The Expression of Peroxisome Proliferator-Activated Receptor-Α and Β in Hypertensive Atrial Fibrillation

Abstract:Objective: Atrial fibrillation (AF) is the most common heart rhythm disorder. The prevalence of AF is increased with increasing age. Evidence has suggested that either hypertension or atrial fibrillation (AF) is associated with systemic inflammation. Peroxisome proliferator-activated receptor-α and β (PPARα and PPARβ) and peroxisome proliferator-activated receptor have been proven to have anti-inflammatory effects and are implicated as a molecular pathway involved in many cardiovascular diseases. We aimed to evaluate the expression of peroxisome proliferator-activated receptor-α and β in hypertensive atrial fibrillation. The correlation between inflammation and AF is still unknown. Methods: Using a case–control study design, 103 patients with hypertensive AF (persistent AF: 55, paroxysmal AF: 48) were included into the study groups. A total of 50 age-matched patients with hypertension, but without AF were selected as the control group. The expressions of PPARα and PPARβ, interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) mRNA in monocytes were detected by using a real time polymerase chain reaction (real time PCR). High sensitive C-reactive protein (hsCRP) and interleukin-1 (IL-1) were measured by immunoenzymetric methods. Results: The PPARα mRNA was markedly decreased in the hypertensive AF group as compared with the hypertensive non-AF group, and it was significantly lower in persistent AF than paroxysmal AF (1.33709±0.17124 vs 1.08839±0.23344 vs 0.84717±0.22419, P<0.001). PPARβmRNA was not statistically significant between non-AF group, paroxysmal AF and persistent AF. TNF-α mRNA (0.60706±0.19676 vs 0.85042±0.17383 vs 1.07765±0.23045, P<0.001), IL-6 mRNA (0.72189±0.22462 vs 0.93440±0.22896 vs 1.13771±0.23025, P<0.001), IL-1 (128.81±29.96 vs 211.19±64.91 vs 334.33±83.01, P<0.001) and hsCRP (3696.10±290.28 vs 4169.94±287.95 vs 4904.68±279.85, P<0.001) were increased in patients with hypertensive AF compared to the non-AF group and it was even higher in persistent AF than in paroxysmal AF. TNF-α, IL-6, hsCRP and IL-1 were in negative correlation with PPARγ, the correlation coefficients were −0.606, −0.532, −0.416, −0.513, respectively (P<0.001). Conclusions: In hypertensive patients, increased inflammatory cytokines were associated with increased incidence of AF and atrial remodeling; PPARα may be involved in the pathogenesis of AF by regulation of inflammation.
