Abstract:Introduction The gastrointestinal tract is the most common extranodal site involved in non-Hodgkin lymphoma (NHL) and most NHL cases are of B-cell origin. Peripheral T-cell lymphoma (PTCLs) involving the gastrointestinal is less commont, comprising only about 4-6% of gastrointestinal lymphomas. This study retrospectively analyzed the clinical characteristics and potential clinical factors influencing survival outcomes in patients diagnosed with PTCLs through gastrointestinal tissue biopsy at our center. Methods Based on the clinical data, patients were reevaluated for clinical staging and risk stratification, including Ann Arbor staging, IPI scoring, and gastrointestinal lymphoma Lugano staging. Patient's follow-up was conducted through outpatient medical records and telephone communication. For the analysis of categorical data, the chi-square test or Fisher's exact test was used. Survival time was estimated using the Kaplan-Meier method. The Cox proportional hazards model was employed to calculate hazard ratios (HR) and their 95% confidence intervals (CI) to analyze prognostic factors. Data analysis was performed using SPSS 23 for Windows software. Results A total of 622 gastrointestinal tissue biopsy samples from October 2014 to September 2022 were retrieved from the medical records system. Among them, 67 cases (10.8%) were diagnosed as PTCLs. Among them, 5 cases had no available clinical data and were excluded. Detailed information regarding patient clinical characteristics can be found in the table. Treatment and Efficacy: Among the 62 patients with available clinical data, 43 received gastrointestinal surgical resection treatment, including 21 cases (33.9%) of elective non-emergency surgery and 22 cases (35.5%) of emergency surgery due to acute abdomen. The median courses of chemotheray was 5 (1~8). 15 patients (31.3%) achieved complete response (CR)/CRu; 10 patients (20.8%) achieved partial response (PR). Among the 59 patients who received chemotherapy and/or surgical treatment, progression-free survival (PFS) was evaluated with a median follow-up time of 48.4 months, and the median PFS was 4.4 months (95% CI: 2.5-6.3) (Figure). Multivariate analysis identified advanced Ann Arbor stage (stage 3/4) (HR=3.53, 95% CI: 1.53-8.17, P=0.003) as independent adverse prognostic factors for PFS. Receiving chemotherapy (HR=0.24, 95% CI: 0.09-0.66, P=0.005) and elective surgery (HR=0.45, 95% CI: 0.22-0.92, P=0.029) were identified as independent favorable prognostic factors for PFS. As of the last follow-up, a total of 49 patients (79.0%) had died and the median overall survival (OS) was 6.4 months (95% CI: 3.8-8.9) (Figure). Multivariate analysis identified Ann Arbor stages 3-4 (HR=3.01, 95% CI: 1.34-6.74, P=0.007) as an independent adverse prognostic factor for OS, and receiving chemotherapy (HR=0.13, 95% CI: 0.05-0.31, P<0.001), elective surgery (HR=0.36, 95% CI: 0.16-0.74, P=0.006) were identified as independent favorable prognostic factors for OS. Conclusion Data from four hematological disease treatment centers located in Zhejiang Province, southeastern China, showed that PTCLs involving the gastrointestinal tract accounted for approximately 10.8% (67/622) of all gastrointestinal lymphoma patients. These patients had a relatively high incidence of gastrointestinal perforation (35.5%). The overall treatment efficacy for patients was poor, with a majority of them succumbing to disease progression. The median PFS and OS were approximately six months. Elective surgery and chemotherapy appeared to improve the patients' outcomes.
Clinicopathologic Features and Treatment Outcome of Primary Intestinal Non-Hodgkin Lymphoma: A Single Center Experience.

Abstract:e19523 Background: Primary intestinal non-Hodgkin lymphoma (NHL) is a rare but heterogeneous disease in East Asia. However, the benefit of multidisciplinary treatment is still in debate. We characterized the clinicopathologic features, and treatment outcome in a single institute database. Methods: Patients with NHL primarily involving the intestine and treated during 1992 to 2008 were selected from the Cancer Registry of National Taiwan University Hospital. The medical charts and pathology records were carefully reviewed. Results: There were 64 men and 17 women with a median age of 51.5 years. Sites involved were colon/rectum (53.2%), small intestine (30.9%), and duodenum (13%). Histopathology subclassification included diffuse large B-cell lymphoma (DLBCL) (61.7%), mucosa-associated lymphoid tissue lymphoma (11.1%), Burkitt’s lymphoma (8.6%), T cell lymphoma (6.2%), follicular lymphoma (2.5%), mantle cell lymphoma (1.2 %) and others (8.6%). Ann Arbor stage IE to IIE accounted for 61.7%, whereas lower IPI score (1-2) were 54.8%. Among them, 27 patients received surgery plus chemotherapy, 60 received chemotherapy, and 4 had radiotherapy. At average follow-up of 48.7 months, 5 year survival rate were 59%, 43% and 51% for colon/rectum, small intestine, and duodenal NHL, respectively (p=0.45). Surgery plus chemotherapy versus chemotherapy alone showed no survival benefit in lower IPI group (p=0.682) nor in higher IPI (3-5) group (p=0.939). A trend of better median overall survival (mOS) was seen in rituximab group than in non-rituximab group in DLBCL subtypes (not reach vs. 39.8mo, p=0.075). In univariate analysis, stage III/IV (p=0.008), IPI score greater than 2 (p=0.011), and T cell histology (p<0.001) were significant prognostic factors for poor OS. In multivariate analysis, T cell histology remained the independent prognostic factor for inferior OS (p<0.001, HR: 20.3, 95% CI: 5.1-80.4). Conclusions: Although B cell NHL was the majority of primary intestinal NHL in our institute, T cell histology has significant inferior survival. Chemotherapy is still the backbone of treatment for primary intestinal NHL. The benefit of rituximab to intestinal DLBCL needs further confirmation.
Analysis of Clinicopathologic Features and Prognosis in 10 Patients with Non-Epithelial Malignant Neoplasmas of the Larynx and Hypopharynx

Abstract:Objective To investigate the clinicopathologic features,diagnosis and treatment of non-epithelial malignant tumors of larynx and hypopharynx. Methods From 1979 to 2002,the clinicopathologic features,diagnosis and treatment of 10 patients with non-epithelial malignant neoplasmas of larynx and hypopharynx were retrospectively analyzed.Primary lesions of 8 cases located at larynx whose pathological types included 5 sarcomas,2 lymphomas and 1 extramedullary plasmacytoma;Lesions of 2 cases located in hypopharynx.whose pathological types included 1 primary carcinosarcoma and 1 metastatic carcinoma diffused from stomach through lymphoma.All patients were treated with surgery combined with radiotherapy and/or chemotherapy. Results 2 patients with free-neoplasm have outlived for 7 years or 8 years respectively;other 7 patients with free-neoplasm had died in 1-6 years after surgery;and 1 case lost follow-up. Conclusion Non-epithelial malignant neoplasm in larynx and hypopharynx is uncommon and has poor prognasis.Immunohistochemistry staining is helpful for the final diagnosis.The therapy must differ from different types of tumors.
[Clinical and Prognostic Analysis of 101 Cases of Primary Gastrointestinal Non-Hodgkin's Lymphoma].

Abstract:This study was purposed to analyze the clinical characteristics and prognostic factors in patients with primary gastrointestinal non-Hodgkin's lymphoma (PGI-NHL). The pathological data of 101 PGI-NHL patients admitted in our hospital in the past 15 years were analyzed retrospectively. The results showed that 101 patients with PGI-NHL accounted for 14.49% of NHL in the same period, there were 64 males, 37 females, the range of ages was from 18 to 87 years old, median age was 61 years old; in disease distribution, the stomach PGI-NHL accounted for 58.42%, intestine PGI-NHL accounted for 39.60%, multiple GI involvements (MGI) accounted for 1.98%; in pathological type, diffuse large B cell lymphoma (DLBCL) accounted for 66.34%, mucosa-associated lymphoid tissue (MALT) lymphoma accounted for 17.82%, mantle cell lymphoma (MCL) accounted for 3.96%, enteropathy-associated T cell lymphoma (EATL) accounted for 7.92%, extra-nodal nasal type NK/T cell lymphoma accounted for 1.98%, follicular lymphoma (FL) accounted for 0.99%, small lymphocyte lymphoma (SLL) accounted for 0.99%. Eighty-nine out of 101 patients were followed up (49 cases live, 40 cases dead), data of the 12 patients were lost; the median survival time was 29 months (1 - 173). The three-year OS and five-year OS were 58.4% and 52.6% respectively. Univariate analysis revealed that the factors affecting OS included sex (P = 0.004), lesion site (P = 0.002), tumor size (P = 0.011), clinical Lugano staging for gastrointestinal non-Hodgkin's lymphoma (P = 0.003), IPI score (P = 0.000), pathological cell phenotype (P = 0.001), and pathological type (P = 0.006), their differences were statistically significant (P < 0.05). Multivariate Cox regression analysis indicated that clinical Lugano staging for gastrointestinal non-Hodgkin's lymphoma, IPI score, pathological type were independent prognostic risk factors affecting OS. It is concluded that clinical Lugano staging for gastrointestinal non-Hodgkin's lymphoma, IPI score and pathological type are independent risk factors affecting OS.
[Analysis of Clinical Characteristics and Prognostic Factors of 110 Cases with Primary Gastrointestinal Tract Non-Hodgkin's Lymphoma].

Abstract:OBJECTIVE:To explore the pathologic types, prognostic factors and effective treatment of PGI-NHL.METHODS:Clinical data of 110 PGI-NHL patients were analyzed retrospectively, and so did prognostic factors and each treatment outcome.RESULTS:Of the 110 patients, the male-female ratio was 1.56:1 with a median age of 58.5 years. Stomach was the most common site and small intestine was the second. The main histological subtypes were diffuse large B-cell lymphoma (DLBCL) and mucosa-associated lymphoid tissue (MALT) NHL. Among the 75 patients followed up, the 5-year overall survival (OS) rate was 62%, the median survival time was 36 months. Univariate analysis showed a similar prognosis between PG-NHL and primary bowel NHL (P > 0.05), and PGI-MALT had better prognosis than PGI-DLBCL. Among PGI-DLBCL, the ratio of germinal center (GC) sub-type to non-GC was 1:2.82. Among three main treatments, chemotherapy combined with surgery had the lowest mortality rate, compared with each alone. Disease progress mortality was 51.7% of overall mortality.CONCLUSION:The percent of non-GC in PGI-DLBCL is higher than that of in non PGI-NHL. The 5-year OS rate is lower than the 5-year OS rate of DLBCL. Combined chemotherapy is the first line treatment. Surgery is important in the control of the complications of GI tract. Maintenance treatment is important even in those who had systemic treatments.
Primary Early-Stage Intestinal and Colonic Non-Hodgkin's Lymphoma: Clinical Features, Management, and Outcome of 37 Patients.

Abstract:AIM: To analyze the clinical features, management, and outcome of treatment of patients with primary intestinal and colonic non-Hodgkin's lymphoma (PICL).METHODS: A retrospective study was performed in 37 patients with early-stage PICL who were treated in our hospital from 1958 to 1998. Their clinical features, management, and outcome were assessed. Prognostic factors for survival were analyzed by univariate analysis using the Kaplan-Meier product-limit method and log-rank test.RESULTS: Twenty-five patients presented with Ann Arbor stage I PICL and 12 with Ann Arbor stage II PICL. Thirty-five patients underwent surgery (including 31 with complete resection), 22 received postoperative chemotherapy or radiotherapy or both. Two patients with rectal tumors underwent biopsy and chemotherapy with or without radiotherapy. The 5- and 10-year overall survival (OS) rates were 51.9% and 44.5%. The corresponding disease-free survival (DFS) rates were 42.4% and 37.7%. In univariate analysis, multiple-modality treatment was associated with a better DFS rate compared to single treatment (P = 0.001). While age, tumor size, tumor site, stage, histology, or extent of surgery were not associated with OS and DFS, use of adjuvant chemotherapy significantly improved DFS (P = 0.031) for the 31 patients who underwent complete resection. Additional radiotherapy combined with chemotherapy led to a longer survival than chemotherapy alone in six patients with gross residual disease after surgery or biopsy.CONCLUSION: Combined surgery and chemotherapy is recommended for treatment of patients with PICL. Additional radiotherapy is needed to improve the outcome of patients who have gross residual disease after surgery. (C) 2005 The WJG Press and Elsevier Inc. All rights reserved.
[Clinicopathological characteristics and prognostic analysis of 92 cases with primary gastrointestinal diffuse large B-cell lymphoma].

Abstract:OBJECTIVE:To analyze the clinical and pathological features, molecular biological markers and prognosis of primary gastrointestinal diffuse large B-cell lymphoma (DLBCL). METHODS:A retrospective study was conducted in 92 cases of primary gastrointestinal DLBCL. The data of clinical characteristics, pathological and immunohistochemical features were analyzed. The relationship between different factors at diagnosis and prognosis were studied. RESULTS:Of the 92 patients, the male-female ratio was 1.56:1 with a median age of 59(7-89) years. The most frequently involved site was stomach (50.0%), followed by small intestine (15.2%), colon (12.0%), ileocecum (10.9%) and multiple gastrointestinal tract involvements (9.8%). The ratio of non-germinal center B cell (non-GCB) subtype to germinal center B cell (GCB) was 2:1. Among the cases, Ki-67 had a high level expression with a median positive rate >80%. Positive expression of Bcl-2 was detected in 52.1% cases (25/48). Evidence of Helicobacter pylori (Hp) infections was detected in 34.0% cases with stomach involvement. Of the patients, 75 were included in a follow-up study with a median time of 48 months (3-130 months). The complete remission(CR)rate was 66.7%(50/75),the partial remission (PR) rate was 22.7% (17/75) and the overall response rate was 89.4% (67/75). The 1-, 3- and 5- year survival rates of the patients were 85.3%, 66.2% and 60.5%, respectively. The incidence of secondary malignancy after chemotherapy was 2.7%. The multivariate analysis indicated that primary colic DLBCL or multiple involvements of gastrointestinal tract and low serum albumin level (<35 g/L) at diagnosis were independently associated with poor outcome. Patients who received chemotherapy combined with rituximab (43 cases) had a higher CR rate (79.1% vs 46.9%, P=0.008) and 5-year overall survival (77.5% vs 53.1%, P=0.0045) than those without rituximab (32 case). CONCLUSION:Primary gastrointestinal DLBCL was a highly invasive and heterogeneous malignancy. In our data, the proportion of non-GCB subtype was higher. Primary colic DLBCL and those with multiple involvements of gastrointestinal tract had poor survival. Low serum albumin at diagnosis indicated poor outcome. Rituximab treatment in addition to chemotherapy might help to improve the clinical outcome. Further prospective trails were needed to confirm our results.
Primary Colonic Lymphoma: Report of Two Cases and a Literature Review

Abstract:Primary colonic lymphoma is a very rare malignant tumor with no standard treatment. We report two cases of primary colonic lymphoma successfully treated with surgery and chemotherapy, and chemotherapy alone, respectively. The first case was a 61-year-old woman who presented with abdominal pain of more than 1 month. The patient was diagnosed with a colonic tumor, and immunohistochemical examinations confirmed the initial diagnosis of colonic lymphoma. The patient underwent laparoscopic-assisted right hemicolectomy followed by postoperative adjuvant chemotherapy with the cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) regimen, combined with targeted therapy with rituximab (R-CHOP). The second case was a 78-year-old man who presented with a complaint of abdominal distention for more than 1 year. Diffuse large B-cell lymphoma was definitively diagnosed by immunohistochemical examinations, and the patient underwent systemic chemotherapy with the R-CHOP regimen. Primary colonic lymphoma is a rare type of non-Hodgkin's lymphoma (NHL), and the clinical treatment is not standardized, unlike for many other types of lymphoma. Therefore, treatment is mainly based on the patient’s symptoms to determine whether surgery or systemic chemotherapy is appropriate. Rituximab is effective in some patients and may play an important role in the treatment of unresectable or asymptomatic colonic lymphoma.
[Diagnosis and Treatment of Primary Pulmonary Mucosa-Associated Lymphoid Tissue Lymphoma].

Abstract:OBJECTIVE:To study the diagnosis and treatment for primary pulmonary mucosa-associated lymphoid tissue lymphoma.METHODS:The clinical data of 12 patients with primary pulmonary mucosa-associated lymphoid tissue lymphoma from August 1992 to May 2005 were analyzed.RESULTS:All the patients survived during the follow-up periods of 6 to 164 months (mean 70.3 months). Gastric mucosa-associated lymphoid tissue lymphoma was found to coexist in 2 patients. No specific symptoms or signs, or specific results of bronchoscopy, ultrasonography or bone marrow examination were found in these patients, except that radiography showed nodules with blurred margins with characteristic air bronchogram. The final diagnosis was based on histology and immunohistochemistry. Surgical resection was performed for 6 patients, including 5 radical operations and 1 partial resection, among which 4 patients received adjuvant chemotherapy. Recurrence occurred in 1 patient 12.7 years after the operation, while the other 5 patients got disease free survival. Chemotherapy alone was administered for 4 patients, among whom 2 patients got complete remission and the others got partial remission. The other 2 patients received no treatment and had survived for 6 and 26 months respectively.CONCLUSIONS:Except for the radiographic findings, there were no specific clinical manifestations for primary pulmonary mucosa-associated lymphoid tissue lymphoma. The final diagnosis should be made by histology and immunohistochemistry. Surgery and chemotherapy can be adopted for the patients with good outcomes.

Abstract:Gastrointestinal tract is the most common involved site of primary extranodal non-Hodgkin lymphoma (NHL).Compared with intranodal NHL,treatment of primary gastrointestinal tract NHL is different.Combinational chemotherapy is the best choice for diffuse lesion.Surgery is used to control the local lesion,and is important in the control of the complications of GI tract.Radiation therapy is used to eliminate the remaining lesions and control serious local lesion.H.pylori eradication should be used for those who had H.pylori infection.Here we reviewed recent progress of treatment in PGI-NHL.
Primary gastric Burkitt's lymphoma: a clinicopathological analysis of four cases

Abstract:Purpose To investigate the clinicopathological features,diagnosis,treatment and prognosis of primary gastric Burkitt's lymphoma. Methods The clinical and pathological features were retrospectively analyzed in 4 case of primary gastric Burkitt's lymphoma. The endoscopic features,immunophenotype and molecular pathology were also analyzed and discussed with review of the literatures. Results Primary gastric Burkitt' s lymphoma mainly occurred in children,with an average age of 8 years (3～15 years) . Endoscopic findings were tuberous protuberance with surface ulcers (3 cases) . The diffuse distribution of atypical lymphoid cells with"starry sky"phenomenon in gastric mucosa was observed under microscope,the cells were of medium size and single shape,the nucleus was round or oval and the shape was uniform,the chromatin was thicker,and there were several small nucleoli,the cytoplasm was rare and slightly basophilic,and the nuclear mitosis was more frequently noted. Immunophenotyping showed that both CD20 and CD79a were all strongly positive,and the Ki-67 proliferative index was higher than 95％. The positive intensity of CD10 and BCL-6 varied from one to another. MUM1 and CD3 were all negative. One case was positive for EBER (in situ hybridization) . Meanwhile, all of these cases showed MYC gene breakage,one of them was detected by IGH/BCL-2 fusion gene but no fusion was found. Conclusion Primary gastric Burkitt's lymphoma is rare. Its endoscopic features,histomorphology,immunophenotype and molecular pathology are relatively typical and it is mainly distinguished from high-grade B cell lymphoma,lymphoblastic lymphoma and myeloid sarcoma. Burkitt's lymphoma is a highly malignant tumor,but the current treatment is relatively mature. Early diagnosis and early treatment can significantly improve the prognosis of these patients.

Abstract:Purpose To explore the histological,immunohistochemical characters and prognosis of primary gastrointestinal anaplastic large cell lymphoma(ALCL).Methods Histology,immunohistochemical markers and follow-up information of five primary gastrointestinal ALCL cases were analyzed and the correlated literature was reviewed.Results The ALCL of five patients had 1.5 ∶1 male/female ratio with an average age of 40.6(range,22~62 years).2 cases were located in the stomach,1 in pancreas and duodenum,1 in ileocecal junction,1 in colon.All patients underwent operation and two patients received chemotherapy after surgery.Four patients were subjected to follow-up of 3 to 12 months,one died and three survived.ALCL showed distinctive morphological characters.The tumors were mainly composed of large and pleomorphic cells with abundant cytoplasm.The nuclei were large and irregular with shape of twist,horseshoed and wreathlike.Multiple nuclei or huge nuclei were seen sometimes.All five cases expressed CD30 and partly expressed CD3,EMA and CD45RO.All five cases were negative for CKpan,CD20,HMB45,CD68,CD15 and CD117.Conclusions Primary gastrointestinal ALCL is extremely rare.It is necessary to differentiate it from other type of tumors such as the diffuse large B cell lymphoma,Hodgkin lymphoma,poor differentiated carcinomas,epithelioil gastrointestinal stromal tumors and malignant fibrous histiocytoma.

Abstract:Introduction: Diffuse large B cell lymphoma (DLBCL) can be classified into germinal center B-cell-like (GCB) and non-GCB subtypes with immunophenotyping. Non-GCB subtype showed poorer survival than GCB subtype in most studies. Primary gastric DLBCL (PGDLBCL) is relative common in China with good outcome. We carried out a retrospective analysis of 139 cases to characterize the clinical features and prognostic factors of PGDLBCL with different immunophenotypes. Methods: From two centers of the China Lymphoma Patient Registry (CLAP), 139 PGDLBCL cases (from January 1994 to December 2015) were retrospectively analyzed. They were categorized into GCB (51 cases) and non-GCB (88 cases) subtypes according to the expression of CD10, Bcl6 and MUM1. Results: Pathological specimens were acquired by gastroscopy biopsy (114 cases) and surgery (25 cases). The following properties were similar in GCB and non-GCB PGDLBCL (p > 0.05): Lugano stage, B symptom, ECOG, International Prognosis Index, lesion location, and gastrointestinal bleeding before treatment. The 3-year progression-free survival (PFS) and 3-year overall survival (OS) of the entire cohort were 78.1% and 81.6% (the median follow-up time was 38.5 months). Patients with GCB did not show any statistically significant difference in survival from those with non-GCB (PFS: 80.5% vs. 76.8%, p = 0.161; OS: 80.5% vs. 82.5%, p = 0.901). Positive expression of Bcl6 (75.1% vs. 93.5%, p = 0.004) and Bcl2 (69.7% vs. 87.7%, p = 0.039) both correlated with poor OS, compared to specimens lacking these two biomarkers. Most patients (94.2%) received chemotherapy ± surgery as initial treatment, while others only received surgery or watch and wait. In the chemotherapy-treated cohort, patients received rituximab-combined chemotherapy appeared to have better outcome, but without statistically significance (PFS: 80.7% vs. 68.0%, p = 0.457; OS: 86.2% vs. 67.8%, p = 0.124). Rituximab did not show clinical benefit in either GCB or non-GCB subtype(p > 0.05). Treatment-related complications included gastrointestinal bleeding (8 cases), perforation (3 cases), and obstruction (2 cases), which showed no correlation with immunophenotypes. Conclusions: This retrospective study revealed that PGDLBCL with different immunophenotypes had similar clinical features, treatment-related complications, and outcome. The positive expression of Bcl2 or Bcl6 predicts poor OS in PGDLBCL. Keywords: extranodal lymphomas; germinal center B cell-like (GCB).
Diagnosis and treatment for primary gastrointestinal non-Hodgkin's lymphoma: A review analysis of 41 cases

Abstract:AIM To present our experience with the diagnosis and to treatment on the primary gastrointestinal non Hodgkin's lymphoma (PGNHL). METHODS Retrospective analysis was performed upon 41 patients with PGNHL and treated by resections in our hospital between 1978 and 1999. RESULTS Fourteen patients (34%) had tumors in the stomach, 11 (27%) in the small bowel, and 16 (39%) in the colon. Tumors were considered by B ultrasound scan (82%), Computer tomography (76%), and X ray barium meal (83%). 28 of 30 patients were confirmed by endoscopic biopsy and pathology at the diagnosis. At presentation, 14 patients had stage I disease, 27 with stage Ⅱ. 28 was classified as low grade, 12 as intermediate, and one case as high grade. 14 (100%) patients were successfully treated by gastrectomy, 10 (91%) small intestinal resection, and 14 (8.9%) hemicolectomy. Postoperatively, 27 patients treated with chemotherapy, others only by operation. Overall 1 year survival was 95%, 3 year 76%, 5 year 63%. When Pearson Chi Square analysis for their prognostic effect only tumor size ( P 0.01), histological grade ( P 0.01), clinical stage ( P 0.05), and postoperative chemotherapy ( P 0.01) was significant. CONCLUSION Endoscopic biopsy may be a better method for confirming diagnosis and operation is the first choice for treatment of the patients with NHL at early stage, Ⅰ E and Ⅱ E, but the postoperative chemotherapy may have a role for the prevention of tumor recurring. Patients' survival might be associated with the tumor size, histological grade, clinical stage, and postoperative chemotherapy.
Primary intestinal diffuse large B-cell lymphoma: novel insights and clinical perception

Abstract:Background: Extranodal Non-Hodgkin lymphoma (NHL) is more prevalent in the gastrointestinal (GI) tract than in other sites. This study aimed to explore the clinical features and prognostic factors of primary intestinal diffuse large B-cell lymphoma (PI-DLBCL), in order to provide new references for basic research and clinical diagnosis and treatment of the rare extranodal malignant lymphoma. Methods: The clinical data of 88 patients with PI-DLBCL admitted to Fujian Medical University Union Hospital from June 2011 to June 2022 were retrospectively studied, the clinical and pathological features, diagnosis and treatment process and prognosis of PI-DLBCL were analyzed, and univariate and multivariate analysis of prognostic factors was carried out. The Kaplan-Meier method was used for survival analysis. Meanwhile, the latest literature from PubMed was retrieved to systematically discuss the research progress in the diagnosis and treatment of PI-DLBCL. Results: Among the 88 patients with PI-DLBCL included in this study, 60 cases were males (68.18%), 28 cases were females (31.82%), and 62 patients (70.45%) were complaining of abdominal pain, and the second most common clinical manifestation was changes in bowel habits in 16 (18.18%), with a median age of onset of 57 (17-82) years. The first-line treatment regimen was surgery combined with R-CHOP chemotherapy (56.82%). The median follow-up time was 72 (1-148) months, 51 (57.95%) of 88 patients with PI-DLBCL survived, 30 patients (34.09%) died, 7 patients (7.95%) were lost to follow-up, and the PFS rates of 1-year, 3-year and 5-year were 57.95%, 29.55% and 15.91%, and the OS rates of 1-year, 3-year and 5-year were 79.55%, 45.45% and 28.41%, respectively. The results of univariate Cox regression analysis showed that ECOG score, Lugano stage, B symptoms, IPI score, white blood cells, serum LDH, albumin, β2 microglobulin were the influencing factors of OS in PI-DLBCL patients, and ECOG score, Lugano stage, B symptoms, IPI score, white blood cells, serum LDH, albumin, β2 microglobulin were all the influencing factors of PFS in PI-DLBCL patients. The results of multivariate Cox analysis showed that Lugano stage may be an independent prognostic factor affecting OS and PFS in PI-DLBCL patients. Conclusion: PI-DLBCL is more common in middle-aged and elderly men, clinical manifestations lack specificity, first-line treatment is mainly surgery combined with standard chemotherapy regimens. The Lugano stage may be an independent prognostic factor affecting OS and PFS in PI-DLBCL patients.

Abstract:To explore the clinical characteristics and prognosis of patients with primary testicular non-Hodgkin's lymphoma.The clinical profiles and prognostic factors of 21 cases newly diagnosed as primary testicular non-Hodgkin's lymphoma at Peking University Cancer Hospital from January 2005 to December 2012 were retrospectively analyzed.Their median age was 59 (34-86) years. And they were classified as Ann Arbor stage I (n = 8), stage II (n = 2) and stage IV (n = 11). There were B symptoms (n = 4), extranodal involvement outside testis (n = 12) and elevated lactate dehydrogenase (LDH) at diagnosis (n = 6). The scores of international prognostic index (IPI) were 0-1 point (n = 10), 3 points (n = 10) and 4 points (n = 1). The regimens included orchidectomy as the initial treatment (n = 15), chemotherapy followed by radiotherapy (n = 7) and CNS prophylaxis during treatment (n = 15). All patients were pathologically diagnosed as diffuse large B-cell lymphoma. And 11 cases belonged to the non-germinal center B cell-like subgroup.First-line chemotherapy was either R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine and prednisolone) or CHOP-like-based regimen. Complete response was achieved in 85.7% of patients. The median follow-up period was 18 (6-58) months. The 1, 2 and 3-year survival rates were 100%, 80% and 60% respectively. Statistical analysis showed that the first-line chemotherapy with rituximab was a prognostic factor (P = 0.038).Other factors included stage (P = 0.275), LDH level (P = 0.179) , β2-microglobulin level (P = 0.229) and IPI (P = 0.275) .The prognosis of primary testicular non-Hodgkin's lymphoma is usually poor. The first-line chemotherapy with rituximab is a prognostic factor.

Abstract:Objective To investigate the early diagnosis and treatment of primary gastrointestinal malignant lymphoma( PGIML). Methods The clinical materials of 33 cases with PGIML from 1997 to 2005 were retrospectively studied. Results The diagnosis rate of gastroenteroscopy was 44. 4% . The most common immune phenotype was B-cell lineage (90. 9% ). Of the 33 PGIML patients, 30 were treated with operation, 2 with H. poli elimination and 1 gave up treatment. Among the operation cases and operation plus chemotherapy cases 19/21 cases (90. 5% )and 6/9 cases (66. 7% ) survived more than 2 years respectively, the other 6 cases died. Conclusion It is hard to diagnose PGIML in early stage. The effectiveness of surgery on treating PGIML should be further confirmed by profound prospective randomized trials.

Abstract:Objective To study the clinicopathological characteristics and prognostic factors in Chinese patients with primary intestinal non-Hodgkin' s lymphomas (PINHL).Methods The clinical symptoms,pathological features,diagnostic and prognostic factors of 273 cases diagnosed with PINHL from our center were analyzed.Results Among 273 cases,189 were male and 84 female,the male to female ratio was 2.3 ∶ 1.The age of patients ranged from 2 to 85 years old with the median age of 46.The most frequent site of the lesions was ileocecus (n=83,30.4％).The clinical symptoms of PINHL were unspecific with abdominal mass frequently seen in B-cell lymphoma,and perforation,hypogastric pain and "B" symptoms more common in T-cell lymphoma.Endoscopic biopsy diagnosis rate was 90.3％.Of 273 cases,B-cell lymphoma (n=232,85.0％) dominated PINHL with the most common subtype of diffuse large B-cell lymphoma-not otherwise specified (DLBCL-NOS) (n=132,48.4％),while the T-cell lymphoma (n=41,15.0％) were much less seen with the most common subtype of enteropathy-associated T-cell lymphoma (EATL) (n=15,36.6％).There were 245 cases were followed up,including 206 cases of B-cell lymphoma and 39 cases of T-cell lymphoma,it was found that the prognosis of B-cell lymphoma is much better than that of T-cell lymphoma (P＜0.05).Operation had no significant effect for the overall survival rate.But for patients with aggressive lymphoma,operation can improve the survival rate.Conclusions It indicates that PINHL often occurs as B cell type,DLBCL-NOS is the most common histological type.Ileocecus is the most common site involved and enteroscope biopsy is a good method of diagnosis.Compared with T-cell lymphoma,B-cell lymphoma has different clinical manifestations and a better prognosis.Patients with aggressive lymphoma can benefit from operation.

Abstract:Purpose To explore the clinicopathological features,immunohistochemical characters and prognosis of primary cutaneous anaplastic large cell lymphoma(ALCL).Methods 13 cases of primary cutaneous ALCL of histopathology,immunohistochemical markers and follow-up information were analyzed and the correlated literature was reviewed.Results The primary cutaneous ALCL of 13 patients had 1.6:1 male/female ratio with an average age of 47.The patients presented with skin nodules,masses,or plaques.All patients underwent operation and received chemotherapy or radiotherapy after surgery.9 patients were subjected to follow-up of 5~55 months,and 1 patients died.Primary cutaneous ALCL showed distinctive morphological characters.All 13 primary cutaneous ALCL expressed CD30.Most of the cases expressed CD3 and/or CD43.EMA,GranB and Perforin were positive in ALCL.All the ALCL were negative for CKpan,CD20,CD79a,HMB45,CD68,ALK,CD15 and CD117.Conclusion Primary cutaneous ALCL is a form of low-grade primary cutaneous T-cell lymphoma.They have a relatively good prognosis.It is necessary to differentiate it from other type of tumors such as the diffuse large B cell lymphoma,malignant melanoma,and poorly-differentiated carcinomas.

Abstract:Purpose To explore the clinicopathological characteristics of primary non-Hodgkin lymphoma ( PNHL) of lung. Methods The clinical features, morphology and immunohistopathological phenotypes were retrospectively studied in 28 cases of PNHL and re-viewed of the literature. Results The composition of this group of cases is 18 cases of male, 10 cases female. The a median age of pa-tients was 57 years old. According to the WHO classification, all of the 28 cases of in our PNHL series were diagnosed as non-Hodgkin lymphoma ( NHL) , including 17 mucosa-associated lymphoid tissue extranodal marginal zone lymphomas ( MALT) , 5 diffuse large B cell lymphomas ( DLBLC) , 2 NK/T cell lymphomas and 2 anaplastic large cell lymphomas, 1 mantle cell lymphoma ( MCL) and 1 pe-ripheral T cell lymphomas, unspecified, respectively. One-third PNHL patients presented with specific clinical symptoms such as cough, chest pain, dyspnea, and fatigue, Imaging examination showed unilateral or bilateral pulmonary infiltrate, single lesions or multiple nodules, and the lesions always involved with the trachea, bronchus and lung. Follow-up was completed in 16 patients ( range, 3 to 38 months) . 3 cases were conducted with pneumonectomy, among which 2 cases were given postoperative adjuvant chem-otherapy. Seven patients were given chemotherapy alone, and 5 patients did not give any treatment following initial diagnosis. At the time of last follow-up, 13 patients were alive with disease, 2 patients were died. The tumor metastasis in the left inguinal lymph node was found in one patient after 2 years by surgery. Conclusions It was shown that there was no specific clinical manifestations and fea-tures of pulmonary PNHL. Among of them, the MALT is the most common diseases, and the highly aggressive lymphomas many be oc-cur, such as the DLBLC, NK/T cell lymphoma. The diagnosis of PNHL depends on pathological examination. The immunohistochemi-cal staining and molecular pathological technology may helpful for the diagnosis and differential diagnosis of the tumors.
