Abstract:OBJECTIVE:To investigate the relationships of childhood obesity and the duration of sleep as well as insulin resistance. METHODS:Two hundred and sixty-nine simple obese children aged from 7 to 15 years with body mass index(BMI) over 23 and 120 normal healthy children were recruited in this study. Obese children were divided into three groups: Group A: 23 < or = BMI < 25; Group B: 25 < or = BMI < 30; Group C: BMI > or = 30. Duration of sleep was calculated according to the reported sleeping hours during working days and weekends. Oral glucose tolerance test and insulin releasing test were performed in all obese children. Seventy of the 120 healthy children received fasting plasma glucose (FBG) and fasting insulin (FINS) detection. Homeostasis model assessment-insulin resistance (HOMA-IR) and islet beta cell secreting index (HOMA-IS) were calculated. RESULTS:The duration of sleep was shorter in all 3 obese groups than that in the control group and the levels of FBG, FINS and HOMA-IR were higher (P < 0.05). The levels of HOMA-IS in Group B and C were higher than that in the control group (P < 0.05), while HOMA-IS did not show much difference among the 3 obese groups and between Group A and the control group (P > 0.05). Logistic analysis showed that the duration of sleep was related to childhood obese (P < 0.05). Results from multiple linear regression model analysis showed that BMI bore a relation to HOMA-IR and HOMA-IS (P < 0.05), as well as one unit increase in BMI accompanied with 0.261 and 11.054 increase in HOMA-IR and HOMA-IS, respectively. CONCLUSION:Children with shorter duration of sleep were susceptible to become obese. A significant correlation did exist between BMI and insulin resistance.

Abstract:To investigate whether obstructive sleep apnea/hypopnea syndrome (OSAHS) is independently associated with insulin resistance (IR).We recruited 60 male obese patients with OSAHS [OSAHS group, age (42.3 +/- 2.6) years, body mass index (BMI) (28.3 +/- 2.1) kg/m2, waist/hip ratio (WHR) 0.95 +/- 0.05], 60 male weight-matched obese patients without OSAHS [OB group, age (41.5 +/- 3.1) years, BMI (27.7 +/- 1.5) kg/m2, WHR 0.94 +/- 0.04] and 60 male normal weight subjects [NW group, age (41.8 +/- 2.4) years, BMI (22.6 +/- 1.9) kg/m2, WHR 0.86 +/- 0.05]. The subjects underwent polysomnography and OSAHS was defined as an apnea-hypopnea index (AHI) > or = 5. The systolic blood pressure and diastolic blood pressure were measured. IR was evaluated by fasting serum true insulin (TI) level and IR index based on the homeostasis model assessment method (HOMA-IR). In the OSAHS group, multiple linear regression was used with either TI or HOMA-IR as the dependent variable, and the corresponding set of independent variables included age, BMI, WHR, AHI and minimum oxygen saturation (MSpO2).After adjustment for age, BMI, and WHR, the OSAHS group was more insulin resistant, as indicated by the higher levels of TI and HOMA-IR. Multiple linear regression showed that the central obesity parameter (WHR) was the major determinant of IR of the OSAHS group, while sleep-disordered breathing parameters (AHI and MSpO2) were also independent determinants of IR of the group (TI: AHI P = 0.017, TI: MSpO2 P = 0.005; HOMA-IR: AHI P = 0.008, HOMA-IR: MSpO2 P = 0.003).OSAHS may be independently associated with IR.
Circulating Endocannabinoids and Insulin Resistance in Patients with Obstructive Sleep Apnea

Abstract:Objectives. The purpose of this study is to investigate the relationship between plasma endocannabinoids and insulin resistance (IR) in patients with obstructive sleep apnea (OSA). Methods. A population of 64 with OSA and 24 control subjects was recruited. Body mass index (BMI), waist circumference, lipids, blood glucose and insulin, homeostasis model of assessment for insulin resistance index (HOMA-IR), anandamide (AEA), 1/2-arachidonoylglycerol (1/2-AG), and apnea-hypopnea index (AHI) were analyzed. Results. Fasting blood insulin (22.9 ± 7.8 mIU/L versus 18.5 ± 7.2 mIU/L, P < 0.05), HOMA-IR (2.9 ± 1.0 versus 2.4 ± 0.9, P < 0.01), AEA (3.2 ± 0.7 nmol/L versus 2.5 ± 0.6 nmol/L, P < 0.01), and 1/2-AG (40.8 ± 5.7 nmol/L versus 34.3 ± 7.7 nmol/L, P < 0.01) were higher in OSA group than those in control group. In OSA group, AEA, 1/2-AG, and HOMA-IR increase with the OSA severity. The correlation analysis showed significant positive correlation between HOMA-IR and AHI (r = 0.44, P < 0.01), AEA and AHI (r = 0.52, P < 0.01), AEA and HOMA-IR (r = 0.62, P < 0.01), and 1/2-AG and HOMA-IR (r = 0.33, P < 0.01). Further analysis showed that only AEA was significantly correlated with AHI and HOMA-IR after adjusting for confounding factors. Conclusions. The present study indicated that plasma endocannabinoids levels, especially AEA, were associated with IR and AHI in patients with OSA.
Relationship Between the Severity of Obstructive Sleep Apnea Hypopnea Syndrome and Blood Pressure Type and Insulin Resistance in "syndrom Z" Patients

Abstract:Objective To investigate the relationships between the severity of sleep apnea,the pattern of blood pressure and insulin resistance in Z patients. Methods One hundred and ninety-two patients were enrolled. They were diagnosed with polysomnography and were divided into three groups according to the severity of apnea:metabolic syndrome alone (n=88),mild obstructive sleep apnea hypopnea syndrome (OSAHS) with metabolic syndrome (n=53),moderate to severe OSAHS with metabolic syndrome (n=51). Twenty-four hours ambulatory blood pressure measurement was performed in all patients. The concentrations of fasting plasma glucose,fasting insulin and plasma lipids were measured in the morning. HOMA-IR was used as insulin resistant index. Results Mean blood pressure in Z groups was higher than that in MS group (P0.05). Night blood pressure was significantly high and the ratio of non-dipper blood pressure pattern was obviously high in syndrom Z patients. The manifestation of serum lipids profile in syndrom Z group was increased triglyceride and decreased level of high density lipoprotein cholesterol,which was accompanied with the severity of sleep disorder breath. No significant change of total cholesterol and low density lipoprotein cholesterol were observed. Fasting insulin and HOMA-IR index in syndrom Z group were higher than those in MS group,and increased with increment of severity of sleep apnea (P0.05). Spearman correlation analysis revealed that apnea hypopnea index was positively correlation with HOMA-IR index and fasting insulin (P0.05). Conclusion The components of metabolic syndrome in syndrom Z was prominent,it include higher blood pressure,increased ratio of non-dipper blood pressure pattern,manifestation of serum lipids profile and insulin resistant.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is increasingly associated with insulin resistance. The underlying pathophysiology remains unclear but rapid eye movement (REM) sleep has been hypothesized to play a key role. To investigate the associations of insulin resistance with respiratory events and sleep duration during REM sleep, 4,062 Han Chinese individuals with suspected OSA were screened and 2,899 were analyzed. Methods We screened 4,062 participants with suspected OSA who underwent polysomnography in our sleep center from 2009 to 2016. Polysomnographic variables, biochemical indicators, and physical measurements were collected. Logistic regression analyses were conducted to determine the odds ratios (ORs) and 95% confidence intervals (95% CIs) for insulin resistance as assessed by hyperinsulinemia, the homeostasis model assessment of insulin resistance (HOMA-IR), fasting insulin resistance index (FIRI), and Bennet’s insulin sensitivity index (ISI). Results The final analyses included 2,899 participants. After adjusting for age, gender, body mass index, waist circumference, mean arterial pressure, smoking status, alcohol consumption, and the apnea and hypopnea index during non-REM sleep (AHINREM), the results revealed that AHI during REM sleep (AHIREM) was independently associated with insulin resistance; across higher AHIREM quartiles, the ORs (95% CIs) for hyperinsulinemia were 1.340 (1.022, 1.757), 1.210 (0.882, 1.660), and 1.632 (1.103, 2.416); those for abnormal HOMA-IR were 1.287 (0.998, 1.661), 1.263 (0.933, 1.711), and 1.556 (1.056, 2.293); those for abnormal FIRI were 1.386 (1.048, 1.835), 1.317 (0.954, 1.818), and 1.888 (1.269, 2.807); and those for abnormal Bennet’s ISI were 1.297 (1.003, 1.678), 1.287 (0.949, 1.747), and 1.663 (1.127, 2.452) (P < 0.01 for all linear trends). Additionally, the results showed that for every 1-h increase in REM duration, the risk of hyperinsulinemia decreased by 22.3% (P < 0.05). Conclusion The present study demonstrated that AHIREM was independently associated with hyperinsulinemia and abnormal HOMA-IR, FIRI, and Bennet’s ISI. Additionally, REM sleep duration was independently associated with hyperinsulinemia. The relationship of obstructive sleep apnea-hypopnea syndrome with hypertension and insulin resistance

Abstract:Objective To explore the relationship of obstructive sleep apnea-hypopnea syndrome (OSAS) with hypertension and insulin resistance.Methods Forty-eight patients with OSAS (OSAS group) and 30 hypertension patients without OSAS were selected as pure hypertension group,and 26 normal subjects served as control group.The OSAS group was divided into two groups according to their blood pressure,namely hypertensive group and normotensive group. Polysomnography and ABPM were examined during sleep,and fasting plasma glucose and fasting plasma insulin(FINS) were detected in all groups.Results Compared with pure hypertension group,the proportion of non-liper-type hypertension and pulse pressure significantly increased in hypertensive group.Fasting plasma glucose,FINS and HOMA-IR were significantly higher in OSAS group than in control group (P< 0.05) regardless of their blood pressure.HOMA-IR and FINS were significantly positively correlated to apnea hypopnea index.Moderate and severe OSAS affects the rhythm of blood pressure variability as well as does the pulse pressure.Conclusion OSAS may be independently associated with insulin resistance.
Insulin Resistance in 54 Cases with Obstructive Sleep Apnea-Hypopnea Syndrome(OSAHS)

Abstract:Objective To investigate the relationship between sleep related hypoxemia,serum leptin level and the extent of insulin resistance in OSAHS patients.Methods Fifty-four male patients(diagnosed) as OSAHS with 8-hour PSG monitoring were enrolled in this study.According to their apnea-hypopnea index(AHI),these patients were divided into two groups: 27 cases in the mild/moderate group and 27 in the severe group.Body height and weight,serum leptin,fasting blood glucose(FBG) and fasting serum insulin(FINS) were measured and their body mass index(BMI) and HOMA-IR index were calculated afterwards.Results The nocturnal average SpO_2 and nocturnal lowest SpO_2 were significantly decreased in the severe OSAHS group than those in the mild-moderate group while the BMI,serum leptin level and HOMA index were increased in the former group with statistical significance(P<0.01). The serum leptin level and HOMA index were positively correlated with BMI and AHI and negatively correlated with the nocturnal lowest SpO_2 respectively with statistical significance.In addition,serum leptin concentration was also negatively correlated with nocturnal average SpO_2 level.Conclusion The extent of respiratory disturbance in OSAHS is closely related with obesity,leptin and other insulin resistance parameters.
[A Study of Relationship Between Obstructive Sleep Apnea-Hypopnea Syndrome in Hypertension and Insulin].

Abstract:To investigate the association between the obstructive sleep apnea-hypopnea syndrome (OSAHS) in hypertension and insulin.A total of 521 patients were divided into 4 groups according to apnea-hypopnea index and OSAHS degrees. The control group (group I), mild OSAHS group (group II), moderate OSAHS group (group III) and severe OSAHS group (group IV) had 89 patients, 194 patients, 118 patients and 120 patients respectively.The BMI [(30.4 ± 3.8) kg/m(2)], apnea-hypopnea index (AHI, 3.8 ± 0.1), Fasting insulin (FIns) [(3.08 ± 0.26) mU/L] and insulin resistance (2.43 ± 0.27) of patients in severe OSAHS group were significantly higher than that of in the control, mild OSAHS group and moderate OSAHS group (P < 0.01). The levels of saturation of minimum oxygen from skin of patients in severe OSAHS group was significantly lower (MSpO(2)) than in that of the control, mild OSAHS group and moderate OSAHS group. Multiple linear regression analysis showed that fasting plasma insulin and insulin resistance was positive correlation with apnea-hypopnea index, while they also negatively associated with saturation of minimum oxygen.FIns and insulin resistance strongly associate with AHI and levels of saturation of minimum oxygen from skin. Hypertensive patients with OSAHS have more chances to suffer with insulin resistance.
Relationship of True Insulin and Proinsulin to Body Fat Distribution in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome

Abstract:Objective: To study the relationship between serum true insulin (TI), proinsulin(PI) and the body fat distribution in patients with obstructive sleep apnea- hypopnea syndrome (OSAHS). Methods: Seventy-two OSAHS patients were divided into three groups, mild group, moderate group and severe group according to apnea hypopnea index (AHI), meanwhile 15 healthy people were selected as control group. The serum levels of TI, PI and body mass index(BMI), waist circumference (WC), neck circumference(NC) were detected in four groups. Results: The levels of PI and TI were higher in OSAHS group than those of control group, and there were significant differences between four groups(P 0.05). The results of Pearson analysis indicat- ed that the levels of TI and PI were positively correlated with BMI, WC and NC in OSAHS patients. Conclusion: Both serum levels of TI and PI were correlated to body fat distribution in OSAHS patients.
Association of apnea-hypopnea index during rapid eye movement sleep with insulin resistance in patients with suspected obstructive sleep apnea: a cross-sectional study

Abstract:BACKGROUND: Obstructive sleep apnea (OSA) is associated with insulin resistance. However, the association between special stages of OSA [rapid eye movement (REM) sleep] and insulin resistance is not clear. This study was designed to assess the association of the frequency of respiratory events during REM sleep with insulin resistance in adults with suspected OSA.METHODS: In this cross-sectional study, 4,062 adult participants with suspected OSA who underwent polysomnography in our sleep center between 2009 and 2016 were screened. Polysomnographic variables, biochemical indicators, and physical measurements were collected. Logistic regression analyses were conducted to determine the odds ratios (ORs) and 95% confidence intervals (95% CIs) for insulin resistance as assessed by the presence of hyperinsulinemia, the homeostasis model assessment of insulin resistance (HOMA-IR) index, the fasting insulin resistance index (FIRI), and Bennett's insulin sensitivity index (ISI).RESULTS: The final analyses included 2,899 adults with suspected OSA. Multivariate adjustments, including the apnea-hypopnea index (AHI) during non-REM sleep (AHINREM), were performed. The AHI during REM sleep (AHIREM) was found to be independently associated with insulin resistance across increasing AHIREM quartiles. For hyperinsulinemia the ORs (95% CIs) followed the order of 1.340 (1.022, 1.757), 1.210 (0.882, 1.660), and 1.632 (1.103, 2.416). For abnormal HOMA-IR, ORs (95% CIs) were 1.287 (0.998, 1.661), 1.263 (0.933, 1.711), and 1.556 (1.056, 2.293). For abnormal FIRI, ORs (95% CIs) were 1.386 (1.048, 1.835), 1.317 (0.954, 1.818), and 1.888 (1.269, 2.807). Additionally, the results showed that REM sleep duration was independently associated with hyperinsulinemia (OR =0.777, 95% CI: 0.615-0.982; P<0.05).CONCLUSIONS: AHIREM was independently associated with hyperinsulinemia and an abnormal HOMA-IR, FIRI, and Bennett's ISI in adults with suspected OSA. Additionally, REM sleep duration was independently associated with hyperinsulinemia.

Comparative analysis of obstructive sleep apnea hypopnea syndrome and insulin resistance in obese young adults

Abstract:Objective To investigate the relationship between insulin resistance and the severity of obstructive sleep apnea hypopnea syndrome (OSAHS) by comparing the clinical conditions of obese young adults with or without OSAHS. Methods A total of 250 obese young adults were collected consecutively from the specialized outpatient clinic for obesity in Ruijin Hospital. Polysomnography was applied to diagnose OSAHS and apnea hypopnea index (AHI) to evaluate the severity of OSAHS. All subjects underwent a standard 75g oral glucose tolerance test (OGTT) in order to evaluate their glucose tolerance levels. Insulin resistance was determined by homeostasis model assessment of insulin resistance (HOMA-IR) and by the insulin sensitivity index (SI) derived from Bergman′s minimal model. Results All subjects were divided into 4 groups according to AHI levels: non-OSAHS group, mild OSAHS group, moderate OSAHS group and severe OSAHS group. With the increasing of OSAHS severity, body mass index (BMI), waist circumference, neck circumference, waist hip ratio, fasting blood glucose (FPG), lipid profiles and hemoglobin Alc (HbAlc) all increased (P<0.05) but SI decreased (P<0.05). After adjustment for age and BMI, FPG, fasting insulin, HOMA-IR and SI were all independently associated with AHI levels. Conclusions The severity of OSAHS was positively associated with insulin resistance and negatively correlated with insulin sensitivity in obese young adults.
Relationships between apolipoprotein E and insulin resistance in patients with obstructive sleep apnoea: a large-scale cross-sectional study

Abstract:Background: Obstructive sleep apnoea (OSA) is commonly associated with insulin resistance (IR) and dyslipidaemia. Apolipoprotein E (APOE) plays important roles in lipid metabolism. The study aimed to disentangle the multifactorial relationships between IR and APOE based on a large-scale population with OSA. Methods: A total of 5,591 participants who underwent polysomnography for OSA diagnosis were finally enrolled. We collected anthropometric, fasting biochemical and polysomnographic data for each participant. Linear regression analysis was performed to evaluate the relationships between APOE, IR, and sleep breathing-related parameters. Logistic regression, restricted cubic spline (RCS) and mediation analyses were used to explore relationships between APOE and IR in patients with OSA. Results: Increasing OSA severity was associated with greater obesity, more obvious dyslipidaemia, and higher levels of APOE and IR. APOE was positively correlated with the apnoea-hypopnoea index (AHI), oxygen desaturation index (ODI) and microarousal index (MAI) even after adjusting for age, sex, body mass index, and smoking and drinking levels (β = 0.107, β = 0.102, β = 0.075, respectively, all P < 0.001). The risks of IR increased from the first to fourth quartiles of APOE (odds ratio (OR) = 1.695, 95% CI: 1.425-2.017; OR = 2.371, 95% confidence interval (CI): 2.009-2.816; OR = 3.392, 95% CI: 2.853-4.032, all P < 0.001) after adjustments. RCS analysis indicated non-linear and dose response relationships between APOE, AHI, ODI, MAI and insulin resistance. Mediation analyses showed that HOMA-IR explained 9.1% and 10% of the association between AHI, ODI and APOE. The same trends were observed in men, but not in women. Conclusions: This study showed that APOE is a risk factor for IR; moreover, IR acts as a mediator between OSA and APOE in men. APOE, IR, and OSA showed non-linear and multistage relationships. Taken together, these observations revealed the complex relationships of metabolic disorders in patients with OSA, which could lead to the development of new treatment modalities and a deeper understanding of the systemic impact of OSA.

Interrelationships among Common Predictors of Cardiovascular Diseases in Patients of OSA: A Large-Scale Observational Study.

Abstract:Background and aims: The apolipoprotein B/apolipoprotein A-I (ApoB/ApoA-I) and insulin resistance has been recognized as common cardiovascular diseases (CVD) risk factors. However, whether they were biomarkers for 10-year CVD risk in obstructive sleep apnea (OSA) had been rarely studied. Besides, interrelationships between the ApoB/ApoA-I, insulin resistance and OSA remain unclear. Methods and results: A total of 4010 subjects were finally included. Anthropometric, fasting biochemical, and polysomnographic parameters were collected. 10-year Framingham CVD risk score (FRS) was calculated for each subjects. The relationships between insulin resistance, OSA risk and the ApoB/ApoA-I was evaluated through logistic regressions analysis, restricted cubic spline (RCS) analysis and mediation analysis. ApoB/ApoA-I, HOMA-IR and AHI were all risk factors for high10-year CVD risk as assessed by FRS (odds ratios (OR) = 5.365, 1.094, 1.010, respectively, all P < 0.001)). The fully adjusted OR (95% confidence intervals) for both OSA [1 (reference), 1.308 (1.027-1.665), 1.517 (1.178-1.953), and 1.803 (1.371-2.372)] and insulin resistance [1 (reference), 1.457 (1.173-1.711),1.701 (1.369-2.113), 2.051(1.645-2.558)] increased from the first to the fourth quartiles of the ApoB/ApoA-I. The RCS mapped a nonlinear dose-effect relationship between the ApoB/ApoA-I and risk of insulin resistance and OSA. Mediation analyses showed HOMA-IR explain 9.7%, 4.7% and 10.8% of the association between apnea-hypopnea index, oxygen desaturation index, micro-arousal index and ApoB/ApoA-I, respectively. Conclusions: Our study revealed that ApoB/ApoA-I, insulin resistance and OSA were risk factors for CVD. Insulin resistance may serve as a potential mediator in OSA-related lipoprotein disorders and further increase CVD risk. Abstract:Objective To systematically review the evidence for the link between obstructive sleep aponea syndrome(OSAS),glucose intolerance,and insulin resistance(IR).Methods Searches of MEDLINE,OVID,ELSEVIER,CNKI and WANFANG data were performed for OSAS and IR.Results Sixty-one articles met the inclusion criteria(40 with English and 21 with Chinese).25 of 35 studies which have used objective measures of OSAS provide support for an independent link between OSAS,glucose intolerance,and IR.14 of 26 studies that used continuous positive airway pressure(CPAP)to treat IR indicated that CPAP was effective,but most of them did not have control group.Conclusions OSAS is independently associated with glucose intolerance and IR.However,randomized controlled studies of large sample are needed to identified the improvement of metabolic disturbance after treatment.

Abstract:Adiponectin is an adipocyte-derived hormone with insulin-sensitizing properties. Recent studies have demonstrated serum adiponectin levels were significantly lower in obstructive sleep apneahypopnea syndrom(OSAHS)patients and insulin resistance is a typical feature of OSAHS and its complications. So low adiponectin may play an important role in the development of OSAHS and its complications.
Metabolic profile and insulin resistance in patients with OSAS

Abstract:Objective To investigate the metabolic profile and its relationship with insulin resistance in patients with obstructive sleep apnea-hypopnea syndrome(OSAS). Methods Fifty-four male patients with OSAS were enrolled and performed polysomnography(PSG).According to the apnea hypopnea index(AHI),all the patients were grouped into OSAS severe group(n=39,AHI>40/h) and non-severe group(n=15,AHI 5-40/h).Anthropometric measurements,fasting plasma glucose,insulin,blood fat,and CT quantitative measurement of abdominal adipose tissue were recorded. Results Insulin resistance index(HOMA-IR) in patients with OSAS was related to hypoxia independently of obesity variables.The severe group was characterized by more serious metabolic disorders and higher prevalence of metabolic syndrome than the non-severe group.OSAS was positively associated with an increased metabolic disorders risk for the severe group versus the non-severe group(OR=8.8).Using the receiver operating characteristic(ROC) curve analysis,waist circumference had the greatest areas under the ROC curves compared with body mass index and neck circumference.The results of multiple stepwise regression of lowest pulse oxygen saturation(LSpO2)during overnight sleep indicated that neck circumference followed by epworth sleepiness score(ESS) entered the equation(P<0.05). Conclusion OSAS is an independent risk factor for insulin resistance.Waist circumference and neck circumference are the most simple indexes in predicting metabolic syndrome and hypoxia,respectively.
