Abstract:Objective To explore the correlation between serum uric acid levels and uric acid excretion indexes in children with primary nephrot?ic syndrome(PNS),and to reveal the prevalence of hyperuricemia and its possible causes in PNS. Methods The clinical data in 74 cases of pe?diatric PNS were retrospectively analyzed. These patients were grouped as hyperuric acid(HUA)or non?hyperuric acid(NUA),and their clinical features and laboratory findings were compared between two groups,including age,serum uric acid,serum creatinine,urea,cystatin C,triglycer?ide,cholesterol,albumin,creatinine clearance rate,24 hours urine uric acid,serum uric acid/serum creatinine,uric acid clearance,and the frac?tional excretion of uric acid and other indicators of differences. In addition ,a correlation analysis was carried out for the excretion of uric acid index. Results Several factors were lower in HUA group than that of NUA group ,such as the serum albumin and fractional excretion of uric acid(P1=0.034,P2=0.025);while the serum cystatin C and serum uric acid/serum creatinine ratio(P1=0.038,P2=0.001)was higher in HUA group. The uric acid excretion was negatively correlated with serum uric acid levels in all 74 children with PNS(r=-0.43,P=0.016),and the serum uric acid/creatinine ratio was positively correlated with serum uric acid(r=0.486,P=0.001). Conclusion Both the increase of blood uric acid production and excretion reduction were observed in children with PNS ,and the serum uric acid levels and renal tubular function is closely related.
Hyperuricemia and Associated Factors in Children with Chronic Kidney Disease: A Cross-Sectional Study

Abstract:Background: Hyperuricemia is increasingly recognized as a risk factor for chronic kidney disease (CKD) just in adults. The purpose of this study was to investigate the clinical characteristics of hyperuricemia and its associated factors in Chinese children with CKD at a single center. Methods: A cross-sectional study of 170 CKD children collected from the Department of Nephrology, The Zhejiang University Children’s Hospital was conducted. The clinical data, including anthropometric data, blood pressure measurements, and biochemical parameters, were recorded and analyzed retrospectively. The factors associated with hyperuricemia in CKD children were evaluated by Pearson and Spearman correlation analysis and multiple logistic regression analysis. Results: The mean age was 9.79 ± 4.10 years, and 72 (42.35%) were girls. Higher blood urea nitrogen (BUN), serum creatinine, cystatin C, D-dimer, lower hemoglobin, albumin, and estimated glomerular filtration rate (eGFR) were significantly associated with higher serum uric acid (SUA). In multiple logistic regression analysis, anemia and higher BUN were both positively associated factors, whereas eGFR ≥ 90 mL/min/1.73 m2 was a negatively associated factor for subjects with SUA ≥ 390 μmol/L (6.5 mg/dL). Conclusions: SUA was significantly associated with kidney risk factors in CKD children. Monitoring and controlling SUA, Hb, BUN, and Scr levels in CKD children may help to prevent the progression of CKD.
Hyperuricemia in Childhood Primary Hypertension

Abstract:Experimental animal models suggest that uric acid might have a pathogenic role in the early development of primary hypertension. We hypothesized that serum uric acid is correlated with blood pressure in children with new-onset, untreated, primary hypertension. We evaluated 125 consecutive children referred to the Baylor Pediatric Renal Program for evaluation of hypertension. None of the subjects had previously been evaluated or treated for hypertension. The children ranged in age from 6 to 18 years (mean, 13.4±3.3) and had normal renal function (creatinine clearance >80 mL · min −1 · 1.73 m −2 ). Sixty-three children had primary hypertension, 40 had secondary hypertension, and 22 had white-coat hypertension. Forty controls with normal blood pressure were recruited from the renal clinic. Uric acid levels were directly correlated with systolic ( r =0.80, P =0.0002) and diastolic ( r =0.66, P =0.0006) blood pressure in controls and in subjects with primary hypertension and were independent of renal function. Serum uric acid concentrations >5.5 mg/dL were found in 89% of subjects with primary hypertension, in 30% with secondary hypertension, in 0% with white-coat hypertension, and in 0% of controls. We conclude that serum uric acid is directly correlated with blood pressure in untreated children and that a serum uric acid value >5.5 mg/dL in an adolescent being evaluated for hypertension strongly suggests primary hypertension as opposed to white-coat or secondary hypertension. These results are consistent with the hypothesis that uric acid might have a role in the early pathogenesis of primary hypertension.

Elevated Interleukin 1β and Interleukin 6 Levels in the Serum of Children With Hyperuricemia.

Abstract:Purposes The aim of this study was to investigate the serum levels and clinical significance of interleukin 1 (IL-1) and IL-6 in children with hyperuricemia (HUA). Methods We included 71 children with HUA and 71 children with no HUA as control subjects. Children with HUA were divided into groups I and II according to whether they had a history of acute gout-like attacks (including sudden monoarthritis of rapid onset with intense pain and swelling). Group I was examined twice (A, in the acute phase; B, in the remission phase). Serum IL-1 and IL-6 levels were measured by enzyme-linked immunosorbent assay. Results Serum IL-1 and IL-6 levels were increased in children with HUA and were overall statistically different from the control group (P < 0.05, respectively). Serum IL-1 and IL-6 were significantly higher in group IA in comparison with group IB, group II, and control subjects (P < 0.05, respectively), as well as in groups IB and II compared with control subjects (P < 0.05, respectively). In group IB, the serum IL-1 and IL-6 concentrations were higher than those in group II, but there were no statistical differences (P > 0.05). In addition, in children with HUA, serum IL-1 and IL-6 levels were positively associated with white blood cell count, neutrophil count, monocyte count, uric acid levels, erythrocyte sedimentation rate, C-reactive protein, blood urea nitrogen, and serum creatinine levels (all P < 0.05), but were not associated with triglycerides, total cholesterol, low-density lipoprotein cholesterol, or high-density lipoprotein cholesterol levels (all P > 0.05). Conclusion IL-1 and IL-6 levels are increased in children with hyperuricemia, even if they have not had acute gout. Further studies are necessary to fully characterize the significance of IL-1 and IL-6 found in HUA children, and whether they could be correlated with long-term prognosis.

Abstract:Recent evidence suggests that hyperuricemia is an important condition in children and adolescents, particularly in association with noncommunicable diseases. This review aims to summarize our current understanding of this condition in pediatric patients. An analysis of serum uric acid reference values in a healthy population indicates that they increase gradually with age until adolescence, with differences between the sexes arising at about 12 years of age. This information should be taken into consideration when defining hyperuricemia in studies. Gout is extremely rare in children and adolescents, and most patients with gout have an underlying disease. The major causes of hyperuricemia are chronic conditions, including Down syndrome, metabolic or genetic disease, and congenital heart disease, and acute conditions, including gastroenteritis, bronchial asthma (hypoxia), malignant disorders, and drug side effects. The mechanisms underlying the associations between these diseases and hyperuricemia are discussed, together with recent genetic information. Obesity is a major cause of hyperuricemia in otherwise healthy children and adolescents. Obesity is often accompanied by metabolic syndrome; hyperuricemia in obese children and adolescents is associated with the components of metabolic syndrome and noncommunicable diseases, including hypertension, insulin resistance, dyslipidemia, and chronic kidney disease. Finally, strategies for the treatment of hyperuricemia, including lifestyle intervention and drug administration, are presented.

Relationship between renal pathology and clinical manifestations in children with Henoch-Schonlein purpura nephritis

Abstract:Objective To investigate the correlations between the clinical manifestations based on pathologic grades and renal pathological features of Henoch-Schonlein purpura nephritis(HSPN) in children. Methods The clinical data of 77 patients with HSPN in the Department of Nephrology, Anhui Provincial Children's Hospital from Ja-nuary 2004 to March 2014 were retrospectively analyzed.The relationship between clinical manifestation and pathological features was analyzed. Results Among the 77 patients, 21 cases(27.3%) had both abdominal symptoms, and arthritis was reported in 15 cases(19.5%), 28 cases(36.4%) had abdominal symptoms and arthritis, and 13 cases(16.9%) had no such symptoms.Hematuria and proteinuria were the most common clinical types [48.1%(37/77 cases)], followed by simple hematuria or proteinuria[27.3%(21/77 cases)], nephrotic syndrome[23.4%(18/77 cases)], and chronic nephritis[1.3%(1/77 cases)]. The major of pathological changes in HSPN were grade Ⅱ[46.8%(36/77 cases)] and grade Ⅲ[45.5%(35/77 cases)], the minority of them were grade Ⅰ[6.5%(5/77 cases)] and grade Ⅳ[1.3%(1/77 cases)] .The severity of urine protein was positively associated with pathologic classification(rs=0.472, P=0.000). According to the glomerular deposition of immune complex, there were 6 types.The percen-tage of deposition of IgA+ IgM was 62.3%(48/77 cases), IgA+ IgG+ IgM was 19.5%(15/77 cases), IgA 14.3%(11/77 cases), that of IgA+ IgG 1.3%(1/77 cases), and the IgM 1.3%(1/77 cases), no Ig 1.3%(1/77 cases). In these cases, 76.6%(59/77 cases) had complements C3 deposition; pathologic stage characterized by Ⅲ level and above were common[54.2%(32/59 cases)], Ⅱ level 42.2%(25/29 cases), Ⅰ level 3.4% (2/59 cases). Among the different types of immune complex depositions, there was no statistically significant difference in pathological types of distribution, while the clinical type and complements C3 deposition were significantly associated with pathologic classification(rs=0.361, P=0.001). Sixty-two cases were rated as level 1 (80.5%), and 15 cases was level 2 (19.5%); in different clinical group, rating in glomeruli was statistically different(χ2=17.2, P=0.004). Renal tubular interstitial rating of all the patients were level 1 (100%). Conclusions The severity of urine protein, complements C3 deposition is associated with pathologic classification.Pathologic classification can basically reflect the renal damage in HSPN. Abstract:To estimate the prevalence of hyperuricemia and its major risk factors among Chinese preschool children. A cross-sectional survey was carried out in six central urban districts of Tianjin from March to June 2015. A cluster sampling was employed to obtain a random sample of preschool children. A total of 4073 children aged from 3 to 6 years participated in this survey. Logistic regression was used to obtain odds ratios and 95% confidence intervals. The overall mean serum uric acid concentrations was 243.0 ± 53.2 μmol/L, corresponding to a mean concentrations of 247.3 ± 53.7 μmol/L among boys and 238.3 ± 52.4 μmol/L among girls. The overall prevalence of serum uric acid ≥ 310 μmol/L among children was 10.1%. Boys, obesity, diastolic blood pressure, and serum triglyceride concentrations were associated with the increased risk of hyperuricemia in univariable models, although the statistically significant association between diastolic blood pressure, serum triglyceride concentrations and hyperuricemia disappeared in multivariable models. The prevalence of hyperuricemia among children aged from 3 to 6 years was relatively high. Several metabolic syndrome components were associated with the risk of hyperuricemia.

Related factors of serum uric acid in patients with primary hypertension and hyperhomocysteinemia

Abstract:Objective: To investigate the related factors of serum uric acid in patients with primary hypertension and hyperhomocysteinemia. Methods: One hundred and ten patients with primary hypertension and hyperhomocysteinemia (homocysteine levels >10 μmol/L) were enrolled into this study, ages from 18 years to 75 years. They were divided into the normal serum uric acid group which contained 74 cases patients (41 cases of male and 33 cases of female) and the hyperuricemia group which contained 36 cases patients (20 cases of male and 16 cases of female). Plasma concentrations of homocysteine, serum uric acid, serum folic acid, blood sugar, triglyceride, total cholesterol, serum low density lipoprotein cholesterol, serum high density lipoprotein cholesterol, blood urea nitrogen, and creatinine were detected in these patients, and the deference of them between the two groups was compared. And then the risk factors of serum uric acid with univariate analysis and multivariate analysis by logistic regression analysis were analyzed. Results: The result of multivariate analysis showed that the incidence of serum uric acid in patients with primary hypertension and hyperhomocysteinemia had significant relationships with systolic blood pressure (OR [odds ratio]: 1.132, 95%CI [confidence interval]: 1.003~1.290, p = 0.043), diastolic blood pressure (OR: 1.353 95%CI: 1.023~1.789, p = 0.034, homocysteine (OR: 1.264, 95%CI: 1.016~1.573, p = 0.035), triglyceride (OR: 9.726, 95%CI: 1.288~73.466, p = 0.027), and creatinine (OR: 1.031, 95%CI: 1.005~1.508, p = 0.018). Conclusion: The indices of systolic blood pressure, diastolic blood pressure, homocysteine, triglyceride, and creatinine were important risk factors of serum uric acid in patients with primary hypertension and hyperhomocysteinemia. It is of great significance to measure multiple risk factors in patients with primary hypertension and hyperhomocysteinemia.
Prevalence and Related Factors of Hyperuricaemia in Chinese Children and Adolescents: a Pooled Analysis of 11 Population-Based Studies

Abstract:BACKGROUND AND AIMS:Hyperuricaemia can lead to gout and is associated with an increased risk of cardiometabolic disease. We aimed to investigate the prevalence of hyperuricaemia and its related factors in Chinese children and adolescents. METHODS:We pooled data from 11 population-based studies comprising 54,580 participants aged 3-19 years. The sex- and age-standardized prevalence of hyperuricaemia was estimated overall and by sex, age, weight status, geographic region and survey year. RESULTS:Serum uric acid (SUA) increased gradually from 3 to 11 years with no significant sex difference, and then increased dramatically during 11-15 years. The estimated overall prevalence of hyperuricaemia was 23.3% (26.6% in boys and 19.8% in girls, p < .001). The prevalence increased with growing age (3.7, 9.8, 15.8, 35.5 and 31.7% among children aged 3-5, 6-8, 9-11, 12-15 and 16-19 years, respectively, p for trend < .001) and with increasing weight status (18.2, 37.6, 50.6 and 64.5% among children with non-overweight, overweight, obesity and extreme obesity, respectively, p for trend < .001). The prevalence was higher in North than in South (24.2 vs. 19.7%, p < .001), and increased markedly from 16.7% during 2009-2015 to 24.8% during 2016-2019. In multivariable regression analyses, sex, age, obesity, region and survey year were independently associated with odds of hyperuricaemia. CONCLUSIONS:The prevalence of hyperuricaemia in Chinese children and adolescents is unexpectedly high. The findings suggest an urgent need to implement effective interventions to reduce risk of hyperuricaemia in Chinese youths.KEY MESSAGESQuestion: What is the prevalence of hyperuricaemia in Chinese children and adolescents?Findings: In this large pooled cross-sectional study comprising >50,000 children and adolescents aged 3-19 years, we found that the prevalence of hyperuricaemia was high in overall population and subgroups of sex, age, obesity, region and survey year.Meaning: Our findings indicate that hyperuricaemia is an important health problem in Chinese children and adolescents, and effective intervention strategies are needed to reduce its burden.
Clinical Significance of Serum Interleukin-1Β and Interleukin-6 Levels in Childhood Hyperuricemia

Abstract:Objective To explore the significance of interleukin (IL)-1β and IL-6 in serum of children with hyperuricemia (HUA). Methods 142 children including 71 children with HUA (HUA group) and 71 control children (control group), healthy and inguinal hernia children were selected as control group. 71 HUA children were subdivided into GA (gout attacks) group (n=28) and NGA (non-gout attacks) group (n=43) according to whether they had a history of acute gout attacks, including sudden monoarthritis of rapid onset with intense pain and swelling or without. Enzyme-linked immunosorbent assay was used to measure the level of IL-1β and IL-6 in serum. Results Serum IL-1β and IL-6 levels of HUA children were significantly higher than those of control group (all P<0.05). Serum IL-1β and IL-6 levels of HUA children in GA group were significantly higher than those of NGA group (P<0.05). Serum IL-1β and IL-6 levels of GA group in acute phase was significantly higher than those of HUA children in remission stage、NGA group and control group (P<0.05). Serum IL-1β and IL-6 levels of GA group in remission stage and NGA group was significantly higher than those of control group (P<0.05). There were no significant differences between HUA children in remission stage and NGA group (P>0.05). The serum IL-1β and IL-6 levels of HUA children were positively correlated with WBC, neutrophils, monocytes, uric acid, ESR, CRP, BUN and Cr (all P<0.05), while not correlated with triglyceride, total cholesterol, LDL-C and HDL-C(all P<0.05). Conclusion IL-1β and IL-6 play an important role in the pathogenesis of HUA in children.
Clinical significance of serum D-dimer and uric microalbumin in children with Henoch-Schonlein purpura

Abstract:Objective This study was designed to investigate the correlations between serum D-dimer and wric microalbumin(mAlb) in children with Henoch-Schonlein purpura(HSP) and to provide a reliable basis for early anticoagulant therapy of HSP children.Methods 84 HSP children were included in the study,30 healthy children were considered as the control group.Serum D-dimer,uric mAlb and α1-M were analyzed.Results The positive rate of serum D-dimer in HSP children was 64.3%,the positive rate of the differences were and α1-M were 59.5% and 22.6%,respectively,compared with the control group,statistically significant(P<0.01).Serum D-dimer,uric mAlb and α1-M of abnormal routine urianlysis group and normal routine urianlysis group,compared with the control group,the differences were statistically significant(P<0.05).53 of 54 children with abnormal serum D-dimer were abnormal uric mAlb and α1-M.1 of 30 children with normal serum D-dimer was abnormal uric mAlb and α1-M.Conclusions HSP children with hypercoagulable state complicated by the body kidney damage.Giving anticoagulant therapy in early stage may be helpful to prevent the occurrence of allergic purpura nephritis.

Abstract:OBJECTIVE: The present study examined the association between uric acid levels and high blood pressure in a multiethnic study of Chinese children and adolescents.METHODS: The participants were divided into four different groups according to the uric acid quartiles. Three logistic regression models were conducted to investigate the relationship between the high blood pressure and uric acid levels. Model 1 adjusted age, sex and ethnicity. Model 2 adjusted age, sex, ethnicity, total cholesterol, high-density lipoprotein, low-density lipoprotein, triglycerides, estimated glomerular filtration rate, fasting glucose and waist circumference. Model 3 adjusted all the confounding factors in model 2 except the waist circumference and BMI. The concentrations of uric acid in high blood pressure participants and normotensive participants were compared with or without adjustment for confounding factors.RESULTS: A total of 3778 participants aged 10-15 years from the Xinjiang Congenital Heart Disease Survey were included in the present study. The percentages of the high blood pressure in the four different uric acid quartiles were 7.4, 8.6, 9.6 and 11.8%, respectively. In model 1, 2 and 3 of the logistic regression, the participants in the third and fourth uric acid quartiles had significantly higher chance of suffering the high blood pressure when compared with the participants in the first uric acid quartile [odds ratio 1.608, 1.587, 1.597, P = 0.005, 0.015, 0.015, respectively, between participants in the first quartile and the third quartile; odds ratio 1.981, 1.945, 1.810, P = 0.001, 0.002, 0.007, respectively, between participants in the first quartile and the fourth quartile). The concentrations of serum uric acid were 220.7 μmol/l in high blood pressure participants and 204.1 μmol/l in normotensive participants (P = 0.024). After adjustment for confounding factors, the concentrations of serum uric acid were 219.7 vs. 204.5 μmol/l in one model (P < 0.001) and 219.3 vs. 204.5 μmol/l in the other model (P < 0.001).CONCLUSIONS: Among Chinese children and adolescents, increasing levels of serum uric acid are associated with high blood pressure.

Abstract:Objective: To investigate the correlations between the level of serum uric acid and renal injury in adults. Methods: Data from 34 212 medical subjects in Songjiang District of Shanghai were collected during 2006 to 2011, and they were divided into different groups based on the gender, urine detection results, renal function and levels of serum uric acid. Epidemiological characteristics of serum uric acid was analyzed. Proteinuria, hematuria and creatinine clearance were compared between different groups. Results: The overall morbidity of hyperuricemia was 18.67% (male 26.82% and female 9.99%). The mean level of serum uric acid in male group was (377.64±79.23) μmol/L, higher than that in female group of (273.87±66.02) μmol/L (P<0.01). There was a positive correlation between ages and the mean level of serum uric acid both in male and in female groups (r=0.22, P<0.01). Hematuria was more common in female group, while proteinuria was more common in male group (P<0.01). The level of serum uric acid was positively related with the level of serum creatinine, and negatively related with the level of creatinine clearance (P<0.01). With the increasing level of serum uric acid, there was no significant difference in morbidity of hematuria, especially in male group (P=0.65), while there was significant increasing trend in morbidity of proteinuria (P<0.01). After logistic regression analysis, hyperuricemia was thought as an independent risk factor for proteinuria and chronic renal dysfunction. Conclusions: The level of serum uric acid is related to the occurrence of proteinuria in medical patients. The increasing level of serum uric acid can aggravate kidney injury.

Abstract:OBJECTIVES: Hyperuricemia has been shown to be a strong correlate of hypertension in children. However, the complex interaction between serum uric acid (UA), systemic blood pressure (BP), and possibly confounding factors has been elucidated only in part. METHODS: We evaluated office BP as well as clinical and biohumoral parameters in a cross-sectional cohort of 501 children (280 boys and 221 girls) aged between 6 and 18 years (mean = 10.8 years) consecutively referred for cardiovascular risk assessment. RESULTS: Overall, 156 (31.1%) were normotensive, 122 (24.4%) showed transient hypertension, 87 (17.4%) had prehypertension, and 136 (27.1%) had hypertension. Altogether 33.3% and 40.5% of the study group were overweight or obese, respectively. There was a trend toward greater weight and waist circumference and higher BMI, Homeostasis Model Assessment index, and UA levels as the BP categories rose. Moreover, the prevalence of pubertal children, obesity, and waist-to-height ratio above 0.50 progressively increased from lower to upper BP categories. After adjusting for puberty, gender, BMI (z-score), Homeostasis Model Assessment index, and renal function, UA was found to be directly related to systolic and diastolic BP values (P = .03). Using normotensive children for comparison, the risk of showing prehypertension or hypertension increased by at least 50% for each 1 mg/dL UA increase (P < .01), whereas it doubled for children in the top gender-specific UA quartile (P < .03). CONCLUSIONS: Increased UA levels showed an independent predictive power for the presence of higher BP levels among a cohort of children at relatively high cardiovascular risk.

Preliminary Research on Syndrome Types of Chinese Medicine in Children with Primary Nephrotic Syndrome

Abstract:Objective: To provide an objective reference for the syndrome types of Chinese medicine (CM) associated with pediatric primary nephrotic syndrome (PNS). Methods: A cross-sectional study was performed. Data on clinical symptoms, CM syndrome types, biochemical indices, and medications used were collected from 98 children with PNS. Then, the correlation between CM syndromes and biochemical indices, as well as medications used, was analyzed. Results: The four most common symptoms in children with PNS were brown urine, red tongue, excessive sweating, and swelling of the face and limbs. The syndromes of qi deficiency of Fei (Lung) and Shen (Kidney) (FSQD) and yin deficiency of Gan (Liver) and Shen (GSYD) were the most common main CM syndrome types. FSQD syndrome score correlated significantly with the total cholesterol level, urine protein/creatinine ratio, and urine IgG and albumin levels (P<0.01 or P<0.05). The use of maintenance glucocorticoids combined with immunosuppressive agents correlated with FSQD syndrome, and the use of maintenance glucocorticoids alone correlated with GSYD syndrome (P<0.05). Conclusion: Two of the most common CM syndrome types were FSQD and GSYD syndromes. FSQD syndrome may be caused by some factors related to lipid levels, protein loss, and the use of immunosuppressive agents. The use of maintenance glucocorticoids may cause GSYD syndrome.

Abstract:Introduction There are various factors responsible for hyperuricemia in children, however, there is extremely limited local data available. In this study, we aim to determine the causes and risk factors associated with hyperuricemia. This study will assist pediatric consultants to identify children who might be at risk of hyperuricemia and manage them accordingly. Methods This study was conducted in pediatric outpatient departments of various tertiary care hospitals from January 2018 to December 2019. Five thousand (n = 5000) children of either gender between the age group of 1-14 years, were enrolled in the study after informed consent from their parents. Uric acid levels were assessed by using the UASure blood uric acid monitoring handheld device. Results In our study, n = 1301 (26.02%) children had hyperuricemia. Hyperuricemia was more common in male compared to females (65.49% vs. 34.51%; p-value <0.00001) and in older children (9 ± 2 years vs. 7 ± 3; p-value <0.00001). In hyperuricemia patients, the most common disorder was gastroenteritis (23.98%), followed by respiratory infection (23.14%) and asthma (15.45%). Conclusion Hyperuricemia in children is very prevalent in the local setting. Patients with pre-existing conditions like congenital heart disease, asthma, epilepsy, and cancers should routinely be screened for hyperuricemia and managed accordingly to avoid long-term complications associated with hyperuricemia.
