Abstract:Purpose: Suprachoroidal hemorrhage (SCH) is a rare but severely feared sight-threatening complication of intraocular surgery, and its management remains debatable. We intended to summarize the existing surgical management options regarding perioperative SCH, describing different techniques and their respective visual outcomes. Methods: A systematic literature search of articles published since 1st January 2011 until 31st December 2022 was performed using MEDLINE (PubMed) and Scopus. Eligibility criteria included the adult population with SCH related to intraocular surgery. Results: Thirty-eight studies enrolling 393 patients/eyes were assessed after a selection process among 525 records. We included 5 retrospective cohort studies, 15 case series and 18 case reports. We documented cases of acute SCH diagnosed intraoperatively and delayed SCH, treated until a maximum of 120 days after the diagnosis. Best corrected visual acuity at diagnosis was generally poor, with variable final visual outcomes. Techniques of external drainage with or without combined pars plana vitrectomy (PPV), type of endotamponade (if PPV performed), anterior chamber maintainer and reports of the use of recombinant tissue plasminogen activator were described. Conclusion: To the best of our knowledge, this is the first systematic review assessing perioperative SCH and its surgical management. There is no standardized surgical approach of SCH and longitudinal intervention studies are lacking. To ensure that patients achieve the best possible visual outcome, prompt diagnosis and treatment are crucial. Therefore, further clinical research is on demand to improve the management of this clinical sight-threatening entity.

Abstract:BackgroundTo identify the clinical characteristics, prognostic factors and visual outcomes in posterior segment IOFBs patients managed by PPV in a tertiary hospital.MethodsA retrospective chart review was performed for 56 patients, who had PPV for IOFBs removal between November 2013 and November 2015. The mechanisms of injury, the nature of the IOFBs, the BCVA before and after the surgery, the penetrating site and the complications of the surgery were all collected. Univariate analyses were conducted to evaluate the prognostic factors.ResultsThe mean age of the patients was 36.4years. The nature of IOFBs was mainly metal. Most injuries were commonly caused by hammering the metal. The mean preoperative VA was 2.30 logMAR, and mean final VA was 0.92 logMAR. From univariate analysis, good visual outcome was correlated with the good visual acuity before surgery and poor visual outcome was correlated with the macular break and multiple surgeries.ConclusionsIn a tertiary hospital of eastern China, most cases of IOFBs were work-related. The prognosis of the patients was really well in the patients with good presenting visual acuity. Nevertheless the prognosis was not good for those patients who had macular injury or underwent several surgeries because of retinal detachment, epiretinal membrane or proliferative vitreous retinopathy. Good facilities for eye protection are urgently in demand for the workers indeed.
Clinical Effect of Treatment on Patients with Ruptured Intracranial Suprutentorial Aneurysms During the Intermediate Period or Delayed Period

Abstract:Objective To compare the effect of microsurgery for ruptured intracranial supratentorial uneurysms during intermediate period versus delayed period and to evaluate the feasibility of surgical clipping during intermediate period. Method A retrospective analysis of 95 patients with Hunt-Hess Grade Ⅰ～Ⅲ on admission who were admitted 4 to 10 days after initial supratentorial aneurysmal subarachnoid haemorrhage. Forty-one patients underwent intermediate clipping while 54 patients accepted delayed obliteration of ruptured aneurysm (days 11-30). Outcome after 6 months was assessed using Modified Rankin Scale(mRS). Results There was no difference in outcome between two groups according to mRS at 6 months follow-up. Thirty patients(73.2%) in the intermediate group and 42 patients(77.8%) in the late group had a favorable functional outcome with a mRS score of 0 to 2. In 39.1% of the 23 patients who had poor outcome, rebleeding before active treatment was the leading factor contributing to poor outcome. Conclusions Surgical clipping of ruptured intracranial aneurysms during intermediate period provided good opportunity for patients to achieve favorable outcomes especially for those in good clinical condition.

Abstract:Objective To analyze risk factors,management,and prevention of suprachriodal expulsive hemorrhage(SEH) during cataract surgery.Methods A retrospective study was done on the SEH in cataract surgery between 2003 and 2008.During this period,2810 eyes of(2540 cases) cataract extractions(phacoemulsification and ECCE) were performed.There were three patients who developed intraoperative SHE.The wound was immediately closed with sutures,intravenous mannitol and other drugs were administered to control the blood pressure and the intraocular pressure.Calmming drugs were also used to the patients.If necesery,intraoperative sclerostomy was performed.After surgery these drugs were also used to further control the blood pressure,the intraocular pressure,hemostasis and inflammation. Results After surgery,one patient had on light perception because of retina detachment;one case had the vision of 0.3 because of discover in time and treatment decisively;the other one also remain the useful vision of 0.1 because of successful draining the blood.Conclusion The venous of choroid expanded significantly when the intraocular tension decreased to zero after the eyeball was incisied,this leaded to the vessel-wall rupture.The other risk factors for the development of intraoperative suprachoroidal hemorrhage were: high myopia,glaucoma and systemic diseases(hypertension,diabetes,cough,asthma,overstress).After above treatment,some patients reserved a part of vision.
Incidence and risk factors of perioperative suprachoroidal hemorrhage: A systematic review and meta-analysis

Abstract:Suprachoroidal hemorrhage (SCH) is a potentially visually devastating complication of intraocular surgery, but estimates of perioperative SCH incidence vary. We performed a systematic review and meta-analysis of perioperative SCH incidence among population-based studies published between 1990 and 2023. Thirty-five studies collectively reported 1,657 cases of perioperative SCH from a population of 3,028,911 surgeries. The estimated incidence of SCH was 0.12% (95% CI, 0.10-0.14%), or about one in every 800 surgeries. The estimated incidence of perioperative massive SCH was 0.06% (95% CI, 0.04-0.08%). In multivariable meta-regression, greater SCH incidence was significantly associated with smaller study population size, comparative study design, multicenter study setting, and intraoperative or delayed SCH timing (vs intraoperative alone), while lower SCH incidence was significantly associated with vitreoretinal or mixed surgery type (vs. cataract) (all P < 0.05). Study year was not a significant predictor of SCH incidence, suggesting that the incidence of SCH has not decreased over the past 3 decades despite improvements in surgical technologies and techniques. Given the rarity of SCH, and the strong effect of study population size on reported SCH incidence rates, future studies of SCH incidence should include a minimum population size of at least 1000 surgeries to obtain an accurate estimate of SCH incidence.

Abstract:Background: Suprachoroidal hemorrhage (SCH) is a rare but dangerous complication of intraocular surgery. There are some risk factors for this devastating complication during pars plana vitrectomy. In this case, we will report the intraoperative occurrence of SCH during pars plana vitrectomy in traumatized eyes.Methods: Retrospective analysis of SCH during pars plana vitrectomy in five eyes with the history of blunt (n = 1) or penetrating (n = 4) trauma was made. Baseline systemic and ocular characteristics, surgical procedures, time point of SCH, management of SCH, and final visual outcomes were measured.Results: One eye with associated myopia developed SCH during the time of producing vitreous posterior detachment under ocular hypotony. In other two eyes, SCH developed under ocular hypotony during fluid-gas exchange. The remaining two eyes got SCH when the depression of the area of pars plana occurred. Sclerotomy closure was performed immediately once SCH occurred. Vitrectomy and posterior sclerotomy were then performed between 8 and 12 days later. After a median follow-up of 12 months (range: 3-20 months), final visual acuity was above 20/400 in four eyes, no light perception in one eye, the best visual acuity was 20/60.Conclusions: Ocular trauma is one of the vital risk factors for the development of intraoperative SCH during pars plana vitrectomy. It is important to control effectively the intraocular inflammation preoperatively and avoid abrupt ocular hypotony and pressure on the area of pars plana intraoperatively to the limit.

Abstract:Purpose: To analyze the characteristics, related risk factors, and prognosis of suprachoroidal hemorrhage (SCH) associated with pars plana vitrectomy (PPV). Methods: Cases of SCH associated with PPV excluding trauma were retrospectively analyzed in Beijing Tongren Hospital between January 2010 and June 2020. The data collected included general data, myopia status, axial length, state of the crystalline lens, SCH onset time, range, treatment method, visual prognosis, and methods of operation and anesthesia. Patients were divided into those with SCH related to the first PPV (Group 1), and SCH related to second intraocular surgery in the vitrectomized eye (Group 2). Patients were also classified by the SCH onset time into either the expulsive suprachoroidal hemorrhage group (ESCH) and the delayed suprachoroidal hemorrhage group (DSCH). The general data, related risk factors, and the visual prognosis of SCH in the different groups were analyzed. Results: SCH associated with PPV was studied in 28 cases with an incidence of 0.06 %; 16 males and 12 females. The mean age of the patients was (53.51 ± 10.21) years old, the mean follow-up time was (24.94 ± 14.60) days, and the mean axial length was (28.21 ± 3.14) mm. Of these cases, 21 were classified as high myopia, 25 as aphakia/ pseudophakic, and 7 as focal hemorrhage. Silicone oil removal occurred in 12 cases (43 %). Patients in Group 2 were younger than Group 1 (P = 0.005). In terms of treatment and prognosis, 5 eyes were simply closely observed, 4 were given single suprachoroidal drainage, 15 were given suprachoroidal drainage combined with silicone tamponade, 2 underwent anterior chamber puncture, and 2 gave up treatment. A follow-up vision: NLP ~ 20/30; among them, 2 eyes with NLP (7.14 %), 6 of ≥ 20/200 (21.43 %). The final outcomes presented a significantly positive correlation with baseline vision but no significant correlation with age or axial length. Conclusions: SCH has a higher incidence rate after a second intraocular surgery in a vitrectomized eye which is associated with the lack of vitreous support and easier fluctuation of intraocular pressure. SCH associated with PPV is more localized and has a relatively good prognosis; high myopia and aphakic/ pseudophakic eyes are risk factors. Active treatment can effectively improve visual prognosis. Suprachoroidal hemorrhage as a complication of vitrectomy

Abstract:Objective: To study the risk factors, management, and end results of suprachoroidal hemorrhage that occur during or after vitrectomy. Materials and methods: This retrospective study involves patients suffering from this complication either during the curing process or immediately after vitrectomy. Preoperative risk factors, operative management, postoperative picture, and end results are reported. Results: During the study period, surgery was performed on 3342 patients with primary vitrectomy. Complications were experienced by 4 patients: 2 occurred near the end of vitrectomy, and 2 in the first postoperative day. The 4 patients were myopic more than 7 diopters, 2 were pseudophakic and 2 were aphakic. Cryopexy was used to treat the retinal breaks in the operative cases. Perfluorophenanthrene was used as a postoperative tamponade in one operative case. Reoperation was done in the 4 patients 3 to 8 weeks after surgery. Silicone oil was used as a prolonged tamponade in all cases. Hypotony persisted in operative cases. All the patients had vision more than 20/400 at the end of follow-up (6 to 24 months). Conclusion: The risk factors for suprachoroidal hemorrhage were old age, high myopia, aphakia or pseudophakia, retinal detachment, and scleral buckle. Postoperative suprachoroidal hemorrhage has a better prognosis than the operative type. Perfluorophenanthrene "vitreon" as an operative and postoperative tamponade has a beneficial effect in keeping the retina attached and in preventing pooling of blood under the macula.

Abstract:Background: To report a case of Suprachoroidal Hemorrhage followed by Swept-Source Optical Coherence Tomography. Case presentation: A 66-year-old woman with a rhegmatogenous retinal detachment in her left eye underwent pars plana vitrectomy. During the intraocular photocoagulation for a retinal tear after fluid-air exchange, a vitreous hemorrhage and suprachoroidal hemorrhage (SCH) developed. The surgical incisions were closed after filling the vitreous cavity with silicone oil. Two weeks later, the hemolyzed hemorrhage was removed, and new silicone oil was injected. After the surgery, a low reflective region was detected near the macula in the swept-source optical coherence tomographic (SS-OCT) images. The low reflective region was caused by the residual hemorrhage. The size of the reflective region gradually decreased and was not present at 3 months. We conclude that SS-OCT can be used to follow the resolution of a suprachoroidal hemorrhage. Conclusion: SS-OCT can be used to detect and follow the natural course of a suprachoroidal hemorrhage including the absorptive processes.
Surgical Management of Silicone Oil Migrated into Suprachoroidal Space after Vitrectomy

Abstract:AIM: To report a successful surgical management of silicone oil migrated into suprachoroidal space after the repair of the retinal detachment with hemorrhagic choroidal detachment. METHODS: Retrospective observational case report. A 30-year-old man with retinal detachment and hemorrhagic choroidal detachment due to severe corneal penetrating injury, underwent a pars plana lensectomy and vitrectomy, endolaser, and silicone oil tamponade followed by transscleral suprachoroidal hemorrhage drainage in the right eye. One week later, a localised temporal choroid elevation was noted. This persistent elevation was confirmed by operation research to be silicone oil migration into suprachoroidal space. RESULTS: The migrated silicone oil was drained via trans-scleral cut down, and the intravitreal silicone oil was removed and replaced by 16% C2F6. Over the next 2 weeks, the elevation vanished and the choroid became completely flat. CONCLUSION: The migration of silicone oil into suprachoroidal space is a rare complication of vitrectomy. The pathway of the migration is most likely through internal orifice of sclerotomy sites. Trans-scleral drainage surgery is an effective method to remove the migrated silicone oil from suprachoroidal space.
Treatment for Suprachoroidal Silicone Oil Migration Following Surgeries for Open Globe Injuries

Abstract:Objective: To evaluate the effect of scleral buckling surgery on the treatment of hypotony caused by choroidal holes and suprachoroidal silicone oil (SO) migration following surgical procedures for open globe injuries.Design: Retrospective, consecutive, interventional case series.Participants: Ten eyes of 10 patients with hypotony caused by choroidal holes with suprachoroidal SO migration and choroidal detachment after vitrectomy for open globe injuries between October 2009 and December 2010.Methods: All cases clinically diagnosed as hypotony caused by choroidal holes with suprachoroidal SO migration and choroidal detachment were identified. Those eyes with retinal detachment, ciliary body damage, ciliary body fibrosis, or cyclodialysis cleft were excluded. Scleral buckling with or without suprachoroidal SO drainage was performed.Results: The mean preoperative intraocular pressure (IOP) was 6.7 +/- 1.4 mm Hg (5.3-9.0 mm Hg). The mean final follow-up IOP was 12.2 +/- 4.7 mm Hg (7.0-21.0 mm Hg; p = 0.005). In 7 eyes, the IOP increased to >= 10 mm Hg, whereas 3 eyes showed no significant IOP elevations. The choroidal hole was closed, and the range of choroidal detachment was significantly reduced in those 7 eyes. Although the choroidal hole was not fully closed in 3 eyes, the choroidal detachment area was less extensive, and the IOP was stable at approximately 7 mm Hg.Conclusion: Scleral buckling surgery combined with suprachoroidal SO drainage is an effective way to manage hypotony caused by choroidal holes and suprachoroidal SO migration in a SO-filled eye after vitrectomy for open globe injuries.
Suprachoroidal Hemorrhage after XEN Gel Implant Requiring Surgical Drainage

Abstract:Aim: To describe a case of a patient on anticoagulation who underwent XEN45 (XEN) gel stent placement and subsequently developed kissing suprachoroidal hemorrhages (SCHs) requiring surgical drainage. Background: Minimally invasive glaucoma surgery (MIGS) aims to achieve effective control of intraocular pressure (IOP) while minimizing the complications associated with traditional glaucoma surgeries. Rarely, a severe complication of intraocular surgery is SCH, which can result from early postoperative hypotony. The XEN gel stent is a MIGS device that theoretically avoids the risk of hypotony due to its outflow resistance properties. However, cases of SCH associated with XEN gel stents have been reported. Case description: A monocular 86-year-old Caucasian male with glaucoma and atrial fibrillation on rivaroxaban underwent routine XEN gel stent placement with mitomycin C in his only seeing eye. On postoperative day 3, he presented with severe eye pain and worsening vision. He was found to have a SCH that gradually progressed to kissing suprachoroidals that required surgical drainage. After extensive discussion, the decision was made to hold the patient's rivaroxaban given his monocular status. The patient subsequently had a cerebral vascular accident (CVA) 1 week after drainage, but his symptoms gradually resolved after restarting anticoagulation. Two months after drainage, the patient's SCH had completely resolved, and he had recovered baseline visual acuity with excellent IOP control-off medications. Conclusion: This case highlights the importance of quick and appropriate management of complications following glaucoma surgery, as well as discussion with patients regarding risks of treatments and return precautions. It also is a reminder that although many new surgical interventions are "minimally invasive," those like the XEN that are more effective at IOP control may have a similar risk profile to more traditional surgeries like trabeculectomy. As such, risk factors like anticoagulation use and older age should be considered in anticipation of surgical intervention. Clinical significance: The XEN gel stent is a new surgical option for glaucoma patients that asserts a better safety profile than traditional surgeries like trabeculectomy, but our case of kissing SCHs requiring surgical intervention following XEN placement reminds us that even minimally invasive surgeries can have devastating complications. Effect of minimally invasive surgery for intracerebral hemorrhage: a single center retrospective analysis

Abstract:Objective:To evaluated the safety, efficacy and longterm prognosis of different minimally invasive surgery treatments in patient with intracerebral hemorrhage (ICH).Methods:Patients with basal ganglia hemorrhage, admitted to Neurosurgery Department of Second Affiliated Hospital, School of Medicine, Zhejiang University from January 2019 to December 2020, were divided into three groups: small bone window craniotomy (SBWC) group, endoscopic surgery (ES) group and stereotactic hematoma aspiration followed by urokinase (SHAU) group. The baseline characteristics, perioperative information and lone-term outcome data of the three groups were analyzed to evaluate the safety and effectiveness of different minimally invasive treatments in the treatment of ICH.Results:There were 356 patients in this group, including 116 cases in ES group, 135 cases in SHAU group and 105 cases in SBWC group. No significant differences were observed among all three groups the baseline characteristics, including age, sex, hematoma location, GCS score, NIHSS score and hematoma volume (P>0.05). There were significant differences among the three groups with respect to the operative blood loss and operative time (P<0.05). The rebleeding rate in the SHAU groups was significantly higher than in the other two groups (P<0.05). The evacuation rate 24 h after operation in AMWC and ES groups was significantly higher than SHAU group (P<0.05). The evacuation rate 3 d after operation in ES group was better than that in SBWC group and SHAU group (P<0.05), but there was no significant difference between SBWC group and SHAU group (P>0.05). On 7 d after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05). Six months after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05).Conclusion:Minimally invasive surgery has a definite effect on the treatment of ICH and can significantly improve the long-term outcome. Among them, endoscopic surgery has high hematoma removal efficiency and fewer complications.
Spontaneous suprachoroidal hemorrhage treated by gas tamponade in a refractory glaucoma patient with rhegmatogenous retinal detachment: a case report

Abstract:We are reporting the successful treatment of a rare case of spontaneous suprachoroidal hemorrhage (SSCH) in a young refractory glaucoma patient with rhegmatogenous retinal detachment (RRD). The 34-year-old male was admitted to our hospital having experienced pain in his right eye for two days. The patient had a history of glaucoma and long axial length in the right eye with chronic poorly controlled intraocular pressure (IOP) and acute hypotony. Following clinical examinations, the patient was diagnosed with SSCH with RRD in the right eye. The patient underwent drainage of the suprachoroidal hemorrhage and vitrectomy combined with gas injection to address retinal and choroidal detachment. During the following two months, the patient's retina remained attached. Long axial length and persistent high IOP are two major risk factors of SSCH with RRD, while acute hypotony appeared to be a precipitating factor in this case. Surgeons can consider alternative tamponade options, such as gases, during vitrectomy to treat SSCH with RRD.
Spontaneous Suprachoroidal and Orbital Hemorrhage in an Older Woman Associated with Prophylactic Antiplatelet Therapy: A Case Report and Literature Review

Abstract:An 80-year-old woman presented to our Ophthalmology Clinic for sudden pain and loss of vision in her right eye for seven days. She had a medical history of atrial fibrillation and cardiac valvular disease and received prophylactic antiplatelet therapy for more than ten years. Spontaneous suprachoroidal and orbital hemorrhage and secondary angle-closure glaucoma was diagnosed according to clinical manifestation and confirmed with B-scan ultrasound and Magnetic Resonance Imaging. The patient was given transscleral cyclophotocoagulation (TSCPC) combined with medical therapy to lower intraocular pressure (IOP). At a follow-up visit of 4 weeks after treatment, the patient's IOP was normal and symptoms such as eye pain and headache disappeared completely. In this case, we found that TSCPC was beneficial for lowering IOP and relieving pain.
Factors associated with outcomes of suprachoroidal hemorrhage: an individual participant data systematic review

Abstract:Purpose: To determine factors associated with visual and anatomic outcomes of suprachoroidal hemorrhage (SCH) in studies published between 1990 and 2022. Methods: Individual participant data (IPD) systematic review. The protocol was prospectively registered on Open Science Framework (https://osf.io/69v3q/). PubMed, EMBASE, Web of Science, and Google Scholar were searched for peer-reviewed studies of SCH with ≥3 patients published between January 1, 1990, and September 1, 2022. The primary outcome was the change in logarithm of the minimum angle of resolution (logMAR) visual acuity (VA) from the time of SCH diagnosis to last follow-up. Results: 413 eyes from 49 studies were included, with mean (SD) age 60.8 (22.4) years and mean (SD) follow-up of 13.8 (12.6) months. Among 145 eyes with at least 6 months of follow-up, the mean (SD) gain in VA was -0.98 (0.89) logMAR. In multivariable regression, treatment with systemic steroids was associated with greater improvement in logMAR VA (adjusted mean (SE) -1.29 (0.09) versus -0.16 (0.30) for no systemic steroids; P < 0.001) and greater odds of achieving anatomic success (adjusted OR 10.59, 95% CI 2.59 to 43.3; P = 0.001). Conclusions: The use of systemic steroids was associated with better visual and anatomic outcomes for SCH.

Effects of the Implementation of an Emergency Surgical Pattern in Patients with Rhegmatogenous Retinal Detachment: A Retrospective Observational Study

Abstract:Background To analyze the effects of the implementation of emergency surgical patterns in patients with rhegmatogenous retinal detachment (RRD) and provide evidence for promoting emergency surgical patterns for RRD. Methods We reviewed the electronic medical records of 346 patients (348 eyes) who underwent surgical repair of RRD at the Zhongshan Ophthalmic Center in Southern China. A total of 140 patients (140 eyes) in the routine inpatient surgery group were collected at the fundus disease department between January 2019 and December 2019, and 206 patients (208 eyes) in the emergency surgery group were collected at the ophthalmic emergency department between January 2021 and December 2021. Demographics, best-corrected visual acuity (BCVA) expressed as the logarithm of the minimum angle of resolution (logMAR), the status of the macula before surgery, time to presentation, treatment interval, and postoperative BCVA measured at least three months follow-up were compared. Results The preoperative BCVA (logMAR) of the emergency surgery group and the inpatient surgery group were 1.0 (0.4–1.7) and 1.4 (0.7–1.7), respectively, with significant differences between groups (P < 0.001). However, patients had a shorter time to presentation (7 days vs. 21 days, P < 0.001), shorter treatment interval (2 days vs. 12 days, P < 0.01), and significantly better postoperative BCVA (logMAR 0.5 vs. logMAR 1.0, P < 0.001) in the emergency surgery group than in the inpatient surgery group. There was no significant difference in primary anatomical success between the two groups (P=0.802). The median follow-up for the emergency surgery group and the inpatient surgery group were 6.08 months and 6.2 months, respectively, with no significant differences (P > 0.05). Conclusions Patients who underwent emergency surgical patterns of RRD had better visual outcomes after surgery than patients with routine inpatient surgery, which might be attributed to a shorter duration, shorter treatment interval, and the preoperative status of the macula in the emergency surgery pattern. Emergency surgical patterns for RRD should be considered to achieve better surgical outcomes in suitable patients.
The Effect of Trabeculectomy Combined with Prophylactic Sclerotomy As a Treatment of Late Stage Juvenile Open Angle Glaucoma and Primary Congenital Glaucoma Patients: a Primary Observational Study

Abstract:Objective: To evaluate the outcomes of trabeculectomy combined with a new prophylactic sclerotomy in late stage juvenile open angle glaucoma (JOAG) and primary congenital glaucoma (PCG) patients at high risks of intraoperative and postoperative suprachoroidal hemorrhage (SCH). Method: A retrospective case series study. Thirty-three eyes of 28 JOAG patients and 15 eyes of 12 PCG patients with high risk factors for severe choroidal effusion or SCH were enrolled from November 2006 to April 2012 at Eye and ENT Hospital of Fudan University. The standard trabeculectomy accompanied by prophylactic sclerotomy was performed (video attached). Ophthalmic examinations were performed before and after the surgery. Surgical outcome was assessed in terms of intraocular pressure (IOP), best-corrected visual acuity, optic nerve head cup disc ratio, visual field, and complications. All patients were followed up for at least 1 year. Complete success was defined as an IOP ≥6 mmHg (1 mmHg=0.133 kPa) but ≤18 mmHg without medication. Qualified success was defined as an IOP ≥6 mmHg but ≤18 mmHg with local application of medications. Kaplan-Meier survival curves were drawn for the success rate calculation. Results: The mean age of all 40 patients was (13.2±9.4) years old. And among them, 22 patients were male and 18 patients were female. The mean follow-up time was (31.8±15.9) months. At the postoperative 3(rd) year, the complete success rate was 75.8%, and the qualified success rate was 90.0%. In our case series, one eye had expulsive SCH intraoperatively and two eyes had delayed SCH postoperatively. All of them were successfully saved without further surgery and with no significant damage to visual function. Conclusion: Trabeculectomy combined with prophylactic sclerotomy is a promising technique developed to control IOP in late stage JOAG and PCG patients, which provides a potential intervention to reduce severe consequences of massive choroidal effusion or SCH.
