Abstract:A low high-density lipoprotein cholesterol (HDL-C) level is an identified risk factor for cardiovascular diseases. However, results on the association between HDL-C levels and adverse outcomes in diabetic status still remain limited and controversial. Herein, we evaluated the association between HDL-C levels and adverse outcomes among acute ischemic stroke (AIS) patients with diabetes mellitus. The cohort comprised 3824 AIS patients with diabetes mellitus (62.7 ± 10.5 years; 34.2% women) from the Third China National Stroke Registry (n = 15,166). Patients were classified into five groups by quintiles of HDL-C. The outcomes included recurrent stroke and major adverse cardiovascular events (MACEs) within 1 year. The relationship between HDL-C levels and the risk of adverse outcomes was analyzed by Cox proportional hazards models. Patients in the lowest quintile of HDL-C had a higher risk of recurrent stroke (hazard ratio (HR) 1.59, 95% confidence interval (CI), 1.12–2.25) and MACEs (HR 1.53, 95% CI, 1.09–2.15) during 1-year follow-up compared with those in the highest quintile of HDL-C. There were linear associations between HDL-C levels and the risks of both recurrent stroke and MACEs. Low HDL-C levels were associated with higher risks of recurrent stroke and MACEs within 1 year in AIS patients with diabetes mellitus.

Abstract:Objective To explore potential main risk factors inducing cerebral hemorrhagic stroke in human from lipometabolism, free radicals reactions, oxidatation and lipoperoxidation. Methods Using a random control design 500 cases of cerebral hemorrhagic patients (CHP) in the acute phase of the disease and 100 healthy adult volunteers (HAV) were investigated by means of contents of cholesterol (CH), lipoperoxides (LPO), nitric oxide (NO), vitamin C (VC), vitamin E (VE) and β － carotene (β － CAR), and activity of superoxide dismutase (SOD) in plasma as well as activities of SOD, catalase (CAT) and glutathione peroxidase (GSH－ Px) in erythrocytes (RBC), and contents of CH and LPO in RBC with spectrophotometric assays. Results Compared with the average values (AV) of every biochemical parameters (BP) in the HAV group, the AV of the CH in RBC and in plasma, and VC, VE, β － CAR and SOD in plasma as well as SOD, CAT and GSH－ Px in RBC in the CHP group were significantly decreased (P=0.0000), while the AV of the LPO in RBC and in plasma, and NO in plasma in the CHP group were significantly increased (P=0.0000). The findings of linear regression and correlation as well as stepwise regression and correlation analysis for 500 CHP showed that there was a significant linear correlation in the each BP with both clinical neurotic functional defective scales (NDS) and intracranial hemorrhagic sizes (IHS) respectively, and that the closest correlation indwelled among the NDS and the values of CH and LPO in RBC as well as NO and VC in plasma, while the closest correlation indwelled among the IHS and the values of CH and LPO in RBC as well as NO and VE in plasma. Conclusion These findings in this study suggested that marked low RBC cholesterol and marked high RBC lipoperoxides were potential main risk factors inducing cerebral hemorrhagic stroke in human. Early measurement of the values of cholesterol and lipoperoxides in RBC for cerebral vascular disease patients which connected with test of hypertension in the patients might help to diagnosis of the cerebral vascular disease.

Abstract:This study aimed to explore the association between serum non-high-density lipoprotein cholesterol (non-HDL-C) and cognitive dysfunction risk in patients with acute ischemic stroke (AIS). This cross-sectional study enrolled 583 AIS patients. Biochemical markers and lipid profile were collected after admission. AIS patients were classified into high group (non-HDL-C ≥3.4 mM) and normal group (non-HDL-C <3.4 mM). Mini-Mental State Examination scale (MMSE), Montreal Cognitive Assessment scale (MoCA), Activities of Daily Living (ADL) scale, Neuropsychiatric Inventory (NPI), and Hamilton Depression scale 21 version (HAMD-21) were applied on the third day after admission. Compared with the control group, patients of the high group had higher body mass index and higher frequency of intracranial artery stenosis, and exhibited higher levels of non-HDL-C, total cholesterol, triglycerides, low-density lipoprotein cholesterol, homocysteine, fasting blood glucose, and glycosylated hemoglobin (HbA1c), and lower levels of high-density lipoprotein cholesterol (all P<0.05). Compared with the control group, patients of the high group had significantly lower MMSE and MoCA scores (MMSE: 26.01±4.17 vs 23.12±4.73, P<0.001; MoCA: 22.28±5.28 vs 20.25±5.87, P<0.001) and higher scores on the NPI and HAMD-21 (both P<0.001). MMSE (r=-0.306, P<0.001) and MoCA scores (r=-0.251, P<0.001) were negatively associated with non-HDL-C level. Multivariate regression analysis revealed that non-HDL-C level (OR=1.361, 95%CI: 1.059-1.729, P=0.016) was independently associated with the presence of cognitive dysfunction after adjusting for confounding factors. High serum non-HDL-C level might significantly increase the risk of cognitive dysfunction after AIS.

Abstract:Background: Diabetes mellitus (DM) was prone to happening in posterior circulation infarction (POCI) and DM also has the impact on the lipids, our study was to investigate the correlation between lipid compositions and POCI. Methods: Data was collected from the patients with acute ischemic stroke (AIS) hospitalization in Affiliated Drum Tower Hospital of Nanjing University Medical School from October 2008 to May 2012. Lipids and other risk factors in the different populations were investigated in relation to occurrence of POCI based on the infarction location. Results: Six hundred ten patients with AIS were included in this study, which had 428 with anterior circulation infarction (ACI) and 182 with POCI. Elevated Triglyceride (TG) and decreased High density lipoprotein cholesterol (HDL-C) were seen in the POCI of total populations and AIS without DM compared to the ACI, but not in the populations of AIS with DM, so did the elevated TG/HDL-C ratios. Also, the percent of low HDL-C level and high TG level were higher in POCI group than that in ACI group. Furthermore, single factors logistic regression demonstrated that TG, HDL-C and TG/HDL-C ratio were correlated to the POCI whatever in the total populations or AIS without DM, but this kind of trend just maintained in the populations of AIS without DM after adjusting by relative interference factors. Conclusion: Dyslipidaemia was prone to happening in POCI compared to ACI in the non-diabetic populations, which was correlated to the pathogenesis of POCI.

Abstract:Objective To evaluate the association of lipids and the risk of ischemic stroke and its subtypes. Methods We evaluated the levels of total cholesterol(TC),triglycerides(TG),low density lipoprotein(LDL) and high density lipoproteins(HDL) in 516 patients with ischemic stroke and 131 controls. With TOAST criteria,we also evaluated lipids in stroke subtypes. Results Compared with controls,the HDL level was significantly lower and the TC/HDL ratio was significantly higher in stroke group. The differences among other components of lipids were not significant. The level of HDL was significantly lower in large artery atherosclerosis (LAA),cardioaortic embolism (CE) and small artery occlusion (SAO). The TC/HDL ratio in them was significantly higher than that of controls. Conclusion The reduced protection resulted from the lower level of HDL may be one of the reasons for the attack of ischemic stroke. The TC/HDL ratio can be taken as one of the measures for the interrelationship between abnormal lipids and ischemic stroke. Further researches on lipids and stroke should be conducted with a larger pool of samples on the basis of stroke classifications.

Abstract:BackgroundNon-high density lipoprotein cholesterol (HDL-C) could be a good predictor of vascular disease outcomes. To evaluate the association between serum non-HDL-C and cognitive impairment in patients with acute ischemic stroke.MethodsA total of 725 hospitalized patients with acute ischemic stroke were enrolled. They received conventional treatment. Cognitive function was assessed on the 3rd day after admission using mini-mental state examination (MMSE), Montreal Cognitive Assessment (MoCA), Activity of Daily Living Scale (ADL), and Neuropsychiatric Inventory (NPI, and Hamilton depression rating scale 21-item (HAMD-21). Lipid profile and biochemical markers were measured, and non-HDL-C was calculated.ResultsCompared with patients with normal non-HDL-C, those with high non-HDL-C showed lower MMSE (23.1 ± 4.9 vs. 26.0 ± 4.6, P < 0.001) and MoCA (20.4 ± 6.4 vs. 22.2 ± 5.3 P = 0.01) scores, higher NPI (6.2 ± 1.2 vs. 3.3 ± 1.5, P < 0.001) and HADM-21 (6.0 ± 2.2 vs. 4.5 ± 1.9, P < 0.001) scores, and higher homocysteine (16.0 ± 3.8 vs. 14.3 ± 2.0 mmol/L, P = 0.044), fasting blood glucose (6.4 ± 2.7 vs. 6.1 ± 2.1 mmol/L, P = 0.041), and HbA1c (6.80 ± 1.32 % vs. 6.52 ± 1.17 %, P = 0.013) levels. MMSE (r = -0.526, P < 0.001), MoCA (r = −0.216, P < 0.001), and NPI (r = 0.403, P < 0.001) scores were correlated with non-HDL-C levels. High non-HDL-C levels were an independent risk factor for cognitive disorders after acute ischemic stroke (P = 0.034, odds ratio = 3.115, 95 % confidence interval: 1.088–8.917).ConclusionsHigh serum non-HDL-C levels, age, education, homocysteine levels, and HAMD score were independent risk factors of cognitive impairment in patients with acute ischemic stroke. The risk of cognitive disorders after acute ischemic stroke increased with increasing non-HDL-C levels. This parameter is easy to assess in the clinical setting.

Abstract:Objective: Expanding on previous investigations, this study aims to elucidate the role of lipid metabolism disorders in the development of intracranial atherosclerotic stenosis (ICAS) and the determination of stroke risk. The primary objective is to explore the connections between lipid parameters and acute ischemic stroke (AIS), while also examining the potential mediating influence of fasting glucose levels. Methods: Retrospectively, we collected data from symptomatic ICAS patients at the First Affiliated Hospital of Soochow University, including their baseline information such as medical histories and admission blood biochemical parameters. Stenotic conditions were evaluated using magnetic resonance imaging, computed tomography angiography, or digital subtraction angiography. The associations between lipid parameters and AIS risks were investigated via multivariate logistic regression analysis. Results: A total of 1103 patients with symptomatic ICAS were recruited, among whom 441 (40.0%) suffered new ischemic events during hospitalization. After adjusting for confounding factors, the RCS curves exhibited a dose-response relationship between the atherogenic index of plasma (AIP), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and AIS. Further multivariate analysis revealed significant associations between these parameters and AIS. Furthermore, mediation analysis indicated that fasting blood glucose (FBG) acted as a mediator in the association between lipid parameters (AIP, TC, and TG) and AIS. Conclusion: Higher lipid parameters in ICAS patients, particularly AIP, TC, and TG, were associated with an increased AIS risk. Additionally, FBG may mediate stroke risk in ICAS patients, highlighting the need for further exploration of underlying mechanisms.
U-Shaped Relationship of High-Density Lipoprotein Cholesterol and Incidence of Total, Ischemic and Hemorrhagic Stroke: A Prospective Cohort Study

Abstract:Background: We aimed to investigate the association between serially measured HDL-C (high-density lipoprotein cholesterol) levels and stroke risk in a prospective cohort study. Methods: We included 96 258 individuals (79.6% men, mean age 51.5 years) without a history of stroke, myocardial infarction, or cancer at baseline from the Kailuan Study, with repeated measurements of HDL-C in 2006, 2008, 2010, 2012, 2014, and 2016. Cumulatively, averaged HDL-C concentrations were calculated using all available HDL-C measurements before incidence stroke or end of follow-up (December 31, 2017). Incident stroke cases were confirmed by review of medical records and further subclassified into ischemic or hemorrhagic stroke. Cox proportional hazards regression and restricted cubic splines were used to examine these associations. Results: During a median follow-up of 10.7 years, 5012 incident stroke cases occurred. Restricted cubic splines analysis suggested a U-shaped association between concentrations of cumulatively averaged HDL-C and risk of stroke (Pnonlinearity <0.001), with the nadir of risk at 1.29 mmol/L. After adjustment for cardiovascular risk factors, individuals with cumulatively averaged HDL-C ≤1.06 mmol/L or ≥2.05 mmol/L had hazard ratios for total stroke of 1.31 (95% CI, 1.15–1.49) and 1.85 (1.63–2.09) compared with those with HDL-C of 1.26 to 1.39 mmol/L. Corresponding hazard ratios were 1.29 (1.11–1.48) and 1.84 (1.60–2.11) for ischemic stroke and 1.54 (1.12–2.12) and 2.29 (1.73–3.04) for hemorrhagic stroke, respectively. Conclusions: Both low and high cumulatively averaged HDL-C were associated with an increased risk of ischemic and hemorrhagic strokes.

Abstract:Objective To discuss the correlation between baseline risk factors and acute-stage pathogenesis of acute ischemic stroke.Methods The general information and relative risk factors were collected from 174 initial cases of acute ischemic stroke in stroke group and 45 cases in control group,and all data was given retrospective study.Results The levels of total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL),high-density lipoprotein(HDL),Apo-A1,Apo-B and history of dyslipidemia had no significant difference between two groups.There was significant difference in age,gender,hypertension,fasting plasma glucose(FPG),WBC count,coronary heart disease(CHD),retinal arteriosclerosis,urine protein(UP),fibrinogen(Fig),creatinine(Cr),smoking and obesity(P0.05).Conclusion Age,gender,hypertension,FPG,WBC count,CHD,retinal arteriosclerosis,UP,Fig,Cr,smoking and obesity are correlated to the occurence of acute ischemic stroke.
High-Density Lipoprotein Is Associated with Leukoaraiosis Severity in Patients with Acute Ischemic Stroke

Abstract:Many patients with acute ischemic stroke (AIS) are found to accompany with leukoaraiosis (LA) in brain imaging. The risk factors of LA in patients with AIS were examined in this study. Patients with AIS were recruited and underwent head magnetic resonance imaging. According to Fazekas scores, patients were divided into LA group and non-LA group. We compared demographic and laboratory characteristics in two groups. Multivariate logistic regression analysis demonstrated that high-density lipoprotein (HDL), age, stroke history, admission SBP, and homocysteine were independent risk factors for LA in patients with AIS (P < 0.05). Multinomial logistic regression analysis demonstrated that HDL was an independent risk factor for moderate LA (OR 4.151, 95% CI 1.898–9.078, P < 0.001) and severe LA (OR 3.151, 95% CI 1.350–7.358, P = 0.008). In order to further explore the correlation between HDL level and the severity of LA, HDL was categorized in quartiles and multinomial logistic regression analysis was presented. Regression analysis showed that HDL ≥ 1.34 mmol/L was correlated with moderate and severe LA after adjusting for corresponding confounding factors in different models. After 1-year follow-up, patients were divided into regular statin therapy group and irregular statin therapy group. There was no significant difference in HDL level between two groups; however, the proportion of patients with increased Fazekas scores in regular statin therapy group was significantly less than that in the irregular statin therapy group (P < 0.05). In conclusion, HDL was an independent risk factor for LA and associated with the severity of LA in patients with AIS; regular statin therapy may be negatively related with the progress of LA. These results provide more evidences for controlling risk factors and severity of LA in patient with AIS.

Abstract:Aims: To prospectively explore the association between non-high-density lipoprotein cholesterol (non-HDLC) and the risks of stroke and its subtypes.Methods: A total of 95,916 participants (18-98 years old; 76,354 men and 19,562 women) from a Chinese urban community who were free of myocardial infarction and stroke at baseline time point (2006-2007) were eligible and enrolled in the study. The serum non-HDLC levels of participants were determined by subtracting the high-density lipoprotein cholesterol (HDLC) from total serum cholesterol. The primary outcome was the first occurrence of stroke, which was diagnosed according to the World Health Organization criteria and classified into three subtypes: ischemic stroke, intracerebral hemorrhage, or subarachnoid hemorrhage. The Cox proportional hazards models were used to estimate risk of stroke and its subtypes.Results: During the four-year follow-up, we identified 1614 stroke events (1,156 ischemic, 416 intracerebral hemorrhagic and 42 subarachnoid hemorrhagic). Statistical analyses showed that hazard ratios (HR) (95% Confidence Interval: CI) of serum Non-HDLC level for total and subtypes of stroke were: 1.08 (1.03-1.12) (total), 1.10 (1.05-1.16) (ischemic), 1.03 (0.96-1.10) (intracerebral hemorrhage) and 0.83 (0.66-1.05) (subarachnoid hemorrhage). HR for non-HDLC refers to the increase per each 20 mg/dl. For total and ischemic stroke, the risks were significantly higher in the fourth and fifth quintiles of non-HDLC concentrations compared to the first quintile after adjusting the confounding factors (total stroke: 4th quintile HR=1.33 (1.12-1.59); 5th quintile HR = 1.36 (1.15-1.62); ischemic stroke: 4th quintile HR = 1.34 (1.09-1.66); 5th quintile HR = 1.53 (1.24-1.88)).Conclusions: Our data suggest that serum non-HDLC level is an independent risk factor for total and ischemic stroke, and that higher serum non-HDLC concentrations are associated with increased risks for total stroke and ischemic stroke, but not for intracerebral and subarachnoid hemorrhage.
Discordant High Remnant Cholesterol With LDL-C Increases the Risk of Stroke: A Chinese Prospective Cohort Study

Abstract:Background: Previous studies focusing on assessing the effects of remnant cholesterol (RC) and low-density lipoprotein cholesterol (LDL-C) on stroke may not consider their mutual influence. We aimed to explore the associations of RC and discordant high RC with LDL-C with stroke, ischemic stroke (IS), and hemorrhagic stroke. Methods: This prospective cohort study was conducted based on 3 cohorts of the China-PAR (Prediction for Atherosclerotic Cardiovascular Disease Risk in China) project. RC was calculated as non-high-density lipoprotein cholesterol minus LDL-C estimated by Martin/Hopkins equations. Concordant/discordant categories for RC versus LDL-C were determined based on cut-points of 130 mg/dL for LDL-C and equivalent percentile (32.50 mg/dL) for RC. Cox models were used to estimate adjusted hazard ratios and 95% CIs for incident stroke. Results: Among 113 448 participants recruited at baseline, a total of 98 967 participants were eligible for the final analysis (mean age of 51.44 years; 40.45% were men). During 728 776.87 person-years of follow-up, 2859 stroke cases, 1811 IS cases, and 849 hemorrhagic stroke cases were observed. RC was positively associated with stroke and IS, but not hemorrhagic stroke, with adjusted hazard ratios (95% CIs) of 1.06 (1.02-1.10), 1.09 (1.04-1.13), and 0.95 (0.88-1.03) for per SD increase in RC. Compared with low LDL-C/low RC group, low LDL-C/high RC group had higher risks of stroke (adjusted hazard ratio, 1.15 [95% CI, 1.02-1.30]) and IS (1.19, 1.03-1.38), while high LDL-C/low RC group had no increased risk of stroke (1.07 [0.95-1.20]) and IS (1.09 [0.94-1.25]). Conclusions: Higher RC was associated with increased risks of stroke and IS but not hemorrhagic stroke. Discordantly high RC, not discordantly high LDL-C, conferred higher risks of stroke and IS. Our findings support further lowering RC by interventions to reduce residual IS risk.

Abstract:Objectives: To compare the predictive value of serum low-density lipoprotein (LDL) cholesterol and nonhigh- density lipoprotein (non-HDL) cholesterol levels for ischemic stroke in the Chinese population. Methods: We performed a four-year cohort study of 95 778 men and women, aged 18-98 years, selected from the Kailuan study (2006-2007). Baseline LDL cholesterol levels were estimated using direct test method. Total cholesterol levels were estimated using endpoint test method. The predictive values of LDL cholesterol and non-HDL cholesterol for ischemic stroke were compared. Results: During the follow-up period, there were 1153 incident cases of ischemic stroke. The hazard ratio (HR) for ischemic stroke in the top quintile of LDL cholesterol was the highest among five quintiles (HR: 1.25; 95% confidence interval (CI), 1.01-1.53). The HR in the top quintile of non-HDL cholesterol for ischemic stroke was also the highest among five quintiles (HR: 1.53; 95% CI, 1.24-1.88). Analysis of trends showed a significant positive relationship between ischemic stroke incidence and serum LDL cholesterol level, and non-HDL cholesterol level, respectively (both P < 0.05). The area under the curve of LDL cholesterol and non-HDL cholesterol for ischemic stroke was 0.51 and 0.56, respectively (P < 0.05 for the difference). Conclusions: Serum Non-HDL cholesterol level is a stronger predictor for the risk of ischemic stroke than serum LDL cholesterol level in the Chinese population.

Abstract:Objective:To evaluate the association of lipids and the risk of ischemic stroke and its subtypes.Methods We evaluated the levels of total cholesterol(TC),triglycerides(TG),low density lipoprotein(LDL),high density lipoproteins(HDL),apolipoproteinA(APoA),apolipoproteinB(APoB) and Lipoprotein a,(LP-a) in 182 patients with ischemic stroke and 82 controls.With TOAST criteria,we also evaluated lipids in stroke subtypes.Results Compared with controls,the TG,LDL level was significandy higher,And the HDL,ApoA level was significantly lower,in stroke group(P<0.05),The differences among other components of lipids t.The level of TG,LDL,APoB was significantly higher in large artery atherosclerosis(LAA),cardioaortic embolism(CE) and small artery occlusion(SAO)(P<0.05), The level of TC was significantly higher in cardioaortic embolism(CE),The level of HDL in stroke subtypes was significandy lower in than that of controls(P<0.05 ),The level of APoA was significantly lower in LAA and stroke of other determinedetiology(SOE),The level of LP-a were not signification conclusion the ischemic stroke have interrelationship between the abnormal lipids and correlated with TOAST subtypes.
The risk of ischemic stroke and hemorrhagic stroke in Chinese adults with low-density lipoprotein cholesterol concentrations < 70 mg/dL

Abstract:Abstract Background The risk of stroke in individuals with very low low-density lipoprotein cholesterol (LDL-C) concentrations remains high. We sought to prioritize predictive risk factors for stroke in Chinese participants with LDL-C concentrations < 70 mg/dL using a survival conditional inference tree, a machine learning method. Methods The training dataset included 9327 individuals with LDL-C concentrations < 70 mg/dL who were free of cardiovascular diseases and did not use lipid-modifying drugs from the Kailuan I study (N = 101,510). We examined the validity of this algorithm in a second Chinese cohort of 1753 participants with LDL-C concentrations < 70 mg/dL from the Kailuan II study (N = 35,856). Results During a mean 8.5–9.0-year follow-up period, we identified 388 ischemic stroke cases and 145 hemorrhagic stroke cases in the training dataset and 20 ischemic stroke cases and 8 hemorrhagic stroke cases in the validation dataset. Of 15 examined predictors, poorly controlled blood pressure and very low LDL-C concentrations (≤ 40 mg/dL) were the top hierarchical predictors of both ischemic stroke risk and hemorrhagic stroke risk. The groups, characterized by the presence of 2–3 of aforementioned risk factors, were associated with a higher risk of ischemic stroke (hazard ratio (HR) 7.03; 95% confidence interval (CI) 5.01–9.85 in the training dataset; HR 4.68, 95%CI 1.58–13.9 in the validation dataset) and hemorrhagic stroke (HR 3.94, 95%CI 2.54–6.11 in the training dataset; HR 4.73, 95%CI 0.81–27.6 in the validation dataset), relative to the lowest risk groups (presence of 0–1 of these factors). There was a linear association between cumulative average LDL-C concentrations and stroke risk. LDL-C concentrations ≤ 40 mg/dL was significantly associated with increased risk of ischemic stroke (HR 2.07, 95%CI 1.53, 2.80) and hemorrhagic stroke (HR 2.70, 95%CI 1.70, 4.30) compared to LDL-C concentrations of 55–70 mg/dL, after adjustment for age, hypertension status, and other covariates. Conclusion Individuals with extremely low LDL-C concentrations without previous lipid-modifying treatment could still be at high stroke risk. Abstract:BACKGROUND Recent research has underscored the potentially protective role of dietary antioxidants against chronic conditions, such as cardiovascular diseases and stroke. The composite dietary antioxidant index (CDAI), which reflects the overall intake of key dietary antioxidants, has been identified as a crucial metric for exploring this relationship. Although previous research has shown a negative correlation between CDAI levels and stroke risk in prediabetic individuals, there remains a substantial gap in understanding this association among individuals with dia-betes, who are at an inherently greater risk for cerebrovascular events. AIM To investigate the association between CDAI and stroke risk in individuals with diabetes. METHODS Using a cross-sectional study design, this investigation analyzed data from the National Health and Nutrition Examination Survey spanning from 2003 to 2018 that included 6735 participants aged over 20 years with diabetes. The CDAI was calculated from 24-h dietary recalls to assess intake of key antioxidants: Vitamins A, C, and E; carotenoids; selenium; and zinc. Multivariate logistic regression and restricted cubic spline analysis were used to rigorously examine the relationship between CDAI and stroke risk. RESULTS The participant cohort, with an average age of 59.5 years and a slight male majority, reflected the broader demographic characteristics of individuals with diabetes. The analysis revealed a strong inverse relationship between CDAI levels and stroke risk. Remarkably, those in the highest quintile of CDAI demonstrated a 43% lower prevalence of stroke compared to those in the lowest quintile, even after adjustments for various confounders. This finding not only highlights the negative association between CDAI and stroke risk but also underscores the significant potential of antioxidant-rich diets in reducing stroke prevalence among patients with diabetes. CONCLUSION Our findings suggested that CDAI was inversely associated with stroke prevalence among patients with diabetes. These results suggest incorporating antioxidant-rich foods into dietary regimens as a potential strategy for stroke prevention.

Correlation analysis of cerebral hemodynamics of trancranial Doppler and hyperglycemia and dyslipidemia in ischemic stroke

Abstract:Objective To explore the association of transcranial Doppler hemodynamic parameters with hyperglycemia and dyslipidemia in ischemic stroke patients.Methods Sixty-seven consecutively ischemic stroke patients recruited in Neurology Department of Huashan Hospital underwent fast blood glucose(FBG),serum lipid level examination and transcranial Doppler ultrasonography;hemodynamic parameters such as the mean velocity(Vm),peak systolic velocity(Vs),end diastolic velocity(Vd)and pulsatility index(PI)were recorded.Intracranial stenosis and collateral circulation were determined with the aid of digital substraction angiography(DSA)or computed tomographic angiography(CTA).The SPSS 12.0 was used to analyze the correlation between cerebral hemodynamics and hyperglycemia and dyslipidemia.Results Intracranial stenosis was significantly correlated with serum low-density lipoprotein(LDL)and FBG(r=0.252,P<0.05;r=0.288,P<0.05).Logistic analysis showed that diabetes history was an independent risk factor for intracranial stenosis(OR=2.106,P=0.041).Collateral circulation showed no significant correlation with hyperglycemia and dyslipidemia(P>0.05).The elevated FBG independently predicted the high PI of middle cerebral artery(OR=0.016,P=0.037),while flow velocity of blood showed weak correlation with hyperglecemia and dyslipidemia(P>0.05).Conclusion Cerebral hemodynamics(intracranial stenosis and PI)is correlated with hyperglycemia and dyslipidemia,although larger cohort study is needed to further confirm our conclusion.
Non-High-Density Lipoprotein Cholesterol Predicts Adverse Outcomes in Acute Ischemic Stroke.

Abstract:Non–high-density lipoprotein cholesterol (non–HDL-C) was significantly related to adverse outcomes in patients with cardiovascular disease. We aim to investigate the associations of non-HDL-C and adverse outcomes in acute ischemic stroke. Among 19 604 patients with acute ischemic stroke admitted to the China National Stroke Registry II, 16 113 with both total cholesterol and HDL-C were analyzed. Patients were classified into 5 groups by quintiles of non-HDL-C. The outcomes included recurrent ischemic stroke, intracranial hemorrhage, and all-cause death within 1 year. The relationship of non-HDL-C with the risk of outcomes was analyzed by Cox regression models. Among the 16 113 patients, the median (interquartile range) of non-HDL-C was 3.41 (2.78–4.10) mmol/L. After adjustment for confounding variables, patients in the top quintile of non-HDL-C were associated with higher risk of recurrent ischemic stroke within 1 year (adjusted hazard ratio, 1.46 [95% CI, 1.20–1.77]), compared with those in the third quintile. Patients in the bottom and top quintile of non-HDL-C were associated with higher risk of all-cause death within 1 year (adjusted hazard ratio, 1.22 [95% CI, 1.01–1.47] and adjusted hazard ratio, 1.40 [95% CI, 1.15–1.70], respectively), compared with those in the third quintile. However, non-HDL-C levels were not significantly predictive in intracranial hemorrhage. Non-HDL-C may be a qualified predictor for recurrent ischemic stroke and all-cause death within 1 year in patients with acute ischemic stroke. Download figure Download PowerPoint

Abstract:OBJECTIVETo explore the change and possible role of blood lipids during the course of plaque formation induced by diabetes mellitus in patients with ischemic cerebral vascular disease.METHODSA total of 1 464 hospitalized patients with a diagnosis of ischemic cerebral vascular disease at our department from February 2008 to May 2012. They were distinguished as plaque (n = 332) or not (n = 132) based on carotid artery intima-media thickness (IMT) on B-mode ultrasound. And there were diabetics (n = 153) and non-diabetics (n = 311). Also blood glucose, blood lipids and new infarction or not were recorded. Comparisons of positive carotid artery plaque and the level of blood glucose and lipids were made between diabetics and non-diabetics. And the level-risk relationship between plaque formation and diabetes mellitus or blood lipids were performed through Logistic regression.RESULTSThe positive rate of carotid artery plaque was higher in diabetics versus non-diabetics (80% vs 67%, P = 0.003), OR (95% CI, P value) was 2.001 (1.258-3.182, 0.001) through Logistic regression; after adjusting for cerebral infarction, 1.875 (1.172-3.000, 0.009). It had a lower level of high-density lipoprotein-cholesterol (HDL-C) in diabetics versus non-diabetics (1.1 ± 0.3 vs 1.2 ± 0.3 mmol/L, P = 0.001). HDL-C was correlated with plaque formation while OR (95% CI, P value) was 0.416 (0.210-0.823, 0.012) through Logistic regression. And there has no correlation between plaque formation and other subtypes of blood lipids.CONCLUSIONA decrease of HDL-C may play an important role in the course of carotid artery plaque induced by diabetes mellitus in patients with ischemic cerebral vascular disease.
