Abstract:Gastroesophageal reflux (GER) is one of the most common causes of chronic cough, and chronic cough due to GER represents a subtype of GER-related diseases. Gastroesophageal reflux-induced chronic cough (GERC) can be divided into two subgroups based on the pH of the GER. Nonacid GERC is less common than acid GERC, and its diagnosis and treatment strategy have not been standardized. However, nonacid GERC usually presents with its unique set of characteristics and features upon diagnosis and treatment in the clinic. Although the underlying molecular mechanism of nonacid GERC is not fully understood, it is considered to be associated with reflux theory, reflex theory and airway hypersensitivity. Multi-channel intraluminal impedance combined with pH monitoring is a promising new technique that can detect both acid and nonacid reflux, and our findings as well as those of others have shown its usefulness in diagnosing nonacid GERC. Development of new diagnostic techniques has led to an increased rate of nonacid GERC diagnosis. We summarize our experience in the diagnosis and treatment of nonacid GERC and provide a guide for future therapeutic approaches.
Relationship Between Chronic Cough and Gastroesophageal Reflux in Children

Abstract:Objectives To investigate the relationship between chronic cough and gastroesophageal reflux(GER)in children.Methods Twenty-four-hour esophageal pH monitoring was performed in 170 cases(patient group)with chronic cough( 4 weeks),and 32 asymptomatic children served as control group.Results All the parameters of esophageal acid reflux including episodes of acid reflux,number of the reflux longer than 5 min,the longest reflux time(min),percentage time of pH 4 and Boix-Ochoa score were significantly higher in patient group than that in control group(P 0.01=.According to the diagnostic criteria of GER(Boix-Ochoa score 11.99 and reflux index ≥ 4.0%),pathological GER was detected in 63 of 170(37.1%)in patient group,while it was only 3.1%(1/32)in controls,with the statistical difference(χ2 = 14.327,P 0.01).The significant differences were also observed in the positive rate of GER among the three different age group,they were 47.4%(27/57),41.5%(17/41)and 26.4%(19/72)in patients aged from two months to one years,less than 3 years and more than 3 years,respectively(χ2 = 6.453,P 0.05).Conclusions It was suggested that GER could be the underlying cause of chronic cough in children,especially in those aged less than 3 years.24-hour ambulatory esophageal pH monitoring is a useful test for identifying the etiology of chronic cough in children.
Gastro-esophageal Reflux Induced Cough with Airway Hyperresponsiveness.

Abstract:To investigate the characteristics of gastroesophageal reflux induced cough (GERC) with airway hyperresponsiveness (AHR). Compared to patients with GERC alone and healthy subjects, cough sensitivity, multi-channel intraluminal impedance combined with pH monitoring and airway inflammation were evaluated in patients with GERC and AHR. 23 patients were definitely diagnosed as acid reflux induced GERC, 9 patients developed AHR concomitantly. When compared with GERC patients, patients with AHR had significantly increased number of proximal extent episodes (21.5 (28.6) vs. 7.5 (1.8), Z = -2.038, P = 0.042) and increased proportion of proximal extent episodes to total refluxes episodes (24.5 (13.5)% vs. 4.2 (7.3)%, Z = -2.138, P = 0.032), and the level of IL-8 in the airway of these patients was significantly higher than that in healthy subjects (71.1 (64.0) vs. 24.3 (35.2) pg/ml, Z = -2.013, P = 0.044). Gastroesophageal reflux may cause neutrophilic airway inflammation due to the acid reflux into the airway, which results in AHR. However, AHR is not definitely able to cause chronic cough. Thus differential diagnosis is required in clinical practice.
Comparison of Gastroesophageal Reflux Disease Questionnaire and Multichannel Intraluminal Impedance Ph Monitoring in Identifying Patients with Chronic Cough Responsive to Antireflux Therapy

Abstract:Background: Empirical therapy has been recommended as an initial clinical approach for treating gastroesophageal reflux-induced chronic cough (GERC). This study compared the predictive accuracy of the Gastroesophageal Reflux Disease Questionnaire (GerdQ) with the accuracy of multichannel intraluminal impedance pH monitoring (MII-pH) for GERC.Methods: A total of 126 consecutive patients with potential GERC were recruited to undergo MII-pH and complete the GerdQ. A final diagnosis of GERC was made after favorable response to consequent medicinal antireflux therapy, regardless of laboratory findings. The predictive accuracy of the GerdQ for GERC was assessed and compared with that of MII-pH.Results: GERC was confirmed in 102 of 126 patients (81.0%); cough was due to acid reflux in 55 (53.9%) and nonacid reflux in 47 (46.1%). The optimal cutoff point of the GerdQ for predicting GERC was defined as 8.0 according to the highest Youden index of 0.584, with a sensitivity of 66.7%, specificity of 91.7%, positive predictive value of 97.1%, and negative predictive value of 42.9%. A subanalysis for only acid GERC showed further improvement in the predictive accuracy of the GerdQ, corresponding to a sensitivity of 90.9%, specificity of 78.6%, positive predictive value of 71.4%, and negative predictive value of 96.4%. However, a meaningful GerdQ cutoff point for prediction of nonacid GERC could not be determined. In general, MII-pH was superior to the GerdQ for predicting GERC and acid GERC.Conclusions: The GerdQ can be used for predicting acid GERC but not nonacid GERC and is inferior to MII-pH.
[Changes and Implications of Esophageal Function in Patients with Chronic Cough Induced by Gastroesophageal Reflux].

Abstract:Objective: To explore the changes in the esophageal function and their association with the therapeutic outcome in patients with gastroesophageal reflux-induced chronic cough (GERC). Methods: One hundred thirty-five patients with definite GERC consecutively referred to our respiratory clinic were recruited into the study between January 2012 and August 2015.Cough was due to acid reflux in 81 patients and non-acid reflux in 54 patients, with the favorable response to the standard antireflux therapy in 88 patients and to the intensified antireflux treatment in 47 patients. The control groups included 26 patients with gastroesphageal reflux disease without cough and 22 healthy volunteers. All the subjects underwent an esophageal manometry from which the parameters were recorded, including the pressure, length and relaxation rate of lower esophageal sphincter, and the peristaltic contractive amplitude, wave velocity and contractive time of esophagus. The data were combined with the results of multi-channel intraluminal impedance combined with pH monitoring to analyze the changes of esophageal function in the patients with acid or non-acid GERC and their relation to the outcomes of antireflux therapy. Results: Compared with healthy volunteers, patients with GERC presented with a lower pressure [(11±5) mmHg vs (15±5) mmHg (1 mmHg=0.133 kPa), q=3.70, P=0.000], shorter overall length [(2.2±0.5) cm vs (3.0±1.0) cm, q=2.78, P=0.017] and similar relaxation rate of lower esophageal sphincter(q=1.14, P=0.258). Furthermore, they also showed a decrease in esophageal peristaltic contractive amplitude [(33±13) mmHg vs (45±11) mmHg, q=2.19, P=0.030] and wave velocity [(2.6±0.8) cm/s vs (3.4±0.6) cm/s, q=2.91, P=0.010] but an increase in esophageal contractive time of esophagus [(4.9±2.2) s vs (3.1±0.8) s, q=3.25, P=0.001] in addition to a linear negative correlation between esophageal peristaltic wave velocity and bolus clearance (r=-0.603, P=0.000). However, these parameters were not different between patients with GERC and gastroesophageal reflux disease without cough. The patients with GERC due to acid and non-acid reflux presented with a similar esophageal dysmotility but different variables reflecting the acidity of refluxates as indicated by multi-channel intraluminal impedance combined with pH monitoring. The esophageal peristaltic wave velocity was significantly lower in the patients with GERC responsive to the standard antireflux therapy than in those responsive to the intensified antireflux therapy [(2.2±0.6) cm/s vs (3.0±1.0) cm/s, t= 2.066, P= 0.041]. Conclusions: Esophageal dysfunction is present in patients with GERC. Its characteristics and severity are not associated with the types of gastroesophageal reflux inducing cough, but may predict the efficacy of medical antireflux therapy.

Abstract:Background: This study aimed to investigate the reflux characteristics in patients with gastroesophageal reflux-related chronic cough (GERC) complicated by laryngopharyngeal reflux (LPR). Methods: Patients with chronic cough were recruited. Reflux symptom index (RSI) scoring, cough symptom scoring, assessment of capsaicin cough sensitivity, and multichannel intraluminal impedance and pH monitoring (MII-pH) were performed. Results: RSI score in GERC patients was significantly higher than that in patients with atopic cough (AC), cough variant asthma, eosinophilic bronchitis (EB), and upper airway cough syndrome (UACS) (P<0.05). The RSI score in non-acid GERC patients was significantly higher than that in acid GERC patients (P=0.003). The cut-off value of the RSI score was defined as 19 during diagnosis of non-acid GERC. In the RSI >= 19 group, there was more proximal reflux and more significant gas and non-acid reflux, and the efficacy of a combined use of baclofen or gabapentin was better than that of the RSI <19 group (P<0.05). The efficacy of proton pump inhibitor (PPI) at a routine dosage together with prokinetic agents in the RSI <19 group was better than that in the RSI 19 group (P=0.009). Conclusions: LPR overlaps with GERC in part. GERC patients with higher RSI scores may present more proximal reflux, non-acid reflux, and gas reflux, and get better efficacy with neuromodulators (gabapentin or baclofen) used as an add-on therapy.

Abstract:Gastroesophageal reflux cough,a special type of gastroesophageal reflux disease,is considered as one of the most common causes of chronic cough.However,gastroesophageal reflux cough has its own characteristics in pathogenesis,diagnosis and treatment.It has associated with distal or proximal reflux and airway hypersensitivity.The diagnosis of gastroesophageal reflux cough is dependent upon the combination of clinical presentation and laboratory investigations.Muhichannel intraluminal esophageal impedance and pH monitoring is a promising technique developed recently which is able to detect both acid and non-acid reflux.Currently,anti-reflux therapy with drugs is still the main approach for gastroesophageal reflux cough.

Abstract:Gastroesophageal reflux disease is one of most common causes of chronic cough.Medical history offers few clues as to the cause of cough induced by this disease.24-h esophageal ph monitoring is considered as the most sensitive and specific test for the diagnosis.When it is unavailable or unsuitable,upper gastrointestinal endoscopy and barium esophagography can be used as an alternative examination.Combined multichannel intraluminal impedance and pH testing is promising because of its ability to detect nonacid reflux as well as acid reflux.Empiric therapy trial is a simple and cheap way to identify suspected patients.Drug therapy is effective in most of the patients,in which proton pump inhibitors is the most powerful.Antireflux surgery is the last choice,used only when intensive drug therapy fails.The definitive diagnosis of cough due to gastroesophageal reflux disease can be established only after cough improves or completely disappears with antireflux therapy.

Association Between Chronic Cough and Gastroesophageal Reflux

Abstract:Objective: To explore the association between chronic cough and gastroesophageal reflux(GER),and to investigate the clinical features,diagnosis and treatment of gastroesophageal reflux induced cough(GERC).Methods: Continuous ambulatory esophageal pH measurement was performed for 24 hour in 43 cases with chronic cough who had normal chest X-ray presentation,negative histamine provocation test and no abnormality of nasal cavity.The symptom association probability(SAP) was adopted to analyze the correlation of cough with gastroesophageal reflux.The unusual GER was defined subsequently if one of the following index was found: ① Total scores of far electrode Demeester≥12.70 in 24 hours surveillance of esophageal pH value.② Total scores of far electrode Demeester 12.70,but SAP ≥75% at least in one electrode.Once the diagnosis of unusual GER was established,anti-reflux medications were utilized to the patients for consecutive 12 weeks.Results: 26 out of 43 cases had GER and were diagnosed as GERC.The GERC patients were treated with anti-refluxdrugs.After treatment,cough in 12 patients were disappeared completely and the other 14 cases had no response.Conclusion: It can be concluded that the GER is closely associated with cough and may be an important inducing factor of chronic cough.Continuous esophageal pH measurement for the frequency and severity of gastroesophageal reflux and the SAP of cough were main diagnostic procedures for GERC.The patients with GERC should be treated with anti-reflux.

Abstract:Background and objective: The aim of this study was to explore the pathogenesis of chronic cough caused by non-acid reflux.Methods: Seven patients with chronic cough due to non-acid reflux, 12 patients with chronic cough due to acid reflux, 10 patients with gastro-oesophageal reflux disease without cough and 12 healthy volunteers were recruited for the study. All subjects underwent oesophageal multi-channel intraluminal impedance measurements combined with pH monitoring, and assessment of cough reflex sensitivity to capsaicin and induced sputum cytology. The concentrations of substance P, mast cell tryptase, prostaglandin D2 and histamine in induced sputum were measured by ELISA.Results: Cough threshold C2 and C5 did not differ between patients with chronic cough due to non-acid or acid reflux, but the values were significantly lower than those for patients with gastro-oesophageal reflux disease without cough and healthy volunteers. Weakly acidic reflux episodes were obviously more frequent in patients with chronic cough due to non-acid reflux than in the other three groups. Sputum substance P and mast cell tryptase concentrations were remarkably increased in patients with chronic cough, but were similar for those with cough due to non-acid or acid reflux. There were significant inverse correlations between substance P levels and cough threshold C2 or C5 in patients with cough due to non-acid or acid reflux, and between mast cell tryptase levels and cough threshold C2 in patients with cough due to acid reflux.Conclusions: Chronic cough due to non-acid reflux may be related to cough reflex hypersensitivity caused by neurogenic airway inflammation and mast cell activation, in which weakly acidic reflux is possibly a major factor.

Abstract:Background: Fractional exhaled nitric oxide (FeNO) measured at multiple exhalation flow rates can be used as a biomarker to differentiate central and peripheral airway inflammation. However, the role of alveolar nitric oxide (CaNO) indicating peripheral airway inflammation remains unclear in gastroesophageal reflux-associated cough (GERC). Objectives: We aimed to characterize the changes in alveolar nitric oxide (CaNO) and determine its clinical implication in GERC. Design: This is a single-center prospective observational study. Methods: FeNOs at exhalation flow rates of 50 and 200 ml/s were measured in 102 patients with GERC and 134 patients with other causes of chronic cough (non-GERC). CaNO was calculated based on a two-compartment model and the factors associated with CaNO were analyzed. The effect of anti-reflux therapy on CaNO was examined in 26 GERC patients with elevated CaNO. Results: CaNO was significantly elevated in GERC compared with that in non-GERC (4.6 ± 4.4 ppb versus 2.8 ± 2.3 ppb, p < 0.001). GERC patients with high CaNO (>5 ppb) had more proximal reflux events (24 ± 15 versus 9 ± 9 episodes, p = 0.001) and a higher level of pepsin (984.8 ± 492.5 versus 634.5 ± 626.4 pg/ml, p = 0.002) in sputum supernatant than those with normal CaNO. More GERC patients with high CaNO required intensified anti-reflux therapy (χ 2 = 3.963, p = 0.046), as predicted by a sensitivity of 41.7% and specificity of 83.3%. Cough relief paralleled a significant improvement in CaNO (8.3 ± 3.0 versus 4.8 ± 2.6 ppb, p < 0.001). Conclusion: Peripheral airway inflammation can be assessed by CaNO measurement in GERC. High CaNO indicates potential micro-aspiration and may predict a necessity for intensified anti-reflux therapy.

Methods Ten patients with GERC,ten patients with GERD without cough and ten healthy volunteers were selected in the study.Cough reflex sensitivity test was carried out with capsaicin stimulation.The examination of lung functions and the cytology of induced sputum were performed.The cough thresholds to capsaicin,lung function parameters,total cell numbers and its category in induced sputum were analyzed.Moreover,levels of mast cell tryptase,histamine,prostaglandin D2,prostaglandin E2,eosinophil cationic protein,interleukin-8 and substance P were measued in the supernatants of induced sputum by ELISA.Results Patients with GERC had significant lower frequency of reflux-related symptoms when compared with patients with GERD without cough as indicated by RDQ(P0.001).Patients with GERC had more severe cough during day time than that at night time(P0.05).Cough thresholds C_2 and C_5 were significantly lower in GERC group than that in GERD without cough group(P0.05).Lung function parameters such as FEV_1,FVC,FEV_1/FVC and PEF had no significant differences among three groups.Although there were no significant differences observed in number of total cell and percentage of different category of inflammatory cells in the sputum,the levels of mast cell tryptase,prostaglandin E2 and substance P were elevated in the supernatant of induced sputum from GERC group when compared with GERD without cough group (P0.05).Concentrations of histamine,prostaglandin D2,eosinophil cationic protein and interleukin-8 were remained similar.More interestingly,there was an apparent negative correlation between Log C_5 and mast cell tryptas(r = - 0.571,P0.01) or substance P(r = - 0.569,P0.01).Conclusion GERC has an increased level of cough reflex sensitivity when compared with GERD without cough.Besides tachykinin in the airway,the activation of mast cell in the lower airway may involve in the pathogenesis of GERC.
Comparison of Cause Distribution Between Elderly and Non-Elderly Patients with Chronic Cough

Abstract:Background: The causes of chronic cough in elderly patients have not been specifically investigated. Therefore, it remains to be determined whether chronic cough differs between elderly and non-elderly patients. Objectives: To investigate the distribution of causes of chronic cough in elderly patients in comparison with etiologies of chronic cough observed in non-elderly patients. Methods: From 2,989 new patients presenting consecutively to the Department of Respiratory Medicine of the Tongji Hospital over a 1.5-year period, 287 patients with chronic cough were enrolled in the study. Patients aged ≥60 years were assigned to the elderly group. The elderly group comprised 104 patients and the non-elderly group the remaining 183 patients. The causes of cough were primarily evaluated according to a modification of Irwin’s anatomic diagnostic protocol which included induced sputum cytology, verified by the specific therapy. Results: Cough-variant asthma (34.6 vs. 41.5%) and upper airway cough syndrome (19.3 vs. 23.5%) were the most common causes of chronic cough both in the elderly and non-elderly groups. The distribution of causes and frequency were significantly different between elderly and non-elderly groups: angiotensin-converting enzyme inhibitor (ACEI)-induced cough (16.3 vs. 1.7%, χ2 value = 22.12, p < 0.001) and gastroesophageal reflux disease (GERD; 10.6 vs. 3.7%, χ2 value = 5.14, p = 0.02) were more common in the elderly group. Conclusions: A more frequent incidence of ACEI-induced cough and GERD is the distinctive feature in the cause distribution between elderly and non-elderly patients with chronic cough.

Abstract:OBJECTIVE:To evaluate the diagnostic value and limitation of multichannel intraluminal esophageal impedance and pH (MII-pH) monitoring on the diagnosis of gastroesophageal reflux-related chronic cough (GERC).METHODS:The patients with suspicious GERC consecutively referred to our respiratory clinic between May 2010 and July 2011 underwent a MII-pH monitoring, and received anti-reflux drug therapy, irrespective of the laboratory findings. Chronic cough due to gastroesophageal reflux was determined when there was a favorable response to anti-reflux therapy. Then, the sensitivity, specificity, false positive and negative rate, total consistence, positively and negatively predictive value, the area under the curve of ROC and the Kappa value of the laboratory investigation were calculated for the diagnosis of GERC.RESULTS:During the research period, 56 patients completed MII-pH monitoring. Among them, the abnormal reflux was found in 35 patients, and GERC was finally confirmed in 30 patients (85.7%) including 25 patients (83.3%) due to acid reflux and 5 patients (16.7%) due to non-acid reflux. In the remaining 21 patients with normal reflux episodes, 6 patients (28.6%) could be explained by non-acid reflux for their cough because of a relatively predominant weakly acid reflux and favorable response to empirical anti-reflux therapy. For the diagnosis of GERC, MII-pH monitoring had the sensitivity of 83.3%, the specificity of 75.0%, false positive rate of 25.0%, false negative rate of 16.7%, total consistence of 80.4%, positive predictive value of 85.7%, negative predictive value of 71.4%, the area under the curve of ROC of 0.792 and Kappa value of 0.577 respectively.CONCLUSION:MII-pH is a sensitive and reliable tool for the diagnosis of GERC due to its ability to detect both acid and non-acid reflux.

Abstract:Objectives: To elucidate the accuracy and advantages of Multichannel intraluminal impedance-pH monitoring (MII-pH) in diagnosing gastroesophageal reflux-induced chronic cough (GERC). Methods: The patients with suspected GERC were recruited and underwent MII-pH, GERC was confirmed by subsequent anti-reflux treatment despite the findings of MII-pH. Its diagnostic accuracy in identifying GERC were evaluated by receiver operating characteristic (ROC) analysis and compared with that of 24-h esophageal pH monitoring. Results: Among 158 patients completing both MII-pH and anti-reflux therapy, GERC was diagnosed in 136 patients, including acid GERC in 96 patients (70.6%), non-acid GERC in 30 patients (22.0%), neither one of both GERC in 10 patients (7.4%). For the identification of GERC, MII-pH presented with the sensitivity of 92.6%, specificity of 63.6%, positive predictive value of 94.0%, negative predictive value of 58.3% and area under ROC curve of 0.863, which was totally superior to 24-h esophageal pH monitoring. As the essential criteria of MII-pH, esophageal acid exposure time and symptom associated probability had a limited diagnostic value when used alone, but improved greatly the diagnostic yield when used in combination, even with a suboptimal efficacy. Conclusion: MII-pH is a more sensitive test for identifying GERC, but with a suboptimal diagnostic efficacy.

Abstract: To investigate the characteristics of esophageal dysmotility in patients with an initial diagnosis of acid/non-acid gastroesophageal reflux-related cough (GERC), and its correlation with the therapeutic response to anti-reflux treatments to search for the useful indicators to screen patients with chronic cough suitable for anti-reflux therapy. A total of 173 patients with suspicious GERC who attended the Chronic Cough Specialist Clinic of Department of Pulmonary and Critical Care Medicine, Tongji Hospital, School of Medicine, Tongji University between June 2020 and December 2022 were retrospectively selected for the study. The age of the patients was (45.1±14.6) years old, including 87 males and 86 females. Their demographic characteristics, clinical manifestations, and the results of high-resolution manometry (HRM) and multichannel intraluminal impedance-pH monitoring (MII-pH) were collected. Information on the etiological identification process and final diagnosis was also recorded. The recruited cases were grouped according to therapeutic outcomes and divided into cases with a favourable response to conventional anti-reflux treatment, cases with a favourable response to intensified anti-reflux treatment, and cases with no response to anti-reflux treatment. Factors influencing the efficacy of anti-reflux treatment were investigated. Differences between groups were compared using the test, Student-Newman-Keuls test, and Kruskal-Wallis () test, where applicable. Logistic regression analysis using forward stepwise regression based on maximum likelihood estimation was used to screen for influence factors. The 175 patients with suspicious GERC included 45 (26.0%) patients who responded to conventional anti-reflux treatment, 54 (31.2%) who responded to intensified anti-reflux treatment and 74 (42.8%) who did not respond to anti-reflux therapies. Esophageal dysmotility was present in 52.0% of patients (90/173), but was less common in patients who responded to conventional anti-reflux treatment (=8.09, 0.018). Although the majority of reflux episodes were non-acid (136/173, 78.6%), the proportion of acid reflux (=19.49, <0.001) and acid exposure time (11.04, 0.004) were significantly higher in patients who responded to conventional anti-reflux treatment. The patients with acid and non-acid GERC had comparable proportion of esophageal dysmotility (64.9% 48.5%, =3.11, 0.078), with a shorter break [2.4 (0.7, 5.6) cm 6.1 (1.4, 10.0) cm, -2.39, 0.017], longer upper esophageal sphincter [(4.1±0.9) cm (3.7±1.3) cm, -2.09, 0.038], higher percentage of normal esophageal contractions [60.0% (17.8%, 90.0%) 30.0% (0, 80.0%), -2.14, 0.032], and lower percentage of large break [10.0% (0, 40.0%) 50.0% (0, 100.0%), -2.92, 0.004] in the patients with non-acid GERC. The mean resting pressure of the lower esophageal sphincter was significantly lower (7.49, 0.024), while the percentage of ineffective esophageal contractions was markedly higher (8.60, 0.014) in the patients who responded to intensified anti-reflux treatment and in the patients who did not respond to the anti-reflux therapies. Multifactorial logistic regression analysis identified the percentage of ineffective contraction as an independent factor predicting the efficacy of conventional anti-reflux treatment, with a cut-off value of≤45% and a moderate predictive value (AUC=0.67, 0.004). Esophageal dysmotility is common in GERC patients with different characteristics in acid and non-acid GERC. The percentage of ineffective esophageal contraction may be a useful indicator for selecting an anti-reflux strategy and predicting treatment outcomes.

Abstract:Gastroesophageal reflux-related cough is a multidisciplinary disease that cannot be diagnosed solely based on typical reflux-related symptoms. Its current diagnostic methods and criteria are largely derived from those used for gastroesophageal reflux disease, with slight differences. Esophageal reflux monitoring can provide objective evidence for the diagnosis of gastroesophageal reflux-related cough and is therefore the first-choice of laboratory tests recommended by the guidelines for cough management. Acid exposure time and syndrome association probability have been accepted as the diagnostic criteria, while esophageal motility assessment also has some certain auxiliary diagnostic value. Based on the existing evidence, we have reviewed how to improve the diagnostic methods and criteria for gastroesophageal reflux-related cough, as well as the issues that need to be addressed in the future.

Abstract:Background: There is a need to find a simple, non-invasive and effective diagnostic tool for diagnosing gastroesophageal reflux-induced chronic cough (GERC) in clinic. This study aimed to evaluate the predictive diagnostic value of Hull airway reflux questionnaire (HARQ) and its combination with gastroesophageal reflux disease questionnaire (GerdQ) for GERC. Methods: Chronic cough patients were enrolled and the diagnosis of GERC was established according to the chronic cough diagnosis and treatment process. The diagnostic value of HARQ and GerdQ alone or the combination of HARQ and GerdQ was analyzed. Results: A total of 402 patients with chronic cough were eventually enrolled, including 166 GERC patients. When the HARQ score was used to predict the diagnosis of GERC, the area under the ROC curve was 0.796. The sensitivity and specificity were 77.19% and 77.06%, respectively. When the GerdQ was used to predict the diagnosis of GERC, the area under the ROC curve was 0.763. The sensitivity and specificity were 70.18% and 76.15%, respectively. When HARQ combined with GerdQ were used to predict the diagnosis of GERC, the area under the ROC curve was 0.848. The sensitivity and specificity were 77.19% and 79.82%, respectively. Conclusions: HARQ used to evaluate the cough hypersensitivity has a certain predictive diagnostic value for GERC. The diagnosis of GERC should be considered when the HARQ score is >24. The predictive diagnostic value of the combination of HARQ and GerdQ is significantly higher, which makes the diagnosis of GERC simpler, quicker and more effective.

Abstract:Background:When combined with 24-hour esophageal pH monitoring,intraluminal impedance technique can detect various types of gastroesophageal reflux events and determine the physical and pH properties of the refluxate.Aims:To study the reflux characteristics of gastroesophageal reflux disease(GERD) patients presenting with chronic cough by 24-hour esophageal pH-impedance monitoring.Methods:Seventy-three consecutive chronic cough patients with normal chest radiography and without typical gastroesophageal reflux symptoms were enrolled.Among them,those with normal endoscopic findings and fulfilling the GERD criteria by pH-impedance monitoring were defined as GERD-chronic cough group.Twenty-one healthy volunteers were recruited as normal controls.The parameters of pH-impedance monitoring were compared between the two groups.Results:Using pH monitoring only,17 patients were diagnosed as GERD;however,26 patients were diagnosed as GERD when pH-impedance monitoring was used.The GERD-chronic cough group was characterized by gas/liquid mixed reflux(46.3%) and weakly acidic reflux(59.9%),while normal control group was characterized by gas reflux (68.9%) and weakly acidic reflux(59.1%).Parameters including DeMeester score and episodes of gas reflux,acid reflux and proximal reflux etc.were significantly different between the two groups(P<0.05).Conclusions:Gas/liquid mixed reflux and weakly acidic reflux are predominant in GERD patients presenting with chronic cough.Pathological refluxes are commonly seen in both upright and supine positions,and proximal reflux is more frequent than that in healthy adult.24-hour esophageal pH-impedance monitoring is of important diagnostic value in chronic cough patients with suspected GERD.
