Abstract:Purpose:Whether the association between diabetic kidney disease (DKD) and diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (T2DM) is leveraged by anemia remains unclear. This study is to evaluate the joint effect of DKD and anemia on DR.Methods:Data were collected from electronic medical records of 1389 patients with T2DM in the Yiwu Central Hospital of Zhejiang Province from 2018 to 2019. Based on retinal examination findings, patients were classified as without diabetic retinopathy (non-DR), non-proliferative diabetic retinopathy (NPDR), and proliferative diabetic retinopathy (PDR). Odds ratio (OR) from multinomial logistic regression models adjusting for potential risk factors of DR were used to evaluate associations of DKD, renal function measures, and anemia with risk of NPDR and PDR. Path analysis was performed to help understand the association of DKD and hemoglobin (Hb) with DR.Results:The study included 901 patients with non-DR, 367 patients with NPDR and 121 patients with PDR. Both high DKD risk and abnormal renal function measures were significantly associated with PDR. Anemia was associated with increased risk of NPDR (OR = 1.75, 95% confidence interval [CI] = 1.18-2.58) and PDR (OR = 3.71, 95% CI = 2.23-6.18). DKD severity and anemia had joint effect on NPDR (OR = 2.29, 95% CI = 1.32-3.96) and PDR (OR = 11.31, 95% CI = 5.95-21.51). These associations were supported by path analysis.Conclusions:DKD severity, abnormal estimated glomerular filtration rate (eGFR), and urinary albumin/creatinine ratio (UACR) were associated with increased risk of DR in patients with T2DM, and anemia had joint effect on these associations. Improving Hb level may decrease the risk of DR in patients with T2DM.
Factors Associated with Diabetic Retinopathy in Chinese Patients with Type 2 Diabetes Mellitus

Abstract:Objective. To investigate the risk factors of DR in Chinese T2DM patients. Methods. 2009 patients with T2DM were included in this cross-sectional study. All patients underwent eye examination, and the DR stage was defined by an ophthalmologist. Correlation analysis was performed to evaluate the relation between DR and clinical variables. Logistic regression models were used to assess risk for those factors associated with DR. Results. A total of 597 T2DM patients (29.7%) had DR, of which 548 (27.3%) were nonproliferative diabetic retinopathy and 49 (2.4%) were proliferative diabetic retinopathy. Positive correlations were found between DR and duration of diabetes, systolic blood pressure (SBP), diastolic blood pressure, glycated hemoglobin, glycated albumin, 24 hurinary albumin excretion, peripheral atherosclerosis (PA), diabetes nephropathy (DN), diabetic peripheral neuropathy, and anemia. Negative correlations were found between DR and C-peptide and glomerular filtration rate. Logistic regression analysis revealed that duration of diabetes, SBP, DN, anemia, PA, and C-peptide were each independent risk factors of DR. Conclusion. The duration of diabetes, SBP, DN, anemia, and PA are positively associated with DR in Chinese T2DM patients, while C-peptide is negatively associated with DR. Monitoring and evaluation of these related factors will likely contribute to the prevention and treatment of DR.

Abstract:AIM: To investigate diabetic retinopathy (DR) prevalence in Chinese renal-biopsied type 2 diabetes mellitus (T2DM) patients with kidney dysfunction, and to further evaluate its relationship with diabetic nephropathy (DN) incidence and the risk factors for DR development in this population. METHODS: A total of 84 renal-biopsied T2DM patients were included. Fundus and imaging examinations were employed for DR diagnosis. Demographic information and clinical measures along with renal histopathology were analyzed for comparisons between the DR and non-DR groups. Risk factors on DR development were analyzed with multiple logistic regression. RESULTS: DR prevalence was 50% in total. The incidences of DN, non-diabetic renal disease (NDRD) and mixed-type pathology were 47.6%, 19.0% and 33.3% in the DR group respectively, while 11.9%, 83.3% and 4.8% in the non-DR group. Systolic blood pressure, ratio of urinary albumin to creatine ratio, urinary albumin, 24-hours urinary protein, the incidence and severity of DN histopathology were found statistically increased in the DR group. Multiple logistic regression analysis showed histopathological DN incidence significantly increased the risk of DR development [odds ratio (OR)=21.664, 95% confidential interval (CI) 5.588 to 83.991, P<0.001 for DN, and OR=45.475, 95%CI 6.949 to 297.611, P<0.001 for mixed-type, respectively, in reference to NDRD)], wherein DN severity positively correlated. CONCLUSION: Renal histopathological evidence indicates DN incidence and severity increases the risk of DR development in Chinese T2DM patients inexperienced of regular fundus examinations.

Abstract:Context: Diabetic retinopathy (DR) and diabetic nephropathy (DN), are major microvascular complications of diabetes. DR is an important predictor of DN, but the relationship between the severity of DR and the pathological severity of diabetic glomerulopathy remains unclear. Objective: To investigate the relationship between severity of diabetic retinopathy (DR) and histological changes and clinical indicators of diabetic nephropathy (DN) in patients with type 2 diabetes mellitus (T2DM) Methods: Patients with T2DM (n=272) who underwent a renal biopsy were eligible. Severity of DR was classified as non-diabetic retinopathy, non-proliferative retinopathy, and proliferative retinopathy (PDR). Relationship between DN and DR and the diagnostic efficacy of DR for DN were explored. Results: DN had a higher prevalence of DR (86.4%) and DR was more severe. The sensitivity and specificity of DR in DN were 86.4% and 78.8%, while PDR was 26.4% and 98.5%, respectively. In DN patients, the severity of glomerular lesions (p=0.001) and prevalence of KW nodules (p<0.001) significantly increased with increasing severity of DR. The presence of KW nodules, lower hemoglobin levels, and younger age were independent risk factors associated with more severe DR in patients with DN. Conclusion: DR was a good predictor of DN. In DN patients, the severity of DR was associated with glomerular injury, and presence of KW nodules, lower hemoglobin levels and younger age were independent risk factors associated with more severe DR. Association between Renal Function and Retinal Neurodegeneration in Chinese Patients with Type 2 Diabetes Mellitus

Abstract:Abstract Background Chronic kidney disease (CKD) and diabetic retinopathy (DR) are two serious complications of diabetes. However, the association between retinal neurodegeneration in DR and renal function decline is still unclear. Our objective was to evaluate the association by measure estimated glomerular filtration rate (eGFR), macular ganglion cell–inner plexiform layer (GC–IPL) and ganglion cell complex (GCC) thickness in patients with type 2 diabetes mellitus (T2DM). Methods We analyzed the baseline data of the Guangzhou Diabetic Eye Study. T2DM patients from communities in Guangzhou were enrolled and all participants went through ophthalmic and general examinations. The thickness of the macular GC–IPL and GCC in their right eyes were measured by swept-source optical coherence tomography. CKD was defined as eGFR < 60 mL/min/1.73 m 2 . Results 1,309 patients were included (mean age 64.4 ± 7.6 years, 59.1% female), and fifty-eight (4.4%) of them had CKD. Average macular GC-IPL thickness was significantly thinner in CKD patients (96.5 ± 9.1 μm) than non-CKD patients (101.3 ± 9.2 μm) ( p < 0.01). Average macular GCC thickness was also significantly thinner in CKD patients (123.5 ± 13.2 μm) than non-CKD patients (129.9 ± 12.8 μm) ( p < 0.01). The significant thinning of macular GC-IPL and GCC thickness presented in every gird in macula (all, p < 0.05) except for central grid (p ≥ 0.05). In the patients without DR, the eGFR was linearly correlated with the average macular GC–IPL thickness (β = 0.07 [95% CI 0.02–0.12], p < 0.01) and GCC thickness (β = 0.09 [95% CI 0.03–0.16], p < 0.01) after adjustment for age, sex, axial length, intraocular pressure and combination of hypertension. However, no linear correlation was found between eGFR and macular GC-IPL or GCC thickness in DR patients. Conclusions Renal function decreases is associated with the thinning of the macular GC–IPL and GCC in T2DM patients, suggesting the potential value of ganglion cell lose to detect early function decline in the kidney in diabetic patients, especially in patients without DR.

Abstract:Aims. Epidemiological data on diabetic retinopathy (DR) in Chinese population is still rather scarce, and risk factors for diabetic retinopathy are inconsistent because of study designs, grading standards, and population samples. Materials and Methods. This hospital-based retrospective study included 1052 type 2 diabetes patients. Diabetic retinopathy was diagnosed by nonmydriatic fundus photography and/or fundus examination apparatus. Logistic regression analysis was performed to evaluate the risk of diabetic retinopathy. Results. A total of 352 (33.5% prevalence) subjects were diagnosed with diabetic retinopathy based on our population. The patients in the DR group not only had significantly higher hemoglobin A1c (HbA1c), fasting plasma glucose (FPG), urinary microalbumin-creatinine ratio (ACR), and systolic blood pressure but also had higher follicle-stimulating hormone (FSH), luteinizing hormone (LH), and sex hormone-binding globulin (SHBG) levels compared to those in the non-DR group. Moreover, we confirmed that diabetes duration and HbA1c are strongly associated with DR risk. We also found that serum LH was an independent risk factor in male diabetic retinopathy patients ( OR = 1.086 , 95% CI 1.024–1.152), and the levels of LH were significantly associated with diabetic retinopathy prevalence ( P = 0.018 ). Conclusions. Our study strengthens the argument that diabetes duration and HbA1c are risk factors for patients with DR. Additionally; we firstly confirmed that serum LH was an independent risk factor in male diabetic retinopathy patients.

Abstract:The concomitant presence of retinopathy may be helpful in discriminating kidney pathology in type 2 diabetes. This cross-sectional study evaluated the ability to predict the likelihood of patients having diabetic kidney disease (DKD) by comparing the percentage incidence of diabetic retinopathy in different subgroups of Chinese patients with type 2 diabetes (n = 668). The prevalence of chronic kidney disease in the patients was 63.3% (6.6% with DKD, 41.0% with possible DKD and 15.7% with non-diabetic renal disease [NDRD]). The percentage of patients with diabetic retinopathy in the DKD group (63.6%) was significantly higher than in the possible DKD group (24.9%) and the NDRD group (19.0%). DKD was independently associated with diabetic retinopathy based on logistic regression analysis. The likelihood of DKD occurrence as defined by the National Kidney Foundation-Kidney Disease Outcomes Quality Initiative (TM) of the USA is applicable to Chinese patients with diabetes. Diabetic retinopathy was more prevalent in patients with DKD compared with patients with possible DKD or NDRD.

Abstract:Objective: To find the associations between diabetic retinopathy and age at diagnosis, C-peptide level and thyroid-stimulating hormone (TSH) level in Chinese type 2 diabetes mellitus.Methods: 3100 hospitalized type 2 diabetic patients in Peking University People's Hospital were included in this retrospective study. Their medical history and the laboratory data were collected. All the patients received examination of diabetic retinopathy (DR) by professional ophthalmologist.Results: Comparisons among patients with NDR, NPDR and PDR showed that with the progression of diabetic retinopathy, patients turned to have older age but younger age at diagnosis of diabetes, and have higher SBP, longer duration of diabetes, higher mean HbA1c but lower fasting and 2 hours postprandial C-peptide level. Moreover, with the progression of diabetic retinopathy, patients turned to have higher prevalence of primary hypertension, higher prevalence of peripheral vascular sclerosis, higher proportion with insulin treatment. TSH level was comparable among the three groups of patients. Association analysis showed that after adjusting for age, sex, duration of diabetes, body mass index, HbA1c, blood pressure and albuminurea creatinine ratio and insulin treatment, age at diagnosis (OR 0.888, 95% CI 0.870-0.907, p = 0.00) and postprandial C-peptide (OR 0.920, 95% CI 0.859-0.937, p = 0.00) are the independent associated factors of DR in Chinese type 2 diabetes.Conclusions: According to the results, postprandial C-peptide level and age at diabetes may be two independent associated factors with DR in Chinese type 2 diabetes. The lower level of postprandial C-peptide, the younger age at diagnosis, may indicate the higher prevalence of DR.
Relationship Between Retinol and Risk of Diabetic Retinopathy: a Case-Control Study.

Abstract:BACKGROUND AND OBJECTIVESThe aim of the present study was to evaluate the relationship between retinol and risk of diabetic retinopathy in Chinese adults.METHODS AND STUDY DESIGNEighty-six subjects with type 2 diabetes mellitus (T2DM) and 40 healthy subjects (healthy comparison group, HCG) were recruited in Beijing Luhe Hospital. Of the 86 T2DM subjects, 43 subjects were diagnosed with diabetic retinopathy (DRG), and 43 subjects had no retinopathy (DNRG).RESULTSDietary intake of retinol (p<0.001) and retinol equivalent (p<0.05) was significantly higher but serum retinol and (p<0.001) retinol binding protein 4 (RBP4) (p<0.05) were significantly lower in DNRG compared with HCG. Dietary intake of retinol (p<0.05) and retinol equivalent (p<0.05) was significantly lower, and serum retinol and (p<0.01) retinol binding protein 4 (RBP4) (p<0.01) were significantly higher in DRG compared with DNRG. In T2DM subjects, per 100 μg/day higher dietary retinol intake was associated with 17% lower risk of diabetic retinopathy (odds ratio (OR), 0.83; 95% CI, 0.70 to 0.98; p=0.032), and after adjusting for potential confounding factors, the OR was 0.82 (95% CI, 0.69 to 0.99; p=0.036); per 100 μg/day higher in dietary retinol equivalent intake was associated with 12% lower risk of diabetic retinopathy (OR, 0.88; 95% CI, 0.79 to 0.97; p=0.010), and after adjusting for potential confounding factors, the OR was 0.88 (95% CI, 0.79 to 0.98; p=0.025).CONCLUSIONSHigher dietary intake of retinol or retinol equivalent is associated with lower risk of diabetic retinopathy.
Diabetic Retinopathy and Chronic Kidney Disease: Associations and Comorbidities in a Large Diabetic Population: The Tongren Health Care Study

Abstract:Background: Diabetic retinopathy (DR) and chronic kidney disease (CKD) in type 2 diabetes tended to have different pathogenesis, while their distinct risk factors have not been fully investigated.Methods: In the cross-sectional, community-based Tongren Health Care Study, type 2 diabetes was diagnosed based on fasting plasma glucose concentration of ≥7·0 mmol/L or a medical history. DR was assessed using color fundus photograph. CKD was defined by a reduced estimated glomerular filtration rate (eGFR) of <60ml/min/1·73mm 2 and/or albuminuria.Findings: Among the 5103 diabetes patients (8·2%) out of 62,217 participants, the prevalence of DR, reduced eGFR, albuminuria and CKD was 12·8% (95%CI:11·8%,13·7%), 4·6% (95%CI:4·2%,5·1%), 10·1% (95%CI:9·3%,10·9%) and 13·3% (95%CI:12·4%,14·3%), respectively. DR was present in 21·0% of the CKD patients while CKD was detectable in 20·9% of the DR patients. The presence of DR was significantly associated with the CKD and albuminuria (P <0·05), but not with reduced eGFR. Factors independently associated with the presence of CKD instead of DR were older age (P <0·001,OR=1·05), a higher body mass index (P <0·001,OR=1·14), a higher serum concentration of triglycerides (P <0·001,OR=1·26), and a lower blood glucose (P <0·001,OR=0·93). Having hypertension was additionally associated with the presence of reduced eGFR as compared with DR (P=0·005,OR=4·47).Interpretation: Type 2 diabetes patients with older age, higher body mass index, with hypertension and dyslipidemia had a higher chance of being affected by CKD rather than DR, while those with a higher blood glucose level were more prone to DR than CKD.Funding: Beijing Municipal of Health Reform and Development Project #2019-4. Comprehensive retinal vascular measurements: a novel association with renal function in type 2 diabetic patients in China

Abstract:Abstract To examine the association between various retinal vascular measurements and microalbuminuria in patients with type 2 diabetes in a northwestern China study. Data from 911 patients with type 2 diabetes were analyzed. Novel retinal vascular measurements from the whole vascular tree were extracted using a validated fully automatic computer program. Retinal vascular measurements were analyzed continuously and categorically for associations with microalbuminuria using multiple logistic regressions, adjusted for related variables. In logistic regression adjusting for multiple variables, microalbuminuria was associated with smaller peripheral arteriolar caliber, larger peripheral venular caliber, larger arteriolar tortuosity, and smaller arteriolar fractal dimension ( p = 0.028, p < 0.001, p = 0.038, p = 0.035, respectively). In further categorical analyses, microalbuminuria was related to smaller peripheral arteriolar caliber [T1 vs. T3: odds ratio (OR) 2.029; 95% confidence interval (CI) 1.186–3.473], larger peripheral venular caliber (T1 vs. T3: OR 0.609; 95% CI 0.362–1.024), and smaller arteriolar fractal dimension (T1 vs. T3: OR 1.659; 95% CI 1.028–2.675). Microalbuminuria in type 2 diabetes is associated with both retinal vascular caliber and geometry. These noninvasive vascular measurements serve as potential preclinical markers to identify populations at high risk of early kidney disease in the course of diabetes.

Abstract:Background The prevalence of diabetic retinopathy is not well studied in the Chinese pre-diabetic population, also known as impaired glucose regulation. Hence, we investigated the prevalence of and risk factors associated with retinopathy in diabetic and pre-diabetic subjects from Chinese communities.Methods A total of 3736 Chinese subjects were recruited from urban communities in Shanghai. The participants were classified as normal glucose tolerance, impaired glucose regulation (IGR) and diabetes based on the 75 g oral glucose tolerance test. The levels of diabetic retinopathy (DR) were assessed with non-mydriatic retinal photographs according to the Diabetic Retinopathy Disease Severity Scale.Results The prevalence of diabetic retinopathy in patients with diabetes and impaired glucose regulation subjects was 9.4% and 2.5%, respectively. In subjects with IGR, hypertension (odds ratio: 3.54, p = 0.028), including elevated systolic and diastolic blood pressure and obesity (odds ratio: 3.53, p = 0.028) were significantly associated with diabetic retinopathy after age and sex adjustments. The factors associated with retinopathy in diabetes included diabetes duration, blood glucose levels, glycated hemoglobin levels, and the presence of albuminuria. Diabetic retinopathy was significantly associated with fasting plasma glucose in known diabetes, whereas in newly-diagnosed subjects, diabetic neuropathy was closely correlated to postprandial plasma glucose.Conclusions Hyperglycemia was a strong risk factor for diabetic retinopathy. Prevalence, Awareness, and Risk Factors of Diabetic Retinopathy among Adults with Known Type 2 Diabetes Mellitus in an Urban Community in China

Abstract:PURPOSE: To understand the prevalence, awareness, and risk factors for diabetic retinopathy (DR) among Chinese with known type-2 diabetes mellitus (T2DM) in eastern China communities.METHODS: A community-based survey including 913 patients with known T2DM was conducted in Suzhou, China. Diabetes was defined as hemoglobin A1c (hbA1c) ≥6.5%, use of diabetic medication or a physician diagnosis of diabetes. Retinal photographs were graded for the presence of DR using the Airlie House classification system. Binary logistic regression models were established to examine the associations of risk factors with DR and vision-threatening diabetic retinopathy (VTDR).RESULTS: The overall prevalence of any DR was 18.0% (95% confidence interval [CI]: 15.5-20.6%) in this population. VTDR affected 4.4% (95% CI: 3.1-5.8%) of the study participants. In multivariate analysis, the presence of DR among T2DM patients was associated with decreased age (odds ratio [OR] = 0.97; p = 0.01; per year increase), longer durations of diabetes (OR = 1.02; per year increase; p = 0.03), higher blood levels of hbA1c (OR = 1.15; per unit increase; p = 0.03). Of the 158 participants with DR, only 12 (8%) had been aware of their condition or had been diagnosed previously as having DR.CONCLUSIONS: The prevalence of DR among diabetes patients in eastern China was lower than those in northern China. The awareness of DR was poor, which emphasizes the pressing need for increasing the public awareness of this vision-threatening eye disorder and routine eye screening should be performed among T2DM patients in the communities.

Abstract:In China, there is a significant lack of awareness of diabetes and its complications. Screening of diabetic retinopathy has important for early detection, prevention, and treatment. This large, cross-sectional study aimed to characterize the demographic, physical, serological, and ocular characteristics of subjects with diabetes mellitus in Shijiazhuang, China. It also aimed to associate these characteristics with the presence of diabetic retinopathy.From May 2, 2018 to August 25, 2019, under diabetes care program, the diabetic patients (n = 1008) were subjected to standardized questionnaires to collect demographical characteristics. Also, telescreens and laboratory tests were performed for the enrolled patients. Multivariate logistic regression analysis was used to evaluate factors associated with diabetic retinopathy.Forty percent of diabetics in its population had some form of diabetic retinopathy. Diabetic retinopathic patients were likely to be elder (P = .0003), men (P = .018), hypertensive (P < .0001), and had high body mass index (P < .0001), metabolic abnormalities, and longer duration of diabetes (P < .0001). Higher intraocular pressure (P = .0008), fasting blood glucose (P < .0001), serum total cholesterol (P < .0001), serum triglyceride (P = .0006), % glycated hemoglobin (HbA1c) (P < .0001), and disc asymmetry including cup-disc ratio (P = .041) reported in patients with diabetic retinopathy. Age (P = .049), male sex (P = .048), hypertension (P = .048), duration of diabetes (P = .012), diabetic neuropathy (P = .048), diabetic nephropathy (P = .048), diabetic foot ulcer (P = .041), foot amputation (P = .042), fasting blood glucose (P = .022), serum total cholesterol (P = .028), serum triglyceride (P = .035), and HbA1c (P = .042) were associated with diabetic retinopathy.Diabetic retinopathy was the most common ocular fundus disease in diabetic patients. Also, aging, the other comorbidities, and metabolic syndrome are associated with diabetic retinopathy.Level of Evidence: III.

Abstract:To investigate the prevalence and risk factors of diabetic retinopathy (DR) in a Chinese population with type 2 diabetes mellitus (T2DM) in a suburb (Qingpu) of Shanghai, China. A population-based cross-sectional study. A total of 7462 residents with T2DM in Qingpu were enrolled according to the resident health archives from January 2020 to December 2020. Blood and urine samples of the subjects were collected. Disc- and macula-centred retinal images were taken to assess DR. SPSS was used to analyse and investigate the prevalence and risk factors of DR. The fundus images of 6380 (85.5%) subjects were of sufficiently good quality for grading. The average (range) age of 6380 subjects was 63.46±7.77 (28–92) years. Six hundred forty-four subjects were diagnosed with DR. The prevalence of DR was 10.1% (95% CI 9.4%-10.8%), with mild, moderate, and severe non-proliferative retinopathy and proliferative retinopathy being 2.1%, 6.3%, 1.3% and 0.4%, respectively. The prevalence of bilateral DR was 6.5%. Higher T2DM duration (OR, 1.057), fasting plasma glucose (OR, 1.063), glycated hemoglobinA1c (OR, 1.269), urea nitrogen (OR, 1.059), and urinary albumin (OR, 1.001) were associated with the higher DR prevalence. The prevalence of DR among Chinese adults with T2DM in Qingpu was 10.1%, in which non-proliferative DR was more common. Higher fasting plasma glucose and glycated hemoglobinA1c are well-known risk factors of DR, consistent with the findings in our study. Our study didn't find the risk between lipid indicators and DR. However, several renal function indicators, like higher urea nitrogen and urinary albumin, were risk factors for DR in this study. Appropriate diagnosis and intervention should be taken in time to prevent and control DR development.

Abstract:Objective:To investigate the prevalence and related risk factors of diabetic retinopathy (DR) among registered residents of type 2 diabetes mellitus in Jiading Industrial Zone Community Health Service Center,Shanghai.Methods:In this cross-sectional study,diabetic retinopathy was screened among registered residents with type 2 diabetes in Jiading Industrial Zone Community Health Service Center,Shanghai.The prevalence and risk factors of DR were evaluated by combining questionnaire and clinical examination.Results:Among 776 subjects who were chosen,695 people completed the study,and the overall response rate was 89.56％.26 cases were excluded because the refractive medium was opaque and the retina could not be seen clearly.DR was detected in 91 patients (13.60％).The mean duration of diabetes in patients with DR and without DR was (9.63±6.44) years and (7.02±5.32) years (OR=1.073,95％ CI 1.036-1.111,P＜0.001),the fasting blood glucose was (8.66±2.66) mmol/L and (7.52±2.15) mmol/L (OR=1.219,95％ CI 1.119-1.328,P＜0.001),glycated hemoglobin was (7.68±1.55)％ and (6.99±1.21)％ (OR=1.495,95％ CI 1.275-1.754,P＜0.001).There was also statistically significant difference in the use of insulin between DR patients and non-DR patients (OR=4.202,95 ％ CI 2.504-7.051,P＜0.001).Conclusions:The prevalence of DR among type 2 diabetes patients is 13.6％ in Jiading Industrial Zone,Shanghai.The longer the duration of diabetes is,the higher the fasting blood glucose is,the higher the glycosylated hemoglobin is,the greater the risk of DR will be.
Risk factors of proliferative diabetic retinopathy in Chinese Patients with Type 2 Diabetes

Abstract:Abstract Background: To investigate the relationship between systemic parameters and proliferative diabetic retinopathy(PDR) in Type 2 diabetes mellitus(T2DM).Methods: This was a cross-sectional study. Based on retinal examination findings, diabetic patients were classified into three groups: non-diabetic retinopathy (non-DR), non-proliferative diabetic retinopathy (NPDR), and PDR. The comprehensive medical data was used for analysis. Odds ratios(OR) from multinomial logistic regression models were calculated to obtain the risk factors of PDR, and a prediction model was built whose predictive ability was assessed by the area under the receiver operating characteristic curves (AUROC).Results: The study comprised 801 patients with non-DR, 330 patients with NPDR, and 99 patients with PDR. The systemic factors associated with PDR were: duration of diabetes(P < 0.000), Haemoglobin A1c (P = 0.015) , fasting plasma glucose (P = 0.001), total cholesterol(P = 0.007), estimated glomerular filtration rate (P = 0.024), and MPV(P = 0.005). The mean platelet volume (MPV) demonstrated an increasing trend with the severity of DR, whose differences were statistically significant among the three groups(P = 0.045). The prediction model which was based on multi-factors showed stronger predictive power for PDR (AUROC=0.790) than these risk factors of PDR. Conclusions: Some systemic factors including MPV are independently associated with PDR in T2DM. The prediction model might be a beneficial prognostic marker of PDR in patients with T2DM.
Relationship between atherosclerotic cardiovascular disease and diabetic retinopathy in patients with type 2 diabetes mellitus

Abstract:This study aimed to explore the potential correlation between atherosclerotic cardiovascular disease (ASCVD) and diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (T2DM). We enrolled 6540 patients with T2DM who were receiving chronic disease management for hypertension, hyperglycemia, and hyperlipidemia in Chengyang District of Qingdao. Among them, 730 had ASCVD (ASCVD group), which 5810 did not (N-ASCVD group). The results showed significantly higher levels of age, blood glucose, glycosylated hemoglobin (HbA1c), systolic blood pressure, ASCVD family history, female proportion, and DR incidence in the N-ASCVD group. Additionally, the glomerular filtration rate was significantly lower in the ASCVD group. Logistic regression analysis revealed a positive correlation between DR and ASCVD risk. DR was further categorized into 2 subtypes, nonproliferative DR (NPDR) and proliferative DR (PDR), based on e lesion severity. Interestingly, only the PDR was associated with ASCVD. Even after accounting for traditional ASCVD risk factors such as age, sex, and family history, PDR remained associated with ASCVD, with a staggering 718% increase in the risk for patients with PDR. Therefore, there is a strong association between ASCVD and DR in individuals with T2DM, with PDR particularly exhibiting an independent and positive correlation with increased ASCVD risk.
