Abstract:Objective To study the imaging manifestation of external and middle ear malformations in children.Methods CT scans and multiplanar reformation(MPR) of the temporal bone in seventy children referred to ENT department of Xinhua hospital and Children Medical Center between November 2007 and December 2009 for hearing loss(n=15) and other external ear malformations(n=55) were retrospectively studied.Results External ear malformations were found in 64 patients(73 ears).Of the 51 cases(57 ears) with middle and external ear malformations,7(10 ears) were found to have middle,external and inner ear malformations,6(6 ears) to have only external ear malformations,4(6 ears) to have middle and inner ear malformations,and 2(3 ears) to have only middle ear malformations.External ear meatus malformations found included stenosis,membranous atresia and bony atresia.Middle ear malformations included problems with mastoid pneumatization,tympanic cavity,ossicle joints and windows.Others included facial nerve malformation,cerebrospinal otorrhea and congenital encephalomeningocele.The results showed that patients with membranous atresia of external ear meatus were more likely to have middle and inner ear malformations than patients with meatal stenosis or bony atresia.Conclusion CT examinations and MPR are of great importance for the diagnosis and treatment of children with external and middle ear malformations,especially in deciding on intervention strategies and choice of therapeutic management.

Abstract:To determine the relationship between external ear malformation and middle ear abnormality, and also the results of surgical treatment.Fifty-two cases with external ear malformation (62 ears) was dividied into two groups:the minor group and severe group. All the patients received surgical treatment, 48 ears by transmastoid approach and 14 ears by "direct entrance" approach.The middle ear abnormality was minor in minor group,and severe in the patients with severe external ear malformation. In 93.5% cases the hearing threshold improved more than 15 dB in 3 months after operation, and reduce to 85.5% in 3 years follow up.The middle ear malformation is corresponding to external ear malformation. The effect of surgical treatment is different according to the degree of malformation and surgical complication.
[Analyses of the Clinical Characteristics of Unilateral Conductive Hearing Loss with Intact Tympanic Membrane].

Abstract:OBJECTIVE:To analyze the clinical characteristics of unilateral conductive hearing loss with intact tympanic membrane, and summarize the key diagnostic points, differential diagnosis and observe the effects of surgical treatment.METHODS:We reviewed data from 82 patients with unilateral conductive hearing loss with intact tympanic membranes who accepted the exploratory tympanotomy from April 2011 to September 2013. There were 41 males and 41 females, aged from 7 to 66( averaged 26.5±13.7)years, with a history of one month to 50 years. The history, clinical symptoms, audiological evaluation, high resolution temporal bone CT, the results of surgical exploration and hearing reconstruction were analyzed.RESULTS:The exploratory tympanotomy revealed 43 cases of congenital middle ear malformations (52.4%), 22 cases of otosclerosis (26.8%), eight cases of congenital cholesteatoma (9.8%), six cases of trauma induced conductive hearing loss (7.3%), three cases of congenital ossicular malformations with congenital cholesteatoma (3.7%). Progressive hearing loss was common in patients with otosclerosis and congenital cholesteatoma, and patients with congenital middle ear malformations described their hearing loss since childhood. High resolution temporal bone CT of congenital middle ear malformation, trauma induced conductive hearing loss, congenital cholesteatoma diagnosis rate was 40.0%, 50.0%, and 83.3% respectively. The preoperative air-conductive threshold of patients with absence of the oval window were increased to (66.9±1.1)dBHL, the preoperative bone-conductive threshold achieved (28.3±10.4)dBHL at 2 000 Hz. While patients with stapes fixation and that with ossicular chain discontinuity were (27.2±9.7)dBHL and (17.8±8.8)dBHL(P=0.000)respectively. Through the tympanic exploration with endaural incision under the microscope, different hearing reconstruction were applied according to different lesions. After the operation, the hearing level of 52 patients with return visit were improved, the mean air-conductive threshold were decreased from (60.0±11.4)dBHL to (32.2±12.1)dBHL(P=0.000); and the mean ABG were decreased from (43.2±12.0)dB to (16.3±9.4)dB(P=0.000).CONCLUSIONS:Congenital middle ear malformations, otosclerosis, congenital cholesteatoma are the most common causes in unilateral conductive hearing loss with an intact tympanic membrane. The diagnosis rate can be improved by analyzing the clinical features. Through exploratory tympanotomy and hearing reconstruction, we can clarify the diagnosis and achieve a satisfying hearing recover.
[Congenital Atresia of the External Auditory Canal].

Abstract:OBJECTIVE:To investigate the clinical characters and treatments of congenital aural atresia.METHODS:From 1986 to 1996, 44 patients (50 ears) with congenital aural atresias were operated in the ENT Department of the PLA General Hospital. Postoperative audiological follow-up ranged from 6 months to 9 years.RESULTS:Ossicular chain anomalies were presented in all cases. This consisted most frequently of fusion (54%) or deficiency (36%) of the malleus and the incus with or without fusion to the tympanic wall. Facial nerve abnormalities were seen in 16 ears (32%). After the operation, hearing improvement was found in 33 ears (66%), in which 13 ears (26%) had hearing improvement (pure tone threshold) up to 40 dB. The postoperative complications were external auditory canal stenosis, lateralization of the tympanic membrane and recurrent infections of the cavity and canal skin, with incidences of 34%, 18% and 14%, respectively.CONCLUSION:Maintenance of width of the aural canal and prevention of lateral healing of the transplantated tympanic membrane were crucial in the treatment of congenital aural atresia.
Middle Ear Foreign Body Causing Cholesteatoma and External Auditory Canal Granuloma: a Case Report

Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Abstract:Congenital microtia is one of the highest incidence of head and facial deformities. It often means the deformity of both auricle and the middle ear cavity. Inner ear dysplasia is relatively rare. The deformity affects not only the appearance of the patient, the more important is the obstacle to cause hearing function. Furthermore, it can seriously affect the patient's day-to-day work of learning and life. Some patients also accompanied by more or less psychological impact, which is more common in bilateral patients. Surgery is the main treatment method. Patients with congenital microtia need to rebuild a normal appearance auricle and get normal or near-normal hearing. Outer ear reconstruction and hearing reconstruction surgery can significantly improve the appearance of the ear and the hearing. This article briefly reviews the etiology and epidemiology of microtia, the classification of microtia, the timing of surgery and the choice of surgical approach, the hearing reconstruction techniques, the ear reconstruction surgery and tissue engineering used in ear reconstruction. It highlights the material of ear reconstruction and the precautions of the surgery.

Knowledge about embryologic developments and established classification systems is an essential requirement when dealing with affected patients. Early detection of deficiencies and introduction of appropriate diagnostic measures is vital to initiate adequate therapies and prevent long-term disabilities. Treatment for malformations of the pinna-if requested-is mostly surgical, infrequently an epithesis is applied. As in other surgical fields, tissue engineering will likely play a crucial role in the future. Treatment of aural stenosis and atresia aims at improvement of hearing levels and prevention of secondary complications like cholesteatoma and chronic otorrhea. Auditory rehabilitation comprises a spectrum from conventional hearing aids to invasive hearing implants, the latter being favored in recent years.

Abstract:Twenty-nine cases of congential atresia of external acoustic meatus underwent the combined treatment of external acoustic meatus and tympanic cavity plastics. The abnormality degrees of their external and middle ear were observed, the relationship between the auricle growing level, the bony fossa of external acoustic meatus, the hearing degree before operation and the abnormalities of middle ear were reviewed respectively. CT scan findings before operation were compared with the observations during operation. The results showed that the degrees of abnormality of middle ear could be estimated before operation based upon the above four factors, and the suggestions were proposed for understanding operative difficulties and selecting operative approach and methods.

Abstract:OBJECTIVE:To analyze the constitute, imaging and auditory characteristics of pediatric patients with congenital malformations of inner ear in sensorineural hearing loss(SNHL).METHOD:Imaging and auditory data of 125 cases (225 ears) in 860 pediatric patients with congenital SNHL who referred to ENT department of Shanghai Children Medical Center from February 2005 to January 2010 were retrospectively studied.RESULT:Congenital malformations of inner ear accounted for 14.5% in 860 pediatric patients with congenital SNHL. Bilateral ear was involved in 98 cases (78.4%), unilateral in 27 cases (21.6%). One hundred and sixty-seven ears (74.2%) were identified as profound deafness, 36 ears (16%) severe deafness and 22 ears (9.8%) moderate deafness in 225 ears, respectively. In present group, large vestibular aqueduct (75.6%) was the most common and next was vestibular deformity (32%), then cochleovestibular deformity (23.1%). Mondini deformity (55.8%) was the most common and common cavity deformity (28.9%) next in cochleovestibular deformity. Profound deafness in cochlea involved congenital malformations of inner ear was significantly more than those in no cochlea involved malformations.CONCLUSION:The results are of importance for understanding the constitute of congenital malformations of inner ear in pediatric patients with congenital SNHL in China, for etiological diagnosis of congenital SNHL, for intervention including hearing aids or cochlear implant and prognosis.

Abstract:Congenital anomalies of the external auditory canal (EAC) are classically divided into congenital aural atresia (CAA) and congenital aural stenosis (CAS). CAA can present as an isolated anomaly, unilateral or bilateral, or in the setting of a craniofacial syndrome. Hearing testing (ABR with air and bone conduction thresholds for both ears) early in the perinatal period is important to document hearing thresholds. Hearing status thus informs parent counseling on options for hearing habilitation: Bone conducting technology is a must for children with bilateral CAA to support normal speech and language development. Bone conducting technology should be considered for children with unilateral CAA; benefits are unclear. In select candidates, atresia repair can provide improved hearing with a clean, dry, epithelialized ear canal. First branchial cleft cyst or sinus is rare; high index of suspicion is needed to diagnose along with high-resolution CT. Congenital aural stenosis (CAS) is a rare condition, and hearing testing should be similar to that in children with CAA. Early (age 4-5) CT imaging is recommended in the setting of a canal <2 mm or pinpoint canal to evaluate for trapped skin/ear canal cholesteatoma.

Abstract:Objective:To study the clinical features of inner ear malformations with sensorineural hearing loss(SNHL)in children.Methods: Retrospective chart review,860 patients with SNHL,of which 125 children(225 ears) with inner ear malformations diagnosed with temporal bone computed tomography(CT) scans who referred to the Ear,Nose and Throat(ENT) Department of Children Medical Center from February 2005 to January 2010,109 cases due to hearing loss and other 3 cases due to external ear malformations,13 cases due to face abnormality,were retrospectively studied.Results: A total of 109(87.2%) cases were found hearing loss,52 cases(41.6%)mothers were noted to have caught a cold in first trimester of pregnancy.Hearing loss was typically bilateral in 100 ears(80%),and profound sensorineural hearing loss in 166 ears(73.8%).Of the total,3 ears were accompanied by outer and middle ear malformations,13 ears accompanied by middle ear malformations.Among the 124 patients,2(4 ears)were found to be Michel deformity,1(1 ears) was found to be Cochlear aplasia,10(14 ears)were found to be common cavity deformity,2(2 ears)incomplete partition,2(2 ear) cochleovestibular hypoplaisa,19(29 ears)incomplete partition II(Mondini deformity),91(170 ears) accompanied by large vestibular aqueduct,30(46 ears)semicircular canal deformity,internal auditory canal deformity 35(49ears).Conclusions: The patients with large vestibular aqueduct are more in patients with inner ear malformations than in patients with other inner ear malformations.Hearing loss is typically bilateralor total deafness.CT examinations are of great importance for the diagnosis and treatment of children with inner ear malformations.

Abstract:Congenital microtia/atresia is one of the common deformities in the head and face.The pathogenesis and etiology of the disease are unclear.The main treatments include total auricular reconstruction and hearing restoration surgery.Auricle reconstruction technology has developed from "shaped ear" and "contours ear" to "natural ear".The hearing restoration surgery was used in external auditory canal stenosis and microtia.The outcomes for patients with three stage combined reconstruction surgery were better.The effect of implantable vibration hearing aid was stable in the patients difficult to reconstruct the hearing.The operation procedures and the indications of "aesthetic and functional" ear reconstruction have been gradually definitely decided.However,every surgical technique has limitations,and the functional reconstruction for congenital microtia/atresia is still one of the greatest challenges in reconstructive surgery.This article will prospect the direction and development of aesthetic and functional ear reconstruction from the cooperation of the otologists and plastic surgeons.

[Congenital auricular anomalies: Early non-surgical correction by splinting]

Abstract:Objectives: Auricular deformities therapy by external molding is possible. It must be undertaken early, ideally before the sixth week of life. Persistency of maternal hormones impregnation during the first six weeks of life permits to maintain the malleable cartilages. They can thus be molded into a new position. After the end of this hormonal therapy, the cartilages gradually become rigid in a specific position. Material and methods: All infants under 6 weeks of age with isolated auricular deformities between July 2015 and July 2016 received a neonatal treatment with an external molding device called EarWell© or a custom-made one with silicone. The device was removed when the infant was 6 weeks old and had a treatment duration of at least 4 weeks. A 6 months minimum follow-up was carried out to evaluate the result. Results: From July 2015 to July 2016, 42 deformed ears were treated in 38 newborns. The average age to apply the device was 2 weeks of life with a treatment duration of 5 weeks. The immediate outcome was evaluated as excellent by the parents in 81% of cases, with an ear judged as "normal" and as excellent by the surgeon in 75% of cases. There were no major complications. Conclusions: External molding therapy for infants with ear deformities has proved to be effective in 75 to 81% of cases. It is a non-invasive treatment that avoids subsequent surgical treatment for the majority of patients.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Purpose: Congenital malformation of the ossicular chain results in challenges with hearing and language development in children. We aimed to analyze the clinical characteristics, prognosis, and surgical treatments of different types of congenital ossicular chain malformations in children. Methods: Eight cases (10 ears) treated between October 2019 and February 2022 were analyzed retrospectively. Patients were divided according to the location of the ossicular chain malformation and whether it was complicated by external ear malformation. Imaging, audiological examination, intraoperative exploration of the middle ear, and postoperative outcomes were recorded. Results: Group 1 incudostapedial joint deformity): 6 ears/60%; Group 2 (simple incus deformity): 2 ears/20%; Group 3 (simple malleus deformity): 2 ears/20%; Group A (with external ear malformations): 4 ears/40%; Group B (without external ear malformations): 6 ears/60%. The average hearing threshold before and after the operation was 51.25 ± 12.88 and 31.94 ± 12.96 dB, respectively. There were differences in the intervention effects of different malformed sites (Group 1: t = 5.139, P = .004; Group 2: t = 13.500, P = .047; Group 3: t = 15.000, P = .042). The effect of the intervention in cases of malformation with mobile stapes footplates was better than that with immobile stapes footplates (t = 4.082, P = .027). The effect of the intervention without external ear malformations was better than that with external ear malformations (t = 7.706, P = .001). Conclusions: The intervention yielded superior results in cases of malformation with mobile stapes footplates compared to those with immobile stapes footplates. Different intervention strategies should be determined through precise deformity assessments, with the degree of stapes mobility serving as a crucial factor in improving the prognosis.

[Congenital middle ear malformation: clinical analysis and discussion of classification]

Abstract:Objective: To analyze the clinical characteristics and appropriate surgical procedures, and discuss the classification of congenital middle ear malformation. Methods: All cases were from the Center of Otorhinolaryngology, the Sixth Medical Center of Department of PLA General Hospital. All of these cases, including 26 male patients (ears) , 10 female patients (11 ears) , aged from 7 to 57 years old, had normal external auditory canal, tympanic membrane, conductive hearing loss, type A tympanogram and negative Gelle's test. Tympanoplasty was performed in all cases. The deformity was classified to three types,i.e., Type I (stapes foot plate mobility): Ⅰa, ossicular chain deformity with normal stapes suprastructure; Ⅰb, ossicular chain deformity with abnormal stapes suprastructure; Type Ⅱ (stapes foot plate fixation): Ⅱ a,normal ossicular chain, Ⅱ b, ossicular chain malformation; and Type Ⅲ: vestibular window osseous atresia or undeveloped, or with round window atresia. The malformation of type Ⅱ and Ⅲ may be accompanied with abnormal facial nerve. In addition, the papers on middle ear malformation published from 1982 to 2017 were analyzed retrospectively. The clinical data of 451 ears malformation were summarized. Results: According to the revisional classification criteria in 37 ear samples from our hospital, 20 ears were type I. 6 type Ⅰa cases were used PORP (partial ossicular replacement prosthesis) to reconstruct the ossicular chain; 14 type Ⅰb cases were used TORP (total ossicular replacement prosthesis) to reconstruct the ossicular chain. For the 5 ears of type Ⅱ, 2 of which were type Ⅱ a and 3 were type Ⅱ b. 4 ear samples of type Ⅱ were implanted with Piston ossicular prosthesis, 1 was implanted with TORP in which the ossificated foot plate was removed with periosteum preserved. 12 ear samples were type Ⅲ, with vestibular window osseous atresia, facial nerve malformation, and stapes suprastructure malformation. The pistons ossicular prosthesis were implanted in vestibular window in 3 ears with facial nerve covering vestibular window partially. The surgery had to be given up in 5 ears, and TORP was implanted in 4 ears at the opening with preserved periosteum at the beginning of the tympanic scala because of facial nerve covering vestibular window totally. 30 ears with complete follow-up data had no sensorineural hearing loss and the average air-bone conduction decreased 23.3±10.7 dB (P<0.05).There were 234 ears of type Ⅰ in 451 ears of congenital middle ear malformation reported in the literature. 113 of which were type Ⅰa, the basic surgery was ossicular chain shaking and artificial or autogenous PORP implantation. Type Ⅰb was 121 ears, with autogenous or artificial TORP and PORP. Type Ⅱ was125 ears, including type Ⅱa 22 ears, Ⅱb 60 ears, and no subclassification for 43 ears. The surgery of type Ⅱ was the same as otosclerosis. The vestibular window atresia of type Ⅲ was 92 ears, the surgery of 17 ears had to be abandoned, the other ears underwent vestibular window, promontory or semicircular canal opening to reconstruct hearing with Piston, autogenous or artificial TORP. Conclusion: Referring to the classification of congenital middle ear malformation combining with appropriate surgical materials and methods, otologists can better understand and choose appropriate surgical method to the middle ear malformation.

Abstract:A variety of congenital and acquired disorders result in pediatric conductive hearing loss. Malformations of the external auditory canal are invariably associated with malformations of the middle ear space and ossicles. Isolated ossicular malformations are uncommon. Syndromes associated with external and middle ear malformations are frequently associated with abnormal development of first and second pharyngeal arch derivatives. Chronic inflammatory disorders include cholesteatoma, cholesterol granuloma, and tympanosclerosis.

Abstract:Cochlear implantation (CI) is the most effective approach to severe to profound sensorineural hearing loss so far. Since the performance of CI relies on the AN population that encodes and transmits acoustic signals to the higher au-ditory processing stations, strict and comprehensive patient selection, surgical skills, effective rehabilitation training and post-implantation psychotherapy are of great importance for the best effects of CI. Based on the surgical approach, structur-al issues relevant to facial recess access and electrode insertion, such as abnormal mastoid pneumatization, malformation of the ossicular chain, vestibular window, round window or facial canal, anteriorly displaced sigmoid sinus and high jugular bulb, have been studied in order to provide a basis for selection of surgical approaches and evaluation of surgical risks. It is of great significance to reduce complications and improve surgery efficacy.
