Abstract:MicroRNAs (miRNAs) are evolutionarily conserved small non-protein coding RNA transcripts that, typically, negatively regulate gene expression at a post-transcriptional level. miRNAs exert these effects on diverse physiological functions in development and homeostasis in a variety of organisms including Homo sapiens. Deregulation of endogenous miRNAs has been found in the pathogenesis of diseases, particularly in cancer. Extensive miRNAs-association studies in cancer have characterized unique 'oncomiRs' (e.g. miR-21, miR-155, miR-17-92 and C19MC miRNAs), 'TS-miRNAs' (tumor suppressor miRNAs e.g. miR-15a/16-1, let-7), 'metasta-miRNAs' (metastasis regulatory miRNAs e.g. miR-10a, miR-34) and 'epi-miRNAs' (epigenetically regulated miRNAs e.g. miR-29a/b/c) in cancer development, progression and metastasis. These findings have inspired the cancer research community to utilize "fingerprint-miRNAs" to aid in cancer diagnosis and prognosis and it has emerged as an exciting new approaches for potential alternative cancer therapy. Restoration of miRNAs in cancer cells with either miRNA mimics or antagomiRs has demonstrated an effective approach to change the cancer cell fate in numerous cultured systems and a few animal models. Due to the nature of miRNAs targeting multiple genes and miRNA-transcription factor co-regulation of gene expression networks, modulation of one or a subgroup of miRNAs could reprogram whole regulatory networks as reflected in multiple core pathways, such as proliferation and apoptosis, differentiation, cell adhesion and invasion as well as epigenomic and genomic stability. In this chapter, we will discuss the miRNA regulation in the genome, highlight the progress of miRNA-based exploratory cancer treatment, and underscore the challenges of developing "miRNA-drugs" with network perturbation.
