Abstract:OBJECTIVE:Inquire into factors influencing recurrence following resection of carcinoma of the head of pancreas to raise survival rate of pancreatic cancer.METHODS:We performed a retrospective review of 56 pancreatic resections with carcinoma of the head of pancreas operated from January 1995 to December 1998. 35 patients tolerated the Whipple operadure in comparison with 21 patients received the extended radical operation contemporaneously. All patients were followed up more than 3 years to find out factors influencing recurrence following resection of carcinoma of the head of pancreas.RESULTS:Deaths within 3-years after operation were due to recurrence, the cumulative rate of death from recurrence 51.4% in the Whipple procedure and 42.9% in the Extended radical operative procedure, significant difference in two groups.CONCLUSION:Tumour size exceed 4 cm with severe retropancreatic invasion, lymph node involvement surpass the second group are at high risk for recurrence.
Diagnosis and treatment of pancreatic islet cell tumor:a report of 35 cases

Abstract:Objective To discuss the diagnosis and treatment of pancreatic islet cell tumor and review its incidence in China based on the recent literatures.Methods Thirty-five patients with pancreatic islet cell tumor from 1953 to 2003 admitted in the 2nd Affilinted Hospital of Zhejiang University School of Medicine were analyzed retrospectively.The diagnosis and treatment of pancreatic islet cell tumor were also discussed based on the recent native literatures.Results Thirty-three patients with pancreatic islet cell tumor received operation,which were confirmed by pathology. Among 33 patients,there were 24 cases classified as insulinoma (72.7%,24/33),7 cases of non-functional pancreatic islet cell tumor (21.2%, 7/33),1 case of glucagonoma and 1 case of gastrinoma.There were 2 785 cases of pancreatic islet cell tumor reported in literature during same period.Conclusion Insulinoma and non-functional pancreatic islet cell tumor are the mostly dominant in pancreatic islet cell tumor.It should be performed simply tumor remove or partial pancreas resection for benign pancreatic islet cell tumor.Extended radical resection combined with other comprehensive treatment is benefited for malignant pancreatic islet cell tumor.It has a better prognosis than other abdominal organ tumors.
Analysis of 300 Consecutive Cases of Pancreatic Adenocarcinoma in a Single-Center in China

Abstract:BACKGROUND: Most of the reports on the prognostic indicators of patients with pancreatic adenocarcinoma are from developed countries. The present study focused on the prognostic indicators of Chinese patients with pancreatic adenocarcinoma. METHODS: A total of 300 patients with pancreatic adenocarcinoma who had undergone curative resection were included. The resection and R0/R1 resection rates for adenocarcinomas from different parts of the pancreas were calculated and clinical characteristics were analyzed.RESULTS: In 3427 patients diagnosed with pancreatic adenocarcinomas, only 300（8.8%） were eligible for radical resection. The total median survival of these patients was 19 months, and their 1-, 3-, and 5-year survival rates were 72.5%, 28.0% and 23.4%, respectively. The prognostic factors included socioeconomic status, smoking history, symptoms, high blood glucose, and various tumor characteristics, including perineural and vascular invasion, lymph node metastases, and CA19-9 levels before and after operation. Operation-associated prognostic indicators included operation time, blood loss and transfusions, pancreatic fistula, and complications. Independent predictors of mortality included poor socioeconomic status, smoking history, symptoms, CA19-9, perineural invasion and lymph node metastasis, grade of fistula and complications. Patient survival was not correlated with either resection margin or adjuvant chemotherapy in multivariate analysis.CONCLUSIONS: The survival rates of patients with curative resection for pancreatic adenocarcinoma in China are close to those in developed countries, but curative resection rate is far below. Socioeconomic status, symptoms, and CA19-9 are the three most prominent prognostic factors, which are helpful in patient selection and perioperative care.

Abstract:Abstract The incidence of pancreatic cancer has been rising worldwide, and its clinical diagnosis and treatment remain a great challenge. To present the update and improvements in the clinical diagnosis and treatment of pancreatic cancer in recent years, Chinese Pancreatic Association, the Chinese Society of Surgery, Chinese Medical Association revised the Guidelines for the Diagnosis and Treatment of Pancreatic Cancer in China (2014) after reviewing evidence-based and problem-oriented literature published during 2015–2021, mainly focusing on highlight issues regarding diagnosis and surgical treatment of pancreatic cancer, conversion strategies for locally advanced pancreatic cancer, treatment of pancreatic cancer with oligo metastasis, adjuvant and neoadjuvant therapy, standardized processing of surgical specimens and evaluation of surgical margin status, systemic treatment for unresectable pancreatic cancer, genetic testing, as well as postoperative follow up of patients with pancreatic cancer. Forty recommendation items were finally proposed based on the above issues, and the quality of evidence and strength of recommendations were graded using the Grades of Recommendation, Assessment, Development, and Evaluation system. This guideline aims to standardize the clinical diagnosis and therapy, especially surgical treatment of pancreatic cancer in China, and further improve the prognosis of patients with pancreatic cancer.
Clinical Analysis of Acinar Cell Carcinoma of the Pancreas: A Single-Center Experience of 45 Consecutive Cases.

Abstract:Backgrounds: Acinar cell carcinoma of the pancreas is a rare malignancy, and its features remain unclear. We aimed to analyze the clinical characteristics, treatment and prognosis of acinar cell carcinoma with our institutional case series. Methods: Patients diagnosed with acinar cell carcinoma in our hospital between 2005 and 2019 were reviewed. Investigations on clinicopathological features, treatment details and long-term survival were performed. Results: A total of 45 pathologically confirmed acinar cell carcinomas were identified. The median age at diagnosis was 58 years with a male-to-female ratio of 3.1:1. There were 24 (53.3%) localized, 5 (11.1%) locally advanced and 16 (35.6%) metastatic cases, with a pancreatic head-to-body/tail ratio of 1:1.4 for all the primary lesions. In the localized group, there were 10 pancreatoduodenectomy, 12 distal pancreatectomy, 1 total pancreatectomy, and 1 distal pancreatectomy combined with proximal gastrectomy. Among the locally advanced and metastatic cases, 13 patients received chemotherapy, 1 received concurrent radiochemotherapy, 1 underwent synchronous resection of primary tumor and liver metastasis, 1 underwent palliative operation, 1 underwent exploratory laparotomy, and 4 required no treatment. The median overall survival of this series was 18.9 months with a 5-year survival rate of 19.6%. Moreover, the resected acinar cell carcinoma patients were associated with prolonged survival compared with the unresected cases (36.6 vs. 8.5 months, P < 0.001). Conclusions: Surgical resection could improve the long-term survival of acinar cell carcinoma patients, which might also improve the prognosis of selected metastatic cases. Large-scale studies are needed to further clarify the biological behavior and clinical features, and to seek the optimal treatments.

Abstract:With the increase of morbidity and mortality, pancreatic cancer will not only become one of the leading causes of death in China, but also a major public health problem in the world. New onset cases of pancreatic cancer all around the world are expected to increase year by year. The prognosis of patients with pancreatic cancer is poor. The 5-year survival rate is only about 6%. The major reason for the poor prognosis is the lack of typical clinical symptoms in the early stage of disease, which makes it difficult to detect and control its relevant recurrence and metastasis. Moreover, lack of effective chemotherapy drugs and resistance to existing chemotherapy drugs are also important reasons for its dismal status. Radical resection surgery is still the most effective treatment. However, even after radical resection surgery, early recurrence and distant metastasis are still the main problems that bother patients. This article reviews recent high quality studies on pancreatic cancer including epidemiological investigation, advances in basic research, and clinical treatment.

Abstract:The prevalence of pancreatic cancer is sharply increasing recently, which significantly increases the economic burden of the population. At present, the primary treatment of resectable pancreatic cancer is surgical resection, followed by chemotherapy with or without radiation. However, the recurrence rates remain high even after R0 resection. This treatment strategy does not distinguish undetected metastatic disease, and it is prone to postoperative complications. Neoadjuvant therapies, including neoadjuvant chemotherapy and radiotherapy, is being increasingly utilized in borderline resectable as well as resectable pancreatic cancer. This review summarized and discussed clinical trials of neoadjuvant therapy for pancreatic cancer, comparing resection rates, outcome measures, and adverse reactions between neoadjuvant chemotherapy and neoadjuvant chemoradiotherapy.

Prognostic Value of Clinicopathological Characteristics in Patients with Pancreatic Cancer.

Abstract:BACKGROUND:The purpose of this study was to analyze the effects of all clinical characteristics on the overall survival time, in order to provide a basis for determining the prognostic factor of patients with pancreatic cancer.METHODS:A total of 103 pancreatic cancer patients were admitted to the Department of Radiotherapy and Chemotherapy of the Ruijin Hospital, Shanghai Jiaotong University School of Medicine, between January 2002 and December 2012. There were 68 men and 35 women; the median age was 62 years. Diagnoses of pancreatic cancer in all patients were confirmed by histopathology, cytology, or clinical diagnosis. The Kaplan-Meier method was performed to calculate the overall survival rate. The log-rank method was used to examine the univariate analysis. The Cox regression model was performed for multivariate analysis.RESULTS:The median survival time was 293 days, the 1-, 2-, and 3-year survival rates were 27.18%, 5.83%, and 1.94%, respectively. Cox regression analysis revealed that age (P=0.015), Karnofsky performance status (PS) (P=0.002), surgical types (P<0.001), and platelet counts (P<0.001) were independent prognostic factors affecting the overall survival of patients with pancreatic cancer.CONCLUSIONS:Pancreatic cancer had a poor prognosis, the general physical condition, age, the availability of radical surgery, and platelet counts were factors influencing the overall survival of patients with pancreatic cancer.

Abstract:OBJECTIVE:To analyze the prognostic factors of pancreatic neuroendocrine neoplasms (PNEN).METHODS:Clinical data of 61 patients with PNEN from March 1992 to December 2012 was retrospectively analyzed. There were 23 male and 38 female patients, with a median age of 52 years (ranged from 22 to 68 years). Forty-one patients were non-functional tumors, and 20 patients were functional tumors. Fifty-nine patients received operation, 13 (22.0%) patients underwent laparoscopic operation, 2 patients underwent puncture biopsy under CT guidance. Survival was analyzed with the Kaplan-Meier method.RESULTS:Among these patients, 53 (86.9%) patients underwent curative resection. The cases of grade G1, G2, G3 were 41 (67.2%), 9 (14.8%), 11 (18.0%), respectively. The cases of stageI, II, III, IV were 47 (77.0%), 7 (11.5%), 2 (3.3%), 5 (8.2%), respectively. Liver metastasis, neural invasion were found in 5 cases (8.2%), 5 cases (8.2%), respectively. The median follow-up period was 40 months (ranged from 3 to 209 months). The overall 1-, 3-, 5-year survival rates were 92.0%, 89.7%, 86.3%, respectively. Univariate analysis showed WHO classification (χ(2) = 18.503), TNM staging system (χ(2) = 23.401), liver metastasis (χ(2) = 18.606), neural invasion (χ(2) = 10.091), resection status (χ(2) = 25.514) were prognostic factors of PNEN (all P = 0.000).CONCLUSIONS:Surgical resection in PNEN results in long-term survival. WHO classification, TNM staging, resection status are effective in predicting the prognosis of PNEN. Liver metastasis, neural invasion predicted poor prognosis.
Surgical Resection and Outcome of Pancreatic Cystic Neoplasms in China: Analysis of a 16-Year Experience from a Single High-Volume Academic Institution

Abstract:Background To investigate the clinicopathological features of surgically resected pancreatic cystic neoplasms (PCNs) at a single institution in China. Methods The medical charts of patients who operated in the Second Affiliated Hospital, Zhejiang University School of Medicine between 1 January 1997 and 30 June 2013, were pathologically shown to have PCNs. Results There was a reliable increase trend not just in the overall number of patients (3 to 75) but additionally in the number of incidentally diagnosed patients across the periods (33.3% to 48.0%). In 83 of 111 cases, preoperative diagnoses matched with pathology, whereas the remaining cases (16/28) were misdiagnosed as pancreatic cancer. The proportion of malignancy in mucin producing neoplasms was 24.3% (9 out of 37). Elevated serum carbohydrate antigen (CA19-9) or carcinoembryonic antigen (CEA) was independently associated with malignancy. The overall survival rate was 96.4%. Conclusions The proportion of PCNs within this series differs with that revealed in Western countries. Appropriate preoperative differential diagnosing of PCNs remains challenging. It is strongly recommended that patients with elevated CA19-9 or CEA levels undergo surgical resection.

Abstract:Objective::In 2015, the Chinese Pancreatic Association of the Chinese Society of Surgery of the Chinese Medical Association launched a national multicenter online system for registration of surgical treatment of pancreatic cancer in China, called China Pancreas Data Center (CPDC). With continued effort, the CPDC has developed over time. Herein, we report the general results of the CPDC from January 2016 to January 2020 to present the real-world situation of surgical treatment of pancreatic cancer in China.Methods::The data of the CPDC from January 2016 to January 2020 were retrieved and analyzed in this real-world study, including the data on patient demographics, comorbidities, diagnostic modalities, neoadjuvant treatment, surgical procedures, postoperative complications and treatment, pathological examinations, postoperative adjuvant treatment, survival, and risk factors.Results::A total of 13,595 cases from 70 centers in 28 provinces were retrieved for analysis. This study reported the largest cohort of patients who underwent surgical treatment for pancreatic cancer in China to date. More cases were derived from the Eastern regions, among which Shanghai, Beijing, and Zhejiang ranked in the top three. The peak age of the patients ranged from 60 to 69 years. The ratio of males to females was 1.5:1. Overall, 64.3% of the tumors were located in the head and neck of the pancreas, and 35.7% in the body and tail of the pancreas. Of the patients, 23.0% underwent positron-emission tomography-computed tomography, 21.6% underwent endoscopic ultrasound, and 4.8% underwent preoperative biopsy. Two percent of the patients underwent neoadjuvant treatment, while 68.9% underwent R0 surgical resection (margin free of tumor cells). Of the latter, 78.6% of the operations were open procedures, 12.6% were laparoscopic procedures, 2.9% were robotic procedures, and 3.7% were converted to open procedures. The in-hospital mortality rate after surgery was 0.4%. The incidence of grade 2 and grade 3 postoperative pancreatic fistulas was 25.5% and 2.5%, respectively. The incidence of complications based on the Clavien-Dindo classification was 17.9% of grade II, 4.3% of grade IIIa, 1% of grade IIIb, and 0.6% of grade IV. Of the patients, 28.9% underwent postoperative adjuvant chemotherapy. The 1-year, 2-year, and 3-year overall survival of these patients were 77%, 51%, and 38%, respectively. In the 8542 patients who underwent R0 resection, the 1-year, 2-year, and 3-year overall survival and disease-free survival were 77% , 54%, and 43%, and 68%, 49%, and 41%, respectively. The factors related to the prognosis of these patients were also identified after uni-and multi-variate analyses.Conclusion::The surgical quality, safety, and long-term survival of the patients in CPDC are similar to those of international high-volume pancreatic centers. However, neoadjuvant and postoperative adjuvant chemotherapy should be improved.
An Analysis of 28 Cases of Pancreatic Carcinoma with Elevated Serum Alpha-Fetoprotein Reported in the Medical Literature

Abstract:Objective To study the clinicopathologic characteristics,the optimal therapy and prognosis of patients with pancreatic carcinoma with elevated serum alpha-fetoprotein (AFP).Methods CNKI,Wanfang database,VIP Database for Chinese Technical Periodicals,PUBMED and EMBASE were searched.Studies which met the inclusion criteria were evaluated.Results 24 reports were found which included 28 patients with pancreatic carcinoma with elevated serum AFP.The carcinoma was characterized clinically by a predilection for older male.The majority of patients had metastases at the time of diagnosis,especially liver metastases.Histopathologically,not all tumors were hepatoid carcinoma,and acinar cell carcinoma was most common.Surgical resection was the optimal treatment for patients at an early stage.Combined therapies were used on patients at an advanced stage.Patients with hepatoid carcinoma of the pancreas might be sensitive to treatment similarly to hepatocellular carcinoma.In AFP-producing pancreatic carcinoma,serum AFP level was a useful marker for diagnosis and evaluation of therapeutic response and recurrence.However,there were no sufficient evidence to support whether pancreatic carcinoma with elevated serum AFP was associated with a higher frequency of liver metastasis and poor survival.Conclusions Further studies are needed to identify the prognosis and the effective therapies for the AFP expression variance of pancreatic carcinoma with elevated serum AFP.Detecting AFP and variants of AFP are important for the early diagnosis of pancreatic carcinoma with elevated serum AFP,and increase in therapeutic response and prog nosis.
An Overview of Pancreatic Cancer Diagnosis and Treatment in China: Current Landscape and Future Prospects

Abstract:This comprehensive literature review is to summarize the most recent findings regarding the causes, diagnosis, and treatments of pancreatic cancer and to encourage additional investigation into this under-researched malignant tumor. Pancreatic cancer is a significant public health issue in China, with annual mortality rates almost equal to incidence rates. The disease is more prevalent in rural areas and has a poor prognosis. The data was collected from the following databases: Pub Med, Cross ref, Science Direct, Scopus, and Google Scholar we reviewed published articles from 2018 to 2023 on the annual incidence of pancreatic cancer in China is 5.1%, with only 5-7% of patients completely cured. The prognosis is extremely poor, with a 1-year survival rate of 8% and a 5-year survival rate of 3%. Pancreatic cancer has no specific clinical manifestations or tumor markers, and its characteristics are not typical of high-risk factors including smoking, alcohol, chronic pancreatitis, abnormal microorganism metabolism, blood type, and glucose and lipid levels. For increased detection and survival rates, pancreatic cancer must be diagnosed as early as possible. However, the low specificity of tumor markers calls for more study. Future treatment strategies could include immunotherapy and a microbiology-free system, and it's anticipated that they'll offer intriguing clinical applications for extending patients' lives with pancreatic cancer. Finally, we suggest measures to improve the health outcomes of pancreatic cancer patients in China.
Surgical Treatment of Pancreatic Cancer: a Follow-Up Study in 216 Cases

Abstract:Objective To investigate the effects of various treatments for pancreatic cancer on survival of patients and summarize the experience in treatment of the disease. Methods The clinical data of 216 patients with pancreatic cancer treated in our hospital between 1990 and 2000 were retrospectively analyzed. All the patients were followed up. SPSS10 0 software package was used for statistical analysis. Meanwhile, Kaplan Meier method and log rank test were used. Results In 169 patients who had been operated on, the surgical resection rate was 40 24% and radical resection rate 31 36%. The median survival time in patients undergoing radical resection, palliative surgical resection, bypass, surgical exploration and none of operations was 304, 138, 134, 123 and 86 d, respectively. The 1 , 3 and 50 year survival rates in patients receiving radical resection were 44 71%, 14 98% and 9 99%, respectively. The 1 , 3 and 5 year survival rates in patients with pancreatic cancer of stage I and II after radical resection were 69 23%, 38 46% and 12 82%, respectively. We have adopted the method of systemic evaluation and standard surgical exploration since 1995. As a result, the surgical resection rate was up to 50 88%, radical resection rate to 38 60% and the 5 year survival rate in patients receiving radical resection to 12 93%. Conclusions Only radical resection is an effective method and palliative treatment can not prolong the lives of patients. The single surgical treatment for pancreatic cancer is dismal and a more effective treatment model should be established.

Abstract:Objective To verify the effect of preoperative and postoperative multiple-phase interventional chemothe-rapy in respectable pancreatic carcinoma.Methods Thirty-eight patients with resectable pancreatic cancer were divided into two groups:the treatment group received multiple-phase interventional chemotherapy and the control group received postoperative interventional chemotherapy only.The serum tumor markers and tumor-related gene expression in resected tumor samples were examined to evaluate the effects of preoperative interventional chemotherapy.The survival time,tumor-free survival time and the rate of liver metastasis were also analyzed in these two groups.Results The ratio of bax/bcl-2 increased,and the levels of serum tumor markers and the expressions of nm23-h1,MBD1,E2F5 and RB decreased remarkably in the treatment group in comparison with the control group(P<0.05).The disease-related symptoms improved obviously in the treatmemt group with lower rate of liver metastasis.No significant difference in survival time was observed between these two groups.Conclusions The multiple-phase interventional chemotherapy constitutes an effective method in the multi-modality therapy of pancreatic carcinoma by enhancing tumor cell apoptosis,inhibition of tumor growth and reduction of liver metastasis.

Abstract:A multi-center population-based study in Shanghai, China was performed to explore the implications for the management of pancreatic cancer by comparing diagnosis and survival rates. Novel imaging modalities including MRI (13.9%), PET/CT (1.8%), and EUS (5.6%) were not widely used in our population. Only 39.7% of cases were histologically verified (surgery with histologic diagnosis 31.0%, cytological diagnosis 8.7%, surgery without histologic diagnosis 12.1%, and clinical diagnosis 48.2%). Overall, 30.0% of patients underwent curative-intent operation, and only 9.8% of patients received comprehensive treatment. The prognosis of pancreatic cancer patients was significantly better for patients who were treated in high-volume centers than in low-volume centers. We propose that more effort should be put on novel diagnostic modalities, histological confirmation, and comprehensive treatment in China. Multidisciplinary teams specialized in pancreatic cancer therapy in high-volume centers are urgently needed.

Abstract:Objective The incidence of a resectable primary cancer of pancreas associated with another synchronous/metachronous resectable primary malignant tumor is low.The aim of this study was to investigate the clinical features and to review our experience and outcomes with surgical treatment for these patients.Methods Between January 2000 and October 2012,13 patients (nine females and four males,median age:70 years,range:56-79 years) were admitted into our hospital with a resectable primary cancer of pancreas which was associated with another synchronous/metachronous resectable primary malignant tumor.The clinical features,treatment and results were analysed.Results The other primary tumors were non-small-cell lung carcinoma (n 2),gastric carcinoma (n=2),lobular carcinoma of breast (n=3),colonic carcinoma (n=2),esophageal carcinoma (n=1),renal cell carcinoma (n=1),malignant thymoma (n=1),and endometrialcarcinoma (n=1).Among these patients,eleven were metachronous pancreatic cancer which occurred after resection of the other organ malignancies.Two patients were synchronous pancreatic cancer.All patients underwent radical operation that included eleven pancreaticoduodenectomy and two distal pancreatectomies with splenectomy.There were no patients who died in the pert operative period,but three patients developed a pancreatic fistula.The morbidity rate was 20％.The median survival of the radically operated patients was 15months (range:4 35 months).Nine patients died of disease,while 4 are still alive at the time of this report.Conclusions Surgical resection could be performed safely in selected patients with a resectable primary carcinoma of pancreas which was associated with another synchronous/metachronous resectable malignant tumor with good long term survival outcomes.The final survival would depend on the primary cancer of pancreas.

Abstract:Pancreatic cancer is one of the more dismal gastrointestinal tumors due to the obscure symptoms in the early stage of disease, rapid progression and insensitivity to chemotherapy. The 5-year survival rate is only approximately 9% for patients with pancreatic cancer. In the recent years, with the change in dietary structure and life style as well as the improvement of clinical detection rate, the incidence of pancreatic cancer is increasing annually, and it severely undermines the health of people. However, the advance in molecular biology provides more potential targets for the treatment of pancreatic cancer. Besides, well-conducted clinical trials continue to translate more theoretical innovations from bench to bedside. These both can improve the prognosis of pancreatic cancer eventually. The present review summarized the important findings in pancreatic cancer research in 2020.
Influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer

Abstract:Objective:To investigate the influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer.Methods:The retrospective case-control study was conducted. The clinicopathological data of 104 patients with pancreatic head cancer who underwent radical resection in Fudan University Shanghai Cancer Center from May 2014 to May 2015 were collected. There were 62 males and 42 females, aged (61±10)years. Patients underwent carative pancreaticoduodenectomy. Observation indicators: (1) surgical situations; (2) follow-up; (3) influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer. Follow-up was conducted using telephone interview to detect recurrence of patients up to postoperative 1 year. Measurement data with normal distribution were represented as Mean±SD. Count data were described as absolutes numbers, and comparison between groups was analyzed using the chi-square test. Multivariate analysis was analyzed using the Logistic regression model. Results:(1) Surgical situations: 104 patients underwent curative pancreaticoduodenec-tomy successfully. The volume of intraoperative blood loss was (474±280)mL and the number of lymph node dissection was 21±10. (2) Follow-up: 104 patients received postoperative follow-up, 44 of whom had early recurrence. Of the 44 patients with early recurrence, 42 cases had intraperitoneal recurrence including 23 cases with liver metastasis, 7 cases with metastasis in surgical site, 7 cases with retroperitoneal lymph node metastasis, 5 cases with omentum metastasis, 2 cases had extraperitoneal recurrence including 1 case with pleural metastasis and 1 case with pulmonary metastasis. (3) Influencing factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer: results of univariate analysis showed levels of preoperative CA19-9, levels of postoperative CA19-9, the number of lymph node dissection were related factors for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer ( χ2=5.833, 9.276, 4.261, P<0.05). Results of multivariate analysis showed that postoperative CA19-9 >37 U/mL was an independent risk factor for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer ( odd ratio=3.599,95% confidence interval as 1.551-8.347, P<0.05). Conclusion:Postoperative CA19-9>37 U/mL is an independent risk factor for early recurrence after curative pancreaticoduodenectomy of pancreatic head cancer.
