Abstract:Objective To analyze the safety, feasibility and operative technique details of non-grasping en bloc mediastinal lymph nodes dissection technique in uniportal video-assisted thoracic surgery(VATS) for lung cancer.Methods From April, 2014 to March, 2015,46 patients with lung cancer received non-grasping en bloc mediastinal lymph nodes dissection after uniportal VATS lobectomy.Clinical data of the cases were analyzed retrospectively.There were 19 males and 27 females.The age was(57.2 ± 9.0) (38-73) years.The first 6 cases were performed in the lateral decubitus position while the later 40 cases were all performed in the semiprone position.Results All cases accepted uniportal VATS non-grasping en bloc mediastinal lymph nodes dissection successfully.Arm fatigue of surgeon and assistant was obviously relieved when the patient was placed in the semiprone position.The thoracic drainage time was(3.2 ± 2.1) (1-12)days and the postoperative length of hospital-stay was(6.0 ± 4.5) (2-27) days.The number of dissected mediastinal lymph nodes stations was (4.3 ± 0.8) (3-6)and the number of dissected mediastinal lymph nodes was (11.8 ± 4.9) (4-30).There were 42 cases with stage No , lease wit stage N1, and 3 cases with stage N2 in pathological examination.Five patients developed minor postoperative complications.No perioperative death occurred.Conclusion Uniportal VATS non-grasping en bloc mediastinal lymph nodes dissection for lung cancer was safe and feasible, which could decrease the interference of the instruments and help to keep the surgical field clear.Non-grasping en bloc mediastinal lymph nodes dissection would be performed more smoothly in the semiprone position with less damage to lung and better ergonomics.

Abstract:BackgroundThe role of video-assisted thoracoscopic surgery (VATS) in mediastinal lymph node dissection (MLND) for non-small cell lung cancer (NSCLC) following neoadjuvant therapy remains controversial. The aim of this study was to demonstrate the sufficiency of VATS by evaluating perioperative and long-term outcomes. MethodsPatients with locally advanced NSCLC and treated with radical surgery after neoadjuvant therapy were identified in our database. The thoroughness of MLND was compared by approach. Multivariable logistic regression analysis was used to evaluate predictors of sufficient MLND. Propensity score matching was performed. Kaplan-Meier and Cox proportional hazard analyses were used to assess long-term survival. ResultsOf the 127 enrolled patients, 56 underwent attempted VATS and 71 underwent thoracotomy. Multivariable logistic regression analysis revealed that approach was not a predictor of sufficient MLND (odds ratio 0.81, 95% confidence interval [CI] 0.364-1.803; P =0.606). After matching, 28 pairs of patients were selected from the two groups. There was no significant difference between the numbers of dissected lymph nodes (15 vs. 20; P =0.191) and nodal stations (7 vs. 7; P =0.315). Recurrence-free (log-rank P =0.613) and overall survival (log-rank P =0.379) was similar in both groups. Multivariable Cox proportional hazards model analysis indicated that VATS was not an independent predictor of recurrence-free (hazard ratio 0.955, 95% CI 0.415-2.198; P =0.913) or overall survival (hazard ratio 0.841, 95% CI 0.338-2.093; P =0.709). ConclusionCompared to thoracotomy, VATS is a sufficient approach for MLND to treat locally advanced NSCLC following neoadjuvant therapy without compromising long-term survival.
Comparison of Video-assisted Thoracoscopic with Open Dissection of Systematic Mediastinal Lymph Nodes for Clinical Stage Ⅰ Non-Small Cell Lung Cancer

Abstract:Objective To evaluate the efficacy of systemic mediastinal lymph node dissection (SND)by video-assisted thoracoscopic surgery (VATS) in patients with clinical stage Ⅰ non-small cell lung cancer. Methods Totally 282 consecutive patients with clinical stage Ⅰ lung cancer who underwent lobectomy with SND between January 2003 and July 2009 were enrolled in this study. Of these patients, 152 received complete VATS surgery and 130 cases through thoracotomy. Completeness measured by the number of dissected stations and nodes, and safety evaluated by postoperative drainage and SND-related complications were compared between the two groups. Results Mediastinal lymph nodes stations dissected [median 4 stations (3-6 stations) vs 4 stations (3-7 stations), Z=0.603,P=0.544], number of mediastinal nodes dissected (13.7±6.1vs 14.6±7.2, t=-1.136,P=0.257), and nodes dissected in each mediastinal zones were not significantly different between the VATS and thoracotomy groups. No significant difference was found in SND-related complications (2 cases of chylothorax in VATS group; 2 cases of chylothorax and 1 case of recurrent nerve injury in thoracotomy group, χ2=0.031,P=0.860) or postoperative drainage [(8.1±3.9) d vs (8.6±4.1)d,t=-1.048,P=0.296]. Conclusion SND by completely VATS is as effective as thoracotomy with respect to completeness and safety issues.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.

Abstract:Single-port video-assisted thoracoscopic surgery (VATS) has recently been proposed as an innovative minimally invasive alternative to the standard three-port VATS for lobectomies, most of which are performed using a conventional rigid thoracoscope. Here, we report a single-port VATS approach for right upper lobectomy and systematic lymph node dissection using a flexible endoscope. A 61-year-old male smoker presented with a pulmonary nodule. A single-port VATS procedure was performed through a 4-cm intercostal incision using a flexible laparo-thoraco videoscope. Right upper lobectomy and systematic lymph node dissection were performed. The total operating time was 106 in. The procedure was successful and the recovery uneventful. The patient's chest tube was removed on the third day, and he was discharged home on the fourth. The use of a flexible videoscope facilitated the single-port VATS procedure by avoiding interference between the videoscope and other operating instruments and providing ample space for the surgeon.

Abstract:[Objective] To evaluate the application of video-mediastinoscopy(VM)and video assisted thoracoscopy surgery(VATS)in diagnosis on mediastinal small cell cancer.[Method] 20 patients with mediastinal lesions undertook 16 VM biopsies and 4 VATS exploration form July 2000 to March 2006.Three patients with massive thoracic pleural effusion received talc pleurodesis and 1 patient with pericardial effusion received pericardial window after malignancy were confirmed.[Results] All patients recovered smoothly without mortality and severe postoperative complications.Small cell cancer was confirmed in all the 20 patients.One of them was hematogenic and lymphagenic,2 of them were from thymus and other 17 lesions were from lung.Both thoracic pleural effusion and pericardial effusion were well controlled.[Conclusion ] VM and VATS is a mini-invasive,safe and accurate method in diagnosis on mediastinal small cell cancer.It is the best way to make a histological diagnosis on such kind of lesions.Also,effective treatment on thoracic effusion and pericardial effusion could be performed through VATS.
Uniportal Video-Assisted Thoracoscopic Right Upper Posterior Segmentectomy with Systematic Mediastinal Lymphadenectomy

Abstract:Uniportal video-assisted thoracoscopic surgery (VATS) has now evolved into a sophisticated technique that can be used in some of the most complex thoracic procedures; however, this approach to segmentectomy is not standardized, and the surgical procedure varies between surgeons. Here, we describe the use of our uniportal VATS procedure during right upper posterior segmentectomy in a patient with a nodule in the right upper lobe. Subsequent mediastinal lymphadenectomy was performed. The patient has recovered well after surgery. We believe that uniportal VATS segmentectomy is a technically safe and feasible alternative approach to conventional thoracoscopic techniques for treating lung cancer.
Perioperative Comparison of Video-Assisted Thoracic Surgery and Open Lobectomy for Pt1-Stage Non-Small Cell Lung Cancer Patients in China: a Multi-Center Propensity Score-Matched Analysis.

Abstract:BACKGROUND:Compared with open surgery, video-assisted thoracic surgery (VATS) has innovated the concept of the minimally invasive approach for non-small cell lung cancer (NSCLC) patients in past decades. This present study aimed to compare the perioperative and lymph node dissection outcomes between VATS lobectomy and open lobectomy for pathological stage T1 (pT1) NSCLC patients from both surgical and oncologic perspectives.METHODS:This was a retrospective multicenter study. Patients who underwent surgical resection for pT1 NSCLC between January 2014 and September 2017 were retrospectively reviewed from 10 thoracic surgery centers in China. Perioperative and lymph node dissection outcomes of pT1 NSCLC patients who accepted VATS or open lobectomies were compared by propensity score matching (PSM) analysis.RESULTS:Of the 11,360 patients who underwent surgery for pT1 NSCLC, 7,726 were enrolled based on the selection criteria, including 1,222 cases of open lobectomies and 6,504 cases of VATS lobectomies. PSM resulted in 1,184 cases of open lobectomies and 1,184 cases of VATS lobectomies being well matched by common prognostic variables, including age, sex, and surgical side. VATS lobectomy led to better perioperative outcomes, including less blood loss (133.5±200.1 vs. 233.3±318.4, P<0.001), lower blood transfusion rate (2.4% vs. 6.4%, P<0.001), shorter postoperative hospital stay (8.6±5.7 vs. 10.1±5.1, P<0.001), less chest drainage volume (1,109.5±854.0 vs. 1,324.1±948.8, P<0.001), and less postoperative complications (4.9% vs. 8.2%, P<0.001). However, open lobectomy had better lymph node dissection outcomes than VATS, with increased lymph node dissection numbers (16.1±9.4 vs. 13.7±7.7, P<0.001) and more positive lymph nodes being dissected (1.5±3.9 vs. 1.1±2.5, P=0.002). Compared with VATS, open lobectomy harvested more lymph node stations (5.5±1.9 vs. 5.2±1.8, P=0.001), including more pathological N2 (pN2) lymph node stations (3.4±1.4 vs. 3.1±1.3, P<0.001).CONCLUSIONS:VATS lobectomy was associated with better perioperative outcomes, such as less blood loss, lower blood transfusion rate, shorter postoperative hospital stay, less chest drainage volume and less postoperative complications. Open lobectomy has improved lymph node dissection outcomes, as more lymph nodes and positive lymph nodes were dissected for pT1 NSCLC patients during surgery.
Clinical Analysis of Single Utility Port Video-Assisted Thoracic Surgery in Surgical Treatment of Non-Small Cell Lung Cancer

Abstract:Objective To explore the feasibility and clinical application value of single utility port video-assisted thoracic surgery(VATS) pulmonary lobectomy and systematic mediastinal lymphadenectomy in surgical treatment of non-small cell lung cancer(NSCLC). Methods We reviewed the clinical data of 42patients with NSCLC received single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy from March 2011 to March 2013. The utility port for observation was about 1.5 cm long and located at the 7th and 8th rib in the middle axillary line. The port for operation was about 4.0-5.0 cm long and located at the 3rd and 4th rib in the outer edge of pectoralis major. Results All of the 42 patients were successfully subjected to single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy, none of them needed converse to transit-assisted incision or thoracotomy. Operative duration was 90-200 min; intraoperrative blood loss was 50-400 ml;lymph node dissection number was 9-16. After the surgery, the patients received chest drainage for 5-8 days. They were discharged from hospital in 6-10 days after the surgery without severe complications. Conclusions Based on the conventional three utility ports VATS, the single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy caused less injury in surgical treatment of non-small cell lung cancer(NSCLC). It has certain clinical advantages and was a minimally invasive surgical treatment for selected patients with NSCLC.
Retrospective Study on Video-Assisted Vs. Open Mediastinal Lymphadenectomy for Non-Small Cell Lung Cancer: a Propensity-Matched Analysis

Abstract:Background: This retrospective study is designed to compare the surgical outcomes of mediastinal lymph node (MLN) dissection between the two groups of non-small cell lung cancer (NSCLC) patients underwent lung cancer surgery through video-assisted thoracoscopic surgery (VATS) or thoracotomy, as well as classify the differences and identify the safer method between two groups. Methods: A total of 497 patients underwent pulmonary lobectomy and systemic MLN dissection were enrolled in this study. They were divided into VATS group (n=242) and traditional thoracotomy (TT) group (n=255). The VATS group received three-port VATS, while the thoracotomy group underwent posterior-lateral or muscle sparing thoracotomy. The range of MLN dissection included groups 2R, 4R, 7, 8, 9 in right-sided lung cancer surgery and groups 4L, 5, 6, 7, 8, 9 in left-sided. The two groups were matched at a ratio of 1: 1 using the propensity score matching (PSM) method. Results: Out of 497 patients, 376 cases were matched for PSM, with 188 cases in each group. Baseline date did not get any significant difference between two groups. The average group number of MLN dissection in VATS group and TT group was 4.14 and 3.97, respectively. While the average amount of dissected lymph nodes in both groups were 16.63 and 17.32, respectively. Compare with the lymph node numbers among all the groups in TT group, only the number of group 7 though left approach (7L) in VATS group was statically significant. Besides, the operation time and postoperative length of hospital stay in VATS group were superior to those in TT group (P<0.05). No significant difference had been found in intraoperative blood loss, accessory surgical injury and postoperative complication between two groups. Conclusion: Compared with thoracotomy, systemic MLN dissection and pulmonary lobectomy via VATS has quicker recovery, less postoperative complication and length of hospital stay with similar surgical outcomes. Moreover, our findings also suggest thoracotomy has an advantage on the dissection of MLN in group 7L.
Applications of Video-assisted Thoracic Surgery for the Diagnosis and Treatment of Patients with Small Pulmonary Nodules

Abstract:BACKGROUND AND OBJECTIVE:Chest computed tomography (CT), particularly thin-slice high resolution CT, has low sensitivity and specificity for detecting pulmonary nodules <10 mm in size. This limitation leads to challenges in clinical diagnosis and treatment of small pulmonary nodules. This study introduces the use of video-assisted thoracoscopic surgery (VATS) for the diagnosis and treatment of small pulmonary nodules.METHODS:From November 2009 to May 2012, 64 patients with small pulmonary nodules without prior preoperative pathologic diagnosis were treated by pulmonary wedge resection through VATS. The diagnosis of small pulmonary nodules was established from rapid frozen section. The type of operation depends on the pathology and the condition of the patients. Twenty patients with primary lung cancer were subjected to lobectomy and radical resection of the lymph nodes by complete thoracospic lobectomy or video-assisted thoracoscopic invasive lobectomy. Pulmonary wedge resection was performed in 44 patients, among whom 21 have benign nodule, 18 have precancerous lesion, 3 have metastatic nodule, and 2 have primary lung cancer for which lobectomy was not fit.RESULTS:Confirmative diagnosis is difficult to obtain among patients with small pulmonary nodules. VATS is effective in the diagnosis and treatment of small pulmonary nodules. With VATS, patients with benign small pulmonary nodules can be cured, and patients with primary lung cancer can receive definite diagnosis and effective treatment in time.CONCLUSIONS:CT-guided hook-wire fixation is useful in precise lesion localization for surgical resection.
Comparative Study of Systematic Thoracoscopic Lymphadenectomy and Conventional Thoracotomy in Resectable Non-Small Cell Lung Cancer.

Abstract:Objective: To assess the feasibility and safety of the video-assisted thoracoscopy surgery (VATS) systematic lymph node dissection in resectable non-small cell lung cancer (NSCLC).Methods: The clinical data of patients with NSCLC who underwent VATS or thoracotomy combined with lobectomy and systematic lymphadenectomy from January 2001 to January 2008 were retrospectively analyzed to identify their demographic parameters, number of dissected lymph nodes and postoperative complications.Results: A total of 5,620 patients were enrolled in this study, with 2,703 in the VATS group, including 1,742 men (64.4%), and 961 women (35.6%), aged 59.5 +/- 10.9 years; and 2,917 in the thoracotomy group, including 2,163 men (74.2%), and 754 women (25.8%), aged 58.5 +/- 10.4 years. Comparing the VATS with the thoracotomy groups, the mean operative time was 146 vs. 157 min, with a significant difference (P<0.001); and the average blood loss was 162 vs. 267 mL, with a significant difference (P<0.001). Comparing the two groups of patients data, the number of lymph node dissection: 18.03 in the VATS group and 15.07 in the thoracotomy group on average, with a significant difference (P<0.001); postoperative drainage time: 4.5 days in the VATS group and 6.37 days in the thoracotomy group on average, with a significant difference (P<0.001); postoperative hospital stay: 6.5 days in the VATS group and 8.37 days in the thoracotomy group on average, with a significant difference (P<0.001); proportion of postoperative chylothorax: 0.2% (4/2,579) in the VATS group and 0.4% (10/2,799) in the thoracotomy group, without significant difference (P>0.05).Conclusions: For patients with resectable NSCLC, VATS systematic lymph node dissection is safe and effective with fewer postoperative complications, and significantly faster postoperative recovery compared with traditional open chest surgery.

Abstract:Objective To appraise the technique and the indication of the Video Assisted Thoracic Surgery(VATS) in particular, the lung cancer. Methods Twenty patients successfully underwent anatomical lobectomy onVATS with 7 cm minithoractomy. The 7 cm minimal thoractomy was made at the third or fourth intercostal space justbehind the anterior axillary line for using conventional or dedicated endoscopic instruments. Mediastinal lymph nodeswere routinely removed through the minithoractomy. Forty four staple-liner(ENDOPATH) were used in these 20 ca-ses. Three operations were performed on VATS with incision extended to 12～14 cm due to pleural adhesion, absenceof fissure and bleeding during hilum dissection. Results There were no operative death. Complication included delayedwound healing in one and air leak for 4～5 days in two. Eight patients with benign lesions recovered uneventfully. Themean follow-up period is 15 months, one patient with adenocarcinoma developed recurrence of tumor at the site of chestdrainage tube with pleural effusion 3 months after operation, the others were well. Conclusion The VATS is a mini-mal invasive procedure and the anatomical lobectomy using VATS is safe and efficient in benign lesions, the mediastinalnodes can be resected with minithoractomy in primary lung cancer but it is important to adhere to the indication, andlonger follow-up is also necessary. Uniportal Video-Assisted Thoracoscopic Right Upper Lobectomy with Systematic Lymphadenectomy

Abstract:Over the past decade, uniportal video-assisted thoracic surgery (VATS) has been reported to be a promising, less invasive alternative with potentially better cosmesis and less postoperative pain and paraesthesia. Although uniportal VATS has now evolved into a sophisticated technique capable of performing some of the most complex thoracic procedures, this approach to lobectomy is not standardized, and the surgical procedure still varies between surgeons. Here, we describe our uniportal VATS procedure during right upper lobectomy in a patient with a nodule in the right upper lobe. Subsequent mediastinal lymphadenectomy was performed to remove lymph nodes from the 2nd, 3rd, 4th, 7th, 8th, and 9th groups. Although there are some details that are different compared to the conventional VATS approach, as experience with uniportal VATS has grown, this approach is a viable alternative approach for lobectomy in selected patients.

Abstract:Objective This comparative study was carried out to define the advantages and disadvantages of the three methods of video-assisted lobectomy and systematic dissection of the mediastinal nodes, close distance two ports VATS, normal two ports VATS and three ports VATS. Methods Between Sep. 2012 and Sep. 2014 in Peking University First Hospital and Beijing Hospital, 319 patients who were diagnosed primary lung cancer and were going to be performed VATS lobectomy and systematic lymph node dissection, were selected, 265 patients were included in this study finally. 113 cases were involved in the group of close distance two ports VATS, 79 cases in normal two ports VATS and 73 cases in three ports VATS. The clinical data were gathered and statistically analyzed. Results No severe postoperative complications or death occurred in the three groups. No significant differences existed among the three groups in intraoperative blood loss, duration and volume of chest tube drainage, hospital stay after surgery, hospitalization expenses and complications (P>0.05). The data about operating time, number of dissected lymph nodes, postoperative pain score was significantly different [(241.5±56.8) min vs (112.5±0.8)min vs (119.1±12.6) min、(14.5±2.9) gold vs (22.4±6.6)gold vs (21.3±3.7)gold、(3.2±6.0) branch vs (7.8±7.1) branch vs (10.3±6.5) branch, P<0.05]. Conclusions The method of close distance two ports video-assistant lobectomy and systematic dissection of the mediastinal nodes is safe and practicable with definite therapeutic effect. Abstract:Video-assisted thoracoscopic surgery (VATS) for a relatively large mediastinal tumor (5.0–10.0 cm) remains controversial. In addition, few studies have focused on comparing different surgical approaches for large mediastinal tumors. Abstract:BACKGROUND:Lobectomy via video-assisted thoracoscopic surgery (VATS) is steadily becoming more commonplace. Concerns regarding oncologic effectiveness are focused on the ability to achieve complete lymphadenectomy using VATS technique. Effective VATS lymphadenectomy depends on a comprehensive understanding of the anatomy.METHODS:In this report, we would provide comprehensive surgical techniques on the management of complicate hilar or interlobar lymphadenectomy via VATS.RESULTS:Two cases of VATS lymphadenectomy were shown in this study. Various techniques have been adapted for lymphadenectomy with VATS. VATS lymphadenectomy is equivalent to dissection via thoracotomy when performed by experienced surgeons. Thoracic surgeons should keep in mind that to handle the sophisticated lymph node (LN) situation could be a tedious and rough job.CONCLUSIONS:Lymphadenectomy is a crucial element of staging for early stage non-small cell lung cancer patients. During VATS lobectomy, lymphadenectomy must be performed safely and effectively, with oncologic results comparable with those of open procedures.
Video-assisted Thoracic Surgery Experience of Calcified Lymph Nodes for Lingular Sparing Lobectomy.

Abstract:BACKGROUND:Video-assisted thoracic surgery (VATS) is commonly used for posterior, superior and lingular segmentectomy. Segmental resections involving the left upper lobe are the following: upper division (S1+2 and S3) (lingular sparing lobectomy), apicoposterior segmentectomy (S1 + S2), and lingulectomy (S4 + S5). Lingular sparing lobectomy is still a challenge for more technical demanding and more anatomic variations, especially when facing calcified lymph nodes.METHODS:A 73 years old woman was admitted for founding a ground glass opacity (GGO) during the screening test (1.0 cm × 1.0 cm). Her pulmonary function result was forced expiratory volume in 1 second (FEV1): 1.51 L (54.7% predicted). She was a non-smoker, with negative bronchoscopy findings. She received general anesthesia with double-lumen endotracheal intubation and right lung ventilation. Right lateral decubitus position was chosen. The first 1.5-cm incision was selected in the 8th intercostal space in the midaxillary line, and was used for the camera. A 4-cm long incision was made in the 4th intercostal space in the preaxillary line. A third 1.5-cm incision was performed in the 9th intercostal space in the postaxillary line for assistant. Pulmonary ligament and the entire left hilum were mobilized. The superior pulmonary vein has usually three major tributaries. The superior branch drains the apicoposterior segments and frequently blocks the access to the apicoposterior arteries. The middle branch drains the anterior segment, and the lowermost branch drains the lingula. The lingular vein must be preserved. The apicoposterior and anterior segment vein was transected with a vascular stapler. Anterior pulmonary artery and anterior bronchus were then divided and stapled. The upper lobe bronchus splits immediately into the lingular bronchus and a common stem. All these segmental bronchi have short course and a calcified lymph node located between the apicoposterior pulmonary artery and apicoposterior bronchus. These situations make the dissection and identification very difficult. Following many failure attempts of trying take the calcified lymph node out. Staple the left apicoposterior pulmonary artery together with the apicoposterior bronchi is completed. And left upper division (S1+2 and S3) was taken out after stapling lung tissue above the level of lingular segment with a 60-mm green linear stapler. Mediastinal lymph nodes of level 9, 7, 4L and 5 were cleared afterwards.RESULTS:Pathology was confirmed with adenocarcinoma (ancinar component dominant). There were no complications and the patient was discharged 6 days postoperatively.CONCLUSIONS:Staple the left apicoposterior pulmonary artery together with the apicoposterior bronchi is a safe and feasible way when facing the difficult dissection of the calcified lymph nodes during segmentectomy.
The Initial Experience of Video-assisted Thoracic Surgery Segmentectomy for Early Stage Lung Cancer

Abstract:Background and objective Segmentectomy can retains more healthy lung tissue than lobectomy, but it remains controversial in oncology for early stage lung cancer. The aim of this study is to discuss the problems of video-assisted thoracic surgery (VATS) segmentectomy in early stage lung cancer, by analyzing the clinical and patho-logical data of 35 cases and reviewing the literature. Methods There were 35 patients who received segmentectomy by complete video-assisted thoracic surgery, from May 2013 to July 2017, in single operation group in the Third Hospital of Peking University. We analyzed the patient's clinical and pathological data, intraoperative and postoperative com-plications, lymph node number and metastasis its situation, and compared postoperative pathology and preoperative computed tomography (CT) imaging type. In 35 cases of segmentectomy, there were 11 males and 24 females, with an average age of 57.7 years old. The lesions located in the right upper lobe were 8 cases, in the right lower lobe were 8 cases, in the left upper lobe were 13 cases, in the left lower lobe were 6 cases. The mean maximum diameter of CT imaging was 12.7 mm, and the largest diameter of hilar and mediastinal lymph nodes was less than 10 mm. 23 of them were ground glass predominating and 12 were solid components predominating. Results All 35 cases were successfully completed VATS anatomical segmentectomy. The average operation time was 153 minutes, the amount of bleeding was 51 mL. There were 10 cases of air leakage after operation, all of which were not more than 3 days. There was contralat-eral atelectasis in 1 case, chylothorax in 1 case. The average length of hospitalization was 6.1 days. There was no other complications outpatient related to surgery, in 30 days after discharge. The pathological changes were as follow, 2 cases of metastatic tumor, 8 cases of benign lung disease and 25 cases of primary lung cancer. In the 25 cases of primary lung cancer, there were 14 cases of invasive lung adenocarcinoma (7 cases were groundglassopacity (GGO) predominating in CT imaging), 4 cases of micro-invasive adenocarcinoma (3 cases were GGO predominating in CT imaging), 6 cases of adenocarcinoma in situ (all were pure GGO in CT imaging), 1 case of lung squamous cell carcinoma (mainly com-posed of solid in CT imaging). An average of 7.2 lymph nodes were removed in 25 cases of lung cancer, and all lymph nodes had no metastasis. Conclusion VATS anatomical segmentectomy is technically safe and reliable, and the indica-tions for lung cancer need to be strictly controlled. Its advantages still need to be confirmed by prospective randomized controlled trials.
