Parapharyngeal Metastasis is Not an Uncommon Finding in Differentiated Thyroid Carcinoma after the Use of 131I-Spect/ct

Abstract:391 Objectives Metastasis to parapharyngeal space is a rare event in differentiated thyroid carcinoma (DTC). Very few cases have been reported in the past literature. 131I-SPECT/CT allows better localization and definition of 131I whole-body scan(131I-WBS) findings for metastases in DTC. The aim of the present study is to assess the value of 131I-SPECT/CT for the diagnosis of parapharyngeal metastasis in DTC. Methods Consecutive patients with DTC (n=561) who underwent total thyroidectomy were treated with 131I for the ablation of remnant and treatment of metastases from April 2003 to October 2008. 131I-SPECT/CT was performed when there were abnormal findings indicative of parapharyngeal metastasis on 131I-WBS obtained after radioiodine therapy considering the site and 131I uptake. Results A total of 14 cases(male=3, female=11) aging from 21 to 81 years old were proved to be parapharyngeal metastases identified by the 131I-SPECT/CT techniques (one case had bilateral parapharyngeal metastases, the other were unilateral). Only four parapharyngeal lesions were CT-positive alone, and the other were metastases in early-stage. Two cases were from follicular and the other from papillary thyroid carcinoma. All the cases had other distant metastases concomitantly. Conclusions Parapharyngeal metastasis may be incidental findings after the use of 131I-SPECT/CT. 131I-SPECT/CT can identify new parapharyngeal metastasis at earlier stage. These findings suggest that 131I-SPECT/CT has the diagnostic value of location and definition for the parapharyngeal metastasis, and parapharyngeal metastasis in DTC is not so uncommon as one expected.
