Abstract:Background: Previous studies have demonstrated that Tc-99m-sestamibi (Tc-99m- MIBI) Single-Photon Emission Computed Tomography/Computed Tomography (SPECT/CT) imaging is an effective isotopic technique for locating the parathyroid in secondary hyperparathyroidism (SHPT). This study aimed to explore further the correlation between Tc-99m-MIBI SPECT/CT imaging and SHPT to demonstrate the value of Tc-99m-MIBI SPECT/CT in evaluating the degree of pathological hyperplasia of the parathyroid gland (PG). Methods: The demographics, surgical records, and follow-up information of 91 patients were recorded and analyzed. A total of 216 paraffin-embedded PGs of 54 patients were obtained and analyzed. Results: Patients with Tc-99m-MIBI negative PG(s) had significantly lower preoperative serum phosphorus and higher serum calcium levels at 6 months postoperatively compared to those with Tc-99m-MIBI positive PG(s) (P<0.05). We also found a higher total uptake ratio of the region of interest (URRI) and higher URRI max in the hypocalcemia group than in the non-hypocalcemia group. Both URRI total (P=0.003) and URRI max (P=0.028) were independent risk factors for hypocalcemia 6 months postoperatively. The URRI values of the PGs were significantly positively correlated with glandular weight (R-2=0.343, P<0.001), glandular volume (R-2=0.240, P<0.001), and degree of pathological hyperplasia (P<0.001). However, the URRI value of the PGs exhibited a notably weak correlation with proliferating cell nuclear antigen (PCNA) (R-2=0.035, P=0.006). The area under the receiver operating characteristic curve showed a URRI evaluative value of 0.771 for diffuse and nodular types in 216 PGs (P<0.001). We further evaluated 167 nodular-type PGs, distinguishing between nodular hyperplasia and a single nodule; the URRI evaluative value reached 0.819, which was higher than the volume or weight (P<0.001). Conclusions: The Tc-99m-MIBI SPECT/CT scintigraphy results were related to serum calcium levels at 6 months after total parathyroidectomy with autotransplantation (TPTX+AT), suggesting the occurrence of hypocalcemia (6 months after TPTX+AT). More importantly, this technique effectively evaluated the pathological hyperplasia of PGs preoperatively, and therefore, could assist surgeons in selecting the PGs with the lowest degree of hyperplasia intraoperatively.

Abstract:Objective To evaluate the role of radionuclide imaging in diagnosis of hyperfunctioning parathyroid tumor and early etiology ascertaining of bone pain and pathologic fracture. Methods Retrospectively studied on the data of radionuclide imaging using 99Tcm-methylene diphosphonic acid (MDP) and 99Tcm-methoxyisobu-tylisonitrile (MTBI). The data was collected from 26 cases with hyperfunctioning parathyroid glands by orthopedics department in this hospital .during the last 11 years. Results Operations and pathologic studies had been performed on all of the 26 cases, including 2 cases with adenoma and 1 case with carcinoma. All cases showed obvious signs of general skeletal metabolic change of hyperparathyroidism oh bone scintigraphy. All the metabolic indexes were ≥5. The features of typical appearance were the ' black skull' and extraordinarily clear bone images. Total 26 cases underwent ultrasonography of the neck and 18 cases of them had double-phase radionuclide parathyroid imaging. All of the 18 cases had positive results and showed accurate position of glands in double-phase hy-perparathyroid radionuclide imaging. Conclusion Signs of bone metabolic change of hyperparathyroidism in bone scintigraphy for the patients with bone pain and pathologic fracture are necessary for the early etiological ascertaining; the hyperfunctioning parathyroid glands can be showed accurately in the parathyroid radionuclide imaging, and that is very important for diagnosing and locating hyperfunctioning parathyroid tumors.
Biochemical and pathologic factors affecting technetium-99m-methoxyisobutylisonitrile imaging results in patients with primary hyperparathyroidism

Abstract:Objective: Technetium 99 m methoxyisobutylisonitrile (Tc-99m MIBI) scintigraphy represents the most commonly utilized imaging modality for the detection of the diseased gland in patients with primary hyperparathyroidism (PHPT). In this study, we aimed to identify potential biological factors with an impact on MIBI sensitivity. Methods: A total of 147 patients with surgically confirmed parathyroid adenomas were assessed retrospectively. Data including medical history, biochemical and hormonal measurements, cervical US, Tc-99m MIBI scans as well as pathology reports were retrieved and recorded. Results: Of the 147 patients, there were a total of 77, 39, and 31 cases with a positive, negative, and suspicious parathyroid adenoma on Tc-99m MIBI scan, respectively. Serum calcium (Ca), parathyroid hormone (PTH) and 25 (OH) D levels were comparable among MIBI positive and negative patients [Ca: 11.5 ± 0.9 vs 11.3 ± 0.9 mg/dL (P = 0.42); PTH: 216 (146-347) vs 194 (140-317) pg/mL (P = 0.45); 25(OH)D: 8.4 (5.7-18.2) vs 10.0 (4.7-23.3) ng/mL (P = 0.64), respectively]. P-glycoprotein (P-gp) staining was negative in both groups. Also, pathological examination of tissue preparations revealed no difference in terms of the volume of the adenomas, incidence of cystic adenomas, cell-type dominance (oxyphilic cell), percent fat, and Ki-67 ratio in MIBI positive and negative groups. The rate of hyalinization was 13% in MIBI positive and 28% in MIBI negative subjects, the difference being statistically significant (P = 0.04). Conclusion: Presence of hyalinization in parathyroid adenomas was found to be negatively correlated with MIBI scan results.

Abstract:Objective To compare double phase 99mTc-MIBI imaging with ultra-sound,CT and MRI in the diagnosis of hyperparathyroidism.Methods Sixty-five patients with clinical diagnosis of hyperparathyroidism were injected intravenously with 370 MBq(10 mCi) of 99mTc-MIBI,and imaged 15 min later as early phase imaging and 120 min later as delayed phase imaging respectively.All patients under went B ultra-sound examination;20 and 11 patients underwent CT and MRI,respectively.Results Thirty-six cases among 65 patients showed high level of parathyroid homone(PTH),which was considered as diagnostic criterion for hyperparathyroidism.Among these,34 showed positive radionuclide parathyroid images,with the sensitivity,specificity and accuracy of 94.4%,100% and 96.9%,respectively.Thirty-two patients showed abnormal ultra-sound results,and the sensitivity,specificity and accuracy were 88.9%,93.1% and 90.8%,respectively.Seventeen CT images and 8 MR images showed positive results,respectively.Thirty-five adenomas and 1 histo-pathologically proved hyperplastic gland were removed from 34 patients.Conclusion Double phase 99mTc-MIBI imaging is a kind of useful and reliable approach for localization of parathyroid adenomas.Combined with other examinations,double phase 99mTc-MIBI imaging can improve the accuracy of diagnosis of the parathyroid lesions before surgery.

Abstract:OBJECTIVE:To evaluate the efficiency of (99)Tc(m)-sestamibi (MIBI) in the diagnosis of parathyroid diseases with primary hyperparathyroidism.METHODS:One hundred and four cases with primary hyperparathyroidism underwent MIBI and parathyroidectomy between May 2010 to November 2013 were reviewed. With MIBI, single adenoma was found in 91 cases, two adenomas in 5 cases, carcinoma and hyperplasia in 3 cases respectively. Primary hyperparathyroidism was diagnosed based on the associated symptoms and signs in 93 patients, while it was indicated in 11 asymptomatic patients due to the presence of hypercalcemia in health examination. Parathroid hormone levels ranged from 98 to 2 800 ng/L, and serum calcium levels were between 2.56-4.23 mmol/L.RESULTS:The sensitivity of MIBI for total diseased parntyroids was 84.0% (100/119), and that for parathyroid adenoma, parnthyroid carcinoma, atypical adenoma and primary parathyroid hyperplasia was 87.1% (88/101), 3/3, 2/2 and 7/13, respectively.CONCLUSIONS:MIBI is efficient in the diagnosis of parathyroid diseases with primary hyperparathyroidism. The size and cystic degeneration of parathroid disease are two common factors decreasing MIBI reliability, and thyroid nodule is additional source leading to false-positive results.
99Mtco4-/99Mtc-mibi Dual-Tracer Scintigraphy for Preoperative Localization of Parathyroid Adenomas.

Abstract:OBJECTIVE:To investigate the accuracy of dual-tracer scintigraphy for locating parathyroid adenomas in patients with primary hyperparathyroidism (PHPT).METHODS:We reviewed 268 patients with PHPT. All patients underwent technetium-99m pertechnetate (99mTcO4-) scintigraphy and technetium-99m methoxyisobutylisonitrile (99mTc-MIBI) dual-tracer scintigraphy of the thyroid and parathyroid glands, respectively. 99mTcO4- planar scintigraphy was carried out initially followed by dual-phase 99mTc-MIBI single-photon emission computed tomography (SPECT)/CT the next day. The findings were combined and interpreted. Individual 99mTc-MIBI and dual-tracer scintigraphy were both analyzed. The sensitivity, specificity, and accuracy were determined in relation to surgical findings. The average interval between scan and surgery was 13 days.RESULTS:The positive and negative predictive values of 99mTc-MIBI SPECT/CT were 92.0% and 71.3%, respectively, and the sensitivity, specificity, and accuracy were 88.3%, 79.2%, and 85.8%, respectively. The positive and negative predictive values of dual-tracer scintigraphy were 96.3% and 82.3%, respectively, and the sensitivity, specificity, and accuracy were 92.9%, 90.3%, and 92.2%, respectively. Youden's index for dual-tracer scintigraphy and 99mTc-MIBI SPECT/CT were 0.83 and 0.63, respectively.CONCLUSIONS:These finding suggest that 99mTcO4- and 99mTc-MIBI dual-tracer scintigraphy is more accurate than other scintigraphy methods for detecting parathyroid adenoma, and may thus be the most suitable imaging technique in patients with PHPT.

Parathyroid scintigraphy and whole-body bone scans can demonstrate abnormal parathyroid uptake and skeletal changes caused by hyperparathyroidism. We illustrate a case of a typical parathyroid adenoma and its bone changes on whole-body bone scans and Tc-99m sestamibi parathyroid imaging.

Abstract:Abstract A 53-year-old man with follicular thyroid carcinoma (FTC) was referred for renal scintigraphy using 99m Tc-DTPA to assess the kidney function. Unexpectedly, the images showed an abnormal uptake of radiotracer in the right pelvic region. It corresponded to the site of metastasis in the right ilium revealed on 131 I SPECT/CT images. The biopsy pathology of the ilium lesion demonstrated follicular thyroid cancer.

The Value of Tc-99m-MIBI Multi-Temporal SPECT and SPECT/CT in Patients after Total Parathyroidectomy with Autotransplantation

Abstract:234 Objectives: Total parathyroidectomy with autotransplantation(tPTX+AT) is a suitable procedure for renal hyperparathyroidism(HPT),which showed significantly often normalization of serum calcium and alkaline phosphatase as well as significant improvement of clinical signs like pruritus and muscle weakness.However,the rate of persistent or recurrent disease after tPTX+AT is high, and it is usually related to hyperplasia of the autografted tissue. 99mTc-MIBI scintigraphy is a reliable non-invasive exploratory tool in the detection of recurrent parathyroid disease in the neck or mediastinum.But little is known about its role in detecting the recurrence of autografted tissue. In this study,we aim to investigate the value of 99mTc-MIBI multi-temporal SPECT and SPECT/CT in detecting the localization of recurrence after tPTX+AT. Methods: The study group comprised 18 consecutive adult patients(4 males,14 females,age range: 33-66years) from December 2011 to July 2017 who have received tPTX+AT. All patients with suspected recurrence due to clinical or biochemical findings(eg.osteodynia,serum elevated level of parathormone) underwent 99mTc-MIBI multi-temporal SPECT and SPECT/CT of the neck, mediastinum and graft area. Multi-temporal SPECT images for these areas were acquired at early phase(15min)and delay phase(60min, 90min and 120min)after injection.SPECT/CT images were obtained at early phase using a 16-slice diagnostic CT system.Imaging results were confirmed by biopsy or further clinical and radiologic work-up. The detection rates for multi-temporal SPECT and SPECT/CT to identify hyperparathyroidism lesions were calculated. The tracer uptake by the lesions in the autograft area was assessed semi-quantitatively employing region of interest ratios to normal adjacent areas. Spearman’s rank correlation analysis was used to determine the association of the tracer uptake with serum levels of intact parathormone(iPTH) ,calcium and phosphorus. Results:99mTc-MIBI multi-temporal SPECT had detected 21 lesions including 16 in the graft site and 5 in the neck.Meanwhile, SPECT/CT had shown 24 lesions including 18 in the graft site and 6 in the neck.Two lesions in the autograft site were both missed by SPECT and SPECT/CT.One false-positive lesion proved by pathology was foreign body granuloma in the graft area,and pathological results were obtained in other 20 lesions,including 4 adenomas and 16 hyperplasias.The detection rates of hyperparathyroidism lesions by multi-temporal SPECT and SPECT/CT were 77.8%(21/27) and 88.9%(24/27),respectively,without statistical significance (P>0.05).In multi-phase SPECT,all 16 lesions in the graft sites showed the highest uptake of 99mTc-MIBI in 15min scan, and gradually reduced in the delayed scan. Five lesions located in the neck were detected in delayed imaging.The Spearman’s correlation analysis showed that the the tracer uptake had no significant correlation with levels of iPTH,calcium and phosphorus (P>0.05). Conclusion: In this retrospective study, 99mTc-MIBI multi-temporal SPECT and SPECT/CT images may provide helps for localization of the hyperparathyroidism glands accurately.It results that it is not necessary to perform delay imaging for autograft area.Future prospective studies in larger patient groups are desirable for confirming our findings.
Evaluation of 99Mtc-Mibi Imaging for Thyroid Carcinoma in Follow-Up and Postoperative Study

Abstract:The aim of this study was to evaluate the value of 99mTc-MIBI imaging in 46 cases of thyroid masses (18 malignant, 28 benign and 24 cases of postoperative relapse or metastatic thyroid cancer. Results showed that radioactivity filling in primary cancer lesion was observed in 14 out of 18 thyroid carcinoma, whereas only 9 out of 28 in benign lesions. 99mTc-MIBI imaging was positive in 19 out of 24 patients wi th postoperative relapse or lymph nods metastases. Its positivity was signicantly higher than 131I imaging, thereby 99mTc-MIBI imaging was highly va luable in detecting early thyroid carcinoma, especially, in finding out distant metastases and postoperative relapse.
99Tcm-Mdp Bone Scan in Diagnosis of Multiple Myeloma

Abstract:Objective To investigate the clinical value of (~(99)Tc~m)-MDP bone scan in multiple myeloma. Methods A retrospective analysis of 15 patients with multiple myeloma verified by laboratory examination and histopathologically finding on marrow biopsy specimens was performed. All of the patients underwent SPECT whole body scan with (~(99)Tc~m)-MDP and X-ray examination, and 7 of them underwent also X-CT scan. Results Foci of 15 patients with multiple myeloma located mainly in rib, spine, lumbar and skull, next in scapula, pelvis, limbs, sternum and calvicle. The SPECT whole body bone scintigraphy showed multiple abnormal increasing or combined with increasing and decreasing or defect of radioactive uptake in the lesions. The findings of X-ray or CT scan indicated osteolytic destruction. Conclusion SPECT combined with conventional X-ray and CT scan might be the preferred imaging examination method for multiple myeloma. SPECT bone imaging has important values in monitoring chemotherapeutic efficacy for multiple myeloma and in evaluating its prognosis prediction.

Abstract:Objective: We evaluated the difference in parathyroid visualization on 18 F-FCH PET/CT images obtained at 5 and 60 min, and quantitatively analyzed the mode of FCH uptake at different time points, to determine the best imaging time for FCH PET/CT. Methods: This retrospective study included 73 patients with hyperparathyroidism (HPT) who underwent 18 F-FCH PET/CT imaging between December 2017 and December 2021. The diagnostic efficiency of 5- and 60-min dual time point imaging for the diagnosis of hyperparathyroidism and parathyroid adenoma and hyperplasia, were compared using visual and quantitative analyses. Results: Dual-time 18 F-FCH PET/CT imaging visual analysis had diagnostic value for HPT. The receiver operating characteristic curve of PET/CT quantitative parameters for the diagnosis of HPT and lesions showed that the parathyroid/thyroid SUVmax ratio for 60-min imaging had a higher sensitivity and specificity (based on patient, sensitivity: 90.90% and specificity: 85.71%; based on focus, sensitivity: 83.06% and specificity: 85.71%) compared to that for 5-min imaging. PET/CT quantitative parameters can distinguish parathyroid adenoma and hyperplasia. The 60-min parathyroid SUVmax value had the highest diagnostic value (cutoff: 3.945; area under the curve: 0.783). Conclusion: The quantitative parameters of 60min 18 F-FCH PET/CT have more advantages in aiding in the pathologica diagnosis and clinical treatment of HPT.

Clinical study on Tc-99m-MDP whole body bone scan combined with MR on the diagnostic value of bone metastasis

Abstract:1427 Objectives Clinical study about bone metastasis using whole-body bone imaging combined with MRI was very few. The aim of the study is to evaluate diagnostic value of Tc-99m-MDP whole body bone scan combined with MRI on the bone metastasis in patients with malignancies. Methods A total of 50 patients with bone metastasis enrolled in this study. All patients underwent MRI and whole body bone scan (BS) with Tc-99m-MDP in two weeks. According to the diagnostic results of patients，the sensitivity，specificity，accuracy, positive predictive value (PPV)and negative predictive value (NPV) of MRI，BS and MRI combined with BS method to detect bone metastasis were calculated．Moreover，we compared diagnostic value between BS and MRI combined with BS methods by McNemar test. Results (1) On the patient biasis : for MRI, BS and MRI combined with BS，the sensitivity of detecting bone metastasis was 100%(50/50), 96%(48/50)and 100%(50/50)，respectively. (2) On the lesion biasis: there were 182 and 12 diagnosed with bone metastasis and belign lesion among all 194 foci by MRI and BS, respectively．For MRI, BS and MRI combined with BS，the sensitivity, accuracy and PPV of detecting bone metastasis was 100%(182/182), 100%(194/194), 90.91%(182/182); 67.03%(122/182), 62.89%(122/194), 91.04%(122/134) and 100%(182/182), 100%(194/194), 90.91%(182/182)respectively. The diagnostic value of MRI combined with WBS was obviously higher than that of BS (P Conclusions All types of bone metastasis were can be detected by MRI. The diagnostic value of MRI combined with BS on bone marrow metastasis and lytic bone metastasis was obviously better than that of BS only.

Abstract:Abstract Background Secondary hyperparathyroidism (SHPT) usually requires parathyroidectomy when drug regimens fail. However, obtaining an exact preoperative map of the locations of the parathyroid glands is a challenge. The purpose of this study was to compare the diagnostic performance of US, dual-phase 99m Tc-MIBI scintigraphy, early and delayed 99m Tc-MIBI SPECT/CT in patients with SHPT. Methods Sixty patients with SHPT who were undergoing dialysis were evaluated preoperatively by US, dual-phase 99m Tc-MIBI scintigraphy, early and delayed 99m Tc-MIBI SPECT/CT. Postoperative pathology served as the gold standard. The sensitivity, specificity, and accuracy were determined for each method. Spearman correlation analysis was used to analyse the correlation of hyperplastic parathyroid calcification with serum alkaline phosphatase (ALP) and parathyroid hormone (PTH). Results A total of 229 lesions in 60 patients were pathologically confirmed to be parathyroid hyperplasia, with 209 lesions in typical sites, 15 lesions in the upper mediastinum and 5 lesions in the thyroid. A total of 88.33% (53/60) of patients had four lesions. US, early and delayed 99m Tc-MIBI SPECT/CT had significantly higher sensitivity and accuracy than dual-phase 99m Tc-MIBI scintigraphy ( P < 0.001). Furthermore, early 99m Tc-MIBI SPECT/CT had significantly higher sensitivity ( P < 0.001) and accuracy ( P = 0.001 and P < 0.001) than US and delayed 99m Tc-MIBI SPECT/CT. In patients with ectopic hyperplastic parathyroid glands, the sensitivity of early 99m Tc-MIBI SPECT/CT (90%) was significantly higher than that of US (55%) and dual-phase 99m Tc-MIBI scintigraphy (50%) ( P < 0.05). The Spearman correlation results showed a significant albeit weak association between calcification and both serum PTH and ALP ( P = 0.002). Conclusion The ability of early 99m Tc-MIBI SPECT/CT to detect hyperplastic parathyroid glands in patients with SHPT is superior to that of US, dual-phase 99m Tc-MIBI scintigraphy and delayed 99m Tc-MIBI SPECT/CT; furthermore, dual-phase 99m Tc-MIBI SPECT/CT is not essential.

Abstract:Objective To investigate the usefulness of ultrasound (US) for the localization of ectopic hyperparathyroidism and compare it with 99m Tc-sestamibi ( 99m Tc-MIBI), 4-dimensional computed tomography (4D-CT), and 11 C-choline positron emission tomography/ computed tomography (PET/CT). Methods Of the 527 patients with surgically confirmed primary hyperparathyroidism , 79 patients with ectopic hyperparathyroidism were enrolled. The diagnostic performance of US, 99m Tc-MIBI, US + MIBI, 4D-CT, and 11 C-choline PET/CT was calculated, and the factors affecting the sensitivity of US and 99m Tc-MIBI were analyzed. Results Eighty-three ectopic parathyroid lesions were found in 79 patients. The sensitivity was 75.9%, 81.7%, 95.1%, 83.3%, and 100% for US, 99mTc-MIBI, US + MIBI, 4D-CT, and 11 C-choline PET/CT, respectively. The difference in sensitivity among these different modalities did not achieve statistical significance ( P > .05). The US sensitivity was significantly higher for ectopic lesions in the neck region than for those in the anterior mediastinum/chest wall (85.9% vs. 42.1%, P < .001). The 99m Tc-MIBI and 4D-CT sensitivity was not significantly different between these two groups (84.1% vs. 94.6%, P = .193 and 81.3% vs. 85.7%, P = 1). The 11 C-choline PET/CT sensitivity was 100% in both groups. Conclusions US is a valuable tool for the localization of ectopic hyperparathyroidism, especially for ectopic lesions in the neck region.

Abstract:Objective To evaluate the relationship between the proliferation of parathyroid cell in rabbit with primary hyperparathyroidism (PHPT) and the bone mineral density (BMD). Methods Eighty adult Chinese rabbits were randomly and equally divided into two groups. The contrast group was fed with normal diet ( Ca ∶ P, 1.0 ∶ 0. 7 ) and the experimental group was fed with high phosphate diet ( Ca ∶ P,1.0∶7.0) to establish the animal model of PHPT. At 3, 4, 5, and 6 months after the diet, bone mineral density of the rabbits was measured by the quantity CT (QCT). Then, the parathyroid and bone of the rabbits were removed for pathological examination. The number of parathyroid cell in PHPT was calculated.Proliferation was determined by immunohistochemistry of proliferation cell nuclear antigen ( PCNA ) and Bcl-2. The t test and Logistic regression was used to analyze the difference of data of two groups. Result The number of parathyroid cell in PHPT group was 1.61 times than that in the contrast group[ (673 ± 151 ) HP,(418 ± 25 ) HP,P ＜0. 01]. The rate of PCNA positive-cell was significantly increased in PHPT group than that in contrast group [(50.52 ± 11.62)％o, (26.70 ± 2. 78 )％, P ＜ 0.01], and so was Bcl-2[ (460. 37 ± 190. 05 )‰, (67. 02 ±:4. 38 )％‰,P ＜0. 05]. The value of BMD was significantly decreased in PHPT group than that in contrast group [ ( 152. 5 ± 34. 3 ), ( 188.6 ± 12. 2 ) g/cm3, P ＜ 0. 05]. There was a negative correlation between BMD and PCNA (r ＝ -0. 749, P ＜ 0. 05 ) and between BMD and Bcl-2 (r ＝-0.800, P ＜ 0. 05 ) in PHPT group. Conclusion The BMD of PHPT is related to the parathyroid cells proliferation which provide a reliable method for early diagnosis of PHPT.

Abstract:Multiple myeloma (MM) is a clonal plasma cell malignancy, which is the most common tumor in bone. The "gold standard" for imaging myeloma bone lesions is the bone X-ray survey. However, there are many restrictions in detecting bone disease. Recently, some studies indicated that 99mTc-MIBI scintigraphy reflected myeloma disease activity in bone marrow with very high sensitivity and specificity. In this article, we will review the researchs of 99mTc-MIBI scintigraphy in diagnosising, prognosting evaluating and determinationg therapy responses of patients with Multiple myeloma.
Identification of independent factors affecting bone mineral density after successful parathyroidectomy for symptomatic hyperparathyroidism

Abstract:Background: Studies have shown that the response of bone mineral density (BMD) to parathyroidectomy for symptomatic primary hyperparathyroidism (PHPT) is heterogeneous and difficult to predict. However, the independent factors affecting BMD in PHPT patients after parathyroidectomy remains limited and inconclusive. This study aimed to explore the independent factors affecting BMD changes in symptomatic PHPT patients after parathyroidectomy. Methods: This study retrospectively analyzed 105 patients with symptomatic PHPT treated at Beijing Jishuitan Hospital between January 2010 and December 2015. The primary outcome was a > 10% increase in BMD at 3 years after parathyroidectomy compared with the preoperative value, whereas the secondary outcomes were BMD changes at various measurement sites. Results: A total of 105 patients with a mean age of 46.37 years were included in this study. Univariate logistic regression analysis indicated that hypertension (odds ratio [OR[: 0.032; 95% confidence interval [CI]: 0.001-0.475; P = 0.012), and parathyroid hormone level (OR: 1.006; 95% CI: 1.004-1.009; P = 0.044) were associated with the > 10% BMD increase. However, these results were not significant after adjustments for potential confounders. Moreover, the BMD values at the lumbar spine, femoral neck, femoral trochanter, Ward's triangle, and whole body after parathyroidectomy were significantly greater than those before the operation (P < 0.05). Conclusions: This study suggests that patient characteristics were not associated with the > 10% BMD increase. However, the BMD values of the femur and lumbar spine were significantly increased in symptomatic PHPT patients after parathyroidectomy.
Diagnostic Value Of Tc-99m-Sestamibi Planar Imaging And Spect/Ct For Secondary Hyperparathyroidism

Abstract:Objectives: To explore the diagnostic value of Tc-99m-Sestamibi multiple-phase static planar imaging and SPECT/CT in secondary hyperparathyroidism. Methods: This was a retrospective analysis of 40 cases of clinically diagnosed secondary hyperparathyroidism with multiple-phase static planar imaging, 15 min SPECT/CT fusion imaging, and ultrasonography. All cases were confirmed by pathological examination. Detection of lesions was compared between dual-phase and multi-temporal planar imaging, the detection rate of lesions using planar imaging, SPECT/CT, and ultrasonography was analyzed, and the characteristics of parathyroid tissue calcification detected by SPECT/CT were summarized. Results: The number of lesions detected was 69 by multi-temporal planar imaging and 57 by dual-phase imaging; scans at both 60 min and 90 min detected 12 lesions. SPECT/CT detected more lesions than planar imaging and ultrasonography; the sensitivities were 93.1%, 47.9%, and 70.1%, the accuracy was 88.7%, 47.6%, and 69.7%, respectively, and the specificity was 100%. Eighteen of 40 patients had parathyroid tissue calcification, 134 lesions in 32 patients exhibited calcification, and 90.6% of the 32 lesions were Tc-99m-Sestamibi positive. Conclusions: Multi-temporal planar imaging detected more lesions than dual-phase imaging. SPECT/CT plays an important role in the qualitative and quantitative evaluation and positioning of secondary hyperparathyroidism lesions.
