Abstract:BACKGROUND:Acute pancreatitis (AP) is the most common pancreatic disease. Predicting the severity of AP is critical for making preventive decisions. However, the performance of existing scoring systems in predicting AP severity was not satisfactory. The purpose of this study was to develop predictive models for the severity of AP using machine learning (ML) algorithms and explore the important predictors that affected the prediction results.METHODS:The data of 441 patients in the Department of Gastroenterology in our hospital were analyzed retrospectively. The demographic data, blood routine and blood biochemical indexes, and the CTSI score were collected to develop five different ML predictive models to predict the severity of AP. The performance of the models was evaluated by the area under the receiver operating characteristic curve (AUC). The important predictors were determined by ranking the feature importance of the predictive factors.RESULTS:Compared to other ML models, the extreme gradient boosting model (XGBoost) showed better performance in predicting severe AP, with an AUC of 0.906, an accuracy of 0.902, a sensitivity of 0.700, a specificity of 0.961, and a F1 score of 0.764. Further analysis showed that the CTSI score, ALB, LDH, and NEUT were the important predictors of the severity of AP.CONCLUSION:The results showed that the XGBoost algorithm can accurately predict the severity of AP, which can provide an assistance for the clinicians to identify severe AP at an early stage.

Abstract:Objective To evaluate the relationship between heart rate variability (HRV) and serum inflammatory cytokines (TNF-α,IL-6),procalcitonin (PCT) and acute physiology and chronic health evaluation (APACHE) Ⅱ score in severe acute pancreatitis (SAP) patients.Methods Forty-one SAP patients admitted to Department of General Surgery,Department of General Surgery,General Hospital of Nanjing Military Area Command within 72 hours of symptoms onset were included in this study.Five-minite ECG signals were obtained within 24 h of admission for HRV analyses.At the same time,blood sample were obtained,then TNF-α,IL-6,PCT were determined.APACHE Ⅱ score were recorded within 24 h of admission.Results Among the 41 SAP patients,there were 22 male and 19 female with a median age of 47 years old.The etiologies were as follows,20 biliary cases,7 alcoholic cases,9 hyperlipidemia cases and 5 other cases.The serum levels of TNF-α,IL-6,PCT were 455 μg/L (301.5-790.0 μg/L),334 μg/L(206.5-735.0 μg/L),0.3 pg/L(0.1-1.2 pg/L).The median APACHE Ⅱ score was 14 (range 11-18).The TP,LF,HF of HRV was 205.6(84.9-921.0),52.9(6.75-125.8),11.0(4.1-40.9) (ms × ms) ; and the nLF,nHF was 36.6 (13.7-64.3),10.2 (4.2-25.0) nu,while the LF/HF was 3.4 (1.7-6.8).The level of nHF was positively correlated with TNF-α,IL-6,PCT(r =0.849,0761,0.480,p ＜ 0.01).The level of LE/HF was negatively correlated with TNF-α,IL-6,PCT(r =-0.525,-0.708,-0.325,p ＜ 0.01 or 0.05).nHF was positively correlated with APACHE Ⅱ score (r =0.346,P ＜ 0.05).However,LF/HF was not correlated with APACHE Ⅱ score.nLF was not correlated with other parameters.Conclusions Heart rate variability is closely correlated with TNF-α,IL-6 and PCT.

Abstract:Objective To evaluate the clinical value of heart rate variability change in the patients with pancreatic cancer. Methods The clinical data of 518 cases of pancreatic cancer admitted from January 2008 to May 2013 in the Second Affiliated Hospital of Zhejiang University School of Medicine were analyzed retrospectively. The moderating effects of gender,age,cancer stages and distant metastasis on HRV including standard diviation of NN intervals (SDNN) and root mean square of successive differences(RMSSD)were examined. The statistical significance of C-ractiveprotein (CRP)and tumor diameter between low HRV group and high HRV group using the cut-off of SDNN=20ms were tested. Results HRV values in cancer patients with early stage was higher than those with advanced stages significantly (P<0.001). HRV values in cancer patients without metastasis had significant higher HRV than those with metastases (P=0.011). CRP was higher in low HRV group than that in high HRV group(P=0.008). Conclusion The later the stage of pancreatic cancer patients is,the more the index of HRV declines, which may be related to the mechanism that vagal nerve excitement inhibits inflammatory reaction.

Abstract:OBJECTIVE:To observe the changes in hemodynamics and oxygen metabolism in pigs with severe acute pancreatitis (SAP) so as to provide a theoretical basis for clinical treatment.METHODS:A porcine model of SAP was reproduced by infusing normal saline (1 ml/kg) of sodium taurocholate (4%) and trypsin (1%) into the pancreatic duct (n=8). Heart rate (HR), central venous pressure (CVP), mean arterial pressure (MAP), pulmonary arterial wedge pressure (PAWP), mean pulmonary arterial pressure(MPAP) and cardiac output were continuously measured with the aid of Swan-Ganz catheter and electrocardiography monitor, and cardiac index (CI) was calculated. Oxygen delivery (DO(2)), oxygen consumption (VODO(2)) and oxygen extraction (ODO(2)ext) were calculated according to the analysis of the blood-gases in arterial and mixed venous blood at 0, 6, 12, 24, 36 hours after taurocholic acid injection, and the result were analyzed.RESULTS:Compared with baseline (0 hour), MAP and CI decreased significantly 12 hours after the acid insult (all P<0.05). Both of partial pressure of artery (PaO(2)) and DO(2) showed a tendency to fall. Compared with 0 hour, PaO(2) decreased significantly at 6, 12, 24 and 36 hours (all P<0.05), and DO(2)at 24 hours (P<0.05). The trends of VO(2) and O(2)ext were consistent, both of them peaked at 6 hours (both P<0.05), then, began to lower, and the difference was statistically significance at 24 hours (both P<0.05).CONCLUSION:There is not only hemodynamic disturbances but also oxygen metabolism dysfunction in pigs with SAP. The pathogenesis of multiple organ dysfunction syndrome might be attributable to the lowering of VO(2) and O(2)ext.

Abstract:Objective: Acute pancreatitis (AP) is a common inflammatory disorder that may develop into severe AP (SAP), resulting in life-threatening complications and even death. The purpose of this study was to explore two different machine learning models of multilayer perception-artificial neural network (MPL-ANN) and partial least squares-discrimination (PLS-DA) to diagnose and predict AP patients' severity. Methods: The MPL-ANN and PLS-DA models were established using candidate markers from 15 blood routine parameters and five serum biochemical indexes of 133 mild acute pancreatitis (MAP) patients, 167 SAP (including 88 moderately SAP) patients, and 69 healthy controls (HCs). The independent parameters and combined model's diagnostic efficiency in AP severity differentiation were analyzed using the area under the receiver operating characteristic curve (AUC). Results: The neutrophil to lymphocyte ratio (NLR) is the most useful marker in 20 parameters for screening AP patients [AUC = 0.990, 95% confidence interval (CI): 0.984-0.997, sensitivity 94.3%, specificity 98.6%]. The MPL-ANN model based on six optimal parameters exhibited better diagnostic and predict performance (AUC = 0.984, 95% CI: 0.960-1.00, sensitivity 92.7%, specificity 93.3%, accuracy 93.0%) than the PLS-DA model based on five optimal parameters (AUC = 0.912, 95% CI: 0.853-0.971, sensitivity 87.8%, specificity 84.4%, accuracy 84.8%) in discriminating MAP patients from SAP patients. Conclusion: The results demonstrated that the MPL-ANN model based on routine blood and serum biochemical indexes provides a reliable and straightforward daily clinical practice tool to predict AP patients' severity. Abstract:Background: Severe acute pancreatitis is still associated with substantial morbidity and mortality. Experimental and clinical studies have demonstrated that reactive oxygen species (ROS) represent early occurring inflammatory mediators contributing to cell dysfunction, both locally in the pancreas and remote organs. Method: A systematic literature review was conducted to investigate the potential roles of intra- and intercellular, as well as interorgan signaling of ROS in the development of pancreatitis-associated multiple organ dysfunction syndrome (MODS). A text word search of the Medline, PubMed and Cochrane databases, and a manual search of the citations from these references, was performed. Results: ROS directly compromise cellular damage and regulate intercellular signals in pancreatitis-associated MODS. ROS are involved in leukocyte activation, production of cytokines, endothelial barrier dysfunction, and microcirculatory barrier dysfunction in acute pancreatitis. Beside effects on intercellular signaling, ROS also affect intracellular events and activate the transcription factor nuclear factor k B that regulates inflammatory cytokine expression. Conclusion: ROS is a critical factor responsible for the development of pancreatitis-induced remote organ dysfunction via intercellular and interorgan signaling. The role of antioxidant treatment, included as a part of multimodal management, remains to be investigated.
Mechanism of Acute Pancreatitis Complicated with MODS

Abstract:Objective To study the mechanism of acute pancreatitis complicated with MODS,and to provide a new way for preventing and treating MODS.Methods Sixty-three pancreatitis patients were divided into two groups:moderate group,patients without organ dysfunction;and severe group,patients with organ dysfunction.Plasma endotoxin,lipopolysaccaride binding protein(LBP),endotoxin inhibitor,sCD14,and nuclear factor-κB(NF-κB)activity in PBMC were determined.Results The incidence rate of MODS and endotoxemia in both groups were 38.1% and 75.64%,respectively.The levels of LBP and sCD14 was increased,but the level of cndotoxin inhibitor was reduced.The NF-κB activity in PBMC was increased,and was related to the severity.Conclusion Continuous endotoxemia,increasing LBP,sCD14,and NF-κB activation may be involved in the development of MODS in patients with acute pancreatitis.
Prognostic Factors of Death in Patients with Severe Acute Pancreatitis

Abstract:Objective To study the prognostic factors of severe acute pancreatitis.Methods We retrospectively analyzed clinical data of 76 patients with severe acute pancreatitis(Ranson score ≥3 points and APACHE Ⅱ score ≥8 points)who were treated in our hospital from January 1998 to March 2010.The patients who died in 60 days after admission was enrolled into death group(n=11),and the others were set as a survival group(n=65).The Ranson score,APACHE Ⅱ score,age,oxygenation index,serum creatine,glucose,calcium,base excess,and hematocrit were compared between the two groups.Results Death group showed significantly higher APACHE Ⅱ(16.57±3.10 vs 12.04±2.95,t=2.859,P=0.006)and Ranson scores(4.71±0.76 vs 3.87±0.73,t=2.859,P=0.006),serum creatine [(103.1±24.4)μmol/L vs(78.4±14.5)μmol/L,t=2.607,P=0.037] and glucose [(13.7±2.8)mmol/L vs(11.0±1.5)mmol/L,t=2.448,P=0.047] than the survival group,but significantly lower oxygenation index [(221±41)mm Hg vs(285±48)mm Hg,t=-3.353,P=0.002],serum calcium [(1.77±0.39)mmol/L vs(1.98±0.39)mmol/L,t=-2.187,P=0.033],base excess [(-1.33±2.93)mmol/L vs(0.70±2.23)mmol/L,t=-2.149,P=0.036],and hematocrit(0.35±0.04 vs 0.40±0.04,t=-2.957,P=0.013).Conclusions The Ranson score,APACHE Ⅱ score,oxygenation index,serum creatine,glucose,calcium,base excess,and hematocrit can be prognostic factors for patients with severe acute pancreatitis in early stage.
Early Predictors of Infected Pancreatic Necrosis in Severe Acute Pancreatitis: Implications of Neutrophil to Lymphocyte Ratio, Blood Procalcitonin Concentration, and Modified CT Severity Index

Abstract:Background: Infected pancreatic necrosis (IPN) accounts for 30% mortality in severe acute pancreatitis (SAP). Early prediction of IPN occurrence is critical for prophylactic measures to be taken. This study aimed to evaluate the predicting value for IPN of combined markers at early stages of SAP. Methods: The clinical records of 324 SAP patients admitted within 48 h after disease onset were retrospectively analyzed. As potential predictors, the neutrophil to lymphocyte ratio (NLR), blood procalcitonin (PCT) concentration on the 1st, 4th, and 7th day post admission, as well as modified computerized tomography severity index (MCTSI) on the 5-7th day post admission, were extracted. Correlations between these features with IPN were analyzed using logistic regression, and predictive values were estimated using the receiver operating characteristic curve analyses. Results: NLR, PCT, body mass index, and MCTSI were significantly higher in the IPN group (p < 0.001) compared to the control, among which NLR, PCT, and MCTSI were identified as independent predictors for IPN in logistic regression model. Combination of these parameters yielded significant predicting values with an area under curve of 0.92, sensitivity of 97.2%, and specificity of 77.2% in receiver operating characteristic curve analysis. Conclusion: Combination of NLR, PCT, MCTSI might facilitate the prediction of IPN occurrence in SAP patients.
Endothelial Markers Are Associated with Pancreatic Necrosis and Overall Prognosis in Acute Pancreatitis: A Preliminary Cohort Study

Abstract:BACKGROUND:Endothelial injury is believed to play an important role in the evolution of pancreatic microcirculatory dysfunction and pancreatic necrosis (PN) in patients with acute pancreatitis (AP). The aim of this study was to investigate the role of three endothelial markers (von Willebrand factor, vWF; E-selectin; endothelial protein C receptor, EPCR) in the early phase of AP, especially the relationship between endothelial markers and PN.METHODS:From March 2015 to March 2016, 57 AP patients admitted within 72 h of symptom onset in our hospital were included for this study. Blood samples were taken on admission and the clinical characteristics and outcomes of these patients were recorded. The levels of vWF, E-selectin and EPCR were measured using ELISA for analysis and compared with other severity markers of AP.RESULTS:All the three markers were significantly different in healthy control, mild, moderate and severe AP patients. Moreover, the endothelial markers, especially vWF, also showed significant difference in patients with different extent of PN, as well as those with or without MODS. Additionally, the levels of endothelial markers correlated well with other commonly used markers of AP severity.CONCLUSION:Elevated endothelium-related mediators (vWF, E-selectin and EPCR) appear to participate in the development of PN and may be a potential indicator of overall prognosis. Our results may help clinicians better understand the pathophysiological process of the development of PN.

Abstract:Prolonged elevated heart rate (peHR) is recognized as a risk factor for poor prognosis among critically ill patients. However, there is currently a lack of studies investigating the association between peHR and patients with acute pancreatitis. Multiparameter Intelligent Monitoring in Intensive Care IV (MIMIC-IV) database was used to identify patients with acute pancreatitis. PeHR was defined as a heart rate exceeding 100 beats per minute for at least 11 out of 12 consecutive hours. Cox regression analysis was used to assess the association between peHR and the 90-Day mortality. A total of 364 patients (48.9%) experienced a peHR episode. The 90-day mortality was 25%. PeHR is an independent risk factor for 90-day mortality (HR, 1.98; 95% CI 1.53–2.56; P < 0.001). KM survival curves exhibited a significant decrease in the survival rate at 90 days among patients who experienced a peHR episode (P < 0.001, 84.5% vs. 65.1%). We revealed a significant association of peHR with decreased survival in a large cohort of ICU patients with acute pancreatitis.

Prediction of the Severity of Acute Pancreatitis

Abstract:We conducted a prospective study to validate our previous finding that serum urea and plasma glucose levels on admission could predict the outcome of acute pancreatitis. Forty-two (24%) of 176 patients developed complications related to the attack of acute pancreatitis and were classified as having severe disease. By logistic regression analysis of 17 admission parameters, serum urea and plasma glucose levels were again the factors with independent significance in defining the outcome. By adopting the same cutoff levels as in our previous study (serum urea level greater than 7.4 mmol/L and plasma glucose level greater than 11.0 mmol/L), and the presence of either factor above the cutoff level as indicative of severe disease, the sensitivity of prediction was 79%, specificity 67%, and overall accuracy 70%. All the deaths were correctly predicted by this urea/glucose criteria. The overall accuracy was also found to be comparable with those of the APACHE II (cutoff level greater than 11) and Ranson's scoring systems. We conclude that the simple prognostic criteria for acute pancreatitis were validated; these criteria have the potential to stratify risk rapidly at the time of admission for patients who might benefit from an aggressive interventional protocol.
Intra-abdominal Pressure and Abdominal Perfusion Pressure: Which is a Better Marker of Severity in Patients with Severe Acute Pancreatitis

Abstract:Background Intra-abdominal hypertension is common in patients with severe acute pancreatitis. The aim of this study was to assess the clinical relevance of intra-abdominal pressure and abdominal perfusion pressure in the first 72 h after admission during severe acute pancreatitis. Methods From January 2009 to February 2011, 50 patients admitted for severe acute pancreatitis were included in this prospective, observational study. The intra-abdominal pressure and abdominal perfusion pressure level were repeatedly measured every 12 h during the first 72 h. The maximum and the mean values of intra-abdominal pressure and the minimum and mean values of abdominal perfusion pressure were used for analysis. Results Both the maximum and mean levels of intra-abdominal pressure were significantly different between patients with or without kinds of clinical variables. But for abdominal perfusion pressure, difference could only be detected in terms of need of vasoactive drugs. Besides that, different from abdominal perfusion pressure, intra-abdominal pressure is associated with high incidence rates of MODS and secondary infection. Conclusion Compared with abdominal perfusion pressure, intra-abdominal pressure is much more valuable as an early marker of the evolution and complications of severe acute pancreatitis.

Abstract:Acute pancreatitis is an inflammatory disease of the exocrine pancreas, usually accompanied by damage or necrosis of the pancreatic parenchyma and peripancreatic tissues. The global incidence of acute pancreatitis has reached 34/100,000, with an increasing trend every year. It also has a high mortality rate, with an annual death rate of 1.16/100,000, higher than chronic pancreatitis. [ 1 , 2 ] Acute pancreatitis is divided into mild, moderate, and severe according to the severity. Approximately 20% of acute pancreatitis cases progress to severe acute pancreatitis, often accompanied by persistent organ failure, a relatively higher Apache II score, and a mortality rate of 36% to 50%. [ 2 , 3 ] Apache II is acute physiology and chronic health evaluation II. It consists of 3 parts: the acute physiology score, the age points, and the chronic health points, and is related to the severity of the disease. [ 4 ]

Abstract:Objective To evaluate the changes of blood coagulation and their clinical significance in pa-tients with severe acute pancreatitis (SAP). Method Two hundred and seven patients with acute pancreatitis ad-mitted from January 2005 to Deeember 2008 in People' s Hospital, Peking University, were recruited in this study. Patients were divided into two groups, severe acute pancreatitis (53 cases) and mild acute pancreatitis (154 cas-es), according to the diagnostic criteria set by the Chinese Medical Association in 2004. Plasma coagulation pa-rameters including prothrombin time (PT), prothrombin activity (PTA), international normalized ratio (PT-INR), activated partial thromboplastin time (APTT), fibrinogen (FIB) and D-dimmer were redorded. Results Com-pared with MAP group, there were significant differences on PT, PTA, PT-INR, FIB, and D-dimmer in SAP group [in MAP group, (12.45 ± 1.13) s,(94.01±21.21)% ,(1.08±0.14),(4.81±1.86)g/L,340.38 ng/ mL, respectively;in SAP group, (13.08±1.47)s,(86.92±17.36) %, (1.14±0.20), (5.49±2.30)g/L, 943.82 ng/mL, respectively; P <0.05]. There was no significant difference on APTT between the two groups (P > 0.05). D-dimmer was correlated to the APACHE Ⅱ score , and it was also correlated to the SIRS score (P < 0.05). The rate of systemic inflammatory response syndrome (SIRS) and the mortality were higher in patients with SAP patients than those in the MAP patients (in SAP group, 35.5%, 5.6%, respectively; in MAP group, 10.4%, 0, respectively; P < 0.05). Conclusions Disorders of coagulation and microcirculation disturbances were observed in patients with severe acute pancreatitis. These alterations may be useful in the assessment of the disease severity and prognosis of patients with acute pancreatitis.

Abstract:To assess the association between the clinical parameters within 48 hours of admission and the occurrence of infected pancreatic necrosis (IPN) during the late phase of necrotizing pancreatitis (NP). All patients were divided into 2 groups, the IPN and non-IPN groups. The clinical data were retrospectively analyzed. Univariate and multivariate logistic regression analyses were performed to evaluate the relationship between clinical parameters and IPN secondary to NP. The performance of each independent variable was plotted by the receiver-operating characteristic (ROC) curve. Consequently, the cut-off level of each independent variable with its sensitivity and specificity was calculated. A total of 215 patients were enrolled in our study. Among them, 87 (40.5%) patients developed IPNs after a median of 13.5 (9.5-23.0) days from admission. Multivariate analysis indicated that the level of hematocrit (HCT) from 40% to 50% (P = .012, odds ratio (OR) = 2.407), HCT over 50% (P < .009, OR = 6.794), blood urea nitrogen (BUN) (P = .040, OR = 1.894), C-reactive protein (CRP) (P = .043, OR = 1.837), and procalcitonin (PCT) (P = .002, OR = 2.559) were independent risk factors of IPN secondary to NP. The ROC cures revealed that the area under the ROC (AUC) of the maximum level of HCT, BUN, CRP, and PCT within 48 hours of admission was 0.687, 0.620, 0.630, and 0.674 respectively. Furthermore, the combination of these 4 individual parameters contributes to a more preferable AUC of 0.789 with a sensitivity of 67.8% and specificity of 77.3%. The maximum levels of PCT, CRP, HCT, and BUN within 48hours of admission are independent factors of IPN and their combination might accurately predict the occurrence of IPN secondary to NP.
