Abstract:PurposeA biomechanical design method of lightweight full contacted insole based on structural anisotropy bespoke (SAB) is proposed, which can better redistribute the stress distribution of SAB designed personalized insole.Design/methodology/approachThe reconstructed joint biomechanics are simulated using finite element analysis (FEA) to develop a lightweight full contact insole. Innovatively, the anisotropic properties of the triply periodic minimal surface (TPMS) structure, which contribute to reducing insole weight, are considered to optimize stress distribution. Additionally, porosity and manufacturing time are included as design objectives. To validate the lightweight insole design, FEA is employed to simulate the stress distribution of the ergonomic insole, which can be fabricated by additive manufacturing (AM) with TPU.FindingsWith a little 0.924% loss in porosity, the maximum stress of lightweight SAB designed insoles is extremely decreased by 19.2917%.Originality/valueThe biomechanical design of the lightweight full contact insole based on SAB can effectively redistribute stress, avoid stress concentration and improve the mechanical properties of the ergonomic individual insole.

Abstract:While rocker-shaped soles have become popular for running shoes, whether or not this type of shoe benefits other functional movements has rarely been discussed. The purpose of this study was to investigate the effect of rocker-soled shoes on lower extremity biomechanics during different exercises. Seventeen healthy university students were recruited. A motion capture analysis system and surface electromyography were used to measure kinematics and muscle activation while walking (10 m), running (10 m), cutting, jumping, and ascending and descending stairs. The results showed that when wearing rocker-soled shoes, greater peak external ankle rotation was present during most exercises. Smaller peak joint angles were observed in hip extension and external rotation when walking, and in ankle dorsiflexion when ascending stairs and jumping. The vastus medialis and vastus lateralis contracted more in most exercises when rocker-soled shoes were worn. However, the biceps femoris and medial gastrocnemius showed less muscle contraction. Wearing rocker-soled shoes during testing movements change the kinematics and muscle contractions of the lower extremity. These findings may provide information for choosing shoes for different exercises or training purposes.

Optimal Design of Rocker-Profile Footwear: How Does Forefoot Rocker Radius Affect Walking Economy in Healthy Individuals?

Abstract:While there have been some studies on the effects of rocker shoes on lower extremity biomechanics, potential efficacies on metabolic costs, in particular to walking economy, have not been fully understood. The purpose of this study is to examine the energy expenditure of healthy individuals when walking with shoes designed with different forefoot rocker radius, to determine whether certain rocker-profile would provide an energy benefit. Sixteen subjects participated in the study. The cumulative energy expenditure (J) is calculated from the oxygen uptake and respiration quotient at steady state, with a wearable oxygen consumption device. The cumulative energy expenditure values and average oxygen consumption rates (VO2: mlkg(-1)min(-1)) under four different forefoot rocker conditions were compared using repeated measures ANOVA, and alpha<0.05 was considered statistically significant. The cumulative energy expenditures were reduced under rocker shoe conditions, and R18.5 with rocker radius at 185 mm was found to be optimal. The results of average oxygen consumption rate in most subjects showed the oxygen consumption rate when wearing rocker shoes was reduced, and mean differences between Flat and R21, R18.5, and R16 were 2.123%, 3.0671, and 3.7761%, respectively. It is suggested that the forefoot rocker-profile footwear design affects walking mechanics from an energy expenditure perspective. Further investigations on sole material compositions and stiffness characteristics are needed to consolidate the rocker-profile designs that are more economic under walking conditions, which may benefit the elderly with reduced walking capacities.

Abstract:The movements of the lower limb joints during walking and the moments acting on the joints,which depend on the footwear and walking style,influence the joint life times.The effect of footwear was studied using a three-dimensional gait measurement system.Testee walked in bare feet,in 7 cm high-heel shoes,and in sneakers.Kinematics and inverse dynamics were used to analyze the angles,torques,and forces at the ankle,knee,and hip.The results show that footwear greatly influences gait patterns of the human lower limbs especially in the coronal plane,with increased peak adduction moments at the knee and ankle and increased flexion/extension moments at the hips with high-heel shoes compared with sneakers.The high-heel shoes resulted in greater loads on the lower limb joints,especially the knees and hips.These added stresses will lead joints to wear and tear.

Abstract:BackgroundDifferent arch support heights of the customized foot orthosis could produce different effects on the internal biomechanics of the foot. However, quantitative evidence is scarce. Therefore, we aimed to investigate and quantify the influence of arch support heights on the internal foot biomechanics during walking stance.MethodsWe reconstructed a foot finite element model from a volunteer with flexible flatfoot. The model enabled a three-dimensional representation of the plantar fascia and its interactions with surrounding osteotendinous structures. The volunteer walked in foot orthosis with different arch heights (low, neutral, and high). Muscle forces during gaits were calculated by a multibody model and used to drive a foot finite element model. The foot contact pressures and plantar fascia strains in different regions were compared among the insole conditions at the first and second vertical ground reaction force (VGRF) peak and VGRF valley instants.ResultsThe results indicated that peak foot pressures decreased in balanced standing and second VGRF as the arch support height increased. However, peak midfoot pressures increased during all simulated instants. Meanwhile, high arch support decreased the plantar fascia loading by 5% to 15.4 % in proximal regions but increased in the middle and distal regions.ConclusionAlthough arch support could generally decrease the plantar foot pressure and plantar fascia loading, the excessive arch height may induce high midfoot pressure and loadings at the central portion of the plantar fascia. The consideration of fascia-soft tissue interaction in modeling could improve the prediction of plantar fascia strains towards design optimization for orthoses.

The impact of toe spring and foot strike angle on forefoot running biomechanics: a finite element analysis

Abstract:The surge in popularity of running has led to a multitude of designs in running shoe technology, notably, there is an increasing trend in toe spring elevation. However, the impact of this design on foot structures during running remains an essential exploration. To investigate the effects of toe spring on the foot during forefoot running, we employed finite element simulation to create two sole models with different toe spring heights (6.5 cm and 8 cm) and ground contact angles (5°, 10°, and 15°). We established and validated two foot-shoe coupling models and compared stress variations in metatarsal bones and the big toe under identical loading and environmental conditions. Higher toe spring resulted in lower peak stress and reduced stress concentration in metatarsal bones. The fourth and fifth metatarsals exhibited increasing stress trends with ground contact angle, with the fifth metatarsal experiencing the most significant stress concentration. In the case of low toe spring, stress on the fifth metatarsal increased from 15.917 MPa (5°) to 27.791 MPa (15°), indicating a rise of 11.874 MPa. Conversely, the first metatarsal showed lower stress, indicating relative safety but reduced functional significance. Moreover, higher toe spring running shoes exerted less pressure on the big toe, with an increasing trend in stress on the big toe with an increase in ground contact angle. Shoes with a higher toe spring design result in reduced pressure on the big toe. Therefore, it is advisable to avoid landing angles greater than 15° to prevent stress fractures resulting from repetitive loading.
Influence of foot posture on immediate biomechanical responses during walking to variable-stiffness supported lateral wedge insole designs

Abstract:Background: Novel designs of lateral wedge insoles with arch support can alter walking biomechanics as a conservative treatment option for knee osteoarthritis. However, variations in foot posture may influence individual responses to insole intervention and these effects are not yet known. Research question: How does foot posture influence biomechanical responses to novel designs of lateral wedge insoles with arch support? Methods: This exploratory biomechanical investigation categorized forty healthy volunteers (age 23-34) into pronated (n = 16), neutral (n = 15), and supinated (n = 9) foot posture groups based on the Foot Posture Index. Three-dimensional gait analysis was conducted during walking with six orthotic insole conditions: flat control, lateral wedge, uniform-stiffness arch support, variable-stiffness arch support, and lateral wedge + each arch support. Frontal plane knee and ankle/subtalar joint kinetic and kinematic outcomes were compared among insole conditions and foot posture groups using a repeated measures analysis of variance. Results: The lateral wedge alone and lateral wedge + variable-stiffness arch support were the only insole conditions effective at reducing the knee adduction moment. However, the lateral wedge + variable-stiffness arch support had a smaller increase in peak ankle/subtalar eversion moment than the lateral wedge alone. Supinated feet had smaller ankle/subtalar eversion excursion and moment impulse than neutral and pronated feet, across all insole conditions. Significance: Supinated feet have less mobile ankle/subtalar joints than neutral and pronated feet and, as a result, may be less likely to respond to biomechanical intervention from orthotic insoles. Supported lateral wedge insoles incorporating an arch support design that is variable-stiffness may be better than uniform-stiffness since reductions in the knee adduction moment can be achieved while minimizing increases in the ankle/subtalar eversion moment.

Can foot orthoses impose different gait features based on geometrical design in healthy subjects? A systematic review and meta-analysis

Abstract:Objective: Foot orthoses (FOs) are popular treatment to alleviate several abnormalities of lower extremity. FO designs might alter lower extremity biomechanics differently, but the association is not yet known. This review aimed to evaluate how different FO designs, namely FO with medial posting, lateral posting, arch support, or arch & heel support, change lower limb kinematics and kinetics during walking. Literature survey: Electronic database search were conducted from inception to March 2019, and 25 papers passed the inclusion criteria. Two independent reviewers checked the quality using a modified Downs and Black checklist (73.7±5.5%) and a biomechanical quality checklist (71.4±17.1%). Effect sizes for differences between with- and without- FO walking were calculated, and meta-analysis was performed whenever at least two studies reported the same variable. Results: Medial posting reduced peak ankle eversion moment. Lateral posting brought about higher peak ankle dorsiflexion and peak ankle eversion for kinematics, as well as higher peak ankle abduction moment, lower peak knee adduction moment, and higher peak mediolateral ground reaction force (GRF) for kinetics. FOs with either arch support or arch & heel support tended to decrease vertical ground reaction force, but it was not significant. Conclusion: The findings of this review reveal that medial or lateral posting work efficiently to change foot and knee kinematics and kinetics. However, the impact force is just slightly decreased by arch-supported and heel supported FOs. Due to the small number of available studies, and heterogeneity in meta-analysis findings, further research with more standardized biomechanical approach are required.

Abstract:In this paper, we use virtual prototype with finite element modeling to customize footwear design for hallux valgus foot with the purpose of offloading plantar peak pressure from the local metatarsal head. The footwear consists of two material layers to achieve deformation patterns during foot-insole contact interactions (i.e., soft tissue squeezes the insole FE model). The deformed model of the footwear is exported and a metatarsal pad is added in different placement. From the results of simulation, the metatarsal pad and the deformation synergistically achieve the purpose of decompression, and an optimal design could be found by virtually varying with the position of metatarsal pad.
Effect of orthopedic insoles on lower limb motion kinematics and kinetics in adults with flat foot: a systematic review

Abstract:Flatfoot is characterized by the collapse of the medial longitudinal arch, eversion of the rearfoot and abduction of the loaded forefoot. Orthopedic insoles are the frequently recommended treatment to support the arch of the foot, adjust the structure of the foot, reduce pain, improve stability and new techniques have been applied to the design of orthopedic insoles in recent years. However, the effectiveness of orthopedic insoles in different motions is still debated from the perspective of biomechanics. Therefore, this study aimed to explore the impact of orthopedic insoles on the kinematics and kinetics of lower limb motion, and to verify effectiveness and propose possible future research directions. We conducted a literature search across three databases employing Boolean operations and filtered results based on eligibility criteria. A total of 671 relevant literature were searched in this review, and 19 literature meeting the requirements were finally included. The results showed that: 1) orthopedic insoles were effective when patients walk, run and jump from the perspective of biomechanics; 2) orthopedic insoles had different result on the change of ankle sagittal angle, moment and peak pressure in the metatarsal region; 3) Whether the effect of insoles, which uses new techniques such as different 3D printed technologies and adds various accessories, can be further improved remains to be further studied; 4) Follow-up studies can pay more attention to the differences between diverse populations, increase the breadth of running and jumping and other movements research and long-term intervention.
An in Vitro and Finite Element Study of Load Redistribution in the Midfoot.

Abstract:A good knowledge of midfoot biomechanics is important in understanding the biomechanics of the entire foot, but it has never been investigated thoroughly in the literature. This study carried out in vitro experiments and finite element analysis to investigate the midfoot biomechanics. A foot-ankle finite element model simulating the mid-stance phase of the normal gait was developed and the model validated in in vitro experimental tests. Experiments used seven in vitro samples of fresh human cadavers. The simulation found that the first principal stress peaks of all midfoot bones occurred at the navicular bone and that the tensile force of the spring ligament was greater than that of any other ligament. The experiments showed that the longitudinal strain acting on the medial cuneiform bone was -26.2±10.8 μ-strain, and the navicular strain was -240.0±169.1 μ-strain along the longitudinal direction and 65.1±25.8 μ-strain along the transverse direction. The anatomical position and the spring ligament both result in higher shear stress in the navicular bone. The load from the ankle joint to five branches of the forefoot is redistributed among the cuneiforms and cuboid bones. Further studies on the mechanism of loading redistribution will be helpful in understanding the biomechanics of the entire foot.
A Three-Dimensional Printed Foot Orthosis for Flexible Flatfoot: An Exploratory Biomechanical Study on Arch Support Reinforcement and Undercut

Abstract:The advancement of 3D printing and scanning technology enables the digitalization and customization of foot orthosis with better accuracy. However, customized insoles require rectification to direct control and/or correct foot deformity, particularly flatfoot. In this exploratory study, we aimed at two design rectification features (arch stiffness and arch height) using three sets of customized 3D-printed arch support insoles (R+U+, R+U−, and R−U+). The arch support stiffness could be with or without reinforcement (R+/−) and the arch height may or may not have an additional elevation, undercutting (U+/−), which were compared to the control (no insole). Ten collegiate participants (four males and six females) with flexible flatfoot were recruited for gait analysis on foot kinematics, vertical ground reaction force, and plantar pressure parameters. A randomized crossover trial was conducted on the four conditions and analyzed using the Friedman test with pairwise Wilcoxon signed-rank test. Compared to the control, there were significant increases in peak ankle dorsiflexion and peak pressure at the medial midfoot region, accompanied by a significant reduction in peak pressure at the hindfoot region for the insole conditions. In addition, the insoles tended to control hindfoot eversion and forefoot abduction though the effects were not significant. An insole with stronger support features (R+U+) did not necessarily produce more favorable outcomes, probably due to over-cutting or impingement. The outcome of this study provides additional data to assist the design rectification process. Future studies should consider a larger sample size with stratified flatfoot features and covariating ankle flexibility while incorporating more design features, particularly medial insole postings.

Numerical Simulation of the Effect of Different Footwear Midsole Structures on Plantar Pressure Distribution and Bone Stress in Obese and Healthy Children

Abstract:The foot, as the foundation of the human body, bears the vast majority of the body's weight. Obese children bear more weight than healthy children in the process of walking and running. This study compared three footwear midsole structures (solid, lattice, and chiral) based on plantar pressure distribution and bone stress in obese and healthy children through numerical simulation. The preparation for the study included obtaining a thin-slice CT scan of a healthy 9-year-old boy's right foot, and this study distinguished between a healthy and an obese child by applying external loadings of 25 kg and 50 kg in the finite element models. The simulation results showed that the plantar pressure was mainly concentrated in the forefoot and heel due to the distribution of gravity (first metatarsal, fourth metatarsal, and heel bone, corresponding to plantar regions M1, M4, and HM and HL) on the foot in normal standing. Compared with the lattice and solid EVA structures, in both healthy and obese children, the percentage reduction in plantar pressure due to the chiral structure in the areas M1, M4, HM, and HL was the largest with values of 38.69%, 34.25%, 64.24%, and 54.03% for an obese child and 33.99%, 28.25%, 56.08%, and 56.96% for a healthy child. On the other hand, higher pressures (15.19 kPa for an obese child and 5.42 kPa for a healthy child) were observed in the MF area when using the chiral structure than when using the other two structures, which means that this structure can transfer an amount of pressure from the heel to the arch, resulting in a release in the pressure at the heel region and providing support at the arch. In addition, the study found that the chiral structure was not highly sensitive to the external application of body weight. This indicates that the chiral structure is more stable than the other two structures and is minimally affected by changes in external conditions. The findings in this research lay the groundwork for clinical prevention and intervention in foot disorders in obese children and provide new research ideas for shoe midsole manufacturers.
Biomechanical response of lower limb joints to lateral wedge insoles

Abstract:Abstract Lateral wedge insole (LWI) is a frequently recommended treatment option for early and midterm stages of medial knee osteoarthritis. However, studies of its effects on the lower limb joints are incomplete and imperfect. The main purpose of this study was to quantitatively analyze the response of intervention of LWI on lower-limb joint kinematics, ground reaction forces (GRFs), and centre of pressure (COP). Gait analysis of 16 healthy subjects was conducted. Three-dimensional motion data and force plate measurements were collected in the control (barefoot) and experimental conditions (wearing a pair of assigned shoes with 0, 7, and 10 mm LWIs). Results showed that the peak knee flexion angle was increased by 3.43°, 3.09°, and 3.27° with 0, 7, and 10 mm LWIs, respectively ( p < 0.01). The ankle peak dorsiflexion angle was significantly decreased by 3.79°, 2.19°, and 1.66° with 0, 7, and 10 mm LWIs, respectively ( p = 0.02). The internal rotation angle was increased by 2.78°, 3.76°, and 4.58° with 0, 7, and 10 mm LWIs, respectively ( p < 0.01). The forefoot with LWIs showed highly significantly smaller inversion, eversion, and adduction angles (all p < 0.01). The 1st peak of the vertical GRF ( p = 0.016) also increased significantly by a maximum of 0.06 body weight (BW) with LWIs. These results indicated that biomechanical changes and limitations of lateral wedges insole should be analyzed in more detail, possibly leading to new guidelines for the design and application.

Do commonly used postoperative orthopaedic shoes help off-load the forefoot?

Abstract:Purpose: The aim of the experiment presented in this study was to determine the pressure distribution within the forefoot depending on the type of orthopaedic footwear used. Methods: The study included 27 women aged 20 to 25. The Zebris FDM-2 dynamographic platform was used in foot pressure measurements. The load distribution was measured in three types of orthopaedic footwear: MedSurg, MedSurgPro and OrthoWedge. The full gait cycle was recorded. The Cavanagh masks were applied to the load distribution results processed into a graphic form. The data were analysed using Statistica v.13.1. Results: In the forefoot area, i.e. the metatarsal bones and toes 1-5, the lowest loads were reported in the shoes that off-load the forefoot (0.2 N/cm², p < 0.001). In the area of the first to fifth metatarsal bones and the hallux, the highest load was observed in the rocker shoe, accounting for 19.7 N/cm² ( p < 0.001). For comparison, high pressure in the flat shoe was found in the area of toes 2 to 5 ( p < 0.001). Conclusions: In the area of the metatarsal and toe bones, the pressure exerted was highest in the commonly used rocker shoe. The flat shoe provides an even and uniform load in all areas of the forefoot, while this type of shoe does not significantly reduce the pressure forces on the forefoot. The shoe that was the most effective in off-loading was the forefoot off-loading shoe (OrthoWedge). Barefoot walking puts less load on the forefoot compared to the flat and rocker shoes used after orthopaedic procedures.
The Effect of Arch Height and Material Hardness of Personalized Insole on Correction and Tissues of Flatfoot

Abstract:Flat foot is one of the common deformities in the youth population, seriously affecting the weight supporting and daily exercising. However, there is lacking of quantitative data relative to material selection and shape design of the personalized orthopedic insole. This study was to evaluate the biomechanical effects of material hardness and support height of personalized orthopedic insole on foot tissues, by in vivo experiment and finite element modeling. The correction of arch height increased with material hardness and support height. The peak plantar pressure increased with the material hardness, and these values by wearing insoles of 40° were apparently higher than the bare feet condition. Harder insole material results in higher stress in the joint and ligament stress than softer material. In the calcaneocuboid joint, the stress increased with the arch height of insoles. The material hardness did not apparently affect the stress in the ankle joints, but the support heights of insole did. In general, insole material and support design are positively affecting the correction of orthopedic insole, but negatively resulting in unreasonable stress on the stress in the joint and ligaments. There should be an integration of improving correction and reducing stress in foot tissues.
Influence of altered torsional stiffness through sole modification of air pressure shoes on lower extremity biomechanical behaviour during side-step cutting maneuvers

Abstract:Directional changes in cutting maneuvers are critical in sports, where shoe torsional stiffness (STS) is an important factor. Shoes are designed based on different constructions and movement patterns. Hence, it is unclear how adjustable spacers into the sole constructions of air pressure chambers (APC) affect the STS in side-step cutting. Therefore, this study investigated the effects of altered STS through adjustable sole spacers on ground reaction force (GRF) and ankle and knee joint moments in side-step cutting. Seventeen healthy recreational athletes performed side-step cutting with experimental conditions including (i) barefoot (BF), (ii) unaltered shoes (UAS): soles consisting of APC, and (iii) altered shoes (AS): modified UAS by inserting elastomeric spacers into cavities formed by APC. Mechanical and biomechanical variables were measured. Significant differences were revealed across shoe conditions for impact peak (p = 0.009) and impulse (p = 0.018) in vertical GRF, time to achieve peak braking (p = 0.004), and peak propulsion (p = 0.025) for anterior-posterior GRF in ANOVA test. No significant differences were observed in GRF peaks and impulses between UAS and AS except for a trend of differences in impact peak (p = 0.087) for vertical GRF. At the ankle and knee joint, peak ankle power absorption (p = 0.019), peak knee internal rotation moment (p = 0.042), peak knee extension moment (p = 0.001), peak knee flexion moment (0.000), peak knee power absorption (p = 0.047) showed significant difference across three shoe conditions. However, no significant differences between the UAS and AS were noticed for peak joint moments and power. Altered shoe torsional stiffness did not significantly affect the peak forces and peak ankle and knee joint moments or powers; hence sole adjustment did not influence the cutting performance. This study might be insightful in sports footwear design, and adjusting shoe torsional stiffness by sole modification might be advantageous for athletes playing sports with cutting maneuvers to reduce the risk of injuries by controlling the twisting force at the ankle that frequently happens during cutting maneuvers.
