Abstract:Macular holes occur in a prevalence of about 0.1 ± 3.0% (95% confidence interval: 0.03, 0.16%) in populations aged 40+ years (Wang et al. 2006). In the recent Beijing Eye Public Health Care Project on 560 000 subjects, macular holes as cause for visual impairment (visual acuity <0.30) were found in 0.01 ± 0.002% of the subjects (Xu et al. 2013). Macular holes have usually been treated by pars plana vitrectomy with removal of the inner limiting membrane and gas tamponade until recently the intravitreal application of ocriplasmin opened the possibility of a medical vitreolysis, release of vitreofoveal traction and closure of some macular holes (Stalmans et al. 2013). In the latter ocriplasmin study, the rate of spontaneous closure of macular holes was 10.1% overall. It agreed with previous reports on the spontaneous closure of macular holes of various aetiology (Yamashita et al. 2002; Michalewska et al. 2008; Yonekawa et al. 2011). There have been no reports so far on the spontaneous closure of macular holes in axially highly myopic non-operated eyes nor in highly myopic eyes with a macular detachment. Here, we present a patient who experienced a spontaneous closure of macular hole in his highly myopic eye. A 76-year-old highly myopic Chinese woman noticed a reduction in vision in her right eye since 10 days before attending the hospital. Her best-corrected visual acuity (BCVA) was 20/200 in the right eye and 20/20 in the left eye. Refractive error in her right eye was −7.5 dioptres, and axial length was 27.8 mm. Slit lamp examination revealed a 2+ nuclear cataract. Ophthalmoscopy demonstrated a macular retinal detachment (Fig. 1A). Spectral-domain optical coherence tomography (Stratus OCT; Carl Zeiss Meditec, Inc., Dublin, CA, USA) revealed a full-thickness macular hole with collateral retinoschisis and retinal detachment and vitreomacular traction (Fig. 1B). Due to the poor general medical condition of the patient, vitrectomy was not performed. Medical intravitreal vitreolysis was not available. Nine months later, the patient reattended the hospital. Her vision had improved to 20/100 in her right eye despite progression in nuclear cataract. Fundus photography showed a decrease in the retinal detachment (Fig. 2A), and OCT revealed a closed macular hole with still existing vitreomacular tractions (Cirrus HD-OCT 4000; Carl Zeiss Meditec, Inc.; Fig. 2B). The contour of the posterior hyaloid looked different as it appeared to have detached from the nasal region of the hole. Spontaneous closure of macular holes, in particular of holes of traumatic origin, has often been described (Yamashita et al. 2002; Michalewska et al. 2008; Yonekawa et al. 2011). For non-traumatic holes, it was noted that the probability of spontaneous closure decreased with increased stage of the hole. Yonekawa et al. (2011) reported on a highly myopic patient who experienced a spontaneous closure of a macular hole, but that patient had previously undergone pars plana vitrectomy for hole closure and the hole had reopened after surgery. To the best of our knowledge, the present article is the first report on a spontaneous closure of a macular hole in an axially highly myopic non-operated eye with additional macular retinal detachment. The macular retinal detachment remained, although it decreased in area and extent, after the spontaneous hole closure. One may assume that the major reason for the detachment was the vitreomacular tractions which still, at least partially, persisted. The change in the contour of the posterior hyaloid with an apparent partial posterior vitreous detachment at the nasal region of the macular hole suggested a decrease in the vitreoretinal tractional force. Furthermore, it cannot be excluded that the macular hole closed during a period of temporary reattachment of the retina. The report may show that also in axially highly myopic eyes, the possibility of a spontaneous closure of macular holes has to be taken in into account in the weighing of therapeutic possibilities. From a pathogenetic point of view, it may be interesting to explore which factors are involved in the process of spontaneous closures of macular holes, in particular if vitreomacular strands and a partial macular detachment partially remain.
