Abstract:Purpose Clinical observations suggest that endplate shape and size are related to complications of disc arthroplasty surgery. Yet, the morphology of the vertebral endplate has not been well defined. This study was conducted to characterize the morphology of lumbar vertebral endplates and to quantify their morphometrics using radiographic, visual and digital measures. Methods A total of 591 vertebral endplates from 76 lumbosacral spines of men were studied (mean age 51.3 years). The shape of the vertebral endplates was classified as concave, flat and irregular, and was evaluated from both radiographs and cadaveric samples. Each endplate was further digitized using a laser scanner to quantify diameters, surface area and concavity for the whole endplate and its components (central endplate and epiphyseal rim). The morphological characteristics and morphometrics of the vertebral endplates were depicted. Results In both radiographic and visual assessments, more cranial endplates (relative to the disc) were concave and more caudal endplates were flat at all disc levels ( p < 0.001). On average, the mean concavity depth was 1.5 mm for the cranial endplate and 0.7 mm for the caudal endplate. From L1/2 down to L5/S1 discs, the vertebral endplate gradually changed into a more oval shape. The central endplate was approximately 70 % of the diameter of the whole endplate and the width of the epiphyseal rim varied from 3 to 7 mm. Conclusions There is marked morphological asymmetry between the two adjacent endplates of a lumbar intervertebral disc: the cranial endplate is more concave than the caudal endplate. The size and shape of the vertebral endplate also vary considerably between the upper and lower lumbar regions.
Quantitative Morphometric Study of the Subaxial Cervical Vertebrae End Plate

Abstract:BACKGROUND CONTEXT: Cervical disc arthroplasty has been gradually adopted as an alternative for the treatment of cervical degenerative disease. However, there is a large discrepancy between footprints of currently available cervical disc prostheses and anatomic dimensions of cervical end plates.PURPOSE: This study aimed to accurately and comprehensively quantify the three-dimensional (3D) anatomic morphology of the cervical vertebral end plate and provide a theoretical basis for designing appropriate disc prostheses. Moreover, we introduced a novel geometric and mechanical model for 3D reconstruction techniques of the cervical end plate.STUDY DESIGN: A descriptive study of the geometry of the middle and lower cervical vertebral end plates in cadaveric spines was carried out.METHODS: A total of 138 cervical vertebral end plates were digitized using an optical 3D range scanning system, and then each end plate was reconstructed using the digitized image. For each end plate, the morphologic characteristics of six surface curves and the end plate concavity depth were symmetrically chosen and depicted.RESULTS: The cranial end plates (relative to the disc) were concave and the caudal end plates were relatively flat at all disc levels, with mean concavity depths of 2.04 and 0.69 mm, respectively. For the caudal end plates, the end plate concavity apex was most often (81.42%) located in the posterior portion, whereas in the cranial end plates, the distribution was relatively even. For the sagittal curves, the foremost point and the rearmost point on the middle curve had a more forward position than those in the left curve and the right curve. Regarding the frontal plane curves, the length of the middle curve was longer than that of the anterior curve and posterior curve. For the cranial end plate, the maximal mean depth was the middle curve, whereas for the caudal end plate, the maximum depth was the posterior curve.CONCLUSIONS: There is marked morphologic asymmetry, in that the cranial end plate is more concave than the corresponding caudal end plate. In the sagittal plane, the caudal end plates are aerofoil-shaped, whereas the cranial end plates are arc-shaped. In the transverse plane, the end plates are kidney-shaped. These morphologic characteristics of cervical vertebral end plates should be taken into consideration when designing cervical devices, such as artificial discs. (C) 2016 Elsevier Inc. Endplate Concavity Variation of Lumbar Motion Segments a Finite Element Analysis

Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.
A Morphometric Study of the Middle and Lower Cervical Vertebral Endplates and Their Components

Abstract:Cervical disc arthroplasty is a common method of treating cervical degenerative disease. However, the footprints of most prosthesis dimensions are obtained from data of Caucasian individuals. Besides, there is a large discrepancy between footprints of currently available cervical disc prostheses and anatomic dimensions of cervical endplates. We aimed to detail the three-dimensional (3D) anatomic morphology of the subaxial cervical vertebral endplate, utilizing high-precision, high-resolution scanning equipment, and provide a theoretical basis for designing appropriate disc prostheses for Chinese patients. A total of 138 cervical vertebral endplates were studied. Each endplate was digitized using a non-contact optical 3D range scanning system and then reconstructed to quantify diameters and surface area for the whole endplate and its components (central endplate and epiphyseal rim). The whole endplate and mid-plane concavity depth were measured. There is marked morphologic asymmetry, in that the cranial endplate is more concave than the corresponding caudal endplate, with endplate concavity depths of 2.04 and 0.69mm, respectively. For the caudal endplates, the endplate concavity apex locations were always located in the posterior portion (81.42%), while in cranial endplates relatively even. The central endplate was approximately 60% of the area of the whole endplate and the anterior section of the ring was the widest. From C3/4 down to C6/7 discs, the vertebral endplate gradually became more elliptical. Chinese cervical endplate anatomic sizes are generally smaller than that of Caucasians. Although Korean and Chinese individuals both belong to the Asian population subgroup, the majority of anatomic dimensions differ. Singaporean cervical endplate morphology is very similar to that of Chinese patients. We performed a comprehensive and accurate quantitative description of the cervical endplate, which provide references to shape and profile an artificial cervical disc without sacrificing valuable bone stock. To design a device with footprint as large as possible to distribute the axial load, we suggest that additional attention should be paid to the marginal rim. It is essential to specifically design appropriate disc prosthesis for Chinese patients. To fit the morphologic and biomechanical variations, we also propose that the disc prostheses for different vertebral segments should be separately designed.
Differences of the Morphology of Subaxial Cervical Spine Endplates Between Chinese and White Men and Women.

Abstract:Objective. The aim of this comparative anatomical study was to specifically investigate endplate morphology differences between Chinese and White men and women. Materials and Methods. Three-dimensional cervical endplate models were constructed using computed tomography imaging of 41 healthy Chinese and 24 White subjects. The morphologic measurements of cervical endplate included linear parameters (EPWu: upper endplate width; EPDu: upper endplate depth; EPWl: lower endplate width; and EPDl: lower endplate depth) and area parameters with a digital measuring system. Results. All linear parameters showed a constant increase from C3 to C7 except for EPDl in both the Chinese and the White subjects. An increase trend was observed on area parameters in both Chinese and White subjects. The ratio of EPWl/EPDl was smaller in Chinese females than in White females at C3, C4, and C6 levels (P < 0.05). The ratio of EPWl/EPDl was significantly different between the Chinese and White men at C4-5 levels (P < 0.05). Conclusions. Our data indicates that the morphology of subaxial cervical spine endplates between Chinese and White men and women is different in most of the linear and area parameters. This information could provide guidelines for the design of CDA implants and the improvement of surgical techniques.
Anchorage of Annulus Fibrosus Within the Vertebral Endplate with Reference to Disc Herniation

Abstract:To describe the microarchitecture of the annulus-endplate region with reference to disc herniation.Twenty-five motion segments (L1-2 to L5-S1) were harvested from five cadaveric spines (four males, one female). Superior and inferior annulus-endplate junctions were divided into anterior, lateral and posterior regions. Tissue blocks were fixed, decalcified, and embedded in paraffin. Thin sections (4 µm) were stained with H&E and Masson's trichrome for light microscopy.Annulus fibrosus presents a laminated structure, with lamellae becoming wider and more disordered close to the nucleus. Each lamella comprises parallel bundles of collagen fibers. On entering the hyaline cartilage endplates, fiber bundles from the outer annulus often change direction, branch into sub-bundles separated by hyaline cartilage matrix, and no longer exhibit the undulating "crimp" structure of the annulus. Their three-dimensional multileaf morphology appears to provide direct mechanical anchorage in vertebral bone. Calcification is present at the annulus-cartilage and cartilage-bone interfaces, especially posteriorly. In the inner annulus and nucleus, annulus fibers branch and merge obliquely with the hyaline cartilage matrix. The diameter of collagen fiber bundles and sub-bundles increased significantly in the outer annulus compared with middle annulus, and increased at lower spinal levels, but no significant differences were observed between superior and inferior endplates.The branching of annulus collagen fiber bundles within the endplates increases the interface area with surrounding matrix, reducing shear stresses and (probably) increasing strength. Calcification may also increase strength. These observations can explain why disc herniations often contain cartilage and bone fragments from the endplate.
A new cervical artificial disc prosthesis based on physiological curvature of end plate: a finite element analysis.

Abstract:STUDY DESIGN: The study aimed to build a new cervical artificial disc C3-C7 segment prosthesis, and perform a biomechanical comparison between the new prosthesis and the Prestige LP prosthesis using a three-dimensional non-linear finite element (FE) model. PURPOSE: The study compared the biomechanical differences between the new cervical artificial disc prosthesis based on the physiological curvature of the end plate and the Prestige LP prosthesis after artificial disc replacement. BACKGROUND CONTEXT: There has been no prior research on artificial disc prostheses based on the physiological curvature of the end plate; studies of biomechanical changes after cervical disc arthroplasty (CDR) are few. METHODS: An FE model of the C3-C7 segments was developed and validated. A new cervical artificial disc prosthesis based on the physiological curvature of the end plate and the Prestige LP prosthesis were integrated at the C5-C6 segment into the validated FE model. All models were subjected to a follower load of 73.6 N and a 1 Nm in flexion-extension, lateral bending, and axial torsion. The segmental range of motion (ROM) and stress on the prostheses were analyzed. RESULTS: The ROM in most segments after CDR with new cervical artificial disc prosthesis was more similar to that of the normal cervical spine than the Prestige LP prosthesis. However, there was no significant difference between the two prostheses. The stress on the new artificial disc was significantly less than that in the Prestige LP prosthesis. CONCLUSIONS: There was no significant difference in ROM in all segments after CDR for the two prostheses. The stress on the new cervical artificial disc prosthesis based on the physiological curvature of the end plate was significantly less than that in the Prestige LP prosthesis. The new artificial disc prosthesis is feasible and effective, and can reduce the implant-bone interface stress on the end plate, which may be one of the causes of prosthesis subsidence. Biomechanical properties of lumbar endplates and their correlation with MRI findings of lumbar degeneration

Abstract:How stiffness and strength of the human lumbar endplate vary with location, spinal level, and its correlation with MRI findings of lumbar degeneration, has not been reported in detail. 27 lumbar spines (16 male, 11 female, 31–49yrs) were harvested from cadavers without history of lumbar lesion or trauma. Disc and endplate degeneration was evaluated from MRI. Micro-CT was used to evaluate endplate microstructure. Indentation tests were performed to quantify stiffness and strength at 23 sites on each endplate from load-displacement graphs. Results showed that stiffness and strength increased from the centre of the endplate towards its periphery. There was no general age-related reduction in endplate stiffness or strength, although strength decreased slightly with age opposite the inner annulus. Disc degeneration was associated with a 39–46% decrease in stiffness, and a 21–30% decrease in strength, with effects being greatest near the endplate periphery. The presence of Modic changes had a similar effect. Strength and stiffness consistently increased at lower spinal levels, and were consistently greater in the inferior endplate (relative to the vertebra). Gender had little influence, although stiffness in the peripheral endplate was greater in males. BV/TV, SMI, Tb.Th and BMD were positively correlated with strength. We conclude that endplate properties reflect compressive stresses within adjacent intervertebral discs. Weaker and softer endplates may indicate reduced mechanical loading in decompressed discs that are stress-shielded by the neural arch. Preoperative MRI evaluation of endplate integrity could reduce the risk of implant subsidence following inter-body fusion.
Biomechanical Analysis of A Novel Prosthesis Based on the Physiological Curvature of Endplate for Cervical Disc Replacement

We first reported a well-designed artificial disc prosthesis based on the physiological curvature of the endplate.MethodsThree motion segments of 24 ovine cervical spines (C2-5) were evaluated in a robotic spine system with axial compressive loads of 50N. Testing conditions were as follows: 1) intact, 2) C3-4 CDR with artificial disc prosthesis based on the physiological curvature of the endplate, and 3) C3-4 CDR with the Prestige LP prosthesis. The range of motion (ROM) and the pressures on the inferior surface of the two prostheses were recorded and analyzed.ResultsAs compared to the intact state, the ROM of all three segments had no significant difference in the replacement group. Additionally, there was no significant difference in ROM between the two prostheses. The mean pressure on the novel prosthesis was significantly less than the Prestige LP prosthesis.ConclusionROM in 3 groups (intact group, CDR group with novel prosthesis and CDR group with Prestige LP) showed no significant difference. The mean pressure on the inferior surface of the novel prosthesis was significantly lower than the Prestige LP prosthesis. Therefore, the novel artificial disc prosthesis is feasible and effective, and can reduce the implant-bone interface pressure on the endplate, which may be one possible reason of prosthesis subsidence.
Three-dimensional finite element analysis on artificial cervical disc replacement for activities of lower cervical spine

Abstract:This study was aimed to establish the three-dimensional model of C_(3-7) segment of lower cervical spine after artificial disc implantation, to analyze the movement of lower cervical spine after artificial disc replacement. According to CT films of 1 patient at 6 months after artificial disc implantation, three-dimensional finite element model that included Bryan~(TM) artificial cervical disc prosthesis of the lower cervical spine was established using finite element method, then introduced into Ansys 9.0, the vertebral cortical bone, cancellous bone and intervertebral disc were meshed and analyzed by using under several states such as flexion/extension, lateral bending and rotation, thus understanding their motion characteristics. By comparison with previous research findings, test results nearly accorded with or exhibited identical trend with previous study. The results suggest that, cervical disc replacement can basically guarantee the stability of lower cervical spine movement.
Can an Endplate-conformed Cervical Cage Provide a Better Biomechanical Environment than a Typical Non-conformed Cage?: A Finite Element Model and Cadaver Study

Abstract:Objectives: To evaluate the biomechanical characteristics of endplate-conformed cervical cages by finite element method (FEM) analysis and cadaver study. Methods: Twelve specimens (C2 -C7 ) and a finite element model (C3 -C7 ) were subjected to biomechanical evaluations. In the cadaver study, specimens were randomly assigned to intact (I), endplate-conformed (C) and non-conformed (N) groups with C4-5 discs as the treated segments. The morphologies of the endplate-conformed cages were individualized according to CT images of group C and the cages fabricated with a 3-D printer. The non-conformed cages were wedge-shaped and similar to commercially available grafts. Axial pre-compression loads of 73.6 N and moment of 1.8 Nm were used to simulate flexion (FLE), extension (EXT), lateral bending (LB) and axial rotation (AR). Range of motion (ROM) at C4-5 of each specimen was recorded and film sensors fixed between the cages and C5 superior endplates were used to detect interface stress. A finite element model was built based on the CT data of a healthy male volunteer. The morphologies of the endplate-conformed and wedge-shaped, non-conformed cervical cages were both simulated by a reverse engineering technique and implanted at the segment of C4-5 in the finite element model for biomechanical evaluation. Force loading and grouping were similar to those applied in the cadaver study. ROM of C4-5 in group I were recorded to validate the finite element model. Additionally, maximum cage-endplate interface stresses, stress distribution contours on adjoining endplates, intra-disc stresses and facet loadings at adjacent segments were measured and compared between groups. Results: In the cadaver study, Group C showed a much lower interface stress in all directions of motion (all P < 0.05) and the ROM of C4-5 was smaller in FLE-EXT (P = 0.001) but larger in AR (P = 0.017). FEM analysis produced similar results: the model implanted with an endplate-conformed cage presented a lower interface stress with a more uniform stress distribution than that implanted with a non-conformed cage. Additionally, intra-disc stress and facet loading at the adjacent segments were obviously increased in both groups C and N, especially those at the supra-jacent segments. However, stress increase was milder in group C than in group N for all directions of motion. Conclusions: Endplate-conformed cages can decrease cage-endplate interface stress in all directions of motion and increase cervical stability in FLE-EXT. Additionally, adjacent segments are possibly protected because intra-disc stress and facet loading are smaller after endplate-conformed cage implantation. However, axial stability was reduced in group C, indicating that endplate-conformed cage should not be used alone and an anterior plate system is still important in anterior cervical discectomy and fusion.
Characterization of the human intervertebral disc cartilage endplate at the molecular, cell, and tissue levels

Abstract:<p>Given the importance of the cartilage endplate in low back pain, there is a need to characterize the humancartilage endplateatthe molecular, cell and tissue level in order to inform treatment strategies whichtarget it. The goal of this study was to characterize the structure, matrix composition, and cell phenotype of the human cartilage endplate compared to adjacent tissues within the intervertebral joint: thenucleus pulposus andannulus fibrosus,andarticular cartilage. Isolated cartilage endplate, nucleus pulposus, annulus fibrosus and articular cartilage tissue and cells were evaluated for cell morphology, matrix composition, collagen structure, glycosaminoglycan content, and gene and protein expression. Thecartilage endplate containedelongated cells which mainly produce a collagen rich interterritorial matrix and proteoglycan rich territorial matrix. The cartilage endplate contained significantly less glycosaminoglycans than the nucleus pulposus tissue. Significant differences in matrix and cell markergene expression were observed between cartilage endplate and nucleus pulposusorannulus fibrosus, with greatest differences between cartilage endplate and articular cartilage. We were able to distinguishnucleus pulposus from cartilage endplatecells using collagen‐10, highlighting collagen‐10 as a potential cartilage endplate marker. Our findings suggest that at the cell and tissue level, thecartilage endplatedemonstrates both similarities and differences when compared with nucleus pulposus, annulus fibrosus and hyaline articular cartilage. This work highlights a unique structure, matrix composition and cell phenotype for the human cartilage endplate and can help inform regenerative strategies that target the intervertebral disc joint in chronic low back pain.</p><p>This article is protected by copyright. Abstract:BACKGROUND: Disc degeneration has been correlated with alteration of bone density of adjacent vertebral bodies. Abnormal mechanical loading appears in scoliosis as compared to normal spines. How vertebral endplate was remodelled in scoliosis is not well understood.METHODS: We conducted a micro-CT analysis of subchondral bone of the vertebral endplate at the curve apex in a porcine scoliosis model. Two adjacent thoracic T(5)-T(6) and lumbar L(1)-L(2) levels were instrumented in six four-week-old pigs with a custom offset implant connected by a flexible stainless steel wire. Two months after implantation, three cylindrical specimens were harvested into the vertebral endplate of each of the scoliosis levels: centre, convexity and concavity, and from the dorsal T(9)-T(10) vertebral units obtained from nine three-month-old non-instrumented pigs used as controls. Micro-CT analysis was carried out on each specimen.FINDINGS: In the concavity of the scoliotic spine, bone volume fraction, trabecular thickness, and trabecular separation significantly increased whereas in the convexity, only trabecular separation increased. Connectivity index and trabecular number decreased significantly.INTERPRETATION: This was the first micro-CT study of subchondral bone microarchitecture of the scoliotic vertebral end plate. At the curve apex, increased compression in the concavity induced an osteogenic process. In the convexity, diminished compression caused an osteolytic process with a local resorption. Clinically, the unbalanced tissue remodelling could play a role in the convective and diffusive transports into the end plate, which is of prime importance for the segment homeostasis in scoliosis treatment with or without surgery.

Three-Dimensional Morphometric Analysis of Lumbar Vertebral End Plate Anatomy

Abstract:Background: Information on the three-dimensional (3D) shape of vertebral end plates is lacking. Previous studies have analyzed two-dimensional shape; however, 3D data are important because they may help improve our understanding of how differences in shape are related to age, gender, race, size, and other parameters, which may subsequently help improve device design for interbody prosthesis. Objective: To study the 3D shape of lumbar vertebral end plates from normal adult lumbar spines and correlate them with age, gender, spinal/end plate level, end plate surface area, concave depth, and size. Methods: An in vivo analysis was undertaken of lumbar vertebral end plate 3D shape. A total of 136 patients' computed tomography scans were used to create 3D models of the lumbar spine for each patient, which were subsequently analyzed. Results: The shape of the superior end plates is different compared with inferior end plates. Across the lumbar spine (L1-S1), the shape of inferior end plates is similar; however, the shape of the superior end plate varies between spinal levels significantly. There was no clear relationship between age and principal component (PC) shapes but there was a strong correlation between end plate concave depth and end plate PC shape scores. Conclusions: Future interbody (disc replacement and fusion) device designs could use the findings that inferior end plate shape is similar throughout the length of the lumbar spine, whereas superior end plate shape changes. Further, future implants could be level-specific because the present study shows that end plate shape varies through the length of the lumbar spine.
Effect of Different Injury Morphology of the Endplate on Intervertebral Disc Degeneration: Retrospective Cohort Study

Abstract:We propose a simplified classification for the morphology characteristics of endplate injury based on 3D CT: mild, moderate, and severe. Our results indicate that moderate and severe endplate injury cause severe disc degeneration and long‐term loss of intervertebral height and Cobb angle. This may allow surgeons to predict patient prognosis and guide treatment for thoracolumbar fractures. Objectives To describe a simplified classification scheme for endplate injury morphology based on 3D CT and to examine possible associations between endplate injury morphology and vertebral space and other variables such as type of fracture and disc degeneration in a group of patients with thoracolumbar fractures. Methods This study was a retrospective cohort study. We collected patients with thoracolumbar fractures admitted from January 2015 to August 2020 and divided them into three groups based on the morphology of endplate injury (45 cases of mild endplate injury, 54 cases of moderate endplate injury, and 42 cases of severe endplate injury, SEI). Data of vertebral body and intervertebral space height and angle, the Pfirrmann grade, endplate healing morphology were collected during preoperative, postoperative, and long‐term follow‐up of patients in each group. One‐way analysis of variance (ANOVA), chi‐squared test, and repeated measurement ANOVA were used to compare and analyze the influence of endplate injury morphology on patient prognosis. Results Most moderate injuries to the endplate (fissure‐type injury) and severe injuries (irregular depression‐type injury, Schmorl's node‐type injury) resulted in significant disc degeneration in the long‐term transition. This study also showed significant differences in the height of the anterior margin of the injured spine and the intervertebral space height index during this process. Conclusions The current study suggests that although the region of injury in endplate fissure‐type injury is small preoperatively, it may be a major factor in leading to severe disc degeneration, loss of intervertebral height, and Cobb angle in the long term. The results of our study therefore may allow surgeons to predict the prognosis of patients with thoracolumbar fractures and guide their treatment.

Abstract:Abstract Background: Cervical disc replacement (CDR) is considered to be an effective alternative treatment to anterior cervical discectomy and fusion (ACDF). However, the use of most artificial disc prostheses can lead to complications due to the high morphological and dimensional mismatch with the human neck anatomy. Objective: By comparing the clinical efficacy of the new 3D printed artificial cervical disc with ACDF in treating cervical spondylosis, to study the clinical value of the artificial disc with the appropriate morphological and dimensional. Methods: This study was a retrospective analysis of 58 patients with single-segment cervical spondylosis who met the inclusion criteria between August 2019 and July 2022 at XXXX Hospital. The patients were divided into two groups based on the surgical method used: 38 cases in the ACDF group and 20 cases in the CDR group. The patients' general information, including operation time, intraoperative blood loss, and hospital stay, were recorded. The clinical effect was evaluated using the visual analog scale (VAS), Japanese Orthopedic Association (JOA) score, and Neck Disability Index (NDI). The radiological effect was evaluated using the range of movement (ROM) by the shell angle. Complications, including dysphagia, heterotopic ossification, and adjacent segment degeneration (ASD), were also recorded. Results: The average follow-up time for the ACDF and CDR groups was 18.18 ± 2.19 and 17.50 ± 7.18 months, respectively ( P =0.682). The surgical time was 134.76 ± 39.35, 144.50 ± 35.69 ( P =0.359), and the hospital stay was 6.74 ± 1.63 and 7.00 ± 2.00 days ( P =0.593). In terms of clinical efficacy, the two groups showed significant clinical efficacy, and there was no difference in VAS ( P =0.046), JOA ( P =0.046), and NDI ( P =0.046) scores at each time point. However, the blood loss of the two groups was 170.00 ± 110.50ml and 118.42 ± 80.15ml, and the CDR group was significantly higher than the ACDF group ( P =0.046). In terms of imaging, the postoperative ROM in the ACDF group was lower than that in the CDR group ( P =0.005). In terms of complications, a few patients in both groups experienced swallowing discomfort after surgery and recovered within 3 months. One case of ASD occurred in the ACDF group, and no ectopic ossification was observed in either group. Conclusion: The new 3D-printed artificial disc for treating single-segment cervical spondylosis has shown proven clinical efficacy, and it maintains better ROM compared to ACDF, although it may have more intraoperative blood loss.
Sagittal morphometry of intervertebral spaces in subaxial cervical region of asymptomatic Chinese

Abstract:Purpose: This study aimed to quantify and evaluate morphology of intervertebral space between neighboring cervical units using radiographic imaging indices, to help spine surgeons when performing anterior cervical discectomy and fusion (ACDF) surgery on the Chinese population. Methods: The background and imaging parameters of the subjects were assessed. Cervical lateral radiographs were employed to measure the intervertebral height (IH), intervertebral height index (IHI), and segmental lordosis (SL). Endplate parameters measurements were conducted on sagittal T2-weighted magnetic resonance imaging (MRI), including endplate sagittal diameter (ESD), and endplate concavity depth (ECD). All individuals were divided into three age groups: individuals aged 20-35 were in group A, individuals aged 36-50 were in group B, and individuals aged over 50 were in group C. A comparison of the variables was conducted among the three groups. Additionally, these radiographic parameters were also compared between males and females. Results: A total of 102 individuals were included in this study. IH was greater at C6/7 than those at other segmental levels (p < 0.001). The largest SL values were found at C6/7, while the least were found at C3/4. The superior ESD (ESDs) and ECD (ECDs) of the intervertebral space were significantly greater than those of the inferior endplates (p < 0.05). The ESD and ECD values were the largest at C6/7, while the least at C3/4. Additionally, age and gender had an influence on several parameters. IH was significantly lower in group A compared to group B (p < 0.05) and group C (p < 0.05) from C3/4 level to C6/7 level. ECDs were lower in group A compared to group B (p < 0.05) and group C (p < 0.05) at each level. IH and ESD in males were generally significantly greater than those in females at all levels (p < 0.05). Conclusion: The current study found that C6/7 had the greatest IH, SL, ESD, and ECD values in asymptomatic Chinese. SL gradually increased from C3/4 to C6/7 levels. IH and ECD were significantly associated with age. Males had greater IH and ESD values than females. These findings provide baseline information for planning for selection of anterior screws and intervertebral implants.

Abstract:Background: Most of the current available cervical disc prostheses present a flat surface instead of an arcuate surface which is most similar to the morphology of cervical endplate. Therefore, we designed a novel prosthesis (Pretic-I, Trauson) based on the physiological curvature of the cervical endplate. Biomechanical evaluation of cervical disc replacement (CDR) with this novel prosthesis was performed and compared with the Prestige LP prosthesis. Methods: Three motion segments of 18 cadaveric cervical specimens (C2-C7) were evaluated with a 75 N follower load. Overall, the biomechanics of three models, intact specimen, CDR with the novel prosthesis and CDR with the Prestige LP prosthesis, were studied to gain insight into the effective function of the novel prosthesis. The range of motion (ROM) of all three segments and intradiscal pressure (IDP) on adjacent levels were measured and analysed. Results: Compared to the intact condition, the ROM of all three segments showed no significant difference in the replacement group. Moreover, there was also no significant difference in the ROM between the two prostheses. Besides, the IDP on the cranial adjacent level showed no obvious difference between the two prostheses; nevertheless, the IDP on the caudal adjacent level of the novel prosthesis was significantly less than the Prestige LP prosthesis. Conclusions: In summary, the novel disc prosthesis was effective to maintain the ROM at the target segment and adjacent segments. Besides, CDR with the novel prosthesis could reduce the IDP on the caudal adjacent level to a certain extent, compared with the Prestige LP prosthesis.

Abstract:Cartilage endplates (CEPs) act as protective mechanical barriers for intervertebral discs (IVDs), yet their heterogeneous structure-function relationships are poorly understood. This study addressed this gap by characterizing and correlating the regional biphasic mechanical properties and biochemical composition of human lumbar CEPs. Samples from central, lateral, anterior, and posterior portions of the disc (n = 8/region) were mechanically tested under confined compression to quantify swelling pressure, equilibrium aggregate modulus, and hydraulic permeability. These properties were correlated with CEP porosity and glycosaminoglycan (s-GAG) content, which were obtained by biochemical assays of the same specimens. Both swelling pressure (142.79 ± 85.89 kPa) and aggregate modulus (1864.10 ± 1240.99 kPa) were found to be regionally dependent (p = 0.0001 and p = 0.0067, respectively) in the CEP and trended lowest in the central location. No significant regional dependence was observed for CEP permeability (1.35 ± 0.97 * 10-16 m4/Ns). Porosity measurements correlated significantly with swelling pressure (r = -0.40, p = 0.0227), aggregate modulus (r = -0.49, p = 0.0046), and permeability (r = 0.36, p = 0.0421), and appeared to be the primary indicator of CEP biphasic mechanical properties. Second harmonic generation microscopy also revealed regional patterns of collagen fiber anchoring, with fibers inserting the CEP perpendicularly in the central region and at off-axial directions in peripheral regions. These results suggest that CEP tissue has regionally dependent mechanical properties which are likely due to the regional variation in porosity and matrix structure. This work advances our understanding of healthy baseline endplate biomechanics and lays a groundwork for further understanding the role of CEPs in IVD degeneration.
