Abstract:Objectives: To determine whether the allele frequencies and genotype distribution of an interleukin (IL)-1 beta TaqI polymorphism and an interleukin-1 receptor antagonist polymorphism are associated with susceptibility to and outcome of severe sepsis. In addition, we analyze a possible linkage disequilibrium between a previously described NcoI polymorphism within the tumor necrosis factor (TNF) locus and the two IL-1 gene family polymorphisms,Design: Prospective, consecutive entry study of patients with diagnosis of severe sepsis.Setting: Intensive care unit (ICU) of a university hospital.Patients: Ninety-three patients with diagnosis of severe sepsis admitted to the ICU between June 1993 and June 1996,Interventions: None.Measurements and Main Results:The polymorphic region within intron 2 of the IL-1ra gene containing variable numbers of a tandem repeat of 86 base pairs was amplified by means of the polymerase chain reaction. Alleles A1-5 are identified according to the size of the amplified DNA product The region that contains the biallelic Tool site within exon 5 of the IL-1 beta gene was analyzed by polymerase chain reaction amplification and subsequent digestion using the TaqI restriction enzyme. A NcoI TNF-beta polymorphism was determined,The allele frequency of the allele IL-1raA2 was increased in 93 patients with severe sepsis compared with normal individuals (p < .01). No association with patients' outcome was observed, Allele frequencies or genotype distribution of the IL-1 beta TaqI polymorphism did not differ between patients and controls, In addition, the allele TNFB2 of the NcoI TNF-beta polymorphism was associated with nonsurvival, Occurrence of the TNFB1 and TNFB2 alleles and genotypes was unrelated to alleles and genotypes of the two IL-1 gene family polymorphisms.Conclusion: In contrast to the TNF-beta NcoI polymorphism, which has been associated with patients' nonsurvival, the allele IL-1raA2 of the polymorphism within the intron 2 of IL-1ra may contribute to susceptibility to sepsis.

Abstract:Purpose: Sepsis is a lethal outcome of the inflammation and coagulation process. Human interleukin (IL)-6 and tumor necrosis factor (TNF) alpha are well-known inflammation factors closely associated with sepsis. In the present study, we aim to investigate the association of promoter-region polymorphisms IL-6 (-174G/C) rs1800795 and TNF-alpha (-308G/A) rs1800629 with pneumonia-induced sepsis.Materials and Methods: A total of 277 Chinese patients with severe pneumonia-induced sepsis were recruited into this study. All study participants were admitted to the intensive care unit until discharge or death in the First Affiliated Hospital of Zhengzhou University from July 2010 to July 2014. The patients were classified as severely septic, septic shock, and mortality. Clinical data and demographic information were recorded. TaqMan genotyping was performed to detect single nucleotide polymorphism distribution.Results: The genotype results demonstrated that carriers of the TNF-a rs1800629 A allele had a 4.28-fold higher risk for septic shock (adjusted odds ratio [OR], 4.28; 95% confidence interval [CI], 2.24-8.18; P < .01) compared with severe sepsis, and carriers of the IL-6 rs1800795 C allele had a 2.42-fold higher risk for septic shock (OR, 2.42; 95% CI, 1.08-5.45; P < .01) compared with severe sepsis. No significant difference of SNP distribution was found between the survivors and the nonsurvivors. After the results were adjusted for age and the outcomes of blood cultures, a multivariate logistic regression analysis showed similar results. Individuals with the TNF-a 308 rs1800629 A allele (adjusted OR, 2.96; 95% CI, 1.30-7.87) or the IL-6 rs1800795 C allele (adjusted OR, 1.87; 95% CI, 1.03-3.61) had a higher prevalence of septic shock. However, these SNP distribution differences were not associated with mortality.Conclusions: In intensive care unit patients, the TNF-alpha-308A allele and the IL-6 rs1800795 allele variants were susceptibility risk factors for septic shock induced by pneumonia. IL-10 Polymorphism is Associated with Increased Incidence of Severe Sepsis.

Abstract:OBJECTIVE To investigate whether three biallelic polymorphisms at positions -592, -819 and -1082 in the promoter region of the IL-10 gene are associated with increased incidence of severe sepsis. METHODS The IL-10 -592, -819 and -1082 polymorphisms were typed using polymerase chain reaction followed by digestion with the restriction enzymes RsaI, MaeIII and MnlI, respectively. RESULTS Patients with severe sepsis were more likely to have IL-10 -1082 allele 1, compared with controls (P < 0.05). Genotype distribution of the IL-10 -1082 polymorphism significantly differed between patients and controls (P < 0.05). However, the allele frequencies and genotype distribution of the IL-10 -1082 polymorphism did not differ between surviving and dead patients (P > 0.05). No significant differences in the genotype distribution and allele frequencies of the IL-10 -592 and IL-10 -819 polymorphisms were observed between patients with severe sepsis and healthy controls, nor between surviving and dead patients (P > 0.05). CONCLUSIONS The polymorphism at position -1082 in the promoter region of the IL-10 gene may be associated with susceptibility to severe sepsis. In contrast, the other two highly linked IL-10 polymorphisms are not associated with incidence or the outcome of severe sepsis.

Abstract:Objective:To investigate whether three biallelic polymorphisms at the position -592,819 and-1082 in the promoter region of the IL-10 gene were associated with the incidence and outcome of severe sepsis.Methods:The IL-10- 592,819 and- 1082 polymorphisms were typed by polymerase chain reaction followed by digestion with the RsaⅠ,MaeⅢ and MnlⅠ restriction enzyme respectively.Results:Patients with severe sepsis had a significant elevation of IL-10 1082 allele 1 compared with that in controls (59.9％ vs.50.4％,P<0.05).Genotype distribution of the IL-10-1082 polymorphism significantly differed between patients and controls (P<0.05).However,the allele frequencies and genotype distribution of the IL-10-1082 polymorphism did not differ between surviving and nonsurviving patients (P>0.05).No significant differences in the genotype distribution and allele frequencies of the IL-10-592 and IL-10-819 polymorphisms were observed between patients with severe sepsis and healthy controls,and between surviving and nonsurviving patients (P>0.05).Conclusions:The polymorphism at position 1082 in the promoter region of the IL-10 gene may be associated with the susceptibility to severe sepsis.In contrast,the additional two high linked IL-10 polymorphisms are neither associated with the incidence nor with the outcome of severe sepsis.

Abstract:•The innate immune responses mediated by IL-1 family cytokines are essential for helping the host respond effectively to sepsis.•On the other hand, IL-1, IL-33, and IL-36 act directly on T cells to strengthen adaptive immunity. IL-37 acts as a negative feedback factor to influence regulatory T cells (Tregs) and thereby dampen lymphocytes.•The levels of various IL-1 family cytokines (IL-1β, IL-18, IL-33, ST2, IL-36α, and IL-38) may provide diagnostic and prognostic value as clinical biomarkers.•Future treatments against sepsis may target certain cytokines (IL-1ra, IL-18, IL-18BP, IL-33, IL-36α, and IL-38), for which studies in humans are needed, since most studies of IL-1 family cytokines except IL-1ra have been carried out in cell culture and small animals.

Abstract:Background The highly polymorphic interleukin 10.G (IL10.G) microsatellite located in the promoter region of the interleukin-10 (IL-10) gene exerts a positive transcriptional regulatory effect on EL-10 gene expression and correlates with the in vitro IL-10 secretion. This study was conducted to investigate whether IL10.G microsatellite is associated with the incidence and/or the outcome of severe sepsis.Methods One hundred and fifteen patients with severe sepsis who had been treated at the intensive care unit of the university hospital were studied. One hundred and forty-one healthy individuals served as controls. IL10.G microsatellite genotyping was performed with the following two methods: fluorescent based polymerase chain reaction (PCR) techniques and silver staining of the amplified DNA fragment in polyacrylamide gel. Alleles were defined according to the size of the amplified DNA product.Results Ten alleles and 36 genotypes were detected both in the patients with severe sepsis and in the healthy controls. Allele IL10.G9 and allele IL10.G13 were the commonest alleles with the frequencies of 32.6% and 21.3% respectively in the patients with severe sepsis, and 34% and 27% respectively in the healthy controls. The allele frequencies of IL10.G microsatellite were neither different between the patients with severe sepsis and the healthy controls (P > 0.05), nor between survivors and non-survivors (P > 0.05). However, the frequency of one common allele IL10.G13 was slightly lower in the patients with severe sepsis than in the healthy controls (21.3% vs 27%, P > 0.05), and the frequency of allele IL10.G9 was slightly higher in the non-survivors than in the survivors (37.1% vs 28.1%, P > 0.05).Conclusion IL10.G microsatellite may neither contribute to the susceptibility to severe sepsis nor to the fatal outcome of severe sepsis.

Abstract:Objective To investigate the clinic value of interleukin-6 (IL-6) in sepsis and septic shock. Method IL-6 concentrations in the peripheral blood of healthy or infected neonates were determined by ELISA. The expression of IL-6 mRNA were also detected by means or RT-PCR. Result IL-6 levels of infection group were significant higher than that of control group and IL-6 levels of septic shock group were the highest. All 5 neonates with sepsis had IL-6 mRNA expression and 3 of 6 control subjects had weak IL-6 mRNA expression. The optimal cut off point for diagnostic reference obtained with the percentile was 23 ng/L or more, allowing a sensitivity of 81%, and a specificity of 83%. Conclusion Increased IL-6 mRNA expression and serum IL-6 levels in the early stage of neonatal sepsis and septic shock are associated with the severity and outcome of illness. Determination of IL-6 levels seems to be an early indicator of neonatal bacterial infection.

Abstract:Background: We evaluated the association of lymphotoxin-alpha (LTA, also known as tumour necrosis factor-beta) promoter +252 A/G polymorphism with sepsis. Methods: A systematic search was performed in MEDLINE, EMBASE, and Web of Science (for the period January 1966 to June 2010). Two reviewers independently selected studies on the genetic association of LTA +252 A/G polymorphism with sepsis and independently extracted data onto standardized forms. Results: Twenty-seven studies with 4399 septic patients were included based on predefined inclusion criteria. As compared to AG + GG, the LTA AA genotype was significantly associated with an increased development of sepsis in the overall population (odds ratio (OR) 1.33, 95% confidence interval (CI) 1.09-1.62; p = 0.006). An association between mortality from sepsis and AA genotype was also found in the overall population (OR 1.89, 95% CI 1.27-2.80; p = 0.002). Stratification by ethnicity indicated that the contribution to both sepsis susceptibility and mortality may be stronger in Caucasians (OR 1.44, 95% CI 1.08-1.91 and OR 2.47, 95% CI 1.52-4.00, respectively) than in other ethnicities. Conclusions: The LTA +252 A/G polymorphism is associated with both susceptibility to and mortality from sepsis.
Influence of Cytokine Genomic Polymorphisms on the Susceptibility and Outcome of Severe Sepsis

Abstract:Objective To investigate the correlations between the polymorphisms of TNFα,TNFβ,IL-1β and IL-1ra gene and the susceptibility or outcome of severe sepsis.Methods A prospective,consecutive entry study was made among 59 patients with severe sepsis admitted in ICU,Nanjing ZhongDa Hospital between 2001 and 2002.The genomic DNA of peripheral blood nucleated was extracted.The region containing the polymorphism sites of TNFα,TNFβ and IL-1β gene was amplified by polymerase chain reaction (PCR) and restrictive fragment length polymorphism (RFLP).The polymorphic region within intron 2 of the IL-1ra gene was amplified by PCR.Results The frequency of allele TNF 2 of TNFα was 15.3% in the 59 patients with severe sepsis,higher than that in normal controls [7.5%(P=0.018)],with an odds ratio of 2.232.The frequencies of genotype TNF 2/TNF 2 in the 59 patients with severe sepsis and normal controls were 10.2% and 1.5% respectively (P=0.020),with an odds ratio of 7.472.The polymorphisms of TNFβ,IL-1β and IL-1ra did not differ between the patients with severe sepsis and normal controls.The frequency of allele TNB 2 of TNFβ was 73.1% in the nonsurvivors,higher than in the survivors 31.5%(P=0.004),with an odds ratio of 5.897.The frequency of TNB 2/TNB 2 was increased in the nonsurvivors compared with survivors,with an odds ratio of 6.500(P=0.004).The allele frequency and genotype distribution of TNFα,IL-1β and IL-1ra did not differ between the surviors and nonsurviors.Conclusion The allele TNF 2 of TNFα NcoI polymorphism may contribute to susceptibility to severe sepsis.The allele TNFB 2 of TNFβ NcoI polymorphism may be a genomic marker indicating poor prognosis.

Abstract:Severe sepsis is an infection-initiated, inflammatory syndrome which complicated with organ dysfunction. Genetic factors affect the incidence and outcome of severe sepsis. Human beta-defensins, cysteine-rich cationic antimicrobial polypeptides with 3 disulfide bridges, play an important role in innate immunity and adaptive immunity in sepsis. Our previous studies have shown that human beta-defensin 1 gene was transiently transcribed following the induction of LPS or heat-inactivated bacterial cells and the inducible expression of beta-defensin1 gene showed interindividual variability. The purpose of this study was to evaluate whether single-nucleotide polymorphisms (SNP) in the 5′-untranslated region of the HBD-1 gene are associated with the susceptibility to severe sepsis or with the outcome of severe sepsis. Two hundred and fifty patients with severe sepsis who had been treated at the intensive care unit of the university hospital were studied. One hundred seventy postoperative patients without sepsis served as controls. The DNA fragment containing the three SNPs (-52G/A, -44C/G and -20G/A) in the 5′-untranslated region of the HBD-1 gene were amplified and sequenced. There was a significant difference in the distribution of genotype and alleles of -20G/A SNP and -44C/G SNP between severe sepsis and controls. While, there was no significant difference in the distribution of genotype and alleles of -52G/A SNP between severe sepsis and controls. Further, there was no significant difference in the distribution of genotype and alleles of -20G/A SNP, -44C/G and -52G/A SNP between survivors and non-survivors. -20G/A SNP and -44C/G SNP is associated with the susceptibility to severe sepsis.

Abstract:Objective: To investigate the association between single nucleotide polymorphism (SNP) in the 11th exon of transient receptor potential melastatin 2 (TRPM2) gene with the susceptibility and outcome of sepsis. Methods: A total of 119 septic patients and 112 normal subjects were enrolled from the First Affiliated Hospital, Zhejiang University School of Medicine. Among 119 septic patients, 62 died (fatal group) and 57 survived (survival group) within 28 days of disease onset. The genotypes of these individuals were detected using TaqMan allelic discrimination assays, and its correlations with susceptibility and outcome of sepsis were analyzed. Results: There was no significant difference in genotype frequencies and allelic frequencies of TRPM2 SNP rs1556314 between septic patients and the controls (all P>0.05). And no significant difference in genotype frequencies and allelic frequencies of TRPM2 SNP rs1556314 was observed between the survivors and fatal cases of septic patients (all P>0.05). Conclusion: The TRPM2 SNP rs1556314 does not have significant association with sepsis, but this result need to be confirmed by large scale studies.
[Association of Tumor Necrosis Factor Microsatellite Polymorphism with Incidence and Outcome of Severe Postoperative Sepsis].

Abstract:OBJECTIVE To investigate whether tumor necrosis factor (TNF) microsatellite polymorphism within the TNF locus is associated with the incidence and outcome of severe postoperative sepsis. METHODS 122 patients with severe postoperative sepsis were included in this study; 138 local ethnically matched healthy individuals served as controls. Microsatellite TNFc polymorphism within the TNF locus was typed using polymerase chain reaction (PCR) followed by polyacrylamide gel electrophoresis with silver staining. RESULTS Microsatellite TNFc polymorphism had two alleles (TNFc1 and TNFc2) and three kinds of genotype (homozygotes TNFc1c1 and TNFc2c2, and heterozygote TNFc1c2). The genotype distribution fit Hardy-Weinberg equilibrium. The frequency of TNFc1 microsatellite allele was significantly higher in patients with severe sepsis (79%) than in healthy individuals (71%) (P < 0.05). The frequency of heterozygote TNFc1c2 was significantly higher in non-surviving patients (46%) with severe sepsis than in survivors (27%) (P < 0.05). CONCLUSIONS TNFc microsatellite polymorphism is significantly associated with the incidence and outcome of severe postoperative sepsis.
A functional CATTn microsatellite in the promoter of macrophage migration inhibitory factor in sepsis

Abstract:Objective MIF, a potent pro-inflammatory cytokine, plays a pivatol role in the pathogenesis of sepsis. A novel CATT microsatellite at position -794 of the human MIF gene functionally affects the activity of the MIF promoter and correlates with the disease severity in rheumatoid arthritis, ulcerative colitis, atopy or sarcoidosis. The purpose of this study is to determine whether MIF microsatellite is associated with the susceptibility to and outcome of severe sepsis. Methods After approval by the hospital and national ethics committee, written informed consent was obtained from the patient or a first degree relative. Blood samples were obtained from 98 postoperative patients with severe sepsis (sepsis group) and 73 controls. Microsatellite was determined using polymerase chain reaction (PCR) with a FAM-labeled upstream primer combining capillary electrophoresis. Statistical analysis was done by Fisher’s exact test. P < 0.05 was regarded as statistically significant. Results The underlying diseases for the severe sepsis patients were peritonitis, pancreatitis, pneumonia and multiple trauma. The APACH II score in sepsis was 22.5±2.1, the mortality in sepsis patients reached 51%. There is no significant difference in the race, age, sexual. Four kinds of alleles with 4, 5, 6, or 7-CATT repeat units and seven kinds of genotypes with 4/4、4/5、4/6、4/7、5/5、5/6、6/6 CATT repeat units were detected among these patients either with or without sepsis. The genotype distribution and allele frequencies between sepsis individuals and controls, between survivors and non-survivors didn’t differ (P > 0.05). Conclusion This study shows that the functional CATTn microsatellite in the promoter of macrophage migration inhibitory factor is neither related to the incidence of severe sepsis nor to the fatal outcome of sepsis.

Abstract:Objective Toll-like receptor 5 (TLR5) is able to recognize bacterial flagellin of both Gram-negative and -positive bacteria, activate NF-κB signal pathway and play a central role in sepsis. In the present study, we investigated whether the polymorphisms of TLR5 gene is associated with susceptibility to and disease severity of sepsis.Methods Three TLR5 polymorphisms, R392X (rs5744168), S592N (rs2072493) and L616F (rs5744174) in exon 6, were genotyped in 255 patients with sepsis and 260 control subjects by the Beckman SNPstream genotyping platform and PCR-RFLP analysis. The associations with susceptibility to and disease severity of sepsis were estimated by logistic regression, with controlling for confounding factors, including age, sex, smoking status, drinking status, chronic diseases status, APACHEII and critical illness status.Result Genotype frequencies of these three polymorphisms were conformed to the Hardy-Weinberg equilibrium in both patients and controls. On the basis of logistic regression analysis with adjustment for confounding factors, no significant association was found between polymorphisms of and susceptibility to sepsis. Also no significant association with septic shock, sepsis caused death and the number of organ dysfunction was found within the three polymorphisms.Conclusion Our findings suggest that the TLR5 polymorphisms R392X, S592N and L616F do not play a major role in mediating susceptibility to and disease severity of sepsis.

Abstract:In a previous issue of Critical Care, Giam arellosBourboulis and colleagues published an article in which they drew a conclusion that inhibition of caspase-1 and defective IL-1β production is an important immunological feature in sepsis [1]. Based on the knowledge of previou s research, we would like to make some rema rks. Firstly, peripheral blood mononuclear cells (PBMC) mainly consist of monocytes, T cells, B cells and natural killer cells, of which only monocytes are the high producer of IL-1β and TNFα. Th e proportion of mono cytes in PBMC may therefore greatly aff ect production of these cytokines. In earlier research, Giamarellos-Bourboulis and colleagues demonstrated that the percentage of monocyte apoptosis was over 50% in about half of their septic subjects [2]. In their recent study, however, these authors found that downregulation of IL-1β foll owed a diff erent pattern to TNFα; release of TNFα was not impaired after lipopolysaccharide stimulation of PBMC of patients with uncomplicated sepsis and with severe sepsis [1], suggest ing that the monocytes of these subjects did not undergo apoptosis. Th e introduction of methods to check the apoptosis of monocytes and th eir proportion in PBMC is thus quite necessary. Secondly , the authors concluded that defective IL-1β production is an important immunological feature in sepsis. According to Fahy and colleagues’ research, however, the plasma IL-1β level was signifi cantly elevated in patients with septic shock in comparison with normal control subjects [3]. A possible explanation for this diff erence is that caspase-1 is not the unique enzyme that can activate IL-1β. Abstract:The biallelic IL-10 single nucleotide polymorphism at 2 1082 of the promoter region linked to individual variation in cytokine inducibility has been strongly implicated in several pathological conditions including the development of, and outcomes in, septic shock during pneumococcal infection, acute respiratory distress syndrome, and cardiac dysfunction. However, the molecular basis of the single nucleotide polymorphism-mediated variable IL-10 production levels has not been explored. In this study, we report that the 2 1082G > A alleles in the promoter region of the human IL-10 gene physically interact with a nuclear protein in an allele-speciﬁc manner that results in different levels of IL-10 transcription. This protein has been identiﬁed as poly(ADP-ribose) polymerase 1 (PARP-1). We show that PARP-1 acts as a transcription repressor, and its DNA-binding activity is strongly regulated in macrophages that engulf apoptotic cells but not stimulated with LPS. These ﬁndings unveil a novel role of PARP-1 in the regulation of IL-10 production in an allele-dependent way, which determines individual susceptibility to sepsis-induced inﬂammatory pathology and the immunological sequelae in a physiological process in which clearance of infection-induced apoptotic cells by professional phagocytes triggers the cytokine synthesis. Association study of MCP-1 promoter polymorphisms with the susceptibility and progression of sepsis

Abstract:Previous studies have indicated that the monocyte chemo-attractant protein 1 (MCP-1), also referred to as C-C motif chemokine ligand 2 (CCL2), plays a significant role in the pathogenesis of sepsis, and this study investigated the clinical relevance of two MCP-1 gene polymorphisms on sepsis onset and progression. The Multiplex SNaPshot genotyping method was used to detect MCP-1 gene polymorphisms in the Chinese Han population (403 sepsis patients and 400 controls). MCP-1 mRNA expression levels were measured using real-time quantitative PCR, and enzyme-linked immunosorbent assays were used to analyze MCP-1, tumor necrosis factor-alpha (TNF-α), interleukin 6 (IL-6) and interleukin-1 beta (IL-1β) plasma concentrations. The rs1024611 polymorphism analysis showed lower frequencies of minor homozygous genotype (AA) and allele (A) in sepsis patients compared to the healthy controls (19.4% vs. 31.5%, P = 0.0001 and 45.9% vs. 54.8%, P = 0.0004, respectively). And the frequencies of GG genotype and G allele were lower in sepsis patients compared to the controls (19.6% vs. 31.3%, P = 0.0002 and 46.0% vs. 54.5%, P = 0.0007, respectively). The rs1024611 AG/GG and rs2857656 GC/CC genotypes were both overrepresented in patients with severe sepsis (both P = 0.0005) and septic shock (P = 0.010 and P = 0.015, respectively) compared to the patients with mild sepsis. Moreover, among sepsis patients, the rs1024611 AG/GG and rs2857656 GC/CC carriers exhibited significant increases in expression levels of MCP-1 (P = 0.025), TNF-α (P = 0.034) and IL-6 (P = 0.043) compared with the rs1024611 AA or rs2857656 GG carriers. This study provides valuable clinical evidence that the MCP-1/CCL2 polymorphisms rs1024611 and rs2857656 are associated with sepsis susceptibility and development. We conclude that MCP-1/CCL2 plays a significant role in the pathogenesis of sepsis, which has potentially important therapeutic implications.

Abstract:Objective To explore the mRNA expressions of NLRP1 (NOD-like receptor 1 ) and NLRP3 inflammasomescomponents and related pro-inflammation cytokines in peripheral blood mononuclear cells (PBMCs).Methods A total of 21 septic patients,20 non-infectious SIRS patients and 20 health subjects were enrolled in this study.The mRNA expressions of inflammasomes components and related proinflammation cytokines were measured by using RT-PCR,and serum levels of IL-1β and IL-18 were measured using ELISA.One-way ANOVA was usedd to compare the data of all three groups,and SNK-q was employed to compare thedata among different groups.Spearman correlation analysis was used to analyze the correlation between these biomarkers and the severity of septic patients.Results ①The mRNA expression of NLRP1 in PBMCs of septic patients in early stage and non-infectious SIRS patients was down-regulated in comparison with healthy subjects (P ＜0.01 ).The expression of NLRP3 in septic patients was not differentfrom that in healthy subjects ( P ＞ 0.05 ),but NLRP3 mRNA expression in non-infectious SIRS patients was significantly higher than that in septic patients and in healthy subjects ( P ＜ 0.01 ).ASC ( apoptosis associated speck like protein containing a CARD) and caspase-1 mRNA expressions in septic patients and non-infectious SIRS patients were higher than those in healthy subjects ( P ＜ 0.01 ).There was no difference in level of IL-1β found among all groups.IL-18 mRNA expression in septic patients and non-infectious SIRS patients was higher than that in healthy subjects ( P ＜ 0.01 ),and that in non-infectious SIRS patients was highest.②)Serum protein level of IL-1β in septic patients was lower than that in healthy subjects (P ＜0.05 ),and serum level of IL-18 in septic patients was significantly higher than that in non-infectious SIRS patients and in healthy subjects ( P ＜ 0.01 ).③NLRP1 was negatively related to SOFA ( sepsis-related organ failure assessment) score (r =-0.44,P ＜ 0.05 ) and negatively related to APACHE Ⅱ score as well ( r =- 0.52,P ＜ 0.05 ).Conclusions NLRP1 mRNA expression was down regulated in patients with early sepsis and its expression was negatively correlated to the severity of septic patients,indicating the lower NLRP1 mRNA expression the severer illness.
