Abstract:BACKGROUND:Trust is important for health at both the individual and societal level. Previous research using Western concepts of trust has shown that a high level of trust in society can positively affect individuals' health; however, it has been found that the concepts and culture of trust in China are different from those in Western countries and research on the relationship between trust and health in China is scarce.METHOD:The analyses use data from the national scale China General Social Survey (CGSS) on adults aged above 18 in 2005 and 2010. Two concepts of trust ("out-group" and "in-group" trust) are used to examine the relationship between trust and self-rated health in China. Multilevel logistical models are applied, examining the trust at the individual and societal level on individuals' self-rated health.RESULTS:In terms of interpersonal trust, both "out-group" and "in-group" trust are positively associated with good health in 2005 and 2010. At the societal level, the relationships between the two concepts of trust and health are different. In 2005, higher "out-group" social trust (derived from trust in strangers) is positively associated with better health; however, higher "in-group" social trust (derived from trust in most people) is negatively associated with good health in 2010. The cross-level interactions show that lower educated individuals (no education or only primary level), rural residents and those on lower incomes are the most affected groups in societies with higher "out-group" social trust; whereas people with lower levels of educational attainment, a lower income, and those who think that most people can be trusted are the most affected groups in societies with higher "in-group" social trust.CONCLUSION:High levels of interpersonal trust are of benefit to health. Higher "out-group" social trust is positively associated with better health; while higher "in-group" social trust is negatively associated with good health. Individuals with different levels of educational attainment are affected by trust differently.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.
Bridging the Gap: How Education Transforms Health Outcomes and Influences Health Inequality in Rural China

Abstract:This study focuses on the impact of education on health and health inequalities in rural areas of China. Education significantly enhances economic status and health, driving economic growth and improving public health standards. Integral to the “Healthy China Strategy,” it provides essential guidance for public policy and underscores the need for strategic human capital investments to achieve these goals. The study utilizes data from the China Family Panel Studies (CFPS) spanning 2010–2020 and employs the average educational level within counties as an instrumental variable. The causal impact of education on health and health inequalities is estimated using the two-stage least squares (2SLS) method. The findings reveal a significant positive correlation between enhancing education levels and health improvements. Specifically, after controlling for endogeneity, the duration of individual education significantly improves both subjective and objective health outcomes. It reduces health inequalities, with these effects being more pronounced among women and low-income groups. Mechanistically, education positively impacts health primarily by altering health behaviors and social network levels and reducing health inequalities through socio-economic factors. This paper provides important implications for public policy, suggesting that enhancing educational investments can drive economic development and improve population health standards.

Abstract:Has China's remarkable reduction in income poverty since 1980 been accompanied by comparable progress in health? The author's findings are fourfold. First, province-level rates of improvement in life expectancy (LE) were higher in the 1990s than in the 1970s and 1980s, and were lowest in the 1980s. Second, even in the 1990s, when the province-level rates of improvement in LE were highest, they were lower than for many countries with similar initial LE level (although higher than the average for all such countries). Third, China's LE improvement between 1980 and 2000 was achieved much more quickly by almost all other countries considered, and in particular by most lower-middle-income countries that had similar LE improvements; similar conclusions are drawn from an analysis of China's LE improvements relative to two other sets of comparator countries—selected presently rich countries and high-growth East Asian countries. Finally, even those Chinese provinces that performed best over the period experienced rates of improvement significantly lower than for comparator countries. China's experience of reducing health deprivations has been notably less impressive than its record of income poverty reduction. There is a need for China to invest in the redevelopment of its public health infrastructure, which has withered during the period of market-oriented reforms.

Abstract:Past research has examined the effects of import competition on workers' health outcomes, but few stud-ies have investigated the impact of export expansion. We take advantage of the exogenous changes in export tariffs on Chinese products after the Reform and Opening-Up policies in China to estimate the effect of reductions in export tariffs on the health of working-age adults. Using China Health and Nutrition Survey data from 1993 to 2009, we create individual-level panels and find that reductions in export tariffs improved the health of working-age adults. Analysis of possible mechanisms shows that reduced export tariffs led to increased earnings and hours worked as well as decreased malnutrition, sug-gesting an income effect driven by export-led growth. This conclusion is further supported by the observation of greater health improvements among those without urban residency and those with low educational attainment. The findings contribute to the limited evidence on this topic among developing economies as well as the ongoing discussion of the relationship between income and health. (c) 2020 Elsevier Ltd. Abstract:This paper examines the changes in equality of health and health care in China during its transition from a command economy to market economy. Data from three national surveys in 1985, 1986, and 1993 are combined with complementary studies and analysis of major underlying economic and health care factors to compare changes in health status of urban and rural Chinese during the period of economic transition. Empirical evidence suggests a widening gap in health status between urban and rural residents in the transitional period, correlated with increasing gaps in income and health care utilization. These trends are associated with changes in health care financing and organization, including dramatic reduction of insurance cover for the rural population and relaxed public health. The Chinese experience demonstrates that health development does not automatically follow economic growth. China moves toward the 21st century with increasing inequality plaguing the health component of its social safety net system.

Abstract:Abstract The Basic Healthcare and Health Promotion Law 2019 became the new constitution of China’s health system in June 2020, giving legal effect to ambitious health reform programmes like Healthy China 2030. The concurrent outbreak of coronavirus disease 2019 must not distract us from appreciating the fact that this Law will comprehensively overhaul the health regulatory framework of the world’s most populous country during the coming decade, if not beyond. This article offers an original evaluation of the Law in its political context. The Law commendably promises to safeguard the right to health, assist citizens to live a ‘complete cycle of life’, and promote health using the resources of the public health system. However, it is also deeply politicised, guaranteeing extensive and penetrative political control in health campaigns, digitalised health data, the governance of health institutions, and the resolution of medical disputes. This can be explained by the consequential roles played by epidemics in China’s historical dynastic cycles, but even more so by powerful tendencies of centralisation on the part of the Leninist Party-state. The Law’s potential is thus subject to the overriding caveat that the Party-state’s existence and influence over law and public health must be secured.

Liberty in Health Care: A Comparative Study Between Hong Kong and Mainland China

Abstract:This essay contends that individual liberty, understood as the permissibility of making choices about one's own health care in support of one's own good and the good of one's family utilizing private resources, is central to the moral foundations of a health care system. Such individual freedoms are important not only because they often support more efficient and effective health care services, but because they permit individuals to fulfill important moral duties. A comparative study of the health care systems in Hong Kong and mainland China is utilized to illustrate the conceptual and moral concerns at stake. Both regions have implemented two-tier health care systems with a public tier of basic health care services together with a second tier of privately purchased health care. As we document, Hong Kong permits patients and doctors significantly greater opportunities to choose private health care of typically higher medical quality than their mainland counterparts. As a result, individuals are able to obtain higher quality health care while also fulfilling important moral duties for themselves and their families. In this sense, Hong Kong's health care system is morally superior to mainland China's. In each case, Confucianism's concerns regarding equality are partly satisfied through the provision of public health care services on the basic tier, while appropriate use of private resources in support of oneself and one's family is permissibly exercised on the private tier. Although it is true that inequalities in health care access and outcome are inevitable within a system that permits such individual freedoms, we argue that such inequalities are morally justifiable in terms of Confucian ethical thought.
Economic Growth, Income Inequality and Life Expectancy in China.

Abstract:China had made dramatic health gains before its economic reform that began in 1978 produced rapid economic growth in the ensuing years. Since the economic reform, China's income inequality has substantially increased, and health gains have stagnated. This article investigates the extent to which China's health stagnation may be attributable to the rise in income inequality in China. By simulating the improvement in life expectancy that could have resulted if, ceteris paribus, income inequality had stayed constant at the lowest level after the founding of the People's Republic of China in 1949, we find that the sharply increasing income inequality in China has contributed to life loss in China's population, about 0.6 years for men and 0.4 years for women. These findings suggest that redistribution of income from rich to poor may be one of the most important policy levers for improving population health in China.
Before the Emergence of Critical Citizens: Economic Development and Political Trust in China

Abstract:Political trust has been declining among the publics of almost all advanced industrial societies in recent years. This has been attributed to a Materialist-Postmaterialist value shift, which has given rise to a public that is less deferential to authority and increasingly ready to challenge government. This phenomenon has been interpreted as a 'crisis of democracy'. Although one might expect to find low level of political trust in repressive authoritarian societies, survey data indicates that political trust in China is actually very high. Does this simply mean that people are afraid to express any opinions that might be viewed as critical of authority? As this article will demonstrate, this does not seem to be the case. The Chinese public expresses fairly strong criticism of some aspects of Chinese society - but they express high levels of confidence in the national government. Although rich democracies provide both a higher standard of living and more personal freedom than is available to the average Chinese citizen, the Chinese public expresses higher levels of confidence in government than those found in most advanced industrial societies. We conclude that economic development has the immediate effect of enhancing public support for the government - but in the long run it also leads to value changes that promote critical citizens. At least for now, the regime-enhancing effect of economic development still dominates the regime-eroding effect. The effect of changing values on distrust of government is largely overwhelmed by the support for government brought by the increased level of affluence.
Occupational health in China: 'rising with force and spirit'

Abstract:Occupational health services in China occupy a much more central role in public life than they do in North America. The Communist ideology on which the People's Republic was built, and which it almost alone today defends, places an idealized concept of the worker at the centre of the economic, social and political system. The present system in China is a parallel, institutionally separate system of medical care and research on occupational disorders that is in some ways better provided for with resources than the general healthcare system. One of the issues facing China today is how to turn the priorities of this vast, elaborate and incompletely developed system of occupational health care away from the provision of medical care to workers made ill from workplace exposure and towards prevention of the exposures that made them ill in the first place. The prevalence of smoking and the intensity of passive smoke exposure in Chinese workplaces make this exposure one of the most deadly occupational hazards seen. This problem of health promotion can probably only be effectively approached in China by worksite programmes but these are not apparent.

Abstract:This paper provides an analysis of the changes that have occurred in the Chinese health care system in the reform era, post-1978. The economic reforms have brought significant benefits to many Chinese people, but the benefits have not been distributed evenly throughout Chinese society. The paper evaluates some of the evidence indicating deterioration in both the quality and quantity of health care services in the reform era. There is some alarming evidence that rural women, especially those living in the country's poorest regions, have been experiencing lower levels of accessibility to health care, and increasing levels of both morbidity and mortality. It is also evident that during the reform era the gap between service levels in rural areas and the cities has widened, and that chronic poverty still exists in large parts of the countryside. It is important to stress that these inequalities were not created by the economic reforms--in fact they were first observed during the supposedly egalitarian era associated with Mao Zedong. The economic reforms and the widespread implementation of market socialism in Chinese society, have, in general, been associated with improved health status and greater access to health care. It is also apparent, however, that pre-existing inequalities have not been eradicated by the reforms, and, particularly along the lines of gender and geography, the evidence points to a widening of inequality.
Does Trade Liberalization Improve Child Health? Evidence from China

Abstract:Positive child health is an important development indicator and is also a source of long-term economic growth. Whilst there has been previous research on the relationship between trade liberalization and child health, the conclusion is mixed and the underlying mechanisms remain limited. This paper estimates the effect of reductions in export tariffs on child health by taking advantage of exogenous changes in export tariffs on Chinese products since the reform and opening-up in 1978. We apply the Bartik approach and refer to China Health and Nutrition Survey data from 1993 to 2009, and find that export expansion causes a significant deterioration in child health, with the more obvious effects among relatively vulnerable families in rural areas, individuals with a low level of education, and girls. In addition, the mechanism analysis shows that export expansion increases the income and employment level of working-age parents. However, the income effect does not offset the negative impact of export expansion on parental care, the children being left behind, and child labor.
Happiness and Health in China: the Paradox of Progress

Abstract:Life satisfaction in China declined dramatically precisely at the time of its unprecedented economic growth and poverty reduction. We posit that a "progress paradox" is among the possible channels explaining these contrasting trends. Using data from the Chinese Livelihood Survey, we explore the role of reported physical and psychological health. The standard correlates of life satisfaction such as age, income, and health hold. In addition, we find that those with insufficient rest and leisure are significantly less satisfied. Urban, educated respondents are more likely to report depression, while rural and uneducated respondents are more satisfied with their lives and are less likely to report poor mental health. We also find that insufficient rest, stress, and low life satisfaction have a strong correlation with mental health problems. Given the gains in growth and poverty reduction in China, it is time to consider policies that focus on quality of life and mental illness. (C) 2017 Elsevier Ltd. All rights reserved.

Abstract:Objectives: Under the system of health decentralization, there are differences in the level of expenditure decentralization at different levels of government, and their impact on residents' health may also be different. This paper is one of the first to evaluate the effect of decentralization of health services at the municipal level from a multi-dimensional health perspective. Data and methods: This paper uses the data of expenditure decentralization of health services at the municipal level to match the panel data from the China Household Panel Survey (CFPS) from 2010 to 2018, and uses the logit model, ordered logit model and two-way fixed effects model to empirically analyze the impact of health decentralization at the municipal level on health outcomes. Results: Based on the perspective of multi-dimensional health, from the three aspects of physical health, depression status and cognitive ability, the six sub-indicators of self-rated health, BMI standards, depression scores (summation method), depression scores (factor method), phrase test scores and mathematics test scores are discussed separately. The results show that the decentralization of health services at the municipal level has a significant promotion effect on the multi-dimensional health of residents. Conclusion: The decentralization of health services at the municipal level has important theoretical significance for promoting the reasonable division of medical and health powers and expenditure responsibilities between provincial and municipal governments, improving the efficiency of health expenditure funds, and establishing a fiscal system that matches financial resources.

Bringing the Local State Back In: the Political Economy of Public Health in Rural China

Abstract:Contrary to what most alarmist reports would imply, China has not witnessed a measurable decline in the overall public health status. What explains the continuous improvement in some important domains of public health despite the pertinacious pricing, financing and institutional‐behavioral problems in China's health system change? This study provides a political economy analysis of the role the local Chinese state played in rural public health provision during the reform era. Through comparative case studies, statistical analysis, and formal modeling, the study shows that state capacity is a principal factor setting the parameters for rural public health in China. It also suggests that the post‐Mao reforms, while generating strong disincentives for the provision of public services, unleashed forces that lay down the institutional bases for sustained state engagement in the health sector.
The Effect of China's Compulsory Education Reforms on Physiological Health in Adulthood: a Natural Experiment.

Abstract:Lower education is related to higher biological risks for physiological health, but it remains unclear whether the risks can be reduced through policies aimed at increasing years of education. We utilized China's compulsory education reforms as a unique natural experiment, which stipulates that primary and lower secondary education is mandatory and free for all school-age children. Using a regression discontinuity design (RDD), we assessed the effect of the reform eligibility on biomarkers. The reforms resulted in an increase in years of education for those from communities with the middle 1/3 per capita income (PCI) (beta = 2.44, 95% CI = 0.23-4.64). Reform eligibility had no impact on allostatic risks for the total sample (beta = 0.065, 95% CI = -0.70 to 0.83) and for those from communities with the lowest (beta = 0.35, 95% CI = -0.77 to 1.47) or highest third of PCI (beta = 0.68, 95% CI = -0.64 to 2.00), while it reduced the metabolic risk (beta = -0.14, 95% CI = -0.26 to -0.015) and total allostatic load (beta = -1.58, 95% CI = -3.00 to -0.16) among those from communities with the middle third PCI. The results were confirmed by sensitivity analyses of different placebo cut-off points and bandwidths. The reforms led to better physiological health to some extent, but the effect only manifested in people from communities with a moderate community PCI, and had little impact on affluent or disadvantaged groups. Our findings stressed that the institutional context and respondents' socioeconomic environment must be taken into account when assessing the health impact of China's compulsory education reforms.
Does social capital enhance health and well-being? Evidence from rural China

Abstract:Despite increasing acknowledgement that social capital is an important determinant of health and overall well-being, empirical evidence regarding the direction and strength of these linkages in the developing world is limited and inconclusive. This paper empirically examines relationships between social capital and health and well-being-as well as the suitability of commonly used social capital measures-in rural China, where rapid economic growth coexists with gradual and fundamental social changes. To measure social capital, we adopt a structural/cognitive distinction, whereby structural social capital is measured by organizational membership and cognitive social capital is measured by a composite index of trust, reciprocity, and mutual help. Our outcome measures included self-reported general health, psychological health, and subjective well-being. We adopt multi-level estimation methods to account for our conceptualization of social capital as both an individual- and contextual-level resource. Results indicate that cognitive social capital (i.e., trust) is positively associated with all three outcome measures at the individual level and psychological health/subjective well-being at the village level as well. We further find that trust affects health and well-being through pathways of social network and support. In contrast, there is little statistical association or consistent pattern between structural social capital (organizational membership) and the outcome variables. Furthermore, although organizational membership is highly correlated with collective action, neither is associated with health or well-being. Our results suggest that policies aimed at producing an environment that enhances social networks and facilitates the exchange of social support hold promise for improving the health and well-being of the rural Chinese population. In addition, China may not have fully taken advantage of the potential contribution of structural social capital in advancing health and well-being. A redirection of collective action from economic to social activities may be worth considering.

Abstract:Human health is an important concern that gradually exists in sustainable development goals. The key aim of this study is to examine the impacts of the rule of law on happiness and health using a time series data of China over the data period 1998-2020. The empirical analysis utilizes the autoregressive distributed lag (ARDL) method to find out the short and long-run effects. Findings reveal that the rule of law stimulates happiness and human health in the long-run. More internet and GDP enhance happiness and human health in the long-run. The results also showed that health expenditure and education could not boost happiness and health in the long run, but unemployment's negative effect on health. Policymakers may use our empirical results to determine applicable policies to increase human health across China provinces.
