Abstract:Mexican Americans are disproportionally affected by metabolic dysfunction-associated steatotic liver disease (MASLD), which often co-occurs with diabetes. Despite extensive evidence on the causative role of the gut microbiome in MASLD, studies determining the involvement of the gut phageome are scarce. In this cross-sectional study, we characterized the gut phageome in Mexican Americans of South Texas by stool shotgun metagenomic sequencing of 340 subjects, concurrently screened for liver steatosis by transient elastography. Inter-individual variations in the phageome were associated with gender, country of birth, diabetes, and liver steatosis. The phage signatures for diabetes and liver steatosis were subsequently determined. Enrichment of Inoviridae was associated with both diabetes and liver steatosis. Diabetes was further associated with the enrichment of predominantly temperate Escherichia phages, some of which possessed virulence factors. Liver steatosis was associated with the depletion of Lactococcus phages r1t and BK5-T, and enrichment of the globally prevalent Crassvirales phages, including members of genus cluster IX (Burzaovirus coli, Burzaovirus faecalis) and VI (Kahnovirus oralis). The Lactococcus phages showed strong correlations and co-occurrence with Lactococcus lactis, while the Crassvirales phages, B. coli, B. faecalis, and UAG-readthrough crAss clade correlated and co-occurred with Prevotella copri. In conclusion, we identified the gut phageome signatures for two closely linked metabolic diseases with significant global burden. These phage signatures may have utility in risk modeling and disease prevention in this high-risk population, and identification of potential bacterial targets for phage therapy.IMPORTANCEPhages influence human health and disease by shaping the gut bacterial community. Using stool samples from a high-risk Mexican American population, we provide insights into the gut phageome changes associated with diabetes and liver steatosis, two closely linked metabolic diseases with significant global burden. Common to both diseases was an enrichment of Inoviridae, a group of phages that infect bacterial hosts chronically without lysis, allowing them to significantly influence bacterial growth, virulence, motility, biofilm formation, and horizontal gene transfer. Diabetes was additionally associated with the enrichment of Escherichia coli-infecting phages, some of which contained virulence factors. Liver steatosis was additionally associated with the depletion of Lactococcus lactis-infecting phages, and enrichment of Crassvirales phages, a group of virulent phages with high global prevalence and persistence across generations. These phageome signatures may have utility in risk modeling, as well as identify potential bacterial targets for phage therapy.

Abstract:Background and AimsHispanics are disproportionately affected by non-alcoholic fatty liver disease, liver fibrosis, cirrhosis, and hepatocellular carcinoma. Preventive strategies and non-invasive means to identify those in this population at high risk for liver fibrosis, are urgently needed. We aimed to characterize the gut microbiome signatures and related biological functions associated with liver fibrosis in Hispanics and identify environmental and genetic factors affecting them. Approach and ResultsSubjects of the population-based Cameron County Hispanic Cohort (n=217) were screened by vibration-controlled transient elastography (FibroScan). Among them, 144 (66.7%) had steatosis and 28 (13.0%) had liver fibrosis. The gut microbiome of subjects with liver fibrosis was enriched with immunogenic commensals (e.g. Prevotella copri, Holdemanella, Clostridiaceae 1) and depleted of Bacteroides caccae, Parabacteroides distasonis, Enterobacter and Marinifilaceae. The liver fibrosis-associated metagenome was characterized by changes in the urea cycle, L-citrulline biosynthesis and creatinine degradation pathways, and altered synthesis of B vitamins and lipoic acid. These metagenomic changes strongly correlated with the depletion of Parabacteroides distasonis and enrichment of Prevotella and Holdemanella. Liver fibrosis was also associated with depletion of bacterial pathways related to L-fucose biosynthesis. Alcohol consumption, even moderate, was associated with high Prevotella abundance. The single nucleotide polymorphisms rs3769502 and rs7573751 in the NCK2 gene positively associated with high Prevotella abundance. ConclusionHispanics with liver fibrosis display microbiome profiles and associated functional changes that may promote oxidative stress and a pro-inflammatory environment. These microbiome signatures, together with NCK2 polymorphisms, may have utility in risk modeling and disease prevention in this high-risk population.

Abstract:Introduction: Dietary patterns high in healthy plant foods play an important role in modulating the gut microbiome and promoting cardiometabolic health. Little is known on the diet-gut microbiome relationship in U.S. Hispanics/Latinos, who have a high burden of obesity and diabetes. Hypothesis: Three healthy dietary patterns - the alternate Mediterranean diet (aMED), the Healthy Eating Index (HEI)-2015, and the healthful Plant-based Diet Index (hPDI), will be associated with gut microbiome composition in U.S. Hispanic/Latino adults, and healthy-diet-related species will be related to favorable cardiometabolic traits. Methods: HCHS/SOL is a multi-site community-based cohort. At baseline (2008-11), diet was assessed via two 24-hr recalls. Shotgun sequencing was performed on stool samples collected in 2014-17 (n=2,985). Analysis of Compositions of Microbiomes 2 (ANCOM2) was used to identify associations of dietary pattern scores with gut microbiome species and functions, adjusting for sociodemographic, behavioral, and clinical covariates. Results: Better diet quality according to multiple healthy dietary patterns was associated with higher abundance of species from class Clostridia, including [Eubacterium] eligens , Butyrivibrio crossotus , and Lachnospiraceae bacterium TF01-11 , as well as functions related to fiber fermentation (e.g. pyruvate:ferredoxin oxidoreductase). Poorer diet quality was associated with higher abundance of Acidaminococcus intestini , as well as functions of manganese/iron transport, adhesin protein transport, and nitrate reduction. Healthy diet pattern-enriched Clostridia species were related to more favorable cardiometabolic traits such as lower triglycerides ( Figure 1 ). Conclusions: Healthy dietary patterns in this population are associated with a fiber-fermenting gut microbiome, consistent with previous studies in other racial/ethnic groups. Gut microbiota may be involved in the beneficial effect of higher diet quality on cardiometabolic disease risk.

Abstract:The prevalence of metabolic-dysfunction-associated steatotic liver disease (MASLD) is alarmingly high; it is estimated to affect up to a quarter of the global population, making it the most common liver disorder worldwide. MASLD is characterized by excessive hepatic fat accumulation and is commonly associated with comorbidities such as obesity, dyslipidemia, and insulin resistance; however, it can also manifest in lean individuals. Therefore, it is crucial to develop effective therapies for this complex condition. Currently, there are no approved medications for MASLD treatment, so there is a pressing need to investigate alternative approaches. Extensive research has characterized MASLD as a multifaceted disease, frequently linked to metabolic disorders that stem from dietary habits. Evidence suggests that changes in the gut microbiome play a fundamental role in the development and progression of MASLD from simple steatosis to steatohepatitis and even hepatocellular carcinoma (HCC). In this review, we critically examine the literature on the emerging field of gut-microbiota-based therapies for MASLD and metabolic-dysfunction-associated steatohepatitis (MASH), including interventions such as fecal microbiota transplantation (FMT), probiotics, prebiotics, short-chain fatty acids, antibiotics, metabolic pathway targeting, and immune checkpoint kinase blockade.

Abstract:BACKGROUND Metabolic dysfunction-associated steatotic liver disease (MASLD) and metabolic dysfunction-associated steatohepatitis (MASH) are a growing health burden across a significant portion of the global patient population. However, these conditions seem to have disparate rates and outcomes between different ethnic populations. The combination of MASLD/MASH and type 2 diabetes increases the risk of hepatocellular carcinoma (HCC), and Hispanic patients experience the greatest burden, particularly those in South Texas. AIM To compare outcomes between Hispanic and non-Hispanic patients in the United States, while further focusing on the Hispanic population within Southeast Texas to determine whether the documented disparity in outcomes is a function of geographical circumstance or if there is a more widespread reason that all clinicians must account for in prognostic consideration. METHODS This cohort analysis was conducted with data obtained from TriNetX, LLC ("TriNetX"), a global federated health research network that provides access to deidentified medical records from healthcare organizations worldwide. Two cohort networks were used: University of Texas Medical Branch (UTMB) hospital and the United States national database collective to determine whether disparities were related to geographic regions, like Southeast Texas. RESULTS This study findings revealed Hispanics/Latinos have a statistically significant higher occurrence of HCC, type 2 diabetes mellitus, and liver fibrosis/cirrhosis in both the United States and the UTMB Hispanic/Latino groups. All-cause mortality in Hispanics/Latinos was lower within the United States group and not statistically elevated in the UTMB cohort. CONCLUSION This would appear to support that Hispanic patients in Southeast Texas are not uniquely affected compared to the national Hispanic population.

Abstract:Hepatic steatosis is a multifactorial condition that is often observed in obese patients and is a prelude to non-alcoholic fatty liver disease. Here, we combine shotgun sequencing of fecal metagenomes with molecular phenomics (hepatic transcriptome and plasma and urine metabolomes) in two well-characterized cohorts of morbidly obese women recruited to the FLORINASH study. We reveal molecular networks linking the gut microbiome and the host phenome to hepatic steatosis. Patients with steatosis have low microbial gene richness and increased genetic potential for the processing of dietary lipids and endotoxin biosynthesis (notably from Proteobacteria), hepatic inflammation and dysregulation of aromatic and branched-chain amino acid metabolism. We demonstrated that fecal microbiota transplants and chronic treatment with phenylacetic acid, a microbial product of aromatic amino acid metabolism, successfully trigger steatosis and branched-chain amino acid metabolism. Molecular phenomic signatures were predictive (area under the curve = 87%) and consistent with the gut microbiome having an effect on the steatosis phenome (>75% shared variation) and, therefore, actionable via microbiome-based therapies.

Abstract:Metabolic syndrome (MetS) is characterized by a cluster of conditions including abdominal obesity, elevated blood pressure, altered lipid profiles and high blood sugar levels. These factors collectively elevate risks for cardiovascular diseases, diabetes, and metabolic dysfunction associated steatotic liver disease (MASLD). MetS impacts approximately a third of the US populace and a quarter of the global population. Despite its widespread prevalence, there are no existing medications to specifically treat MetS. With the global rise in MetS cases, understanding its underlying mechanisms is paramount. Our research emphasizes the potential physiological and metabolic disruptions within the gastrointestinal (GI) tract that might contribute to MetS manifestation. Using a mouse model of MetS (MS NASH) and healthy lean controls (C57BL/6J), we investigated the colon environment. 16S rRNA sequencing from fecal samples revealed a distinct microbiota profile in MS NASH mice, highlighted by an increase in the Lactobacillus genus. Colonic metabolome and transcriptome in MS NASH mice displayed an upregulation in anabolic pathways, like glycolysis, the pentose phosphate pathway, and the tryptophan/kynurenine pathway. Subsequent integrated analysis of these datasets unveiled potential compensatory mechanisms and pathway crosstalk, within the colonic environment. Despite these alterations, MS NASH mice showed no signs of intestinal inflammation, a finding confirmed through detailed histological and LCN-2 assessments. Additionally, there were no changes detected in genes typically associated with increased intestinal inflammation or permeability. This study provides insight in the nuanced relationship between the metabolic state, gut microbiota, and potential progression towards other metabolic disorders. Our findings highlight the significance of multi-omics data convergence in understanding the changes within the colonic environment and their implications for MetS onset and/or progression. Previous data from Asian countries indicate varying prevalence rates of epidemiology of metabolic syndrome (MetS) and metabolic dysfunction-associated steatotic liver disease (MASLD) in CHB patients. Methods: This study aims to investigate the epidemiology of MetS and MASLD and their association with liver histology and hepatitis B virus (HBV) biomarkers in Asian Americans (AAs) with CHB. We used the AoU Research Program which contains 14,162 AAs in the Controlled Tier Dataset v7 and we created a cohort containing 274 (1.93%) AAs with CHB. Results: Of the 274 AAs, 48 (17.52%) have type-2 diabetes, 41 (14.96%) have obesity, 75 (27.37%) have high cholesterol, 97 (35.40%) have hypertension, and 49 (17.88%) have liver steatosis. MASLD is characterized by having liver steatosis along with at least one component of the MetS. Based on the definition, there are 46 individuals classified as having MASLD, which was 16.79% of all AAs with CHB. The high prevalence indicates that liver steatosis may be a significant risk factor for the development of MASLD in this population. Currently, we are still analyzing the data to examine the relationship and impact of MetS, MASLD, and other relevant factors. In this presentation, we will share our experience in utilizing the AoU datasets and discuss our next steps and timeline. In this presentation, we will also share our experience navigating the Researcher Workbench for data cleaning and analysis for our current NCI/NIH-funded projects that focuses on examining the metabolic and steatotic profile and risk factors among Asian Americans living with chronic hepatitis B. Conclusion: While the AoU Research Program offers unprecedented access to electronic health records, the dataset has proven challenging to work with. Addressing the remaining challenges entails several key aspects. Firstly, it involves the meticulous task of accounting for single-concept variables sourced from diverse origins. Secondly, there's the crucial step of synthesizing variables that essentially gauge identical concepts. Additionally, it requires a deep comprehension of the data structure and hierarchy, particularly when coding for disease presence. Equally vital is ensuring data accuracy and reliability throughout the process. Finally, we'll delve into previous setbacks, elucidating the solutions we've since devised. Through this presentation, we hope to facilitate communication and build connection with researchers in cancer health disparities field.
