Psychology and Behavioural and Clinical Neuroscience Institute, United Kingdom The prefrontal cortex has several distinct executive functions associated with working memory, response inhibition and cognitive flexibility that are modulated by the ascending monoamine systems in apparently different ways. For example, in studies on humans, marmosets and rats it is apparent that the catecholamine systems (dopamine and noradrenaline) influence working memory and extra-dimensional set-switching, as well as aspects of response inhibition, as measured by the stop-signal reaction time task. By contrast, manipulation of the serotoninergic (5-HT) systems appears to have distinct effects which affect reversal learning and decision-making cognition, but not for example, set-shifting or stop-signal inhibition. It will be shown how such effects are relevant to neuropsychiatric conditions such as attention-deficit hyperactivity disorder, obsessive compulsive disorder and schizophrenia by means of studies in clinical patients. Studies of the therapeutic mechanisms of action of drugs such as methylphenidate and the selective noradrenaline reuptake inhibitor atomoxetine will also be described using pharmacological functional magnetic imaging methods that allow us to begin to understand their neural modes of action. Abstract:Schizophrenia (SCZ) is a complex mental disorder characterized by a range of symptoms, including positive and negative symptoms, as well as cognitive impairments. Despite the extensive research, the underlying neurobiology of SCZ remain elusive. To overcome this challenge, the use of diverse laboratory modeling techniques, encompassing cellular and animal models, and innovative approaches like induced pluripotent stem cell (iPSC)-derived neuronal cultures or brain organoids and genetically engineered animal models, has been crucial. Immortalized cellular models provide controlled environments for investigating the molecular and neurochemical pathways involved in neuronal function, while iPSCs and brain organoids, derived from patient-specific sources, offer significant advantage in translational research by facilitating direct comparisons of cellular phenotypes between patient-derived neurons and healthy-control neurons. Animal models can recapitulate the different psychopathological aspects that should be modeled, offering valuable insights into the neurobiology of SCZ. In addition, invertebrates' models are genetically tractable and offer a powerful approach to dissect the core genetic underpinnings of SCZ, while vertebrate models, especially mammals, with their more complex nervous systems and behavioral repertoire, provide a closer approximation of the human condition to study SCZ-related traits. This narrative review provides a comprehensive overview of the diverse modeling approaches, critically evaluating their strengths and limitations. By synthesizing knowledge from these models, this review offers a valuable source for researchers, clinicians, and stakeholders alike. Integrating findings across these different models may allow us to build a more holistic picture of SCZ pathophysiology, facilitating the exploration of new research avenues and informed decision-making for interventions.

Abstract:The objective : of the study was to present the main provisions of a new clinical and biological model of schizophrenia, which establishes links between disorders in the field of biological processes and the formation of positive and negative in the general psychopathological space of schizophrenia on the basis of the analysis of long-term results of clinical and biological research of the Mental Health Research Centre. Material and method: by keywords “schizophrenia”, “biological hypotheses of schizophrenia”, “neuroinflammation”, “neurodegeneration”; “positive disorders”, “negative disorders”, “neuroimmune relationships”, “catabolismof tryptophan”, “activation of microglia”, publications from PubMed/MEDLINE databases, RSCI and other sources were analyzedover the past 10 years in comparison with the results of clinical and biological studies of schizophrenia at the Mental Health Science Center. Results: comparison and analysis of current biological hypotheses of schizophrenia indicates that immunological studies are the most promising for solving the problem of establishing links between disorders of neurobiological processes and psychopathological specificities of schizophrenia. Within the framework of the new clinical and biological model, a key role is assigned to the process of neuroinflammation, which determines the pathogenesis of both negative and positive disorders by various, albeit interrelated, molecular mechanisms. One of these mechanisms, associated with the development of reversible positive symptom complexes, is based on an imbalance in the neurotransmitter systems, which is formed as a result of the effect of proinflammatory cytokines on tryptophan catabolism. Another mechanism that determines the development of negative symptom complexes is associated with the influence of cytotoxic metabolites on the processes of neurodegeneration. Conclusion : a new clinical and biological model of schizophrenia establishes a paradigm of the relationship between disorders in the sphere of biological processes determined by neuroinflammation/inflammation and the formation of the main procedural dimensions — positive and negative disorders in the general psychopathological space of schizophrenia. This model makes it possible to clarify some general provisions related to the pharmacotherapy of schizophrenia and the relief of negative disorders, and also serves as the basis for the development of new approaches to early diagnosis, clinical and social prognosis.

Abstract:This overview describes the goals and objectives of the third conference conducted as part of the Cognitive Neuroscience Treatment Research to Improve Cognition in Schizophrenia (CNTRICS) initiative. This third conference was focused on selecting specific paradigms from cognitive neuroscience that measured the constructs identified in the first CNTRICS meeting, with the goal of facilitating the translation of these paradigms into use in clinical trials contexts. To identify such paradigms, we had an open nomination process in which the field was asked to nominate potentially relevant paradigms and to provide information on several domains relevant to selecting the most promising tasks for each construct (eg, construct validity, neural bases, psychometrics, availability of animal models). Our goal was to identify 1-2 promising tasks for each of the 11 constructs identified at the first CNTRICS meeting. In this overview article, we describe the on-line survey used to generate nominations for promising tasks, the criteria that were used to select the tasks, the rationale behind the criteria, and the ways in which breakout groups worked together to identify the most promising tasks from among those nominated. This article serves as an introduction to the set of 6 articles included in this special issue that provide information about the specific tasks discussed and selected for the constructs from each of 6 broad domains (working memory, executive control, attention, long-term memory, perception, and social cognition).

Abstract:Psychiatry is the science of psychopathology, and as such, the measurement of behaviour and cognition is central to its theory and methodology. Neuropsychology provides such measures, which can be used to augment the psychiatric interview and other clinical instruments, as well as to provide an interface with other important approaches including functional brain imaging (see Dolan, this volume), functional genomics, and clinical psychopharmacology. Clinical neuropsychology has developed via the need to assess brain-damaged patients, whereas in most neuropsychiatric disorders, such damage is much less well defined if it is present at all. Thus, whilst there is growing information about specific brain abnormalities in many forms of neuropsychiatric disorder, the lesion model is not necessarily the most appropriate. Moreover, some of the deficits in disorders such as depression and anxiety involve subtle interactions between specific emotional and attentional mechanisms with cognitive function. Therefore, the study of neuropsychiatric patients has also enriched our understanding of clinical neuropsychology. We predict that these aspects of the discipline will develop considerably in the next few years, particularly in combination with data from other domains such as functional brain imaging and pharmacogenetics. Indeed, the specification of specific neural systems implicated in core behavioural or cognitive processes may well aid the enterprise of psychiatric genetics by providing more precise definitions of phenotypes (or endophenotypes) than are currently feasible in nosology (e.g. as defined by the Diagnostic and Statistical Manual).

Abstract:Most research concerning premorbid functioning in schizophrenia has focused on measures of psychosocial adjustment, but biological variables may also provide valuable information about the nature of premorbid adjustment. Research dealing with biological aspects of premorbid functioning has focused on the relationship between premorbid status and two types of variables. One large group of studies has investigated psychophysiological patterns of resting activity and reactivity to stimulation. These investigations have been carried out on patients diagnosed as schizophrenics and also on nonschizophrenic subjects considered at increased risk for becoming schizophrenic (e.g., Mednick and McNeil 1968). A second major group of studies has investigated reactions to neuroleptic medication by patients with various levels of premorbid functioning. In this report, we will review the findings from these research approaches so as to clarify the relationship between premorbid function and biological variables.

Abstract:Schizophrenia is a debilitating disease that presents with both positive and negative symptoms affecting cognition and emotions. Extensive studies have analyzed the different factors that contribute to the disorder. There is evidence of significant genetic etiology involving multiple genes such as dystrobrevin binding protein 1 (DTNBP1) and neuregulin 1 (NRG1). There is no clear link between neurotransmitter changes and the pathophysiology of schizophrenia; however, studies have shown that subcortical dopamine dysfunction is the key mechanism. Specific regions of gray and white matter changes are observed in patients with schizophrenia; gray matter changes being more significant after the onset of psychosis. These pathological changes may be implicated in the impairment of executive functioning, attention, and working memory. The disease can be managed with pharmacological treatments based on individual patient profile, patient compliance, and disease severity. The challenge of disease management sometimes persists due to the side effects. A better understanding of the pathological processes in schizophrenia may lead to more specific and effective therapies.

Abstract:New data from the MRC funded project "Integrating psychological models with biological pathways in psychosis" will be presented. The overall objective of this project is to use both environmental and genetic data to understand the biological pathways in patients with schizophrenia and bipolar disorders. Specifically, to find out if polygenic risk and childhood adverse events increase the relative risk of mental illness above that of its individual case-control explained variance, and secondly, the effect of both polygenic risk and childhood adverse events on clinical characteristics and ageing processes. Abstract:The Cognitive Neuroscience Treatment Research to Improve Cognition in Schizophrenia (CNTRICS) initiative, funded by an R13 from the National Institute of Mental Health, seeks to enhance translational research in treatment development for impaired cognition in schizophrenia by developing tools from cognitive neuroscience into useful measures of treatment effects on behavior and brain function. An initial series of meetings focused on the selection of a new set of tasks from cognitive neuroscience for the measurement of treatment effects on specific cognitive and neural systems. Subsequent validation and optimization studies are underway and a subset of validated measures with well-characterized psychometric properties will be generally available in 2011. This article describes results of the first meeting of the second phase of the Cognitive Neuroscience Treatment Research to Improve Cognition in Schizophrenia, which seeks to develop imaging biomarkers and improved animal models to enhance translational research. In this meeting, we considered issues related to the use of methods such as functional magnetic resonance imaging, electroencephalography, magnetoencephalography, and transcranial magnetic simulation as biomarkers for treatment development. We explored the biological nature of the signals measured by each method, their validity and reliability as measures of cognition-related neural activity, potential confounds related to drug effects on the signal of interest, and conceptual, methodological, and pragmatic issues related to their use in preclinical, first into human, and multicenter phase II and III studies. This overview article describes the background and goals of the meeting together with a summary of the major issues discussed in more detail in the accompanying articles appearing in this issue of Biological Psychiatry.
Schizophrenia and neurosurgery: systematic review and theories

Abstract:Objective: Despite its relatively low prevalence, schizophrenia has a high burden of illness due to its lifelong effects and the fact that it is often refractory to psychotropic treatment. This review investigated how neurosurgical interventions, primarily neuromodulation through deep brain stimulation (DBS), can mitigate treatment-refractory schizophrenia. Pathophysiological data and ongoing clinical trials were reviewed to suggest which targets hold promise for neurosurgical efficacy. Methods: A systematic review of the literature was conducted via an electronic search of the PubMed, Scopus, and Web of Science databases. Included papers were human or animal studies of neurosurgical interventions for schizophrenia conducted between 2012 and 2022. An electronic search of ClinicalTrials.gov and the International Clinical Trials Registry Platform was conducted to find ongoing clinical trials. The ROBINS-I (Risk of Bias in Nonrandomized Studies of Interventions) assessment tool was used to evaluate risk of bias in the study. Results: Eight human and 2 rat studies were included in the review. Of the human studies, 5 used DBS targeting the nucleus accumbens, subgenual anterior cingulate cortex, habenula, and substantial nigra pars reticulata. The remaining 3 human studies reported the results of subcaudate tractotomies and anterior capsulotomies. The rat studies investigated DBS of the nucleus accumbens and medial prefrontal cortex. Overall, human studies demonstrated long-term reduction in Positive and Negative Syndrome Scale scores in many participants, with a low incidence of surgical and psychological side effects. The rat studies demonstrated improved prepulse and latent inhibition in the targeted areas after DBS. Conclusions: As identified in this review, recent studies have investigated the potential effects of therapeutic DBS for schizophrenia, with varying results. DBS targets that have been explored include the hippocampus, subgenual anterior cingulate cortex, habenula, substantia nigra pars reticulata, and medial prefrontal cortex. In addition to DBS, other neuromodulatory techniques such as neuroablation have been studied. Current evidence suggests that neuroablation in the subcaudate tract and anterior capsulotomy may be beneficial for some patients. The authors recommend further exploration of neuromodulation for treatment-refractory schizophrenia, under the condition that rigorous standards be upheld when considering surgical candidacy for these treatments, given that their safety and efficacy remain to be determined.

Abstract:Schizophrenia is a complex, heritable brain disorder characterized by psychotic, negative, cognitive, mood, and motor symptoms. This pictorial review explores the multifaceted nature of schizophrenia, from its etiology to prevention strategies. We discuss the interplay of genetic and environmental risk factors, neurobiological underpinnings, and stepwise progression. Recent advances in understanding circuit-level pathophysiology and neurotransmitter systems beyond dopamine are highlighted along with neuropathological findings, particularly the exaggerated synaptic pruning hypothesis. Based on these developments, we present an updated perspective on pharmacological interventions. Finally, we outline preventative strategies across different stages, emphasizing early intervention. This overview, designed as a teaching resource, aims to provide trainees, clinicians and researchers with a current understanding of schizophrenia's neurobiological underpinnings and the implications of such understanding to the evolving landscape of its diagnosis and management.

Abstract:Background Peripheral immune profiling studies have consistently identified inflammatory subgroups within primary psychiatric disorders such as major depressive disorder. However, clinical trials that have rationally selected immunomodulatory therapeutic agents have had mixed results, and few have stratified patients by immune biomarker status. Therefore, there are critical gaps in our knowledge about how peripheral inflammatory markers affect central immune status and which immune biomarkers can be used to inform clinical trial design. Methods Accordingly, we conducted a narrative review of immune biomarkers in psychiatric illness: circulating immune markers including C-reactive protein (CRP), non-human primate and human experimental studies of central immune responses to administration of peripheral immune molecules, inflammatory biomarker measurements in clinical trials, the utility of Mendelian randomization for ascribing causality to inflammatory biomarkers in depression, and the relationship between peripheral-central immune gene expression. Results Immune biomarker studies suggest that circulating immune markers can identify patients who would benefit from anti-inflammatory treatment and can inform selection of antidepressants. Although elevated CRP levels have been linked to poor response to SSRIs, other immune biomarkers may serve as better stratification indices for treatment trials. Detailed results from our findings will be discussed during the NSAS workshop (June, 2023) and presented at ISPNE. Conclusion We will synthesize our findings to identify: which immune biomarkers may be useful for patient stratification in clinical trials of depression and other psychiatric illnesses, unappreciated rational immune targets based on existing literature, and gaps in our understanding of immune biomarkers that should inform future experimental research studies.

Epidemiology-driven neurodevelopmental animal models of schizophrenia

Abstract:Human epidemiological studies have provided compelling evidence that the risk of developing schizophrenia is significantly enhanced following prenatal and/or perinatal exposure to various environmental insults, including maternal exposure to stress, infection and/or immune activation, nutritional deficiencies and obstetric complications. Based on these associations, a great deal of interest has been centered upon the establishment of neurodevelopmental animal models which are based on prenatal and/or perinatal exposure to such environmental stimuli. In the present review, we describe this relatively novel class of epidemiology-based animal models in relation to the etiology, neurobiology and psychopharmacology of schizophrenia. Thereby, we discuss the general design and practical implementation of these models, and we provide an integrative summary of experimental findings derived from diverse epidemiology-based models, including models of maternal exposure to psychological stress, glucocorticoid treatment, viral infection, immune activating agents, protein deprivation, vitamin D deficiency, as well as models of obstetric complications in the form of birth by Caesarian section and perinatal/postnatal hypoxia. We highlight that the long-term consequences of prenatal exposure to these environmental challenges in animals successfully capture a broad spectrum of structural and functional brain abnormalities implicated in schizophrenia, some of which can be normalized by acute and/or chronic antipsychotic drug treatment. We thus conclude that epidemiology-driven neurodevelopmental models of schizophrenia are characterized by a high level of face, construct and predictive validity, including intrinsic etiological significance to the disorder. They also fulfill the expectation of the neurodevelopmental theory, such that the effects of prenatal environmental insults often only emerge after puberty. Epidemiologically based animal models not only provide indispensable experimental tools to test the hypothesis of causality in human epidemiological associations, but they also offer important new avenues for the elucidation of neurobiological, neuroendocrine and neuroimmunological mechanisms involved in the etiopathogenesis of schizophrenia and related disorders.

Abstract:The brain integrates multiple scales of description, from the level of cells and molecules to large-scale networks and behaviour, and understanding the relationships between these layers may be fundamental to advancing our understanding of how the brain works in health and disease. Recent neuroimaging research has shown that alterations in brain function that are associated with schizophrenia spectrum disorders (SSD) are already present in young adults at clinical high-risk for psychosis (CHR-P), yet the cellular and molecular determinants of these alterations are not well understood. Here, combining regional cerebral blood flow (rCBF) data with existing transcriptomic and neurotransmitter data, we show that cell-types involved in stress response and inflammation, as well as the dopamine, acetylcholine, GABAA and NMDA receptor systems, align as shared and distinct cellular and neurochemical signatures of rCBF phenotypes in people with SSD and those at CHR-P. Decoding the biological pathways involved in neuroimaging-based psychosis phenotypes may provide a basis for the development of novel interventions.

Abstract:This overview describes the generation and development of the ideas that led to the Cognitive Neuroscience Treatment Research to Improve Cognition in Schizophrenia (CNTRICS) initiative. It also describes the organization, process, and products of the first meeting. The CNTRICS initiative involves a series of three conferences that will systematically address barriers to translating paradigms developed in the basic animal and human cognitive neuroscience fields for use in translational research aimed at developing novel treatments for cognitive impairments in schizophrenia. The articles in this special section report on the results of the first conference, which used a criterion-based consensus-building process to develop a set of cognitive constructs to be targeted for translation efforts.

Abstract:This symposium, organized by the ECNP Network for Immuno-NeuroPsychiatry, aims to show where immunoneuropsychiatry stands on the road to clinical application and to identify remaining obstacles. Immune processes contribute to the functioning of the central nervous system, but the risk of developing mental illness is greater in patients with dysfunctions of the immune mechanisms. Some brain diseases, such as autoimmune encephalitis, may even be causally induced by immune dysfunction, for example, when autoantibodies bind to brain receptors and disturb their function. The detection of such processes in psychiatry is highly significant for clinicians, as patients with autoimmune-mediated diseases can potentially be treated by immunomodulatory drugs (Pollak et al., 2020, Lancet Psychiatry). Prof. Marion Leboyer will act as discussant to moderate and integrate the three presentations: In the first presentation, Livia de Picker (from Duffel/Belgium) provides insights into the patterns of microglial activation across different brain disorders. In particular, she presents the results from a transdiagnostic meta- analysis of 156 human CNS TSPO PET case-control studies (De Picker et al., 2023, in review) and the findings from a human TSPO PET study and post-mortem study of patients with psychosis (De Picker et al., 2019 and 2021, Brain, Behavior, and Immunity). Next, Dominique Endres (from Freiburg/Germany) will present exemplary cases and the first case series and clinical experiences with immunotherapies from patients with suspected autoimmune encephalitis and isolated psychiatric manifestations (Endres et al., 2022, Molecular Psychiatry). He will also show that the spectrum of autoantibodies against brain structures may be broader in psychotic patients than initially believed (Endres et al., 2022, Biological Psychiatry). In the last presentation, Michael E. Benros (from Copenhagen, Denmark) will provide an overview of biomarkers for autoimmune psychiatric syndromes, with particular reference to recent results from cerebrospinal fluid (CSF) studies. His research group is currently conducting the largest controlled CSF studies in patients with psychosis and depression (e.g., Mousten…Benros, 2022, JAMA Psychiatry; Sørensen…Benros, 2022, Biological Psychiatry; Jeppesen…Benros, 2022, Schizophrenia Bulletin). He will also present meta-analytical results of studies with anti-inflammatory drugs in patients with severe mental disorders (Jeppesen…Benros, 2020, Brain Behavior and Immunity; Kohler-Forsberg…Benros, 2019, Acta Psychiatr Scand).

Abstract:Objective and quantifiable markers are crucial for developing novel therapeutics for mental disorders by 1) stratifying clinically similar patients with different underlying neurobiological deficits and 2) objectively tracking disease trajectory and treatment response. Schizophrenia is often confounded with other psychiatric disorders, especially bipolar disorder, if based on cross-sectional symptoms. Awake and sleep EEG have shown promise in identifying neurophysiological differences as biomarkers for schizophrenia. However, most previous studies, while useful, were conducted in European and American populations, had small sample sizes, and utilized varying analytic methods, limiting comprehensive analyses or generalizability to diverse human populations. Furthermore, the extent to which wake and sleep neurophysiology metrics correlate with each other and with symptom severity or cognitive impairment remains unresolved. Moreover, how these neurophysiological markers compare across psychiatric conditions is not well characterized. The utility of biomarkers in clinical trials and practice would be significantly advanced by well-powered transdiagnostic studies. The Global Research Initiative on the Neurophysiology of Schizophrenia (GRINS) project aims to address these questions through a large, multi-center cohort study involving East Asian populations. To promote transparency and reproducibility, we describe the protocol for the GRINS project.

Abstract:Numerous studies have demonstrated that inflammation has effects on neurotransmitter systems and neurocircuits in the brain and thereby can affect behavior. Moreover, studies have reliably demonstrated reproducible relationships between inflammatory markers and psychiatric disorders. The goal of this project was to review the literature to determine which specific symptoms across disorders are most reliably associated with inflammation and its effects on the brain. The clinical subgroup of the NSAS Challenge Workshop on Brain Immune Interactions focused on reviewing a range of symptoms including anhedonia, fatigue, appetite, sleep, anxiety, pain, cognition, suicidality, and irritability. PubMed searched for these symptoms and for immune dysregulation: (“Inflammatory” OR “Inflammation” OR “Inflammat*” OR “Cytokine*” OR “Interleukin” OR “C-reactive Protein” OR “CRP” OR “IL-6” OR “Interleukin 6” OR “immune” OR “Immun*” OR “nucleophiles” OR “lymphocytes” OR “immunogenetics” OR “white blood cell” OR “WBC” OR “neutrophil*”). There has been a recent surge of reports with past-five-years accounting for over a third of publications on these topics. Multiple meta-analyses have also been published related to these searches for psychiatric disorders including 3 on anhedonia, 5 on fatigue, 2 on appetite, 9 on sleep, 29 on anxiety, 5 on pain, 17 on cognition, 20 on suicidality, an 0 on irritability. Detailed results of the review of these findings from the workshop (01-05JUN2023) will be presented. Identification of transdiagnostic symptoms reliably associated with inflammation will help further elucidate inflammatory phenotypes that can be used as outcome variables in future studies, especially clinical trials using anti-inflammatory treatments of psychiatric disorders.
