Abstract:Background: Small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK) have been extensively studied as the main surgical methods for corneal refractive surgery. However, there is no consensus on whether SMILE is superior to FS-LASIK in corneal biomechanics. Therefore, this systematic review and meta-analysis used the results of ocular response analyzer and corvis ST to explore whether SMILE is superior to FS-LASIK in corneal biomechanics. Methods: The literature was searched in PubMed, EMBASE, and Controlled Trials Register databases. The Cochrane Collaboration's "risk of bias" tool was used to evaluate the quality of the included randomized clinical trials, and the Newcastle-Ottawa Scale was used to evaluate the included non-randomized controlled trials. The results were analyzed using Revman 5.3. Results: Sixteen studies (3 randomized clinical trials and 13 non-randomized controlled trials) were included in this meta-analysis. There was no statistical difference in corneal biomechanics between SMILE and FS-LASIK in corneal hysteresis [mean difference (MD), 0.20; 95% confidence interval (CI): -0.09, 0.49; P = .18] and corneal resistant factor (MD, 0.31; 95% CI: -0.09, 0.71; P = .13), A1 time (MD, -0.02; 95% CI: -0.11, 0.07; P = .66), A1 length (MD, 0.01; 95% CI: -0.01, 0.03; P = .42), A1 velocity (MD, 0.00; 95% CI: -0.01, 0.01; P = .85), A2 velocity (MD, -0.01; 95% CI: -0.11, 0.09; P = .86), HC time (MD, 0.12; 95% CI: -0.13, 0.38; P = .35), The stiffness parameter at first applanation (MD, -7.91; 95% CI: -17.96, 2.14; P = .12), The ratio between the deformation amplitude 2 mm away from apex and the apical deformation (MD, 0.01; 95% CI: -0.26, 0.27; P = .96). Conclusion: A comprehensive assessment of the parameters of ocular response analyzer and corvis ST showed that SMILE is not superior to LASIK in corneal biomechanics 3 months post-surgery.

Abstract:Myopia, which is the result of the uncoordinated development of the eyeball, has become a major public health focus worldwide. Laser in situ keratomileusis (LASIK) and small-incision lenticule extraction (SMILE) have been successfully used in modern corneal refractive surgery. However, there are still controversies about postoperative results of LASIK and SMILE. In this study, a three-dimensional finite element model of the cornea was constructed based on the elevation and pachymetry data of a female volunteer. Surgical parameters, magnitudes of myopic correction, and intraocular pressure (IOP) were varied. Furthermore, an iterative algorithm was applied to retrieve the free-stress state of the intact corneal model, LASIK model, and SMILE model. To better evaluate the differences between LASIK and SMILE procedures, the displacement and Von Mises stress on the anterior and posterior corneal surface along the x- and y -axes were analyzed. Results for the zero-pressure model showed larger displacement compared to the image-based corneal model, suggesting that the initial corneal pre-stress stiffens the response of the cornea, both in the intact cornea and under refractive surgery. In addition, the displacement on the corneal surface in LASIK (both zero-pressure and image-based model) was obviously higher than that of the SMILE model. In contrast, SMILE increased Von Mises stress in the corneal cap and reduced Von Mises stress in the residual stromal bed compared with the LASIK model. However, the maximum Von Mises stress in the SMILE model was still smaller than that of the LASIK model. Moreover, the displacement and Von Mises stress on the residual stromal bed increased linearly with IOP. Overall, LASIK and SMILE refractive surgery could change biomechanical behaviors of the cornea. Compared to LASIK refractive surgery, SMILE may present a lower risk of ectasia. Creating a corneal cap rather than a corneal flap may have an advantage in improving corneal biomechanical stability.

Abstract:<h3 class="Heading">Background</h3><p class="Para">The aim of this study was to compare the postoperative corneal biomechanical properties between small incision lenticule extraction (SMILE) and other corneal refractive surgeries.</p><h3 class="Heading">Methods</h3><p class="Para">A systematic review and meta-analysis were conducted. Articles from January 2005, to April 2019, were identified searching PubMed, EMBASE, Web of Science, and International Clinical Trials Registry Platform. Studies that compared SMILE with other corneal refractive surgeries on adult myopia patients and evaluated corneal biomechanics were included. Multiple effect sizes in each study were combined. Random-effects model was conducted in the meta-analysis.</p><h3 class="Heading">Results</h3><p class="Para">Twenty-two studies were included: 5 randomized controlled trials (RCTs), 9 prospective and 6 retrospective cohort studies, and 2 cross-sectional studies. Using the combined effect of corneal hysteresis (CH) and corneal resistance factor (CRF), which were obtained from ocular response analyzer (ORA), the pooled Hedges' g of SMILE versus femtosecond laser-assisted in situ keratomileusis (FS-LASIK) was 0.41 (95% CI, 0.00 to 0.81; <em class="EmphasisTypeItalic">p</em> = 0.049; I<sup>2</sup> = 78%), versus LASIK was 1.31 (95% CI, 0.54 to 2.08; <em class="EmphasisTypeItalic">p</em> < 0.001; I<sup>2</sup> = 77%), versus femtosecond lenticule extraction (FLEX) was − 0.01 (95% CI, − 0.31 to 0.30; <em class="EmphasisTypeItalic">p</em> = 0.972; I<sup>2</sup> = 20%), and versus the group of photorefractive keratectomy (PRK) and laser-assisted sub-epithelial keratectomy (LASEK) was − 0.26 (95% CI, − 0.67 to 0.16; <em class="EmphasisTypeItalic">p</em> = 0.230; I<sup>2</sup> = 54%). The summary score of Corvis ST (CST) after SMILE was comparable to FS-LASIK/LASIK with the pooled Hedges' g = − 0.05 (95% CI, − 0.24 to 0.14; <em class="EmphasisTypeItalic">p</em> = 0.612, I<sup>2</sup> = 55%).</p><h3 class="Heading">Conclusions</h3><p class="Para">In terms of preserving corneal biomechanical strength after surgeries, SMILE was superior to either FS-LASIK or LASIK, while comparable to FLEX or PRK/LASEK group based on the results from ORA. More studies are needed to apply CST on evaluating corneal biomechanics after refractive surgeries.</p>

Abstract:PurposeTo compare posterior corneal elevation (PCE) changes and corneal biomechanical changes post fematosecond laser-assisted laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) procedures using the Pentacam system and the Ocular Response Analyzer (ORA).DesignRetrospective observational case series study.Method106 patients with bilateral myopia who underwent either FS-LASIK (56 patients) or SMILE (50 patients) were reviewed, only the right eye was used in the analysis. Inclusion criteria include a spherical equivalent of −6.00DS and completion of 12 months follow up. The main outcome evaluated was change in PCE at 3 months, 6 months and 12 months post-operation using the Pentacam system. Corneal biomechanical parameters were evaluated at 6 and 12 months with the ORA.ResultsPCE change at 3 and 6 months were not significantly different between the two procedures (p=0.064, p=0.109 respectively). At 12 months, PCE change was greater in FS-LASIK than SMILE (p=0.048). One-way ANOVA showed that for either procedure, the change in PCE did not differ at 3, 6 or 12 months post operation. CH and CRF values were reduced after both procedures, with FS-LASIK demonstrating a greater reduction in CRF than SMILE at 6 and 12 months (p=0.037 and p=0.001). Both CH and CRF reduction correlated with PCE increase at 6 and 12 months.ConclusionFS-LASIK demonstrated a greater increase in PCE than SMILE only at 12 months, as well as a greater reduction of CRF than SMILE. There were no significant differences in PCE change over time within either group.

Abstract:Background: To compare the corneal biomechanical changes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) with the same programmed optical zone (POZ) and similar refractive correction in patients with high myopia.Methods: In this prospective comparative study of the contralateral eye, 50 patients with high myopia with the same POZ and similar refractive correction who underwent SMILE in one eye and FS-LASIK in the other eye. Corneal biomechanical parameters and central corneal thickness (CCT) were measured using a Corvis ST II. All the patients were evaluated during follow-up visits beyond one year. Additionally, the corneal volume (CV) of the 10-mm diameter region was measured using a Pentacam.Results: Ambrosio relational thickness to the horizontal profile (ARTh) and stiffness parameter A1 (SP-A1) decreased significantly after SMILE and FS-LASIK, whereas deformation amplitude ratio 2.0 mm (DA ratio 2.0 mm) and integrated radius (IR) increased significantly in both groups. The ARTh and SP-A1 were greater after SMILE than those after FS-LASIK at all the follow-up visits. In addition, there were greater amounts of CCT and CV after SMILE compared with that after FS-LASIK. Moreover, a positive correlation was found between ARTh and SP-A1 and postoperative CCT, while a negative correlation was found between IR and DA ratio 2.0 mm and postoperative CCT. A moderate correlation was observed between SP-A1 and CV after both SMILE and FS-LASIK, whereas there were no relationships between CV and ARTh, IR, or DA ratio 2.0 mm.Conclusions: SMILE had greater CCT, CV, ARTh, and SP-A1 than FS-LASIK in high myopia with the same POZ and similar refractive correction. Confocal Comparison of Corneal Reinnervation after Small Incision Lenticule Extraction (smile) and Femtosecond Laser in Situ Keratomileusis (Fs-Lasik)

Abstract:Purpose: To evaluate corneal reinnervation, and the corresponding corneal sensitivity and keratocyte density after small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK).Methods: In this prospective, non-randomized observational study, 18 patients (32 eyes) received SMILE surgery, and 22 patients (42 eyes) received FS-LASIK surgery to correct myopia. The corneal subbasal nerve density and microscopic morphological changes in corneal architecture were evaluated by confocal microscopy prior to surgery and at 1 week, 1 month, 3 months, and 6 months after surgery. A correlation analysis was performed between subbasal corneal nerve density and the corresponding keratocyte density and corneal sensitivity.Results: The decrease in subbasal nerve density was less severe in SMILE-treated eyes than in FS-LASIK-treated eyes at 1 week (P=0.0147), 1 month (P=0.0243), and 3 months (P=0.0498), but no difference was detected at the 6-month visit (P=0.5277). The subbasal nerve density correlated positively with central corneal sensitivity in both groups (r=0.416, P < 0.0001, and r=0.2567, P=0.0038 for SMILE group and FS-LASIK group, respectively). The SMILE-treated eyes have a lower risk of developing peripheral empty space with epithelial cells filling in (P=0.0005).Conclusions: The decrease in subbasal nerve fiber density was less severe in the SMILE group than the FS-LASIK group in the first 3 months following the surgeries. The subbasal nerve density was correlated with central corneal sensitivity.
Corneal Biomechanical Properties after Small Incision Lenticule Extraction Surgery on Thin Cornea.

Abstract:Purpose To explore the biomechanical changes in thin corneas after the small incision lenticule extraction (SMILE) surgery. Methods This prospective survey screened patients scheduled for the SMILE surgery from November 2017 to March 2018. Patients with thin corneas (central corneal thickness [CCT] <= 500 mu m) and those with normal corneal thickness (CCT > 500 mu m) were enrolled. Corneal biomechanics were examined by the ocular response analyser and Corvis ST, preoperatively and at 1 day, 3 weeks, and 3 months postoperatively. Results Twenty-seven patients (46 eyes) with mean spherical equivalent of -5.1 +/- 1.7 D were assigned to the thin cornea group, and 28 (45 eyes) cases with mean spherical equation of -5.3 +/- 1.8 D were assigned to the control group. Safety and efficacy did not differ between the two age- and refraction-matched groups (safety index, 1.15 +/- 0.14 vs 1.14 +/- 0.17 [P= .7]; efficacy index, 1.11 +/- 0.13 vs 1.16 +/- 0.22 [P= .2]). All biomechanical parameters changed significantly after SMILE. The thin cornea group showed less decrease in the second applanation time (A2 Time), stiffness parameters at first applanation (SP-A1). Pooling data from two groups, lower CCT was correlated with less A2 Time change (r = 0.37,P< .01) and less SP-A1 change (r = -0.33,P< .01). Less bIOP change was correlated with higher residual stromal thickness index and thicker CCT (P= .003, R-2 = 0.12). Conclusion Over a short-term observation period, less corneal biomechanic deterioration may have contributed to the safety of SMILE on thin cornea.

Abstract:PURPOSE: To compare the impact on corneal sensation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted LASIK (femto-LASIK) in patients with myopia.METHODS: In this prospective, nonrandomized comparative study, 71 subjects were enrolled. Thirty-eight eyes of 38 patients underwent SMILE and 33 eyes of 33 patients underwent femto-LASIK. Corneal sensation was tested with Cochet-Bonnet esthesiometry in five corneal areas preoperatively and at 1 week and 1, 3, and 6 months postoperatively. Comparison of corneal sensation was performed for the SMILE and femto-LASIK groups. Additionally, the correlations were evaluated between the postoperative corneal sensation, preoperative spherical equivalent, and ablation depth.RESULTS: All tested areas within the cap or flap demonstrated corneal hypoesthesia immediately after both surgeries. SMILE-treated eyes showed less compromised corneal sensation than femto-LASIK-treated eyes at all postoperative visits in the central, inferior, nasal, and temporal areas at the 1-week and 1-month visits. In the SMILE group, the inferior, nasal, and temporal quadrants recovered faster than other areas. In the femto-LASIK group, the sensation over the flap did not recover to preoperative levels by postoperative 6 months. There was no correlation between postoperative corneal sensation, preoperative spherical equivalent, and ablation depth in both groups.CONCLUSIONS: The impairment of corneal sensation was less significant in the SMILE group than in the femto-LASIK group and was independent of preoperative spherical equivalent or ablation depth.

Abstract:Aim To evaluate and compare early corneal wound healing and inflammatory responses after small incision lenticule extraction (SMILE) versus femtosecond laser laser in situ keratomileusis (LASIK). Methods Thirty-six eyes of 36 rabbits underwent SMILE, while another 36 eyes of 36 rabbits were treated with femtosecond laser LASIK. All the eyes were subjected to the same refractive correction of -6.00 DS/-1.00 DC. Twelve eyes that had no surgery were included for control. After euthanisation, corneal tissue sections were evaluated with terminal deoxyribonucleotidyl transferase-mediated dUTP-digoxigenin nick-end labelling (TUNEL) assay to detect apoptosis at postoperative 4 and 24 h, immunocytochemistry for Ki67 to detect keratocyte proliferation at postoperative day 3, week 1 and month 1, and immunocytochemistry for CD11b to detect inflammation at postoperative day 1, day 3 and week 1, respectively. Results No adverse effects were noted after SMILE or LASIK. Corneal healing postoperatively was uneventful in all cases. There were significantly fewer TUNEL-positive corneal stromal cells after the SMILE procedure at 4 and 24 h postoperatively (p<0.01) compared with the LASIK procedure. In addition, immunocytochemistry showed significantly fewer Ki67-positive cells in the SMILE group than those in the femtosecond laser LASIK group at day 3 and week 1 postoperatively (p<0.05), but there was little expression of Ki67 at month 1 postoperatively in both groups. The CD11b-positive cells were significantly fewer in the SMILE group at day 1, day 3 and week 1 postoperatively (p<0.01). Conclusions SMILE induces less keratocyte apoptosis, proliferation and inflammation compared with femtosecond laser LASIK.

Abstract:·AlM:To compare the corneal biomechanical changes at early stage after femtosecond laser assisted in situ keratomi ( LASlK) and microkeratome LASlK according to different residual corneal stroma thickness/cornea thickness ( RCST/CT) . <br> · METHODS: Ninety -four patients ( 187 eyes ) with femtosecond LASlK ( FS-lK ) and 96 patients ( 192 eyes ) with microkeratome LASlK ( M-lK ) were included in this study. LASlK group was divided into three groups according to the ratio of RCST/CT, which were lK-l group ( RCST/CT<55%) , lK-Ⅱ group ( 55%≤RCST/CT<60%) and lK-Ⅲ group ( RCST/CT≥60%) .Corneal hysteresis ( CH) and corneal resistance factor ( CRF) were measured by reichert ocular response analyzer ( ORA ) at pre -operation, postoperative 1and 3mo. <br> ·RESULTS:Compared with FS-lK and M-lK,there were no statistical difference in CH and CRF ( F=0.44, F=2.56, P=0.51, P=0.11 ) .Based on different RCST/CT, there were statistical differences in CH and CRF between groupⅠand group Ⅱ (F=103.03, 128.48,P<0.05), group Ⅱand groupⅢ ( P<0.05 ) , group Ⅰ and group Ⅲ ( P<0. <br> 05 ) . There were statistical differences in CH and CRF between pre-operation and different postoperative times ( F =576.99, 1162.06, P <0.05 ). CH and CRF of postoperative 1mo and 3mo were significantly smaller than pre-operation ( P<0.05) , but there was no statistical difference between postoperative 1and 3mo (P>0.05). <br> · CONCLUSlON: The biomechanical parameters decreased at early stage after femtosecond LASlK and microkeratome LASlK, but there is no statistical difference between femtosecond LASlK and microkeratome LASlK. The less the ratio of RCST/CT, the more effect on biomechanics.
Contralateral Eye Comparison Between 2 Cap Thicknesses in Small Incision Lenticule Extraction: 110 Versus 130 μm.

Abstract:Purpose: To evaluate the posterior corneal elevation (PCE) and biomechanical changes after small incision lenticule extraction (SMILE) at depths of 110 mm and 130 mm. Methods: One hundred sixteen eyes from 58 consecutive patients who underwent SMILE were included in this prospective study. Each patient underwent SMILE in one eye to a depth of 110 mm and in the other eye to a depth of 130 mm. A Scheimpflug camera and HartmannShack WASCA aberrometer were used to assess the PCE and wavefront aberrations, respectively, before SMILE and at 1 and 3 months after surgery. The PCE was analyzed along 3 optical zones (apex and 2 and 4 mm diameters) as a function of the meridian. Dynamic Scheimpflug imaging was used to evaluate the biomechanics preoperatively and at 1 day, 1 month, and 3 months postoperatively. Results: No significant difference was found in either vision correction or corneal biomechanics between the 2 groups. In both groups, the PCE became significantly flattened at the apex and at 2 mm annulus 1 month postoperatively, especially for the 110-mm cap group. Three months postoperatively, the 110-mm cap group was still flattened significantly, whereas the displacement at the apex in the 130-mm cap group had disappeared. There was no significant difference in wavefront aberrations between the groups after surgery, except in the vertical coma (P, 0.001). Conclusions: The differences in corneal biomechanics between the 110-mm cap group and 130-mm cap group were small; however, the superficial lenticule might cause displacement of the PCE to be more persistent in one eye than in the other.

Abstract:Significance: Our study found that SP-A1 (stiffness parameter at time of first applanation) was statistically different between post-laser-assisted subepithelial keratomileusis (LASEK) and post-small incision lenticule extraction (SMILE) eyes. The stiffness of keratoconus was lower than that of post-refractive surgery corneas. Purpose: To compare corneal biomechanics among post-SMILE, post-LASEK, and keratoconic eyes. Methods: In this retrospective study, 36 eyes of 36 patients after SMILE, 36 eyes of 36 patients after LASEK, and 36 eyes of 36 patients with keratoconus with matching (±5 μm) central corneal thickness (CCT) were examined using Scheimpflug corneal topography (Pentacam HR) and dynamic Scheimpflug analyzer (CorVis ST). Mixed linear model analysis with Bonferroni-adjusted post-hoc comparisons was performed to compare the differences in corneal biomechanics and topographic parameters among the three groups with the CCT and the bIOP (intraocular pressure with biomechanical correction) as the random factor. Results: All groups had comparable CCT at baseline. The bIOPs between the three groups were comparable. The mean values of the Belin/Ambrósio Deviation (BAD-D) in the keratoconus group were significantly higher than those of the SMILE (post hoc p < 0.001) and LASEK groups (post hoc p < 0.001). The SP-A1 in the keratoconus group was the lowest when compared with those of the SMILE (post hoc p = 0.003) and LASEK groups (post hoc p < 0.001). The SMILE group SP-A1 values were slightly lower than those of the LASEK group (post hoc p = 0.044). Conclusions: Keratoconus eyes were significantly softer when compared to post-refractive surgery corneas with comparable corneal thickness in terms of SP-A1 and BAD-D values, while the LASEK group may have the greatest stiffness. Post-SMILE and post-LASEK corneas showed significant differences in SP-A1.

Abstract:Abstract Background Few studies have reported the visual outcomes of small-incision lenticule extraction (SMILE) and laser-assisted subepithelial keratomileusis (LASEK) for myopia correction. This study aims to compare the visual quality and corneal wavefront aberrations after SMILE and LASEK for low-myopia correction. Methods In this prospective study, we included 29 eyes of 29 patients who received SMILE and 23 eyes of 23 patients who received LASEK between June 2018 and January 2019. The following measurements were assessed: uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, corneal wavefront aberrations, and subjective visual quality. All patients were followed up for two years. Results All procedures were uneventful. An efficacy index of 1.19 ± 0.17 was established in the SMILE group and 1.23 ± 0.20 in the LASEK group. No eyes lost more than two lines of CDVA. We found that 93% (27/29) of the treated eyes in the SMILE group and 91% (21/23) in the LASEK group had spherical equivalent (SE) within ± 0.25D. The increases in the total corneal spherical aberration and the corneal front spherical aberration were lower in the SMILE group than in the LASEK group (P < 0.01). In contrast, the increases in the total corneal vertical coma and the corneal front vertical coma in the SMILE group were greater than those in the LASEK group (P < 0.01). Conclusion Both SMILE and LASEK have good safety, stability, and patient-reported satisfaction for low myopia. SMILE induced less corneal spherical aberration but greater vertical coma than LASEK.

Abstract:PURPOSE: To evaluate theoretical biomechanical advantages of human corneas treated with small incision lenticule extraction (SMILE) compared with femtosecond laser-assisted LASIK (FS-LASIK) Patients and methods: In a prospective, comparative, non-randomized, consecutive case series patients with moderate to high myopia and/or astigmatism underwent corneal refractive surgery. Patients either received standard FS-LASIK or SMILE. Preoperatively and up to 3 months postoperatively data were analyzed including tomography with Pentacam HR, Goldmann tonometry, non-contact tonometry, and deformability of the cornea using an ultra-high-speed camera (Corvis ST). Data were analyzed and p < 0.05 was considered as statistically significant (t-test).RESULTS: Seventy-three patients (128 eyes) were treated. Forty-eight eyes of 29 patients underwent FS-LASIK and 80 eyes of 44 patients underwent SMILE. Preoperative spherical equivalent (SE) was -3.23 ± 1.64 D (FS-LASIK group) and -4.83 ± 1.63 D (SMILE group) (p < 0.0001). Almost all parameters obtained by Corvis ST between preoperative and postoperative measurements showed significant changes after refractive surgery. Significant changes were seen between both groups postoperatively. However, matched pair subgroup analysis (n = 69; 26 eyes FS-LASIK; 43 eyes SMILE) of eyes with initially equal pachymetry, intraocular pressure, SE, and difference of pre- to postoperative pachymetry (p>0.05), showed no significant changes in parameters measured with Corvis ST (p>0.05).CONCLUSIONS: Corneal biomechanical parameters measured preoperatively with Corvis ST showed significant differences postoperatively in total and in both groups. In subgroup analysis with homogenous groups, FS-LASIK showed no significant changes in biomechanical data measured with Corvis ST compared with SMILE.

Femtosecond laser-assisted small incision lenticule extraction (SMILE) is becoming the new direction in refractive surgery, and the ocular surface changes after SMILE are attracting increasingly more attention. This article reviews adverse effects, including dry eye, injury of corneal nerves, and ocular surface inflammation, occurring after SMILE.

Abstract:Purpose: To compare early changes in the corneal biomechanical parameters after photorefractive keratectomy (PRK) and small incision lenticule extraction (SMILE) and their correlations with corneal shape parameters. Methods: One hundred twenty four eyes received myopic PRK and SMILE for similar amounts of myopia. Corneal tomography with Pentacam HR, biomechanical parameters using Corvis ST, and Ocular Response Analyzer (ORA) were evaluated before and 2 weeks after surgery. The change in each parameter was compared between groups, while the difference in central corneal thickness and cornea-compensated intraocular pressure measured before and after surgery were considered as covariates. Results: A significant reduction was seen in the corneal stiffness parameter at first applanation, and an increase in deformation amplitude ratio (DAR), and integrated inverse radius (IIR) in both groups after surgery (p < 0.001) Changes in DAR, and IIR were significantly greater in the SMILE than in the PRK group (p < 0.001) Corneal hysteresis (CH) and corneal resistance factor (CRF) decreased in both SMILE and PRK groups after surgery, (p < 0.001) with no statistically significant difference between groups (p > 0.05) Among new Corvis ST parameters, DAR showed a significant correlation with changes in Ambrosio relational thickness in both groups (p < 0.05). Conclusions: Both techniques caused significant changes in corneal biomechanics in the early postoperative period, with greater elastic changes in the SMILE group compared to the PRK group, likely due to lower tension in the SMILE cap and thinner residual stromal bed in SMILE. There were no differences in viscoelastic changes between them, so the lower CH may reflect the volume of tissue removed.

Abstract:PURPOSE: To investigate changes in corneal backscatter after small incision lenticule extraction (SMILE).METHODS: One eye was randomly selected from 34 patients who underwent SMILE procedures. Corneal backscatter was analyzed using Scheimpflug technology before surgery and at postoperative 1 day, 1 week, 1 month, 3, 6, and 12 months, and 3 years. Three different corneal depths (the anterior 120 mu m, the center, and the posterior 60 mu m) and three concentric radial zones (0 to 2, 2 to 6, and 6 to 10 mm) were analyzed. Another 40 eyes of 40 patients who underwent femto-second laser-assisted LASIK (FS-LASIK) were examined before surgery and at postoperative 1 day and 3 years as the control group.RESULTS: At postoperative day 1, a significant increase in corneal backscatter was detected in the SMILE group compared to preoperative values in the central 0 to 6 mm zone of the anterior layer and the 0 to 2 mm zone of the central layer (all P < .01). This increase was higher than the increase in the FS-LASIK group in the central 0 to 6 mm zone of the anterior and central layers (all P < .01). No significant differences in corneal backscatter were detected between preoperative values and the 1-week, and 1-, 3-, 6-, and 12-month follow-up visits in the SMILE group (all P > .05). Moreover, a significant decrease in corneal backscatter was detected in the three zones of all three layers in the two groups at 3 years postoperatively compared to preoperative values (all P < .001) and no significant difference was found between the two groups (all P < .05).CONCLUSIONS: Corneal backscatter peaks and declines to the baseline in the early period after SMILE and corneal transparency achieves an improvement within 3 years postoperatively.

Abstract:PURPOSE:The goal of this study was to compare small incision lenticule extraction (SMILE) with femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for treating myopia. METHODS:The CENTRAL, EMBASE, PubMed databases and a Chinese database (SinoMed) were searched in May of 2016. Twelve studies with 1,076 eyes, which included three randomized controlled trials (RCTs) and nine cohorts, met our inclusion criteria. The overall quality of evidence was evaluated using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) working group framework. Data were extracted and analysed at three to six months postoperatively. Primary outcome measures included a loss of one or more lines of best spectacle corrected visual acuity (BSCVA), uncorrected visual acuity (UCVA) of 20/20 or better, mean logMAR UCVA, postoperative mean spherical equivalent (SE) and postoperative refraction within ±1.0 D of the target refraction. Secondary outcome measures included ocular surface disease index (OSDI), tear breakup time (TBUT) and Schirmer's 1 test (S1T) as dry eye parameters, along with corneal sensitivity. RESULTS:The overall quality of evidence was considered to be low to very low. Pooled results revealed no significant differences between the two groups with regard to a loss of one or more lines in the BSCVA (OR 1.71; 95% CI: 0.81, 3.63; P = 0.16), UCVA of 20/20 or better (OR 0.71; 95% CI: 0.44, 1.15; P = 0.16), logMAR UCVA (MD 0.00; 95% CI: -0.03, 0.04; P = 0.87), postoperative refractive SE (MD -0.00; 95% CI: -0.05, 0.05; P = 0.97) or postoperative refraction within ±1.0 D of the target refraction (OR 0.78; 95% CI: 0.22, 2.77; P = 0.70) within six months postoperatively. The pooled analysis also indicated that the FS-LASIK group suffered more severely from dry eye symptoms (OSDI; MD -6.68; 95% CI: -11.76, -2.00; P = 0.006) and lower corneal sensitivity (MD 12.40; 95% CI: 10.23, 14.56; P < 0.00001) at six months postoperatively. CONCLUSIONS:In conclusion, both FS-LASIK and SMILE are safe, effective and predictable surgical options for treating myopia. However, dry eye symptoms and loss of corneal sensitivity may occur less frequently after SMILE than after FS-LASIK.

Abstract:Purpose: To investigate the changes in dry eye symptoms and clinical signs and corneal sensitivity after small incision lenticule extraction (SMILE) and femtosecond LASIK (femto-LASIK).Design: Prospective, non-randomized comparative study.Methods: The study included a total of 71 eyes of 71 patients; the SMILE group comprised 38 eyes of 38 patients, and the femto-LASIK group comprised 33 eyes of 33 patients. Ocular Surface Disease Index (OSDI), Tear film breakup time (TBUT), the Schirmer test without anesthesia (S1T), corneal fluorescein staining, and central corneal sensation were evaluated before surgery and at 1 week, 1 month, 3 months, and 6 months after surgery.Results: OSDI scores in both groups were increased immediately and returned to preoperative level at 1 month after surgeries. The TBUT values in both groups were reduced after surgeries relative to their preoperative scores. Patients in SMILE group were less likely to have corneal staining compared with those in the femto-LASIK group ([odds ratio] OR = 0.50, 95% [confidence interval] CI 0.28 to 0.93, P = 0.03). Central corneal sensitivity was decreased at all postoperative time points in both groups. However, the central corneal sensation scores in the SMILE group were greater than that in the femto-LASIK group at all of the postoperative time points (all P<0.05).Conclusions: SMILE surgeries resulted in a short-term increase in dry eye symptoms, tear film instability, and loss of corneal sensitivity. Furthermore, SMILE surgeries have superiority over femto-LASIK in lower risk of postoperative corneal staining and less reduction of corneal sensation.
