Abstract:Objective: To investigate the clinical manifestation and follow-up of melanocytoma of the optic disc. Methods: Retrospective case series study was taken to study the clinical data of the cases with melanocytoma of the optic disc at Peking Union Medical College Hospital from 1978 to 2007. The examination included direct and indirect ophthalmoscopy, fundus photography, fundus fluorescein angiography (FFA), fundus indocyanine green angiography (ICGA), optical coherence tomography (OCT) and B-scan. The clinical manifestation and the outcome in follow-up period were analyzed. Results: Ten cases aged 31-58 years (mean 43 years) were included in this study. Among them, 9 cases were male, 1 case was female. All cases were unilateral involved, 5 cases were right eye, 5 cases were left eye. The distribution of the visual acuity was follows: light perception in 2 eyes, 20/200 in one eye, 20/20 or above in 7 eyes. The tumor mass at the optic disc with extensive serous detachment of retina was detected on B-scan in one eye. The benign melanocytoma was diagnosed by vitreous fine needle biopsy for this eye. In other case, the tumor was found to be enlarged, visual field defect deteriorated, pigment granules appeared in the vitreous adjacent to the tumor during on year follow-up period. On the patient request, enucleation was performed to rule out the malignancy of the tumor. Pathologic study showed that the tumor cells were active, but benign state. The tumor size and visual acuity in other eight eyes remained stable in the follow-up period with one to 18 years (mean 3.5 years). On FFA and ICGA, bilateral choroidal nevi were found in one eye. Conclusions: Melanocytoma of the optic disc is a benign tumor that rarely causes visual impairment. However, visual acuity may be impaired due to enlargement of the tumor or necrosis in tumor. It is important to follow-up every case with melanocytoma of the optic disc as long as possible.
Vitreous metastasis from cutaneous melanoma: diagnosis and management

Abstract:Purpose: To report the clinical findings, treatments, and outcomes in a series of patients with vitreous metastasis from cutaneous melanoma. Methods: This single-center, retrospective, interventional case series included patients with biopsy-confirmed vitreous metastasis from cutaneous melanoma diagnosed between 1997 and 2020. Standard 23- or 25-gauge pars plana vitrectomy was performed for diagnostic sampling. Sclerotomies were treated with double or triple freeze-thaw cryotherapy. Perioperative intravitreal injections of melphalan (32 µg/0.075 mL) were administered, when indicated. Visual acuity, intraocular pressure, and systemic and ocular treatment responses were reported. Results: Five eyes of five patients with unilateral vitreous metastasis from cutaneous melanoma were identified. The median age at diagnosis was 84 (range, 37-88) years. The median follow-up after ophthalmic diagnosis was 28 (8.5-36) months; one patient did not have a follow-up. The initial visual acuity ranged from 20/30 to hand motions. Baseline clinical findings included pigmented or non-pigmented cellular infiltration of the vitreous (5/5), anterior segment (4/5), and retina (3/5). Four patients had secondary glaucoma. Systemic therapy included checkpoint inhibitor immunotherapy (n=3, all with partial/complete response), systemic chemotherapy (n=2), surgical resection (n=3), and radiation (n=2). The median time from primary diagnosis to vitreous metastasis was 2 (2-15) years. One patient had an active systemic disease at the time of vitreous metastasis. The final visual acuity ranged from 20/40 to no light perception. Ophthalmic treatment included vitrectomy in all five patients, intravitreal administration of melphalan in three, and intravitreal administration of methotrexate in one. One patient required enucleation, and histopathology revealed extensive invasion by melanoma cells. Conclusions: Vitreous metastasis from cutaneous melanoma can present as a diffuse infiltration of pigmented or non-pigmented cells into the vitreous and may be misdiagnosed as uveitis. Diagnostic pars plana vitrectomy and periodic intravitreal chemotherapy may be indicated.

Abstract:Purpose: To report an atypical case of a patient with symptomatic retinocytoma associated with diffuse calcified vitreous seeds. Methods: Retrospective chart review. Results: A 46-year-old healthy woman presented with a history of floaters in the right eye for several months. She had been referred for abnormal findings in the retina and vitreous on routine examination. Visual acuity was 20/20. An incidental retinocytoma associated with extensive calcified vitreous seeding was observed. Enhanced depth optical coherence tomography showed an absence of normal retinal layers with numerous cystoid cavities throughout the lesion. High-resolution 20-MHz posterior B-scan ultrasonography demonstrated that the calcified vitreous seeds emanated from the peaked portion of the retinal tumor. Conclusion: Calcified vitreous seeding is a rare finding associated with retinocytomas. Enhanced depth imaging optical coherence tomography and high-resolution B-scan ultrasonography may be useful tools in the diagnosis of this uncommon retinal tumor.
Delayed Closure of Macular Hole Secondary to Terson Syndrome after Vitrectomy A Case Report and Literature Review

Delayed closure of persistent MH after pars plana vitrectomy (PPV) is occasionally reported in literature, none of them is MH secondary to Terson syndrome. We describe a case of MH secondary to Terson syndrome and delayed closure occurred after PPV, and we also study the characteristics of delayed closure of persistent MH by reviewing related literatures. Patient concerns: A 61-year-old man presented with vitreous hemorrhage in right eye following a subarachnoid hemorrhage due to spontaneous rupture of his right vertebral artery dissecting aneurysm. The visual acuity was hand motion in the right eye and 20/30 in the left eye. Fundus examination showed dense and diffuse vitreous hemorrhage in the right eye. Diagnoses: Terson syndrome was diagnosed according to his subarachnoid hemorrhage history and vitreous hemorrhages in right eye. Interventions: PPV combined with phacoemulsification and intraocular lens implantation was performed in his right eye, and internal limiting membrane (ILM) peeling was also performed due to a MH noted during the surgery. Outcomes: One week after PPV, optical coherence tomography (OCT) showed a persistent MH. Without any intervention, the MH became smaller and flattened, with the best corrected visual acuity (BCVA) improved to 30/200 at 1 month after surgery. Six months later, the MH completely closed with BCVA improved to 20/40. According to our literature review, there are 8 cases of the delayed MH closure, which includes idiopathic MH (4 eyes), traumatic MH (2 eyes), and vitreomacular traction (2 eyes). There is no report about delayed closure of MH secondary to Terson syndrome. The times for these delayed closure occurred following PPV were ranged from 1 to 28 months. Holes even with obviously raised edges after PPV may spontaneously close, just like the case presented here. Lessons: Delayed closure of persistent MH after PPV is rarely reported. The significance of this case is to suggest that similar patients should be monitored carefully by OCT, and additional surgery for the MH may be delayed, since delayed closure is possible. The exact mechanisms of delayed closure of persistent MH still need to be clarified.

Malignant melanoma complicated with cataract and secondary glaucoma: A case report

Abstract:Uveal melanoma is the most common intraocular malignant tumor in adults. For patients presenting with cataracts and glaucoma, it is recommended to assess whether an intraocular lesion is present as the primary cause. The present study describes the case of a 52-year-old man with primary intraocular malignant melanoma. The patient experienced painless vision loss in the right eye for 1 year, with recent onset of eye swelling and pain in the week prior to seeking medical attention. A slit-lamp examination revealed a shallow anterior chamber in the right eye, a visibly opaque lens and a faint reflection of the tumor surface in the vitreous humor. In addition, the intraocular pressure of this eye was >60 mmHg. Magnetic resonance imaging revealed a large tumor behind the lens measuring 16×18×14 mm. Pathological examination confirmed the diagnosis of malignant melanoma. No BRCA-associated protein-1 somatic mutation was detected, whereas germline mutations of MutL protein homolog 1, RAD54 like, and SWI/SNF related, matrix associated, actin dependent regulator of chromatin, subfamily a, member 4 were identified. Extensive systemic examination excluded the possibility that the tumors originated from another part of the body. The present case report highlights the crucial role of slit-lamp examination in the detection of ocular tumors. It is advocated that for patients presenting with cataracts, attention should be paid to the possibility of intraocular tumors. Meticulous slit-lamp microscopy may reveal a reflection of the surface of a malignant melanoma, preventing misdiagnosis.
Melanoma Arising Beneath the Lateral Rectus Muscle in a Teenager With Ocular Melanocytosis: Possible Origin From Intrascleral Melanocytes

Abstract:Ocular melanocytosis is a well-established risk factor for choroidal melanomas but, despite its reported associations in the literature, it is infrequently discussed in relation to orbital melanomas. The authors describe a teenage patient with ocular melanocytosis who presented with an asymptomatic ipsilateral right orbital mass associated with the lateral rectus muscle. An exploratory orbitotomy revealed a lesion lightly adherent to the underlying sclera. Histopathology demonstrated a markedly atypical epithelioid melanocytic proliferation, bound by a thin rim of superficial sclera, implying an origin from intrascleral melanocytes, likely within an emissary canal. Next-generation sequencing identified GNAQ and NF1 mutations. The histopathology and molecular genetics designated the lesion as having a uveal melanoma-like profile, suggesting that it may behave as a choroidal melanoma. This case underscores the importance of the association between ocular melanocytosis and orbital melanoma and provides additional evidence for primary orbital melanoma etiopathogenesis.

Abstract:Background/Aims: The presence of a posterior vitreous detachment (PVD) may play a role in the development of severe retinal toxicity following intravitreal melphalan (IVM) injection for vitreous seeding. We aimed to evaluate the incidence of PVD in retinoblastoma eyes and its association with retinal toxicity after IVM. Methods: We reviewed 112 eyes of 81 retinoblastoma patients with B-scan images available for review from 2010 to 2017. A cohort with vitreous seeding treated with IVM was compared to a cohort that did not undergo injection. The primary outcome measure was the presence of PVD at diagnosis and after treatment. Secondary measures included IVM-associated retinal toxicity and other ocular complications. Results: The incidence of PVD was 20% at diagnosis, and in eyes with B-scans available both at diagnosis and after treatment 18% of eyes developed a PVD over the course of therapy, more frequently after IVM (p = 0.05). Of 34 eyes receiving IVM treatment, the incidences of posterior segment toxicity and globe salvage were similar between eyes with and without PVD (p = 0.4015 and 0.52, respectively). Conclusion: In this cohort of patients, there did not appear to be an association with the presence of PVD during IVM and the development of retinal toxicity.
Surgical management of a case of double optic disc pits with maculopathy

Abstract:Background: Double optic disc pit maculopathy is a rare entity. It can be difficult to manage because of excessive leakage and chronic maculopathy. Purpose: To describe surgical management in a case of double optic disc pits with maculopathy. Synopsis: A 42-year-old male presented with double optic disc pits with macular detachment in the left eye. The best-corrected visual acuity (BCVA) was 20/60, N12. Preoperative OCT showed the presence of two disc pits. The macular region had large retinoschisis and subretinal fluid (SRF) with a central foveal thickness of 879 microns and loss of the ellipsoid zone. A shallow communication from the temporal aspect of the disc to the submacular area was also noted. Among the options of observation, laser photocoagulation, and surgery, the patient opted for surgical management. Surgical technique: A standard-3 port 23-gauge pars plana vitrectomy was done. After staining the ILM with brilliant blue, ILM peeling was done with the help of forceps and Finesse loop. ILM flaps were inverted over to cover the optic disc pits and sealed with a drop of fibrin glue. Next, 20% SF6 gas was used for tamponade. Pre- and post-surgery parameters such as visual acuity and OCT were evaluated. Postoperative evaluation: After 6 weeks, left eye BCVA was 20/40 with OCT showing reduced SRF and reduced intraretinal schisis with a foveal thickness of 546 microns. At 3 months of follow-up, the vision in the left eye had improved to 20/30 with further reduction in the retinoschisis and foveal thickness of 482 microns. Highlights: In this interesting case, we demonstrate a unique way of sealing the defect surgically by vitrectomy and inverted ILM flap with fibrin glue over the disc pits. Abstract:Free-floating, pigmented vitreous cysts were documented in two patients. In a 15-year-old girl with intermittent symptoms, a 2.4-mm cyst was observed; origin was attributed to prior trauma, and clinical observation was pursued. In a 35-year-old woman with progressive symptoms, a 11.5-mm cyst was observed; origin was attributed to a history of multiple ocular surgical interventions, and surgical excision by pars plana vitrectomy was performed. Intravitreal Melphalan for Vitreous Seeds: Initial Experience in China

Abstract:Purpose. To evaluate the efficacy of intravitreal melphalan for vitreous seeds from retinoblastoma in Chinese patients. Methods. This is a retrospective review of 17 consecutive Chinese patients (19 eyes) with viable vitreous seeds from retinoblastoma. The patients received multiple intravitreal injections of 20 ug melphalan. Results. The International Classification of Retinoblastoma groups were B in 1 eye, C in 5 eyes, D in 11 eyes, and E in 2 eyes. On average, 6 injections (range: 1–15) were given to each eye at the interval of 2–4 weeks. Successful control of vitreous seeds was achieved in 16 of 19 eyes (84.21%). Globe retention was achieved in 73.68% (14/19) eyes. The patients were followed up for 27 months on average (median: 26; range: 17–42 months). There is a significant difference in response to intravitreal melphalan for cloud, spheres, and dust seeds with a median number of injections of 9, 6, and 3, respectively (P=0.003). Complications related to intravitreal melphalan included vitreous hemorrhage, cataract, salt-and-pepper retinopathy, and pupil posterior synechia. There was no case of epibulbar extension or systemic metastasis within the period of follow-up. Conclusion. Intravitreal melphalan achieved a high local control rate for vitreous seeds without extraocular extension and with acceptable toxicity in Chinese retinoblastoma patients.
Optic Disk Hemorrhage And Vitreous Hemorrhage After Phacoemulsification In A Normal Tension Glaucoma Patient A Case Report

Abstract:Rationale: Optic disk hemorrhage has been closely correlated with glaucoma for its development and progression. Phacoemulsification surgery results in large intraocular pressure (IOP) fluctuation. We report a case of optic disk hemorrhage and consequently progressive vitreous hemorrhage after an unsuccessful phacoemulsification surgery in an advanced normal tension glaucoma (NTG) patient.Patient concerns: An advanced NTG patient of 82 years old with chronic hypertension underwent an unsuccessful phacoemulsification surgery complicated by posterior capsule rupture. During the postoperative 2 weeks, recurrent episodes of fresh hyphema occurred and B ultrasonography scan revealed the progressive vitreous hemorrhage. The IOP went out of control under the maximum tolerable IOP-lowering medications.Diagnosis: Vitreous hemorrhage after phacoemulsification in an advanced NTG patient.Interventions: Vitrectomy was performed to search for the cause of the progressive vitreous hemorrhage. After removal of the thick vitreous hemorrhage, a fresh spot of optic disk hemorrhage was detected at the nasal margin of the significantly-cupping disk.Outcomes: Postoperatively, the hemorrhagic vitreous opacity gradually resolved and the IOP remained stable at 10 similar to 13mmHg with topical prostaglandin analogue drops. Fundus examination revealed the dilated disk vessel with localized angiomatous change at the nasal disk margin.Lessons: Severe optic disk hemorrhage may occur after phacoemulsification in advanced glaucoma patients. Systemic vascular factors, such as chronic hypertension and old age, and surgical complications, such as posterior capsule rupture and postoperative IOP elevation, would further increase the risk. For phacoemulsification in advanced glaucoma cases, extra care should be taken to control intraoperative IOP fluctuations and monitor postoperative IOP.

IntroductionCongenital arterial peripapillary loops are rare entities and very few cases are described in literature.Case descriptionA 25-year-old Asian man presented a diffuse vitreous hemorrhage in his Left Eye (LE). OCT-A revealed the presence of bilateral vascular loops at the optic nerve head. Fluorescein angiography (FA) confirmed the vascular abnormality in both eyes, with arterial filling in early phases and no dye leakage. At twenty days of follow up, the vitreous hemorrhage in the LE completely reabsorbed and BCVA improved from 20/63 to 20/20.ConclusionCongenital peripapillary loops should be considered in the differential diagnosis of vitreous hemorrhage, especially in young patients with no history of ocular/head trauma. Multimodal imaging is highly recommended to properly manage the patients, avoiding unnecessary therapeutic choices.

Abstract:According to Reese-Ellsworth Classification for retinoblastoma, retinoblastoma with vitreous seeding is grade 5B with poor prognosis and was initially proposed for enucleation. Chemotherapy combined with local treatment for retinoblastoma has been proposed as conservative treatment.1 Because of the lack of blood supply to vitreous seeding, the chemotherapy drugs could not reach the vitreous body through blood-retinal barrier. This has been one of the important limiting factors in the treatment of children with retinoblastoma with vitreous seeding. Melphalan proved to be one of the 12 kinds of chemosensitive drugs for anti-retinoblastoma cell colony formation. 2 So, we used intravitreal melphalan injection to observe the therapeutic effect on the vitreous seeding. A 6-year old male child was admitted for whitening of left pupil for 6 years regularly and extensively pretreated with 4 courses of cryotherapy, 17 cycles of chemotherapy, two courses of selective intra-arterial chemotherapy, and laser photocoagulation during follow-up. In February 2011, he was found to have vitreous seeding of retinoblastoma. Ophthalmologic examination: visual acuity: Left eye: 0.1, intraocular pressure (IOP) 12 mmHg, the anterior segment was normal, three spots of whitish seeding was seen at the upper temporal region of vitreous cavity (Figure A) about 1/2 disc diameter (DD) × 1/2 DD, around 10 spots of grayish white satellite lesions, 5 DD above optical disc a yellow whitish tumor was seen, diameter of 6 DD × 5 DD, superficially calcified, avascular. From 2011 May to 2011 July, intravitreal injection of melphalan with anterior chamber puncture procedure (left) was done. It was given every two weeks for six times each dose of 20 μg/0.05 ml.Figure 1.: Fundus photograph. A: Three spots of seeding were seen at the upper temporal region of vitreous cavity (arrows). B: After 12 month follow up, no vitreous seeding was seen (arrow).After the 3rd injection, the seeding in vitreous cavity started decreasing. After the 6th injection, visual acuity of left eye: 0.1, IOP: 11 mmHg, and no vitreous seeding was seen. Twelve months follow-up showed no sign of recurrence of vitreous seeding. The intravitreal injection was proceeded, according to the Kaneko3 surgical methods and dose. After six courses of intravitreal melphalan injection, 12 months follow-up did not show any recurrence of tumor, disappeared vitreous seeding and no new invasions were seen(Figure B), no significant change in vision. Thus, we can see that intravitreal injection of melphalan is safe and effective eradication of the vitreous seeding of retinoblastoma. Intravitreal injection of melphalan can inhibit tumor vascular supply to the fundus of retinoblastoma is not yet observed in this case, and large number of clinical trials and evidence are still needed for support.
Idiopathic Pigmented Vitreous Cyst Without Autofluorescence: a Case Report.

Abstract:BACKGROUND:Vitreous cysts are rare clinical findings and seldom cause visual disturbance. They are generally classified as congenital or acquired and are considered idiopathic when the etiology can not be determined. A previous electron microscopic observation on an idiopathic pigmented vitreous cyst has confirmed its pigment epithelial origin. However, the specific kind of pigment epithelium involved remains unclear.CASE PRESENTATION:A 39-year-old female presented with a round-shaped floater causing frequent visual disturbance in the left eye. A pigmented, non lobulated and freely mobile vitreous cyst was observed in the anterior vitreous by slit lamp examination and anterior segment optical coherence tomography. The pigment clumps on the cyst wall showed no autofluorescence. No persistent hyaloid artery or connection between the cyst and ocular structures was found by fundus fluorescein angiography and B-scan ultrasound. Serum tests for cysticercoids, sparganosis and toxoplasma were negative. A diagnosis of idiopathic vitreous cyst was made and no intervention was given. The cyst sank to the inferior part of the vitreous and the patient felt less visual disturbance during one-year follow-up.CONCLUSIONS:We described the features of a pigmented vitreous cyst revealed by autofluorescence and anterior segment optical coherence tomography for the first time. The intact retina, the absence of lipofuscin of the cyst and its location in the anterior vitreous led to the hypothesis that the cyst may originate from the ciliary pigment epithelium rather than the retinal pigment epithelium.
Vitrectomy for macular hole following Nd:YAG laser injury

Abstract:The Q-switched Nd:YAG laser can cause significant ocular injury, because it can emit an invisible and powerful load of energy in a short period of time. One of these injuries is macular hole formation. We report the case of a 21-year-old woman who presented with acute floaters in her left eye after exposure to a Q-switched Nd:YAG laser. Her initial best-corrected visual acuity (BCVA) in the left eye was 20/80. Examination demonstrated an acute vitreous hemorrhage, and spectral-domain optical coherence tomography (SD-OCT) confirmed a full-thickness macular hole (FTMH). Four months after her injury, her BCVA deteriorated to 20/400, and she underwent vitrectomy, internal limiting membrane peeling, and gas injection. Three weeks following the procedure, her FTMH was closed, however, her BCVA remained 20/150. At her last office visit, 19 months after the surgery, the examination was unchanged. There are few reports of vitrectomy to close an FTMH after Nd:YAG laser exposure, and the factors that contribute to visual recovery remain unclear. The aim of this case report is to emphasize the importance of early diagnosis and surgical repair, and to review the literature and surgical outcomes of cases of Nd:YAG laser-induced macular holes. Imaging with SD-OCT was essential in evaluating the visual outcome, as it was dependent on the degree of photoreceptor and retinal pigment epithelium injury.
Vitrectomy for Localized Vitreous Seeds of Retinoblastoma in an Only Eye

Abstract:Vitreous seeds of retinoblastoma are one of the main reasons for treatment failure using globe-conserving therapy due to their poor response to therapy. This is most likely secondary to the difficulty for chemotherapeutic drugs to permeate into the vitreous cavity due to the bloodeye barrier.1 Currently, laser coagulation or cryotherapy is not used for vitreous seeds. External beam radiotherapy has been seldom used for vitreous seeds due to radiationrelated complications.2 Recently, several reports have showed repeated intravitreal injections of anticancer drugs, such as melphalan, may be an option for vitreous seeds.3 However, long-term efficacy of this approach is unsure and this treatment is unavailable at many treatment centers. Furthermore, repeated intravitreal injections entail an increased risk of extraocular dissemination of tumor cells. With the advances in technology and equipment for vitrectomy, the indications for surgery have also expanded. Vitrectomy has been performed to manage complications of conservative treatment for retinoblastoma, including rhegmatogenous retinal detachment, tractional retinal detachment with vitreous and epiretinal membranes, and vitreous hemorrhage.4 Here, we described a case of localized vitreous seeds of retinoblastoma that was successfully treated with vitrectomy. The patient described in this case was diagnosed with bilateral retinoblastoma at 6 months of age. The right eye was classified as group E according to the International classification of Retinoblastoma. The left eye was classified as group B. Enucleation had been performed on the right eye. The tumor in superior retina of the left eye had regressed to flat scar and calcification after four concurrent cycles of vincristine, etoposide, and carboplatin chemotherapy, four sessions of cryotherapy, and two sessions of laser coagulation. At 16 months of follow-up, one vitreous seed was noted (Figure 1A). Scarred tumor in the upper peripheral retina of the left eye was unchanged. However, increased vitreous seed growth had been noted during the subsequent followup of 6 months (Figure 1B). The patient underwent 23-gauge vitrectomy under wide field viewing for vitreous seeds after the consent was obtained. During the surgery, the localized vitreous seeds were removed first. To prevent vitreous seed movement due to the flow of irrigation fluid, we removed the vitreal seeds by vitreous cutter with dry vitrectomy technique. Then, core vitrectomy was regularly carried out. When vitrectomy for peritumoral vitreous cortex was performed, a cut rate of 2500 r/min and vacuum of 0-40 mmHg were set in order to avoid disturbance of tumor. Finally, 12% perfluoropropane gas tamponade was performed. The sclerotomies were closed using an 8-0 vicryl suture. During postoperative follow-up at 26 months, the tumor was stable and no vitreous seeds were noted (Figure 1C). There were no signs of orbital or systemic metastasis. The best-corrected visual acuity of the left eye was 120/200.Figure 1.: Fundus photograph of the left eye. C: The tumor in the upper peripheral retina remained quiescent with no vitreous seeds 26 months after vitrectomy.Vitrectomy is an aggressive modality for mechanical removal of intraocular retinoblastoma due to the risk of tumor cell dispersion out from the sclerotomies and recurrence of intraocular tumor. It should be performed with caution. We recommend the following: (1) retinal tumor has been stable for enough time before vitrectomy. Previous reports have shown a decreased incidence of recurrence and metastatic disease in patients undergoing vitrectomy who had maintained tumor quiescence for a longer period.4 (2) A limited number of vitreous seeds. Localized vitreous seeds can be removed completely in visualization. Moderate to severe vitreous seeds will increase the risk of extraocular dispersion of tumor cells during surgery and incomplete removal of seeds. (3) The operation should be carried out by experienced pediatric vitrectomy surgeons. Vitrectomy in pediatric eyes presents unique challenge and is often different from that in adults. Familiarity with pediatric vitrectomy will lower complications of vitrectomy. Some complications such as retinal detachment, proliferative vitreoretinopathy, and vitreous hemorrhage require revitrectomy which in turn would increase the risk of recurrence and metastasis. Vitrectomy for vitreous seeds of retinoblastoma is different from that for noncancerous retinal disease. Some points should be kept in mind during the procedure. Pay attention to reduce the frequency of intraocular instrument exchange for minimizing intraocular fluid flow through the sclerotomies throughout the surgical procedure. Vitreous seeds should be removed at the beginning of the surgical procedure. Otherwise, vitreous seeds may float within the vitreous cavity and disperse to some invisible zones such as ciliary process and posterior surface of the iris due to intraocular fluid flow, resulting in incomplete removal of vitreous seeds. We recommend intraocular irrigation with anticancer drugs such as melphalan during vitrectomy, although it is not used in our patient. Optimal concentration of anticancer drugs in the vitreous cavity and subconjunctiva around the sclerotomies will reduce the risk of metastasis of tumor cells.5
Analysis of the Etiologies, Treatments and Prognoses in Children and Adolescent Vitreous Hemorrhage

Abstract:AIM: To determine the etiologies, treatment modalities and visual outcomes of vitreous hemorrhage (VH; range from birth to 18y). METHODS: A total of 262 eyes from 210 patients between January 2010 and September 2016 were included. All children underwent an appropriate ocular and systemic examination. Data collected included demographics, clinical manifestations, details of the ocular and systemic examination, management details, final fundus anatomy and visual acuity (VA). RESULTS: The most common etiologies were non-traumatic VH (64.89%), most of which were due to retinopathy of prematurity (ROP; 37.10%); while traffic accidents, including 16 (21.00%) eyes, was the most common ocular traumas. Surgery, performed in 143 (54.58%) eyes, was the most common management modality. The initial mean baseline visual acuity was 2.77±0.21 logarithm of the minimal angle of resolution (logMAR) in children and adolescent with traumatic VH, which was significantly improved to 2.15±1.31 logMAR (P<0.05). CONCLUSION: VH in children and adolescent has a complicated and diverse etiology. ROP is the primary cause of non-traumatic VH, which is the most common etiology. Appropriate treatment of traumatic VH is associated with obvious improvement in visual acuity. The initial VA is one of most important predictors of outcome.

Abstract:A 63-year-old male presented with a round-shaped floater and visual obscuration in the right eye. Clinical evaluation showed a nonpigmented vitreous cyst connected to a lattice degeneration by a stalk. Immunostaining of the vitreous cyst obtained from vitrectomy showed its origin of retinal neuroepithelium. The cyst was formed by continuous vitreous traction, which might tear up the disrupted retina at the area of lattice degeneration. Spontaneous and Surgical Resolution of Maculopathy in Bilateral Optic Disc Pit After Posterior Vitreous Detachment: A Case Report

Abstract:Purpose: To report a case of bilateral optic disc pit (ODP) maculopathy using a different treatment approach. Methods: Observational case report. Results: A 17-year-old male presented with blurred vision in both eyes. His visual acuity was 20/400 in the right eye and 20/200 in the left eye. Fundus examination revealed bilateral temporal ODP and maculopathy. Optical coherence tomography scans showed severe intraretinal schisis-like changes, subretinal fluid, and parafoveal vitreomacular traction due to posterior hyaloid thickening in both eyes. Pars plana vitrectomy, in combination with internal limiting membrane peeling and gas tamponade, was performed on his left eye. A conservative approach was preferred due to high myopia and milder maculopathy in the right eye. Regressed maculopathy was found after spontaneous posterior vitreous detachment in the right eye. Conclusion: Our case report may contribute to the knowledge regarding the role of vitreomacular abnormalities in the pathogenesis of ODP maculopathy.
