016 than in 1999, AACP = –0.8%, P < 0.01, with a stable trend in males (AACP = –0.2%), and noticeable decreasing trend in females (AACP = –1.4%). The lung cancer mortality rate (ASRW) in urban areas was higher than that in rural areas in 1999, with a ratio of 1.99:1. However, it was lower in 2016, with the ratio of 0.98:1. For the truncated rate (35–64 years), it had decreased in urban areas compared with rural areas since the year 2013. ConclusionLung cancer remains the most fatal cancer in Tianjin. However, the age‐standard YLL rate of lung cancer has decreased considerably accompanied by a decline in smoking rate years ago, especially in women and people living in urban areas. Considerable attention is therefore needed in the rural areas where cases of lung cancer are still rapidly increasing.

Abstract:Background: Many studies have illustrated that ambient air pollution negatively impacts on health. However, little evidence is available for the effects of air pollution on cardiovascular mortality (CVM) in Tianjin, China. Also, no study has examined which strata length for the time-stratified case-crossover analysis gives estimates that most closely match the estimates from time series analysis.Objectives: The purpose of this study was to estimate the effects of air pollutants on CVM in Tianjin. China, and compare time-stratified case-crossover and time series analyses.Method: A time-stratified case-crossover and generalized additive model (time series) were applied to examine the impact of air pollution on CVM from 2005 to 2007. Four time-stratified case-crossover analyses were used by varying the stratum length (Calendar month, 28, 21 or 14 days). Jackknifing was used to compare the methods. Residual analysis was used to check whether the models fitted well.Results: Both case-crossover and time series analyses show that air pollutants (PM10, SO2 and NO2) were positively associated with CVM. The estimates from the time-stratified case-crossover varied greatly with changing strata length. The estimates from the time series analyses varied slightly with changing degrees of freedom per year for time. The residuals from the time series analyses had less autocorrelation than those from the case-crossover analyses indicating a better fit.Conclusion: Air pollution was associated with an increased risk of CVM in Tianjin, China. Time series analyses performed better than the time-stratified case-crossover analyses in terms of residual checking. (C) 2010 Elsevier B.V. All rights reserved.

Abstract:PP-31-032 Background/Aims: To study the association between particulate matter less than 10 μm in aerodynamic diameter (PM10) and daily mortality in urban population in Tianjin. Methods: We collected air quality data, daily mean temperature, relative humidity, and daily cause-specific death counts, and used generalized additive models to explore the relationship between ambient particulate matter and daily mortality, after adjusting the effects of long-term and seasonal trend, weather conditions and other gaseous pollutants, such as sulfur dioxide and nitrogen dioxide. Results: An increase of 10 μg/m3 in PM10 was associated with 0.45% (95% CI: 0.21%–0.69%) nonaccidental morality, 0.60% (0.29%–0.91%) circulatory morality, and 0.82% (0.04%–1.61%) respiratory morality, respectively. Conclusion: The findings indicate that exposure to PM10 is significantly associated with daily mortality in urban population in Tianjin, especially with the circulatory mortality and respiratory mortality.

Abstract:Objective: To analyze the changing trend of mortality of malignant tumors in Pudong New Area in Shanghai from 1993 to 2007. Methods: The data from Pudong New Area Diseases Surveillance and Death-Reporting System in 1993-2007 were collected. The mortality rates and age-standardized rates (ASR) by world population were calculated. The annual percent changes (APC) in mortality were estimated by means of a logarithmic linear regression. Results: The mortality of malignant tumors in Pudong New Area increased from 1993 to 2007, but the ASR by world population decreased. The mortality rates for lung cancer, colon cancer, and female breast cancer increased. The standardized mortality rates for colon cancer and female breast cancer increased as well. The mortality rates for female stomach cancer, liver cancer and esophageal cancer decreased. The standardized mortality rates for male lung cancer, stomach cancer, liver cancer and esophageal cancer decreased. Conclusion: Attention should be especially focused on the malignant tumors including lung cancer, colon cancer and breast cancer in cancer prevention and control in Pudong New Area in Shanghai. Abstract:Background: Ambient fine particulate matter (PM2.5) pollution is currently a most severe and worrisome environmental problem in China. However, current knowledge of the health effects of this pollution is insufficient.Objectives: This study aims to provide an overall understanding regarding the long-term mortality effects of current PM2.5 pollution in China and the potential health benefits of realizing the goals stipulated in the ongoing action plan of Air Pollution Prevention and Control (APPC) and the targets suggested by the WHO.Methods: Three typical causes and all-cause of PM2.5-related mortality were considered. The log-linear exposure-response function was adopted, and a meta-analysis was used to determine the exposure-response coefficients, based on newly available data in China and abroad.Results: In the 74 leading cities of China, approximately 32% of the reported deaths, with a mortality rate of 1.9%, were associated with PM2.5 in 2013, in which deaths from cardiovascular, respiratory and lung-cancer causes accounted for 20% of the reported deaths, with a mortality rate of 1.2%. The regional difference is remarkable for the mortalities and proportions of the different causes. If the PM2.5 concentration goals of the APPC plan, the first interim and the guideline targets of the WHO could be achieved, the PM2.5-related all-cause mortality would be reduced by 25%, 64% and 95%, respectively, compared with that of 2013.Conclusions: PM2.5 pollution in China has incurred great health risks that are even worse than those of tobacco smoking. The health benefits of the APPC plan could be outstanding, although there is still great potential to improve future air quality. (C) 2016 Elsevier B.V. All rights reserved.
Long-Term Changes in Premature Death of Lung Cancer in a Developed Region of China: A Population-Based Study From 1973 to 2019

Abstract:Abstract Background: Lung cancer is a leading cause of deaths worldwide, and its incidence shows an upward trend. The study in the long-term changes in premature death of lung cancer in a developed region of China has a great exploratiory significance to further clarify the effectiveness of intervention measures.Methods: Cancer death data were collected from the Mortality Registration System of Shanghai Pudong New Area (PNA). We analysed crude mortality rate (CMR), age-standardized mortality rate by Segi’s world standard population (ASMRW), and years of life lost (YLL) of lung cancer from 1973 to 2019. Temporal trends of CMR, ASMRW, and rate of YLL were calculated by joinpoint regression expressed as an annual percent change (APC) with corresponding 95% confidence interval.Results: 80,543,137 person-years were enrolled in this study in PNA from 1973 to 2019. There were 42,229 deaths in permanent residents from lung cancer. The CMR and ASMRW were 52.43/105 person-years and 27.79/105 person-years. YLL due to premature death from lung cancer was 481779.14 years, and the rate of YLL was 598.16/105 person-years. The CMR, and rate of YLL for deaths had significantly increasing trends in males, females, and the total population (P<0.001). The CMR in the total population increased by 2.86% (95% CI=2.66%-3.07%, P < 0.001) per year during the study period. The YLL rate increased with an APCC of 2.21% (95% CI = 1.92% to 2.51%, P < 0.001) per year. The contribution rates of increased values of CMR caused by demographic factors were more evident than non-demographic factors.Conclusion: The aging population, progress of treatment, smoking, and even environmental pollution may be the factors that affect the long-term changes in premature death of lung cancer in PNA from1973 to 2019. Our research can help us to understand the changes of lung cancer mortality, and our results could also be used for other similar cities in designing future prevention plans.
Urban-rural comparisons in health risk factor, health status and outcomes in Tianjin, China: A cross-sectional survey (2009-2013)

Abstract:Objective: To investigate the urban-rural disparities in health risk factors, health status and outcomes in Tianjin, China and to make an international comparison with urban-rural health in Australia. Design: A descriptive analytical cross-sectional survey. Setting: Mobile research teams conducted door-to-door field surveys of each house or department. The teams included local administrative staff and Tianjin Center for Disease Control and Prevention's epidemiologists, clinicians and laboratory technicians. Participants: A total of 25 288 residents were interviewed and clinically observed, including 8583 urban residents and 16 705 rural residents. Main outcome measure: Health risk factors, health status and outcomes. Results: The age structure in urban areas of Tianjin was growing older. Rural residents received less high school education and university education than did urban residents. Urban residents had higher medical insurance coverage and paid more out-of-pocket medical expenditures than did rural residents. The prevalence of smoking and the crude alcohol consumption rate were higher in rural areas than in urban areas. Rural residents had feelings of higher self-satisfaction concerning their health status than did urban residents. The prevalence of hypertensive disease, type 2 diabetes and heart, stroke and vascular diseases were significantly lower in rural areas than in urban areas. The incidence rate of serious injuries resulting from traffic accidents was significantly higher in rural areas than in urban areas. Conclusion: Contrary to Australian urban-rural survey outcomes, the health status and outcomes of residents in rural areas of Tianjin seemed to be better than those of their counterparts in urban areas. The underlying determinants of these outcomes need to be explored with further study.

Analysis of injury death trends among women in Macheng City, China, 1984-2008

Abstract:Abstract Background There are few studies on trends in injury death rates in China during the recent decades of tremendous change in this society. This paper examined trends in injury mortality rates among women aged 15 years or older in Macheng City from 1984-2008. Methods Data on injury deaths in women from 1984 to 2008 were obtained from the Death Registry System in Macheng City. Injuries were classified using the International Classification of Diseases (ICD), 9th and 10th editions. Results The average overall injury death rate of women aged 15 or older was 87.6/100,000 in Macheng City from 1984-2008. Injuries were the leading cause of death for women aged 15-44 and the fourth cause of death for all women during this period. The all-cause injury mortality rate decreased from 149.01 per 100,000 population in 1984 to 32.90 per 100,000 population in 2008 for women. Road traffic injury (RTI) was the only injury for which the mortality rate increased dramatically from 1984 (1.35 per 100,000) to 2008 (4.63 per 100,000). For all age groups, suicide was the leading cause of injury death. For women aged 15 to 64, RTI and drowning were the second and third leading causes of injury deaths, respectively. Conclusions The injury mortality rate for women aged 15 years or older decreased by 77.92% from 1984-2008. In contrast, RTI deaths increased sharply in the 2000s compared with the 1980s. Although the suicide rate decreased dramatically, it was still the leading cause of injury death for women. Research is needed to identify risk factors contributing to the increase in RTI and decrease in suicides.

Coarse Particles and Mortality in Three Chinese Cities: the China Air Pollution and Health Effects Study (Capes)

Abstract:Evidence concerning the health risks of coarse particles (PM10-2.5) is limited. There have been no multi-city epidemiologic studies of PM10-2.5 in developing Asian countries. We examine the short-term association between PM10-2.5 and daily mortality in three Chinese cities: Beijing, Shanghai, and Shenyang. PM10-2.5 concentrations were estimated by subtracting PM2.5 from PM10 measurements. Data were analyzed using the over-dispersed generalized linear Poisson models. The average daily concentrations of PM10-2.5 were 101μg/m3 for Beijing (2007–2008), 50μg/m3 for Shanghai (2004–2008), and 49μg/m3 for Shenyang (2006–2008). In the single-pollutant models, the three-city combined analysis showed significant associations between PM10-2.5 and daily mortality from both total non-accidental causes and from cardiopulmonary diseases. A 10-μg/m3 increase in 1-day lagged PM10-2.5 was associated with a 0.25% (95% CI: 0.08 to 0.42) increase in total mortality, 0.25% (95% CI: 0.10 to 0.40) increase in cardiovascular mortality, and 0.48% (95% CI: 0.20 to 0.76) increase in respiratory mortality. However, these associations became statistically insignificant after adjustment for PM2.5. PM2.5 was significantly associated with mortality both before and after adjustment for PM10-2.5. In conclusion, there were no statistically significant associations between PM10-2.5 and daily mortality after adjustment for PM2.5 in the three Chinese cities.
