Abstract:The laparoendoscopic single-site (LESS) technique is the latest technical innovation in laparoscopic surgery to undergo exponential development in urology. This study undertaken to illustrate our initial experience LESS radical prostatectomy (RP) and analyze early outcomes. Nineteen patients diagnosed with prostate cancer underwent LESS-RP in our institute. The patients were divided into two groups: conventional LESS and transurethral assistant LESS. Preoperative, perioperative, postoperative, pathologic, and functional outcomes data were assessed. With the help of a transurethral assistant, the mean operation and anastomosis time were decreased markedly. No focal positive margins were encountered. No prostate-specific antigen recurrence was detected 1 month postoperatively. Complete continence recovery (no pad) was observed in 32% of the patients at 1 month after the operation. No intraoperative and postoperative complications were reported. LESS-RP is a feasible and effective surgical procedure for treatment of prostate cancer. Moreover, transurethral assistant LESS could reduce the difficulty of LESS-RP and shorten the operation time.

Abstract:Objective To present our experience on the first 100 cases receiving urological laparoendoscopic single-site surgery(LESS).Methods Between Dec.2008 and May 2011,a cohort of 98patients received a total of 100TriPort TM LESS urologic surgeries for various indications,including 2patients undergoing bilateral surgeries.The proceducres included radical nephrectomy(n=24),partial nephrectomy(n=3),simple nephrectomy(n=10),kidney cyst excision(n=21),adrenlectomy(n=12),transvesical simple prostatectomy(n=9),ureterolithotomy(n=6),and others(n=15).The demographics,perioperative outcomes,and follow-up data were prospectively analyzed.Results Of the 100surgeries,87were smoothly completed.A 5mm ancillary trocar was required in 9cases.Conversion to standard laparoscopy and open surgery were needed in 1and 3patients,respectively.The overall complication rate was 15%,with 6%intraoperatively and 9% postoperatively.The complication rate was 5%for the first 50cases and 10%for the second 50cases.The patients were followed up for 9.9± 5.61(ranging 3-24)months.At the latest follow-up,all patients remained clinically healthy with a delightedly hidden scar.No evidence of recurrence or metastasis was detected in patients receiving tumor resection.Conclusion The LESS is safe and technically feasible for urological surgery,with minimal invasiveness and better cosmesis.Emphasis should be made on patient selection and indication criteria.Improvement in instrumentation,accumulation of clinical practice,and well-designed multicenter prospective clinical trials will promote the use of LESS surgery for treatment of urologic diseases.

Abstract:Background Laparoendoscopic single-site surgery (LESS) approaches have been reported for treating various kidney and pelvic procedures, and are feasible and effective in selected patients. In this study, we aimed to present the initial experience and evaluate the efficacy of laparoscopic radical prostatectomy performed through a single incision using a multichannel port.Methods Between July 2010 and April 2011, six patients diagnosed with early stage prostate cancer underwent LESS radical prostatectomy (RP) in our institute. A multichannel port was inserted transperitoneally through a 2-cm umbilical incision. Specially articulating and flexible laparoscopic were used. Some technical tricks and points were applied during the operation to overcome the drawbacks and reduce the difficulties of this approach. Two continuous urethrovesical sutures in both sides were performed to complete both lateral aspects of anastomosis. The two ends of the suture threads were fixed by double Lapro-Clips, instead of the difficult knot-tying.Results Total operative time was (265 +/- 43) minutes. Mean blood loss was (230 +/- 65) ml. All cases were completed successfully, without conversion to open surgery or adding additional abdomen ports. No patient required a blood transfusion and no intraoperative complications occurred. The Foley catheter was removed at the 14th day (range 12th-16th) after surgery. At the 12th week of follow-up, all patients had an undetectable prostate-specific antigen level. Two patients used 2 or 1 pad for continence daily; other patients had achieved good continence.Conclusion In selected cases, LESS-RP is feasible and effective; these technic points and the flexible-articulating instruments are helpful to reduce the operation difficulties. Laparoendoscopic radical prostatectomy (LRP): stepwise transition from multi-site to single-site with the aid of the transurethral port

Abstract:Purpose: To describe our initial experience with laparoendoscopic radical prostatectomy (LRP) and a stepwise transition towards transurethral assisted laparoendoscopic single-site RP (TU-LESS RP). Patients and methods: From Jan. 2007 to Dec. 2016, 195 patients underwent RP, of which 89 patients were performed by LRP (Group A), 106 by TU-LESS RP (Group B). The peri-operative data were collected and analyzed. All data referring to patient demographics, surgery, pathology, and peri-operative outcomes were recorded. The cosmetic result was investigated by the Patient Scar Assessment Questionnaire (PSAQ). Analysis of variance or Chi squared test were adopted to analyze the data. Results: 195 procedures were completed successfully. The operation time (109.6 ± 31.9 vs. 151.5 ± 87.3, P = 0.025) and anastomosis time (10.1 ± 4.8 vs. 21.8 ± 9.9, P < 0.001) of Group B was significantly reduced compared with Group A. Estimated blood loss in Group B was significantly lower than that in Group A (95.9 ± 11.1 vs. 180.2 ± 99.7, P = 0.006). About perioperative complications, Group B was also less compared with Group A (1.9% vs. 7.9%, P = 0.047). As to the usage of postoperative analgesics, Group B apparently used less than that in Group A (6.6% vs. 62.9%, P < 0.001), which is consistent with the visual analogue scale (VAS) of the two groups (1.7 ± 1.3 vs. 7.8 ± 1.1, P < 0.001). Patients in Group B were significantly more satisfied with incision healing than in group A (74.9 ± 9.3 vs. 49.7 ± 5.8, P < 0.001). There was no significant difference both in BCR rate and time between Group B and Group A. In urination control, more patients in Group B did not have urinary incontinence 3 month after RP compared with Group A (81.1% vs. 67.4%, P = 0.028). Conclusions: LESS RP is proved to be feasible for the proper patients, but it is difficult to popularized due to inconvenient operation. While by means of TU-LESS, operating difficulty can be significantly decreased. TU-LESS RP will be wildly accepted by surgeons and patients because of cosmetic satisfaction and quicker recovery.
Transumbilical Laparoscopic Single-site Surgery for Upper Urologic Diseases: Single-center Experience

Abstract:Objective To evaluate the feasibility of a novel homemade single-port device for transumbilical laparoendoscopic single-site surgery(LESS) on the upper urinary system.Methods The clinical data of 30 cases of LESS that were performed(24 cases on the left side,and 6 cases on the right side) from January 2010 to May 2011 in our hospital were analyzed retrospectively.The cases included 14 cases of radical nephrectomy,12 cases of renal cyst resection,3 cases of non-functional kidney resection and 1 case of adrenal cystectomy.With side-lying position at the healthy side(45°-60°),the patients received LESS by a homemade single-port device through a 4-to 6-cm umbilical incision.Laparoscope(30°),ultrasonic scalpel,and flexible or non-flexible instruments were employed for the operation.Results The procedure was completed in all the 30 cases within 28 to 185 min,without conversion to open surgery.Intraoperative blood loss ranged from 10 to 200 ml,and the hospital stay ranged from 4 to 8 days.Postoperative pathological examination confirmed preoperative diagnosis in all the patients.No hematoma,infection,intestinal obstruction,or incisional hernia occurred in any of the patients.A follow-up for a mean of 21.4 months was achieved in all the patients(ranged from 12 to 29 months),no recurrence was detected during the period.Conclusions Because of its cosmetic outcomes,LESS especially suits female patients.We recommend our homemade single-port device to be widely used for its convenience,flexibility,and low cost.

Abstract:OBJECTIVE:To evaluate the efficacy and safety of transumbilical single-port laparoscopic radical nephrectomy.METHODS:The clinical data of laparoscopic radical nephrectomy performed from June 2010 to October 2010 in Peking University Third Hospital were analyzed retrospectively. In the study, 10 patients underwent transumbilical single-port laparoscopic radical nephrectomy (LESS) and 15 received retroperitoneal laparoscopic radical nephrectomy. The data on general presentation, tumor size, tumor location, operative time, blood loss, complications, Visual Analog Pain Scale (VAPS), postoperative hospital stay, pathological results were collected to compare between the two groups. Our homemade single-site equipment was used in the LESS approach and the kidney was dissociated after being cut off the renal vessel and extracted through the umbilical incision. The retroperitoneal approach followed the standard surgical procedures, and the specimens were removed from the extended incision.RESULTS:All the procedures were completed without conversion to open radical nephrectomy. Compared with traditional laparoscopic surgery, the operative time (P=0.001) and VAPS (P=0.002) showed significant difference in LESS group,and no difference was noted in other factors (P>0.05). There was no secondary bleeding, wound infection, intestinal obstruction, incision hernia and other severe postoperative complications. The follow-up of 6 to 10 months showed no local recurrence.CONCLUSION:Transumbilical single-port laparoscopic radical nephrectomy is feasible, effective and safe. It gives a more mini-invasive and cosmetic option for young or female patients.

Abstract:Introduction: Laparoendoscopic single-site (LESS) technique is a less invasive approach for radical cystectomy (RC), which is promising in reducing the incisional morbidity and improving the cosmesis of laparoscopic surgery. This study aimed to investigate the clinical and oncological outcomes for patients with malignant urinary bladder tumors that underwent the transurethral-assisted transumbilical LESS-RC. Methods: From December 2014 to June 2017, 47 patients underwent LESS-RC combined with unilateral or bilateral cutaneous ureterostomy were enrolled in this study. The urethra was used as a potential approach without additional incision, which could allow for trocar insertion through natural orifices. Assessments were also conducted on preoperative, perioperative, postoperative, pathologic, and functional outcome data. Results: Mean patient age was 73 years. Mean body mass index was 24.0 kg/m(2). Median operating time and estimated blood loss measure 217 min and 178 mL, respectively. Four patients were diagnosed with positive lymph nodes. Two patients had positive surgical margins. No major perioperative complications occurred. Median postoperative follow-up time was 20.1 months. Two patients died due to their progressive disease. Conclusion: LESS can serve as a feasible and effective surgical procedure for RC to treat bladder cancer. With increasing experience and improvements, LESS-RC is promising to be a relatively acceptable alternative for minimally invasive surgery in some specific patients (with generally poor conditions that cannot be tolerated for a long time surgery, short life expectancy, advanced cancer, or associated with intestinal disease).
Synchronous Bilateral Laparoendoscopic Single-Site Urological Surgeries Via the Umbilicus: a Report of 2 Cases

Abstract:Objective:To describe two cases of bilateral transumbilical laparoendoscopic single-site（LESS） surgeries in a single operative session and assess the safety,feasibility and efficacy.Methods:One patient underwent right ureterolithotomy and left varicocelectomy,and the patient was performed with right simple nephrectomy for nonfunctioning kidney due to ureteral calculus and left ureterolithotomy using a novel multichannel TriPort? via a single 2-3 cm transumbilical incision. Results:The right-side ureterolithotomy and left-side varicocelectomy were finished in 229 minutes,with a total estimated blood loss of 50 ml.The right-side simple nephrectomy and left-side ureterolithotomy in the other patient were finished in 340 minutes,with a total estimated blood loss of 100 ml.There were no major complications.At the latest follow-up,both patients remained symptom-free and there were no evidences of recurrence.Both of them were delighted for the single scar. Conclusion:Synchronous LESS urologic procedure via a single umbilical incision is technically feasible,safe and efficacious with a promising potential.

Abstract:Objectives: To evaluate the feasibility and safety of bipolar-plasmakinetic transurethral enucleation and resection of the prostate (B-TUERP) in day surgery. Methods: From January 2021 to August 2022, 34 patients with benign prostatic hyperplasia (BPH) underwent B-TUERP in day surgery in the First Affiliated Hospital of Anhui Medical University. Patients completed the screening and anesthesia evaluation before admission and received the standard surgery which implements "anatomical enucleation of the prostate" and "absolute bleeding control" on the same day of admission, and by the same doctor. Bladder irrigation was stopped, catheter was removed and the discharge evaluation was performed on the first day after operation. The baseline data, perioperative conditions, time of recovery, treatment outcomes, hospitalization costs, and postoperative complications were analyzed. Results: All operations were successfully conducted. The average age of the patients was (62.2±7.8) years, average prostate volume was (50.2±29.3) mL. The average operation time was (36.5±19.1) min, the average hemoglobin and blood sodium were decreased by (16.2±7.1) g/L and (2.2±2.0) mmol/L, respectively. The average postoperative length of hospital stay, and total length of hospital stay were (17.7±2.2) and (20.8±2.1) h, respectively, and the average hospitalization cost was (13 558±2320) CNY. All patients were discharged on the day after surgery except for one patient who was transferred to a general ward. Three patients received indwelling catheterization after catheter removal. The 3-month follow-up results showed a substantial improvement in the International Prostate Symptom Score, quality of life score and maximum urinary flow rate (all P<0.01). Three patients experienced temporary urinary incontinence, 1 patient experienced urinary tract infection, 4 patients were diagnosed with urethral stricture and 2 patients experienced bladder neck contracture. No complications above Clavien grade Ⅱ occurred. Conclusions: The preliminary results showed that B-TUERP ambulatory surgery is a safe, feasible, economical and effective treatment for appropriately selected patients with BPH.

Abstract:Objective To summarize the clincical experience of transumbilical Laparoendoscopic Single-site (LESS) nephrectomy and to evaluate its safety and efficacy. Methods From December 2008 to August 2010, we have performed 20 cases of transumbilical LESS nephrectomy by Tri-Port system, of which 9 patients underwent LESS radical nephrectomy (left 8, right 1, stage T1 ), 1 patient underwent LESS radical resection of right ureteral carcinoma, 10 patients underwent LESS simple nephrectomy (left 5, right 5). The Tri-Port system was inserted transperitoneally through a 2 cm umbilical incision. A 5-mm 30° telescope was introduced through the port to visualize the operative field. Flexible equipment and standard laparoscopic equipment were used to perform the procedures.The incisions were extended to about 6cm in order to remove the specimens. Results Conversion to open surgery was necessary in one LESS radical resection of right ureteral carcinoma and one LESS simple nephrectomy, while the remaining 18 cases were successful (the addition of a single 5-mm port was necessary in 2 cases of LESS radical nephrectomy). The mean operative time was 197 min (85-510 min), mean estimated blood loss was 126 ml (50-400 ml), without blood transfusion in the perioperative period, mean postoperative hospital stay was 6.3 d (3-14 d), and mean duration of catheter drainage was 3.6 d (0- 14 d). Conclusions Transumbilical LESS nephrectomy is feasible, safe,minimally invasive and cosmetic. Long-term follow-up and a clinical control study are needed for evaluating clinical outcomes.
[Application of Modified Single Port Laparoscopic Radical Cystoprostatectomy and Orthotopic Ileal Neobladder].

Abstract:OBJECTIVE:To evaluated the feasibility and therapeutic effect of laparoscopic-endoscopic single-site surgery (LESS) for radical cystoprostatectomy and orthotopic ileal neobladder.METHODS:Between November 2008 and August 2009, 8 male patients with bladder cancer underwent modified single port laparoscopic cystoprostatectomy and orthotopic ileal neobladder with a home-made multichannel port made up of two stretchable rings and a surgical glove with trocars and valves attached to its fingers. A 4 - 5 cm midline incision was made in lower abdomen and the multichannel port placed into this incision. Laparoscopic instrument was extended through the multichannel port. Another port was placed at the umbilicus site for laparoscope. The investigators performed the modified single port transperitoneal technique neobladder extracorporeally and urethro-pouch anastomosis under laparoscopy with a slipknot running suture technique. The perioperative and postoperative data were collected and analyzed prospectively.RESULTS:In all cases, no conversion into conventional laparoscopic or open surgery was necessary. The mean operative duration, including extended lymph node dissection, was 399 min (range: 355 - 455 min). Mean estimated blood loss was 154 ml (range: 90 -210 ml). The mean postoperative hospital stay was 15 d (range: 14 - 18 d). No peri-operative mortality was reported. One case had delirium 2 days postoperatively and there was no port-related complication. The pathological report confirmed surgical margins were tumor-free for all cases. The average number of removed lymph node was 20. The mean follow-up period was 6.1 months (range: 2.0 -10.0 months). All patients remained disease-free until the last follow-up.CONCLUSIONS:With a reduction of 4 ports, modified single port laparoscopic radical cystectomy with orthotopic ileal neobladder is technically feasible with a low complication rate. Our initial outcome is encouraging, but additional work and a further follow-up are warranted to critically compare the oncological outcome with open surgery.
Transvesical approach laparoendoscopic single-site radical prostatectomy: for organ-confined prostate cancer: report of 8 cases

Abstract:Objective　To investigate the feasibility of applying transvescal approach laparoendoscopic single-site radical prostatectomy (TVSSLRP) and assess the oncological and functional outcomes.Methods　Eight patients with clinically localized prostate cancer (PCa) of low risk underwent TVSSLRP.Demographic data were accrued including patient age,body mass index (BMI),preoperative PSA level,the International Index of Erectile Function 5,biopsy Gleason score,clinical TNM stage and D'Amico risk classification.One surgeon performed all TVSSLRP procedures.A homemade triple-port was introduced percutaneouly into the bladder to establish pneumovesicum through a 4 cm incision.The major steps of the surgery were described as follows:initial incision was made along posterior margin of the bladder neck to expose bilateral vas deference and spermatic vesicle.After opening Denonvilliers' fascia and extending the space to lateral prostatic pedicles,an intra-fascial nerve sparing procedure was performed.The puboprastatic ligaments were then separated close to the prostate surface and the dorsal vein complex was cautiously swept off.Subsequently,careful apical dissection and urethral transection was sequentially conducted. To reduce the tension of vesico-urethral anastomosis,3 additional incisions parallel to vesio-urethral margin were created and a novel tension - reduced V-LocTM barbed polydioxanone sutures was used.　Results　All the operations were successfully performed and there was no conversion to standard laparoscopic approach or open surgery.The total operative time range was 75 - 180 min with mean time of 125 min.The blood loss was 85 -450 ml with mean 140 ml and no blood transfusion was required.The catheter was removed after a mean (range) of 14 (9 -16) days.No intra-operative complications occurred. No patient had positive surgical margins.The mean (range) hospital stay was 17 (13 -25) days after surgery. All the cases were continent after removal of the catheter.No cases demonstrated vesico-urethral stricture and biochemical recurrence on 12 - 18 months follow up postoperatively.　Conclusions　TVSSLRP is technically feasible for cases with organ-confined prostate cancer with good oncological and functional results.

Abstract:Purpose: The aim is to report our 1-year experience in transumbilical Laparoendoscopic single-site (LESS) radical nephrectomy (LESS-RN) and assess its learning curve. Patients and Methods: From September 2009 to December 2010, 18 consecutive patients with a solitary renal tumor underwent transumbilical LESS-RN. We grouped measured parameters on the basis of the first nine (group A) and last nine (group B) patients, and compared both subgroups with the control group (C), which was composed of the initial nine consecutive patients who underwent conventional laparoscopic RNs performed by the same surgeon. Results: There was one LESS patient converted to conventional laparoscopic surgery for intraoperative hemorrhage and transfused. The mean for measured parameters of the groups A, B, C were: age: 58.6, 57.2, and 59.2 years, respectively (P > 0.05); American Society of Aneshesiologists score: 1.9, 2.0, and 2.0, respectively (P > 0.05); body mass index: 25.2, 23.5, and 23.1 kg/m(2), respectively (P > 0.05); lesion size at CT scan: 5.0, 4.6, and 6.2 cm, respectively (P = 0.039); operative skin to skin time: 191.6, 123.9, and 122.2 minutes, respectively (P = 0.000); estimated blood loss: 172.2, 246.7, and 116.7 mL, respectively (P > 0.05); incision length: 5.6, 5.9, and 7.4 cm, respectively (P = 0.000); hospital stay: 5.8, 5.8, and 5.8 days, respectively (P > 0.05). At the latest follow-up, all patients did not present any local recurrence or metastasis. Conclusions: In selected patients, the transumbilical LESS-RN can be safely performed with favorable outcomes. While a learning curve certainly exists, it appears to be short for an experienced laparoscopic surgeon.

Abstract:Objective To sum up the experience and tips of transumbilical single-site laparoscopic chole-cystectomy (TUSSLC) with conventional devices and make further investigations on its safety and feasibility. Methods Retrospective analysis was made on 491 patients undergone TUSSLC with conventional devices from Jun. 2010 to Jun. 2015. All cases were enrolled according to inclusion and exclusion criteria. There were 381 cases of cholecystolithiasis, 95 of polyps and 15 of adenomyosis. Perioperative and intraoperative treatment abided by the rationale of standard laparoscopic cholecystectomy (LC). An innovative method of transumbilical approach we-called as “single-site with two Trocars and two incisions” was utilized. Operating time, operative blood loss, intraoperative conversion, postoperative hospitalization length and complications were observed. Results of the 491 cases, 3 cases were converted to three-port LC by a 0.63% conversion rate. of the 3 cases, operative time was 50~95 min (mean 68.3 min) and operative blood loss was 5~50 mL (mean 21.7 mL). of the other 488 cases, operative time was 30~90 min (mean 45.2 min) and operative blood loss was 1~70 mL (mean 16.5 mL). All cases were uneventful postoperatively with 1~5 d (mean 2.3 d) postoperative hospitalization length. Conclusion Our innovative method of TUSSLC with conventional devices is safe and feasible technically. Com-pared with standard LC this approach has the superiority of less postoperative pain, better cosmetic result and no more cost of surgical instruments. Thus it deserves a wide clinical application.

Abstract:Objective: To evaluate the feasibility and clinical value of suprapubic-assisted laparoendoscopic single-site surgery (SA-LESS) in nephroureterectomy using method of transvaginal natural orifice specimen extraction (NOSE) (SA-LESS+TV-NOSE NU). Methods: Four patients (three cases of renal pelvic carcinoma and one case of ureteral carcinoma) undergoing SA-LESS+TV-NOSE NU were enrolled between April 2015 and January 2016. After general anesthesia, the patients were placed in the lithotomy position with the affected side elevated by 60°. Two trocars were inserted at the medial margin of umbilicus, and the third one was inserted into abdominal cavity at the superior margin of pubic symphysis. The operation was performed under a direct vision with a 5.4 mm 0° flexible-tip laparoscope. Firstly, the distal ureter was isolated completely and blocked by a Hem-O-lok clip. Then, the laparoscopic nephrectomy was performed according to the standard method. Finally, the bladder cuff excision was executed and the incision was sutured. The intact specimen was placed inside a homemade bag and removed through the incision at posterior vaginal fornix. Results: All the procedures were successfully performed. The median operative time was 150 (range: 120 to 210) minutes, and the median estimated blood loss was 180 (range: 80 to 350) ml. No major perioperative complications occurred. The mean visual analogue score (VAS) of 24 hours and 48 hours after operation were 3.25 (range: 2 to 5) and 2.25 (range: 2 to 3). All the patients resumed ambulation on postoperative day 1. Pelvic drainage tube was removed on postoperative day 2-4. On postoperative day 7, urethral catheter was removed. The patients were discharged on postoperative day 7-9. During the follow-up of 20-29 months, the patient recovered well with no case of incisional hernia and pelvic, abdominal infections. The vaginal fornix incision healed well, and the umbilical and suprapubic puncture scars were not obvious. All the patients completed the patient-assessed acromegaly symptom questionnaire PASQ. The average PSAQ score of 3 months after surgery was 34.5. Three of them restarted their sex lives, with an average female sexual function index score of 16.0, which was not significantly different with that of preoperation (15.6). There was no tumor recurrence, metastasis and implantation in all cases. Conclusion: SA-LESS+TV-NOSE NU is safe and feasible for upper tract urothelial carcinoma with faster postoperative recovery, less pain, shorter hospitalization time, better cosmetic results, and does not cause negative effect on the female sexual function.

Abstract:Objective:To present clinical experience of retroperitoneal laparoendoscopic single-site(LESS)upper urinary tract radical surgery using self-made single-port device.Methods:Between July 2010 and July 2011,a total of 12 patients underwent retroperitoneal LESS upper urinary tract radical surgery by one experienced surgeon,including 5 radical nephrectomy and 7nephroureterectomy.All procedures were carried with retroperitoneum through an incision 3cm above the iliac crest.After extending the retroperitoneum with a ballonet,a self-made single-port device was placed and surgery procedures were performed with flexible and standard laparoscopic instruments.All specimens were extracted through the extended incisions or a separated incision.Results:All procedures were completed successfully,with no conversion to standard multiport laparoscopic or open surgery.The mean laparoscopic operation time was 94(60-191)mins,and the mean estimated blood loss(EBL)was 83(0-400)ml.Postoperative hospital stay was 7(5-10)days.One patient(8.3%)was noted to have urinary tract infection post-operatively.Conclusions: For properly selected patients,the retroperitoneal LESS radical nephrectomy and nephroureterectomy is safe and feasible,but observation for long-term outcome as well as improvements of the instruments and surgical technique are needed.

Abstract:Objective To investigate the feasibility and safety of transumbilical laparoendoscopic single-site surgery(LESS) for acute abdomen.Methods We reviewed 100 cases of acute abdomen who were admitted to our hospital between January and September 2011.With a silicon incisional protector,LESS for exploration and further procedures were carried out via a 20-mm transumbilical vertical incision.All the procedures were completed with a 5-mm laparoscope through the transumbilical port.ResultsOf the 100 patients,85 cases were acute appendicitis,of which,76 cases underwent transumbilical single-site laparoscopic-assisted extracorporeal appendectomy,4 cases underwent single-site intraabdominal appendectomy,and the other 5 needed a 5-mm suprapubic port to remove the appendix.In the other 15 patients,gastric perforation was detected in 8,6 of them received transumbilical single-site laparoendoscopic repair,and the other 2 needed a 5-mm suprapubic port to complete the procedure;hemorrhagic enteritis was found in one patient,who received transumbilical resection of the effected ileum,and then jejunostomy;pelvic infection was detected in two female patients,who then underwent abdominal and pelvic irrigation;torsion of ovarian cyst and lower abdominal intestinal adhesion were found in 2 patients,respectively,who then received resection of the ovarian cyst or lysis by transumbilical LESS;intraabdominal chyle effusion was explored intraoperatively in the other 2 patients,one of them was confirmed with spontaneous chyle peritonitis,and was cured by closing the leaking lymphatic with titanium clips and intraabdominal irrigation,and the other one was diagnosed with lymphangioma of the jejunum mesentery,and thus was converted to an open surgery for resection of the tumor.After the operations,3 patients developed umbilical secretion,and two showed enteroparalysis.All the patients were followed up for 6 to 9 months,during which the umibical incision was healed well in all the cases without any incisional hernia occurred.ConclusionTransumbilical LESS with a silicon incisional protector is feasible and safe for acute abdomen with good cosmetic results.

Abstract:BACKGROUND AND PURPOSE:Laparoscopic endoscopic single-site surgery (LESS) has recently emerged as an attempt to enhance cosmetic benefits and reduce morbidity; however, LESS for radical cystectomy is still not well established. Here we describe the technique of hybrid LESS for radical cystoprostatectomy and orthotopic ileal neobladder (RC-OIN), and evaluate its feasibility and safety.PATIENTS AND METHODS:Between November 2008 and October 2009, 12 men with bladder cancer underwent hybrid LESS for RC-OIN. A homemade multichannel port, made from two stretchable rings and a surgical glove with trocars and valves attached to its fingers, was placed into a 4- to 5-cm midline incision in the lower abdomen and was used for laparoscopic instruments. Another subumbilical port was placed for the laparoscope. Extended bilateral pelvic lymphadenectomy was performed by the lateral view; radical cystoprostatectomy was completed laparoscopically; construction of the ileal neobladder was performed extracorporeally; and the neobladder was anastomosed to the urethral stump laparoscopically, with a slipknot running suture technique. Perioperative, functional, oncologic data and complications were collected and analyzed.RESULTS:All operations were performed successfully without conversion to conventional laparoscopic radical cystectomy or open surgery. There was no perioperative mortality or port-related complications. The median operative time was 383 minutes. Median blood loss was 150 mL. A median of 25 lymph nodes were removed. Surgical margins were tumor free in all cases.CONCLUSIONS:Hybrid LESS for RC-OIN is technically feasible with effects similar to those of conventional laparoscopic procedures. Further instrument and technique improvement are necessary to shorten operative time and reduce intraoperative difficulties.
Modified Transperitoneal Laparoscopic Radical Prostatectomy: Technique and Clinical Outcomes

Abstract:OBJECTIVE:To evaluate the technique and clinical outcomes of modified transperitoneal laparoscopic radical prostatectomy.METHODS:A total of 105 patients received the operation with age ranging from 51 to 73 years from January 2008 to June 2010. Mean level of serum prostate specific antigen was 13.6 µg/L and mean prostatic volume was 45 ml. Pathological studies of biopsy confirmed the prostate carcinoma with Gleason score 6-8. Radionuclide bone scan revealed no metastasis. Based on previously retroperitoneal radical prostatectomy, modified technique was applied involving surgical approach, bladder neck dissection and vesicourethral anastomosis.RESULTS:Mean operative time was 93 min (65 - 150 min). Intraoperative blood loss was 115 ml (50 - 400 ml). No complication of bowl injury occurred. Positive surgical margin was present in 24 patients. Normal continence were seen in 64 patients after catheter removed. Recovery of incontinence within 3 months was seen in 33 patients and 3 to 12 months in 5 patients respectively. Three patients with incontinence were still in the follow-up.CONCLUSIONS:Transperitoneal laparoscopic radical prostatectomy provides large working space and clear anatomic exposure. Higher efficiency and lower complication rate are obtained through modified laparoscopic technique involving seminal vesicle isolation, bladder neck dissection and vesicourethral anastomosis.

Abstract:OBJECTIVE:To summarize the technical modification and experiences of transumbilical laparoendoscopic single-site nephrectomy (LESS-N) by homemade device.METHODS:The clinical data of LESS nephrectomy performed from June 2010 to April 2013 in Peking University Third Hospital were analyzed retrospectively. All the cases were divided into two groups according to the technique method and operative date. Group 1 included 10 cases that underwent LESS radical nephrectomy and 2 that received LESS simple nephrectomy from June 2010 to April 2011. Group 2 included 7 cases that underwent LESS radical nephrectomy and 3 that received LESS simple nephrectomy from May 2011 to April 2013. The data on the general presentation, tumor size, tumor location, operative time, blood loss, complications, Visual Analog Pain Scale (VAPS), postoperative hospital stay, pathological results were collected to compare between the two groups. The modified technique included homemade single-ring glove technique, fast access to the pedicle, pulling-up technique to retract the kidney or liver. The kidney was dissociated after the renal vessel was cut off and extracted through the umbilical incision.RESULTS:All the procedures were finished without conversion to open radical nephrectomy. Compared with group 1, operative time showed significant difference in group 2 [Group 1:(220.6±51.0) min; Group 2: (178.9±34.0) min; P=0.04],and no difference was noted in other factors (P>0.05). There was no secondary bleeding, wound infection, intestinal obstruction, incision hernia and other severe postoperative complications. follow-up of 2 to 36 months showed no local recurrence.CONCLUSION:Transumbilical LESS-N is feasible, effective and safe. It gives a more mini-invasive and cosmetic option for young or female patients. Learning curve and operative time can be reduced by modified techniques, such as single-ring glove technique and pulling-up technique.
