Abstract:Background and objective: Pulmonary alveolar proteinosis (PAP) is a rare pulmonary disease. It was first described in China in 1965, and more cases have been reported subsequently. A systematic review was performed on 241 cases of PAP in China and progress in the diagnosis and treatment of this disease is discussed. Methods: The Chinese biological and medical databases from 1965 to 2006 were searched and 241 cases with complete clinical and pathological data were identified. The clinical characteristics of the disease were summarized and longitudinal comparisons were made of diagnostic and treatment methods over time. Results: The morbidity associated with PAP has increased in recent years. The clinical manifestations were non-specific. Progressive dyspnoea, cough and sputum were the most common symptoms. The percentage of patients undergoing CT examination has increased over the years. The combination of bronchoscopic biopsy and bronchoalveolar lavage (BAL) was usually sufficient to establish the diagnosis. Treatment was reported for a total of 142 cases. BAL and whole lung lavage were both effective and were only required once by most patients. Conclusions: The demographic characteristics and clinical manifestations of PAP patients in China are largely consistent with previous reports. Morbidity has increased dramatically in recent years, mainly due to the broad application of bronchoscopy since 1995. CT is very important for diagnosis of the disease. The long-term effects of treatment by whole lung lavage and BAL are similar.

Abstract:Objective To improve the knowledge,diagnosis and treatment level of pulmonary artery sarcoma (PAS).Methods The clinical data of four patients with PAS confirmed in Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from September 2013 to February 2016 were retrospectively analyzed,and relative literatures were reviewed.Results There were two males and two females,39-58 years old.They all showed chest distress and shortness of breath in their chronic course of the disease.All the patients had different degrees of high D-dimer level.Color doppler ultrasonography showed no abnormity in their deep veins of lower limbs.Echocardiography of three patients revealed echo of clumps in their pulmonary arteries.CT pulmonary angiography examination showed filling defect in the related pulmonary arteries.PET-CT examination of three patients showed that fluorodeoxyglucose intake in the lesions was high.All the patients were misdiagnosed as pulmonary thromboembolism at first,but were confirmed PAS through histopathology after operation finally.Conclusions PAS is a high malignancy with insidious onset.The clinical features and imaging examination have no obvious specificity.The understanding of the disease should be strengthened in clinic,early diagnosis and treatment can prolong the survival time of PAS patients.

Abstract:Objective: To investigate the clinical features and the reason of misdiagnosis of consolidative primary pulmonary lymphoma. Methods: Retrospective study was conducted for three cases with consolidative primary pulmonary lymphoma(PPL). The relevant literatures of primary pulmonary lymphoma in recent years were also reviewed. Results: All patients had been misdiagnosed for a long time due to the non-specific clinical features. Conclusion: Consolidative PPL is difficult to diagnose and is misdiagnosed frequently in clinic. Definitive diagnosis of PPL requires an adequate biopsy specimen. The most important prognostic factor is the histology.

Abstract:Primary pulmonary hypertension (PPH) is a fatal cardiovascular disease characterized by increased pulmonary vascular resistance and progressive right heart failure. It is associated with a very high morbidity and mortality rate. The therapeutic effects of current pharmacological management of PPH are limited. Recent studies have demonstrated that endothelial dysfunction and cell loss play a critical role in the pathogenesis of PPH. Emerging evidence also shows that circulating endothelial progenitor cells instigate new vessel formation via vasculogenesis and revascularization, and provide ongoing endothelial repair by homing to site of endothelial damage. We hypothesized that autologous endothelial progenitor cells transplantation may be a feasible adjunctive therapeutic option for PPH.
AN ANALYSIS OF THIRTY-SIX CASES OF FATAL PULMONARY THROMBOEMBOLISM CONFIRMED BY AUTOPSY

Abstract:Objective To determine the clinical characteristics of hospitalized patients who died of pulmonary thromboembolism (PTE) confirmed by autopsy. Methods The medical and autopsy records of 36 cases of autopsy proven fatal PTE in General Hospital of PLA from 1982 to 2001were reviewed. Cases of tumor embolism, fat embolism, and amniotic fluid embolism were excluded from the study. Results Six patients [16 7% (95% confidence interval, 6 4% to 32 8%)] were diagnosed as PTE prior to death. The diagnosis of PTE was significantly lower in patients with COPD or coronary artery disease. No risk factors were noted in only 4 patients (13 9%). Only a minority of patients had pleuritic pain or hemoptysis(8 3% and 11 1% respectively), while a significantly larger proportion of patients (25%, P 0 05) experienced circulatory collapse. Prophylactic measures against thromboembolism had been used in 4 patients. Conclusion Fatal PTE could not be diagnosed on the basis of its common clinical manifestations, because of lack of typical symptoms and signs. Furthermore, there was often lack of verbal exchange between the patient and physician, and sudden death of the patient and the pre existence of other illnesses added further the difficulty in correct diagnosis.

Abstract:In recent years,people have gradually realized that chronic obstructive pulmonary disease (COPD) is not only a kind of chronic lung disease,but also a systemic disease.In many complications of COPD,pulmonary hypertension ( PH) is a common complication.Although the primary disease progressed slowly,the presense of PH is associated with aggravated symptoms,shortened course of disease and increased mortality.It has a direct impact on the patients′ health and prognosis,and no effective drugs can be treated.Here is to make a review of the present research progress in pathogenesis, diagnostic strategies,treatment,prognosis evaluation and other factors of COPD-associated PH.
[Pulmonary Sclerosing Hemangioma: Report of 15 Cases and Review of the Literature].

Abstract:OBJECTIVE To improve the understanding of pulmonary sclerosing hemangioma (PSH). METHODS The clinical data of 15 cases of PSH were analyzed, and the literature was reviewed. The etiology, clinical manifestations, differential diagnosis, treatment and outcome of PSH were described. RESULTS The etiology and histological origin of PSH were unclear. Most cases were asymptomatic or only with mild symptoms. The radiology of PSH often showed isolated nodule with distinct margin in the lung field. The characteristics of its pathological manifestation were as follows: (1) background of cell gathering or mucin matrix with scattered white blood cells; (2) proliferation of hemangioma with sclerosis of vessel wall; (3) papillary proliferation of small vessels intruding into the air space; (4) existence of hemorrhage or sclerosis zone. Immunohistological studies had not defined the correct histological origin of PSH. A pre-operation diagnosis of PSH was difficult. Thirteen cases had been misdiagnosed as malignancy. The outcome of the disease was good when early surgical resection was performed. CONCLUSIONS PSH is an uncommon disease, and can be easily misdiagnosed. More attention should be paid to its clinical features and management.

Abstract:We reported a case of pulmonary capillary hemangiomatosis (PCH) and introduced its diagnosis, differential diagnosis, pathogenesis and development of treatment based on the review of Dana Point 2008 Classification of Pulmonary Hypertensiona and current literatures. A 43-year-old female presented progressive dyspnea, elevated pulmonary arterial pressures and CT pulmonary angiography (CTPA) imaging of main pulmonary arterial enlargement and wide spread ill-defined centrilobular nodules of ground-glass opacity. Her histologic features were proliferation of capillary channels within alveolar walls as well as muscularization of arterioles and medial hypertrophy of muscular pulmonary arteries. The treatment with diuretics and warfarin was used promptly, but unfortunately was ineffective. The patient died three months after diagnosis. PCH is a very rare vascular disease with poor prognosis. The diagnosis of PCH rests on the integration of clinical and radiographic information with pathologic features, however pathology is the most reliable means. Because clinical symptoms, imaging and histological features of pulmonary veno-occlusive disease (PVOD) and PCH broadly overlap, differential diagnosis should be made carefully. Among the various pathologic features proliferation of capillaries within alveolar walls is the key point for diagnosing PCH, which is also the most critical criteria for differentiating PCH from PVOD. So far the only definitive treatment for PCH is lung transplantation, without which the patient will die several months after diagnosis.

Abstract:Primary pulmonary hypertension (PPH) is a pulmonary vascular disease characterized by an elevation in mean pulmonary artery pressure and pulmonary vascular resistance. Recently, PPH gained national attention because of its association with appetite suppressants. PPH may also be associated with pregnancy, hypothyroidism, autoimmune disorders, human immunodeficiency virus infection, and the use of drugs such as oral contraceptives and cocaine. Patients with PPH may report dyspnea on exertion and fatigue. Early diagnosis is crucial. New therapeutic regimens have dramatically reduced mortality rates and improved quality of life by halting the progression of pulmonary vascular remodeling and averting right-sided heart failure. These therapies include high-dose calcium channel antagonists, anticoagulants, and continuous intravenous prostacyclin. Lung or heart-lung transplantation remains a viable therapeutic option for patients who are treated late in the disease process, who are not responsive to medical management, or who remain symptomatic and continue to deteriorate.

Abstract:OBJECTIVETo improve the diagnosis of hemoptysis caused by rare lung diseases.METHODSFour cases of hemoptysis caused by rare diseases which were diagnosed by angiography or operation were reported and related literature were reviewed.RESULTSThe 4 cases of hemoptysis were caused by intercostal-to-pulmonary arterial fistula (IPAF), pulmonary hamartoma (PH), congenital unilateral absence of the left pulmonary artery (ALPA) and primary pulmonary hypertension (PPH). Bronchial arteriography and bronchial artery embolization were performed for IPAF. For PH, a benign tumor, the diagnosis was based on the history and radiography examination, and operation was the choice for therapy. ALPA was confirmed by radiography, if massive hemoptysis occurred, surgical intervention was the best choice. The diagnosis of PPH was firstly evaluated by chest film and echocardiogram, and the final diagnosis relied on right cardiac catheter examination.CONCLUSIONSHemoptysis is a common symptom of respiratory diseases, including some rare cases which are easy to be misdiagnosed. The correct diagnosis and therapy rely on comprehensive thinking, careful history collection and reasonable examinations.

Abstract:Background: The primary pulmonary lymphoma (PPL), with a low incidence, was highly misdiagnosed in clinic. The present study analyzes the clinical features, laboratory and imaging data, pathologic characteristics, and summarizes misdiagnosis reasons of PPL cases, aims to provide a better understanding and increase the accuracy of early diagnosis and minimize the misdiagnosis of PPL. Methods: The clinical data of 19 cases were collected from the first affiliated hospital of Wenzhou medical university (PRC) from April 2010 to May 2016. All cases were confirmed by pathology. The process of misdiagnosis was described. This study retrospectively analyzed the incidence, clinical presentation, laboratory examination, Chest CT scan and diagnosis of the cases. Results: The symptoms of the 19 cases were dyspnea, fever, hemoptysis, chest pain or physical findings without obvious symptoms. Five patients were pneumonia-like, nine patients had lung single nodule or mass and four patients got pleural effusion, which were reported by computed tomography (HRCT) scan. There were 2 cases of Hodgkin lymphoma (HL), and 17 cases of non-Hodgkin lymphoma (NHL). In NHL cases, 12 cases were confirmed mucosa associated lymphoid tissue B lymphoma type, 3 cases were confirmed diffuse large B-cell lymphoma, angioimmunoblastic T-cell lymphoma and ALK positive anaplastic large cell lymphoma were one case separately. Clinical and imaging manifestation of PPL is untypical, but there are still some hints: 1) Fuzzy shadow at the edge of lung mass with air bronchogram; 2) Lung mass shadow stable for a long time; 3) Pneumonia-like changing without infections clinical and lab manifestation. Thirteen patients (68.4%) were misdiagnosed as pneumonia, lung cancer and tuberculosis initially. The term between initial diagnosis and final diagnosis lasted for half a month up to 2 years, with median time of 6 months. Two cases were misdiagnosed as tuberculosis. One case was misdiagnosed as small cell lung cancer. Conclusion: Clinical and imaging manifestation of PPL is untypical. Biopsies should be taken actively if the imaging findings don't match the symptoms or the anti-infection treatments to "lung infection" don't work. Accurate diagnosis requires adequate tissue sampling with appropriate ancillary pathologic studies. If clinical manifestation and the diagnosis don't match, repeated biopsy should be ordered.
[Hypersensitivity Pneumonia Diagnosed by Excisional Lung Biopsy: a Clinicopathological Features of 47 Cases].

Abstract:Objective: To analyze the clinicopathological and immunohistochemical characteristics of the hypersensitivity pneumonia (HP) cases that were diagnosed by excisional lung biopsy, to improve the diagnosis accuracy of HP. Methods: The data of 47 HP cases diagnosed by excisional lung biopsy during the last 20 years were collected in Peking Union Medical College Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College, from January 2000 to June 2020. The clinicopathological features and the immunohistochemical profiles of CD3, CD4, CD8 and CD20 were analyzed using light microscopy and immunohistochemical stains. Results: The age of the patients ranged from 15 to 68 years, while the ratio of male to female was 22∶25. The main symptoms were shortness of breath and cough, and 32 cases had a history of allergen exposure. Most of the patients had restrictive impaired ventilatory function, and impaired diffusion function. The proportion of lymphocytes in bronchoalveolar lavage fluid (BALF) increased in 85.3%(29/34) of the patients, and the CD4+/CD8+ T cell ratio was not higher than 0.8 in 31.3%(10/32) of the patients. Microscopically, 59.6%(28/47) of the cases showed nonnecrotizing granuloma/giant cells in the stroma, and 40.4% (19/47) of the cases had only giant cells. Ninety-three-point six percent of the cases(44/47) had peribronchiolar metaplasia, while 70.2%(33/47) had focal cell-rich inflammation. Forty-four cases (93.6%, 44/47) had fibrosis, which was manifested as usual interstitial pneumonia-like fibrosis (47.7%, 21 cases), nonspecific interstitial pneumonia-like fibrosis (29.5%, 13 cases), purely peribronchiolar fibrosis (6.8%, 3 cases) and mixed fibrosis (15.9%, 7 cases). Immunohistochemistry showed that CD4+ T cells were fewer than CD8+ T cells in 24 cases (55.8%, 24/43) of HP. Thirty-two HP patients had follow-up data. Among them, 7 patients had improved symptoms, 12 patients were stable, and 13 patients had worsening symptoms. Conclusions: The most common type of HP diagnosed by excisional lung biopsy is chronic HP. The clinical characteristics are long course of disease, restricted ventilation disorder and impaired diffusion function with the increase of lymphocyte proportion in BALF. Nearly 1/3 of the patients have no known history of allergen contact. The degree of fibrosis in HP patients diagnosed by excisional lung biopsy is severe, and the histologic types of fibrosis varies, but the UIP-like and/or NSIP-like fibrosis is most common. The CD4+/CD8+ T cell ratio in lung tissues is less than 1 in more than half of the cases. Pulmonary fibrosis continued to increase in some of the patients with chronic HP, even after treatment.

Abstract:OBJECTIVE:To explore the demographic characteristics and clinical features of patients with idiopathic pulmonary arterial hypertension (IPAH) in China.METHODS:Between March 2007 and September 2010, IPAH diagnosis was confirmed by right heart catheterization in 150 adult patients from 31 clinical centers in China. Clinical and hemodynamic data were analyzed and patients were divided into WHO functional class I/II and WHO functional class III/IV group.RESULTS:The mean age of 150 patients were 36 ± 13 years with female patient/male patient ratio of 2:1, and mean BMI was (21.3 ± 3.5) kg/m(2). Fatigue (n = 123, 82.0%) and dyspnea (n = 112, 74.7%) are the most common symptoms. Accentuated pulmonic second sound (P(2)) was detected in 92.0% (n = 138) of patients during physical examination, which was also the most common sign. About 49.0% (n = 73) patients were WHO functional class I/II patients and 46.0% (n = 68) patients were WHO functional class III/IV patients. Six minutes walking distance (6MWD) and Borg dyspnea score was (337 ± 101) m and 2.0 (2.0, 4.0), respectively. Right ventricular hypertrophy was suggested by ECG in 93.1% (n = 140) patients. Right atrial pressure was (10 ± 6) mm Hg, mean pulmonary artery pressure was (61 ± 16) mm Hg, cardiac index was (2.3 ± 0.8) L×min(-1)×m(-2) and pulmonary vascular resistance (1484 ± 699) dyn×s(-1)×cm(-5) in this cohort. 6 MWD (305 m ± 89 m vs. 377 m ± 88 m) was significantly shorter while Borg dyspnea score [3.0 (3.0, 5.0) vs. 2.0 (2.0, 3.0)] was significantly higher in WHO functional class III/IV patients than in WHO functional class I/II patients. Similarly hemodynamic parameters were also worse in WHO functional class III/IV patients than in WHO functional class I/II patients (all P < 0.05).CONCLUSION:Idiopathic pulmonary arterial hypertension patients in this cohort affect mostly young adults, dominated by female gender and lower body mass index. Fatigue and dyspnea are the most common symptoms and accentuated pulmonic second sound (P(2)) is the most common sign. IPAH patients are often displaying severe functional and hemodynamic disturbance at first visit to hospitals. Dyspnea and hemodynamic impairment are related to 6MWD and WHO functional class.
Three Cases of Primary Pulmonary Lymphoma and Review of the Literature

Abstract:Objective To study the clinical characteristics,diagnosis and treatment of primary pulmonary lymphoma.Methods A retrospective review of primary pulmonary lymphoma cases at a single institution from 2006 to 2008 was performed,and relevant literature was reviewed.Results Primary pulmonary lymphoma is a rare disease.The diagnosis was difficult because of the lack of specific characteristics.The most common symptoms were cough and fever.X-ray feature included solitary or multiple nodules and consolidation.Definite diagnosis was made by pathologic and immunohistchemical examinations.The recommended first-line therapy is chemotherapy.Conclusion Appropriate invasive biopsy is necessary for early diagnosis of primary pulmonary lymphoma.
Clinical Characterizations and Severity Evaluation of Pulmonary Hypertension

Abstract:Objective Analyze clinical characterizations and severity of 102 cases of pulmonary hypertension from Apr 2003 to Dec 2011.Methods Investigate the age,etiology,symptoms,disease severity and the correlation between echocardiography and right heart catheterization (RHC) in pulmonary hypertension (PH) patients by retrospective methods.Results Female was majority (72.5％)in PH and the peak age period of the disease was middle-aged and youth people.The major symptoms of PH were breathless after exercise (81.4％) and chest congestation (51.9％).The top 3 etiologies were idiopathic pulmonary arterial hypertension (IPAH) (49 cases,48.0％),congenital systemic-pulmonary shunts (19 cases,18.6 ％),chronic pulmonary thromboembonic disease (10 cases,9.8 ％).The systolic pulmonary artery pressure (sPAP) measured by Echo is different from those got by RHC.The discrepancies of sPAP between these two methods (absolute value) were 1-10 mm Hg (33.4％),11-20 mm Hg(33.3 ％),and ≥21 mm Hg (33.3 ％) respectively.The echocardiography might be overestimate or underestimate the real pressure,when the discrepancy was more than 10 mm Hg.The correlation between echocardiography and RHC was good (r =0.634,P ＜0.01).Echocardiography had been found to overestimated 9 cases of mild or moderate PH.The echocardiography could provide an accurate evaluation of PH magnitude,especially for a severe PH (accuracy rate:95.23％).Conclusions Echocardiography has a good relationship with right heart catheterization.It is a helpful clue for diagnosis of PH.The absence of an elevated pulmonary artery pressure appears to exclude PH reliably,Echocardiography could provide relatively accurate evaluation for severe PH.Mild or moderate elevated pulmonary artery pressure by echocardiography should be analyzed with other information like history,symptoms and biomarkers.For those who is hard to get accurate diagnosis,RHC.should be suggested.

Abstract:Objective To highlight the characteristics of pulmonary langerhans cell histocytosis (PLCH).Methods The clinical,radiological and pathological data of three patients with PLCH were analyzed,and relevant literature was reviewed.Results Three male smoker presented with cough,one of them with chest pain.The chest CT scan revealed multiple,bilateral nodules distributing randomly in one case and a mix of nodules and cysts in the lungs in other two cases.Two patients were diagnosed after lung biopsy and another one by bone biopsy,the pathological examination of three patients revealed a mixture of langerhans cell,eosinophils infiltration around the bronchioles and positive immunohistoehemical staining for CD1a,S100,which was consistent with PLCH.The patients showed significant clinical and radiological improvement after therapy.Two patient was stable and the other one died of severe infection because of chemotherapy.Conclusions Patients of PLCH without special clinical manifestations are diagnosed by radiographic and pathologic data.If the imaging characteristics is atypical,pathology could help us.
Clinical characteristics and risk factors of pulmonary hypertension associated with chronic respiratory diseases: a retrospective study.

Abstract:BACKGROUND Chronic respiratory disease-associated pulmonary hypertension (PH) is an important subtype of PH, which lacks clinical epidemiological data in China. METHODS Six hundred and ninety three patients hospitalized from 2010 to 2013 were classified by echocardiography according to pulmonary arterial systolic pressure (PASP): mild (36≤ PASP <50 mmHg); moderate (50≤ PASP <70 mmHg) and severe (PASP ≥70 mmHg). RESULTS Dyspnea (93.51%) was the most common symptom. Hemoptysis observed in the severe group (6.42%) was significantly higher than the other two groups (P<0.05). COPD (78.35%), lung bullae (44.16%), tuberculosis (including obsolete pulmonary tuberculosis) (38.82%), and bronchiectasis (30.45%) were frequently present. Mild group occupied the highest proportion (84.7%) in COPD, while severe group occupied the highest proportion (19.3%) in pulmonary embolism (P<0.01). Age, partial pressure of oxygen (PaO2), hematocrit (HCT), partial pressure of carbon dioxide (PaCO2), increase of N-terminal pro brain natriuretic peptide (NT-proBNP) and right ventricular (RV) diameter (>20 mm) were associated with moderate-to-severe PH, while RV [odds ratio (OR) =3.53, 95% CI, 2.17-5.74], NT-proBNP (OR=2.44, 95% CI, 1.51-3.95), HCT (OR=1.03, 95% CI, 1.00-1.07) and PaCO2 (OR=1.01, 95% CI, 1.00-1.03) were independent risk factors. CONCLUSIONS PH related to respiratory diseases is mostly mild to moderate, and the severity is associated with the category of respiratory disease. Increased HCT can be an independent risk factor for PH related to chronic respiratory diseases.

Pulmonary hypertension is a syndrome characterized by marked remodeling of the pulmonary vasculature and a progressive rise in the pulmonary vascular load, leading to hypertrophy and remodeling of the right ventricle. Death results from right ventricular failure if pulmonary hypertension is left untreated. Pulmonary hypertension is currently defined hemodynamically by a mean pulmonary arterial pressure of higher than 20 mm Hg at rest, as measured by right heart catheterization.1 Precapillary pulmonary hypertension due to pulmonary vascular disease is further defined by an elevation in pulmonary vascular resistance of at least 3 Wood units (WU), in contrast to isolated postcapillary pulmonary hypertension, in which the pulmonary vascular resistance is less than 3 WU and the elevation in the mean pulmonary arterial pressure is due to elevated filling pressures on the left side of the heart. The several forms of pulmonary hypertension are categorized into five clinical groups (Fig. 1). This review focuses on the relatively rare form of pulmonary arterial hypertension (group 1). Immense progress has been achieved in gaining an understanding of the mechanisms, natural history, and genetic features of pulmonary arterial hypertension and in establishing targeted therapy. A full appreciation of the pathophysiology of the syndrome is important, since the diagnosis requires a thorough clinical investigation to rule out other, more common forms of pulmonary hypertension, for which treatment of the underlying disease should be the primary goal. The first anatomical description of pulmonary hypertension is credited to von Romberg.2 However, it is the advent of human right heart catheterization, first performed by Forssmann on himself in 1929,3 that led to a flurry of physiological observations on the heart and pulmonary circulation by Cournand and Richards in the 1940s. The three investigators received the Nobel Prize in Physiology or Medicine in 1956 for their seminal work. In his illuminating Nobel Lecture, Richards noted that, as a result of their collective work, ì many forms and degrees of failure were defined, and their responses to treatment measured.î 4 In 1951, Dresdale, a disciple of Cournand and Richards, and colleagues presented the first case series of patients with pulmonary hypertension of unknown cause, defined as ì primary pulmonary hypertension.î 5 Greater awareness of the disease developed in the 1960s, after an epidemic of primary pulmonary hypertension that was associated with the use of the appetite suppressant aminorex.6 This prompted the World Health Organization to convene a first meeting of pulmonary hypertension experts in 1973, to standardize the clinical and pathological nomenclature of primary pulmonary hypertension, the first attempt at an organized classification.7 The first and second meetings about pulmonary hypertension were separated by 25 years, but thereafter the World Symposium on Pulmonary Hypertension (WSPH) was convened every 5 years. Abstract:OBJECTIVE:To summarize and analyze the clinical manifestations of pulmonary alveolar proteinosis (PAP) and its treatment.METHODS:Retrospective analysis of 2 cases with literature was reviewed.RESULTS:PAP presented a protracted chronic clinical course with overt symptoms but few physical signs, distinct chest radiography, and was a benign process. It was manifested as primary or secondary pathological changes. Typical chest CT scan showed "map" or "cobble stone" changes. Clinically, PAP was characterized by its incompatibility between symptoms and signs, and also radiographic picture and physical signs. The examination of bronchoalveolar lavage fluid could be used to confirm the diagnosis of PAP with periodic acid-schiff staining and electro microscopic examination. Whole lung lavage was an efficient therapy for PAP.CONCLUSION:It is very important for physicians to pay much more attention on the early diagnosis and treatment of pulmonary alveolar proteinosis.
